In Retirement

etting older is not a guarantee that you will stay married. Along with the freedom from yard work, fixing household problems and working, couples are rediscovering themselves and their partners.

Over dinner one evening, our friend looked at his wife and said, “I don’t know you in retirement.” My eyes grew wide and I happily expressed, “Wow, an honest man.” In truth, we never know our partner completely, and this is what keeps marriage exciting. We all laughed, but it was laughter over sharing a bittersweet truth. I don’t know many people who aren’t looking forward to retirement. Many of our friends have retired and they may talk about their trips, leisure golf games and weekends with the grandkids, but if you listen closely, you will also hear them talk about the difficulty of their transition. Preparing for retirement involves taking small steps. Letting go of work is a huge step, and it doesn’t matter if you complained about your job every day; when it is gone you will feel a void. grief work is part of the transition of retirement. Many couples don’t prepare for retirement emotionally. Whenever you idealize a transition, no matter if it is a marriage, a new baby or retirement, you are sure to be disappointed. Couples need to reacquaint themselves with their partner and revitalize their marriage. In essence, no one really knows their partner completely and when the other partner is going through a transition along with you, navigating the relationship can be tricky. Your marriage has to transition in a way it has not done before. Prior to retirement your outlook or vision together is always the future. During retirement, there is a heavy emphasis on the present. People become more aware of their mortality and the importance of living each day in the present. Developing a sense of exploration and curiosity is important, and actually helps predict how healthy you will be through your retirement, both physically and emotionally. If you are on the verge of retirement, these suggestions may help you make the emotional transition as well as the physical ones of actually ceasing your employment:

IT’S OK TO GRIEVE Grieving is an important part of retirement, and it does happen whether you acknowledge it or not. It usually presents as sadness, anger or irritability. The more you can journal about how you feel or have a good friend—hopefully your spouse is a good listener—the easier it will be to get through the process.

• STAY ACTIVE

Physical activity has never been more important than in your retirement. No matter what happens in your life, take 30 minutes each day to be active. Marriages improve with walks; there is no healthier way to begin conversation.

BE INTIMATE Keep intimacy with your partner in your retirement years. Couples who are more intimate have healthier hearts, minds and bodies. Date nights are not exclusive for the young. Keep them in your marriage ’til death do you part.

• FORM FRIENDSHIPS

Make new friends and keep the old. The more social you are with other couples, the better your own marriage becomes. Other couples give us new thoughts, new reflections and help us think out of our own box. They also keep laughter in our life.

• TRY NEW THINGS

This sounds like a cliché, but try new things. It can be a new spiritual retreat, a new boating club, or a pet owners club and anything in between. When you try new things, you help the brain form new connections. If your brain ever needed new connections, it needs them in the senior years.

• SEPARATE INTERESTS

Keep your own interests separate from your partner. The more interests you can bring to the relationship, the more the relationship is going to thrive. Also, the more open and accepting you can be of your partner— who is no longer in the mom or dad role—the more interesting and exciting your retirement can be. Kids may help stabilize a marriage, and work gets us in a routine—when those two roles are finished, the greatest role may be freedom; re-learning who we are and who we are married to.

RECIPE

The Original Tres Leches Courtesy of Churrascos Ingredients: Sponge cake 7 eggs 2 cups sugar 2 cups flour 2 tablespoons baking powder ½ cup milk Milks: 3 cups condensed milk 3 cups evaporated milk 3 cups heavy cream 2 tablespoons vanilla

Meringue: 1 cup egg whites 1 teaspoon cream of tartar 2 cups sugar 1 cup water 1 teaspoon vanilla

Directions: Preheat oven to 350 degrees. Whip the eggs until fluffy, about 5 to 7 minutes. Add the sugar under creamed, about 3 minutes. Sift together the dry ingredients and gradually add to the mixer. Reduce the speed and add the milk a little at a time. Mix at low speed for 10 minutes. Pour into a greased 9x13x2–inch cake mold. Bake for 30 minutes, then lower heat to 325 degrees and bake an additional 20 minutes, until the top of the cake is nicely browned. Meanwhile, combine the milks, cream and vanilla, whisking until smooth, about 3 to 4 minutes. Remove the cake from the oven and, using a serrated cake knife, slice off the brown top (this makes a nice, crisp snack for the baker). Pour the milk mixture over the top. Let the cake soak for about 20 minutes. Prepare the meringue by heating the sugar and water to 240 degrees in a saucepan, then removing from heat. Beat egg whites with the cream of tartar in a mixer on high until soft peaks form, about 3 minutes, then slowly add the syrup. Add vanilla and keep mixing on slow speed until the bowl cools to the touch, about 15 minutes. Pour excess milk from cake and top with meringue. Use the tines of a fork to draw wavy ridges lengthwise across the cake for decoration. Cut into squares. Serves 15. Find 36

