De-stress and Refresh for the Holidays

E

very year, do you find yourself in full panic attack mode, starting before Thanksgiving Day and going all the way through to the New Year? With Christmas displays appearing in stores by the end of September, do you find yourself bracing for the massive blitzkrieg of full-scale holiday marketing – and all the stress that can go with it? A Consumer Reports survey recently found that 90% of Americans find at least one thing stressful about the holiday season. Throughout the holidays, you’re faced with shopping and crowds. Back-to-back diet-busting parties. Interminable chats with distant family and in-laws. And the list goes on and on. It’s easy to set unrealistically high expectations for everything to be perfect during the holidays. Then, when you take on too much, anxiety and stress surely follow, since our reality seldom measures up to the flawless fantasy in our heads. So, why should you allow a season that’s supposed to be about joy and hope to become something quite the opposite? This year, resolve to unwrap more happiness throughout the holidays and dodge holiday stress, while relaxing and actually enjoying your holidays. One of the most effective ways to fight stress and find serenity is by practicing yoga. This mind-body practice combines physical poses, controlled breathing, and meditation or relaxation to reduce stress, lower blood pressure, and lower your heart rate. And almost anyone can do it. In fact, introducing your kids to yoga at an early age can help them learn healthy lifestyle habits and set the foundation for a fit future. While yoga has many styles, forms and intensities, hatha yoga is a good choice for stress management, and beginners like its slower pace and easier movements. Many of the movements and poses are kid-friendly, so your whole family can enjoy the stress-reducing benefits of yoga. The core components of hatha yoga are: • Poses. This series of movements increases strength and flexibility, and can range from lying relaxed on the floor to difficult postures that may have you stretching your physical limits. • Breathing. Yoga teaches you to control your breathing, to help control your body and quiet your mind.

• Meditation or relaxation. Most yoga sessions incorporate these to help you learn to be more mindful and aware of the present moment, while allowing your head to clear out some of the clutter of daily living – and holiday madness. Studies have shown that one of the biggest health benefits of yoga is stress reduction. In fact, the sense of calm and well-being that flows from yoga have been shown to reduce risk factors for chronic diseases, such as heart disease and high blood pressure. Experts also believe yoga helps alleviate chronic conditions, like depression, anxiety, pain and insomnia. And particularly around the holidays, we could all use these kinds of health benefits. Other ways to de-stress during the holidays include: • Take care of your body. Get regular exercise – including yoga; go easy on the caffeine, sugar and alcohol; get enough rest; and eat healthy. • Take a “time out” when you’re feeling crazed. Whether you have 15 minutes or 5, step away from all the activity and madness. Take a short walk, snuggle with your favorite toddler, play with the dog, call a good friend, or listen to some music. • Find humor wherever you can. Look for the ludicrous, paradoxical and nonsensical in daily life. And laugh out loud. A good belly laugh changes your biochemistry and clears out emotional baggage like nothing else. • Be realistic and know your limits. Don’t let unrealistic expectations push you into attempting more than you can reasonably manage. Learn to say no graciously. Then enjoy the satisfaction of working mindfully and getting essential things done. • Throw something out every day. Clutter is a proven, lowlevel, subliminal stress inducer. So, commit to getting rid of one or two extraneous items daily, and toss out some of your stress along with them. • Don’t be too proud to ask for help. When things start piling up, don’t get grumpy – get help. Reach out to family and friends and look for ways you can cooperatively work together to achieve common goals – while helping each other out. • Take time to enjoy the moment. Be mindful of what you’re doing at various moments throughout the day, and use even mundane tasks as a chance to meditate and gain peace and balance. Notice beauty, kindness, and love around you, and take time to breathe it in – and even return it to those around you. This year, resolve to get more enjoyment from the holidays and celebrate the season for what it should be. Take positive actions now to plan the upcoming season to reduce the pressure and stress – and immerse yourself fully in this lovely time of year.

What is PHYSIATRY?

Maybe you’ve never heard of physiatry, but it could just be the medical specialty that rescues you from a restrictive, painful, life-altering injury or illness.

