THE SRI LANKA MEDICAL ASSOCIATION 1887 to 2012

The historic Wijerama House The Sri Lanka Medical Association (SLMA) is the oldest national organisation of medical professionals in Asia and Australasia. It brings together medical practitioners of all grades, from all branches of medicine in Sri Lanka.

The SLMA started life as the "Ceylon Branch of the British Medical Association" on 17 December 1887 with 65 members on its roll, and Dr P D Anthoniz as its first President. However, the moving force behind its inception was Dr W R Kynsey (later Sir William Kynsey), who persuaded 15 doctors to meet on 26 February 1887, at the Colonial Medical Library on Maradana Road, Colombo and resolve to form that Association. He had declined to be the first President as he was going abroad on furlough. The change of name to "Ceylon Medical Association" came in 1951, and in 1972 when Sri Lanka became a Republic, the name changed again to the "Sri Lanka Medical Association".

The SLMA office is at "Wijerama House", named after Dr E M Wijerama, who formally gifted the house he lived in at McCarthy Road (now Wijerama Mawatha) to the Association in 1964. Although his offer was made in writing in October 1957, resolution of the many issues that arose took 7 years!

The Ceylon Medical Journal, was first published in August 1887, and fittingly, the first article in the first issue was authored by Dr W R Kynsey. The name changed to Journal of the Ceylon Branch of the British Medical Association in 1904, but changed to its pristine appellation in 1952, and continues to be published as the Ceylon Medical Journal (CMJ). At it’s 125th year, it is the oldest surviving English medical journal in Asia and Australasia, and the leading scientific journal in Sri

Lanka. The CMJ is indexed in BIOSIS, CAB International, EMBASE and the Index Medicus. CMJ's editorial policies and quality are of international standards, and it is listed by the International Committee of Medical Journal Editors as conforming to their editorial guidelines.

The historic Sri Lanka Medical Library, which is over 168 years old, is also accommodated in Wijerama House. The Sri Lanka Clinical Trials Registry, started as recently as 2006, has achieved recognition from the World Health Organisation by being selected as a WHO Primary Clinical Trials Registry.

The Anniversary International Medical Congress is the most important activity of the SLMA’s calendar. It has been held uninterrupted every year since it was started in 1937 to mark the 50th anniversary of the association.

The SLMA today is a vibrant organization. It is run by the President and the Council. There are many committees dealing with its day to day affairs. These include the Finance, Management and Housing Committee; Corporate Planning Committee; Ethics Review Committee; Ethics Committee; Honours Committee; Research Promotion Committee; the Sri Lanka Clinical Trials Registry Management Committee; and the Ceylon Medical Journal Editorial Board. The SLMA also has Expert Committees which deals with various subject areas. They include Expert Committees on Communicable Diseases; Disabilities; Ergonomics; Health Equity; Health Management; Herbal Medicine; Medicinal Drugs; Non Communicable Diseases; Prevention of Road Traffic Accidents; Snake Bite; Tobacco, Alcohol and Illicit drugs; and Women’s Health.

MESSAGE FROM THE PRESIDENT OF THE DEMOCRATIC SOCIALIST REPUBLIC OF SRI LANKA I am pleased to send this message of felicitation to the Sri Lanka Medical Association (SLMA) on the occasion of its 125th anniversary, being celebrated with an international medical congress in Colombo and a series of medical conferences across the country.

Over the past 125 years the SLMA has made an immense contribution to uplift the standards of the medical profession in Sri Lanka and thereby played a significant role in improving the healthcare services in the country. Its continued efforts to keep the standards of the medical profession at the highest levels are highly commendable.

In the current post-conflict era of peace, the SLMA has recognized the larger role it has to play in improving the quality and standards of health in all parts of the country, including the North and the East. The International Medical Congress with the theme “The Global Forum of Sri Lankan Doctors� has attracted Sri Lankan doctors from all parts of the world to which they have taken the knowledge and experience gained in Sri Lanka and improved on them with their studies and practice abroad. I trust that all of them will come together for further development of the health sector in Sri Lanka as we move to the challenging phase of post-conflict development.

I extend my good wishes to the SLMA on reaching this important milestone in professional development and service to the people and wish every success in the years to come.

MESSAGE FROM THE PRESIDENT OF THE SLMA The Sri Lanka Medical Association was founded 125 years ago on 17 December 1887. The 125th Anniversary of the SLMA therefore is a time to celebrate the achievements of the SLMA as well as the health sector of Sri Lanka, of which the SLMA has been an integral part, and look to the future. The 125th Anniversary is also a time for all Sri Lankan doctors to come together under one roof. It is in that spirit that we organised the 125th Anniversary International Medical Congress – the “Global Forum of Sri Lankan Doctors”. The Congress has something to offer to everyone – whatever specialty you belong to; whatever your professional standing is, whether you are a newly graduated doctor or a retired senior super specialist; wherever your place of work is, whether you are in government service, in academia or in private practice; and whichever part of the country or outside the country you are located. The Congress programme – both academic and social – was designed to cater to the needs of the entire medical community.

