Remind your athletes to tell coaching staff right away if they think they have a concussion or that a teammate has a concussion.

Make sure that athletics follow the rules of safety and the rules of the sport.

Encourage athletes to practice good sportsmanship

Keep the Heads Up Action Plan at all games and practices.

DANCE Children should be ready to attend class independently, while parents and caregivers wait outside of the studio unless otherwise specified below. Ballet and Tap Learn basic ballet and tap steps and dance terminology through fun dance activities like tippy toe turns and alligator walks in this combination class. Props and fun music are incorporated. Bring both ballet shoes with leather soles and tap shoes.

No instruction is provided, but our master pottery kiln techs can provide technical assistance with equipment and firing advice. Become a lifeguard this summer with the training and certification in lifeguarding, first aid and CPR needed for the professional rescuer. Participants must complete a 300 yard swim and a timed event with a 10 lb. brick on the first day. See page 40 for refund policy. Attendance and participation at all class meetings is required. Tai Chi Chuan Exercise your body and mind with the slow motion, continuous movements of this traditional Chinese martial art. Missed classes If a class meeting is cancelled by the City, staff will make every effort to notify you as soon as possible by phone or email. Staff will let you know if there is a make-up class or credit will be issued. Please make sure your contact information is current so we can reach you. The City does not offer make-up classes or issue credits when individual customers miss a class for illness or other personal reasons.

Non-participants The City of Sunnyvale does not allow unregistered individuals in the program space, except when family and friends are invited to attend youth performances or demonstrations.

Fee Waiver Assistance & Subsidized Program for Youth Programs Fee Waiver Assistance: Eligible Sunnyvale residents may apply to offset the expense of class registration fees up to $234 for youth (ages 17 and under) per eligible family member, per fiscal year. Fee Waiver Assistance Program resources are limited and subject to the availability of funds. Subsidized Program: If preapproved, families can pay a subsidized rate on select programs. To apply: Fee Waiver Applications are available at the Sunnyvale Community Center or online at Sunnyvale.ca.gov. Please allow 30 days for a response to your application. WAIVER OF LIABILITY AND PHOTO RELEASE: In consideration of participation in a class or activity offered by the City of Sunnyvale Department of Library and Community Services – Community Services Division, I, the undersigned for myself and/or as the parent/guardian of the Minor named above, agree to indemnify and hold the City of Sunnyvale harmless and hereby waive, release and discharge any and all claims for damage, for death, personal injury, bodily injury or property damage which I and/or the Minor may have or which hereinafter may accrue to me and/or the Minor against the City of Sunnyvale, its City Council, employees, agents, volunteers, independent contractors, and instructors from and against any liability arising out of or connected in any way with my and/or the Minor’s participation in this class or activity, even though that liability may arise out of negligence or carelessness on the part of the person or entities mentioned above. For camps (including Summer Express), refund requests must be received at least four weeks prior to the first day of camp. Transfer requests for camps must be received at least four weeks prior to the first day of camp. A $10 processing fee per transaction applies for all refunds. A $5 processing fee applies for all transfers. Refunds and transfers will not be issued for one and two-day classes, activities or camps. If the City cancels a class, activity or camp, you will be given the option to transfer or receive a full refund.
