Here are a few plants that are sure to create a colorful, nectarrendering, butterflyfriendly habitat right in your own backyard. Blue sage is a native Texas perennial, drought-resistant and blooms in shades of purple and blue nearly year-round. Texas lantana thrives in hot Texas temperatures. It produces bright colorful blossoms and attracts swallowtails, sulphurs and brush-foot butterflies. Asclepias is also known as milkweed and comes in several varieties. The most common variety in Texas is also known as Antelope Horn. Tropical Milkweed is not native to Texas but is widely available in garden centers and blooms in bright bold colors. Zinnias grow well in the Texas heat and are easy to grow from a seed. They are annuals, so be prepared to plant them each spring. Buddleia (butterfly bush) usually bloom mid-July through frost, in vibrant shades of purple, red and white. A butterfly garden will not only attract colorful creatures to your space, but will also create a colorful atmosphere in which you can relax and enjoy. Don’t forget to feed the next generation of butterflies— the caterpillars! hether you’re a caregiver or interested in getting your own business in order, we’ve identified five recommended legal documents that every adult should have. There are many online resources that explain in further detail about each document. If you have an attorney, be sure to ask about these documents or there are online resources that provide a free template for you to create your own. Don’t wait until it’s too late—take a moment to get organized.

Advance Directive Also known as a living will or health care declaration/directive, this document outlines your wishes for medical treatments and life-sustaining measures, in the case that you are not able to communicate them verbally. Details might include a “do not resuscitate” order (DNR), the use of breathing machines, tube feeding or organ donation. Making these decisions in advance lets medical professionals know what measures to take and grants your loved ones peace of mind, should they have to communicate these decisions for you. 42

Durable Power of Attorney for Health Care While an advance directive is limited to end-of-life concerns, a durable power of attorney for health care accounts for all health care decisions in the case that you are incapable of making decisions for yourself. In this document, you are able to outline how your designated power of attorney should act. Their authority to make these decisions ends when you are once again capable of making your own decisions. Last Will and Testament Many people know this document simply as a “will.� It outlines how your property (including digital property such as email accounts and blogs) should be distributed and to whom. Different states have different requirements for one’s will. In Texas your will must be signed by two witnesses. Living Trust A living trust designates someone (trustee) to help manage and distribute your assets rather than going through the court system, as in a last will. This often prevents your last wishes from becoming public record. This document also designates terms in which the trustee would set into action, should you be unable to make decisions regarding your assets during and after your life. Financial Power of Attorney This document designates someone (principal) to do business on your behalf, should you be unable to take care of necessary matters yourself. Medicaid Myths ..... Debunked 1) You have to spend down all your money to apply for Medicaid. I recently assisted an individual with total assets of $309,896 that met Medicaid eligibility. The amount paid to the Nursing facility for her care is her Social Security check of $950 and $2300 in the form of a one year annuity. Her long term insurance pays the additional $3,000 of her monthly $6,000 bill for her care. The excess assets were placed in an irrevocable trust for the benefit of her family members. 2) If there is a couple, and one enters the Nursing Facility, you have to spend down 1/2 of your assets. In the situation of a couple where one member goes into the facility and the other remains in the community, there is no limit on the equity value of the home (typically $552,000). With 215,000 in total assets, they were able to conserve the total amount of the assets by purchasing a 4 month annuity and getting all the funds back to the community spouse in 4 equal installments of $25,000 each. Most all cases of one spouse in the NF, the entire amount of the assets can be saved for the spouse remaining in the community. 3) If you give away assets, you have to wait 3 years to apply for Medicaid. Actually, there is a look back period of 5 years for all transfers. The penalties begin when the nursing facility resident meets all eligibility requirements, not at the time of the transfer. For every $156.34 transferred or given away, there is a one day penalty for the resident. Once the Medicaid requirements are met, the resident can receive Medicaid but due to the penalty must pay their daily rate. There are also many programs for those low income individuals in the community to obtain many types of services, an attendant to assist them with personal care, meal preparation or meals on wheels, housekeeping, respite, emergency response buttons to enable them to stay in the community for a longer period of time. There are other programs to assist with payment of the Medicare premiums each month and some low income individuals qualify for a program that in addition to the Medicare premium will also pay for co-payments and deductibles for Medicare. VeeCee Spear is a former Medicaid Eligibility Worker, Mentor, Supervisor and also worked in the Policy Section for Long Term Care Medicaid for 20 years. She has been actively consulting/assisting Texans through the very complex Medicaid system for the past 5 years.

