Welcome Welcome to the inaugural issue of the Montana Mature Living Guide. First, let me take this opportunity to thank the many people who supported our idea to create this guide. They offered a multitude of helpful suggestions and in some cases provided much needed content. This publication was inspired by the personal experiences of several staff members who needed to move their aging parents into a different living situation – a process that left them feeling confused and overwhelmed by an entire new world of industry jargon and acronyms. I am one of those people, and although I know much more now than I did when I entered this arena three years ago, I’m continually discovering enormous amounts of information pertaining to a wide variety of individual situations. From our research into other regional senior living guides, we developed a concept we thought would best serve Montanans. In this first issue, we printed the name, address and phone number of each state-licensed housing option by city. Note that advertiser listings also include website, ad page number, and the directory page number. You’ll find definitions of various categories of service, veterans’ and Medicare benefits, a guide on how to determine when a senior should no longer live on his or her own, and how to get started on the whole process. Our companion website www.montanamatureliving.com contains all of the content you see in this guide plus additional details and links to helpful sites. We are especially pleased to present VIRTUAL TOURS from participating communities. I can’t imagine how great it would have been to visit just one website and see photos of various properties before driving my mom around to see them. How to Know
If It’s Time to Make a Change in Your Parents’ Housing Situation

The decision to move your aging parents out of the family home is a complex one -- both emotionally and practically. It requires a delicate balancing act between your parents’ safety and their emotional stake in staying put. Each of these is important, and helping your parents make the right decision (while remembering that as long as they are of sound mind, it’s ultimately their decision) requires care and planning.

Key questions to ask Each family is different, and the decision to move is an intensely personal one. Asking yourself and your parents the following questions can help all of you navigate this difficult terrain. •H ave there been any accidents recently or close calls? Who responded, and how long did it take? •A re activities of daily living getting harder? If the answer is yes, are you able to get in-home help for your parents with chores like shopping, cooking, or laundry? •A re your parents becoming socially isolated? Lack of companionship can leave elderly people more vulnerable to heart problems and other health conditions. If your parents no longer see friends or visit with neighbors, moving to a place where they would be around other people could actually be a lifesaver. • Is the house clean and well cared for, and are basic home-maintenance tasks getting taken care of? If not, are your parents open to getting more inhome help, can you or they afford it, and do you know how to find it?

•C an someone check in on your parents on a regular basis? If a family member, friend, or neighbor isn’t nearby and available to do this, are your parents willing to consider a home-safety alarm system or daily calling service?

•W hat’s the plan for a worst-case scenario? If there’s a fire, earthquake, flood, or other disaster, is someone nearby prepared to assist your parents? • I s your parent clean and well-groomed? If your father has always been known for his crisply ironed shirts but starts looking disheveled, that may be a clue it’s time for another level of support. •W hat’s in the refrigerator? Is the freezer full of TV dinners and the vegetable drawer empty? Has the milk gone sour? A quick look can tell you whether your parents are eating well or whether they’ll do better someplace where trained staff could make sure they’re getting balanced meals. •H ow are the pets doing? What about the plants? Your parents’ ability to take care of other living things may offer clues to their ability to manage their own care.

What do your parents want? This may be the most important question of all - and you may be surprised by the answer. How to Know
•H ow did your parents weather their most recent illness (for example, a flu or bad cold)? Is your mom able and willing to seek medical care when needed, or did last winter’s cold develop into untreated bronchitis? •W hat does the doctor think? With your parents’ permission, talk to their doctor. The doctor may share your concerns about your parents’ safety at home but may also be able to alleviate them. Sometimes your closeness to the issue can exaggerate your worries, and a little professional distance (and expertise) is just what’s needed to clarify the picture. •H ow often do your parents get out - especially in the winter? Are they spending days without leaving the house because they can no longer drive or are afraid to take the bus alone? While many elders fear being “locked away” in a retirement home, many such facilities offer regular outings that may actually keep your parents more mobile and active, not less. •H ow are your parents doing compared with this time last year? The holidays can be a good time to reflect on the previous year and take note of any significant changes. A marked decline from one year to the next may mean it’s time to start looking - and planning - for a more supportive environment. •H ow are you doing? While this decision is not primarily about you, your own exhaustion can be a good gauge of a decline in your parents’ ability to care for themselves. If your parents’ need for care is cutting into your ability to spend time with your own family, interfering with your job, or just plain wearing you out, that may be a sign that it’s time to start looking at other options. • How old is your parent? Many continuing care facilities have age ceilings after which your parents can’t get in, no matter how healthy they are, so if you have your eye on a particular