Creating a butterfly garden is more than just planting colorful flowers to attract them. Like any other creature, butterflies have specific needs when it comes to their habitat.

Here are a few plants that are sure to create a colorful, nectar-rendering, butterfly-friendly habitat right in your own backyard. Blue sage is a native Texas perennial, drought-resistant and blooms in shades of purple and blue nearly year-round. Texas lantana thrives in hot Texas temperatures. It produces bright colorful blossoms and attracts swallowtails, sulphurs and brush-foot butterflies. Asclepias is also known as milkweed and comes in several varieties. The most common variety in Texas is also known as Antelope Horn. Tropical Milkweed is not native to Texas but is widely available in garden centers and blooms in bright bold colors. Zinnias grow well in the Texas heat and are easy to grow from a seed. They are annuals, so be prepared to plant them each spring. Buddleia (butterfly bush) usually bloom mid-July through frost, in vibrant shades of purple, red and white. A butterfly garden will not only attract colorful creatures to your space, but will also create a colorful atmosphere in which you can relax and enjoy. Don’t forget to feed the next generation of butterflies— the caterpillars! Making these decisions in advance lets medical professionals know what measures to take and grants your loved ones peace of mind, should they have to communicate these decisions for you. 48

Durable Power of Attorney for Health Care While an advance directive is limited to end-of-life concerns, a durable power of attorney for health care accounts for all health care decisions in the case that you are incapable of making decisions for yourself. Home Care vs Home Health Care How can I determine if home care is necessary? When self-care at home becomes difficult and/or family caregivers are not available to assist, home care may be the answer. Home care often relieves family members of basic caregiving functions and makes it possible to spend quality time, rather than the functional caregiving time. Assisted Living Licensure Differences in Texas

I

n order to choose the “right fit” for your loved one, you should first know the various types of elder care facilities, their licenses and some of the basic requirements. First, there are two types: Nursing Homes and Assisted Living. Nursing Homes are a medical model and are not discussed in this article. Assisted Living licenses for the care of the elderly and disabled are issued by the Texas Department of Aging and Disability Services (TXDADS) (www.dads.state.tx.us). The Assisted Living licenses are designated “A” small or large and “B” small or large. To be designated an Alzheimer’s Facility, the facility must carry a “B” license (small or large). The Alzheimer’s Certified Facility must also meet many other requirements as directly related to the care and stimulation of residents with a dementia diagnosis. So now, let’s begin: Small facilities are 4 to 16 residents; large facilities are 17 residents or more. Having one to three residents in a home does not require licensing by the State. Anyone who houses more than 3 elderly or disabled residents in Texas without a license is breaking the law.

Assisted Living Licensure Differences in Texas An “A” license is for residents who can perform most of their activities of daily living (ADLs) themselves. An “A” requires that the resident can self-evacuate the building without assistance in 13 minutes or less. An “A” requires a caregiver to be on premises at all times; but may sleep during the night. An “A” provides all meals and provides some activities. Also an “A”, large or small, may not administer medications or lock their doors to the outside. At this time in Houston (2011), an “A” licensed facility requires a fire alarm system, but may or may not require a sprinkler system. A “B” license, on the other hand, covers frail residents who are not able to get around as well. The “B” facility, large or small, must have a fire sprinkler system as well as a fire alarm system before they can obtain a license in the State of Texas. Due to the resident’s fragile condition, the regulation states that the facility must have “adequate” staff to care for them. The State Surveyor makes this determination based on many safety factors. In addition, the night staff of a “B” facility, small or large, must have awake staff all night. A small “B” facility may plan its own menus and activities. A large “B” facility must also have a licensed nurse

on duty at least 40 hours per week and employ a licensed dietician and certified activity director. On the other hand, when a facility says or advertises that they are an Alzheimer’s Facility, they must have an Alzheimer’s Certification, which cannot be obtained until they have the “B” license described above.