Photography | Brenlee McKnight Ingredients: 4 asparagus stalks, cooked and cooled 2 cups all-purpose flour 2 eggs 3 cups Parmesan cheese, grated 1 oz. clarified butter 1/2 teaspoon garlic, chopped 1/2 teaspoon shallot, chopped pinch lemon zest

1/2 oz. chicken or veal stock 1/2 oz. white wine 1/2 oz. lemon juice 3/4 oz. butter, cold 3 oz. Roma tomato, diced pinch basil, chopped 4 oz. jumbo lump crabmeat salt and pepper, to taste

Directions: Heat oil in skillet to 325 degrees. Take asparagus, coat in flour, then egg, then flour again, and then egg again, then Parmesan cheese. Fry until golden brown. While the asparagus is frying, start the sauce. Heat clarified butter in a skillet over medium heat. Add garlic, shallot and lemon zest. Cook for 2 to 3 minutes, and then add stock, wine and lemon juice. When liquids have reduced, add crabmeat, diced tomato, then cold butter, basil and salt, and pepper to taste. When the sauce has a creamy consistency, remove from heat and top the asparagus. Find 46

Home Care vs Home Health Care

What is home health care? When the need is for more than home and personal care, a home health care agency may be required. Home health care provides skilled medical services (such as administration and adjustment of medication levels, monitoring and dressing of wounds, etc.) in the home by trained medical personnel. Home health care is always ordered and supervised by a physician. What is home care? Simply put, home care is non-medical, personal assistance provided in the home. Home care may include help with bathing, dressing, and personal hygiene. It may include a wide range of services: taking and recording vital signs, assistance in the bathroom, light housekeeping, meal preparation, companionship, grocery shopping and pharmacy errands, home safety supervision, medication reminders, bill-paying, laundry, and transportation. Will my insurance cover the costs? Medicare, Medicaid, and private insurances are sources of payment for receiving home and home health care. Home Healthcare services are typically medically necessary. They are paid for by Medicare or other health insurance, and have been ordered by a physician. Private duty home care services are typically paid for privately, or using long-term care insurance, reverse mortgage proceeds, or Veterans aid and attendance pension benefits, or some combination of any of these programs.

for Effective Communication

A

lzheimer’s disease presents many challenges, and communication is a big one. Because Alzheimer’s disease slowly erodes verbal communication skills, your loved one’s words and expressions may make little or no sense to you. In turn, your loved one may have trouble deciphering your words. The resulting misunderstandings can fray tempers all around, making communication even more difficult. Here’s help easing the frustration.

What to expect Alzheimer’s damages pathways in the brain, which makes it difficult to find the right words and to understand what others are saying. Your loved one may incorrectly substitute one word for another or invent an entirely new word to describe a familiar object. He or she may get stuck in a groove—like a skipping record—and repeat the same word or question over and over. 92

A person living with Alzheimer’s may also: • Lose his or her train of thought • Struggle to organize words logically • Need more time to understand what you’re saying • Curse or use offensive language Despite the challenges, you can communicate effectively with a loved one who has Alzheimer’s. Consider these tips: • Speak clearly. Introduce yourself. Speak in a clear, straightforward manner. • Show respect. Avoid secondary baby talk and diminutive phrases, such as “good girl.” Don’t assume that your loved one can’t understand you, and don’t talk about your loved one as if he or she weren’t there. • Stay present. Maintain eye contact, and stay near your loved one so that he or she will know that you’re listening and trying to understand. • Avoid distractions. Communication may be difficult—if not impossible—against a background of competing sights and sounds. • Keep it simple. Use short sentences and plain words. As the disease progresses, yes-no questions may work best, and only one question at a time is best. Break down requests into single steps. • Don’t interrupt. It may take longer than you expect for your loved one to process and respond. Avoid criticizing, hurrying and correcting. • Use visual cues. Sometimes gestures or other visual cues promote better understanding than words alone. Rather than simply asking if your loved one needs to use the toilet, for example, take him or her to the toilet and point to it. • Don’t argue. Your loved one’s reasoning and judgment will decline over time. To spare anger and agitation, don’t argue with your loved one. • Stay calm. Even when you’re frustrated, keep your voice gentle. Your nonverbal cues, including the tone of your voice, can send a clearer message than what you actually say. • Communicating with your loved one may be challenging, especially as the disease progresses. Remember, however, your loved one isn’t acting this way on purpose. Don’t take it personally. Use patience and understanding to help your loved one feel safe and secure. Photography | Brenlee McKnight Ingredients: 4 cups chicken stock, light 3 tablespoons olive oil 1½ cups white wine pinch of saffron 1 tablespoon butter 1 small onion, finely chopped

