“Don’t brush all of your teeth, just the ones you want to keep” This joke may seem amusing, but the truth is that most people are aware that regular brushing is the key to maintaining healthy teeth and gums. But did you know that your choice of toothbrush and brushing technique are equally important to your oral health? If brushing is done correctly, it only needs to be done twice per day, in the morning and the evening, for about three minutes to safeguard healthy teeth and gums. It takes time to brush correctly, and the process should not be rushed. Begin by choosing a toothbrush that has the ADA (American Dental Assoc.) seal on the package. Be sure that the bristles are not too stiff, and find one that fits comfortably in the mouth.

To brush effectively:

• brush at least twice a day (of course, after every meal is optimal); • take time to brush at least three minutes – and don’t forget to floss! • brush along your gum line, and angle the brush slightly toward your gums; don’t brush too roughly, but be sure to cover all tooth surfaces; • brush your tongue to avoid bad breath; • rinse your brush well after each use, and replace it every three months.

Bear in mind that a toothbrush alone can’t get between your teeth; only flossing can do that, so floss regularly, and visit your dentist for regular cleanings. If you are prone to tooth decay or gum disease, consider investing in an electric toothbrush. They are easy to use, more fun for children and more effective at removing plaque. “ WILL CONTESTS: How Do They Work? ”

We discussed A.) Fraud and B.) Due Execution in the Atlantic County Woman July/August 2017 issue, Part 1 of 2. Today, we will continue in our discussion with C.) Testamentary Capacity and D.) Undue Influence.

C) Testamentary Capacity The test for determining testamentary capacity is rather permissive. Indeed, the level of capacity needed to execute a Will is less than that required to enter into a contract. The key questions are whether the person who made the Will (called the “Testator”) understood:

The nature and extent of the property subject of the Will; in other words, what assets the Testator had and planned to leave under his or her Will.

The “natural objects of his or her bounty,” that is, who are the Testator’s next of kin (whether or not he or she wants to leave them anything under the Will, the Testator needs to know who they are).

The effect of the Will; that the Will would dispose of property following death. The relation of each of these factors to the other.

Notably, the law presumes that all Testators are of sound and competent mind when a Will is executed (assuming he or she was at least 18 years of age and has not been declared by a court to be legally incapacitated). To overcome the presumption of capacity, the contestant must prove lack of testamentary capacity at the time the Will was executed, by clear and convincing evidence. It is important to know that it is not enough to show that the Testator was addicted to drugs or alcohol; the contestant must prove that the person was under the influence of a substance at the time of the execution of the Will. Nor is it sufficient to show that the Testator was forgetful at the time, so long as he or she had some capacity to manage his or her business affairs.

enjoyed a special or “confidential relationship” with the Testator, and there are “suspicious circumstances” surrounding the execution of the Will, the burden of proof will shift to the defender of the Will. A “confidential relationship” can be found where there is some special trust between the Testator and a beneficiary under the Will and where the nature of that special relationship creates some type of reliance or dependency. At times, a special or confidential relationship has been found to exist between a Testator and, an adult child, other relative, a healthcare provider, a neighbor, an attorney, or an accountant. Notably, while a confidential relationship naturally exists between a husband and wife, the presumption of undue influence will not generally arise even when the Testator’s children are disinherited. As to the “suspicious circumstances” component of an undue influence Will contest, the evidence on this issue can be “slight.” Suspicious circumstances have been found where a beneficiary of the Will engaged his attorney to prepare the Testator’s Will, and where a nursing home patient was removed from the facility by the beneficiary of a Will that was signed a few days later. If you believe that a Will should be challenged because it was not executed properly (i.e., it was forged, did not have the correct number of witnesses, etc.) or because the deceased was not competent or was unduly influenced when he or she signed it, you should hire a seasoned attorney who has significant experience in this area of law. Do not delay with your decision to engage an experienced attorney because the law allows only a narrow window of time during which a caveat and/or a Will contest may be filed.

D) Undue Influence Undue influence has been defined as any type of exertion that can prevent the Testator from following his or her free will. This can include physical, mental, or moral exertion. Mere suggestions, persuasions, or the exertion of some influence is not enough to invalidate a Will. Ordinarily, the challenger has the burden of proving undue influence. If, however, the Will benefits one who

First and foremost, my primary goal is to continue the same high standard of magazine content, client service, and sense of community that our readers deserve. Beyond that, we will be undertaking a project this year to survey our readers for their insight and feedback so that we can provide the best readership experience possible. So be sure and keep an eye out for that! Have you learned anything new about yourself since starting this new endeavor? Yes, there are so many discoveries I have made. In particular, I have enjoyed using my creative side more than ever, and have been mastering the art of time management. There are countless things that happen behind the scenes of each issue and to be able to produce and distribute the County Woman on a bi-monthly basis takes an enormous amount of work. It would not be possible without our team members performing to their full potential.

What are your goals for the magazine this year? As Production Manager, I have been able to oversee each cycle very closely. These days technology has become a huge part of everyday life. I would like to see the magazine become even more tech savvy. As a millennial, designing an app, or creating the website to become more mobile-friendly would be an ideal incentive to offer our clients. Additionally, I would like to see our social media continue to thrive. Over the past few years, the combination of print and social media is one of the most effective ways to advertise businesses. What do you enjoy most about your job? There are many things that I enjoy about working at the County Woman Publications. From the talented contributing writers, editors, and designers, whom I have learned so much from, to the clients which I have built long-lasting relationships, and lastly, the team of wonderful individuals, whose synergy results in making the publication a success. I would like to thank all of them for their commitment and dedication! If you were not in publishing, what kind of work would you be doing? When I am not in the office, I enjoy spending time with my Chocolate Lab, Bear, and my three cats. I have always had a passion for animals and my love for them continues to grow. If I were not in publishing, I would enjoy a job where animals are the focal point.

What are your goals for the magazine this year? My goal is to remain a quality and leadgenerating tool in our clients’ marketing plan. I would like to reach even further into the communities we serve with helpful information, especially in the areas of health, women, and children. We have exciting growth on the horizon!

