Claims Management Program for Spectrum Specialty Builders Project: MSCM Parking Garage Injury and Claim Management Process Goal: Spectrum SB's goal is to create a standard streamlined procedure for timely, professional handling of all on the job injuries, accidents, and near misses. What is a Spectrum SB employee expected to do when an accident or near miss has occurred? An accident is defined as: any event resulting in personal injury or property damage. 1. Determine if there is an injury and immediately follow the Injury Procedures spelled out below. 2. Determine if there is any property damage and immediately follow Property Damage Procedures spelled out below.

Injury Procedures The Injury Procedure are intended for any on-the-job injury no matter how small or severe. All employees, regardless of position or stature, are expected to adhere to the following steps when an injury has occurred: 1. Immediately evaluate the severity of the injury and call 911 if the injury could be life threatening. 2. Once the injured employee is deemed stable, and the injury non-life threatening, the employee's Direct Supervisor must be notified immediately. If the injury occurred on a jobsite, all work will be stopped immediately. The Supervisor is responsible for notifying Spectrum SB's Safety Director or Site Safety Manager. If a Supervisor cannot be reached immediately, the Safety Director or Site Safety Manager must be notified directly. 3. Upon notification of an injury, the Supervisor, Site Safety Manager, or Safety Director will facilitate communication and Spectrum SB's procedure from that point forward. They will communicate with the employee to determine whether the injury needs First Aid or professional medical attention. 4. If the injury demands professional medical attention, the Supervisor, Site Safety Manager, or Safety Director will assign an employee to escort the injured employee to a medical facility. In most cases the Supervisor will escort the employee to the medical facility. The Supervisor or assigned employee will communicate and inform the injured employee of their treatment options and provide to them a Spectrum SB Doctor's Letter. 5. If the injury is determined by the employee and/or the medical provider to be an Industrial Insurance


Claim, the Supervisor or assigned escort will report the claim number and any appropriate information to the Safety Director immediately 6. If Spectrum SB has reason to believe that drugs or alcohol may have caused or contributed to the injury, the injured employee will be subject to drug and/or alcohol testing at a medical facility. A Supervisor or assigned escort will take the injured employee directly from the medical facility to a UA testing facility. In no case will the employee be left unsupervised until after a UA test has been submitted 7. If an injury requiring First Aid or professional medical attention happens on an active jobsite, no employees will be allowed to return to work until all of the following have occurred: An accident investigation has been conducted and cause has been determined a. All accident report forms have been completed and turned in b. Safety measures have been taken to prevent another similar accident c. A site safety meeting has been conducted to discuss the accident and new safety measures taken d. Both the Supervisor and Safety Director/Site Safety Manger have given permission to return to work. 8. In the case where an Industrial Insurance Claim is opened, the Safety Director will start an employee claim file. Upon receiving a claim number and conducting a full investigation, the Safety Director will complete the necessary forms to send to the appropriate Industrial Insurance carrier or entity. If there is any doubt that the claim is allowable, the following will be mailed to the appropriate Industrial Insurance carrier or entity within 48 hours of an accident: a. A cover letter with a general statement about the accident. b. Job description for the work being performed at the time of injury. c. Any employee records pertaining to previous injuries. d. Copy of Spectrum SB's internal accident report forms. e. Any additional findings, statements, or relevant information related to the accident. f. All pages of the documents submitted will have the claim # in the upper right hand corner and Spectrum SB will retain copies of all paperwork in our Spectrum SB Claim File. 9. In the case of an active Industrial Insurance Claim, a Spectrum SB Safety Director will be the assigned single point of contact for the injured employee, insurance carrier, and medical provider. Modified Return to Work Program Spectrum SB does have a Modified Return to Work Program available to employees. A Spectrum SB Safety Director will work directly with the injured employee to tailor a return to work program that mutually benefits Spectrum SB and the employee. We consider our employees our most valuable asset. We will do everything we can to assist


employees in returning to regular work activities. Returning to work following an industrial insurance injury is an important component of managing Spectrum SB's injury claims. The benefit of rapid return to work is reduced cost to the employee and Spectrum SB. If for any reason you are unable to do your regular job because you are injured on, or off the job, Spectrum SB will require a release from your doctor before you return to any type of work. Spectrum SB will provide a job description that your doctor can use as a tool to release you back to full or part time work. Employees are expected to participate in the process of identifying tasks that could be integrated into a temporary work assignment during the recovery process. A high degree of communication with a Spectrum SB Safety Director is expected during this process. Once a written release has been provided by a Doctor, you will be expected to have a meeting with a Spectrum SB Safety Director. The result of this meeting will be a mutually agreed Return to Work Agreement outlining your approved duties. If you are approved for modified work duties, you should expect to update your Return to Work Agreement at least once a month.