1 cup Arborio rice 1 clove garlic, chopped 1 cup fresh English peas, shelled 20 to 25 fresh mussels, scrubbed and beard removed

Directions: Heat oil in skillet to 325 degrees. Take asparagus, coat in flour, then egg, then flour again, and then egg again, then Parmesan cheese. Fry until golden brown. While the asparagus is frying, start the sauce. Heat clarified butter in a skillet over medium heat. Add garlic, shallot and lemon zest. Cook for 2 to 3 minutes, and then add stock, wine and lemon juice. When liquids have reduced, add crabmeat, diced tomato, then cold butter, basil and salt, and pepper to taste. When the sauce has a creamy consistency, remove from heat and top the asparagus.

hen everyday tasks such as cooking or maintaining a house appear to be burdensome for a loved one, or signs of loneliness become prevalent, it may be time explore a new living situation. So, where do you go from here?

Eldercare professionals such as a geriatric care manager (GCM) might be a good place to start. They can be instrumental in helping determine the specific services your loved one may need and are also familiar with local options and associated costs. Most GCMs charge a fee; so be sure to get references and fee information before hiring one. However, if you choose to do the research yourself, the first step is to research options available in your area. From there, you can begin to locate places that meet your specific criteria and the needs of your loved one. There are five major housing options to consider: living with a family member or friend; independent retirement communities; assisted living facilities; nursing homes; and continued care retirement communities. Moving in with a family member or friend is an ideal option when it is convenient for both parties. This allows seniors to remain in a comfortable environment and maintain social relationships. Home care providers can be considered in addition to or instead of this option. Home care services provide specialized non-medical care, as well as create respite opportunities for caregivers—allowing your loved one maximum freedom. Home care should not be confused with home health companies that provide medical assistance in the home. Be sure to look for organizations that provide the type and level of care that is needed. Independent retirement communities are housing options specifically designed for active aging adults who need no assistance with daily activities. This can range from apartments to townhouses and single family homes. This housing option gives seniors maximum independence. Retirement communities generally provide opportunities for socialization, such as games night in a centrally located place on the property. They are ideal for independent seniors looking to retire among people their own age and retain a social life. These communities, however, are not a viable option for those who need help with everyday tasks, such as cooking, cleaning or dressing. Assisted living properties provide a low level of care for seniors who need additional help, but still wish to remain independent. Most are residence hall-style with a choice of single or double rooms. Housing options vary depending on location and can range from a single room to an individual apartment, with or without a kitchen. These properties can offer help with tasks such as dressing, bathing and cleaning, around the clock. In general, most places provide three meals per day, light transportation, 24-hour security, medication management and social activities. Be sure to ask each facility which services they provide. Assisted living is typically not covered by insurance and most families use private pay options such as personal savings, pensions or social security to cover the costs. Because this option can be expensive, it is important to check your budget and make sure you or your loved one can handle the cost long-term. There are a growing number of assisted living properties that specialize in Alzheimer’s and memory care. They provide similar services as a regular assisted living property, however they have trained staff in working with an Alzheimer’s resident’s special needs. In addition, they provide a safe, secure unit for their residents. Skilled nursing facilities provide the highest level of care available. Unlike assisted living, they maintain constant care, with a physician or trained nurse on call at all times. Skilled nursing facilities include residence hall-style housing, three meals per day, and help with dressing, bathing, eating and medication management. 102

