President’s Message Wow! What an honor to have recently introduced 18 outstanding nominees for Texas Dentist of the Year. It is both refreshing and humbling to hear about the activities and accomplishments of our esteemed dental colleagues. We are so fortunate to practice in a profession where individuals expand their horizons through continued learning, deliver the best possible care to their patients, and serve beyond their profession to make a difference in the lives of people and our world. The Lone Star meeting continues to grow and improve every year. It was an outstanding program and a great venue to learn, connect with old and new friends and colleagues and meet with vendors for the latest and greatest. Mark your calendars for next year, September 11-13, 2014. There will be courses for the entire team. Advocacy for the General Dentist and our patients continues to be at the forefront of our activities. Latest activities include working with the State Board regarding sleep dentistry rules and regulations. Please consider a donation to the Advocacy fund to help insure TAGD’s voice remains among the significant players at the table. We focus on continued growth and development throughout all levels of our organization. FellowTrack programs have been established in all three dental schools, New Dentist Study Club participants and leaders are growing in numbers, and MasterTrack and Master Study Club continue to expand. This year we are seeking to impact a much larger percentage of general dentists in Texas and I personally challenge each of you to invite at least one colleague to join AGD. Editor’s Message This issue of the TexasGP celebrates the Texas AGD New Dentist of the Year, Doug Bogan. Dr. Bogan was one of the first people that I met when I began participating in organized dentistry. Even though he was already a big shot and I was just starting out, he was very approachable, interested in talking shop with me and clearly committed to dental education and advocacy. We are fortunate to have his leadership. Way to go Doug! Well deserved. Fourth time was a charm. There’s still hope for you McCauley. TAGD had another successful Lone Star Dental Conference, our premiere event of the year. We are happy to showcase in this issue many of the highlights. For the second consecutive year, the LSDC took place in the Domain which features convenient shopping and dining all within walking distance.

Chosen as New Dentist of the Year was Dath Collins. Dath was chosen from a strong field of nominees all practicing dentistry 7 years or less. I’m so happy that your family was there to share it with you. All of the nominations were well deserved and it is reassuring to see such leadership so early in their careers. The 2nd annual TexasGP photo contest saw an increase in participation over last year. The photos were displayed at Lone Star. Thank you to everyone for the great photo submissions. As with every LSDC, the TAGD Board transitions to its new leadership. After an excellent year as President, Dr. Mark Peppard handed over the gavel to Dr. Donna Miller from Waco. DR. GRANT HARTUP Receives Prestigious Weclew Award

In 1953, the Academy of General Dentistry created the Thaddeus V. Weclew Award. Named after the organization’s founder, the award is meant to recognize members who not only exhibit excellence in dentistry, but dedicated role models that inspire other dentists to reach the heights of their potential. In the sixty years since the award’s inception, only 115 of the organization’s 38,000 members have earned the distinction of this recognition, and it is one of the AGD’s highest honors. This past July, Texas AGD member Dr. Grant Hartup joined this exclusive list, winning the 2013 Thaddeus V. Weclew Award. Dr. Hartup graduated from the University of Minnesota School of Dentistry in 1984 and accepted his commission in the U.S. Air Force the following year. That decision was one of the most important of his career, and he credits his first boss in the Air Force, with influencing his career path. “I have very strong

memories of his repeated statements about getting our priorities right.” In 1990, Dr. Hartup began his AEGD at Keesler Air Force Base. Two years later, he completed his residency, and he became a diplomate of the Federal Services Board of General Dentistry and the American Board of General Dentistry in 1994. Needing to work his way through dental school, Dr. Hartup says, “Sleep was optional in those days”. As a result, he has always had the perspective that it is part of his job to help others. “You need to pay it forward!” he explains. “When you get the chance, help someone else—help your kids pay for their school, help others along the way with their journeys.” Dr. Hartup has always made it a priority to reach out to the dental community and help young dentists grow and learn. He recently completed an assignment as the AEGD flight commander and AEGD program director of Wilford Hall Medical Center at Lackland Air Force Base in Texas where he oversaw the Air Force’s largest dental residency program. He also served as the military consultant to the Air Force surgeon general for general dentistry, providing subject matter expertise, as well as career and professional guidance, to hundreds of general dentists in the Air Force. The Thaddeus V. Weclew Award is presented to a dedicated educator who embodies the spirit of comprehensive dental care, and certainly nobody fits that description better than Dr. Hartup. Presently, Dr. Hartup serves as the director of Air Force dental operations, overseeing the delivery of dental care to 330,000 Air Force personnel at 79 facilities around the world. AGD President Linda J. Edgar has said of Dr. Hartup, “he has dedicated himself to his country, as well as to ensuring that dental residents in the Air Force receive exemplary training and career guidance.” The TAGD has also lauded Dr. Hartup for his work, as he was also nominated for the 2013 Dentist of the Year AwardTM.

