Housing needs for people aged 55 and older are changing across the country, and Montana is no different. A recent Harvard University Joint Center for Housing Studies report estimates that “by 2035, one in three U.S. households will be headed by someone 65 or older.” That’s more than 30 million people over the next 20 years. The problem is that only 1 percent of the housing stock is ready to meet the needs of people as they age. So how can baby boomers in Big Sky Country begin planning for a future of independent living now, so they can comfortably stay in their own homes with accommodations they can depend on, in a safe community? One option is Mostad Construction, which 8

The largest cottage has 14 residents, each with their own bathroom, and common areas that give it a more home-like feel. One of the biggest differences between assisted living homes and nursing homes is the level of independence a resident has. Individuals at nursing homes often times need more medical care and assistance in activities of daily living. Maintaining these seniors’ independence is important – and little things like cooking go a long way. “We cook for them but they’re welcome to come into the kitchen,” Humphrey said. “They get three meals a day prepared by a dietitian so that they get all the nutrients required but they’re not limited to just those three meals, they can request food at any time.” BeeHive Homes has a multitude of activities for residents to engage in, offer memory and respite care; as well as hospice and physical therapy. Humphrey said the quality of the care they offer is what really sets them apart. “We’re not in it for the money,” Humphrey said. “We’re in it to take care of them and it shows with the way we feed them, the quality of food, the activities and just the quality of care they receive from our employees.” The thing Humphrey loves most about BeeHive Homes is that it allows residents to “age in place.” He said some assisted living facilities will try and move someone to a nursing home once they start requiring more assistance but that’s something he doesn’t want to do. “We really don’t want them to go to a nursing home,” Humphrey said. “We’ve noticed they kind of decline in their overall status once they go to a nursing home.” According to Humphrey, BeeHive Homes in Helena has sent one person to a nursing home in the past eight years, and that was under the previous owners. In order to take care of increasing demands, there is a rent scale that may increase the price slightly to stay, but it ensures the individual does not have to deal with moving. “Once they require a lot of attention and a lot of care, they won’t be kept in their facilities,” Humphrey said. “Whereas if they come here, there’s usually not another move they have to contend with and deal with. They can age in place and stay here.” MT55

Dentures vs. implants: What’s the best option for you? Story by Kolby Kickingwoman, for Montana 55 • photo by Thinkstock The loss of teeth can have an adverse effect on life. Whether not being able to eat the foods you enjoy or having a lower self-esteem because of your appearance, tooth loss is no laughing matter. According to the American Academy of Implant Dentistry, one in four people over the age of 74 have lost all their natural teeth. Fortunately, with the advancement of technology and techniques, there are ways to ensure a full smile. 12

While dentures and bridges have been the more traditional method for tooth replacement, more and more people are opting to receive dental implants. When it comes to dentures, they are almost always cheaper than implants but often require more maintenance over time. Dentures are made after a series of measurements and impressions are taken of your upper (maxilla) and lower (mandible) jaw to see how they relate to one another.

A model is then produced and tried on several times where it is examined for color, shape and fit before the final mold is cast. After the final denture is complete, adjustments are made as necessary. Conventional, or complete, dentures are made after teeth are removed and the gum tissue has started to heal. This type of denture is ready for placement in eight to 12 weeks. For someone who doesn’t want to wait, immediate dentures can be made in advance and

put in as soon as the teeth are removed. While the patient doesn’t have to be without teeth during the healing period, immediate dentures normally require more adjustments during the healing process. These dentures should be considered temporary until conventional dentures can be made. Until recently, complete and partial dentures were a procedure covered by Medicaid. During a special state legislative session late last year budget cuts resulted in $8.9 million in cuts to Medicaid dental services. The cuts were made due to a revenue shortfall and last year’s fire season, the most expensive in state history. The Montana Department of Health and Human Services (DPHHS) proposed Medicaid rule changes that would eliminate coverage for certain dental procedures. The proposed changes will eliminate Medicaid coverage for full maxillary and mandibular dentures as well as partial dentures. Crowns and bridges for adults also will cease to be covered. Executive director for the Montana Dental Association David Hemion doesn’t believe the rollback of Medicaid coverage is a good idea. “It’s definitely a bad choice by the state to do away with a dental service like dentures, especially for the elderly and disabled,” Hemion said. “They are essential for nutrition and without dentures the mouth structure can deteriorate to a point it will be more costly to maintain good oral health.” In an email, communications specialist for Montana DPHHS Chuck Council said many factors were considered in the reductions and that the department did the best it could to protect the people it serves. “These are not funding reductions any of us want to make,” Council said in the email, “but we have been forced into making these very difficult and painful decisions.” The changes were set to take effect March 1. On the other hand, dental implants provide a more longterm solution to teeth loss, even though the initial costs are more expensive. According to clinical studies, dental implants have a success rate above 95 percent and are designed to last a lifetime when maintained properly. The surgery process is usually done in stages and can take several months to be completed. Additional surgeries may be required such as a bone graft or a sinus lift, which elongates the process in order to allow for the proper healing time. Compared to dentures, implants have some distinct advantages, the natural look and ability to care for them as if they are normal teeth is just one of them. Dentures that don’t fit properly can make talking and eating difficult, implants act like your own teeth allowing you to eat and

