Inslee said Sunday that the decisions were not made lightly but were necessary to slow the spread of COVID-19. “We understand that this is both an economic and a public health crisis,” Inslee said at his press conference. “We do not take lightly the impact these restrictions will have on local businesses, many of which have already had a very difficult year. You are not alone, and we will continue to provide supports.” On Friday, Inslee issued a travel advisory, recommending residents stay close to home and for interstate and international travelers to quarantine 14 days after entering the state or returning home from

After Sunday’s announcement, Seattle Mayor Jenny Durkan stated in a press release that Inslee and public health officials had no choice but to act. Men should understand prostate cancer risks
By Dr. Yaw Nyame
Seattle Cancer Care Alliance
Prostate cancer is the second most common cancer in men in the United States. According to data from the American Cancer Society, there are more than 3 million men who have had a prostate cancer diagnosis living in the United States today. More than 190,000 new cases will be diagnosed, and more than 33,000 American men will die from prostate cancer this year. Prostate cancer is a serious disease, but if it is detected early, it is highly treatable. This is why it is important for men to understand their risks for prostate cancer, know the signs and symptoms of the disease, and consider regular screenings if in good health. Understanding Your Risks Knowing your prostate cancer risk will help inform conversations with your doctor and may impact recommendations on when you should receive screen-

Screenings The most common prostate cancer screening test is a prostatespecific antigen test which has resulted in early diagnosis in about 80 percent of men with the disease. The test measures the amount of PSA, a protein produced by tissue in the prostate, in your blood. PSA testing has been associated in a percent reduction in dying from prostate cancer among men who undergo routine testing. Screening however can be associated with harms which include detecting indolent cancers. The American Urological As-
sociation recommends beginning PSA testing for men at high risk for prostate cancer, including men with a family history, as early as age 40. For those with normal risk, the Association recommends beginning regular screening at age 55. Genetic testing also may be appropriate for certain men — particularly those who have strong family histories of prostate, breast, ovarian and colorectal cancers. I encourage all men to talk to their physician about their prostate cancer risks and recommended screening options. This vital step may mean the difference between early detection and advanced disease for some men and can save lives. For more information about prostate cancer, visit https://www. seattlecca.org/diseases/prostatecancer/prostate-cancer-facts. Time to get informed about COVID vaccines By JoNel Aleccia and Liz Szabo Kaiser Health News
Drugmaker Pfizer is expected to seek federal permission to release its COVID-19 vaccine by the end of November, a move that holds promise for quelling the pandemic, but also sets up a tight time frame for making sure consumers understand what it will mean to actually get the shots. This vaccine, and likely most others, will require two doses to work, injections that must be given weeks apart, company protocols show. Scientists anticipate the shots will cause enervating flu-like side effects — including sore arms, muscle aches and fever — that could last days and temporarily sideline some people from work or school. Even if a vaccine proves 90 percent effective, the rate Pfizer touted for its product, 1 in 10 recipients would still be vulnerable. That means, at least in the short term, as population-level immunity grows, people can’t stop social distancing and throw away their masks. Left out so far in the push to develop vaccines with unprecedented speed has been a large-scale plan to communicate effectively about those issues in advance, said Dr. Saad Omer, director of the Yale Institute for Global Health. “You need to be ready,” he said. “You can’t look for your communication materials the day after the vaccine is authorized.” Omer called for the rollout of a robust messaging campaign based on the best scientific evidence about vaccine hesitancy and acceptance. The Centers for Disease Control and Prevention has created a strategy called “Vaccinate with Confidence,” but it lacks the necessary resources, Omer