Dr. Hartup, of course, is quick to give credit to those who have helped him get where he is. One of Dr. Hartup’s mentors throughout his career has been fellow TAGD member Dr. Kevin Gureckis, who he says has given him countless insights into how to be a better dentist and a better teacher. “Kevin and I worked together directing two different residency programs,” Dr. Hartup explained. “He taught me an awful lot about effective teaching, and the ways residents learn both didactically and clinically. Kevin also showed me how important it is to keep your perspective and have a sense of humor about life, and yourself.” Dr. Hartup has taken that last lesson to heart, and while he is certainly dedicated to his work, he always makes sure to relax and enjoy life. His top priorities in life are his children, Lindsay and Jake, and his girlfriend Lori. He says that “my family has kept me focused on what is truly important.” He is also a sports nut, with a particular passion for soccer, and he loves going on hikes with his dog Parker, a labrador. For nearly thirty years, Dr. Hartup has been a dentist and an educator who loves his job as much today as he did when he first started.

“Dentistry,” he says, “is an incredible profession that offers the opportunity to be a healing healthcare professional, a leader, a craftsman and an artist all within the same moment!”

Beyond everything that dentistry has allowed Dr. Hartup to be, the title he is most proudest of is role model. The Thaddeus V. Weclew Award was created by the AGD specifically to make sure that dentists who work to inspire those around them get the recognition they deserve, and there is truly nobody more deserving of this prestigious award than Texas’ own Dr. Grant Hartup.

That “deer in the headlights” look many people saw on my face on the evening of Saturday, September 28 was actually astonishment, not fear. I had just been announced as the recipient of the TAGD Dentist of the Year Award for 2013. I salute all of the nominees! Several of the nominees are great friends and role models for me.

New Dentist of the Year Dr. Dath Collins Dr. Dath Collins was announced as the New Dentist of the Year on Thursday September 26th during the leadership reception. Dr. Collins is a 2006 graduate of the University of Texas School of Dentistry at Houston and now practices in Houston, Texas. In addition to serving in numerous positions within organized dentistry, he also is an active volunteer throughout his community with Chancellor Elementary and Paul Rever Middle School, the Houston Zoo and Ducks Unlimited.

Jack T. Clark Foundation Silent Auction hanks to several people’s generous donations and participation, the Jack T. Clark Foundation was able to raise nearly $12,000 towards the work of our organization during the Lone Star Dental Conference. The mission of the Jack T. Clark Foundation is to support the oral health of all Texans affected by cancer through education, access to appropriate oral health care and collaborative research. Since its inception, the foundation has performed 3,500 oral cancer screenings, distributed educational information to

About the New President: Linda J. Edgar, DDS, MEd, MAGD has been a AGD member for more than 20 years. Dr. Edgar has served the association in numerous leadership roles. She was a Board of Trustees member for Region 11, which is composed of Alaska, Idaho, Montana, Oregon, and Washington. She also has held all elected offices for the Washington AGD and served as its executive director for four years. During her tenure on the AGD Membership Council, Dr. Edgar developed a student transition manual and helped to create an AGD constituent marketing program, which aids constituents with their local recruitment efforts. Because of this work, Dr. Edgar received the AGD’s Overall Recruitment Award in 2003.


End of the Year Tax Tips By Cortney Cooperthwaite, EA  •  Tax Manager with HDA Accounting An unpredictable political environment is a tax planning nightmare, but here are a few pitfalls to watch out for as you do your end of the year planning: • Depreciation changes in 2014 – For the last two years, business owners have enjoyed increased 179 Expense and bonus depreciation of 100% and 50%. These benefits lapse in 2014. The section 179 Expense is scheduled to drop to $25,000, this is the lowest it has been since 2002. The bonus depreciation is going away completely. So make sure that piece of equipment you are planning to buy is paid for, delivered, and placed in service by December 31, 2013. • Increased tax rates for high income taxpayers – The 2013 tax year will be the biggest change in tax rates since the Tax Reform Act of 1986. Although the tax rates haven’t changed for the majority of Americans, if your taxable income is over $450,000 (for a married filing jointly taxpayer), you can plan for an increased tax rate of 39.6% on every dollar over

