Combating depression in older adults BY AMY POLLICK WEALTH OF GEEKS
While depression is not a natural part of aging, about 7% of the general population of elderly adults has been diagnosed with major depressive disorder, according to psychologist Dr. R.Y. Langham. The Centers for Disease Control estimates up to 13.5% of elderly patients receiving home health care or who are hospitalized may struggle with this disorder. Dr. Langham notes older adults have several risk factors that contribute to depression. “Chronic medical illnesses, cognitive decline and the side effects of medications can play a significant role. Additionally, life events like decreased mobility, social isolation, bereavement or even a history of depression can make older adults more vulnerable.”
Recognizing the symptoms
If someone notices an older friend or loved one is feeling sad or listless, is that person depressed? Not necessarily. According to the CDC, it is appropriate to be concerned if these feelings last for weeks at a time. Ryan Hetrick, a therapist and psychologist, says depression symptoms include “feeling hopeless and/or helpless, difficulty concentrating, withdrawal from family and friends, changes in appetite, difficulty sleeping and lack of interest in activities that were once enjoyed.” For those who may notice these symptoms in themselves, the key to dealing with them is getting help. You can contact your primary care physician to get connected to a
therapist or psychiatrist. A referral may be required for some insurance plans, so it is always a good idea to see the primary care doctor first. As most adult children know, however, getting an elderly parent to the doctor can be a fight, and that struggle may be more difficult if the conversation is about seeing a therapist. According to Licensed Clinical Social Worker Mindy Crouch from Care, a website helping provide care options, “Many of our older relatives grew up in a different time with institutionalization or very negative societal attitudes towards mental health care.” Crouch advises having a conversation with loved ones to help them understand they deserve to feel better, and therapy may be a helpful part of that self-care. Loved
ones should work to reassure these older adults that seeking therapy is not shameful or a sign of weakness. This is where the primary care physician can assist. The doctor can urge the person to seek help. Most older people will do as their physicians ask, even if reluctantly. Once they start therapy, they may be more inclined to continue if they see a positive change in how they are feeling.

SMART CHOICES encourages older adults to help themselves by “… engaging in social activities, seeking purpose (be it through hobbies, volunteering, or even a part-time job), and maintaining physical health …” According to Licensed Professional Counselor Marissa Moore, keeping up with doctors’ appointments is crucial to avoiding depression or spotting it early. “Regular medical check-ups can help identify and address any underlying health issues or medication-related factors that may contribute to depression,” she said. Like Landeros, Moore also recommends older adults stay as active as possible. Even if they are engaging in light physical activity, “… exercise has been shown to improve mood and reduce the risk of depression.” If an older adult has been diagnosed with depression, what are the treatments? Modern mental health has come a long way, and there are many solutions that can help alleviate symptoms of depression.
Psychiatrist Ketan Parmar, M.D., offers a list of treatments that includes medication, therapy, lifestyle changes, and even neuromodulation. This method involves using electrical or magnetic stimulation on parts of the brain that control mood. Although electroconvulsive therapy has a checkered past, times have changed, and it can be
used effectively and safely. Other neuromodulation methods include transcranial magnetic stimulation, vagus nerve stimulation and deep brain stimulation. “Neuromodulation can help older adults with depression who do not respond well to psychotherapy or medication by improving their mood and cognitive functioning,” Dr. Parmar explained.
He says medication can be an effective method of treating depression. Because of potential drug interactions and side effects, he advises it should always be under close supervision from a doctor. Physicians should follow up frequently to adjust dosages as necessary. According to Dr. Parmar, depression in older adults may not have exactly the same symptoms as it does in younger people. He said this population may not cry or seem sad but may instead be more irritable and apathetic. He also said depression can increase cognitive impairment, such as memory loss or confusion. This underlines the importance of seeing a physician for these symptoms to distinguish between depression and early signs of dementia. Early diagnosis and treatment help preserve function for a longer time. Although depression may crop up, it does not have to be a normal part of aging. Early detection, treatment and frequent follow-ups can help keep this disorder from taking over someone’s life.

