milies Children, Young People, and their Fa r them sometimes need a little extra help fo l lives. to have happy, healthy and successfu r advice, The Team Around the Family can offe the help and support in pulling together right people to help.

How it works Someone - it could be you - has some concerns or worries about a child, young person or family.

The Family or Young Person is contacted and these concerns are discussed with them. Together we look at ways to help.

A small number of people who we think can really help are asked to come to a TAF meeting. At the meeting we work together to plan the best way to help. ONE person will make sure the plan is followed.

This way of doing things means ■

Listening to you, talking to you and working with you to see what you need.

Sharing information, with your permission of course, to save you having to repeat yourself to a lot of different people.

All of this should result in the right help being offered at the right time to make your life a happier one.