that amount. If your modified adjusted gross income is over $250,000 in 2013, all your investment income (this includes interest, dividends, capital gains, rental income, and income from businesses where you are not an active participant) will be subject to a net investment income tax of 3.8%. If your wages are over $200,000, you are also subject to additional Medicare tax of 0.9%. • Entity type selection – Whether you are starting up a new practice or you’ve been in business for years, you should have a conversation with your accountant before the end of the year, whether your entity is still appropriate. Maybe you’ve been a sole proprietor for years and it’s time to make the election to be a corporation. Maybe you are C Corporation and need to issue a bonus to yourself to decrease your corporate tax liability. Whatever your situation, waiting until you prepare your 2013 tax return is not the time to make major changes. Having this conversation before the end of the year will allow you to have a plan in place and take the

appropriate actions before it’s too late to do so. • Individual health mandate – Do you have health insurance? Do you provide insurance to your employees? In 2014, health insurance will need to be addressed. If you do not have health insurance personally you will pay a penalty of $95 per adult and $47.50 per child or 1% of your household income on your 2014 tax return. These penalties increase every year and will be $695 per adult (half that per child) or 2.5% of your household income in 2016. There are exceptions for lower income taxpayers and the health marketplaces have lower premiums for taxpayers who qualify. Keep in mind that if you offer your employees insurance, they are not eligible to apply for the tax subsidies on these marketplaces even if they elect to refuse the coverage you offer. Consult with your tax preparer regarding how these changes apply to you personally.

Connie Sonnier, joins elite group Inducted as honorary member into the International College of Dentists

The International College of Dentists rarely inducts non-dentists into their organization unless there is someone who has made an outstanding achievement or contribution towards the promotion of the dental profession. Connie Sonnier joins only four others in the State of Texas to receive this honorary fellowship that requires a nomination by the board of regents and the endorsement of at least five members. On November 1, 2013 at the Fellow of the International College of Dentists 84th Annual Convocation in New Orleans,

Connie joined 300 dentists from all over the United States to be inducted into the college. Connie Sonnier began working with the Texas Academy of General Dentistry (Texas AGD) in 1999 and has served as the Executive Director since 2003. The Academy serves nearly 2700 members in Texas and serves to provide continuing education support and represents the interest of general dentists. With a staff of nine, Ms. Sonnier administers activities of the state along with nine active components and will support over one hundred continuing

education programs for Texas in 2013. In addition, along with the TAGD lobbyist, she serves as a liaison to legislative and regulatory representatives to guide and protect the interests of general dentistry. Ms. Sonnier feels her most important accomplishment for the Texas AGD has been in identifying and then implementing opportunities that have helped the organization grow membership and increase benefits to members. Ms. Sonnier and her husband, Andy, live in Georgetown, Texas and have three grown daughters and five grandchildren.

Your attention is held by the subject matter, very well composed and the use of light and dark, contrast and shadow was very well implemented. Not an easy shot to get in many cases because of the contrasts. 2nd place: Once in a Blue Moon by Dr. Yana Kushner Judges’ comments: Nicely different from some of the typical “moon” shots (which are difficult to take and get right. Haunting, evocative, moody, mysterious and serene. I was drawn to this one immediately. Interesting composition. Well done and avoiding the temptation to place the moon in the center of the photo.

There is so much of this in nature that we fail to see simply because it IS hidden from view in an isolated location. This photo conveys that to me. Honorable mention: Flying by Dr. Steven Vaughan Judges’ comments: Nicely done! Difficult for sure because of the back lighting of the sun. Different treatment of carnival rides like this. Update on Sleep Medicine By Dr. Jennifer Bone

The latest version of TSBDE’s proposed rule governing the dentist’s involvement in treating sleep disorders (22 TAC 108.13) represents an important shift toward improving access to care for patients struggling with sleep disordered breathing conditions. TSBDE’s initial rule proposal sparked written and/or oral testimony from numerous individuals and organizations, TAGD included. In response to these comments, the overall content of the new proposal clears away ambiguity noted in the original draft and permits greater contribution from the dentist in the management of these patients.