Most provide recreational activities for residents and some provide rehab opportunities. This housing option is ideal for those who can no longer live independently and need 24-hour care. Many also offer temporary stay options for those who need constant care after hospitalization and wish to regain their independence post-recovery. Temporary nursing home stays are often covered by Medicare, but long-term or permanent stays are not. Continued care retirement communities, also known as CCRCs, provide increasing levels of care as needs change. These communities are equipped for all levels of care, from completely independent housing options to constant care options. Seniors can live independently at the outset and transition into minimal care options or nursing care as their needs change. A major benefit is that your loved ones do not have to keep moving as their condition changes. Although cost plays a large factor with this option, it creates a stable environment for retiring and allows seniors to form and maintain social relationships. Overall, choosing to transition your loved one into a different living environment is a big decision with many factors to consider. It is important to honestly assess your loved one’s financial situation, current care needs and how those factors may change in the next five to 10 years.

Eulogy

T

he purpose of a eulogy is to acknowledge and share information about a person’s life. It is a great honor for someone to present and it’s an important part of the grieving process for the presenter, as well as the audience. A good place to start is to make a list of thoughts and memories you’d like to share. While others might not be comfortable or able to give their own eulogy, they might find great comfort in sharing memories with you that you could incorporate into the eulogy. Things you might include are stories, quotes, expressions of love or prayers, all of which can be delivered in a serious or humorous manner depending on what you feel is appropriate. In an effort to have the audience connected and involved in your tribute, the eulogy can be compiled from private memories that your audience might not be aware of as well as personal traits and details of the loved one that all will remember. You will want to include any accomplishments or honors no matter how big or small that were especially important to your loved one. Memories shared should always acknowledge the person in a positive manner and recognize them for who they were accurately. Once you are ready to write the eulogy, decide on how you will begin, then pick the main thoughts and memories that you wish to share and finally determine how you will close. You need to properly introduce yourself when you begin and have a summarizing statement to share when you close. You can read from your notes, but practice giving the eulogy and change any part that does not feel like natural conversation in your own words. Presenting a eulogy can cause great anxiety. Stopping at any time to collect your thoughts or emotions and then continuing when you are ready is understandable and acceptable. If you decide that you cannot continue, simply skip to your final statement. It is the message about the impact of your loved that is important, not the delivery, so just be yourself.

Helping Patients and Families to Choose the Best End-of-Life Care Option Provided by Houston Solari Hospice • Educate yourself on local hospices. Your doctor or local hospitals should be able to recommend hospice providers in your area. Be sure to take note whether the hospice providers you are considering also have inpatient hospice homes in the event that pain and symptoms become unmanageable and 24-hour care is needed. • Contact the hospices. Compile a list of potential hospices to contact. Ask whether they are accredited, certified and licensed. If accredited by a nationally recognized accrediting body, such as the Joint Commission on Accreditation of Healthcare Organizations, the hospice has sought accreditation and is committed to providing quality care. State health departments certify hospices, guaranteeing that the facility meets safety and health regulations. Licensing requirements vary by state and are generally overseen by its respective health department. • Ask for and check references. Ask for references from professionals, such as a hospital or community social workers. Talk with these people about their experiences. Also check with the Better Business Bureau or the State Attorney General’s office to find out whether any complaints or lawsuits have been filed against the hospices on your list. • Inquire about a preliminary evaluation and plan of care. As part of a patient’s preliminary evaluation, a hospice should determine the types of services needed for the patient. The evaluation should contain what the patient can and can’t do for themselves, consultation with family physicians already providing the patient with medical treatment and consultation with family members. • Discuss the treatment plan process. After the preliminary evaluation, the hospice should develop a treatment plan that is carefully created with the patient and family and given to all service providers to ensure smooth coordination of care. The plan of care should be clearly written, with copies given to all loved ones involved. The plan should list specific duties, work hours and days, and contact information for the hospice care supervisor. Inquire how frequently the plan is reviewed and updated to accommodate any changing needs. • Learn about the hospice staff. Hospice patients are cared for by an interdisciplinary team of doctors, nurses, social workers, counselors, clergy, volunteers and therapists. Ask about the patient-to-caregiver ratios to make sure that the right amount of care is received. Ask about the continuity of care to determine whether or not the patient will have the same caregivers. This insight can give an indication how satisfied the hospice staff is or if there is a high turnover rate.