Some Basic Requirements for Alzheimer’s Certification: Minimum of 1.5 hrs cognitive, recreational and physical activities per day. An RN on staff to train caregivers in Medication Administration and perform all RN Delegation duties surrounding Medication Administration in an Alzheimer’s Facility. An RN on staff to perform initial assessments. Each caregiver must have an additional 12 hours of Alzheimer’s specific training above the usual 6 hours per year required in other Assisted Living facilities.

Photography | Brenlee McKnight Ingredients: 4 cups chicken stock, light 3 tablespoons olive oil 1½ cups white wine pinch of saffron 1 tablespoon butter 1 small onion, finely chopped

1 cup Arborio rice 1 clove garlic, chopped 1 cup fresh English peas, shelled 20 to 25 fresh mussels, scrubbed and beard removed

Directions: Heat oil in skillet to 325 degrees. Take asparagus, coat in flour, then egg, then flour again, and then egg again, then Parmesan cheese. Fry until golden brown. While the asparagus is frying, start the sauce. Heat clarified butter in a skillet over medium heat. Add garlic, shallot and lemon zest. Cook for 2 to 3 minutes, and then add stock, wine and lemon juice. When liquids have reduced, add crabmeat, diced tomato, then cold butter, basil and salt, and pepper to taste. When the sauce has a creamy consistency, remove from heat and top the asparagus.

W

hen everyday tasks such as cooking or maintaining a house appear to be burdensome for a loved one, or signs of loneliness become prevalent, it may be time explore a new living situation. So, where do you go from here?

Eldercare professionals such as a geriatric care manager (GCM) might be a good place to start. They can be instrumental in helping determine the specific services your loved one may need and are also familiar with local options and associated costs. Most GCMs charge a fee; so be sure to get references and fee information before hiring one. However, if you choose to do the research yourself, the first step is to research options available in your area. From there, you can begin to locate places that meet your specific criteria and the needs of your loved one. There are five major housing options to consider: living with a family member or friend; independent retirement communities; assisted living facilities; nursing homes; and continued care retirement communities. Moving in with a family member or friend is an ideal option when it is convenient for both parties. This allows seniors to remain in a comfortable environment and maintain social relationships. Home care providers can be considered in addition to or instead of this option. Home care services provide specialized non-medical care, as well as create respite opportunities for caregivers—allowing your loved one maximum freedom. Home care should not be confused with home health companies that provide medical assistance in the home. Be sure to look for organizations that provide the type and level of care that is needed. Independent retirement communities are housing options specifically designed for active aging adults who need no assistance with daily activities. This can range from apartments to townhouses and single family homes. This housing option gives seniors maximum independence. Retirement communities generally provide opportunities for socialization, such as games night in a centrally located place on the property. They are ideal for independent seniors looking to retire among people their own age and retain a social life. These communities, however, are not a viable option for those who need help with everyday tasks, such as cooking, cleaning or dressing. Assisted living properties provide a low level of care for seniors who need additional help, but still wish to remain independent. Most are residence hall-style with a choice of single or double rooms. Housing options vary depending on location and can range from a single room to an individual apartment, with or without a kitchen. These properties can offer help with tasks such as dressing, bathing and cleaning, around the clock. In general, most places provide three meals per day, light transportation, 24-hour security, medication management and social activities. Be sure to ask each facility which services they provide. Assisted living is typically not covered by insurance and most families use private pay options such as personal savings, pensions or social security to cover the costs. Because this option can be expensive, it is important to check your budget and make sure you or your loved one can handle the cost long-term. There are a growing number of assisted living properties that specialize in Alzheimer’s and memory care. They provide similar services as a regular assisted living property, however they have trained staff in working with an Alzheimer’s resident’s special needs. In addition, they provide a safe, secure unit for their residents. Skilled nursing facilities provide the highest level of care available. Unlike assisted living, they maintain constant care, with a physician or trained nurse on call at all times. Skilled nursing facilities include residence hall-style housing, three meals per day, and help with dressing, bathing, eating and medication management. 106