What do you find to be the most challenging aspect of your role as Sales Director? Because I travel quite a bit for my work, inclement weather can really put a damper on my disposition and safety. Sometimes, it can also be challenging to educate business owners on the importance of marketing. Spending money to create public awareness can add up. Our goal is to design a marketing plan using the combination of print and online media which best fits the client’s business goals. What is the most rewarding part? I enjoy getting the opportunity to speak with community leaders and hear about their triumphs and issues. Sales in any field can be a very rewarding position when the goal sits ultimately with the success and satisfaction of your client. Our staff takes a vested interest in the outcome for our clients and in the education of our readers. What do you think makes the County Woman so unique among other publications? Our commitment to solid and helpful information creates a connection between our contributors and our readers. We have bridged the gap for many organizations to work together and for community members to learn more about what’s happening in their own backyard.

What do you enjoy most about the Graphic Design Field? To me, graphic design is a way to express yourself and be able to create anything from your imagination. What is the most challenging aspect of your position? Working for a publication that covers multiple counties throughout the state and the many businesses far and in between is stimulating and rewarding. With so much going on, we continually try our best to be precise in showcasing exactly what our clients want in each issue. The goal is always to make their pages fresh and exciting so that it grabs our reader’s attention. This is a fast paced position that requires high levels of customer satisfaction, working towards the same goal of making an outstanding magazine with a personal touch. What do you think makes the County Woman so unique among other publications? The magazine remains very special to our readers and writers. Breastfeeding: “A True Gift” The topic for this issue is breastfeeding. This is a true gift! It is a gift that a woman has the ability to feed her baby. It is also a gift of love from Mom to her newborn baby. This ability to sustain life is a special thing. During pregnancy, a woman’s body begins to prepare for milk production. The breasts hypertrophy, meaning they get larger. The nipples (areola) also enlarge and may become darker in color (pigment). There are glands (milk ducts) that are contained in the areola that also change. This is the way Mother Nature prepares a pregnant woman for the birth and feeding of her baby. During labor and following delivery, certain hormones are produced that cause the body to begin to produce a thin white fluid called colostrum. Colostrum contains properties that help to protect the newborn until his or her own immune system is functioning properly. The breast gradually starts to produce breast milk. The fluid will become more opaque and white to yellow in color. As the baby begins to breast feed, a complex hormonal release of prolactin and oxytocin (to name a few) fosters the breastfeeding process. The benefits of breastfeeding to a mother are many. Breastfeeding uses a tremendous amount of calories, which translates into weight loss for Mom. Imagine that—losing weight without exercise! If a woman has breastfed she will also experience several long-term protective health effects, such as a lower rate of breast disease. When prolactin is produced and released, a woman can feel a sense of relaxation. Some may refer to this as a breastfeeding high. The calming effect prolactin has can help a new mom cope with sleep deprivation and other life stressors. There is also an emotional aspect to breastfeeding. The skin-to-skin contact between the two fosters a maternal-child bond.

Bonding is so important for the baby as well. The mother’s breasts are over her heart and lungs. The newborn is able to feel and hear her heart beating. The rhythmic rise and fall of her chest, as well as the sounds of breathing, are perceived by the baby. The breasts are in close approximation to the mother’s face so the baby can easily see her and make eye contact with her. There are also positive health effects for the baby. Contained in colostrum and breast milk are maternal antibodies that are transmitted to the baby. This provides passive immunization for the baby, reducing the risk of the baby becoming sick. Many new moms have concerns that the baby is not getting enough milk. Well, the baby is born with a special fat layer that helps to provide additional calorie support until milk production increases. The size of a newborn’s stomach is very small. It is approximately the size of a large marble. So not too much can fit in there at any one time. Although the art of breastfeeding on face value may seem to be an easy process, it takes patience and practice for both the mom and baby. Essentially they both need to learn how to breastfeed. One can find volumes on the ways to breastfeed. The one that is correct is the one that works for both the mom and baby. This is a trial and error process. Most hospitals now have lactation consultants. These are health care professionals that have completed additional training in the art and science of breastfeeding. They are usually available soon after delivery, and during your stay on the postpartum unit. Advanced Care now has a lactation counselor available in our office. I encourage each of my pregnant patients to meet with her during the pregnancy. This allows the patient to become educated on the art of breastfeeding. It is helpful to do this before the delivery because it greatly reduces breastfeeding stress. Meeting with her while you are pregnant helps to educate you prior to actually having to breastfeed your child, eliminating stress or worry about not doing it right. She is also available after delivery if the need arises.

We at Advanced Care fully support breastfeeding. With the addition of a lactation counselor in our office, we attempt to educate and support all women in their endeavor to breastfeed. For pregnant women, maintaining a well-balanced diet and continuing prenatal visits is necessary. Seek education and support from a lactation counselor and your doctor. When Do You Need To See A Specialist For Low Back Pain? Low back pain is one of the most common reasons for patients to seek medical attention.

W

ith some pain episodes, the sudden reaching, bending or lifting event that began the misery is remembered, but sometimes the circumstances of onset are uncertain. Common causes of back pain include overstretching muscles or ligaments, the so-called “strain/sprain,” and these tend to resolve quickly. Many adults experience episodes of low back pain from time to time and fortunately, most resolve with no more than a single day’s rest, a few days of over-the-counter medications, such as ibuprofen or naproxen, and intermittent use of ice. These problems do not generally require X-rays or MRI studies. Other pain episodes may resolve with physical therapy or chiropractic within 4-6 weeks. Injuries to the lumbar discs, nerves or spinal joints tend to be the cause of most persistent pains. When back pain doesn’t resolve after a period of four to six weeks, progressively worsens despite treatment, or includes severe leg pain, weakness, incontinence, falling or an inability to walk, it is time to see a physician. If you have several episodes of recurring severe back pain over several months, evaluation is also merited. Often, family practitioners and general internists see initial episodes of low back pain, but after imaging studies, refer the patient to a specialist. While it was once traditional to refer patients to a surgeon for consultation, most patients don’t require surgery. Over 20 years ago, physicians who were previously called upon to address pain problems with specific techniques such as injections or rehabilitation by their surgical colleagues began to focus on the intricacies of pain diagnosis and treatment, creating the new field of Pain Management. Pain Management This field represents a new kind of multi-disciplinary thinking, focusing specifically on the understanding and treatment of pain as a symptom. Multispecialty training in pain combines physical medicine, anesthesia, psychiatry, neurology and radiology with some additional experience, often in orthopedics or neurosurgery. The American Board of Medical Specialties recognizes expertise in Pain Medicine by awarding the Certificate of Added Qualifications in Pain Medicine. This certificate shows expertise exceeding the physician’s original Board Certification in Anesthesia or Physical and Rehabilitation Medicine. Holders of this certification must pass a rigorous re-certification examination every 10 years. While few practicing pain physicians are primarily trained in orthopedic surgery or neurosurgery, all pain physicians are trained to recognize the serious problems