said. “We need to communicate, and we need to communicate effectively, and we need to start planning for this now,” he said. Such broad-based outreach will be necessary in a country where, as of mid-October, only half of Americans said they’d be willing to get a COVID-19 vaccine. Initial doses of any vaccine would be limited at first, but experts predict they may be widely available by the middle of next year. Discussing potential side effects early could counter misinformation that overstates or distorts the risk. “The biggest tragedy would be if we have a safe and effective vaccine that people are hesitant to get,” said Dr. Preeti Malani, chief health officer and a professor of medicine at the University of Michigan in Ann Arbor. Pfizer and its partner, the German firm BioNTech, said last week their vaccine appears to protect 9 in 10 people from getting COVID-19, although they didn’t release underlying data. It’s the first of four COVID-19 vaccines in large-scale efficacy tests in the U.S. to post results. Data from early trials of several COVID-19 vaccines suggests that consumers will need to be prepared for side effects that, while technically mild, could disrupt daily life. A senior Pfizer executive told the news outlet Stat that side effects from the company’s COVID-19 vaccine appear to be comparable to standard adult vaccines but worse than the company’s pneumonia vaccine, Prevnar, or typical flu shots. The two-dose Shingrix vaccine, for instance, which protects older adults against the virus that causes shingles, results in sore arms in 78 percent of recipients and muscle
pain and fatigue in more than 40 percent of those who take it. Prevnar and common flu shots can cause injection-site pain, aches and fever. “We are asking people to take a vaccine that is going to hurt,” said Dr. William Schaffner, a professor of preventive medicine and health policy at Vanderbilt University Medical Center. “There are lots of sore arms and substantial numbers of people who feel crummy, with headaches and muscle pain, for a day or two.” Persuading people who experience these symptoms to return in three to four weeks for a second dose — and a second round of flu-like symptoms — could be a tough sell, Schaffner said. How public health experts explain such effects is important, Omer said. “There’s evidence that suggests that if you frame pain as a proxy of effectiveness, it’s helpful,” he said. “If it’s hurting a little, it’s working.” At the same time, good communication will help consumers plan for such effects. A COVID-19 vaccine is expected to be distributed first to health care staffers and other essential workers, who may not be able to work if they feel sick, said Dr. Eli Perencevich, a professor of internal medicine and epidemiology at the University of Iowa Health Care. “A lot of folks don’t have sick leave. A lot of our essential workers don’t have health insurance,” he said, suggesting that essential workers should be granted three days of paid leave after they’re vaccinated. “These are the things a well-functioning government should provide for to get our economy going again.” Making sure consumers know that a
COVID-19 vaccine likely will require two doses — and that it could take a month for full effectiveness to kick in — is also crucial. The Pfizer phase 3 trial, which has enrolled nearly 44,000 people, started in late July. Participants received a second dose 21 days after the first. The reported 90 percent efficacy was measured seven days after the second dose. Communicating effectively will be vital to ensuring consumers follow through with the shots and — assuming several vaccines are approved — that their first and second doses are from the same maker. Until full protection kicks in, Omer said, people should continue to take measures to protect themselves: wearing masks, washing hands, social distancing. It’s important to let people know that appropriate action now will pay off later. “If we just show them the tunnel, not the light, then that results in this mass denial,” he said. “We need to say, ‘You’ll have to continue to do this in the medium term, but the long term looks good.” The best communication can occur once full data from the Pfizer trial and others are presented, noted Dr. Paul Offit, a vaccinologist at the Children’s Hospital of Philadelphia who sits on the federal Food and Drug Administration’s advisory board considering COVID-19 vaccines. “When you look at those data, you can more accurately define what groups of people are most likely to have side effects, what the efficacy is, what we know about how long the efficacy lasts, what we know about how long the safety data have been tested,” he said. “I think you have to get ready to communicate that. You can start getting ready now.”