People are living longer, a reality that can be traced to a number of factors, including advancements in medicine and greater dissemination of information regarding preventive health care. According to data from the United Nations Population Division, the average life expectancy in the United States is 81.65 for women and 76.61 for men. Canada has even higher life expectancies, at 84.74 for women and 81.15 for men. As people live longer, some may outlive their significant others and, ultimately, find themselves once again interested in sharing experiences with a special someone. Seniors ready to re-enter the dating pool may find that things are quite different from what they experienced as naive teenagers or young adults. Dating used to be about hanging out with friends and meeting people at shared events,
such as school dances or work parties, or even while enjoying a night out with friends. Nowadays, dating often begins in cyberspace. This can be confusing and anxiety-inducing for adults who did not grow up with technology guiding their every move. According to a report in The Atlantic, more than one-third of baby boomers are not married, and this generation has had higher rates of separation and divorce, and lower rates of marriage than the generations that preceded them. Many have years ahead of them to devote to new relationships. Here is what they may want to know before navigating 21st century dating waters: • You are not in this alone. While online dating may be portrayed as a young person’s game, plenty of older adults are now finding connections online. In fact,

As a senior, you may have more time to devote to recreation and leisure. But experts say that common misconceptions lead many to assume this aspect of financial planning is not right for them. “If you think you can’t afford life insurance or are not eligible, keep in mind that there are many different types of life insurance. To help you navigate the complexities of finding and choosing life insurance, the team at Assurance IQ is debunking some of the most common misconceptions: MYTH: It is impossible to secure life insurance with pre-existing conditions. REALITY: While it is more difficult to acquire certain kinds of life insurance with pre-existing health
conditions, you have options. If you are denied a policy, consider enrolling in group life insurance through your employer, as preexisting conditions may not be considered when signing up. Another option could be guaranteed issue life insurance, which can offer lifetime coverage without a medical exam or any health questions. MYTH: Retired people do not need life insurance. REALITY: Life insurance is often viewed as a source of income replacement, but depending on your situation, you may still benefit from coverage even after you retire. For example, if you have debt, like a mortgage or medical bills, life insurance could help your family settle it in the event of your passing. It can also help fund final arrangements, relieving financial stress during a difficult time.
MYTH: Buying life insurance is too complicated. REALITY: Approximately 106 million adults in the United States are underinsured or have no life insurance coverage. SMART CHOICES Too many Americans are missing crucial planning steps to make sure they are set up for success as they age.
Many Americans believe AI will provide their in-home care as they age BUSINESS WIRE
Less than a year since the launch of the popular Artificial Intelligence chatbot ChatGPT, Americans are embracing AI-based care as they age. One-third of Americans and over half (58%) of millennials believe AI and robotics will provide their future in-home long-term care. According to the annual Nationwide Retirement Institute Long-Term Care survey, conducted in partnership with LIMRA in May, this survey of 1,439 U.S. adults aged 24 and over found that younger generations were particularly apt to embrace the technology. Among the key findings: • Daily functions: More than one in three (35%) Americans would accept help from a robot for activities such as toileting, dressing and transferring. This rises to 52% among millennials and declines with older generations (23% for boomers and 36% for Gen Xers). • Companionship: Nearly the same amount (32%) say they would talk to robots/AI if they are feeling lonely. This increases to about half (52%) of millennials. • Physical safety: Almost seven out of 10 (68%) Americans would
use AI to alert family/friends if they were to experience a fall or physical danger (63% boomers, 70% Gen X, 72% millennials). • Medical history: Nearly half (48%) of Americans say they would share their medical history with AI to help support their care needs. This figure increases to 65% for millennials. In response to the increasing acceptance and adoption of AIbased care, Nationwide is testing eldercare robots in homes of select policyholders with mobility issues. The goal of this trial is to assess if the robots increase the potential for policyholders to age in their home and remain independent. “It is difficult for many families to find quality care for their loved ones. We are considering AI and robotics as potential solutions for this and are identifying if eldercare robots could become credible, compelling examples of extraordinary care for our members,” said Holly Snyder, president of Nationwide’s Life Insurance business. “As we continue to see advancements in AI and an uptick in consumer adoption, AI and

robotics could permanently change how people receive their longterm care and provide them with more opportunity to safely remain independent for longer.”
Many mistakenly believe they have long-term care coverage
The survey reveals that 18% of adults report that they currently own long-term care insurance, including 27% of millennials. However, industry data shows only 3.1% of Americans have purchased longterm care insurance and most of those are older consumers. More than half (51%) who mistakenly thought they owned long-term care insurance confused it with long-term disability insurance. Almost a third (30%) confused it with health insurance. In conjunction with the survey, Nationwide conducted focus groups made up of four different demographics: baby boomers, Gen Xers, millennials and those who believe they had long-term care coverage. Unsurprisingly, these sessions revealed that, like survey respondents, focus group participants also often mistakenly confused long-
term care insurance with their longterm disability insurance or health insurance. Neither of these cover long-term care expenses. “Many Americans — mostly millennials — mistakenly believe they have long-term care coverage, usually in their company’s benefit package, when in fact they do not,” said Snyder. “Though this misconception is understandable, it puts them at danger of discovering that they don’t have coverage much later on when they really need it.”
Americans have concerns about how to manage aging
Though many Americans do not own long-term care policies, they do have very real concerns about how to manage the aging process and its associated costs. For example: • Nearly half of respondents (49%) are worried they will become a burden to their family as they age. • Almost one-third (28%) say they would rather die than live in a nursing home. • More than one-quarter (26%) believe paying for long-term care will diminish their children’s inheritance.