Hospice is a program, not a place. Hospice care is usually provided in the comfort of an individual’s home but can also be provided in any environment in which a person lives, includng a nursing home, assisted living facility or residential care facility.

IS HOSPICE ONLY FOR CANCER PATIENTS?

Hospice can be utilized with end-stage organ disease or with end-stage neurological diseases such as Alzheimer’s, dementia or stroke. We also care for patients who have congestive heart failure, chronic lung disease or other conditions.

WHEN SHOULD A DECISION ABOUT ENTERING A HOSPICE PROGRAM BE MADE, AND WHO SHOULD MAKE IT?

Any time during a life-limiting illness is an appropriate time to discuss care options, including hospice. By law, the decision belongs to the patient. Understandably, many people are uncomfortable with the idea of stopping an aggressive effort to “beat” a disease. When a Social Security beneficiary dies, report the death to the Social Security Administration as soon as possible. Fees charged for copies of birth and death certificates by the different county offices may vary slightly – contact your local office about cost for copies of Birth and Death Certificates. Certified copies of the birth and death certificates are issued with a state seal and are printed on security paper. Heirloom birth certificates have a different cost and are printed on special paper that boasts a design celebrating Texas. The heirloom certificate ($60) is only offered through Austin, Texas. Funeral homes will supply certified copies of a death certificate for a small fee if requested at the time funeral arrangements are made. It is recommended that you get at least twelve copies of the certificate and notify anyone with whom the deceased had an account.

Texas Law dictates that both birth and death certificates are deemed confidential for a set period of time. Birth certificates are confidential for 75 years, and death certificates are confidential for 25 years after the death has occurred. If a birth or death certificate is requested before this period of time elapses, certified copies of these documents can only be obtained by a qualified applicant. A qualified applicant can be an immediate family member (by blood or marriage), a legal guardian, or the legal representative of the person for whom the document is requested. If you are not a qualified applicant, you may request a verification of a birth or death record. 124

VERIFICATION OF BIRTH AND DEATH The verification of a birth or death record is NOT a certified copy of the birth or death certificate, but a letter with the person’s name, date of occurrence, county where the birth or death took place and a state file number. Letters may not be available at all birth and death registrars. Verifications are available from 1903 to the present date. Fees for birth and death verification (for regular mail or walk-in service) range from $22–$30. This fee is non-refundable for records that are not located. Additional fees will apply for faxing and overnight services.

Deceased Identity Theft To avoid identity theft of a deceased person, it is recommended that you take the following steps: • Scale back the obituary—don’t provide too much information. • Notify banks and creditors. • Contact credit reporting bureaus— request decedent’s credit report and flag as “deceased.” • Report death to Social Security; they will flag decedent’s number as inactive. Also notify your local passport office, if applicable.

Hospice

ospice care is a philosophy of care that accepts dying as a natural part of life. When death is inevitable, hospice seeks neither to hasten nor postpone it. Hospice provides care, comfort and support for persons with life-limiting conditions as well as their families.