which merit prompt referral to their surgical colleagues. A well-trained and Board Certified Pain Management physician is a good choice to help understand and relieve your back pain. At our practice, Relievus, most patients are referred from their primary care providers, orthopedic or neurosurgeons, or from a physical therapist or chiropractor. In our practice, we seek to discover why your pain exists and to explain this to you. Only after a specific cause of pain is identified can treatment options be thoughtfully discussed. While some pain management practices seem to offer epidural or caudal injections to all of their patients with back pain, we are highly selective, routinely using a variety of advanced injection techniques to target your specific problem, but only where your pain pattern and physical examination findings support this specific treatment. It is our policy to personally review your MRI or CAT scan images, where available, as part of providing you a comprehensive and accurate diagnosis. We often identify subtle findings relevant to your diagnosis, which alter our recommendations for treatment. Frequently, a bulging or herniated disc is not the cause of your pain. While this condition is often (and wrongly) called “non-specific” low back pain, most patients are found to have pain originating in the spinal joints. Epidural injections do not help this problem. Unfortunately, many patients and some physicians fail to realize that at least half of all back problems will not improve with epidural injections. Long ago, research showed that the practice of administering three identical epidural injections was without merit, but unfortunately, some pain physicians still practice this way. We think that patients deserve better care and better value. With rising deductibles and co-payments, we think that you have a right to expect that your pain doctor will practice not just good, but better medicine.

Plastic Surgery 101 As you begin your research to determine who you will choose as your plastic surgeon, you will probably already have many questions. There are many things to consider when making such an important decision. You want to know as much as possible about your prospective surgeon’s skills, experience, and credentials. Some people don’t realize that there’s a lot more to your total fee than just the surgeon’s fee. There are anesthesia fees, operating room fees, lab fees, and a number of other variables that can affect the bottom line. Ask for a written breakdown along with the total, so you’re not facing any ugly surprises when it’s all said and done.

The truth is, there are several certifying boards for health care professions, as well as surgical societies of which any given surgeon may be a member. Certification specifically in plastic and reconstructive surgery indicates that a surgeon has completed extensive specialized training that not only shows his superior knowledge and experience, but also speaks of his commitment to excellence in the field. Remember, pediatricians are also board certified in their professions, but it is unlikely you want them performing your cosmetic procedure. A ‘weekend seminar’ in liposuction doesn’t replace years of physician training!

2. Where will the procedure be performed?

Some procedures are performed in a hospital, others in an outpatient ambulatory center, and still others are routinely performed as in-office procedures. The complexity of your procedure, as well as any health issues specific to your case, should always be considered when determining where it would be best for your surgery to take place.

3. Do you have hospital privileges to perform this procedure? (and at which hospitals?)

6. What is your complication rate with this procedure?

Although a surgeon cannot violate the privacy of his or her patients, he or she should absolutely be able to tell you how many complications have occurred with your particular surgery, and how many of them have been serious. This can be in the form of an actual count or a percentage, but your surgeon should be willing to provide this information so that you feel comfortable about making the most informed choice for your own health and safety.

7. What is the exact breakdown of fees for this procedure?

Many people are reluctant to ask all of these questions of their surgeon, fearing that they will aggravate or offend the surgeon. Yet asking these questions is important in order for you to be your own advocate in your health care. As a final note, online reviews can be somewhat helpful when evaluating a potential surgeon, but there is a caveat. Some people may be unhappy about how a surgery turned out through no fault of the surgeon and may leave a scathing review. An example would be someone who has poor healing after a procedure due to continued smoking, even if the surgeon has informed the patients that smoking can hamper healing and result in poor outcomes. Always check with the state board of medical examiners to evaluate the true history of patient complaints filed against a provider.

Home Health Care For A Loved One With Alzheimer’s Or Dementia

Interview With Marla Marinucci, Esq. Written by Marla Marinucci, Esq. When Marla Marinucci first started out as a young matrimonial attorney, she felt a strong desire to change the world. After several years and a myriad of challenging cases, she now realizes that was not a viable option. “Family law is constantly evolving and an Appellate or Supreme Court decision can change everything in the blink of an eye,” she adds. It takes a very strong personality to deal with people during the darkest hours of their lives. But Marla has the tools and the experience to help her clients make it through these difficult times. ACW: What do you find to be the most challenging aspect of your profession? Marla: Explaining the law to clients can be very challenging. For example: it’s much harder financially to live separate and apart than it is together. Thus, if during the marriage you enjoyed going out to dinner; getting a massage when you felt like it; or taking vacations two times per year, after the divorce you may not be able to afford those luxuries because there are now two households. Those luxuries are not an entitlement and not something that by law must continue once you and your spouse are divorced. The reality in most divorces is that there is simply not enough money to go around. Litigants have a difficult time grappling with this concept. Also, the stress level that comes with the job can be at times quite high.

MARLA MARINUCCI, ESQ. Marla is the first to say that in her profession, it’s all about getting the “right” result vs. actually “winning” a case. She adds, “Something I tell all of my clients is: There is no winning in family law. It’s about advocating and getting the right and just result based upon the circumstances.” She suggests advocating to the best of your ability without hitting below the belt. Most importantly, she says, “Don’t be afraid to ask your colleagues for advice if you have a difficult issue and you need a second opinion—I still do this almost every day.”

