SMA is prepared to transition seamlessly into the role of CSAM’s partner AMC considering the following: • SMA staff is highly trained on the association management software platform YourMembership. • SMA has a strong team of in-house IT specialists who are charged with evaluating and recommending technology solutions for clients to increase their efficiencies, reduce costs and improve their members’ experience. • The firm’s accounting department uses Quickbooks Pro. • SMA Launch is an in-house marketing, graphics and branding department that offers access to graphic designers and marketing professionals to support SMA’s clients. The highly qualified Launch staff would be able to support CSAM’s marketing and branding needs. • To support volunteers engaged in public policy efforts, SMA is located in downtown Sacramento, within walking distance to the Capitol. Additionally, SMA offers a contemporary, fully equipped headquarters for our clients, including a conference room that seats up to sixteen (16). SMA’s approach to identifying the right team of professionals to serve a client may be unique; however, we have found that fully understanding the culture and expectations of each client before pulling the team together breeds success. CSAM has identified desired skills and characteristics of their next Executive Director, and SMA wants to ensure the identified leader is the right fit. Until that individual is identified, one of SMA’s Account Executives will be assigned as the temporary executive lead under the guidance of Founder and President Catherine Smith. INTRODUCTION Smith Moore & Associates (SMA) is a full-service association management company offering excellence in administration, governance, strategic planning, meeting planning, accounting, communication, design and more. With a focus on timeliness and quality services, SMA distinguishes itself from other association management firms by truly listening to clients’ needs and embracing the association’s mission and values to realize its full growth potential. By integrating a strong work ethic and developing long-term, trusted relationships, SMA develops and maintains reliable services aimed at meeting the California Society of Addiction Medicine (CSAM) members’ needs. SMA will work closely with CSAM leadership to achieve its mission to advance the treatment of addiction through education of physicians, physicians-in-training, and other health professionals. Additionally, the Society promotes research, prevention, and implementation of evidence-based treatment. CSAM’s vision, to be a thriving state chapter, with growing membership, impactful education, effective advocacy for members and their patients, upwards leadership to our national organization (ASAM), and a track record of contributing to the establishment of access to high quality addiction treatment in California will always be part of volunteer leaderships deliberations. In support of CSAM, SMA’s team will manage CSAM’s operations and elevate the professionalism through best practices and standards, professional design and tenured advice, allowing CSAM volunteers to focus on the policy and governance of the association. Based on the request for proposal, SMA understands the key objectives to be in partnership with CSAM as an association management company include: Providing an Executive Director who is a proven leader, has a facilitator leadership style, diplomatic and mission-driven; Building upon the past success of CSAM’s conference and other professional development opportunities ensuring Continuing Medical Education (CMEs) are plentiful for attendees; Working with CSAM on implementing its solid strategic plan focused on deepening membership; increasing engagement; increasing professional outreach success; developing strategies to attract young learners and executing a successful budget plan; • Providing successful services in support of public policy, governance, strategic and financial management; systematic and efficient administrative services; membership maintenance and development and marketing; vendor relations; excellent conferences and meeting; online webinars; and website, social media and publications management; • Providing solid administrative support to the Medical and Research Foundation (MERF) and the California Patient Protection and Physician Health (CPPPH); and • Embracing the CSAM’s mission and vision. Founded in 2006, SMA’s philosophy is to provide clients with unmatched services and expertise in administration, governance, accounting, meeting planning, strategic planning and communication services, going above and beyond client expectations by: • Tailoring services to the specific needs of our association clients; • Ensuring our clients’ leadership can focus on the vision of the association while remaining confident that the day-to-day operations are running smoothly; • Striving to realize growth potential for each client; and • Working closely with clients’ volunteer leaders to assess the needs of their membership. SMA’s team of association management experts effectively manages the day-to-day operations with the goal of positioning the association for future success. Recognizing the importance of associations being responsive and nimble to create value for members, SMA has developed an infrastructure that focuses on enhancing client membership, education and events as well as effective member communications. SMA’s goal is to expertly manage each client’s business and develop and/or maintain a strong financial foundation that will allow Board members and volunteers to fulfill their purpose. SMA will assemble a team of professionals who will work in partnership with CSAM to ensure its strategic initiatives are implemented and desired outcomes are evident. SMA has 19 fulltime employees, several of whom are certified in association management and meeting planning. Additionally, SMA employs accountants, designers and information technology professionals with years of experience and maintains excellent relationships with a stable of professional individuals and organizational specialists to deepen the bench of experts on legislative advocacy, strategic planning and public relations. The following outlines the SMA team, the proposed scope of services, SMA fees to provide the services, conclusion and appendices.