talk with confidence. Although they may have a higher initial cost, a study published from 2013 in the U.S. National Library of Medicine showed single-tooth implants were generally cost saving or cost-effective over the long term. The same was said of multiple-teeth implants, with oral health improvements compared to other options. According to dentalimplantcostguide.com, dentures for one arch of teeth typically would cost between $300 and $500, whereas a full set costs $1,000 on average. Top quality dentures made from the best available materials can run $8,000 or more for a full set. Conversely, according to the same website, a single-tooth implant normally costs between $1,000 and $3,000 with an added fee depending on what material the dentist uses for the implant crown. Multiple implants can cost upwards of $30,000 depending on the number of implants and treatment choice. Every person is different and consultation with a dentist can help steer you in the direction that’s best. As with anything, there are several variables that factor in including price, insurance coverage and personal opinion. Whether it’s dentures or implants, both can give you a reason to smile. MT55

Missoula’s St. Thomas House offers intimate care in a home setting Story by Elizabeth Rolle, for Montana 55 • photo by Thinkstock Visiting with various people who run small assisted living homes in the Missoula area spurred my inspiration. Each is unique in their approach, but most have a common history; one owner had to quit a job to take care of an aging family member, thus opening a home. These homes exemplify personal 14

care of each resident and an attention to each individual’s needs. I researched the variety of options for caring for aging adults and each search resulted in the same answer with regard to the best care: intimate care in small home settings with deliberate effort at creating value for each member of the home.

Tips and tricks for successful aging in place Story by Autumn Gaylor, for Montana 55 • Photos by Associated Press As the baby boomer population reaches 65 years of age and older, the growing concern for safety in the home and in the community rises. The number of people age 65 and over has continued to grow over the past 18 years and now accounts for more than 15.2 percent of the total population, according to a June 2017 U.S. Census Bureau report. 16

General wear and tear of the body, increased risks of falling, and medical declines have prevented people from aging in place and maintaining an independent lifestyle. This increase in risk level may be due to physical, medical and cognitive changes associated with aging as well as environments that have not been adapted for the aging population.

According to the World Health Organization, approximately 37.3 million falls severe enough to require medical attention occur each year. Common risk factors associated with falls are lower body weakness, vision changes, poor foot wear, balance deficits, medication side effects, joint pain, poor diet and home hazards in the form of clutter, uneven surfaces, and dangerous tubs and showers. Here are a few tips to successfully age in place.

Make healthy food choices

Never stop moving Maintain stability and balance by establishing a proper exercise routine right at home. Home exercises can help improve both balance and strength. Having a well-developed sense of balance is essential for health and well-being. Balance exercises can consist of these simple activities: • Walk a straight line, one foot in front of the other, alongside your kitchen counter or along the back of the couch (this allows you to have a solid surface to assist when needed). • Stand on one foot, then the other, as you count to 10. Make sure to tighten your glutes and stabilize at the hip. Place a chair in front of you or next to you for added stability. • Stand with one hand on a chair. Turn your head from left to right with your eyes open, then repeat with your eyes closed. • Do the Kneeling Superman. Start on your hands and knees. Lift and extend your left arm forward as you simultaneously extend your right leg behind you. Hold for five counts. Repeat with the opposite arm and leg. Strengthening exercises in the form of body weight movement can increase muscle tone and strength and can help prevent injury. Strengthening exercises include: • Wall pushups. Place both hands on the wall in front of you with your feet positioned 1 1/2 -2 feet away from the wall. Bend your elbows and slowly bring your torso toward the wall, then slowly push yourself away from the wall and extend your arms. • Body weight squats. Position your body in front of a sturdy chair or couch. Place your feet just beyond shoulder-width apart and begin to sit down, not allowing your knees to go beyond your feet. As you lower your body to sit in the chair, keep your head up and your back straight to support the spine. Once you meet the chair in a sitting position, stand up and complete the exercise again for a 10 count. Exercise is not just for the body, it also is for the mind. Keep the mind healthy by reading books, completing word puzzles,

