Holistic Care Going beyond the physical At SwanCare, aged care goes beyond the physical. Often when someone moves to a residential care facility they are no longer able to continue many of their past spiritual practices such as attending church services and participating in church community events and celebrations. SwanCare supports our residents through the following services:

Pastoral care The goal of our pastoral care program is to provide for residents of all faiths. Pastoral support can be arranged at the request of the resident or family.

Chapel services Throughout the month, chapel services and fellowship gatherings are held for our residents. In all our residential care facilities, it is recognised that residents come from many different religious traditions. Hence, services are designed to support and appeal to residents of all faiths.

Visitation Chaplains visit regularly with residents, especially during times of crisis, illness, death and dying, or other special needs. Residents and families are asked to request visits if desired.

Community support Regular visits from volunteers who provide companionship and practical assistance are all part of the residential care way of life. This service is particularly important to those residents who find themselves immobile. The kind donation of time given to our residents by this dedicated group positively contributes to the wellbeing of those in care.

Respite Services Short-term care Respite care enables carers to take a break from the daily demands of care-giving, and provides an individual with the chance to experience residential aged care for a short period. Award Winning SwanCare was the winner of a recent National Aged Care Award for Innovation in Service or Design, awarded by Aged & Community Services Australia. The award reflects two joint programs that work to support future doctors and nurses: the Curtin Medical School program and the University of Notre Dame Australia’s Graduate Nurse Program.

SwanCare Innovations Leading the way SwanCare has a strong reputation for delivering a high standard of care and it’s also leading the way in reshaping aged care through innovation and collaboration.

Staff care & recognition

Curtin Medical School Program SwanCare has partnered with Curtin Medical School for the Senior Citizen Partnership Program – an initiative that sees students pair up with members of the SwanCare community to form long-lasting bonds.

Not only is SwanCare a great place to live, it’s also a great place to work. SwanCare invests in its staff with various programs including the award-winning Feathers Employee Recognition Program and the annual Letters Week campaign.

This enables students to improve their knowledge of elderly peoples’ experiences, witness the benefits of social support networks and form a better understanding of the medical system from a different perspective.

SwanCare was a silver medal recipient in the Human Resources Magazine’s Employer of Choice Award for 2019, as voted by staff across Australia.

This program starts the first year of their medical training and continues through with the same resident for the full five years. The residents are invited to the student’s graduation.

Interactive I.T. Technology is also integrated into the fabric of SwanCare. There’s an interactive feature wall for residents with dementia, virtual reality headsets for residents to visit faraway places, assisted Skype sessions and much more – with an onsite IT helpdesk, assistance is just a phone call away.

SwanCare also partners with the University of Notre Dame Australia for a post-graduate nursing program.