“Too many Americans are missing crucial planning steps to make sure they’re set up for success as they age,” said Snyder. “The first step is to understand what’s important to you and your family. From there, we recommend talking to a financial professional to help build a plan.” Financial professionals need to bring up long-term care planning While more than half (51%) say it is important that a financial professional discuss long-term care costs with them, fewer than one in five adults (18%) say they have discussed long-term planning with their financial professional. In fact, more than one in four adults (27%) across all age groups have not discussed long-term care costs with anyone. The good news is 30% say they would discuss long-term care costs with a financial professional in the future. “Financial professionals need to be proactive in helping families plan for important long-term care issues, including how to pay for it,” Snyder added. “Even if a client believes they are covered, we suggest probing them with some followup questions to be sure they are, in fact, protected. Survey participants were drawn from a nationwide consumer panel and data collection occurred between April 25 and May 12. SMART CHOICES NUTRITIONAL NEEDS FOR AGING SENIORS:
It is never ‘one size fits all’

Having a poor diet can damage a person both mentally and physically, so good nutrition is essential at any age to stay strong and prevent dietrelated illness. It is especially important for the elderly, whose bodies do not absorb the nutrients they need for proper function as efficiently as a younger person, and who are at a higher risk for chronic diseases such as high blood pressure, diabetes, osteoporosis and heart disease. “As we age, there are nutritional needs that shift,” said Registered Dietitian Andrew Wade, co-owner of Case Specific Nutrition, a counseling service that also offers
a selection of dietitian-approved prepared meals. As a result, certain foods and proper hydration become even more important for well-being. Among the trickiest changes to navigate is the increased need for protein. To prevent the loss of muscle mass and strength that occurs naturally with age, an older person requires roughly 50% more protein than a young adult. The amino acids in protein also power up the body’s immune functions, decreasing one’s chance of getting sick. But because people in their 70s or 80s typically move less throughout the day, and their metabolism slows
down, they might not feel hungry — and no matter how hard you nag or tempt your loved ones with favorite foods, it can be difficult to convince them otherwise. Simply being unhappy — a common emotion following life changes like the death of loved ones or declining health — can also cause a loss of appetite. Also a concern: The body does not absorb vitamin B12 as well in old age, leading to a reduction of red blood cells. Besides anemia, that can cause neurological problems such as numbness or tingling in the hands and feet, vision problems and confusion.
Certain medications can also decrease absorption of vitamin B12. In addition, our digestive system slows as we age. So, it is essential for seniors to get more fiber in their diets to improve digestion and prevent constipation, said Wade. “It feeds gut bacteria and keeps things moving.” Complicating matters is that older adults often have more trouble chewing or swallowing food because of dental issues or the loss of control of mouth and throat muscles following a stroke or conditions such as Parkinson’s disease. Seniors also are more apt to become dehydrated because they CONTINUED ON PAGE 16

