Pre-Workshop Survey Before the Workshop, all members were encouraged to complete an online survey through a link which was provided (www.surveymonkey.com/s/SQHN). The survey an indication of what the workshop will present as well as a tool to help the participant understand a bit of what the workshop would have contained. Over 50 people responded by filling the questionnaire online before the link was shut down to analyze the data that had been inputed via the questionnaire. The link was closed down after 2 weeks for proper analysis. Interactive session It was an interactive session for all the participants who broke into groups frequently to discuss issues arising and identify challenges and lapses as well as proffer solutions as they understood it. Videos of practical and real life experiences of people who had suffered losses as a result of oversight on the part of the medical personnel, as well as cases of mistakes that could easily have been avoided were also played for the participants to help them understand the Patient Safety issues better.

After watching such videos, all participants were encouraged to break into discussion groups to discuss their personal observation, the lapses identified, how some mistakes could be avoided and proffer solutions to the challenges they observed which they could make it practical in their various hospitals and organizations. Workshop Materials Upon registration, every participant was given an interactive CD and a mini-hand flyer that had the 2011 National Patient Safety Goals for Hospitals with a checklist for safe practices. The aim of the materials is to ensure that the participants have something to refer to even after the workshop which will serve as a guide against avoidable mistakes and reminder for what was taught at the workshop. It is also to serve as a refresher course and reminder for the need for constant improvement in patient safety at all times.