hysiatry is non-surgical physical medicine and rehabilitation (PM&R) for patients who have been disabled as a result of a disease, condition, disorder or injury. When you visit a physiatrist, you’ll experience a personalized method of treatment, designed to improve your quality of life – one based on a comprehensive approach that expands the framework of resources at your disposal. And your recuperation will involve every aspect of your life, from exercise to diet to your job. Physiatrists are medical doctors who have completed training in the specialty of PM&R, with broad medical expertise that allows them to treat disabling conditions throughout your lifetime – no matter what your age. Treatment focuses on function, including the diagnosis and treatment of pain, and physiatrists work with other medical professionals on your treatment team, including neurologists, orthopedists, physical therapists, psychiatrists, counselors, and others. While other specialists use similar diagnostic tools, physiatrists are trained in electrodiagnostic medicine to detect areas of nerve and muscle damage. This includes electromyography (EMG) and nerve conduction studies (NCS), to measure the condition of your nerves and muscles in both their relaxed and stimulated states. This head-to-toe approach to your health, focusing on your entire body, can be especially beneficial if your problem is difficult to diagnose, such as spinal and knee pain which can be the result of anything from arthritis to muscle strain. Because physiatrists look at the whole patient, they often notice aspects of your health that may have been previously overlooked. Once your physiatrist has a diagnosis, you’ll participate in creating a treatment plan that looks at the “big picture” to improve function and reduce pain. “It’s our mission to restore mobility to patients affected by everything from stroke and brain injuries to back pain and shoulder immobility,” says Amrish Patel, M.D., a physiatrist with Sports and Spine Institute in McDonough. “We focus on helping you become as functional and pain-free as possible in order to

participate in your favorite activities and enjoy life as fully as possible.” “A doctor may have told you that you have arthritis in your hip or a repetitive stress injury to your knee,” Patel says. “But that doesn’t mean you need surgery. Often, we can treat patients with physical limitations using conservative, holistic approaches – with less risk of complications.” To help you achieve your health goals, physiatrists coordinate a variety of non-surgical treatments. Such treatments include prescribing and directing physical therapy, educating on home exercises including stretching and strengthening, determining appropriate bracing and assistive devices, recommending massage therapy, performing peripheral joint injections, spinal injections including epidural steroid injections/facet injections, nerve stimulators, nerve ablation, as well as prescribing pain medications. Other treatments include lifestyle modifications with diet, exercise, weight loss, tobacco cessation and mindfulness to improve your overall health and achieve your long term goals. “What’s more, if you should end up requiring surgery, we can work with you before the surgery to ensure the best possible outcome, following up with treatment after the surgery to help speed your recovery,” Patel continues. Common Questions About UROGYNECOLOGY

any women are accustomed to visiting their OB/GYN each year for their routine gynecological care. But for those past childbearing years – or simply those whose needs no longer require the services of an obstetrician – visiting a physician who provides only gynecological and urogynecological care may be a more straightforward, convenient and comfortable experience. This month, we talk with Dr. Stephanie Gordon of The Women’s Center in Conyers and Stockbridge about how a urogynecologist may better suit your needs for your life stage. Question: What is a urogynecologist? Dr. Gordon: A urogynecologist manages clinical problems associated with dysfunction of the pelvic floor and bladder. Pelvic floor disorders can encompass a wide range of issues, from those affecting the bladder to disorders of the reproductive organs to bowel issues. Question: What are some of the services your office provides patients? Dr. Gordon: In addition to Well Women Health Care, we offer menopausal management, a full range of contraception options, infertility workups, and options to treat bladder problems, bowel leakage, pelvic prolapse, abnormal bleeding as well as screening for sexually transmitted diseases. Question: Can a urogynecologist help with problems like incontinence? Dr. Gordon: Yes, we understand that women wrestle with this issue, which can cause untold embarrassment, inconvenience and discomfort. We also recognize that incontinence is much more common than many people realize, affecting about 35% of women. That’s why our staff is specially trained to help with bladder problems and incontinence, as well as pelvic floor conditions which can lead to these issues. Our staff includes both gynecologists and urogynecologists – physicians who have specialized training in surgical and nonsurgical treatment of pelvic floor disorders. So patients can feel comfortable discussing these kinds of issues, knowing that we have the expertise to treat these disorders and find solutions for patients. Question: What type of non-surgical treatments are available for incontinence? Dr. Gordon: Depending on your diagnosis, the severity of your symptoms and your overall health, we might try medications, pelvic muscle exercise, lifestyle and dietary modifications, use of a vaginal support device called a pessary, or even biofeedback. Often, these provide relief of incontinence, but if your condition doesn’t improve, surgery is another option. Question: What kind of procedures are there? Dr. Gordon: Many of the available surgical procedures for