often just do not feel thirsty and forget to drink or fear incontinence; a UCLA School of Nursing study published in 2019 found that up to 40% of elderly people may be chronically underhydrated. Also, our taste receptors start to break down as we age, resulting in diminished taste for most seniors. That can make food bland and unappetizing. “There’s a lot going on,” said Laura Ali, a registered dietitian, culinary nutritionist and author of “Mind Diet for Two,” a cookbook focused on “brain healthy” foods. “And that can be a challenge.”
Clues and conversations
So how do you know if your senior is eating and drinking properly, especially if you do not live nearby and visit often? And just as important, what can you do to help them get the healthful calories and fluids needed to nourish an aging body? For starters, it is important to have frequent and respectful conversations. Simple questions like “how is your appetite,” “what’s in the fridge” and “what did you have for dinner” can give important clues, so long as you come from a place of curiosity and don’t pander, says Wade. “You don’t want to sound patronizing.” Keeping in touch with your loved one’s physician — by email or virtually, if you are not local or they do not want you to tag along on appointments — can also help provide data points, since they are probably getting weighed during visits; trending down on the scale can be a sign of undereating. Doctors and pharmacists also can provide information about which medications might affect appetite or cause issues with chewing or swallowing.
It is never ‘one size fits all’
When Pittsburgh private chef Brandon Blumfield prepares meals for elderly clients, “I always get to know them before cooking,” he said. “That includes not learning just what they want but what they need.” That often means modifying a recipe to reduce fat and sodium, and portioning the food to fit each eater’s appetite. For those who have issues with gripping utensils, he also cuts the food to smaller sizes that are more manageable. To help with hydration, Ali recommends a refillable water bottle
with time markers placed next to a chair or bed for easy access if the senior has mobility issues or keeping a lightweight pitcher and cup handy. If they do not like plain water, add some fruit or berries for flavor, or a squeeze of lime or lemon. Providing ready access to fruits and vegetables with high water content, such as watermelon, cucumbers, lettuce and tomatoes, also can prevent dehydration, along with liquid foods such as soups and broths. Encouraging them to take small, consistent sips throughout the day instead of trying to chug several ounces in one (uncomfortable) fell swoop is another smart move. Wade suggests giving them a shot glass necklace they can wear around their necks and refill every 15 or 20 minutes.
Counting calories
Calorie-wise, it all depends on the individual, as some seniors need more and some need less. One useful tool in figuring out which camp your loved one falls into is the USDA’s “MyPlate” guide, available both on the web and as an app. It helps calculate what and how much to eat based on age, sex, height, weight and physical activity level. For example, an 80-yearold woman who stands 5-foot-4 and has less than 30 minutes of moderate activity a day requires 1,600 calories, including 5 ounces of protein, 5 ounces of grains, 3 cups of dairy, 2 cups of veggies and 1 1/2 cups of fruit. If your senior is undereating, Ali says one solution is to figure out ways to concentrate calories and protein into smaller amounts of food. Adding a bit of powdered milk to a glass of milk or plate of scrambled eggs is a great way to sneak protein into a meal without changing taste. Several small meals throughout the course of a day instead of three squares is another. Regular exercise can also boost appetites. Another strategy would be to start the day with one of the body’s main sources of energy: carbohydrates. A simple breakfast of toast or a breakfast biscuit is easy to digest and will get the appetite going, says Wade. If their taste runs salty, add a little peanut butter; if it is sweet, pair the toast with some jam. For those with chewing or swallowing issues, texture is key. Softer foods such as cottage cheese, milkshakes, smoothies, cereal and milk, oatmeal or mashed

High-calorie egg muffins These are like mini crustless quiches. You can swap the cheese with other varieties, use different vegetables, and add in canned salmon or other meats to boost the protein. These freeze well and can be reheated in the microwave. They are an easy breakfast or midmorning or afternoon snack. Makes 12 muffins. 6 large eggs 1/2 cup nonfat powdered milk 1/2 cup half and half 1 cup shredded cheddar cheese 2 cups thinly sliced spinach 1/4 teaspoon each black pepper and nutmeg 1. Preheat oven to 350 degrees Fahrenheit. Spray the wells of a 12-cup muffin tin with nonstick cooking spray. 2. In a medium bowl, whisk the eggs until fluffy. In a small measuring cup, mix the powdered milk with the half and half until the powder has dissolved. Add the milk mixture to the eggs and whisk to combine. 3. Add the cheese and the spinach to the egg and milk mixture and mix well. Evenly distribute the egg mixture between the 12 muffin tin cups. 4. Place in the middle of the preheated oven and bake for about 20 minutes or until the muffins puff up and are firm to touch. 5. Cool on a wire rack. The muffins will deflate as they cool.
potatoes are good options, along with berries and fruit. So are eggs, which have the added benefit of providing protein. Nutritional shakes like Ensure are easy to criticize because they have very little protein and a lot of sugar, notes Wade, but they are a source of calories that are intentionally filling for older adults. “It’s not right for everybody, but if they have no appetite and are withering away and losing tissue, having them sip on something that tastes like a milkshake and building on that is a tool in the toolbox,” he said. When it comes to adding protein, both animal sources (lean meats, fish, poultry, eggs, dairy) and plant sources (tofu, lentils, chickpeas, beans, quinoa) will work their magic. Vitamin B12 is also found naturally in foods like meat, eggs and dairy products, and seafood; good sources of fiber include beans, whole grains, brown rice and my elderly mother’s favorite nosh: popcorn.
Also, do not be afraid to salt to taste when cooking from scratch — it can make food taste better — but keep a watchful eye if your senior suffers from high blood pressure. Current Dietary Guidelines for Americans recommends adults limit sodium intake to less than 2,300 mg per day, or about a teaspoon of table salt. Cooking with ingredients that naturally contain high-enough levels of sodium can keep your senior from reaching for the salt shaker. Chef Blumenthal, for instance, seasons roasted vegetables with low-sodium tamari he waters down with apple cider vinegar and spices. He also adds flavor with a lot of mustard, capers and olives. Finally, keep in mind that older adults are at a higher risk for foodborne illness because their body’s immune response to disease is weaker. So, if you or they are cooking meals at home, make sure you always wash your hands, are careful not to cross contaminate and always cook (and store) food at the right temperature.

