Joint Replacement in Outpatient Surgery Centers: Debunking the Myths Written by Fabio Orozco, MD, Board Certified Orthopaedic Surgeon

Hip and knee joint replacement surgery has long been associated with long hospital stays followed by physical therapy in rehabilitation centers, then months of outpatient physical therapy. Today, with advanced surgical techniques, hip and knee replacement surgeries can be performed on an outpatient basis. What’s even more exciting is that joint replacement surgeries can now be performed at stand-alone surgery centers. This reality is due to healthcare systems creating surgical centers that are equipped to perform these procedures.

Myth 1. I must have surgery in a hospital.

Fact: While it is true not all patients are candidates for outpatient surgery centers, there are those who will meet the criteria to undergo surgery in an outpatient facility. Patients who meet the criteria for an ambulatory surgical center procedure are those who are typically younger (less than 70 years old), have few medical comorbidities or conditions, have a healthy body weight of less than a body mass index (BMI) of 35 kg/m2, and have a good support system at home. Like all procedures, your surgeon will discuss what option is best for you based on your individual needs.

Myth 2. A hospital is better than an ambulatory outpatient surgery center for surgery.

Fact: Ambulatory outpatient surgery centers have grown exponentially in the last decade to handle more complex procedures that used to be reserved strictly for inpatient hospital stays. Outpatient facilities have undergone million-dollar makeovers to match the requirements necessary to provide the same level of care equal to that of the hospital setting. The future of surgeries in the hospital setting will still be available to manage those deemed high risk due to other medical conditions.

Myth 3. My insurance says I must have surgery in a hospital.

Fact: Just like determining what hospital or physician you may see based on your insurance plan, your insurance will play a significant role in the decision where your surgery can take place. In most cases, a surgery center is a lower-cost facility compared to a hospital, sometimes by more than 50% less, and therefore is associated with fewer out-of-pocket costs for both the patient and the insurance company. This is a benefit for the patient and the insurance plan if the patient is a candidate for such a facility.

Myth 5. I can determine where my surgery will take place.

Fact: Like all joint replacement surgeries, patients should have a medical evaluation to determine the risk associated with undergoing any procedure. A medical clearance will help determine if a patient is a candidate for a surgery center or if surgery should be performed in the hospital setting. The next vital step in determining whether the patient is a candidate to go home the same day is a good support system. If a person does not have someone who can be with them the first night, it would not be a good idea to have surgery on an outpatient basis. It is imperative to have a good support system to be available in the first 24 hours after surgery to help with medication administration and to insure you are able to ambulate around your home as you may be groggy from anesthesia after surgery. There are also those who fall into the gray area. They are good, motivated patients but have medical conditions that may warrant watching a little closer after surgery. These patients can also have the same-day surgery experience in the hospital and if they are doing well, may be discharged in the afternoon. Dear Readers, I know I am not the only one excited to say farewell to 2020. It has been a year like no other. Although we are not quite “back to normal,” there is undoubtedly more hope in the air these days. During times of great strife, it is so important to try and stay positive. After all, with hard times comes growth and change. We all know the pandemic has wreaked havoc on the economy and has caused illness for so many. Now more than ever, we must create some positivity in our lives. Maybe it’s practicing a random act of kindness for a friend, or supporting a small business. I know I am very much looking forward to what the new year has to bring. The month of February is American Heart Month. Let’s try and raise some awareness about the importance of heart health and urge our friends and loved ones to prevent heart disease. Did you know that 47% of all Americans have at least 1 in 3 key risk factors for heart disease? These days, when it comes to factors we can control, we must do just that! Live a healthy lifestyle and keep your blood pressure and cholesterol levels normal. Also in the month of February we have Valentine’s Day. Send someone you love some red roses, or better yet some chocolate. Did you know that dark chocolate is packed with antioxidants and can improve brain function?

You May Ask What Can My Skin Tell Me About My Heart Health? By Lisa Ledden

Your skin can warn you about possible heart and circulatory disease. Pay attention to your largest organ, the skin, which may indicate problems with the heart or circulatory system. Do you notice swelling in your feet and lower legs (called edema)? It may be telling you your heart function is not working properly. Dermatologists often treat a condition called stasis dermatitis caused by fluid in the lower legs. The skin may become discolored, thickened and is occasionally itchy. Is there a blue or purple color on your skin? This is called cyanosis. Cyanosis is a sign blood isn’t getting enough oxygen or there is a blockage in a blood vessel. This could be due to inadequate heart function, extreme cold, or a pooling of blood in the veins. If you notice clusters of waxy bumps that appear suddenly, this could indicate cholesterol deposits (called xanthelasma). This is often triggered by hyperlipidemia (elevated fats in the blood), thyroid dysfunction, or diabetes. If your nails curve downward at the ends (clubbing), this could indicate heart or lung disease. This indicates chronically low blood levels of oxygen. Pay attention to your largest organ, the skin, and schedule an appointment with your doctor for further evaluation.