place, find out what their age cutoff is and plan accordingly. •A re your parents happy? Safety is crucial, of course, but so is your parents’ emotional well-being. If they’re riddled with anxieties or increasingly lonely, then that may tip the scales toward a move that may not be 100 percent necessary at this point for health and safety reasons. On the other hand, if your parents have a full life, close neighborhood and community connections, or simply enjoy being at home, it’s worth exhausting every option before pushing them to move out of the home they love. •H ow do others think your parent is faring? Sometimes it helps to get a second opinion, either from a family friend or relative or from a professional geriatric care manager who visits your parent’s home and does an informal evaluation. While your parents may initially resist the notion of a “total stranger” checking them out, this one may be worth insisting on (offer to pay for it as a holiday gift). You may be surprised to find your parent is willing to share doubts or vulnerabilities with a sympathetic, experienced stranger that he’s loathe to admit to his own children, easing the family conversations that follow. •W hat do your parents want? This may be the most important question of all - and you may be surprised by the answer. While an initial response may be a knee-jerk “I’ll cross that bridge when I come to it,” many older people harbor the same fears for their current and future safety and security that their children do, even if pride keeps them from voicing them. Take the time to sit down with your parents, draw out their concerns, and find out what they fear most about moving out and what they want to change about their lives. Rather than launching into your worries for them, or what you think they ought to do - may give you and your parents all the information needed to make the right decision for the whole family. Types of Care
An Introduction to Different Care Facilities
Adult Day Care Center (ADC) – a facility, freestanding or connected to another health care facility, that provides adults, on a regularly scheduled basis, with the care necessary to meet the needs of daily living but that does not provide overnight care. Adult Foster Care Home (AFH) – a private home or other facility that offers only light personal care or custodial care to four or fewer disabled adults or aged persons who are not related to the owner or manager of the home by blood, marriage, or adoption or who are not under the full guardianship of the owner or manager.

Rehabilitation Facility – a facility that is operated for the primary purpose of assisting in the rehabilitation of disabled individuals by providing comprehensive medical evaluations and services, psychological and social services, or vocational evaluation and training or any combination of these services and in which the major portion of the services is furnished within the facility. Home Health Agency – a public agency or private organization or subdivision of the agency or organization that is engaged in providing home health services to individuals in the places where they live. Home health services must include the services of a licensed registered nurse and at least one other therapeutic service and may include additional support services.

Retirement Home (RH) – a building or buildings in which separate living accommodations are rented or leased to individuals who use those accommodations as their primary residence. Residential Care Facility – an adult day-care center, an adult foster care home, an assisted living facility or a retirement home. Assisted Living Facility (ALF) – a congregate residential setting that provides or coordinates personal care, 24-hour supervision and assistance, both scheduled and unscheduled, and activities and healthrelated services.
Long Term Care Facility (LTC) – a facility or part of a facility that provides skilled nursing care, residential care, intermediate nursing care, or intermediate developmental disability care to a total of two or more individuals or that provides personal care. Skilled Nursing Care – the provision of nursing care services, health-related services, and social services under the supervision of a licensed registered nurse on a 24-hour basis. Hospice – a coordinated program of home and inpatient health care that provides or coordinates palliative and supportive care to meet the needs of a terminally ill patient and the patient’s family arising out of physical, psychological, spiritual, social and economic stresses experienced during the final stages of illness and dying and that includes formal bereavement programs as an essential component.

