Two (2) color photos, size 3x4: 1 glue at the form and 1 with the name of the applicant on the back. Instructions to fill the Accreditation form a) Name of Organization to be accredited: to write the name of the Organization to be accredited to observe Early Election 2018; b) Organization responsible: name of the person who is the responsible for the organization mentioned at a); c) Address of the organization: where is physically located the mentioned organization; d) Focal point: name of the person which was selected to be the focal point of the Observation Mission; e) Type of the Organization: to select one (1) option of ONG, Religious or the academic. If it is not none of this options, please select others and describe the type of organization; f)
Total number of members to be accredited: to write the specific total number of observers to be accredited by the mentioned Organization;
g) To be filled only by STAE officer: to be filled by the STAE officer accreditation (name, date and signature);
