Swansea’s Exercise Referral Programme What is the National Exercise Referral Scheme? The scheme works in partnership with health professionals in Swansea that include local GPs, practice nurses, dieticians, physiotherapists, diabetic nurses, psychologists and occupational therapists among others. The scheme is aimed at people who: L Are aged 16 years and above L Are currently inactive L Are overweight or clinically obese L Have specific health conditions The referral instructors: L Offer one to one assessments L Are highly qualified professional staff L Provide community based sessions L Give advice and guidance on increasing participation in physical activity The referral scheme lasts for a minimum of 4 weeks up to maximum of 16 weeks and is delivered across our Active Swansea Leisure Centres and a variety of community buildings throughout Swansea.

National Exercise Referral Scheme
How do I join the scheme? Firstly, you must be referred by a health professional. Members of the exercise referral team will then assess each individual and establish whether you are suitable for this particular scheme. What does the programme consist of? Participants who are referred onto the scheme will get a chance to take part in a range of activities which include chair based sessions, fitness classes, Nordic walking and gym sessions. These classes last up to an hour (approximately) and are delivered in small groups of people. The long term aim of this programme is to ensure you continue to be physically active for the rest of your life. How much will it cost? Swansea’s Exercise Referral Programme Cardiac Rehabilitation What is the Cardiac Rehabilitation Scheme? The Cardiac Rehabilitation Scheme is aimed at people who have experienced a cardiac event within the last 6 months or those who have been referred from the cardiac team in Morriston or Singleton Hospital. Phase 4 of the Cardiac Rehabilitation Scheme provides participants with specific exercise classes, gym sessions and/or walks as well as general advice on health and well-being. All sessions are run from a range of community buildings across Swansea. How do I join the scheme? You must be referred by the cardiac team either in Morriston or Singleton Hospital.

What does the programme consist of? A tailored exercise programme based on your medical condition and general health and fitness will be put together, which builds on the work completed in Phase 3. The scheme lasts a minimum of 4 weeks up to a maximum of 12 weeks. After those 12 weeks you can go on to the National Exercise Referral Scheme to continue the programme for a further 16 weeks. The long term aim of this programme is to ensure you continue to be physically active for the rest of your life. How much will it cost? Swansea’s Exercise Referral Programme What is the Falls Prevention Scheme? The Falls Prevention Scheme is aimed at those who have been referred because of: L L L L
a fall difficulty with balance and general mobility a risk of falling or anxiety about falling
What does the programme consist of? A tailored exercise programme based on the individual’s medical condition and general health and fitness will be put together. The scheme lasts for 12 weeks and aims to assist individuals through a dedicated programme of activities, mainly chair based, to assist those who have fallen and to prevent any further falls. After those 12 weeks participants will then have the opportunity to take part in an exit class. The exit class will build on skills and practices developed in the previous 12 weeks and will help the individual improve their functional ability and quality of life.