Most provide recreational activities for residents and some provide rehab opportunities. This housing option is ideal for those who can no longer live independently and need 24-hour care. Many also offer temporary stay options for those who need constant care after hospitalization and wish to regain their independence post-recovery. Temporary nursing home stays are often covered by Medicare, but long-term or permanent stays are not. Continued care retirement communities, also known as CCRCs, provide increasing levels of care as needs change. These communities are equipped for all levels of care, from completely independent housing options to constant care options. Seniors can live independently at the outset and transition into minimal care options or nursing care as their needs change. A major benefit is that your loved ones do not have to keep moving as their condition changes. Although cost plays a large factor with this option, it creates a stable environment for retiring and allows seniors to form and maintain social relationships. Overall, choosing to transition your loved one into a different living environment is a big decision with many factors to consider. It is important to honestly assess your loved one’s financial situation, current care needs and how those factors may change in the next five to 10 years.

Photography | Capella Pedregal Ingredients: 1 stalk of celery 2 avocados, peeled and pitted 2 onions 2 cups grated parmesan cheese 1 carrot 1 cup lime juice

3 serrano chiles 2 cups chipotle chiles 10 habanero chiles 4 tomatoes, peeled and quartered 12 basil leaves salt and pepper to taste

Directions: Preheat oven to 350 F. Roughly chop vegetables into 1 inch size chunks. In a large roasting pan, place celery, onions, carrot, serrano chile, habanero chile and basil leaves; roast in an oven at 350 F until vegetables are brown (approximately 10 minutes without oil). Place roasted vegetables along with grated parmesan cheese, avocado, peeled tomato, chile chipotle and the lime juice into a blender or food processor and blend until the salsa its thick and well mixed. Add salt and pepper to taste.

Helping Patients and Families to Choose the Best End-of-Life Care Option Provided by Houston Solari Hospice • Educate yourself on local hospices. Your doctor or local hospitals should be able to recommend hospice providers in your area. Be sure to take note whether the hospice providers you are considering also have inpatient hospice homes in the event that pain and symptoms become unmanageable and 24-hour care is needed. • Contact the hospices. Compile a list of potential hospices to contact. Ask whether they are accredited, certified and licensed. If accredited by a nationally recognized accrediting body, such as the Joint Commission on Accreditation of Healthcare Organizations, the hospice has sought accreditation and is committed to providing quality care. State health departments certify hospices, guaranteeing that the facility meets safety and health regulations. Licensing requirements vary by state and are generally overseen by its respective health department. • Ask for and check references. Ask for references from professionals, such as a hospital or community social workers. Talk with these people about their experiences. Also check with the Better Business Bureau or the State Attorney General’s office to find out whether any complaints or lawsuits have been filed against the hospices on your list. • Inquire about a preliminary evaluation and plan of care. As part of a patient’s preliminary evaluation, a hospice should determine the types of services needed for the patient. The evaluation should contain what the patient can and can’t do for themselves, consultation with family physicians already providing the patient with medical treatment and consultation with family members. • Discuss the treatment plan process. After the preliminary evaluation, the hospice should develop a treatment plan that is carefully created with the patient and family and given to all service providers to ensure smooth coordination of care. The plan of care should be clearly written, with copies given to all loved ones involved. The plan should list specific duties, work hours and days, and contact information for the hospice care supervisor. Inquire how frequently the plan is reviewed and updated to accommodate any changing needs. • Learn about the hospice staff. Hospice patients are cared for by an interdisciplinary team of doctors, nurses, social workers, counselors, clergy, volunteers and therapists. Ask about the patient-to-caregiver ratios to make sure that the right amount of care is received. Ask about the continuity of care to determine whether or not the patient will have the same caregivers. This insight can give an indication how satisfied the hospice staff is or if there is a high turnover rate.