incontinence and prolapse are minimally invasive, including some robotic-assisted surgeries that may offer a more effective treatment option. Using tiny, 1-2 cm incisions, a surgeon can operate with greater precision and control, so your recovery is faster and less painful, with a better clinical outcome. Some common surgeries for incontinence include suburethral slings, which are small mesh slings placed under the urethra to help prevent urinary leaks. Another is called a sacrocolpopexy, which is a robotic-assisted procedure that uses mesh to hold the vagina in the correct anatomical position to correct vaginal vault prolapse. There are also other types of vaginal reconstruction for prolapse, with or without mesh implantation. And these are just some of the procedures which can be offered to treat incontinence or prolapse. The right procedure for you will depend on your diagnosis. Question: A gynecologist sounds ideal for a woman dealing with menopause. Dr. Gordon: Women who are approaching menopause are shifting to a different way of looking at their reproductive health, and our practice offers a positive healthcare experience for those facing this journey. We embrace both traditional and alternative methods of managing the symptoms of menopause, including hormone replacement therapy. Menopause doesn’t have to be miserable! We can treat all symptoms, including hot flashes, night sweats, vaginal dryness, low libido, sleep disorders, and osteoporosis. We have many tools, including bio-identical and compounded hormones, hormone injections and pellets, and more. We also take osteoporosis seriously, offering oral medications, quarterly Boniva injections, twice-yearly Prolia, or once-yearly Reclast infusions. Question: Are there any specific advantages of a urogynecologist over an OB/GYN? Dr. Gordon: A urogynecologist can provide the full range of services, without the inconveniences an OB/GYN practice can sometimes cause. The holidays can be healthy, but you have to put diet and exercise at the top of your list above shopping and holiday cheer. Don’t let the cold months of winter be an excuse to cut back on your exercise routine. Consider these calorieburning indoor activities: • Walk your local mall. • Take the stairs whenever possible instead of an elevator or escalator. • Walk around your building’s hallways during your lunch or coffee break. • Don’t wait for spring for a thorough house cleaning. Wash the inside of your windows, vacuum or shampoo the carpet, or clean out your closets. • Watch and follow an exercise video or DVD rather than a movie. • Get up and move during TV commercials—a one-hour show can yield 20 minutes of stepping in place, using a stretchy band, or lifting weights. When it comes to muscle: use it, lose it or build it! It allows us to stand, walk, balance, lift and breathe. Building and maintaining muscle is important for both men and women, because strong muscles help reduce back pain, prevent falls and more. People over 30 start losing bone mass and are more susceptible to osteoporosis (weak bones) and fractures if they fall. Resistance training (using light weights) at least twice a week and consuming a balanced diet may help to slow or partially reverse the effects of natural, gradual bone loss. Mix on low speed using an electric mixer until thickened in regular eggnog. (adjust sugar substitute to taste). 3. Keeping Active Can Help Reduce Risk of Falling for Elderly

F

alls don’t “just happen,” and people don’t fall simply because they get older. Often, there’s more than one underlying cause or risk factor involved when an elderly person falls. It’s true that our risk of falling and being injured increases as we age. In fact, after age 65, there’s a onein-three risk of falling. But there’s plenty you can do to avoid becoming one of the statistics, with the broken bones or other injuries that can lead to declining health, isolation and a loss of independence. Risk factors Aging brings many physical changes, and many medications including diuretics, sedatives and high blood pressure medications can alter your sense of balance. Health conditions that affect older adults such as cataracts, glaucoma, stroke, Parkinson’s disease, congestive heart failure, heart arrhythmias, emphysema, arthritis and nerve damage can increase your risk of falls.

Other common risk factors for falls include: • Muscle weakness, especially in the legs. • Poor balance or difficulty walking caused by lack of exercise, a neurological cause, arthritis or other medical conditions or treatments. • Blood pressure that drops too much when you rise from lying down or sitting – called postural hypotension – that can result from dehydration, certain medications, or conditions such as diabetes, Parkinson’s disease, or an infection. • Slower reflexes which can make it harder for you to catch your balance if you start to fall. • Sensory problems such as poor vision, numbness in your feet, or wearing multi-focal glasses while walking. Exercise helps While your risk of falling increases with age, you can take steps to help prevent falls. First talk with your doctor. You may need to have your vision as well as your balance and movement checked. Your prescription

medications may need to be changed. Many people can reduce their risk of falls by exercising, improving their balance, and implementing safety measures at home. One of the best exercises to help prevent falls is walking. Walk regularly to avoid getting rusty. Water or pool exercises can also help prevent falls by helping you practice the skills needed for walking.

• Arrange furniture so you can easily move around it. • Don’t use throw rugs. All carpeting should be secured to the floor. • Use a step-stool to reach something from a high shelf or move items to lower shelves. • Install grab bars on walls around the tub and beside the toilet. • Use nonskid mats or adhesive strips on surfaces that will get wet. • Put a light switch and the telephone within reach of your bed. • Use a nightlight between the bedroom and bath. • Keep stairs and hallways clear of clutter. • Install handrails on both sides of the stairway. • Wear rubber-soled shoes that have low heels.