ACW: What advice would you give to someone just starting out in this profession? Marla: New lawyers starting out should network and get involved. Whether that means joining your local bar association or going to local events for young professionals— just get involved. It may lead to a job offer, referral, or both. Also, showing respect at all times for your colleagues, Judges, and court staff, no matter how hard that can be at times, is paramount to surviving in this profession.

ACW: Share with our readers the first three steps a married couple should do when contemplating a divorce. ACW: What is the most important advice you would give Marla: My advice would be to first try and have an adult to a prospective client? conversation with the other party to see if they can agree on Marla: Different clients require different advice. I often anything. Start Good Foot Health At Birth At birth, foot problems may not be obvious until your child begins to walk. Counting the toes is important but also take a look at the shape of the baby’s feet; notice if there is something different. Some problems resolve on their own, others need treatment and early intervention. So let’s talk about a few common findings. Every parent wants to see five perfect toes on each foot, but there are times when a sixth toe will show up. This is called polydactyly. The main problem with an extra digit is fitting shoes properly and in later years the child may feel self- conscious. This extra digit can be removed with a minor surgical procedure as an outpatient. Flatfeet are common and usually inherited. Arches generally become apparent when the child reaches their second or third birthday. This problem may not cause pain now but could affect gait resulting in back or knee pain in

the future or a common deformity such as bunions. Many sneakers have arches incorporated into the product, but if not, an arch can easily be added and, in time an orthotic can be made to supply the arch your child did not develop. In-toeing also known as “pigeon toed” may be cute in a toddler but no one wants to see their teenagers tripping over their own feet because their in-toeing was not addressed early on. Most in-toeing resolves on its own over time. It’s the children who do not grow out of it who need treatment. A foot and leg evaluation by a Podiatrist can help find the cause and appropriate intervention can be started. Treatment may require changes in sitting positions, i.e. do not tuck legs back and out to the sides in a “W” formation, children should sit with legs in criss-cross. Maybe the child has tight tendons where simple stretching exercises with a night splint will help. If the problem arises from a torsion or “twist” in the leg or femur and bracing has not helped, then a surgical evaluation would be encouraged. Out-toeing is the opposite of in-toeing and usually does not correct on its own. The child’s foot will point noticeably out to the side rather than forward as it should. Night splints and orthotics are indicated. A more obvious deformity at birth, and one that was diagnosed with ultrasound in utero, is clubfoot also known as Talipes equinovarus. One or both feet can be involved and this can occur in approximately 1 in every 1,000 births in the U.S. The foot appears to be in a C-shape and tends to point in and down. If the foot and ankle are flexible, casting followed by night splints can manage the problem. Custom made orthotics will follow as well. If the deformity is fixed or rigid, then surgery by a Podiatric Foot and Ankle specialist would be indicated. Flexible or not, this deformity will cause pain and handicap in the future if not diagnosed and managed early.

A Friendly Reminder To My Diabetic Friends… Top Reasons To Check Your Blood Sugar!

• You are hungry. If you are hungry, it’s possible your

numbers, I may ask what is going on in his/her life. If you are feeling stressed over family, job, or other personal matters, consider how that is impacting your blood sugars. You might need to make some serious changes in your life and seek professional help.

blood sugar is on the low side or you are in an official state of hypoglycemia. It’s better to know your number before treating versus eating, without testing, based on hunger.

• You are thirsty. Thirst is one of the symptoms of

hyperglycemia (high blood sugar). Again, it’s always better to test than to assume you just need a glass of water. Testing can reveal a high blood sugar that requires your attention. If you are high, drinking plenty of fluids is important to rehydrate.

• You are tired. One can become tired when levels are high or low, so if you’re feeling especially exhausted, check your blood sugar!

• You are going to drive. Getting behind the wheel with a high or low blood sugar not only endangers you and your passengers, but other people on the roads and sidewalks as well. Consider driving to be a privilege and a great responsibility. If you are too low to drive, it is best to treat your low, check fifteen minutes later to see where you stand. • You are going to bed. Some people are incredibly deep sleepers,

and a high or a low blood sugar (called hypoglycemia unawareness) will not awake them. Be sure to have a good number before you hit the sack!

• You are irritated. One of the tell tale signs of a sugar low is if your alter-personality comes out. The beauty of hypoglycemia, if there is such a thing, is that our bodies do a pretty good job warning us that we need carbohydrates. Symptoms include irritability or nervousness, shaking, a racing heart, sweating, hunger, and more. • You are stressed. When a patient is experiencing a series of high

a beast of diabetes. Initially, exercise does increase blood sugars, but the after effects of exercise generally lower blood sugars, sometimes for days afterward. Talk to your doctor about what an acceptable pre-exercise blood sugar number should be. You also need to figure exercise into your meal planning. Always wear your medical identification, carry an emergency source of carbohydrate, and have a partner or a cell phone nearby when exercising.

• You just do not feel right. Everyone, diabetic or not, has days

when they just feel off. However, people with diabetes must be ultrasensitive to their body’s cues and not blow off a strange or unfamiliar feeling. For example, one might feel anxious up to an hour before a low blood sugar, and other times, hypoglycemia comes on quickly and without much warning. Because you may not have a pattern of common symptoms, check your sugar at the first sign of discomfort or uneasiness. Remember, you hold most of the power in your diabetes management, and of course a key component of living well with diabetes is testing, testing, and testing more. May you find the conviction and motivation to keep your meter handy!