Methodology
An online poll about Social Security commissioned by MassMutual was conducted by PSB Insights from March 31 to April 5, among 1,500 Americans age 55-65 who have not filed for Social Security retirement benefits. The results indicate that near retirees’ knowledge about Social Security is lower than in 2022.

Signs it could be time to move to an assisted living facility METRO SERVICES
Assisted living facilities provide a vital service to families across the globe. In fact, more than 800,000 people in the United States are currently living in assisted living facilities, according to a 2023 report from Zippia, an online recruitment service that gathers information from the Bureau of Labor Statistics and its own internal data. The decision to move oneself or an aging relative into an assisted living facility is complicated. Though such facilities typically offer an excellent standard of care, it can still be bittersweet for an individual to leave his or her home. It also can be difficult to identify when the best time is to make such a transition. Though each individual case is unique, the following are some indicators that the time might be right to move oneself or an aging loved one into an assisted living facility.
Emotional challenges
The National Council on Aging notes that certain emotional challenges may indicate the time is right for an individual to move into an assisted living facility. The NCOA encourages individuals making such a decision independent of others to ask themselves if they are experiencing feelings of loneliness or depression. Assisted living facilities can provide much-needed social interaction that seniors may not be getting at home. If feelings of loneliness or depression are present, then the time might be right to make the move to an assisted living facility.
Physical challenges
Many individuals move into assisted living facilities because the physical challenges of daily life have become too significant to handle alone. The NCOA notes that individuals who are feeling overwhelmed by tasks such as buying groceries, cooking and cleaning may benefit from moving into assisted living facilities that handle such tasks for them.

In addition, assisted living facilities may be housed in single-story buildings
or equipped with elevators, which means residents will not be forced to navigate potentially dangerous staircases at any point during the day. Staff also can respond quickly if residents experience a fall, providing much-needed assistance that seniors living alone or with an aging spouse may not get at home. Various signs may indicate that the time is right for individuals to move into an assisted living facility so they can more safely navigate daily life.

Volunteering in late life may protect the brain PR NEWSWIRE
Volunteering in late life is associated with better cognitive function — specifically, better executive function and episodic memory — according to research presented in July at the Alzheimer’s Association International Conference in Amsterdam, the Netherlands and online. “Volunteers are cornerstones of all communities and imperative to the success and impact of many organizations, including the Alzheimer’s Association,” said Donna McCullough, Alzheimer’s Association chief mission and field operations officer. “We hope these new data encourage individuals of all ages and backgrounds to engage in local volunteering — not only to benefit their communities, but potentially their own cognitive and brain health.”
Volunteer activities, such as supporting educational, religious, health-related or other charitable organizations, provide a means for older adults to be more physically active, increase social interaction and provide cognitive stimulation that may protect the brain. However, there is a lack of information on the relationship between volunteering and cognitive function, especially in large, diverse populations. As first reported at AAIC 2023, Yi Lor, MPH, an epidemiology doctoral student at the University of California Davis, alongside principal investigator of the studies, Rachel Whitmer, Ph.D., examined volunteering habits among an ethnic and racially diverse population of older adults in the Kaiser Healthy Aging and Diverse Life Experiences Study and Study of Healthy Aging in

African Americans. In the combined group, 1,167 (43%) participants reported they had volunteered in the past year. The researchers found that volunteering was associated with better baseline scores on tests of executive function and verbal episodic memory in this study group after adjusting for age, sex, education, income, practice effects and interview mode (phone vs. in-person). Volunteering was also associated with a trend toward less cognitive decline over the follow-up time of 1.2 years, but this association did not reach statistical significance. Additionally, those who volunteered several times per week had the highest levels of executive function. “Volunteering may be important for better cognition in late life and
could serve as a simple intervention in all older adults to protect against risk for Alzheimer’s disease and associated dementias,” said Lor. “Our next steps are to examine whether volunteering is protective against cognitive impairment, and how physical and mental health may impact this relationship.”