How Does Hospice Work? Hospice care is for any person who has a life-threatening or terminal illness. All hospices consider the patient and family together as the unit of care. Most reimbursement sources require a prognosis of six months or less if the illness runs its normal course. Patients with both cancer and non-cancer illnesses are eligible to receive hospice care. The majority of hospice patients are cared for in their own homes or the homes of a loved one. “Home� may also be broadly construed to include services provided in nursing homes or hospitals. Typically, a family member serves as the primary caregiver and when appropriate, helps make decisions for the hospice patient. Members of the hospice staff make regular visits to assess the patient and provide additional care or other services. Hospice staff is on call 24 hours a day, seven days a week. Who is the Hospice Team? Hospice care is a family-centered approach that includes, at a minimum, a team of doctors, nurses, social workers, counselors, and trained volunteers. They work together focusing on the dying patient’s needs; physical, psychological, or spiritual. The goal is to help keep the patient as pain-free as possible until death. The hospice team develops a care plan that meets each patient’s individual needs for pain management and symptom control. How is Hospice Paid for? Hospice is paid for through the Medicare Hospice Benefit, Medicaid Hospice Benefit, and private insurers. If a person does not have coverage through Medicare, Medicaid or a private insurance company, hospices will work with the person and their family to ensure needed services can be provided. Who is Eligible for Medicare Hospice Benefits? Hospice care is covered under Medicare Part A (hospital Insurance). You are eligible for Medicare hospice benefits when you meet all of the following conditions: • You are eligible for Medicare Part A (hospital Insurance), and • Your doctor and the hospice medical director certify that you are terminally ill and probably have less than six months to live, and • You sign a statement choosing hospice care instead of routine Medicare covered benefits for your terminal illness, and • You receive care from a Medicare-approved hospice program. Please note: Medicare will still pay for covered benefits for any health problems that aren’t related to your terminal illness. What Does Medicare Cover? Medicare covers these hospice services and pays nearly all of their costs: • Doctor services and Nursing care • Medical equipment (like wheelchairs or walkers) • Medical supplies (like bandages and catheters) • Drugs for symptom control and pain relief • Short-term hospital care, including respite and inpatient for pain/symptom management • Home health aide and homemaker services • Physical, Occupational therapy and Speech therapy • Social work services and Dietary counseling • Grief support to help you and your family Please note: You will only have to pay part of the cost for outpatient drugs and inpatient respite care. The Medicare Hospice Benefit Does Not Cover the Following: • Treatment intended to cure your terminal illness. You should talk with your doctor if you are thinking about potential treatment to cure your illness. • Medications not directly related to your hospice diagnosis. Hospice team 128

members will consult with the hospice physician and will inform you and your family which drugs and/or medications are covered and which ones are not covered under the Medicare Hospice Benefit. • Room and board aren’t covered by Medicare. You may receive hospice services wherever you live, even in a nursing home, however, the Medicare Hospice Benefit does not pay for nursing home room and board. Respite Care and Medicare Respite care is care given to a hospice patient by another caregiver so that the usual caregiver can rest. As a hospice patient, you may have one person who takes care of you every day, like a family member. Sometimes your caregiver needs someone to take care of you for a short time while they do other things that need to be done. During a period of respite care, you might be cared for in a Medicare-approved facility, such as a hospice residential facility, hospital, or nursing home. How Do I Begin Receiving Hospice? Anyone can inquire about hospice services. You or your loved one may call a local hospice and request services. The hospice staff will then contact your physician to determine if a referral to hospice is appropriate. Another way to inquire about hospice is to talk with your physician, and he or she can make a referral to hospice. Hospice can begin as soon as a ‘referral’ is made by the person’s doctor. As a hospice patient, you always have the right to stop getting hospice care and receive the “traditional” Medicare coverage you had before electing hospice. All care that you receive for your illness must be given by your hospice team. Questions to Ask When Choosing a Hospice Program • What services are provided? • What kind of support is available to the family/caregiver? • What roles do the attending physician and hospice play? • How does hospice work to keep the patient comfortable? • How are services provided after hours? • How and where does hospice provide short-term inpatient care? • Can hospice be brought into a nursing home or long-term care facility? • Are any of the staff members certified or credentialed in hospice? • Do physicians visit the patient in the home? • What is the staff to patient ratio? • Are any alternative therapies provided to manage symptoms? • What ‘extras’ does the hospice provide? roviding care for a parent or an adult with a disability can be overwhelming, and many people – 2.7 million in Texas – find they need information, assistance and support. This type of help is available from the Texas Department of Aging and Disability Services (DADS) and the Texas Health and Human Services Commission (HHSC). Three resources – Take Time Texas, 2-1-1 Texas and a Resource Sheet – take much of the guesswork (and hopefully much of the stress) out of finding help. Take Time Texas The Take Time Texas (TTT) website is designed to help caregivers continue to provide support to their loved ones through respite care. “Based on surveys, DADS learned that many caregivers do not know how or where to find respite care. Some do not even know what respite care is, or that it is available,” said Joyce Pohlman, DADS Access and Intake division. “Take Time Texas was created to help caregivers find respite more quickly and easily.”