SMA TEAM AND STRUCTURE SMA’s employees are trained and experienced in delivering excellent customer service to each client (see appendix B). Supported by SMA’s positive and transparent culture, staff are organized in function-related teams as well integrated teams to support collaboration on behalf of the association clients’ needs (see appendix C). The membership and events division is supported by responsive staff in the communications and IT departments to ensure clients’ needs are met in a timely and efficient manner. Additionally, SMA maintains a stable of professional experts with a variety of expertise, including but not limited to, governance, advocacy, public relations and media. SMA’s approach to delivering high-level and high-quality client services is to assign a dedicated team based on the unique needs of CSAM. The team, comprised of an account executive, account coordinator and account assistant will work closely with CSAM and its volunteers to achieve the organization’s mission and vision. The core team will be fully supported by all members of SMA in their efforts to manage CSAM. The following is a description of SMA’s executive leadership as well as the core team members supporting client membership and events; design and communications, information technology and accounting staff.
EXECUTIVE LEADERSHIP AND PARTNERS Catherine A. Smith is SMA’s Chief Executive Officer (CEO). With more than 35 years of experience in association and nonprofit administration on
local, state, national and international levels, Catherine founded SMA in 2006. Previously, she served as the executive director of a statewide trade association for 15 years. Catherine graduated from the University of Toledo, Ohio, with a bachelor’s degree in communications and earned a master’s in communications and public affairs at California State University. She received the prestigious Certified Association Executive (CAE) designation from the American Society of Association Executives (ASAE) in 2000 and has presented at state and national forums, speaking on association management and governance issues.
Justin Lewis is SMA’s Chief Information Officer. Justin has worked in the IT field for 20 years and has experience with installation, configuration
and support for Internet connections, networks, websites, listservs and more. He has experience in programming languages Python, R and the Web framework Django. Justin is often an invited speaker on association management technology. Prior to joining SMA, Justin was a systems administrator for an Internet service provider, giving him experience and preference for command line interfaces and the Linux operating system. He has knowledge of database administration, including everything from the DOS-based PC File to software such as MySQL, the world’s most popular open-source database.
CORE SERVICES TEAM - ACCOUNT EXECUTIVES, COORDINATORS AND ASSISTANTS Account Executives Account Executives (AEs) are the key point of contact between clients’ and SMA and are responsible for managing a client’s overall operations to ensure client and member satisfaction. AEs are responsible for overseeing membership management, event management, accounting, boards and committees, design and communications, general customer service and administration. As lead of the client’s team, AEs rely on their collaborative skills and communication to ensure client needs are effectively translated and addressed. Representing SMA’s core services, AEs are carefully selected based on proven skills and a positive attitude as well as demonstrated enthusiasm. Below is an overview of SMA’s Account Executives.
Carmen Berry has provided years of high-level services to a multitude of nonprofit organizations. Her extensive background in association management results in strong organization, direction and strategy for SMA clients. Carmen previously served as the Director of Membership for a Sacramento association management company where she increased clients’ educational offerings and focused on increasing clients’ returns and profit margins. Thoughtful and thorough, Carmen is known for her attention to detail and professional work product. She is forward-thinking and keeps a creative eye on opportunities for growth while addressing challenges to uncover rewards for clients and SMA.