Texas Law dictates that both birth and death certificates are deemed confidential for a set period of time. Birth certificates are confidential for 75 years, and death certificates are confidential for 25 years after the death has occurred. If a birth or death certificate is requested before this period of time elapses, certified copies of these documents can only be obtained by a qualified applicant. A qualified applicant can be an immediate family member (by blood or marriage), a legal guardian, or the legal representative of the person for whom the document is requested. If you are not a qualified applicant, you may request a verification of a birth or death record. 128

Fees charged for copies of birth and death certificates by the different county offices may vary slightly – contact your local office about cost for copies of Birth and Death Certificates. Certified copies of the birth and death certificates are issued with a state seal and are printed on security paper. Heirloom birth certificates have a different cost and are printed on special paper that boasts a design celebrating Texas. The heirloom certificate ($60) is only offered through Austin, Texas. Funeral homes will supply certified copies of a death certificate for a small fee if requested at the time funeral arrangements are made. It is recommended that you get at least twelve copies of the certificate and notify anyone with whom the deceased had an account. VERIFICATION OF BIRTH AND DEATH The verification of a birth or death record is NOT a certified copy of the birth or death certificate, but a letter with the person’s name, date of occurrence, county where the birth or death took place and a state file number. Letters may not be available at all birth and death registrars. Verifications are available from 1903 to the present date. Fees for birth and death verification (for regular mail or walk-in service) range from $22–$30. This fee is non-refundable for records that are not located. Additional fees will apply for faxing and overnight services.

Deceased Identity Theft To avoid identity theft of a deceased person, it is recommended that you take the following steps: • Scale back the obituary—don’t provide too much information. • Notify banks and creditors. • Contact credit reporting bureaus— request decedent’s credit report and flag as “deceased.” • Report death to Social Security; they will flag decedent’s number as inactive. ospice care is a philosophy of care that accepts dying as a natural part of life. When death is inevitable, hospice seeks neither to hasten nor postpone it. Hospice care is for any person who has a life-threatening or terminal illness. All hospices consider the patient and family together as the unit of care. Most reimbursement sources require a prognosis of six months or less if the illness runs its normal course. Patients with both cancer and non-cancer illnesses are eligible to receive hospice care. The majority of hospice patients are cared for in their own homes or the homes of a loved one. “Home� may also be broadly construed to include services provided in nursing homes or hospitals. Typically, a family member serves as the primary caregiver and when appropriate, helps make decisions for the hospice patient. Members of the hospice staff make regular visits to assess the patient and provide additional care or other services. Hospice staff is on call 24 hours a day, seven days a week. WHO IS THE HOSPICE TEAM? Hospice care is a family-centered approach that includes, at a minimum, a team of doctors, nurses, social workers, counselors, and trained volunteers. They work together focusing on the dying patient’s needs; physical, psychological, or spiritual. The goal is to help keep the patient as pain-free as possible until death. The hospice team develops a care plan that meets each patient’s individual needs for pain management and symptom control. HOW IS HOSPICE PAID FOR? Hospice is paid for through the Medicare Hospice Benefit, Medicaid Hospice Benefit, and private insurers. If a person does not have coverage through Medicare, Medicaid or a private insurance company, hospices will work with the person and their family to ensure needed services can be provided. WHO IS ELIGIBLE FOR MEDICARE HOSPICE BENEFITS? Hospice care is covered under Medicare Part A (hospital Insurance). You are eligible for Medicare hospice benefits when you meet all of the following conditions: • You are eligible for Medicare Part A (hospital Insurance), and • Your doctor and the hospice medical director certify that you are terminally ill and probably have less than six months to live, and • You sign a statement choosing hospice care instead of routine Medicare covered benefits for your terminal illness, and • You receive care from a Medicare-approved hospice program. • Please note: Medicare will still pay for covered benefits for any health problems that aren’t related to your terminal illness. WHAT DOES MEDICARE COVER? Medicare covers these hospice services and pays nearly all of their costs: • Doctor services and Nursing care • Medical equipment (like wheelchairs or walkers) • Medical supplies (like bandages and catheters) • Drugs for symptom control and pain relief • Short-term hospital care, including respite and inpatient for pain/symptom management • Home health aide and homemaker services • Physical, Occupational therapy and Speech therapy • Social work services and Dietary counseling • Grief support to help you and your family Please note: You will only have to pay part of the cost for outpatient drugs and inpatient respite care. THE MEDICARE HOSPICE BENEFIT DOES NOT COVER THE FOLLOWING: • Treatment intended to cure your terminal illness. You should talk with your doctor if you are thinking about potential treatment to cure your illness. • Medications not directly related to your hospice diagnosis. Hospice team members will consult with the hospice physician and will inform you and 132