Improving your balance To improve your balance and coordination, practice standing on one leg for short periods. You can hold onto a chair while you’re doing this to help keep your balance. Tinnitus: the false perception of sound where no sound is present.

O

ften called “ringing in the ears,” tinnitus can also be a buzzing, roaring, clicking, hissing, swooshing, or cricket-like sound, affecting over 25 million Americans. While it may appear to be a minor annoyance, however, for some individuals it can be quite disruptive. “I’ve had ringing in my ears for years now,” says Ted Watkins, a 59-year-old male, who says the problem interferes with life on a daily basis. “Sometimes, it makes it hard for me to concentrate on the job. Other times, it literally keeps me awake at night.” What Watkins and countless other sufferers have discovered is that there’s no way to measure a sound that only the patient can hear. But this persistent sound – while not usually caused by anything serious – can be the source of real mental and emotional anguish. WHAT CAUSES TINNITUS? Scientists believe tinnitus really starts in the brain, in the network of brain cells (neural circuits) that interpret the sounds our ears hear. While scientists aren’t sure what’s happening in the brain to create the illusion of sound, they agree that the first thing you should do when tinnitus appears is see your ENT (Ear, Nose, and Throat) physician. Sometimes, tinnitus is the result of something as simple as ear wax blocking the ear canal. But it can also be the result of a number of health conditions, including: · Noise-induced hearing loss · Ear and sinus infections · Diseases of the heart or blood vessels · Ménière’s disease · Auditory nerve tumor · Hormonal changes in women · Thyroid abnormalities · Medication side effect

he natural aging process, genetics and childbirth can all contribute to changes which affect feminine wellness. Multiple births, a particularly strenuous vaginal childbirth, and the effects of menopause can cause physical changes which rob women (and their partners) of their sense of well-being and happiness. Many other types of skin problems are only temporary and can be treated with medications or simply with time. Examples of these include cold sores, plantar and palmer warts, and skin tags. For example, plantar warts – which appear on the bottom of the foot – can now be treated by injecting Candida yeast directly into the wart. As a general rule, you should see a dermatologist for the following: ➧ Cystic or stubborn acne that doesn’t respond to over-the-counter products: If 2-3 months of using products containing benzoyl peroxide and salicylic acid doesn’t clear up your acne, you probably need prescription medications to tackle the problem. ➧ Dark discolorations or moles on your skin that are changing, won’t heal or bleed: These may be skin cancer, so see a dermatologist as soon as possible. Even if you don’t see any of these warning signs, you should be examined regularly for signs of skin cancer, particularly if you are fair skinned or have a history of tanning or burning. ➧ Itching or rashes that won’t go away: This could be eczema or some other type of dermatitis. You could also be having an allergic reaction. A dermatologist can prescribe medications to take care of these bothersome skin conditions. ➧ Skin disorders or skin that won’t heal: Psoriasis, chronic seborrhea or dandruff, rosacea, diabetesrelated calluses, or wounds that don’t heal may require prescription oral or topical drugs. ➧ Scars, skin discolorations, stretch marks, or other cosmetic concerns: Dermatologists have access to prescription options, laser treatments, and other cosmetic procedures that can address these concerns. s a kid, you could count on your parents to be there for you and take care of your every need, no matter what. Today, those roles have been reversed, as aging parents now often depend on their adult children for care, at least on some level. The challenges of a changing age structure in America today are making themselves felt through families in ways we could never have imagined decades ago. “There’s a greater likelihood today that, as a 55-year-old, you’ll have surviving parents, than there was, for example, in the 1920s, when both parents passed away before you turned 50,” says Neal Cutler, Ph.D., executive director of the Center on Aging for the Motion Picture and Television Fund in Woodlawn Hills, California. “Now middle-agers who are planning their own retirement are also having to think about their parents.” This role reversal can be fraught with serious – and sensitive – issues. Aging parents may no longer be able to perform certain activities of daily living, such as cooking or bathing. When we step in to help with those functions – or arrange for a caregiver to handle that role – it requires the right attitude and an appreciation for the rewards of the situation for all concerned. One of your parent’s biggest fears may be the fear of losing their position as the parent. But as long as you remember that this is the person who brought you into this world and who has always loved and cared for you, everyone can learn to work with whatever the situation may be. Sometimes, caring for a parent in your own home—with or without assistance – may be a workable solution. Other situations may call for placing your parent in an assisted living home, where residents can get help with activities of daily living. That’s never an easy decision, but when you come to the conclusion that assisted living is the right choice at this time in your parent’s life, choosing the right facility can
