August 2012 bentley park resident newsletter Across My Desk CEO’s Report This month I thought that I would devote the majority of my article to the exciting new service delivery opportunity that we will be launching very soon to our village residents, namely SwanCare Group’s Community Care Service. This service has been many years in the planning and is seen as the perfect complement to our existing services of independent accommodation and residential care. Community Care sits nicely in between these two established services and effectively completes the cycle of our care delivery.

What is Community Aged Care?
Community Aged Care is essentially a planned and A SwanCare Group Service controlled delivery of specialty care services that is delivered directly into the individuals own home. The level and type of care is structured according to the specific needs of the individual, and a planned and coordinated care plan is tailored for each individuals own personal needs. Community Care ranges from low level care, similar to the type of care that one would receive if they were a resident in a low care nursing home like Waminda, through to high level care, which is aligned to that which would be received in a high care residential care facility such as Kingia or Tandara. The concept behind Community Aged Care is that it can in many cases enable an individual to remain in their own home for longer periods. It however, is not a replacement for Residential Care, and

there are certain individuals and particular circumstances where the individual would not be suited to the receipt of Community Care, and would be much better suited to receiving residentially based services within a nursing home environment. All of this is discussed with the individual, and the best option is determined at the time. From our perspective, we simply see Community Care as providing another option that we can offer to our residents and therefore increasing the level of comprehensive services we are able to provide. SwanCare Group has been awarded 20 Community Aged Care Packages as part of its initial allocation, and these packages are all for the provision of low level care into the home. At this stage of our growth we are not contemplating anything other than Low Level Community Care, however, as our experience grows and we become more comfortable with the delivery of community care into individuals own homes we will be seeking to expand the service to include high level care, as well as increasing the number of low care packages.
What services will initially be available ? An individual that is receiving one of our Community Care packages would likely be availing themselves of the following level of in-home support. This is by no means an exhaustive list, but merely indicative of the types and level of services that are all potentially available:Personal care, in the form of assistance and support with showering, dressing, laundry, medication management and other activities of daily personal living. • Social support. • Transport to medical appointments, shopping or social activities. • Home help, for example minor maintenance and gardening services. • Meal preparation, and, • General assistance with maintaining your health and well-being.

It is really important to know that these services are very flexible and can be changed as your specific needs change.
How can you access a Community Aged Care Package (CACP) ? To be eligible to receive a Community Aged Care package you must be assessed by an Aged Care Assessment Team, commonly referred to as an ACAT. Generally you are referred to an ACAT by your GP or allied nursing professional, and in our case with village residents, our Resident Support Services (RSS) Team will often facilitate this referral process in conjunction with your GP. The bottom line however is that you cannot receive a CACP until you have been assessed as being eligible for one.
Who will provide your Community Care ? We will be utilising our own fully trained and competent qualified nursing staff to deliver this in home care. Our staff are supported with education and training on an ongoing basis, so you can be assured that the delivery of your in-home care will be of the best quality and level of service that we are able to provide. Community Care comes with the same level of stringent quality and accreditation control as exist with the delivery of services in a residential nursing home setting. Thus the level of care you receive in your own home must, at all times, meet and maintain specific quality levels.
How is the Community Aged Care Package arranged ? Once you have been assessed and have been allocated with a package, you will receive a visit from your allocated care coordinator who will sit down with you and develop a care plan that is specific to your needs and requirements. CEO’s Report frequency of visits, the costs and the overall objectives that will form part of your specific in-home community care plan. Essentially the care plan becomes a binding service agreement between you and your care coordinator.
How is the program funded ? Community Aged Care packages are funded in two ways. The bulk of the funding comes from the Commonwealth Government, who provide a certain daily amount to an approved provider to assist them with operating a Community Aged Care package. The second source of funding comes from the recipient of the package who will be asked to pay a weekly fee as a contribution towards their cost of care. This weekly fee is tied to the standard pension, but does vary according to the individuals specific financial situation. For instance a person on the maximum basic pension will be asked to contribute an amount that does not exceed 17.5% of that pension. If you have a higher income you may be asked to pay additional fees.
What is the next step ? We will be looking to formally roll the service delivery out by the 1st October, 2012. As an indication of the strong demand and need for this service we have already received expressions of interest from a handful of village residents who are in need of a CACP. As soon as we are given the final green light from the Department of Health and Ageing, we will likely commence delivery of the services to these early starters before the 1st October official start date. We are fully expecting that the 20 initial packages will be taken up in a relatively quick time by our village residents. The Community Aged Care service will be coordinated jointly by Julie Atkinson, our General Manager of Aged Care and Natalie Hodi our General Manager of Retirement Living. Both of these ladies will be working very closely with our Resident Support Services (RSS) team and general nursing staff. With respect to the RSS team they, as always, are roaming the village campus with the objective of looking after residents welfare and in the process of doing this are actively seeking out those residents who may be in need of these additional care services. If you feel that you are in need of additional care, please let one of our Resident Support Service personnel know, and they can take up the matter from there. Remember, this level of care delivered into your home is to help you maintain your independent lifestyle and is designed to enable you to continue with your normal activities of daily living. If your health is preventing you from carrying out normal daily activities then you may be in need of care assistance, if you fall into this
category it is worth having a confidential, no obligation chat with one of our Resident Support Services staff. To round off this month’s article I would like to offer the following snippets of information that have come across my desk.
Village Maintenance budget for 2012/13 This year’s budget has identified a substantial increase in funding allocated to the maintenance of our village assets. Our financial performance has improved over the last couple of years such that we are now in a position to allocate an appropriate level of annual funding towards the establishment of a scheduled maintenance program for our village assets. Our newly appointed Facilities Manager Mick Harrison, together with our GM of Retirement Living, Natalie Hodi are in the final throws of pulling together this year’s schedule of routine maintenance work. Once this plan is completed we will have great pleasure in circulating it to our residents so that you will be aware of where, (and when) the intended works are to be undertaken. The allocation of funds, some $500K, will be spread across the site and will be primarily put towards remedial painting, roadwork, roofing repairs, and internal refurbishments of major community areas.
We are on the lookout once again for more SwanMobile drivers Yet again we seem to have experienced a fall-off in the numbers of our volunteer SwanMobile drivers. If you would like to consider volunteering for this service we will be more than happy to speak with you. This is a vital village service and I can honestly say from personal experience that driving the buggy is very pleasurable and rewarding. If you feel that you can contribute your time to helping out then please contact Karen Parker on 6250 0010 who will be more than happy to arrange for your training and can answer any of your concerns or queries.
Australind Rise relaunch and re-energise The South West real estate market has been experiencing a very difficult time over the last couple of years, and it is no surprise that we have been finding it challenging to sell our units in this area. The Australind Rise village is now looking superb, and the soon to be completed Social Club and Community Centre will add another new dimension to this already outstanding development. To coincide with the completion of the community centre we are planning a new launch of the Australind Rise development. It is our intention to map out a new and locally focused advertising and marketing
campaign that we feel will re-energise and bring life back into the messaging and promotion of this wonderful village. Working in conjunction with Cooch Marketing Consultants, our Australind Rise Manager, Margaret Robinson and our GM of Retirement Living Natalie Hodi will form a formidable team to drive this rejuvenation program. The initial concept plans that I have seen for this promotion give me a strong feeling of optimism that it won’t be too long before we see a renewed level of sales activity flowing through at Australind.
Another SwanCare stalwart says farewell The poorly kept rumour is now official, Maxene McLean, our Resident Maxene McLean Support Services Coordinator, has finally decided to hang up the boots and bring to a close her wonderful career with SwanCare Group. Maxene will be finishing up in August, and is looking forward to enjoying her own retirement lifestyle in her new villa in the newly constructed Settlers Hills Retirement Estate in Baldivis. Maxene, throughout her 17 year career with us has been an exceptional tower of strength and support to many thousands of residents. She has always, without hesitation, given so willingly, she displays a rare level of compassion and caring that has endeared her to so many of our residents. Never one to shy away from making the tough decisions, Maxene always displays a true even handedness and applies balanced ethical standards in dealing with what are quite often very difficult and emotional situations. To say that we will miss Maxene is a true understatement, but she can leave us with the firm knowledge that she has left behind a wonderfully trained team that are ready and willing to continue carrying on her great work. We cannot thank Maxene enough, and we all truly wish her a wonderfully relaxing retirement. She will forever remain part of the SwanCare Group family. And finally for this month … It’s that time of the year again when we ask our village residents to have their say about how they feel about living in our community and to seek their feedback on the level of services and support that we provide. The Schedule for this year’s Show has now been distributed. A big thank you to the tidings team who not only delivered them but assisted with collating and folding - your assistance was greatly appreciated. All Village residents and staff are invited to enter exhibits, including those at our sister villages at Australind and Carlisle. Most residents should be able to find a section or three to enter and be in the running for the new Bentley Park Committee Trophy.
Almost daily we hear of break and entries. The television shows us vicious attacks made on elderly people because we are easy targets, although our Security Guards do their best our village covers a wide area, so we need to help ourselves, by keeping doors and windows locked at all times, and looking after our neighbours and reporting to Security anything that seems suspicious.
The “Ladies of the Night” will be demonstrating Quilting and Patchwork again this year – a great opportunity to talk to them and see how it is done, which could well give you inspiration to start a new hobby. We also have a demonstrator of Spinning and Weaving – a great pastime which has the added benefit of giving you very soft hands from the lanolin in the sheep’s fleece and much satisfaction in creating an item which is completely your own creation from start to finish. Kingia, Tandara and Waminda are once again making art/craft displays, in addition to our Village Archives and the Uthando Doll Project. This year Natalie Hodi will be on hand both days between 10.00am and 2.00pm to show any prospective village residents around the New Villas, so this is an excellent time to invite any friends or family who may be in the market for entering a Retirement Village to come and have a look. Once again a reminder of the dates of the Show – Saturday 27th and Sunday 28th October. There is still plenty of time to get your exhibits ready, fertilize/repot/ prune your potted plants, maybe even plant one or 2 vegetables to mature in time, and these colder days are excellent for staying indoors or finding a sunny corner outdoors to work on your knitting or craft project. The residents and staff of Waminda took part in Australia’s Biggest Morning Tea. They managed to raise $531. They couldn’t have raised such a fantastic amount without the help (as always) of Rita who worked so hard with her fabulous raffle prizes!

