How it works Someone - it could be you - has some concerns or worries about a child, young person or family.
The Family or Young Person is contacted and these concerns are discussed with them. Together we look at ways to help.
A small number of people who we think can really help are asked to come to a TAF meeting. At the meeting we work together to plan the best way to help. ONE person will make sure the plan is followed.
This way of doing things means ■
Listening to you, talking to you and working with you to see what you need.

Sharing information, with your permission of course, to save you having to repeat yourself to a lot of different people.
■
All of this should result in the right help being offered at the right time to make your life a happier one.

What is a Team Around the Family (TAF) meeting? ■
It is a small meeting between you and people who really want to help you.
■
It can only be called with your permission.
■
You will be listened to - be assured.
■
We will work with you to get the help you need.
■
You will be involved in decisions made about how help will be provided.
■
A person is chosen by everyone at the meeting who will take the lead and work very closely with you.

Finding out what help is needed The My World Assessment is a form used to look with you at all the important areas in a Child or Young Person’s life - Health, Education, Relationships, and Behaviour are all considered. Issues which affect parenting such as ill health, substance misuse, domestic abuse, housing and financial difficulties are also considered.
Information sharing Information is usually only shared with the permission of the Young Person or Parents except where there may be a Child or Young Person at risk of harm. We know just how difficult it is to ask for help when you need it:
