Dear FRG’s: We as DFMWR Entertainment/ Special Events would like to extend the invitation to you for your participation in this year’s Oktoberfest on October 15, 2010 from 5-11pm at Phantom Warrior Lanes Bowling. The participation requirements go as follows:  Each FRG will be provided a booth space at event (approximately 10 ft. by 10 ft.)  Groups may set up from 2:30-5 on October 15  All revenues from the day of the event will be the profit of your company In order to participate, the following items must be completed by 4pm on October 1  Please read carefully the preventive medicine general provision and supervision packets. Fill out a preventive medicine application and meet all of their requirements.  Submit a memorandum, which cannot be an on letterhead and cannot be signed by the commander requesting a fundraiser at Oktoberfest. Along with a copy of all food handling certificates and good booth permit. Submit these to Debbie Wiles at Debbie.wiles.@us.army.mil  Please return the attached form to Angela.Oakley@us.army.mil