The Congress would be preceded by the first ever SLMA Run and Walk which would be held on the day before the Congress with the participation of doctors as well as the community at large. It is aimed at promoting a healthy lifestyle for all. The first day of the Congress is devoted to celebrating the achievements of the health sector and looking at future developments. The entire medical profession is coming together for this purpose.

Presidents of 32 professional medical colleges and associations, representing all specialties and subspecialties in the field of medicine are talking in one forum for the first time in Sri Lanka. They will also deliberate on financing the health sector and health sector manpower requirements. This is a unique day for the medical profession. The second day is devoted to medical research. The top medical researchers in the country are presenting 244 research papers on this day. The keynote address on this day is on the national strategy on science, technology and innovation. This is the first time it is being presented in a medical forum.

The day is full of other interesting activities including training on ethics for researchers and ethics committee members and symposia on research funding, publishing research and clinical trials. The third day is devoted to continuing professional development with symposia dealing with a wide range of interesting topics such as infectious disease, respiratory diseases, stroke, cerebral palsy, rational use of antibiotics, road traffic accidents to name a few. A special feature of the day is that a complete stream is dedicated to raising awareness about creating a healthy working environment. The last day of the conference 6 July 2012 is dedicated to advocacy. There are four key workshops on that day. The first one is on eHealth which is fast becoming essential in the drive to modernise the health sector. The second is on reproductive health dealing with sensitive issues such 11

as abortion, gender based violence, and sexual medicine. The third one is on birth defects surveillance – a key need in Sri Lanka’s march to surpass the Millennium Development Goals. The final one is on quality assurance in diagnostic laboratories which is an essential need in Sri Lanka. The Congress is also made colourful by four orations – the SLMA Oration, the S C Paul Oration, the Professor NDW Lionel Oration and the inaugural S Ramachandran Oration.

This diverse programme was made possible due to the tremendous support given to the SLMA by all the professional medical colleges, associations, and societies; the Ministry of Health; the World Health Organisation; the National Science Foundation; the Sponsors and many individuals. I thank them all. I also wish to thank the Past Presidents, Council Members, Members of various committees in the SLMA, Sponsors and Staff of the SLMA who contributed to the success of this Congress in numerous ways. A special word of thanks go out to Honorable Maithripala Sirisena, Minister of Health, for his support for all activities of the SLMA this year, Dr Lasantha Malavige for his untiring efforts to make this year’s Congress extra special by single handedly taking on the enormous task of organizing the SLMA run and walk, and Mr. Rajasingham and his team at the SLMA for their hard work. I also wish to thank the South Australian Doctors Association and the UK-Sri Lanka Immunology Foundation for providing funds which made it possible for us to provide free congress registration to 15 medical students and 40 doctors from remote areas to attend the Congress.

MESSAGE FROM THE HONORARY SECRETARY OF THE SLMA It is my pleasure and privilege to welcome you to the 125th Anniversary International Medical Congress of the Sri Lanka Medical Association.

This year’s Congress marks yet another significant milestone in the history of the SLMA; the apex medical body in Sri Lanka and the oldest in Asia and Australasia. The highlight of this year’s activities include in addition to the usual academic programme, the much publicised SLMA ‘Walk and Run’ which, adds a new dimension to this year’s activities.

The top quality scientific programme which, has evolved through intense collaboration between local and international experts consists of 4 orations, over 230 lectures and other plenaries and 169 poster presentations. I am sure the distinguished speakers, both from overseas and our own will create new awareness and insights on the latest advances and developments in the field of modern medicine.

I wish to extend my sincere thanks to all those who have collectively made their contribution towards organising an event of this magnitude. The untiring efforts and guidance by our president Prof. Vajira H. W. Dissanayake and the support extended by vice presidents, assistant secretaries and the members of the Council in organising this congress deserves special mention. My thanks also go to the Co Editors of the Ceylon Medical Journal for undertaking a laborious task of reviewing more than 400 abstracts within a short space of time.

I also thank all our speakers, authors of scientific papers, chairpersons, judges of oral and poster presentations and all our sponsors for their support. My special thanks also go to Drs. Luckshi Gamachchi and Amjad Kariapper both pre-intern doctors for their valuable contribution. SLMA ORATION 2012 Control of Non Communicable Diseases: Where do we begin?