your family which drugs and/or medications are covered and which ones are not covered under the Medicare Hospice Benefit. • Room and board aren’t covered by Medicare. You may receive hospice services wherever you live, even in a nursing home, however, the Medicare Hospice Benefit does not pay for nursing home room and board. RESPITE CARE AND MEDICARE Respite care is care given to a hospice patient by another caregiver so that the usual caregiver can rest. As a hospice patient, you may have one person who takes care of you every day, like a family member. Sometimes your caregiver needs someone to take care of you for a short time while they do other things that need to be done. During a period of respite care, you might be cared for in a Medicare-approved facility, such as a hospice residential facility, hospital, or nursing home. HOW DO I BEGIN RECEIVING HOSPICE? Anyone can inquire about hospice services. You or your loved one may call a local hospice and request services. The hospice staff will then contact your physician to determine if a referral to hospice is appropriate. Another way to inquire about hospice is to talk with your physician, and he or she can make a referral to hospice. Hospice can begin as soon as a ‘referral’ is made by the person’s doctor. As a hospice patient, you always have the right to stop getting hospice care and receive the “traditional” Medicare coverage you had before electing hospice. All care that you receive for your illness must be given by your hospice team. QUESTIONS TO ASK WHEN CHOOSING A HOSPICE PROGRAM • What services are provided? • What kind of support is available to the family/caregiver? • What roles do the attending physician and hospice play? • How does hospice work to keep the patient comfortable? • How are services provided after hours? • How and where does hospice provide short-term inpatient care? • Can hospice be brought into a nursing home or long-term care facility? • Are any of the staff members certified or credentialed in hospice? • Do physicians visit the patient in the home? • What is the staff to patient ratio? • Are any alternative therapies provided to manage symptoms? • What ‘extras’ does the hospice provide? roviding care for a parent or an adult with a disability can be overwhelming, and many people – 2.7 million in Texas – find they need information, assistance and support. This type of help is available from the Texas Department of Aging and Disability Services (DADS) and the Texas Health and Human Services Commission (HHSC). Three resources – Take Time Texas, 2-1-1 Texas and a Resource Sheet – take much of the guesswork (and hopefully much of the stress) out of finding help. Take Time Texas The Take Time Texas (TTT) website is designed to help caregivers continue to provide support to their loved ones through respite care. “Based on surveys, DADS learned that many caregivers do not know how or where to find respite care. Some do not even know what respite care is, or that it is available,” said Joyce Pohlman, DADS Access and Intake division. “Take Time Texas was created to help caregivers find respite more quickly and easily.”

The Take Time Texas website offers: • Statewide database of respite providers that caregivers can search to find nonprofit, commercial, faith-based and governmental respite providers. • Content to help caregivers and providers resolve problems and improve caregiving. • Resources to help respite providers, outreach materials, and information on best practices and training tools. 2-1-1 Texas 2-1-1 Texas, an HHSC program, helps Texans connect with the services, such as healthcare through Medicaid, food assistance and short-term financial help. 2-1-1 Texas offers accurate, well-organized and easy-to-find information from more than 60,000 state and local health and human services programs. The free, anonymous information and referral line is answered by nationally certified specialists 24 hours a day, seven days a week. When callers dial 2-1-1, they are connected to area information centers in their region. Specialists have access to the most comprehensive database of community-based organizations, government agencies and nonprofits in Texas. No matter where you live in Texas, you can call 2-1-1 and find information about resources in your local community. Texas 2-1-1 can be accessed online at www.211Texas.org, or by phone at 2-1-1. Resource Sheet Developed by DADS for the Age Well Live Well program, this resource sheet covers topics that include Medicare and Medicaid, in-home assistance and home health care, caregiving services, legal services, transportation, veterans’ services and more. Age Well Live Well program manager Holly Riley said, “Many people don’t realize the breadth of services available through the federal government and the state, much less local providers. We polled several state agencies and found the top 10 questions they were getting from consumers. The result is a resource guide founded upon real-world issues.” The sheet is updated at least annually to ensure the information is timely and accurate, Riley said.
