How can dentistry improve your overall health? Courtesy Dan Mannikko, DDS Reason 1 – Bleeding gums Bleeding gums across all ages is a common clue to show poor gum health. Underneath the gums the bone that secures teeth is slowly eaten away as the infection progresses from gum to bone. Gum disease is a bacterial infection of the mouth often painless until late stages are reached. This chronic infection causes inflammation of blood vessel walls and athlersclerosis as well as linkage to dementia and arthritis. Recent research has also established a gut flora/brain relationship but we are unsure how mouth flora and gut flora affect each other. Reason 2 – Healthy Smile Charles Darwin first posed the idea that emotional responses influence our feelings in 1872. “The free expression by outward signs of an emotion intensi­fies it,” I’d say he’s pretty close. I like these benefits as well. Smiling makes us attractive, we are drawn to people who smile. There is an attraction factor. Smiling reduces stress, blood pressure and boosts our immune system. Studies have shown smiling releases endorphins, natural pain killers and serotonin. Together all help to make us feel healthy. Smiling is a natural drug.


Reason 3 – Tooth Loss A recent study found that tooth loss can speed memory and function decline. More than 3,000 adults over the age of 60 from the English Longitudinal Study of Aging were evaluated. Tooth loss does not have to occur with age, in fact teeth are designed to last a lifetime. If lost or damaged there are many treatment options available. Losing the ability of chew nutritiously dense foods is significant in the geriatric population and can greatly effect health. In some, the loss of teeth/ tooth can cause shifting the jaw or bite leading to painful muscles, joints, teeth and neck. Reason 4 – Sleep Apnea Children and adults can suffer mild to severe symptoms from this condition. You don’t need to snore to have it. Many years of a minimized/obstructed and/or stoppage of breathing during sleep is damaging

to the body. Affected children can lack ability to keep concentration/attention and struggle with behavior. Linkages to many degenerative conditions of the heart, circulatory system and the brain have been found. Dentists can provide a jaw stabilization alternative to masks/cpap once a proper diagnosis is made to keep an open airway. Reason 5 – TMJ Is a collective term created by dysfunction of the chewing system. Primary structures include teeth, jaw joints, muscles, tendons and ligaments. Headaches/migraines, ear symptoms, neck pain are common for these people. Many other symptoms can be present including ear pressure/pain, ringing/buzzing and congestion. This is a treatable condition that can significantly improve ones quality of life once treated.

What is Moxibustion? Courtesy Maureen Lamerdin, O.M.D. Moxibustion is a traditional Chinese medicine therapy using mugwort or moxa, which comes from the plant Ai Ye or Artemisia vulgaris and has been professionally used by acupuncturists and Oriental physicians for centuries.

Moxibustion therapy consists of many different applications of heated ‘moxa’ over certain acupuncture points or along acupuncture meridians in the body. The treatments include using either direct moxibustion or indirect moxibustion. Direct moxibustion consists of using a


grain-size cone of moxa or dry yellow spongy substance made from the mugwort and burning it directly on the skin, the burning moxa is then pinched out or taken away by the therapist before it burns down completely to the skin. The patient experiences a very comfortable sensation of heat that penetrates deep in the skin. In some cases, the moxa is allowed to burn all the way to the skin, such as with its use on warts. In this case, the patient will have a brief moment of pain while the heated moxa touches the skin. The practitioner will then apply burn cream to prevent scarring. Indirect moxibustion consists of burning a moxa cone on the head of inserted acupuncture needles, or using a cigar-type stick moxa to warm the acupuncture points above the skin. There are also many devices that are used for indirect moxibustion, such as belly bowls, back boxes, tiger and lion warmers. In this case the patient will feel warmth in the area of treatment. The most notable use of moxibustion

is for increasing the cephalic positioning of fetuses that are in a breech position. A study of 260 women at 33 weeks of pregnancy demonstrated cephalic version (turning of the breech baby) within two weeks in 75% of fetuses carried by patients who were treated with moxibustion. There are a myriad of other reasons why one would use moxibustion therapy for treatment, some of which include digestive disorders, Raynaud’s, arthritic conditions, chemotherapy induced nausea, asthma, allergies, immune deficiency, dysmenorrhea, amenorrhea, irregular periods and infertility. Biochemical changes to the body by the use of moxibustion show an increase in the production of white blood cells, an increase in the production of red blood cells and hemoglobin and improvement in the circulation of blood and lymph. As with any treatment moxibustion is not for everyone and one should consult an acupuncturist to determine if it may be effective for their condition.

