The South Asian Heart Center offers you a unique opportunity to work with a diverse and experienced team focused on reducing the incidence of coronary artery disease and diabetes in the South Asian community. Heart Health Coaches are central to the mission of the center and make a big impact on our participants’ experience as well as the effectiveness of the Center’s mission. Studies indicate that behavioral changes are easier to make with regular and frequent follow up by health coaches. A recent study also showed that culturally competent heart health coaching by the Center’s volunteer coaches, resulted in significant improvement in markers like Total Cholesterol (TC), LDL, HDL, Triglycerides and TC/HDL ratio. Volunteer Position/Title: Clinical Follow up Intern Goal of Volunteer Position: Follow up with program participants and motivate them to embrace recommended lifestyle changes for better heart and diabetes health. Volunteer Responsibilities: • Connect with an assigned set of participants at their annual touch point • Monitor progress against lifestyle management goals and recommendations • Motivate program participants to embrace better lifestyles and periodic testing • Record progress and compliance • Update electronic medical records, test results, correspondence etc. • Participate in health fairs and other events hosted by the Center Qualifications: • Ideal position for pre-health, prospective medical school students or foreign medical graduates • Good verbal and written communication skills in English • Good computer skills especially with MS Word and MS Excel • Enthusiastic attitude and strong work ethic • Ability to work independently or as part of a team • Experience with one-on-one counseling/interviewing and knowledge of one or more South Asian languages is a plus Timeframe: Length of time/duration: Start and end dates are flexible, however a minimum commitment of 3 months is requested. Estimated hours: As this is a volunteer position, the hours are flexible; however a minimum of 18-24 hours per week is requested. Remote working opportunities are not available for this position. Volunteer work can be performed at the Center locations in Mountain View or Los Gatos, any time between 8 AM and 5 PM on weekdays. Scheduling: Scheduling is flexible but may vary based on organization’s needs, project deadlines, etc. Mandatory Training: An initial training and orientation will be provided in order to familiarize volunteers with the Center’s mission, goals and programs, staff and other team members. Training on updating electronic medical records and the heart health coaching process is also included. Commitment to training the first 2 weeks, at least 4 times per week, 10am – 4 pm, is required, but accommodations can be made on a case-bycase basis.