Introduction: Obesity the new form of malnutrition is spreading in epidemic proportions all over the world with fast penetrating the paediatric population. Obesity associated metabolic consequences are common among south Asian populations. Onset of many of these metabolic derangements is seen at young age. Adiposity is directly related to the development of these adverse health consequences.

However, currently used BMI cutoffs are not sensitive to detect individuals with metabolic derangements in most populations. This study was carried out to identify the prevalence of obesity related metabolic derangements and insulin resistance among 5-15year old school children in the Colombo district of Sri Lanka and evaluated the validity of currently used obesity diagnostic methods in this population and impact of birth weight and growth on the development of metabolic consequences later in life.

Materials and Methods: A cross sectional descriptive study using two-stage proportionate probability cluster sampling technique. Blood glucose, insulin, lipid profile and ALT were assessed with anthropometric measurements.

Results: 920 children were studied (boys 547). 3.5% were obese. 63 had hypertension and 15 had abnormal glucose metabolism. 15.1% had hypercholesteraemia. Metabolic syndrome in the general population was 1.6% and 40.6% among obese individuals. Mean fasting insulin, post prandial insulin and HOMA-IR increased significantly with age. Significantly higher mean metabolic parameter and insulin resistance values were noted among those in the lower birth and higher current BMI tertile compared to those in the lower birth and lower current BMI tertile. No differences were noted across birth weight tertile but were seen across BMI tertiles. International BMI cut off values had poor sensitivity in diagnosing obesity among this population, however newly developed Sri Lankan WC and BMI cutoff values improved the diagnosis.

Conclusions: Significant number of Sri Lankan children had NCD related metabolic abnormalities despite having normal BMI values. Although many children were euglycaemic, many showed early insulin resistance. Rapid postnatal growth is the main environmental factor contributing to the development of metabolic derangements in genetically predisposed groups. Population specific screening tools are important for the early detection of metabolic derangements thus enabling effective control of NCD in future. NCD prevention has to be started from a very young age and behaviour modification need to be done via having a National programmes at school level to increase awareness of obesity related metabolic consequences and their long term effects. More opportunity for physical activity, availability of healthy food especially at school canteens and code of advertisement would help to build a healthy nation.

Transplantation has revolutionised the management end stage organ failure. Advances in surgical techniques, anaesthesia and critical care along with the development of immunosuppressive regimens have vastly expanded the number of transplants performedworld-wide. Coupled to this success has been an exponential rise in the demand for donor organs posing a major challenge for transplant programs.

Modern immunosuppression haslargely succeeded in controlling acute rejection, but chronic rejection remains the major cause of graft loss, morbidity and mortality in graft recipients and its pathogenesis is still poorly understood. Pan immunosuppression of organ recipients comes with its attendant problems of opportunistic infections, neoplasia and economic consequences. The ability to equip a recipient with donor organ specific tolerance would counter the twin problems of chronic rejection and non-specific immunosuppression. This utopian state, often referred to as the ‘Holy Grail’ of transplantation, is the goal towards which most research in transplant immunology is directed.

The immune response to transplantation is distinct because donor alloantigen recognition by host T-cells occurs via two pathways. Though recognised as being predominantly responsible for acute rejection, the impact of direct allorecognition diminishes with the depletion of donor APCs from the graft. There is also increasing evidence that the indirect pathway plays a significant role in both early and chronic rejection.

Contributing to this was the observation that CD4+ T-cells with indirect allospecificity could provide help for graft rejection mediated by CD8+ T-cells with directly allospecificity. Theunlinked four-cell model proposed to explain this, consisting of a CD8+ T-cell paired with a donor APC that received help from a CD4+ T-cell paired with a recipient APC,however, did not clarifyhow help was provided. The ability of DCs to re-present unprocessed antigen raised the possibility of a novel pathway of allorecognition, whereby recipient DCs, rather than process alloantigen, could present it as intact protein for direct pathway activation. If these DCs could also concurrently present processed alloantigen to indirect pathway CD4+ T-cells, this three-cell cluster model would enable linked help and provide a rational alternative to the four-cell model. The aim of this study was to investigate this hypothesis using a well characterised BALB/c to B6 MHC class I and II disparate mouse model of CD8+ T-cell mediated acute rejection. B6 DCs acquired and presented both intact and processed allo-MHC molecules simultaneously after co-culture with BALB/c DCs. When transferred into syngeneic hosts these DCs generated an anti-donor CD8+ T-cell response and induced division of CD4+ T-cells withindirect allospecificity. The failure of MHC class II deficient B6 DCs to generate this responseunder similar conditions indicated that the presence of both intact and processed forms of the alloantigen was a prerequisite for this pathway to be effective. DCs from B6 but not MHC class II deficient B6 recipients of BALB/c cardiac transplants generated CD8 + T-cell responses in syngeneic hosts replicating the findings in vivo. These results confirmed that help to direct cytotoxic CD8+ T-cells could be provided by indirect CD4 + T cells throughinteraction on a single APC. This mechanism could be of importance in the development of delayed anti-graft CD8+ T-cell cytotoxicity and the pathogenesis of chronic rejection. A better understanding of these pathways will facilitate the evolution of strategies to prevent chronic rejection and develop graft tolerance.