How to Get Started Navigating through the maze of senior housing choices can be overwhelming. Most people experience a big learning curve as they begin the search process. In many cases, people have to work through long-standing misperceptions about senior living communities. Overall, gathering information on options before a crisis situation occurs is extremely helpful as people prepare for these transitions. Independent Living Communities are great options for people who want to live on their own but free themselves from the responsibilities of home ownership. Typical services include meal plans, housekeeping, maintenance, transportation and social/wellness activities. Payment options can be rental or entrance fee-based; the latter allowing for lower monthly service charges. Some places also offer a health care plan that can help cover the cost of future care needs. These communities can often help people remain independent longer and with a higher quality of life than if they had continued to live in their own homes. Assisted Living Communities are licensed and regulated by the state and provide help with everyday living tasks (e.g. bathing, dressing, appointment reminders) and often some health care needs (e.g. medication management) to residents
living in private apartment homes. Other typical services include 24 hour staffing, all daily meals, laundry service, housekeeping, transportation, and social/wellness activities. Memory Care Communities are licensed by the state and specialize in caring for people with Alzheimer’s or other forms of dementia. Around-the-clock staff provides a secure and more intimate living environment for the resident’s safety and family’s peace of mind. Skilled Nursing Communities (or nursing homes) offer 24 hour care for people who are too frail or unsafe to live alone. They are licensed by the state to provide nursing and personal care (e.g. feeding, toileting, bathing, dressing) as well as medical services. Staff must be licensed nurses or certified nursing assistants. Care is supervised by a medical director.

Make every effort to visit several communities. Each has a feel of its own. To make the best use of time, make an appointment beforehand, if possible. Check out the community’s website before you visit. Limit the number of appointments each day to avoid becoming overwhelmed. Look for transparency in pricing – ask if there are any extra charges. Ask whether other levels of care are offered if a person’s needs change.

Are the residents happy and do they greet you? Is the staff friendly? Are the residents engaged in activities that are purposeful and meaningful to them? Is there a posted activity calendar? Are the rooms shared or private? Are the bathrooms shared or private? Is the call system accessible and usable for you? Are you allowed to furnish your own room? Do the units have TV and telephone hookups? Is there a posted menu? Does the facility prepare food for special diets? Are guests allowed to stay for dinner? Is there adequate staffing? Are there any religious clauses in the resident agreement or house rules? Are there any odors when you walk through the door? Are smoking and pets permitted? Are resident rights promoted? Is the facility insured?
Sight: Are the surroundings homelike? Does the atmosphere invite visitors? What color are the walls? Are they bright and cheerful or dull and drab? Are there activities? Are the call lights answered in a timely fashion?
Smell: Is there a strong urine odor or disinfectant odor? Is the odor offensive or unpleasant? Do residents smell? Taste: Is the food good? Is it cooked completely through? Is hot food hot and cold food cold? Is the water fresh? Feel: Is the building too hot or too cold? Is the atmosphere warm and inviting?

Medicaid
How Does Skilled Nuring Care Get Paid For? By Karen Powers, The Goodman Group
Until it becomes a factor in our lives, most of us do not know how skilled nursing care is paid for. The first hours during admission to a skilled nursing facility are full of information. This is the time when financial responsibility will be addressed with several options presented by the admissions coordinator. For a short-term rehabilitation stay, using a Medicare benefit and a Medicare supplemental insurance might be all that is needed. When skilled nursing care is required for a long term living situation the options become private pay, long-term care insurance or Medicaid benefit. w w w. m o n ta n a m atureliving.com
Questions we all have: ow much of my nursing care is paid for by Medicare? H With a qualifying hospital stay and qualifying diagnosis, your first 20 days will be 100% paid for. Thereafter, Medicare will pay 80% and the resident is responsible for a co-pay of $141.50 per day (2011 rate). How long does my Medicare benefit last? Your Medicare benefit can last up to 100 days as long as you are showing improvement with treatment and rehabilitation. What is Private Pay? Private pay is using personal funds to pay for staying in a skilled nursing facility. When Medicare benefits are exhausted and a person continues to need care, the monthly expense is billed directly to the resident. Can Medicaid help me with my nursing home bills? Medicaid can be a resource for paying for long term care. However, there are age and income requirements with variables involving marital status and financial assets. What if I still have a spouse at home? Medicaid has guidelines that take into account a spouse still living at home and relying on the family income How do I apply for Medicaid? If it seems Medicaid services might be an option, you will be directed to the social services representative of the skilled nursing facility. This person is very knowledgeable about the Medicaid benefits, how to qualify and how to apply. They have the applications at hand and can walk you through the process. Long Term Living
in a Nursing Home

A Hard Decision
At what point in our lives do we need to transition to a skilled nursing home? Well, mostly it comes down to health and physical needs, when one or more personal care issues have become facts of life AND are beyond what can be managed at home or past the extent that assisted living services can cover. • Multiple health conditions, symptoms and treatments that need ongoing monitoring by nursing, therapists and physicians. • Help with tasks throughout the day: meal preparation and eating, medications, social activities and transportation. • Personal assistance with bathing, dressing and incontinence care. • Special diet needs, food consistencies or assistance eating.

