patient and family advisory program Patient- and family- centred care in action at BC Children’s, Sunny Hill and BC Women’s Hospital
Partners in Care Patient and Family Advisory Program is a group of patients, families and healthcare professionals dedicated to promoting excellence in principles and practice of patient and family centred care. We provide human resources through patient and family volunteers, patient and family centred care education and guidance to strengthen collaboration of professionals and families at BC Children’s Hospital (BCCH), Sunny Hill Health Centre (SHHC), and BC Women’s Hospital Neonatal Program (BCW). We educate patients & families and healthcare providers about the patient and family centred care that happens at the bedside which enables patients and their families to work with healthcare providers as equal partners on the healthcare team. We encourage, educate and support the patient and family centred care that happens on the organizational level so that patients and family are embedded into the shaping of policies, programs, and facility design. The group works with and represents all families who use BCCH, SHHC and the BCW Neonatal Program to give them a voice in both patient care and organizational decision-making. All of this leads to better health outcomes, wiser allocation of resources, and greater patient and family satisfaction. We recognize that the patient and family flow through healthcare is one experience regardless of how many departments, organizations or agencies are involved. We encourage patient and family centred care into the broader community through partnerships with Provincial and local community support groups, organizations and agencies.
Patient & Family Centred Care Definition Patient- and family-centered care is an approach to the planning, delivery, and evaluation of health care that is grounded in mutually beneficial partnerships among health care providers, patients, and families. It redefines the relationships in health care. Patient- and family-centered practitioners recognize the vital role that families play in ensuring the health and well-being of infants, children, adolescents, and family members of all ages. They acknowledge that emotional, social, and developmental support are integral components of health care. They promote the health and well-being of individuals and families and restore dignity and control to them. Patient- and family-centered care is an approach to health care that shapes policies, programs, facility design, and staff day-to-day interactions. It leads to better health outcomes and wiser allocation of resources, and greater patient and family satisfaction. Institute for Patient- and Family- Centered Care
Patient & Family Centred Care Core Concepts Respect and dignity: Health care practitioners listen to and honor patient and family perspectives and choices. Patient and family knowledge, values, beliefs and cultural backgrounds are incorporated into the planning and delivery of care. Information Sharing: Health care practitioners communicate and share complete and unbiased information with patients and families in ways that are affirming and useful. Patients and families receive timely, complete, and accurate information in order to effectively participate in care and decision-making.
Participation: Patients and families are encouraged and supported in participating in care and decision-making at the level they choose.
Collaboration: Patients and families are also included on an institution-wide basis. Health care leaders collaborate with patients and families in policy and program development, implementation, and evaluation; in health care facility design; and in professional education, as well as in the delivery of care.
The Patient & Family Centred Care Journey System Centred Care Care, programs and polices are created and organized for the system and for those that work in it.
Family Focused Care Care, programs and policies are created by those who work in the system while taking patient and family needs into consideration. Things are done “to” and “for” them.
Patient & Family Centred Care Care, programs and policies are created in equal partnership with patients and families. Whether it is making a plan of care, building a new hospital, creating an educational resource, or developing a policy, things are done “with” patients and families from the very beginning.
BCCH/SHHC 2012/13 Strategic Action Plan PHSA Values, Mission & Strategic Directions 1. Improving quality outcomes and better value for patients. 2. Promoting healthier populations 3. Contributing to a sustainable health care system

www.bcchildrensnews.com FACT: PiC created a blog called The Family Centred Care Courier in 2010. In 2012 it was changed to Patient & Family News. The blog is a very effective tool in connecting us with patient and family volunteers. With site development construction the new blog is proving to be a very useful source of up to date information before coming to the campus.

PiC in the News
PIC In the News PiC Receives International Recognition – June 2012 Annual reports published by Patient & Family Advisory groups are intended to give organizational leaders and other interested people information about the collaborative endeavors and activities and to help them understand the Council's and Committee's future direction.
When the Institute for Patient & Family Centred Care put a call out for examples of Annual reports, The PiC Patient and Family Advisory for BC Children’s Hospital, Sunny Hill Health Centre and the BC Women’s Hospital Neonatal Program answered the call. Two examples, the 2006-2008
and 2008-2010 PiC Activities reports were chosen and posted to the website.
Patients & Families Partner on New Hospital Building a hospital in cardboard? Yes, one floor at a time. Global reporter Linda Aylesworth interviewsedLarry Gold, BC Children’s Hospital President, RN Anaylyn Perez and Physician Mason Bond. PiC Family Liaison, Susan Greig, a parent whose longest stay with her daughter at the hospital was 237 days, explained the value of patient and family involvement in Integrated Facility Design.
Samantha Sullivan, Chair for the BCCH PiC Committee and Susan Greig, Partners in Care Family Liaison were interviewed for a three minute CBC radio presentation by a BCIT Journalism student. They discussed reasons why a new hospital was necessary from a patient and family perspective.
“Planning the new ACC” This video chronicles the preliminary process & planning work . Heather Fowlie, PiC Senior Family Partner said in an interview, “It’s important (for patients & families) to have a voice early on as to what we think works and doesn’t work”.

Educational Resources and Partners in Pajamas
FACT: Before developing or revising any educational resource for patients and families staff should contact Learning and Development and PiC. The first step is to identify through Partners in Pajamas what kind of resource the target audience would find most helpful and useful. The new BCCH Surgical Virtual Tour (available in several languages) is an excellent showcase of the quality work created through this partnership.
Closing the Loop
FACT: One of the most important factors in the success of patient and family engagement is showing participants the results of their efforts and feedback.
Andrew Griffith, blogger at My Lymphona Journey commented on the parental input for the Cardiology Video.

BlAH, BLAH, BLAH….
BC Children’s Hospital is looking for patients and family members to join the patient & family advisory program and share their stories and to give us feedback on hospital policies, programs and the building of the new hospital. There are three ways to be involved: 1. Become a Patient or Family Partner: As one of our partners, you can tell us your ideas and share your experience by sitting on hospital committees, or being part of a working group . 2. Become a teacher: Share your patient or family story at staff education sessions, meetings or special events. 3. Become a Partner in Pajamas member: Give important feedback through email and social media on ideas to help improve the hospital; all in the comfort of your own home.
A LOOK AHEAD