Elizabeth Cardwell, Certified Meeting Professional (CMP) joined SMA in 2007. Prior to that time, she worked in the communications office of the California Department of Alcohol and Drug Programs where she served as the assistant to the Public Information Officer. Since that time, she has incorporated that knowledge and experience into her role as an AE. Elizabeth has a bachelor’s degree in interpersonal communications from California State University, Sacramento. In 2012, Elizabeth received the CMP designation from the Events Industry Council (EIC). She is known for her attention to detail and excellent follow-through with clients in all areas of management, including board and committee work; membership and events; and communication and more. SMA TEAM AND STRUCTURE Sarah Erck, CMP joined SMA in 2013 after serving as a co-owner of a local family business. While there, Sarah managed product and service contract
negotiations, branding, employee management and human resources, as well as supervised customer service and vendor relationships. Additionally, she represented the company at tradeshows and professional association events. Sarah has applied her experience in astutely managing client operations. In 2017, she earned the CMP designation from the Events Industry Council (EIC). Sarah graduated from Diablo Valley College with an associate degree in liberal studies and later earned a paralegal certification from St. Mary’s College of California.
Yelena Martynovskaya has been with SMA for seven years. Yelena is an exceptional multi-tasker on behalf of clients She particularly excels in organizing process; accessing technology to address client needs as well as data collection and analysis to yield excellent outcomes for clients. Her focus on creative solutions leads to increased client satisfaction. Yelena graduated from California State University, Sacramento, with a master’s degree in art studio and is an accomplished artist. She has exhibited through the Sacramento Center for Contemporary Art and The Verge Gallery in Sacramento, California. Account Coordinators Account Coordinators (AC) are a key member of a client’s team. ACs are responsible for numerous work processes on behalf of clients. Providing direct support to AEs, ACs perform autonomous tasks relating to membership renewal, database maintenance, exhibitors and sponsors, accounting, marketing e-blasts and boards and committees. ACs work directly for the AEs and oversee the work of Account Assistants.
Angelique Grellus has an extensive background in customer service and communications. Angelique is exceptional at managing complicated projects
while staying focused on details and outcomes. Her patience and determination are assets in dealing with stressful situations and bringing them to a successful close. Angelique has a bachelor’s degree in psychology from California State University and is a strong asset to the SMA team. Abi Hague has a background in working as a member services representative with a large statewide trade association. She excels at member services, event support and database management. Abi’s kind and skilled approach to addressing the client’s needs is an asset to both the client and the SMA team.
Zach Seals has a background in both customer service and administrative work. As an AC, his responsibilities include database and member account management, communications, event assistance, and committee support. Zach’s experience and professionalism translate into his ability to handle anything that comes his way. He focuses on superior customer service that results in delighted clients. Account Assistants Account Assistants (AA) are members of the core association management team. Working closely with ACs, AAs closely manage the membership database for clients and coordinate with accounting on invoicing, event registration, dues renewal and post event accounts receivable. They play a key role on event preparation, including creating name badges, packing collaterals and materials for onsite services and support to facilitate timely, effective events.
Rayneedae Garcia has experience in multiple professional fields with an emphasis in communications. Prior to joining SMA, she worked in public affairs
and communications at a statewide trade association. Rayneedae places an emphasis on concise, transparent and effective client communications. Her can-do attitude leads to positive results. Rayneedae has a bachelor’s degree in public relations, communications from Sacramento State University.
Kristina McKittrick recently joined SMA having worked for several years in customer service and event planning with organizations across the United States and internationally as well. Kristina focuses on responsive communications and problem solving for clients. She earned her bachelor’s degree with a double major in political Science and international studies from Elon University in North Carolina.
Eddie Moore is a recent graduate from the University of Portland with a bachelor’s degree in psychology. Having worked for four years as a summer assistant with SMA, Eddie recently joined the SMA team as a full-time employee. With a penchant toward leadership, he was elected as the captain of the University’s lacrosse team. Additionally, he worked as resident dorm staff and completed an internship with the Multnomah County Probation Department.