The truth about cavities Courtesy Kevin Olson, DMD Recently I was asked about some statements that have been made on social media claiming “all natural” and “organic” ways to repair dental cavities. I wanted to take a moment to clear up some misconceptions about this. I have heard statements saying, “If you break your arm, your body can heal that. Why not teeth too?” First of all, teeth are not mouth-bones. They do have some similarities, but they are not the same. When a tooth erupts into your mouth in childhood, it has all the enamel (the hard, white outer layer of a tooth) that it will ever have. There is no blood supply or nerves to enamel. Once part of that enamel is gone, through wear, trauma or a cavity, it is gone forever. Tooth decay is the result of bacteria in the mouth. These bacteria consume carbohydrates (“organic” or not) of all kinds, not just candy – soda pop, sports drinks, juice, and even milk. As they consume the carbohydrates they produce acid as a byproduct. They form a sticky

film called plaque which helps to hold the acid against the tooth. Over time the acid erodes the tooth. Initially the affected area of the tooth will look chalky and flat rather than the normal shine. At this point, the damage to the tooth CAN be reversed. Remineralizing the enamel with calcium or fluoride will restore the health of the tooth. Some foods can be helpful in this, but remember, anything with carbohydrates will feed the bacteria. If no action is taken, over time the bacterial acid erodes a hole or cavity into the tooth. This is more than just a hole, though. It is a bacterial infection, and left untreated will continue to grow, as the bacteria continue to feed on the carbohydrates in our mouth. At this point, no amount of calcium or fluoride will restore the tooth. Remember, you can’t grow new enamel once the tooth erupts into the mouth (remineralizing only strengthens weakened enamel). Now the only way to repair the tooth is to go to a dentist where he or she will remove the decayed part of the tooth, the part that is infected with bacteria, and restore the tooth with a filling or crown. Of course, the best plan is to prevent cavities in the first place. Avoid frequent sugary snacks and drinks. Brush your teeth twice a day with a fluoride toothpaste and floss every day. And see a dentist regularly to catch problems when they are still small and possibly reversible.

The big effect of a little drop Courtesy Janet Corbit-Drakulich, O.D., M.Ed. SOME SURPRISING STATISTICS You might be surprised to know that eye examinations can begin at a very early age. In fact, as an important part of eye health and vision evaluations, a dilated eye examination can offer a great deal of information. Take a look at the following statistics from the American Optometric Association, InfantSEE and other various studies: • • • • • •

1 in 10 children are at risk from undetected vision problems 1 in 30 will be affected by amblyopia (lazy eye) which is a leading cause of vision loss in those younger than 45 years 1 in 25 will develop strabismus (often seen as crossed eyes) which is a risk factor for amblyopia 1 in 33 will show significant refractive error 1 in 100 will exhibit evidence of eye disease 1 in 20,000 children will have retinoblastoma (cancer in the eye) which is the 7th most common pediatric cancer

To conduct a dilated eye exam, Tropicamide 1% ophthalmic solution may be used for example to dilate (enlarge) the pupils so that the eye doctor can

see into the back of the eyes. This is especially helpful in infants who will often look away when lights are being shined in their eyes to check them. The drops are also used to help in the assessment of refractive error, like nearsightedness, farsightedness or astigmatism. This drop relaxes the focusing system of the eyes allowing a more accurate assessment of whether or not the infant needs glasses. In deciding whether to use any medicine, the risks of taking the medicine must be weighed against the benefits of using it. Few studies are available concerning these agents in pediatric patients; however, they are widely used. InfantSEE is a nationwide program that provides a one time, no-cost, comprehensive eye assessment of infants from 6 months to one year of age. This service is provided by eye doctors who have instruments and resources not available to general-care doctors. In connection with the InfantSEE program, we have not heard negative reports about the use of Tropicamide and similar agents in infants.


