why SpringHill? Our mission is to create life-impacting experiences that enable young people to know and grow in their relationship with Jesus Christ.

6窶ポ窶ピpringhillcamps.com

SPIRITUAL We are about Jesus Christ and His message of grace, hope, and love. The entire SpringHill experience focuses on these messages; in our relationships, in our worship, in our teaching – around the campfire and in every activity that we do!

STAFF SELECTION We have searched the country high and low for the most amazing and caring staff just for your child. This year we anticipate receiving 2,200 staff applicants, conducting over 1,500 interviews and hiring only 800 staff – each personally interviewed and background checked. We focus on adult staff who are spiritually, mentally, and emotionally mature to ensure that your child is not only in good hands, but also with good hearts. Once they arrive at camp, staff receives 40-80 hours of intensive job specific training.

SAFETY In every area, safety is our first concern for your child. We engage multiple licensing and accreditation agencies, consultants, partners, and vendors in areas ranging from medical and emergency procedures to food service and staff training. We are also accredited by ACA (American Camp Association) and licensed by the State of Michigan.

CAMPER: COUNSELOR RATIO We want your child to have the best experience during their week at camp, so we keep our in-cabin counselor-to-camper ratio at one counselor for every seven campers. For our younger campers, that ratio is even lower: one counselor to every five campers. We also have additional staff hired specifically to run activities so counselors can focus their energy on your child, making our total on-camp, staff-to-camper ratio one staff person for every three campers!

INCLUSION Inclusion is about participating and belonging, therefore we put forth every effort to make camp a place where every child is welcome and has the chance to experience all that we have to offer. Many of our campers with special needs feel SpringHill is the only place where they can truly “do everything.” We want your camper to have a fulfilling camp experience and we are fully prepared to provide specially trained staff, transportation, or specific housing.

AFFORDABLE We want every kid to experience SpringHill regardless of financial need. If you have financial need, we will work with you to get your kid to camp. If you have any questions call us at 231.734.2616 or please apply for a scholarship on springhillcamps.com. We are so excited for your child to grow, laugh, swim, play, explore, climb, probably eat too many s’mores around the campfire, and experience the love of God in their hearts. See you this summer! For the Kids,

Todd Leinberger, Vice President, Regional Director SpringHill Michigan

what’s new? We’re very excited to announce the relaunch of our Storybrook center. We have renovated cabins with fresh paint, landscaping, and playscapes to create an extraordinary environment for younger campers. We’re adding new activities and play areas. This year, we’re focused on dialing into the specific needs of each camper to challenge them at their own level of development. Combined with our 1:3 staff-to-camper ratio and our outstanding Bible-based curriculum – camp can be the beginning of a life-impacting experience for your child! Come and take a tour of all of our camp centers at our Evart, MI camp on one of these days: • April 21, 2012 • May 12, 2012 • June 2, 2012 • August 4, 2012 We’ve got tons of new stuff for our older campers too! We’re thrilled about reaching more kids this year with new housing for campers with special needs. We’ve added new sweet indoor waterslides, and our biggest addition this year is a 500-foot inflatable waterslide!

Camper Connect.

This year we’re offering a new program that allows parents to receive a daily email featuring a photo and quote from their child. This will allow parents to experience the fun and excitement of SpringHill Camps through their child’s eyes each day! Parents can choose from a variety of packages: + $20 for 3-4 days of camp + $40 for a week of camp + $80 for 2 weeks of camp To register, visit springhillcamps.com/registration or call 231.734.2616

Release of Liability, Waiver, Indemnication, and Consent to Medical Attention and Use of Images

