Heritage Sierra Group Helps Patients and the Community

he mission of Heritage Sierra Medical Group of Santa Clarita is to ensure fully coordinated, technology-driven patient care to the highest level of service. “We have diligently invested our time and resources in the development of programs and services responsive to the healthcare needs of today’s patient, while adapting to the increasingly diverse needs of tomorrow’s healthcare agenda,” said Tarana Sherzada, director of network management. Founded in 1984, Sierra Medical Group joined the Heritage Provider Network in 2008. Services include internal medicine, family medicine, pediatrics, lab draw, women’s health, x-rays, specialist referrals, urgent care and walkin care, as well as a doctor who is always oncall. “We offer very unique services such as disease management, health education, dedicated patient advocates who are here to assist our members, and a physician is on call 24 hours a day,” Sherzada said. “And we welcome walk-ins and same day appointments, which seems to be an issue nowadays.” Heritage Sierra Medical Group is one of the very few Medical Groups in Los Angeles County to be recognized by Integrated Healthcare Association for providing top quality of care for three consecutive years to Medicare beneficiaries. Treating patients of all ages, Heritage Sierra Medical Group offers access to primary care physicians and specialists with medical offices in the Santa Clarita Valley, and also utilizes Henry Mayo Newhall Hospital for inpatient services. “Aside from our staff primary care physicians, we have affiliations to 16 community primary care physicians for a total of 21 primary care physicians in the Santa Clarita Valley,” said Sherzada. Since its founding in 1984, Heritage Sierra Med-

ical Group has established medical offices in Lancaster, Palmdale and Valencia. The majority of patients reside in the Antelope Valley and Santa Clarita Valley. “However, we are able to treat patients who live within a 30 mile radius from any of our three locations,” Sherzada said. “Our patients do not need to travel outside of the Santa Clarita Valley for primary care, specialty care or in-patient services.” Patients particularly appreciate minimal wait

times, she said, and the ability to make primary care appointments within the same day for urgent matters, and within the same week for non-urgent matters. “We are a family and treat our patients as such by building a relationship with our members and ensure they are cared for and seen timely,” Sherzada said. “We also offer Telemedicine for certain

providers who have more than one location to ensure the patient will always have face-to-face interaction.” Heritage Sierra Medical Group is also known for supporting local philanthropic causes, including sponsoring the Santa Colorita 5k Fun Run, Golden Years Expo, Cowboy Festival, Summer Bash and Kids Expo. One time, “we helped Little Light Pre-school located in Newhall pay for the children’s t-shirts that were stolen,” Sherzada recalled. In other charitable efforts, Heritage Sierra Medical Group served the SCV Backs the Blues group by donating more than 300 bottles of water to the supporters. Additionally, 400 bottles of water were donated to the men and women fighting the 2016 Sand Fire, and more than 600 bottles were donated in support of the vigil held for fallen officer Steve Owen in both the Antelope Valley and Santa Clarita Valley. “We also support local fundraising and support groups such as the Boys and Girls Club, Santa Clarita Valley Senior Center, Alzheimer’s Association and more,” Sherzada said. Additionally, through its role with the Santa Clarita Valley Chamber of Commerce, “we have established relationships to help educate and provide healthcare services such as blood pressure, glucose screenings and flu shots at no cost,” Sherzada said. Physicians have also given free educational talks at places like Oakmont of Santa Clarita, the Santa Clarita Senior Center and Santa Clarita Christian Fellowship. Above all, “we emphasize the importance of getting to know you and your history to ensure that you receive the highest quality medical care,” said Sherzada. “We are proud to serve the Santa Clarita Valley residents for all their healthcare needs from infancy to geriatrics.” For more information, call the member services department at 661-273-7346.