WHAT TO TELL YOUR EYE DOCTOR Make sure you tell your doctor what other prescription and non-prescription medicines your child is taking and of any other medical conditions, especially brain damage, Down’s syndrome, glaucoma and spastic paralysis. Also, tell your doctor if your child has ever had an allergic reaction to Tropicamide or to any other substances including preservatives. Symptoms of allergic reaction include: hives; difficulty breathing; or swelling of face, lips, tongue or throat. NORMAL EFFECTS OF THE DROPS After this medicine is applied to the eyes, the child will have enlarged pupils and will be sensitive to bright light, especially sunlight. When you take your infant outside during daylight hours, even on cloudy days, protect their dilated eyes from ultraviolet (UV) light. Infants may not be willing to wear the disposable sunglasses that are normally given after dilation so parents often protect the eyes with a blanket over the baby carrier, with a sunshade or with a wide brimmed hat. Infants will often close their eyes in bright light as well. Also, your child’s vision will be blurred, especially for objects that are close up. These effects usually last 4-6 hours. Stinging when the medicine is applied is common. POSSIBLE SIDE EFFECTS Some of the known possible side effects of this medicine are: clumsiness or unsteadiness; confusion; fast or uneven heartbeat; flushing or redness of face; hallucinations (seeing, hearing or feeling things that are not there); increased thirst or dry mouth; skin rash; slurred speech; bloated or swollen stomach in infants; unusual behavior; unusual drowsiness; weakness. Contact your doctor if these side effects occur. Headache and swelling of eyelids are more common side effects that often do not need medical attention.

WRAPPING IT UP Undetected visual problems may contribute to learning and developmental issues. Without the drops, certain glasses prescriptions can be missed, especially in the case of farsightedness, and the accuracy of prescriptions may be reduced. In addition, pupil dilation is essential in order to be able to tell parents whether or not the inside of their child’s eyes are healthy. The benefits of these drops usually far outweigh the risks as serious eye and health problems can be missed without their use. Remember, it’s never too early to start good eye health habits! Stroke: Serious as a heart attack Courtesy Carson Tahoe Health You’re a working mom in your 40s with two kids and a jam-packed schedule. Who has time to think about stroke risk? True, most strokes don’t occur until at least age 65. But consider this: In 2009, 34 percent of all people hospitalized for a stroke were younger than age 65. It’s not a man’s disease, either. According to the National Stroke Association, stroke takes the lives of twice as many women as breast cancer.

A HEAD START The best stroke prevention begins with investing in your well-being now, while you’re still healthy. Start by addressing risk factors for stroke, such as:

What you can do: You can’t always avoid migraines. However, if you have a migraine with aura, hormonal contraceptives are not recommended. Talk with your primary care provider about alternative family planning options. Certain oral contraceptives have been known to cause strokes, and frequent migraines could be an early warning sign.

Anxiety. Families and full-time jobs offer plenty of opportunities for worry. Symptoms of a stress-filled life, including high blood pressure, weight gain, sleeplessness and smoking, are also risk factors for stroke. What you can do: Find healthy ways to unwind daily. Research shows simple pleasures, such as listening to your favorite

Migraines. As the International Headache Society reports, severe headaches occur in at least a third of strokes. Not surprisingly, studies have shown a link between frequent migraines accompanied by visual disturbances (called aura) and an increased risk for stroke.

Excess weight. If your waistline has expanded a few inches in recent years, you’re not alone. More than half of all U.S. women are overweight or have a body mass index of at least 25. In 2008, German researchers found that women with 34.6-inch waistlines were four times more likely to have a stroke than women with waist sizes considered in the healthy range for their height. What you can do: Control your weight by making sustainable dietary changes. This slow-andsteady method involves making small, practical adjustments to your food intake for long-term success. Start by drinking one fewer soda each day or replacing white-flour rolls with another serving of vegetables for dinner.