•M oderate to extensive memory loss that results in difficult behavior, wandering and loss of communication. • Limited physical mobility, standby assistance to help walk or maneuver a wheelchair, the inability to sit for any length of time, resulting in resting in bed for many hours of the day.
These are physical needs and concerns. What about the quality of life? What will it be like? For any of us, significant keys to quality of life are a balance of personal happiness, a sense of self worth and feeling relevant. Nursing homes need to extend beyond their medical facility identity and be cognizant that residents and families have turned over the trust for care and well-being as well as of quality of life.

•P ersonalized service is paramount; residents are individual, so make sure the staff takes the time to get to know their desires, concerns and wishes for their personal care. • Residents have choices and input as to when and what they would like to eat, rest, have company and be as active as they want. • Nursing home staff should be enhancing independence, not just doing for the residents. Dressing, bathing and grooming assistance enhances self-esteem. • While physical, cognitive and health conditions can affect a person’s energy level, and ability to communicate or participate, the activity programs should be focused on bringing life to their doorstep and adjusted to their ability to join in.
•A n activity program provides residents opportunities to enjoy music, continue a spiritual life, contribute to the community by volunteering, social opportunities, fitness, readings, dancing, games, coffee socials, baking clubs, craft groups, shopping, movies, performances, and other entertainment. Activity programs also honor and assist with residents’ desires for solitary pursuits like reading, one to one conversations and visitors. • Common spaces create many comfortable areas inside and outside so residents feel they are living in an entire community, not just a room.

Being the sole caregiver for a person with memory loss is a challenge from the first diagnosis. As the memory loss progresses, behaviors start to develop that are very hard to cope with: the increased confusion that that can result in anger or aggression; unpredictable behavior; the need for constant supervision to prevent wandering; loss of continence and more. Trying to handle any of these care issues on your own can be exhausting and impossible when you add them all together. Many spouses and children begin to recognize their own fatigue and frustrations and realize that this is not good for them or the person with memory loss. Where to turn? Memory Care is the term used by senior care providers to define care services and programs for those suffering from memory loss due to stroke, Alzheimer’s, Parkinson’s or other diagnosed dementia. Many years and much thought has been put into finding the best way to provide for the special needs of a person with memory loss. Doctors, researchers, organizations like the Alzheimer’s

Association and many senior housing companies are dedicated to developing resources and providing services for the best possible care. The benefits of the programs are many: • Secure wings, wander guard systems and enclosed courtyards Residents with memory loss can be restless and need room to wander safely. • Multiple caregivers to be of assistance around the clock. As the sole spouse or child knows- help is good thing and a person with memory loss can have nighttime issues as well. • Nursing staff to handle medication issues and secondary medical conditions Residents often cannot express how they feel or what they need and it takes a trained person to interpret and monitors those needs. • Caregivers that are chosen for their patience Trained to handle the challenging behaviors and interpret the needs of residents who cannot ask for help.

• Meal programs designed to encourage eating and adapt to a residents needs Beyond three meals and snacks, we can help them eat and find ways to make sure they are getting the best nutrition. • Activities that are stimulating and designed to reach the memory loss resident Quality of life can be found through social interaction, mental stimulation, music therapy and feeling purposeful.
When the personal care of a family member with memory loss becomes too much of a challenge, transitioning to a secure memory care community, whether in a nursing home or assisted living facility, can be a viable choice for your family.

Veterans Benefits Attention Veterans or the Surviving Spouse of a Veteran: You may qualify for assistance to help cover the costs of assisted living. If you are a United States veteran or surviving spouse of a veteran, you may qualify for the Aid & Attendance Program and receive monthly benefits to help cover the costs of assisted living care.
What is Aid & Attendance?
Aid & Attendance is a Federal benefit that was added onto a need-based pension offered through The Department of Veteran Affairs. It provides benefits for veterans and surviving spouses who need assistance with their activities of daily living and meet specific requirements. Care provided in an assisted living facility qualifies.