TAGD believes the dentist is critically positioned as a point of entry leading to appropriate diagnosis and treatment for sleep disordered conditions and their associated dental comorbidities. For this reason, we have submitted comments in favor of the new proposal, and TAGD representatives will be present at the next TSBDE meeting on Friday, November 8, 2013. At this time the new version of the rule may be adopted by the Board as written, adopted with minor revisions, or subjected to major revisions necessitating an additional opportunity for public comment.

The rule recognizes the physician’s role in interpretation of sleep studies and diagnosis of Obstructive Sleep Apnea (OSA). It also respects the dentist’s front-line position in identification of the signs and symptoms of sleep disordered breathing conditions. If adopted, the rule would allow dentists to screen patients for sleep disorders and to independently order sleep studies. Additionally, the dentist would be allowed to autonomously treat dental comorbidities related to snoring and Upper Airway Resistance Syndrome. Treatment of OSA using an oral appliance would still require collaboration with a physician.

A Lifelong Journey By: Nicole A. Nellis, UTHSCSA Dental School For some people, their journey to become a dentist began a few years before dental school. For others, the dream to become a dentist began at a young age – that would be my story. My fascination with dentistry began around the age of 3 with positive experiences at visits to my family dentist’s office. By the age of 7, I had decided that I wanted to become a dentist and I never looked back. Dentistry soon began to find its way into my playtime activities. In 1997, I got my first dentist Barbie doll, and disappointed with its limited array of instruments, I set out to build a Barbie dentist office in the corner of my bedroom that eventually grew to 11 operatories. When I began shadowing dentists during middle school, I paid close attention to the specific instruments and materials they used and eventually incorporated these into my model dentist office. In order to learn as much as possible about dentistry, I would spend my free time on the internet searching about dentistry, in which I ended up at the AGD’s website as they had a wide variety of patient information that was totally fascinating to an interested 12 year old. I even learned what Fellowship and Mastership in the AGD meant at a young age and added that to my career goals as well. School breaks meant opportunities to shadow dentists so I eagerly got up early on those days to watch dental procedures. I would make every attempt to incorporate dentistry into my school projects and essays. Moreover, I had my first job in dentistry at 15 as a receptionist during the summer. Now that I am a fourth year dental student, I do not regret my decision to become a dentist. I love how dentistry affords me with the opportunities to interact with my patients, provide them the best dental care possible, and the ability to always learn something new. Dentistry has truly been my lifelong passion and I am excited for the future of my career.

Painting with Patel

Orhan Pamuk once wrote that “painting is the silence of thought and the music of sight.” That quote is dear to Dr. Manshi Patel’s heart, and when Pamuk wrote those words he might as well have been talking about Dr. Patel, who, in addition to being an accomplished dentist, is also an artist. It is remarkable that Dr. Patel is able to find the time to paint, considering her busy schedule. Born in Fort Worth, Texas, Dr. Patel attended the Baylor College of Dentistry in Dallas, where she won multiple awards for her devotion to and talent for research. She received her AEGD in San Antonio and currently splits her time between two dental practices, one in Mansfield, the other in Grapevine. In addition, Dr. Patel strives to make an impact on those less fortunate. While earning her DDS, she volunteered for multiple programs that provide free dental care for children and families living in poverty, and she still makes it a priority to donate at least one day a month to helping the less fortunate. Yet despite her workload, Dr. Patel takes time to pursue her love of painting. Dr. Patel began painting the summer after her first year of dental school, and found that allowing herself to explore her artistic side was the perfect stress reliever.

“For me painting is extremely peaceful and relaxing, It takes me away from stress and allows me to be creative.” Dr. Patel describes her artistic work as “free style paintings,” and says that she draws inspiration from various sources. “I take note of something I like when I’m out or some color combinations I like

and just work from there. I’ll try to put my twist on something I like and just paint.” There are many benefits that Dr. Patel gets from her painting, which she says, “allows me to forget about anything that’s stressful in my life and takes me away from reality, in a way.” Her work also gives her a different perspective on the world around her. “Now I’m always looking for an idea and inspiration anywhere I go. I take my time noticing little things in life and I’m constantly looking for art.” Of all of the paintings she has done over the last five years, Dr. Patel says that the work she is most proud of is a multicolor tree painting. The piece, which she made while she was completing her residency in San Antonio, is exemplary of the way she draws inspiration from nature and then uses her own artistic perspective to put a unique spin on what she has seen. It was this piece, too, which convinced her that painting should be more than just a passing hobby, but that she should take classes and work hard to develop her artistic talent. In both her dentistry and her painting, Dr. Patel draws strength and support from her family. “Whether it comes to school, dentistry, painting, or anything else, they’re my biggest inspiration and strength.” Her family is her main audience for her work, and she often shows new pieces to either her parents or her brother and her sister-in-law before anyone else. Dr. Patel says that her greatest ambition for her art is always to impress her family: “I want to make a painting that leaves them speechless…hopefully soon!”