SAFEWAY, FROM PAGE 1 Safeway. The current Safeway, 2100 Queen Anne Ave. N., will be demolished. The housing is actually broken into three separate buildings, on Boston and First Avenue, Crockett and First Avenue North and on Queen Anne Avenue, which is also where the Safeway storefront will be. Project developer Maria Barrientos, barrientosRyan LLC, said the biggest changes to the concept from the last time the design team went in front of the Design Review Board was to the Safeway. Community members were concerned Queen Anne would end up with a single, “monolithic” storefront. At the last meeting, Design Review Board members advised architects work with Safeway’s team to break that up and present a look more in line with the neighborhood’s plan for retail, possibly with different business fronts. With Safeway’s approval, changes were made to the deli, Starbucks, florist and pharmacy. “This is quite a departure for them,” Brian Runberg, Runberg Architecture Group, said of the changes at last week’s meeting. According to the plans, the Starbucks includes four new components: 1. A direct entrance off of Queen Anne Avenue so people can walk in off the street. 2. An indoor/outdoor counter

How do you make cider? We can make cider with juice from apples. There are many different kinds of apples and a few different ways to squeeze out the juice. My friend Bri Valliere told me all about it. She’s a food scientist at Washington State University who knows a lot about cider. The first step is to pick out the apples. Honeycrisp apples will make a sweet cider. Granny Smiths are more acidic and will make a tart cider. “We could make a single batch of one kind, or we could mix different kinds of apples together and see how it turns out,” she said. “No matter what, it’s going to taste good.” It’s important to wash the apples to remove any dirt or bacteria. Next, it’s time to squeeze out the juice. “It’s not like oranges or grapes where you can just squeeze them and the juice comes out,” Valliere said. “We need to get the apples into smaller pieces.” After a grown-up helps cut the apples into quarters, the slices can go into a juice press. One kind of press is a basket press, which is like a small barrel with a device to grind up apples. There’s also a wooden plate that goes inside the barrel on top of the ground-up apples. As we push down on the wooden plate, the force squeezes out the juice from
the fruit. The juice flows out the bottom of the press, which works a bit like a pasta strainer. It separates the apple solids from the liquids. As the juice flows from the press, we can catch it in a jug or bucket. Valliere said another kind of press is called a bladder press, which has a balloon-like device that pushes against the fruit to squeeze out the juice. While you may not have a basket or bladder press at home, with the help of a grown-up you can find a recipe online that uses similar steps: Use small pieces of fruit, press the fruit to make some juice, and pasteurize it. When we pasteurize the juice, it helps kill any harmful bacteria that could possibly make us sick. All we have to do is heat up the juice up to at least 160 degrees Fahrenheit. Finally, it’s time to enjoy the cider. While some cider comes from small farms or our kitchens, there are also machines in factories that produce thousands of gallons of juice. Valliere told me that juice processors sometimes will add something called enzymes to the pressed juice. The enzymes help turn the light brown, cloudy cider-looking juice into the clear, golden liquid we know as apple juice. A big part of the reason we
and window will allow people to make purchases from the outside. 3. An indoor seating counter with windows will open directly to the sidewalk. 4. A large seating area outside with chairs and overhead protection was added. The floral shop will also open to the sidewalk with operable windows. People will be able to buy flowers from outside at another window. The pharmacy will have a separate entrance from the outside for people who only want to go there. The new store will have an expanded deli with an outside entrance, a walk-up order window, an indoor/outdoor seating counter and covered seating space with tables and chairs. In front of the deli will be a public plaza, a little more than 2,000 square feet, for everyone in the community to use. It will include seating, artwork and a children’s area. “So we’re trying to add flexibility to the space and a certain amount of accommodation for all ages,” Runberg said. After the meeting, Barrientos said, while it took all summer to work out the details to make such changes possible, Safeway took little convincing. “Honestly, it wasn’t very hard,” she said, adding Safeway is rebuilding in other dense urban settings and hearing many of the same comments. “The feedback in dense neighborhoods is exactly the same.
“It’s kind of a national move,” she added. Another detail that will make the entire area feel special is the landscaping. “There’s a huge amount of landscaping,” Barrientos said. “It’s going to feel very lush and dense, more than any other building up there.” To ease neighbors’ concerns, Barrientos said the team deliberately also did not propose building to the density allowed and opted to not include 100 units that were permissible. As well, she said they worked very hard to take into consideration all the public comments made in earlier phases. “Just being a local developer here in the city, you sort of get a sense of what’s appropriate and what’s not appropriate,” Barrientos said. If all goes well, construction on the Safeway and the housing units could start in June, and, by July of 2023 apartments, will be ready to lease. Barrientos said a project that will bring up to 500 residents to upper Queen Anne will only benefit the neighborhood. “I do think that even before COVID, some of the commercial spaces — retail — struggled a little because Queen Anne is not that large of a community to support as much retail that is there. ... So adding 500 residents is going to be huge to all the existing commercial spaces,” she said.
have apple juice and apple cider is also because of the hardworking farmers and farmworkers who take care of our apple orchards— and we have a lot of orchards here in Washington State.