In order to protect our families from having to make difficult decisions for us, everyone over the age of 18 should have an advance directive for healthcare and should discuss their wishes with their family, friends and healthcare providers. There are two types of advance directives. The first type is a living will, a legal document that provides instructions regarding the kind of healthcare interventions a person would or would not want if they were too ill to speak for themselves. Living wills are used to determine healthcare treatment decisions if a person has a severe illness or injury and is unable to communicate their wishes.
The second type of advance directive is called a durable power of attorney for healthcare, also called a medical power of attorney. A durable power of attorney for healthcare is a legal document in which you designate someone to make your healthcare decisions if you are unable to make or communicate your own decisions due to severe injury or illness.
Discussing end-of-life issues is not an easy thing to do. Often the best approach is to be open and honest and to be respectful of another person’s wishes. By documenting your wishes for healthcare treatment ahead of time and discussing these wishes with your families, friends, and healthcare providers, you will help more than just yourself during a potentially difficult time.
Here are just a few topics that should be addressed in a living will. Discuss them also with the person you designate as your durable power of attorney for healthcare. • Directions for providing CPR if your heart stopped.

Living Well at the End of Life
By Cathy Scribner, MDiv Chaplain, Hospice of Missoula The most common thing I hear from families of patients who have chosen hospice care is, “I sure wish we’d learned about hospice earlier!” Though many patients with terminal diagnoses are informed about hospice through their health care providers, there are just as many patients, families, and health care providers who still perceive hospice as “the last stop” before their number is called. I have learned from my work that nothing is farther from the truth. Through my own work as a hospice chaplain, my perceptions of hospice care have changed dramatically. What I now perceive as “dying well” has merged with my understanding of “living well”. In our western culture, we tend to stave off an awareness of our death and dying process for as long as possible. We push our mortality behind closed doors, where aging and dying happen to “other people”. Every day, we are bombarded by internet, television and other media images that portray the importance of staying young, beautiful, and physically fit. “Or else…” is what these images seem to imply. Because illness and death are perceived as a sort of failure in our culture, we tend to be surprised and horrified when disease enters our lives with its foreboding brushstrokes and dark colors. We want to grab our paintbrushes and palates and repaint our canvases with images that are acceptable to us - images full of bright color and hope. We tend to forget that every color exists in relation to another color, whether dark or light in hue, and the more colors we ultimately include on a canvas, the greater the depth and quality of the painting. w ww ww. w.m mo on nta tan na am matureliving.com atureliving.com
I think it is the same with our lives. The more we embrace the “dark colors” of our fears and feelings of vulnerability, the richer our experiences ultimately become. When we are able to accept dying as an essential part of our own living process, we are better able to “companion” loved ones who are dying, with the love, compassion and support we all deserve to keep on “living well” until the end. People often feel alone and afraid after receiving a “terminal diagnosis”. A once-familiar world suddenly feels foreign, as family members shift their daily home and work routines in order to meet the care giving needs of a loved one. Families tend to call hospice when they have reached a point of exhaustion and are ready to ask for help. But once they experience the wrap-around support hospice has to offer, these are the very families who wish they’d called hospice earlier. Patients and families discover that a good hospice team consists of care providers from many disciplines within a hospice organization, and the team provides comfort care not only for the patient, but also for the families involved. Patients receive palliative medical support and tender care from RNs, CNAs, pharmacists, and a medical director, who provides oversight and supervision for medical decisions. With this team approach to medical support and pain relief, patients can choose to remain at home and enjoy their last days with family, in a comfortable environment. Patients and families also receive emotional, spiritual, and psychological

support from hospice social workers, chaplains and music thanotologists, through whom life stories, spiritual concerns, and final wishes are often named and shared. Most importantly, the support of a hospice team allows family members the opportunity to take off their “caregiving hats” and spend precious quality time with their loved ones. When I was first working as a hospice chaplain, I remember overhearing a team member say, “We sure witness a lot of laughter, a lot of tears and a lot of storytelling at the end of life.” And don’t we all deserve to live and die with such dignity? Hospice patients and family members are able to go on living into their dying process, which is a gift I think we all need to remember and reclaim in our culture. The words of a lovely elderly man and artist, whose wife died on hospice care, will always stay with me. “I will never forget how hospice took care of my Mary. You knew what to do at the end of her life when I was so exhausted I could only listen to her and hold her hand. For that last bit of time with her, I will be forever grateful.” He loved like he painted, with every color he had available.
