Dear FRG’s: We as DFMWR Entertainment/ Special Events would like to extend the invitation to you for your participation in this year’s Oktoberfest on October 15, 2010 from 5-11pm at Phantom Warrior Lanes Bowling. The participation requirements go as follows: Each FRG will be provided a booth space at event (approximately 10 ft. by 10 ft.) Groups may set up from 2:30-5 on October 15 All revenues from the day of the event will be the profit of your company In order to participate, the following items must be completed by 4pm on October 1 Please read carefully the preventive medicine general provision and supervision packets. Fill out a preventive medicine application and meet all of their requirements. Submit a memorandum, which cannot be an on letterhead and cannot be signed by the commander requesting a fundraiser at Oktoberfest. Along with a copy of all food handling certificates and good booth permit. Submit these to Debbie Wiles at Debbie.wiles.@us.army.mil Please return the attached form to Angela.Oakley@us.army.mil