• Install grab bars in all bathtubs and showers. Install shower chairs, bath benches and non-slip mats to ensure safe bathing. • Place dishware and frequently-used kitchenware at shoulder height and on accessible cupboard shelves. This will eliminate the risk of having to stand on chairs or stools to reach top cabinets or having to get on the floor to dig through low drawers.

• Sit down when dressing and undressing. • If stairs are present in the home or outside of the home, make sure to have railings installed or grab bars positioned.

Be aware of your environment Situational awareness is a key factor in environmental safety. Keep an eye out for uneven surfaces, curbs, water on floors or sidewalks, ice and snow. Wear proper

shoes with good traction and arch/ankle support.

Medical management It is important to attend regular doctor, dentist and optometrist visits to ensure and maintain health and well-being. Be proactive by researching the over-thecounter and prescription medications you are taking. This will allow you to discuss these medications with your doctors and

But a new generation of services and technology is making it possible to stay at home longer, safely and happily, experts say. “Most people would rather stay in their own homes as they age, and technology has made that easier in so many ways,” says Amy Goyer, a family and caregiving expert with the AARP and author of “Juggling Life, Work, and Caregiving,” published by the AARP and the American Bar Association. “There are a lot of resources to tap into, even for those on a limited budget.” She recommends starting with the “caregiving” page of AARP.org and your local Area Agency on Aging network (see n4a.org), which is federally funded and also can lead you to a range of state and local resources. Beyond technology, a little creativity often goes a long way toward helping people manage to live at home longer, Goyer says. “If a person can’t do stairs, for example, consider ways to put everything they need on the main floor, like maybe bringing a washing machine up from the 18

Digital locks, which can be part of a smart home system, can be set so the door is unlocked for a small window of time to allow a caregiver into the house. Different codes can be set up for different people. They can be monitored from afar on phones, as can digital doorbells, which might help both the hearing and mobility impaired. Digital medication dispensers can send text notifications to loved ones to let them know whether someone has taken their pills. Cameras can be installed so loved ones know whether home health aides have come by. And there is a wide range of medical alert systems, some even including a GPS. “My aunt fell in a parking lot and luckily someone was there and picked her up, but if they hadn’t been there, a medical alert system could have made a world of a difference,” Goyer says.

Transportation

Many counties and community agencies have some kind of senior taxi run by volunteers to take seniors to doctors’ appointments, grocery stores,

senior centers and other errands. Ride-sharing companies have also proven helpful for many. The site GoGoGrandparent.com, for example, is designed to be easy to use for seniors — they don’t need to use a phone — and taps into local ride-sharing services. It can be paid for by relatives living out of town, who also receive notifications of pickups and drop-offs. Justin Boorgaard co-founded the company with friend David Lung in 2016 to help Boorgaard’s grandmother maintain her mobility and independence. “Her independence, and the independence given back to her family is something we believe the world needs,” he says. “We screen drivers and use only those with the best reviews. We filter them to make sure they have cars with room for walkers, canes, foldable wheelchairs or service dogs, and we step in to help if something’s not going right.”

Food

“Meals are a big thing when you’re trying to set everything up for aging at home, and a lot of people don’t have the energy or ability to cook for themselves,” Goyer says. In addition to Meals on Wheels, which is administered by local communities and delivers reasonably priced prepared

Caregivers, too, should make sure they have supports in place for themselves as well as their loved ones.

