Enrolment Forms
The Education and Care Services National Regulations and Law outlines that the following documentation is required to be completed in full prior to your child’s commencement in the ELC for 2025.
• Confidential Enrolment Form – this electronic form will be emailed to you from the Admissions Department on 2 December. Please note that it is mandatory to complete all sections of this form. You must also nominate two people in addition to the parents as emergency contacts.
• Operoo Profile – you will receive email notifications from Operoo to set up a profile for your child and will continue to receive regular email reminders until this is complete. This platform is used by the school to store medical information and other forms of permission. It is essential that this is fully complete and up to date as it will be used to obtain information in the event of an emergency.
• All documentation must be fully completed and returned by email to admissions@stcatherines.net.au by Monday 9 December 2024.
Health and Medical Information
• Immunisation History Statement – you will be required to provide a current and up to date Medicare Immunisation History Statement for your child when you submit your child’s enrolment form. We can only accept the Medicare Immunisation History Statement as this provides all of the necessary immunisation details and complies with legislation. We are unable to accept other forms of Immunisation Certificates. You will then be required to submit subsequent up to date immunisation history statements at periods no greater than 7 months during your child’s time in the ELC. This is an ongoing regulatory requirement.
• Medical Conditions - If your child is diagnosed as having Allergies, Anaphylaxis, Asthma or Diabetes, you are required to have a specific Action Plan completed by your child’s doctor. This needs to be submitted to the school with your child’s enrolment records and a copy uploaded to Operoo. You will also need to provide the ELC with any associated medication prior to commencement.
• If your child does have one of these conditions, you will be sent an information pack from the School Nurses prior to commencement.

St Catherine’s ELC Uniform
• Uniform items can be purchased from Bob Stewart store in Waverley Road Malvern East.
• Information regarding the uniform requirements is outlined on the orientation portal and in the ELC Handbook.