SMA TEAM AND STRUCTURE SMA DESIGN AND COMMUNICATIONS TEAM SMA believes transparent, efficient and compelling communication is a core client service. In light of this, the SMA communications team supports the membership and events’ teams in developing and distributing effective communications. Furthermore, clients often request and/or demonstrate interest in design and photography services. Consequently, SMA implemented a new service line – Launch. With a focus on providing marketing support and sterling design and communications, Launch includes employees focused on marketing strategies, branding, graphic design and layout, web design, photography and more. David Blue Garrison is the SMA Director of Marketing and Design Development. He is responsible for the design and layout of marketing and communication materials for client conferences and education events as well as branding. With a background in editorial design, David has ten years of working in the freelance world creating more than 100 publications and marketing materials for art organizations in Sacramento. Audie Whitt is the SMA Communications Director. With experience in graphic and web design, he is responsible for providing clients with innovative publication designs and website updates. Audie serves as the liaison between the communications and IT departments to ensure the client’s brand remains cohesive. He earned a bachelor’s degree in art studio. Dayna Dixon is the SMA Graphic Designer. Dayna works comfortably in all forms of graphic design in illustration and brings a fresh perspective to client projects. With a focus on working with client exhibitors and sponsors, Dayna ensures consistency of design with branding. She graduated with a bachelor’s degree in graphic and web design from the Art Institute of California. Information Technology (IT) Amanda Rae Smith, SMA’s Information Technology Coordinator is a key member of SMA’s information technology infrastructure, which is led by executive leader, Justin Lewis. Amanda is SMA’s Information Technology Coordinator and focuses on the development of client websites, databases, email systems and software solutions. Before developing her IT skills, she worked in a physical fitness center and honed her excellent customer service skills. Amanda is creative and has can-do attitude as she works on a wide variety of projects covering numerous.
SMA FINANCE TEAM Sandy Goree is SMA’s Controller and is responsible for GAAP compliance, accounting procedures, creation of financial reports and budgets, tax and audit oversight and investment policies. Prior to joining SMA, Sandy owned a successful financial consulting company specializing in nonprofit accounting. Joanie Bockus is an SMA Accountant with more than 35 years of experience. Joanie is a key team member in preparing timely and accurate financials. From 1987 to 2013, Joanie advanced from staff accountant to accountant directly below the finance director for Disability Rights California. Amelia Clarke is an SMA Accountant and brings more than 23 years in accounting and management from a variety of industries, including banking, insurance to non-profits. Amelia has owned successful businesses where she increased her knowledge of organizational systems.

SCOPE OF SERVICES The following outlines the scope of services SMA is committed to providing CSAM. Association Management • Provide a fully-operational office in downtown Sacramento (8:30 a.m. – 5 p.m. except weekends holidays) that is walking distance to the State Capitol. • Receive and process mail, host visitors and occasional business-related meetings. • Provide dedicated telephone line that is answered “California Society of Addiction Medicine” and fax line. General office supplies provided. • Manage one central email box for CSAM (csam@csam-asam.org) and separate emails for all identified staff. • Supervise contractors (e.g. lobbyist, writers, consultants, etc.) as necessary with Executive Director executing all contracts within the scope of the budget approval process and at the direction of the board. • Serve as the CSAM secretary maintaining and protecting the official records and files and submitting all required filings with the IRS, Franchise Tax Board, Secretary of State, CSAM trademark, etc. • Maintain storage archives and ensure compliance with records retention policy. • Secure policies for directors and officers liability insurance (D&O) and general liability. • Governance. • Participate in all in-person and telephonic board and Executive Committee meetings. • Arrange logistics and participate in the meetings providing advice and information. In partnership with the President, prepare the agenda and supporting documentation. Distribute packets and take minutes. • Maintain Board roster and update contacts. Committees • Provide administrative support and guidance to CSAM’s ten (10) standing committees (Public Policy, Education, Conference Planning, Membership, Communications, Physician Well-Being, Opioids, Awards, Nominating and Integration and Access to Systems of Care (IASC). • Maintain current lists of all committee members, the Executive Director or staff is typically assigned to support each committee, facilitate communications, and assist with the implementation of activities. • Support Nominations Committee in making nominations to the board; prepare award plaques and other awards; and maintain list of past award winners. Ensure nominations compliance with CSAM bylaws and manage online election process. Financial Management • Management of financial activities in QuickBooks Pro. • Process, record, and deposit income from member dues (collected and transmitted by ASAM), conference registration fees and related sales of online educational materials, grant funds, and royalties. Maintain an accurate accounting of income coded to proper accounts. • Pay invoices after receiving approval of the treasurer. Maintain an accurate accounting of expenses coded to proper accounts. • Prepare periodic financial reports, including comparison to prior years, in advance of board meetings and prepare consolidated balance statements that include detail on A/R and A/P for review by the treasurer and presentation to the CSAM Board. • Prepare the annual operating budget, including prior year transaction detail reports, for review by the Treasurer and approval by the CSAM Board. • Monitor and report on investments managed at Capital Advantage investment services. • Process CSAM credit card charges including coding to appropriate account. • Maintain historical data pertaining to CSAM financial performance compared to prior years. Membership Support Services • Maintain YourMembership database for all members, prospects, and others who have participated in CSAM activities. • Manage membership renewals yearly starting in October and processing of membership applications and dues collection is handled by ASAM. Publication Support Services / Website • Ensure CSAM newsletters are published 2-3 times a year. Write, collect and renew content for all of the newsletters and act as a liaison between the editor(s) and those submitting content. • Maintain CSAM website to keep content up to date. • Work with volunteers to manage social media and post regularly.