There are about 100 different forms of arthritis, each with unique features, but all can cause pain and discomfort in the joints. The term arthritis refers to a wide range of conditions that cause inflammation of the joints. Osteoarthritis is the most common type. Others, such as rheumatoid arthritis, psoriatic arthritis, reactive arthritis, hemorrhagic arthritis, gout and septic arthritis, may occur less frequently but still have a serious impact on the lives of those who have them. THE USUAL SUSPECT “Osteoarthritis—also known as degenerative arthritis—is the leading cause of disability in aging adults,” says Susan Knowles, MD, FACP, FACR, Rheumatologist with Carson Tahoe Health.

“Osteoarthritis occurs when the cartilage of a joint wears down or is injured,” Dr. Knowles adds. “When this happens, the bone underneath can become damaged, leading to pain and discomfort.” There isn’t always an obvious cause of osteoarthritis, but it is more common in those who are obese and those who have a family history of the condition. Typical symptoms of osteoarthritis include joint pain and discomfort, particularly after activity, exercise or overuse. There may be mild swelling and warmth at the joint, but this is usually not severe. Joints may feel stiff after sitting for a prolonged period of time. There can be bony or knobby changes of the joint, limited range of motion, as well as cracking or grinding noises with joint movement. FINDING RELIEF A variety of medications are available to help alleviate symptoms, including creams, pills and injections. Braces or other joint-support devices may be helpful in providing relief from pain. Physical therapy can often help reduce pain and improve stability. “There are several types of treatments for osteoarthritis that help manage the symptoms and improve function, but unfortunately, there is no cure,” Dr. Knowles says. “Weight loss and exercise are imperative for people with arthritis as they keep the joints loose and help with inflammation.” A rheumatologist is a doctor who specializes in all types of arthritis and can recommend individualized treatment for you.

The free-of-charge 24-hour Carson Tahoe nurse health line is run through REMSA and staffed by specially trained registered nurses (RN) who can assess your medical condition, answer questions, suggest likely causes for your symptoms, and direct you to the most appropriate cost-effective level of care. Depending on the severity of your symptoms, an RN from the nurse health line may recommend any of the following levels of care: Rest at Home – Sometimes rest is the best option. Moderately painful strains and sprains can often be treated with rest, ice, compression, and elevation. Cold symptoms, moderate allergies, mild stomachaches, and many other common ailments may resolve on their own. Speak with an RN from the nurse health line for guidance as to whether

you should monitor your symptoms at home, call your doctor, or visit a care clinic. See your Physician – Your physician manages your overall care, coordinates ancillary care, refers and consults with health specialists, and maintains records of all health services. Ideally, patients should try to be seen by


their primary care physician first for nonemergency issues, but illnesses do not always strike during normal office hours. Fevers, sore throats, cuts, broken bones, earaches, and many other conditions can be discussed with an RN from the nurse health line, and depending on the severity of the injury or illness, they can direct you to the appropriate care. Level 1: Walk-in Clinics ($) A nurse practitioner or physician assistant can treat the lowest severity of health concerns at a walk-in retail clinic. The cost is low, the wait is minimal, and most insurance plans are accepted. The clinics can treat earaches, cough/ congestion, sinus infections, and sore throats. Additional services provided include physicals and preventative care. Many of these clinics offer extended hours and weekend service. Level 2 ($$): Urgent Care – Patients with illnesses or injuries of moderate severity may seek treatment from an urgent care center. At urgent care centers, physicians work with the nurse practitioners and physician assistants to evaluate and care for minor health issues such as lacerations, sprains or strains, infections, upper respiratory infections, and non-life threatening emergencies. Level 3 ($$$): Emergent Care – Emergent care centers treat similar ailments to urgent care clinics, however they also have the expertise to treat more complicated matters and offer onsite radiology services for diagnostics. Emergent care centers not only treat everything from sprains/broken bones, lacerations, mild chest pains, and upper respiratory infections, but they can also stabilize events such as stroke or cardiac emergencies.