Social network

Isolation and loneliness are health threats that should not be taken lightly, Goyer says. Faith-based organizations often have networks of people who can stop by and say hello every so often. Goyer says it’s also worth looking into national programs like the Foster Grandparent Program, which pairs seniors with younger people, and also the Senior Companion Program and the Senior Corps volunteer program. All are administered by the Corporation for National and Community Service, the same organization that runs the

Americorps volunteer program, and can be found at nationalservice.gov. Many communities have started a Village to Village Network, where people can pool resources to get things done more efficiently; for example,

Live long and prosper:

How long will you live in retirement? The answer to the age-old question truly is: we never know our own mortality. Sure, we may have an idea of our family medical history and longevity. We understand healthy eating habits and regular exercise should keep us on the right side of the ground for as long as possible. However, the answer holds profound impact on our retirement and estate planning. A sound financial plan takes into consideration the key risks in retirement. Longevity (yes, living long!) is the multiplier of these risks. According to the Center for Disease Control and Prevention’s December 2017 report, Americans’ life expectancy has decreased for the second year in a row. Several factors contribute to this. Obesity, the opioid crisis and increasing terminal illnesses all point to the lowered life expectancy. However, the opposite also is true. We are living longer. A Pew Research Center article from April 2016 said that centenarians, by percentage, are the world’s fastest growing population segment. We are living past 100 more prevalently than ever before. How cool is that? Live long and prosper: this is what we’re supposed to do. However, this is also the key risk to most retirees’ retirement plans. Five clear risks exist to retirees’ retirement portfolios: interest rate risk; market volatility or economy risk; inflationary risk; health care costs; and longevity risk. First, retirees must consider interest rate risk. This risk addresses the consequences of rising interest rates and/or a prolonged low interest rate period. Rising or falling interest rates can have a significant impact on retirement assets. Second, market volatility or economy risks address how market loss can impact retirement portfolios. A natural tendency exists for advisors and those at or near retirement to compensate for lack of savings by being overly aggressive with their portfolio. Market volatility can impact our portfolios. Third, inflationary risk addresses the fact that over time, the price of the goods and services increases. Americans over the age of 55 remember when gas was below $1, a stamp was 10 cents, and houses were close to the price of a new car today. Fourth, the health care costs, both short-term and long-term, will impact retirement. The ever-increasing costs of health care, prescription drugs and long-term care are well-documented. Perhaps one of the biggest wealth-eroding factors in families’ estates is the cost of long-term care or end-of-life care. Fifth, longevity risk, also known as the risk multiplier. What a double-edged sword: we all want to live as long and as meaningfully as we can. Longevity is a risk, you say? Yes – the longer we live, the more likely we are to experience one of the first four risks and with bigger impact. The longer we live, the more likely we will face interest rate changes. The longer we live, the more likely we will experience marginal to significant market fluctuations. The longer we live, the more we will recognize inflationary challenges on our portfolios. The longer we live, the higher likelihood we have to experience health care setbacks, prescription drug expenses and the more likely we are to need long-term care at some level.

Pavini said asking is important, as a retailer or a restaurant may offer discounts on your current purchases or have special days earmarked for seniors. Pavini and James said some establishments give seniors discounts if they show an ID, but other places tie the discount to having a card from AARP, a special-interest group for people 50 years and older that requires a paid membership. Some establishments also ask seniors to sign up for free loyalty programs. By doing so, they get access to special promotions and other discounts. Those may or may not be used in conjunction with senior specials. “I think a lot of it is goodwill. Many retailers realize seniors are living on a fixed income. And a lot of them, if they get a discount at a certain store, they’ll tell their friends and family members, so it’s great word of mouth advertising. The discounts aren’t huge, so it makes sense for retailers to offer them,” James said. Search the web. There are plenty of shopping websites that list senior discounts, so it’s worth it for anyone 55 and older to investigate what’s available. James said he’s found at least 60 types of specials geared to seniors in various retailer categories. Some places offer senior discounts, and the establishment’s websites may offer printable coupons, making it possible to stack discounts. Most restaurants have steered away from early-bird dining to offering price reductions throughout the day. For example, Pavini and James said Applebee’s gives people 55 and older who ask for discounts five to 15 percent off their bill. James notes on his website that seniors can stack discounts at Boston Market, which has 10 percent off a meal for people over 65, plus printable coupons on its website. Denny’s offers 20 percent off to AARP members, but will still give seniors 10 percent off if they aren’t members, James said. A lot of retailers have discount days for seniors, James and Pavini said. On Wednesday, people over 60 can save 15 percent at Kohl’s, and Ross earmarks Tuesday for seniors to get an extra 10 percent off. Check the website or ask an associate if there are any limitations, such as the discount not being applicable to gift-card purchases or the buyer has to purchase a full-priced item. More entertainment places are offering senior discounts, they said. Many museums and zoos have discounts for seniors, Pavini said, as do parks and recreation centers. Movie theater chains AMC and Regal have 30 percent off tickets for people over 55, James said. James said he was surprised to see a number of hotels are offering discounts for seniors, anywhere from 10 to 15 percent. Again, some require an AARP membership, but for others its proof of age. “You just have to ask for them,” he said. MT55