SCOPE OF SERVICES Conference Management • Oversee annual meeting held in January of each year. Work closely with conference chair to finalize the conference schedule, topics, speakers, assign tasks, etc. • Research potential future conference sites; visit site if necessary; and negotiate contract terms. • Negotiate all auxiliary contracts, including AV, overflow hotel, services for those with disabilities, exhibitor drayage company, etc. • Prepare and maintain the conference timeline. Work with conference chair and course planners to ensure deadlines for key tasks happen as scheduled. • Prepare and maintain speaker management platform for the management of all speakers, communications, collection of CME and MOC-related requirements. Coordinate speaker material review (PPTs) with conference planners. • Prepare and maintain CSAM’s online courses currently housed in EthosCE learning management system (LMS). • Prepare course syllabus in print form (for purchase) and for conference MobleApp. • Develop a conference brochure for distribution via mail, email and posted online. • Conduct various marketing activities to attract registration (e.g. press releases, posting to other organization calendars, GoogleAds, eBlasts to various targeted lists, etc.) • Develop and/or update a prospectus for soliciting exhibitors. • Foster relationships with key staff from aligned organizations to facilitate marketing and promotion exchanges. • Manage online and onsite for 600-800 attendees. Develop registration materials, process advance registrations, prompt participants to register via e-mail notification (e.g. early bird deadlines), process on- site registrations, distribute materials, badges, collect fees, track and report the number of people registered, follow-up after conference to those who did not complete evaluations, etc. • Arrange for accommodations (rooming list) for speakers and conference planners. Make arrangements for President’s Suite and room upgrades to VIPs where applicable. • Order and coordinate the presentation of recognition and gifts (e.g. speakers, planners, conference chair). • Prepare Banquet Event Orders (BEOs) for all conference activities within budgeted amounts. • Coordinate all logistics related to conference AV including recording of selected sessions for online sales. • Organize the participation at the conference of 20+ vendors including a vendor who sells academic books. • Prepare conference budget and monitor compliance with budget. Coordinate with Treasurer, Education Chair, and Conference Chair of any pending variances. Review all conference- related expenses for accuracy and authorize payments of invoices, reimbursements, etc. • Provide sufficient staff (typically ~5 CSAM staff members) to handle on-site registration, distribute printed materials and name badges, distribute tickets for special events, supervise set-up and monitor exhibits, supervise set-up and monitor posters, assist plenary speakers, prepare signage, manage conference MobileApp, manage course evaluation process, orient and coordinate work of temp help, volunteers, and assist conference program chair and participants. • Record feedback from post conference meeting with planners, sent out attendee and exhibitor satisfaction survey, ship all supplies back, reconcile receipts, handle conference closing matters with hotel and other vendors. Continuing Medical Education Program • Oversee all aspects of the Continuing Medical Education (CMEs) program including California Medical Association (CMA) and the American Academy of Family Physicians (AAFP) and any continuing education credit made available for psychologists, therapists, substance use counselors, nurses and physician assistants. File and maintain all necessary provider licenses, agreements and fully understand the breadth of the CME requirements. Public Policy • Provide assistance to the committee as needed in setting the agenda for monthly meetings. • Become knowledgeable on CSAM’s stated positions and communicate important issues as they develop. • Work with CSAM leaders to take best advantage of the opportunities to make CSAM’s voice heard on public policy matters by publishing position papers, communicating CSAM’s positions to policy makers. • Support the CSAM legislative advocate with bill tracking, ballot measure evaluation, committee support and grassroots lobbying efforts. • In conjunction with CMA, when hosted, support legislative action day and coordinate Capitol visits for CSAM attendees.