his article serves as a general overview regarding the Pre Trial Intervention (PTI) program. PTI is a supervisory treatment program that was enacted prior to the New Jersey Criminal Code Title 2C being codified. It can be found in NJSA 2C:43-12. The goal is “it is policy of the State of New Jersey that supervisory treatment should ordinarily be limited to persons who have not previously been convicted of any criminal offense under the laws of New Jersey, or under any criminal law of the United States.” The law has gone through many modifications over the years and is constantly changing. When a person is charged with a crime, or in some instances an offense (domestic violence disorderly persons offense), that person may be eligible for supervisory treatment in PTI depending on a panoply of variables. These variables include the PTI Director (court personnel) and their willingness to accept the person into the program, the prosecuting agency and their willingness to accept the person into the program, the charge itself, a victim’s input if applicable, and many other considerations. If a person has received a prior PTI, a conditional discharge or a conditional dismissal, he or she is ineligible for this type of supervisory treatment. NJSA 2C:43-12(g)(1). Ordinarily, when you are accepted into the PTI program, you DO NOT have to plead guilty to the charge(s) subject to certain exceptions found in NJSA 2C:43-12(g)(3). The accused should consult a qualified criminal defense attorney immediately and make a timely application so as not to delay or prejudice their rights. The fee for the application is $75 and almost always no statement that is made to court personnel can be disclosed to the prosecuting agency nor can it be used in a subsequent civil or criminal proceeding. The maximum term limit that a person can be supervised in PTI is thirty-six (36) months. If a person does well, and upon approval of the program director, the prosecuting agency and its consent, and the court’s approval, the term may be shortened or even terminated early. If a person violates the terms and conditions of PTI, dismissal from the program can occur. Again, hire a qualified criminal defense attorney to motion to the court for remaining in the program. If the term is completed successfully, the charge(s) are

dismissed. However, the charge and/or arrest will still remain on a person’s record and they have to wait six (6) months in order to expunge it. NJSA 2C:52-6(c). Additionally, if a person is rejected from the program, a PTI appeal can be filed arguing that there has been a “patent and gross abuse of discretion” in rejecting the individual. An example would help illustrate how the process works. On January 1, Mr. X gets arrested at a local department store for shoplifting $650 worth of merchandise. He ultimately gets caught on surveillance video and the local police arrest him and charge him with third degree shoplifting, a crime with a maximum punishment up to 3-5 years in state prison with various fines and assessments. He is released on a summons (he has no prior record, lives locally, has no other pending charges and was born in the United States). He hires a qualified criminal defense attorney and makes a timely application for the PTI program. The program director agrees to let him in and the prosecuting agency is at first hesitant due to the department store and its concern of recent spikes in shoplifting, but nonetheless acquiesces. Mr. X comes to court on April 1 on an accusation (waives his right to have this matter presented to a grand jury) and the court admits him into the program for a period of 12 months with conditions, such as making restitution to the department store and keeping away from that department store. Mr. X finishes the program in the requisite time frame and the indictable (felony) charge is dismissed. Mr. X then waits until October 1 (6 months after) and moves to expunge his arrest, with which he is successful, and thus avoids traditional prosecution and a criminal record. As a famous proverb tell us: “Everyone makes mistakes in life, but that doesn’t mean they have to pay for them the rest of their life. Sometimes good people make bad choices. Their physical therapy center provides patients with state-of-the-art equipment in a supervised environment and their fitness center is a fun and non-intimidating environment that makes the process quick, easy and beneficial without overcrowding or long waits.

Meet The Staff At Body In Balance Ellie Hagan, PT, MPT, GCS, CEEAA, Owner

Tell us about your background and what led you to begin Body in Balance. As a physical therapist working for corporate-owned physical therapy clients, I was frustrated at the limitations imposed on me. I saw a need for the patient to have access to a supportive post-therapy program under the watchful eye of our professionals so they do not fall back into their prior injured conditions, and I also knew that the Parkinson population was underserved in South Jersey. After years of working at corporate locations, I decided to open Body in Balance (BIB) and the Parkinson’s Life Center of South Jersey in 2005. I am now able to provide my patients with access to movement classes, education, and support as well as premium services from physical, occupational and speech therapists trained in Parkinson specific exercises. Where do you see Body in Balance in the next five years? My vision is to expand the services offered at BIB by continuing to grow our team of high-caliber professionals. Our services continue to grow, including most recently adding Pilates training to our list of wellness programs. Lastly, I see BIB growing into a community wellness center for the mature residents of Atlantic County by offering a wide range of health programs that will include, but not be limited to, educational seminars, art, music, dance and aquatics.

Tell us about your educational background. I started out in the wellness field as a yoga and Pilates teacher in 2005 while working on my BA in psychology. I then went on to earn my MS in occupational therapy with a specialty in lymphedema management. I recently combined my work as a yoga teacher and an occupational therapist by earning my certification as a yoga therapist. What do you like best about working at Body in Balance? What makes BIB unique, and one of the main reasons I wanted to work at BIB, is the continuum of care that we offer. Not only do we have the full spectrum of therapy services, but we also help clients to make fitness and selfcare a daily habit. We are the only facility in the area to offer such a wide variety of services to help clients at all stages of their healing journey.

What do you like best about working at Body in Balance? The most rewarding part about working at BIB is the family environment. Unlike a lot of other places, the patients are always the first priority. It is truly a joy to come to work every day.

Tell us about your educational background. I joined BIB as a physical therapist assistant in December 2016. I graduated from Atlantic Cape Community College in 2002 with an associate’s degree in applied science. To further my education, I received my LSVT BIG certification in January 2017. In addition, I currently serve in the U.S. Army Reserves as a Mortuary Affairs Specialist and hope to soon be able to assist in providing physical therapy services for veterans with PTSD. What do you like best about working at Body in Balance? There are many advantages to working at BIB. Besides the experience of all of the therapists and the equipment for progression, the professionalism presented by the staff and the family atmosphere is what keeps us above the rest.

Tell us about your educational background. I am a graduate of the University of Maryland with a bachelor’s degree in kinesiology. I have been a personal trainer for almost five years. Besides my degree, I have several professional certifications, including Personal Trainer, Sports Nutrition Consultant, Group Fitness Instructor, Kids & Teen Fitness Instructor, and Personal and Child Nutrition Coach. I am certified through the American Heart Association in CPR/First Aid/AED. I am additionally certified as a PiYo! Live instructor and I recently completed a course in sensorimotor training. What do you like best about working at Body in Balance? BIB differs from other healthcare facilities. We are an outpatient total rehabilitation facility. We offer our clientele a wide range of services. From physical therapy to nutrition counseling, we have the tools to help our patrons reach all their health and wellness goals. We also offer an intimate setting and warm, friendly atmosphere. Offering our customers a non-intimidating space to improve their health, BIB is an ideal place for me to share my passion for health and fitness.