Montana veterans homes The State of Montana operates and manages the Montana Veterans’ Nursing Home in Columbia Falls and oversees a contract for the operation of Eastern Montana Veterans’ Nursing Home in Glendive. Both facilities are restricted to honorably discharged veterans who have served in the armed forces of the United States, and their spouses (spouses are admitted only if space is available). Eligibility for admission is determined without regard to race, color, sex, culture, social origin or condition, political or religious ideas, or ability to pay for the cost of care.

Medicaid helps pay for nursing home care for those who could not otherwise afford care. There are specific “resource limits” that must be met in order to receive Medicaid. Approximately 2/3 of all nursing home residents in Montana use Medicaid. types of care Adult Day Care Center (ADC)

–a facility that provides adults with the regularly scheduled care necessary to meet the needs of daily living. These centers, which may be freestanding or connected to another health care facility, do not provide overnight care.

Adult Foster Home (AFH) – a private

home or other facility that provides light personal and custodial care to four or fewer disabled adults or aged persons who are not related to the owner or manager of the home by blood, marriage, or adoption or who are not under the full guardianship of the owner or manager.

Assisted Living Facility (ALF)

–a congregate residential setting that provides or coordinates personal care, scheduled and unscheduled 24-hour supervision and assistance, activities and health-related services.

Home Health Agency (HHA)

– a public agency or private organization that offers health services to individuals at home. Home health agencies must include the services of a licensed registered nurse and at least one other therapeutic service. Additional support services may also be available.

Hospice (HPC) – a coordinated program of home and inpatient health care that provides or coordinates palliative and supportive care to meet the needs of a terminally ill patient and the patient’s family. Caregivers help relieve the physical, psychological, spiritual, social and economic stresses experienced during the final stages of illness and dying. Practices are rooted in formal bereavement programs.

Independent Living (IL) - any housing

arrangement designed exclusively for seniors, generally those aged 55 and over. Housing varies widely, from apartment-style living to freestanding homes. Often, the units are designed to be senior-friendly, offering a more compact space with easier navigation and assistance with yard maintenance.

Long Term Care Facility (LTC) – a

facility that provides skilled nursing care, intermediate nursing care, or personal care, to a total of two or more individuals.

Memory Care (MEM) – a facility that

provides care services and programs for residents suffering from memory loss due to stroke, Alzheimer’s, Parkinson’s or other diagnosed dementia.

Retirement Home (RTH) – a building

or buildings in which separate living accommodations are rented or leased to elderly persons for use as their primary residence.

Respite Care (RC) – a service that provides

temporary relief periods and other stress-relieving support for family members who provide continuous care for developmentally disabled relatives at home.

Skilled Nursing Facility (SNF) – the

provision of nursing care services, health-related services, and social services under the supervision of a licensed registered nurse on a 24-hour basis.

Types of Independent Living: • Subsidized senior housing. The U.S.

Department of Housing and Urban Development (HUD) subsidizes senior housing complexes for low income seniors. Waiting lists are common, so it is essential to plan ahead for this option.

• Senior apartments. Apartment complexes

restricted by age, usually 55+. Rent may include community services such as recreational programs, transportation services, and meals served in a communal dining room.

• Retirement communities. Groups of

housing units for those aged 55+. These housing units might be single family homes, duplexes, mobile homes, townhouses, or condominiums. Monthly fees may cover services such as outside maintenance, recreation centers or clubhouses.

• Continuing Care Retirement Communities (CCRCs). CCRCs offer service and housing packages that allow access to independent living, assisted living, and skilled nursing facilities in one community. If residents begin to need help with activities of daily living, for example, they can transfer to an assisted living or skilled nursing facility on the same site.