MICHAEL ADAME, DDS JOINS AN ELITE GROUP OF ONLY 8 OTHER DENTISTS IN THE STATE OF TEXAS TO EARN HIS ABDSM CERTIFICATION

ABDSM Certification by the American Board of Dental Sleep Medicine is one of the most prestigious awards that a dentist can receive, as it requires an intense level of dedication, passion, and study to achieve. Until recently, there were only eight dentists to earn this honor In Texas, but Michael Adame, DDS, a native Texan who was born in McAllen, has become the ninth. The certification that the ABDSM gave to Dr. Adame “signifies to patients, colleagues and professional organizations in both medicine and dentistry that Dr. Adame has met established qualifications and demonstrated a thorough understanding of oral appliance therapy.” Sleep dental medicine was not yet the prominent topic that it has become when Dr. Adame graduated from UT Health Science Center in San Antonio in 1987, but he has been helping patients with snoring problems since he began his practice. However, three years ago, a Dental Sleep Medicine course led him to the realization that the issue for which he had been treating his patients for was sleep apnea, and he became dedicated to learning as much as he could about this disorder and how to most effectively treat it. “Since overall wellness has always been an interest of mine, this area of sleep quality had revealed another facet of wellness that I thought would be important and beneficial for me to understand.” Dr. Adame has spent the last two years pursuing the requirements that the ABDSM has established before even allowing a dentist to take the certification exam. The examination includes testing on a thorough understanding of oral appliance therapy, and

is extremely rigorous and comprehensive. Attaining the certification puts Dr. Adame in a rarified group, and he says that he is

very glad to be part of such an esteemed group. As dentists around the country study sleep disorders, a thorough understanding of disorders like sleep apnea have become crucial to a successful dental practice. Of the advantage that his certification will give him, Dr. Adame says that “I am very prepared for most any condition that a patient will present with concerning sleep disordered breathing.” The diagnosis of sleep disorders is becoming more common as dentists are learning how frequently these issues have gone undiagnosed in the past; and Dr. Adame now has a thorough understanding of sleep disorders matched by only a handful of other dentists in the state of Texas. “I see the future of this disorder playing a more significant role in the dental office,” Dr. Adame explained. This certification will also allow Dr. Adame to work closely with board-certified sleep physicians to treat

sleep-disordered breathing. He strongly recommends pursuing ABDSM certification to other dentists who want to learn more about the burgeoning field of treating sleep disorders. “If they are considering offering oral appliance therapy for sleep disordered breathing of any kind, I think board certification will prepare them very well for the multi-dimensions of this medical illness.”


LIFETIME ACHIEVEMENT

DR. O.Z. HELMER Dr. O.Z. Helmer of Fort Worth was presented with the Lifetime Achievement Award by Texas AGD’s immediate past president, Dr. Mark Peppard at the Lone Star Dental Conference in September. Dr. Helmer has been a lifelong supporter and contributor to organized dentistry. He has served organized dentistry in both TDA and AGD on numerous levels including on the PACE and Communication Councils for AGD. He, along with Dr. Craig Armstrong, helped establish the current Texas AGD MasterTrack program. Dr. Helmer is currently involved with 3 different study clubs and also mentors younger dentists. Outside of dentistry, he and his wife, Brenda, have spent

two weeks at Christmas time serving disadvantaged children at a dental clinic in Israel for the past eleven years. He has also provided dentistry on many other shorter missions in Belize, Haiti, Honduras and Mexico. In addition, he has worked at TMOMs, Texas Dentists for Healthy Smiles, Give Kids a Smile and currently serves at Happy Hill Farm, a home for underprivileged children. Dr. Helmer is also a recipient of the National AGD Humanitarian Award, the highest humanitarian award given by AGD. Regarding Dr. Helmer’s Lifetime Achievement Award, Current President, Dr. Donna Miller, says, “It is no surprise and a much deserved recognition of O.Z.’s dedication not just to Texas AGD but to improving the lives of those around him. Having been able to witness his impact on our organization over the years has been a tremendous inspiration.”