The pleasant atmosphere and the opportunity to work with clients after rehabilitation to teach them a safe and effective method of exercise is the best part about working at BIB. It is extremely important for patients to continue their care so they do not reinjure themselves and gain core strength along with flexibility and general body conditioning.

Whether kids are packed into a classroom or messing around on the playground, they do tend to share lots of health problems in school. What can you do about it? And when should you be concerned? When A Mind Departs From Reality; Why And What To Do

If you have a close family member with a psychotic disorder, genetically you are more susceptible to develop one yourself. The accurate differential diagnosis of psychosis requires an experienced well trained medical/psychiatric professional who is qualified to assess the individual’s mental status and determine what simultaneous medical/ neurological tests would be appropriate to rule in or out other possible causes of the presenting symptoms and congruent dysfunction. Tests may include selective labs, neurological imaging studies (CT scan, MRI), X-rays, structured psychological evaluations and collecting as much corroborating input from friends and family members as is available. Treatment of psychosis can include several modalities such as psychopharmacology---principally antipsychotics, which are divided into two categories of Typical and Atypical medications. The atypical medications are newer. All these medications exert their beneficial effects through the decreasing of dopamine activity in specific brain locations. Unfortunately, some of the side effects which may occur are caused by dopamine changes in other brain locations simultaneously. If an individual is also agitated during psychosis, a combination of meds are utilized for a process known as Rapid Tranquilization which is usually done in an emergency room/crisis center setting for safety and close monitoring. Some forms of structured Cognitive Behavioral Therapies have demonstrated improvements with certain forms of delusional content. Early recognition and intervention lead to the best results. Psychosis can be a disorder of its own or a symptom of another disease state. We must differentiate and appropriately treat accurate diagnoses with individualized and focused therapies to address the underlying diseases themselves. The goal is to ameliorate the psychotic symptoms or attempt to alleviate the primary psychotic disorder. All of these requirements should only be done with a well trained, experienced medical/psychiatric professional or team of professionals.

Psychosis is what occurs when a mind’s tetherings to reality become frayed and interrupted. When this happens, substitutions can be inserted. These may take the form of delusions, which are mistaken beliefs. Commonly, they come in the form of paranoid ideations---irrational fears that can get to extreme levels and are the basis for bizarre, even dangerous behaviors. These delusions can be about literally anything. Other symptoms of psychosis can include so called positive ones, like hallucinations. These are generally psychiatrically caused. Most commonly these occur in the forms of auditory or visual hallucinations. Gustatory, olfactory and tactile hallucinations are usually the result of other more serious neurological/medical diseases. These are usually misinterpretations of sensory signals from any of our five senses. Tenth Annual Active Adult & Senior Expo at the Katz JCC in Margate September 28, 2017 A Lifestyle, Health and Active Aging Expo for Adults 50+ The Milton & Betty Katz JCC and the Greater Atlantic City Chamber announce the return of the Active Adult & Senior Expo on Thursday, September 28, from 10 a.m. to 2 p.m. at the Katz Jewish Community Center, 501 N. Jerome Avenue, Margate.

The Milton & Betty Katz JCC and the Greater Atlantic City Chamber Senior & Adult Expo in partnership with AtlantiCare has become the largest active adult health and wellness exhibit in southern New Jersey. Its focus on lifestyle and active aging for baby boomers and seniors has reached more than 3,000 adults, seniors and caregivers each year in its ten-year history. Thousands of area residents will attend this lifestyle, health and active aging expo for baby boomers and seniors for free health screenings by AtlantiCare including blood pressure and heart screenings. The event includes access to information on products and services from more than 60 vendors, a panel of speakers on various topics pertaining to active adults in our community, and interactive demonstrations and classes offered by the fitness staff at the JCC. The morning speaker panel will offer a free continental breakfast at 10:30 a.m. and provide guests an opportunity to ask the panel of professionals questions on the topics being covered.

Erin Intessimoni, Relievus Erin Intessimoni’s dedication to caring for others started early: growing up, she helped care for her father, who suffered from heart disease and diabetes. After his untimely passing, Erin committed herself to finding ways to help care for others. This decision led to pursue a career in nursing. She first attended Gwynedd Mercy College where she received her Associate’s Degree of Science in Nursing and then studied at Thomas Jefferson University for her Bachelor of Science in Nursing. After graduating, Erin was hired as a registered nurse at Thomas Jefferson University Hospital, where she cared for heart failure/heart transplant patients for 11 years. During this time, Erin continued her studies, going on to receive her Master’s Degree as a Nurse Practitioner, which set her on a new career path. Erin was hired at Relievus treating patients with chronic pain. Working in tandem with patients’ physicians, she helps determine the best treatment to meet their needs. Leah Schwartz, Owner/Instructor of Breathe Studio, Barre and Pilates Leah’s journey started with her love for Barre. She connected with the fluidity of the movements and how her body felt. Pilates was a natural progression. The more the she practiced, the more she desired to perfect her skills. In 2013, Leah began her training and certifications in Barre, and in 2016 she completed her reformer certification from Balance Body. Throughout her education she has collaborated with mentors of varying practices that have diversified her technique and knowledge, and molded her abilities as an instructor. As any instructor will tell you, it takes a few tries before you find where you belong. After working for multiple gyms and studios, gaining as much experience and knowledge as she could, Leah began working at Breathe Studio for Marlena Baylinson. Little did she know when she accepted this once a week class, Breathe would become her forever home. Inspired by her time as a hospice volunteer, Lisa Nguyen moved on to pursue a career in home health care and started an internship at BAYADA’s Linwood office during her senior year of college. While there, she learned as many facets of the business as possible, making her a valuable asset to her office. Now, three years later, she is a client services manager, supporting the needs of both her clients and their caregivers. “I take pride knowing that my clients are living happily and safely in their homes with the help of our home health aides and nurses. I love hearing from the clients about how much they appreciate the care they receive and how much they love their caregivers.” Nguyen attributes her success to the support she receives from BAYADA. The Best of the Bass