Inaugural S Ramachandran Oration Is Surgery for Diabetes Effective?

Objectives: To study the effects of Roux en Y gastric bypass surgery on weight loss and glycaemic control.

Design and Methods: We conducted a longitudinal cohort study of 15 obese individuals with a BMI exceeding 35 Kg/m2 who underwent the Roux en Y procedure from 2003 to 2009. The reduction of weight, excess weight, BMI, Waist circumference, incident diabetes and pre diabetes was assessed. The reduction in the use of oral hypoglycaemic drugs and the time to plateau of the loss in weight was also determined.

Results: The average decrease in weight, excess weight, BMI, Waist circumference was 26.2 Kg, 49%, 9.8, 14.7cms ( 13.8%) respectively. Loss was greater in females vs males 29 vs 24 Kg,32.9 vs20.9kg , 11.1 vs 7.4 and 15.2 vs 13.6cms for the same parameters respectively.

The change in diabetics for HbA1C, FBS, 75gm OGTT and drug usage was - 2.2%, 70.8mg/dl, 107mg/dl and 1.7 drugs respectively. All diabetics prior to surgery lost their diabetic status.

Conclusions: Roux en Y gastric bypass surgery is a very effective procedure for losing weight in obese patients who have failed conservative treatment. It also results in remarkable improvements in glycaemic control.

Unfortunately, some patients develop allergic reactions whilst receiving either one or a combination of these agents. Some reactions result in significant morbidity or even mortality and also raise important management questions about possible medications patient may safely receive in the future. My talk will highlight important advances made in the pathogenesis, diagnosis, management and prevention of drug allergies and outline how these contributed to the development of specific management guidelines by our working group. Recent research contributions made by my group in understanding the epidemiology and pathogenic mechanisms of different drug allergies and its management would be discussed.

International Course on Surveying and Evaluating Ethical Review Practices Course Description This course develops local and regional expertise in evaluating ethical review practices.

It is based on the published TDR WHO Surveying and Evaluating Ethical Review Practices, a Companion Guideline to the TDR WHO Operational Guidelines for Ethics Committees That Review Biomedical Research (2000). The course targets ethics committee members in the Asian-Pacific region with extensive experience in ethical review. The aim is to lay the foundations for professionals to evaluate and monitor ethical review in the region. The course is also intended to train local surveyors in Sri Lanka.

Course Objectives The primary objective of the course is to promote good ethical review practices in health research in the Asia & Pacific Region by implementing international criteria for surveying and evaluating ethical review practices. Within this framework, the following specific objectives will be pursued:  to establish the role of the surveyors and evaluators for ECs in the Asian-Pacific region;  to implement the TDR WHO Guidelines Surveying and Evaluating Ethical Review Practices, a companion guideline to the TDR WHO Operational Guidelines for Ethics Committees That Review Biomedical Research (2000);  to develop surveying and evaluating practices within a framework of international guidelines, including WHO & ICH GCP Guidelines and national guidelines in the Asian-Pacific region;  to develop a regionally and internationally appropriate methodology for surveying and evaluating ethics committees;  to prepare surveyors to work according to the requirements of the local conditions and regulations;  to prepare national and local ethics committees to work according to international standards in ethical review.

Course Outcomes The course will aim to achieve outcomes in the following areas:  Training and certification of in-country surveyors of ethical review practices;  Translation and in-country adaptation of the Surveying and Evaluating Ethical Review Practices guideline;  Development of in-country and regional standards for recognition of ethical review committees;  Development of international human subject protection standards.

Course Format A four-day course consisting of lectures and discussion in working groups, focused on presenting a comprehensive overview of the requirements for surveying and evaluating 99

ethical review practices with an emphasis on developing practical approaches. The following themes are treated:  Practical Challenges to Ethical Review  Establishing a Framework for Evaluating Ethical Review Practices  Conducting Site Evaluations  Follow-up to Evaluations of Ethical Review Practices The course participants will be divided into working groups, each group will have one facilitator. The second, third and half of the fourth day of the course will consist of site visits. The working groups will carry out an evaluation of the ethics committee at designated sites.