I understand that all camping and recreational programs carry with them significant risks. Although SpringHill Camps (“SpringHill”) has taken reasonable and prudent steps to reduce foreseeable risks, they still exist. Accordingly, in exchange for my child or ward being allowed to participate in the SpringHill Camping Program (the “Program”), to be conducted at the location identified above, we, the parent(s) or legal guardian(s) (individually and collectively referred to below in the first person singular) of the child or ward identified on this Registration Form agree to be bound by each of the following: 1. Voluntary Participation. I understand and confirm that our child or ward’s participation in the Program is voluntary. 2. Identication of Risks. I understand that the Program takes place at various locations in Michigan and Indiana. I understand that there are certain dangers, hazards, and risks inherent in camping activities and in the activities included in the Program and Activity. I also understand that medical facilities or treatment may be inadequate or unavailable during portions of the Program. I understand that our child’s or ward’s participation in the Program may involve risk of injury and loss, both to person and to property. I also understand that the risk of injury may include the possibility of permanent disability and death. There may be other risks not known to SpringHill and not reasonably foreseeable at this time. I understand that this Release of Liability, Waiver, Indemnification, and Consent is intended to address all of the risks of any kind associated with my child’s or ward’s participation in any aspect of the Program, including, particularly, such risks created by actions, inactions, or negligence on the part of SpringHill or its directors, officers, employees, agents, volunteers, successors, or assigns. 3. Assumption of Risk. I assume all risks, known and unknown, foreseeable and unforeseeable, in any way connected with my child’s or ward’s participation in the Program. I accept personal responsibility for any liability, injury, loss, or damage in any way connected with my child’s or ward’s participation in the Program. 4. Release and Waiver. I release SpringHill and its directors, officers, employees, agents, volunteers, successors, and assigns from any and all liability for and waive any and all claims for injury, loss, or damage, including attorneys’ fees, in any way connected with my child’s or ward’s participation in the Program (a “Claim”), whether or not caused in whole or in part by the negligence or other misconduct of SpringHill or any of the individuals mentioned above. 5. Indemnication. I agree to indemnify and to hold harmless (in other words, to reimburse and to be responsible for) SpringHill and its directors, officers, employees, agents, volunteers, successors, and assigns, from all claims for any liability, injury, loss, damage, or expense, including attorneys’ fees (including the cost of defending any Claim I might make, or that might be made on my or my child’s or ward’s behalf, that is released or waived by this instrument), in any way connected with or arising out of my child’s or ward’s participation in the Program, whether or not caused in whole or in part by the negligence or other misconduct of SpringHill or any of the individuals mentioned above. 6. Binding Effect. This instrument shall be binding upon my child’s or ward’s relatives, personal representatives, heirs, beneficiaries, next of kin, or assigns and shall inure to the benefit of SpringHill, the Program, and their respective directors, officers, employees, agents, volunteers, successors, and assigns. 7. Consent to Medical Treatment. I authorize and appoint SpringHill and its representatives, each to act alone as personal representatives, to consent on our behalf to all emergency treatment, transportation and/or medical care (except elective surgery) of my child or ward should my child require such assistance. This consent does not impose a duty upon SpringHill or its representatives, to provide such assistance, transportation, or services. This Limited Purpose Medical Power of Attorney shall continue until revoked by the undersigned or for (thirteen) 13 months after its date, whichever is earlier. Physicians or the hospital’s medical staff may assume and rely that this authorization is currently in effect during such thirteen month period unless notified. I also authorize SpringHill, or representative, as Medical Power of Attorney to have medical information disclosed to the representative of their choice. This is in compliance with HIPPA Privacy Rules which defines a Required Disclosure as “A covered entity must disclose protected health information to individuals (or their personal representatives) specifically when they request access to, or an accounting of disclosures of, their protected health information”. 8. Use of Images. By signature below, I grant to SpringHill, its successors and assigns, the right to use, publish (in any medium, but not limited to, print, digital, or display and transmission on the Internet), publicly display and perform (in any format, but not limited to, film, slide, television, digital display and/or transmission on the Internet), and copyright for its fundraising, marketing, educational media project, and other commercial purposes (the “Project”), photographs and/or video-taped interviews of my child or ward (collectively, the “Multi-Media Works”). By signature below and in consideration for SpringHill’s agreement to have the Multi-Media Works taken for the possibility of being featured in the SpringHill Project, and the expense that it may incur in having the Multi-Media Works taken, processed, and produced, I release, on behalf of myself and my child or ward, SpringHill, its directors, officers, employees, agents, successors, assigns, and all persons acting under SpringHill’s permission or authority, from any liability for, and waive any and all claims for injury, loss, damage, or compensation, or any other claims (including libel and slander, invasion of privacy, and violation of publicity rights) in any way connected with me and my child or ward, and arising out of SpringHill’s use of the Multi-Media Works in its Project. Finally, by signature below, I waive any right that I, or my child or ward, may have to inspect or to approve the Multi-Media Works’ finished product or other copy that SpringHill may choose to use for the Project or in connection with other materials related to SpringHill, its business, and products. By signature below, I understand and agree to the terms above. I understand that I am giving up rights by signing it. I am signing it voluntarily. 9. Severability. If any provision (or portion of any provision) of this instrument is held to be invalid or unenforceable, that provision shall be enforceable in part to the fullest extent permitted by law, and such invalidity or unenforceability shall not otherwise affect any other provision of this instrument. 10. Applicable Law. Because the SpringHill Program is conducted in the States of Michigan and Indiana respectively and in order to provide certainty in the law to be applied to the construction of this instrument, this instrument shall be governed, construed, and enforced in accordance with the laws of the States of Michigan and Indiana respectively. THIS IS A RELEASE OF LIABILITY, WAIVER, INDEMNIFICATION, AND CONSENT. I HAVE READ THIS RELEASE OF LIABILITY, WAIVER, INDEMNIFICATION, AND CONSENT. I UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS AND MY CHILD’S OR WARD’S RIGHTS BY SIGNING IT. I AM SIGNING THIS RELEASE OF LIABILITY, WAIVER, INDEMNIFICATION, AND CONSENT VOLUNTARILY. If the person participating in the Program is not yet 18 years old, a parent or the legal guardian must sign: In exchange for my/our child or ward being allowed to participate in the SpringHill Program and as the parent(s) or legal guardian(s) of the above named individual, I/we verify that I/we fully understand, agree to, and accept all provisions of this Release of Liability, Waiver, Indemnification, and Consent.