A long, long time ago in the world before I had children, my husband and I would enjoy fishing together in the summer months. We would fish for flounder and weakfish in the Greate Bay inlet and filet our catch right at the dock. One fall the tide was turning and my husband convinced me to bundle up and go out striped bass fishing with him. After we began our first drift with lines in the water, a fish hit my line just as a boat was passing by. My husband instructed me to keep my rod tip down so the other boat would not catch on to the location of potential future fishing grounds. After the boat passed and we brought the fish onto the boat, I was lucky enough to have reeled in my first and last keeper striped bass. I recall asking my husband why he spent so much time out in the water fishing all of the tides from the early morning hours to the middle of the night during striped bass season because I was able to take one trip out and within an hour catch a keeper. Suffice to say that I was able to catch my first keeper because my captain had spent countless hours fishing, braving the elements in the back bays. When we fish with individuals with vast knowledge of the water, they make fishing appear easy. However, there is a great skill to the wide world of fishing.

It is easy to take for granted that fishing is actually a professional sport with high stakes involved, countless hours on the water, and not to mention the tournaments with cash prizes. In the summer months there are many articles about families catching large flounder fish along the Jersey coastline. Then as the tides shift and the weather changes, so do the species of fish. In southern New Jersey, anglers fish for striped bass in the fall and a portion of the winter months. In other parts of the country, bass fishing is a sport with a season as well. Bass can be caught in lakes that also endure various temperature changes that affect the schools of fish that are accessible. As I was preparing my research for this publication, I came across an article in the BassMaster.com website about the first female to place in a co-ed competitive bass fishing tournament. What was so fascinating about the article was that it was dated 2013! The woman who made history back in 2013 is Christiana Bradley. She placed fourth in the tournament that was hosted by Bass Pro Shops and it was noted as the

“best finish ever made by a woman in a co-ed B.A.S.S. event.” The article went on to report that Christiana’s finish was evidence of a “trend” of female fisher people moving up in the leader boards of top finishers. In fact the article further reported a brief history of female bass fisher people and the time line that reflected the period from 1991 to 2010 before the 2013 acknowledgment. As female anglers make their way into the fishing tournament scene, there are many tournaments that exclude females due to the level of difficulty in qualifying. In 2012, In Fisherman reported that professional angler Marianne Huskey had made fishing history when she won the Anglers Insight Marketing (AIM) Angler of the Year award. The article reported that her win was the first time in history that a female won a major professional fishing tournament and Angler of the Year award. The higher level tournaments are by invitation only, which is through a point system of winnings and placements throughout the fishing season. Just to give an idea of how difficult the sport of bass fishing can be, in the 2017 tournament through the Bass Elite circuit, there are no women listed out of 16 fishermen. Christiana Bradley was raised in Bealeton, Virginia, and started fishing at the age of 12. In a feature by ESPN, it was reported that Christiana started competitive fishing with her brother in 1998. Christiana has sponsors that include GEICO Powersports, Triton Boats and Mercury, to name a few. Christiana also is known in the corporate industry as an Internet systems engineer. After 19 years of fishing, Christiana’s passion for fishing is strengthened by the quest to achieve higher rankings amongst her fellow co-ed anglers. She is a trailblazer who has continued to weather the storm in reaching her status as a pro angler. Christiana is a positive role model for young girls and women in the sport of bass fishing by her display of perseverance and skill as well as her ability to maintain a professional demeanor in a sport that is currently dominated by men. Christiana is certainly a female angler to watch as she continues to make history in the bass fishing arena, one catch at a time! Rita King, MSW, LCSW is a full-time mental health therapist and advocate of women’s history. During her undergraduate studies in psychology, Rita developed a strong passion for women’s history after completing classes in political science. As an activist, Rita has raised awareness of the importance of women’s history through lectures, history lessons in elementary and middle schools and creating women’s history displays at local libraries. Rita also utilizes her knowledge of women’s history to empower women and young girls in her current role as a mental health therapist.

Collaboratively Creating Change

The concept of leadership sounds simple, but the authentic practice of it is difficult for many. As someone who leads individuals and organizations, I cannot think of a more important concept. Leaders are not just CEO’s and managers; parents, community members and many others assume leadership roles daily, without even understanding the associated responsibilities. Every great success and failure I have seen as an employee, therapist and human being has been driven by either poor or exceptional leadership and fits into one of the below categories: The ‘dictator’ leader - this leader leads through control and power, rather than encouragement and teamwork. They are usually in it for the money and/or the power; they couldn’t care less about morale, building culture or anything other than their paycheck and exerting control over others. This leader often has no knowledge of the true responsibility of leadership, and even less desire to assume the role. Often overcompensating for an array of other shortcomings, this leader will suck the life out of individuals, cultures and entire organizations. The ‘follower’ leader - this leader has no clue what they are doing, and lacks confidence. They absorb the energy and ideas from those around them and are constantly changing course, with many around them departing, usually for lack of leadership. There is no clear vision, little passion and even less emphasis on growth and development. With a dedicated team around them,

this leader could succeed, but team members desiring a strong leader won’t be able to hold on. The ‘maxed out’ leader - this leader has given up; they are tired, beaten down and going through the motions. Oftentimes these leaders are the worst for morale, as they are one of the only leaders who truly make teams feel hopeless and checked out. When the ‘maxed out’ leader is not in proverbial fetal position, they can often be found complaining about the company, the benefit package, the cafeteria food or any other topic that fosters negativity YouTube ‘Debbie Downer’ for a visual representation. The ‘saddle up’ leader - passion, drive and an ability to foster the growth of those around him/her, the saddle up leader is the driver of change and growth. This leader has buy-in, likeability and the energy to forge forward. Authenticity is a key characteristic of this leader, encouraging comfort & respect from those around him or her. The ‘saddle up’ leader has skills that cannot be learned; the desire, motivation and wherewithal to foster other leaders and bring about long-term, sustainable change. It is likely that you have been surrounded by one or all of these leaders at some point in your life. Being a leader does not mean you have all the answers; it simply means that you are humble enough to ask for them and fearless enough to lead the charge for new, innovative ones. Leadership is a responsibility, not a title, and unfortunately, a responsibility that many are not willing to assume. Start today by being the leader you always wanted and ‘saddle up.’