SCOPE OF SERVICES Leadership Development Retreat • Select venue for the retreat every other year (next one is in 2021). • Develop the curriculum with the President and President-elect. • Coordinate a selection process for participants (invitation only) and send invitations. • Handle logistics with venue such as rooming list, food and beverage orders, etc. • Confirm speakers, topics, and schedule. • Prepare a schedule booklet that includes attendee bios. • Send staff to attend retreat to coordinate speakers and onsite logistics. Technology • Manage and maintain a central server (e.g. SharePoint), containing networked resources essential for the smooth and rapid exchange of information to among staff in order support operations and archive essential association data in one location. • Use budget software (e.g. QuickBooks Pro) that updates and can be quickly shared between both CSAM headquarters and the treasurer if requested. Administrative Support to Collaborative Organizations
Medical Education and Research Foundation (MERF)
• Provide minimal accounting support for the Foundation and work with the Treasurer. • Act as the administrator and secretary. • Oversee scholarship program and provide administrative support for fundraising activities. • Maintain and protect the official records and files, submitting all required filings with the IRS, Franchise Tax Board, Secretary of State, etc. • Ensure CSAM and MERF meet all established grant requirements. • During the annual conference, fully engage in coordinating the activities of 60-80 scholarship recipients who are participating in a structured mentored learning experience with its own curated content, curriculum, and events designed to meet the needs of the scholar audience.
California Patient Protection and Physician Health (CPPPH)
• Maintenance of the CPPPH website • Maintenance of the CPPPH database with the same functioning as is provided by CiviCRM currently • Continuation of the CPPPH newsletter on a schedule determined jointly by CPPPH and CSAM • Accounting and financial management of funds earmarked for CPPPH • Completion of reporting requirements to state and IRS • Ensure representation for CPPPH within the committee structure of CSAM Committee on Physician Wellbeing.
American Society of Addiction Medicine (ASAM)
• Provide a primary staff liaison with ASAM and fulfill the staff requirements of the Chapter Affiliation agreement. • Participate in the ASAM Chapter Council calls (usually 3rd Thursday of every month), also at the ASAM Annual Conference. • Attend various meetings of ASAM as a representative of the society.
California Medical Association (CMA)

FEES SMA association management contracts are billed as a monthly retainer and includes all of the services listed in the final contract. Based on CSAM (RFP), SMA will provide the Scope of Services referenced herein for $23,225 per month or $278,700 annually. In addition, CSAM agrees to allow SMA to include commissionable rates on any hotel contracts negotiated on behalf of CSAM. Direct expenses, including staff travel and meal reimbursements, telephone, office supplies, storage, etc. used on behalf of CSAM are in addition to the fees outlined above and will be billed back to CSAM monthly. SMA provides a detailed invoice with supporting documentation for all reimbursable expenses. SMA does not mark up any of our client’s direct expenses. Graphic design services are not included in the proposal and are billed at $75 per hour. SMA has two exceptional graphic designers on staff that have extensive experience in all forms of print and electronic design as well as marketing and branding.
CONCLUSION SMA is pleased to have the opportunity to submit a proposal to CSAM. As outlined above, SMA is prepared to deliver a depth of excellent services and management for success. Through timely and effective deliver as well as innovative solutions, volunteer leaders’ time will be freed up to focus on (client’s) vision and strategic direction. In delivering the scope of services, SMA has the know-how and resources to partner CSAM deliver value-driven services, state-of-the-art educational programs, compelling member communications and more that will increase and engage membership.