Level 4 ($$$$): Emergency Room – The emergency room is appropriate for those who have a serious, life-threatening medical emergency, suffer from several various medical conditions, or are in need of immediate treatment after hours. If you are having a medical emergency, you should call 911 immediately. By considering location, cost and severity of the care needed, an RN from the nurse health line can help you determine which healthcare setting has the providers and technology that best suits your illness or injury in the most costeffective, timely manner. Cholesterol: The good, the bad and the fluffy Courtesy Carson Tahoe Health Small quantities of cholesterol are necessary to support human life. There are two main kinds of this waxy substance, LDL and HDL (high-density lipoprotein). While high levels of HDL are a good thing helping to prevent heart disease—an excess of a certain kind of LDL can contribute to plaque buildup, increasing your risk of heart disease and stroke. “All LDL is not created equal,” says Susan O’Leary, MD, PhD, FACC, Cardiologist with Carson Tahoe Cardiology. “Some LDL particles are large and fluffy, and others are small and dense. The small, dense particles are the ones that are particularly bad. They move through the body easily, meaning they can cross the inner lining of blood vessels, where they collect and form plaque.” UNDERSTANDING YOUR NUMBERS The American Heart Association recommends everyone 20 years of age and older have their cholesterol levels checked at least every five years. An ideal LDL concentration for a healthy adult is

fewer than 100 milligrams per deciliter of blood. Could cholesterol be putting you at risk for heart disease even if you have low levels of LDL? The number of LDL particles may be more important than the overall concentration of LDL. “Someone with a low LDL concentration but a high particle count is still at risk of developing plaque buildup and having heart trouble,” Dr. O’Leary says. “But an individual with a high LDL concentration and low particle numbers fares almost as well as someone with low numbers across the board. The large, fluffy particles do not carry the same risk as the smaller, dense particles.” A standard cholesterol screening does not test particle count, but a special blood test offered at Carson Tahoe using nuclear magnetic resonance (NMR) does. Your doctor can help you decide if NMR testing is necessary for you.


Depression – Not your normal signs of aging Courtesy Carson Valley Medical Center Depression affects more than 6.5 million of the 35 million Americans aged 65 years or older. Depression in older persons is closely associated with dependency and disability and causes suffering for the individual as well as for his or her family. Roughly 17% of the senior population in Douglas County suffers from a mental health disorder and greater than one in ten meets the diagnostic criteria for Major Depression. When looking at only those seniors who are dealing with a physical health condition, the percentage of depressed individuals more than doubles. Because elderly people do face challenges to their connections and their independence, depression in seniors often goes unrecognized, and may be shrugged off as a “normal” part of the aging process. Symptoms of depression in older persons may differ from symptoms in other age groups. Some of the signs of depression in seniors are characterized by:

Memory problems Confusion Social withdrawal Loss of appetite Weight loss Vague complaints of pain Inability to sleep Irritability Delusions Hallucinations Moving in a slower manner Demanding behavior

Untreated depression can lead to serious consequences including increased isolation, worsening physical health problems, and even suicide. “The often misunderstood disease of depression is not a result of weakness, poor life-choices, or morale failings.” Says Frank Jacobelli, LCSW. “Advances in our understanding of the brain have made clear that depression, like diabetes, is a biological disorder. And like diabetes, depression is treatable.”

Frank Jacobelli is a Clinical Social Worker and Primary Therapist at Vitality for Life, an outpatient mental health and wellness program at Carson Valley Medical Center. I was able to pull my life back together.” Vitality for Life offers seniors the opportunity to explore the challenges they face from day to day, learn and practice the coping skills they need to overcome them, and live life to its fullest. “If I’ve learned one thing working with seniors over the past three years it’s that their issues aren’t much different from those we face in the earlier stages of life: Loneliness, isolation, unresolved trauma, relationship conflict, and grief and loss.” Says Jacobelli, “But the best part of working with this population is watching them begin to get excited about meeting their futures head on. And our clients tend to do it with such amazing grace and style. In addition to improving blood flow to discs, joints and muscles, exercise makes muscles more flexible, stronger and resistant to future strain.