Halitosis Here are some tips and tricks to fight off halitosis: 1. Practice good oral hygiene. Brushing our teeth and tongue (in

I

t is estimated that 1 in every 4 adults has chronic bad breath, known as halitosis. Oftentimes we are not aware of it, but others certainly are. So how do we determine if we have halitosis? I read about a simple self-experiment that could help you make friends, meet that special someone, or even save a relationship—just make sure no one is looking. Lick the inside of your wrist, wait 60 seconds, and then sniff it. Sulphur salts that are responsible for bad breath will be transferred from our tongue to our skin. While unconventional (and may not identify all types of halitosis), it can solve the mystery of why everyone is always standing far away, offering us breath mints, or making statements such as “I’m bored let’s go brush your teeth.”

Dr. Nina’s What You Need to Know About Exiling Halitosis: Our mouths are breeding grounds for germs Dental plaque describes the colorless, sticky film of bacteria that forms on our teeth. If it is not brushed away, it can cause irritation of our gums (known as gingivitis) and advance into plaque-filled pockets between our teeth and gums (known as periodontitis). Also, our tongues are like a shaggy, sticky carpet that holds onto and traps bacteria that produce odors. • You are what you eat (and smoke). Food particles can easily linger and stagnate in and around our teeth, gums, and tongue, causing halitosis. Additionally, foods are absorbed into our bloodstreams and carried to our lungs, where their odor can be emitted with our breath. That’s why some foods--onions, garlic, certain spices--can result in “dragon breath” until they have passed through our bodies. And let’s not forget that tobacco products not only cause bad breath, but can irritate the gums. • Dry mouth, or xerostomia. Our bodies produce approximately 3 pints of saliva a day in order to clean and moisten our mouths. Saliva neutralizes acids produced by plaque; washes away dead cells that would otherwise accumulate and decompose on the tongue, gums, and cheeks; and prevents drying that irritates the soft tissues in the mouth. Some causes of xerostomia include not drinking enough water, certain medications, salivary gland problems, smoking cigarettes, and mouth breathing. • Certain health conditions. Bad breath may indicate an infection (yeast, sinus, H. pylori); acid reflux (excessive acid can have a sour odor); postnasal drip; diabetes mellitus; liver or kidney disease; a foreign body in young kids (e.g., small toy or piece of food); or certain cancers or metabolic diseases.

Routine dental visits. Seeing your dentist every 6 months allows for thorough teeth cleaning as well as early identification (and, possibly, prevention) of gum disease, plaque, tartar, and cavities. 3. Avoid dry mouth. Drink plenty of water to not only flush down food particles and bacteria, but also to produce enough saliva. 4. Avoid certain foods. Sticky, sugary foods and beverages, as well as onions, garlic, and certain spices, can exacerbate bad breath. And so can tobacco products! Let’s add bad breath to the lengthy list of reasons we should not smoke. 5. Gargle with peroxide. The oxygen in the hydrogen peroxide kills mouth bacteria that can cause bad breath. 6. Low-fat yogurt. Yogurt is a probiotic, meaning it contains bacteria that are believed to be good for you and, in doing so, has the potential to replace foulsmell-producing bacteria in the gut and mouth. 7. Other tricks. Consider chewing on a carrot, celery, or apple. They can help “brush” off and remove stinky bacteria on your teeth; increase saliva production; and decrease bad breath caused by hunger. Also, eating parsley between meals can help--it has antibacterial and antifungal properties. In most cases, these simple tips can stop bad breath. However, if it persists, make an appointment with your dentist or medical provider to determine what is causing it. In some cases, halitosis can be a warning sign of an infection, cancer, or kidney or liver disease.

A Breath for Every Step Over 221,000 Americans are diagnosed with lung cancer each year and over 50% will die within one year without the development of new diagnostic and treatment methods. For the past 12 years, the Every Breath Counts Foundation has taken steps to battle this devastating disease with the “Walk, Run, Survive 5k” event in November, where passionate, local individuals come together to make a difference in the lives of those affected by lung cancer.

On Saturday, November 11, we are hosting the annual Walk, Run, Survive 5k in Margate, NJ. Proceeds from the event are donated directly to three lung cancer-focused organizations:

“We’re a grassroots effort that depends on local support. It’s encouraging to see so many people share our passion for raising awareness of lung cancer,” said Michele Gaguski, MSN, RN, AOCN, CHPN, APN-C, who founded Every Breath Counts in 2006 when she was president of her local chapter of the Oncology Nursing Society. “It’s amazing to look back and see how far we’ve come.” Although lung cancer is the number one cancer killer in America, it receives only a small fraction of research funding compared to other cancers. RNS is proud to be known as an “ All Volunteer “, humanitarian based, Not-for-Profit Organization. More than $ 18 million dollars has been raised & given back to the community since it was established in 1961 by Rose Newman. Our many successful fundraising events & day to day activities are handled with care & creativity by our members & community supporters. Thanks to our founders “ Spirit of Volunteerism”, RNS is able to “ Make A Difference “ in the fight against cancer & heart disease. Through its’ generous donations to our local hospitals & medical facilities, RNS is able to help those who need it most. The RNS Membership & its’ fundraising efforts benefit the region’s hospitals at AtlantiCare Regional Medical & Cancer Centers, The Bacharach Institute for Rehabilitation, Shore Medical & Cancer Centers, Cape Regional Medical & Cancer Centers & Gilda’s Club of South Jersey. RNS assisted in bringing Gilda’s Club to our local South Jersey Community by providing the necessary Start-Up -Funding & also is the RNS Children’s Noogieland Sponsor. RNS funded Shore Medical Center’s Early Detection Program, which provides cancer education, screening & treatment to low income, noninsured & underinsured individuals. We also provide Ceed monies for those unable to afford Chemotherapy treatments as well as Transportation Vans, Cars & Golf Carts to area treatment centers.