The last Sunday Concert was a bit different to those previously held this year and allowed those present to meet, and listen to, a combination of an old hand in the music world who provided some jokes and nostalgic music and his young niece who has a wonderful soprano voice used to advantage in musical comedy and some Italian dramatic solos. She is currently a student at WAPA - no doubt set for big things in the future. The afternoon had a different format as the

Bentley Park Diners Our outing for August 2012 is on Tuesday the 14th & 21st. It will be a little different, I am trying something new. One bus will go to the Ten Ten Chinese Restaurant Karawara, and the other bus to Nero’s Italian Restaurant Gosnells. The bus to Nero’s will depart the Village at 5:15pm and the fare is $6.00. The bus to Ten Ten will depart at 5:30pm, the bus fare is $5.00. You don’t need a partner as there will be a bus full of friendly Villagers to welcome you. Residents Advisory Council - Brian Dale
Each meeting follows a general pattern which includes enquiries and discussion of current Village issues and provides a forum where any resident can express an opinion without recrimination – minutes are NOT taken, only general notes which are then issued as part of the flyer advertising the next meeting. These notes never record names. Interesting Guest Speakers from both outside and inside the Village, including the Chief Executive Officer, keep us informed and up-to-date on a variety of topics. (As a recent example, guest speaker Bob Ziegler from COTA gave a simple piece of advice which could save your life. ALWAYS carry a list of your medications on you; it is the FIRST thing looked for if you require an ambulance). Why am I mentioning this? Simply to say that the 90% of residents who DON’T attend may be simply unaware of what they are missing. (And it is a commitment of only about 90 minutes PER MONTH). For our July outing we travelled to Guildford to see “Deathtrap”. I’m sure all enjoyed the show by all reports. August takes us to Kalamunda to see “Private Lives”. All tickets have been sold. Occasionally, we have cancellations, if you are interested let me know and I will place your name on the waiting list. Next to the Garrick Theatre in Guildford to see “Night Was Our Friend” a drama directed by Fred Peterson, on Friday 14th September. Tickets $21.00, includes sherry/port on arrival, tea & coffee at interval. Bus pick up and return. Sally Raynors’ husband Michael is presumed dead after he went missing in the jungles of South America. Nothing has been heard from him in two years. Amazingly he survives and returns home to Sally who has moved on with her life. She is confronted with his strange behaviour. With support of her family doctor and the new man in her life she tries to come to terms with her tormented husband. We had a busload for our visit to “Wheelchairs for Kids” and were very impressed by the dedicated work put in by the volunteers. Their work gives quality of life to many disadvantaged children all over the world. On the way home we called in at Sizzler’s for lunch which was enjoyed by all.