The Take Time Texas website offers: • Statewide database of respite providers that caregivers can search to find nonprofit, commercial, faith-based and governmental respite providers. • Content to help caregivers and providers resolve problems and improve caregiving. • Resources to help respite providers, outreach materials, and information on best practices and training tools. 2-1-1 Texas 2-1-1 Texas, an HHSC program, helps Texans connect with the services, such as healthcare through Medicaid, food assistance and short-term financial help. 2-1-1 Texas offers accurate, well-organized and easy-to-find information from more than 60,000 state and local health and human services programs. The free, anonymous information and referral line is answered by nationally certified specialists 24 hours a day, seven days a week. When callers dial 2-1-1, they are connected to area information centers in their region. Specialists have access to the most comprehensive database of community-based organizations, government agencies and nonprofits in Texas. No matter where you live in Texas, you can call 2-1-1 and find information about resources in your local community. Texas 2-1-1 can be accessed online at www.211Texas.org, or by phone at 2-1-1. Resource Sheet Developed by DADS for the Age Well Live Well program, this resource sheet covers topics that include Medicare and Medicaid, in-home assistance and home health care, caregiving services, legal services, transportation, veterans’ services and more. Age Well Live Well program manager Holly Riley said, “Many people don’t realize the breadth of services available through the federal government and the state, much less local providers. We polled several state agencies and found the top 10 questions they were getting from consumers. The result is a resource guide founded upon real-world issues.” The sheet is updated at least annually to ensure the information is timely and accurate, Riley said. The Original Tres Leches Courtesy of Churrascos Ingredients: Sponge cake 7 eggs 2 cups sugar 2 cups flour 2 tablespoons baking powder ½ cup milk Milks: 3 cups condensed milk 3 cups evaporated milk 3 cups heavy cream 2 tablespoons vanilla

Meringue: 1 cup egg whites 1 teaspoon cream of tartar 2 cups sugar 1 cup water 1 teaspoon vanilla

Directions: Preheat oven to 350 degrees. Whip the eggs until fluffy, about 5 to 7 minutes. Add the sugar under creamed, about 3 minutes. Sift together the dry ingredients and gradually add to the mixer. Reduce the speed and add the milk a little at a time. Mix at low speed for 10 minutes. Pour into a greased 9x13x2–inch cake mold. Bake for 30 minutes, then lower heat to 325 degrees and bake an additional 20 minutes, until the top of the cake is nicely browned. Meanwhile, combine the milks, cream and vanilla, whisking until smooth, about 3 to 4 minutes. Remove the cake from the oven and, using a serrated cake knife, slice off the brown top (this makes a nice, crisp snack for the baker). Pour the milk mixture over the top. Let the cake soak for about 20 minutes. Prepare the meringue by heating the sugar and water to 240 degrees in a saucepan, then removing from heat. Beat egg whites with the cream of tartar in a mixer on high until soft peaks form, about 3 minutes, then slowly add the syrup. Add vanilla and keep mixing on slow speed until the bowl cools to the touch, about 15 minutes. Pour excess milk from cake and top with meringue. Use the tines of a fork to draw wavy ridges lengthwise across the cake for decoration. Cut into squares. Serves 15. Find

IS HOSPICE A PLACE?

Hospice is a program, not a place. Hospice care is usually provided in the comfort of an individual’s home but can also be provided in any environment in which a person lives, includng a nursing home, assisted living facility or residential care facility.

IS HOSPICE ONLY FOR CANCER PATIENTS?

Hospice can be utilized with end-stage organ disease or with end-stage neurological diseases such as Alzheimer’s, dementia or stroke. We also care for patients who have congestive heart failure, chronic lung disease or other conditions.

WHEN SHOULD A DECISION ABOUT ENTERING A HOSPICE PROGRAM BE MADE, AND WHO SHOULD MAKE IT?

Any time during a life-limiting illness is an appropriate time to discuss care options, including hospice. By law, the decision belongs to the patient. Understandably, many people are uncomfortable with the idea of stopping an aggressive effort to “beat” a disease. Altus Hospice staff members are highly sensitive to these concerns and are always available to discuss them with the patient, family and physician.