One area that has changed drastically is the world of medicine. Now more than ever the use of telemedicine is imperative for countless reasons. Once considered a “disruptive technology,” telemedicine has the benefit of cutting costs and offering incredible convenience and safety. Because of COVID-19, there is a huge uptick in patients and physicians wanting to connect via remote healthcare. While still evolving, telemedicine is a model that fosters doctor-patient and doctor – doctor communication via telecom technologies. Thanks to the powers of technology, you (or your children) can receive expert advice or receive a medical diagnosis without ever leaving your home. In fact, some recent studies have shown that telehealth companies have reported up to 250-700% increase in patient visits. Telemedicine can include a myriad of disciplines including teledentistry, telepsychiatry, and teledermatology. There are also various remote monitoring apps that can send patients health data to providers in real time. You can do a live video or audio consultation. Because of the pandemic, many states have lifted the barriers for delivering health care over the state’s border for properly licensed providers. So essentially, any doctor that is licensed to serve in a particular state can also assist with delivering care in other states by using telemedicine software. Another benefit of this technology is that doctors can use it to meet with patients who live in rural areas or places that specialized care would usually require travelling hundreds of miles. For children who have anxiety about a doctor’s visit, telemedicine offers the best of both worlds. The days of “going to the doctor” and the fear that would conjure up could be a thing of the past. Besides the obvious convenience factor, there are also cost benefits associated with telemedicine. Another benefit is that your child is not a risk of catching (or spreading) any illness in the waiting room. Patient portals allow for easy follow up with your doctor and in some cases your child can be diagnosed right away. For children with special needs, telemedicine is a valuable tool. Special health care needs can include children with autism, cerebral palsy, or any developmental delays. Although many of these professionals depend on interactive play and other kinds of play therapy for their treatment plan, seeing patients in their own homes and comfortable in their environments has proven to be quite beneficial. While virtual visits definitely lack the intimacy these therapists are used too, the tradeoff is that children can be less guarded and more apt to open up when in their own homes. Learning how to pivot when in the throes of a pandemic is something all health care professionals and parents around the world have had to do. During this time it is critical to try your best to maintain healthy routines for your children. Routines create a sense of order in this highly uncertain time. Keep them predictable but also flexible enough to address everyone’s needs. Plantar Fasciitis Not Intense During Exercise

Pain from plantar fasciitis is usually not felt during exercise. However, the stress of running or other weight-bearing activity aggravates the inflammation, so your pain may be worse after your workout or the next morning. Running, jumping, high-impact aerobics, and dancing are particularly irritating to an inflamed plantar fascia.

Long-Term Dysfunction

Plantar fasciitis is an inflammation of the thick band of connective tissue that connects your toes to your heel. It helps absorb shock as you run or walk, but can become stretched or injured. When the plantar fascia becomes inflamed, you feel stabbing pain that’s worst in the morning and can interfere with daily activities, too. The recommendations and guidelines are clear. All women over the age of 21 should have an annual gynecological exam. This is considered routine, and it is independent of sexual activity. I continue to recommend that women perform monthly self-breast exams (SBE). A woman should start her SBE when mature enough to perform such an exam or by the age of 21. There are several different ways to perform an SBE. Each differs in technique, but all are equally effective. If you are not aware of how to do an SBE, we can instruct you in the office. You can also visit sites like the American Cancer Society or Web MD, which are helpful. Doing the exam empowers women to take charge of their health care and allows them to become familiar with the breast tissue. Knowing the feel of the breast tissue will help one distinguish what has been there, what is new, what may be abnormal, etc. Concerns regarding your SBE should be referred to your gynecologist for prompt evaluation.

Annual gynecological exams should consist of a review of medical and surgical history, medication history, sexual history, menstrual, STD, and urologic history. It would also include the examination of the breasts and abdomen, genital, and vaginal areas. I also examine the patient’s social and marital history. An important part of this exam is the psycho-social aspect of women’s health care. I try to provide an open and free-flowing environment that allows a patient to discuss other issues and aspects of her life. The annual exam allows the time to listen and provide the care each woman deserves. Annual pap screening is recommended to begin at age 21, independent of sexual activity. The pap is done with the annual gynecological exam.

For all women ages 21 to 25, testing for gonorrhea and chlamydia are part of the exam. In this age group, education is a key component of the visit. I try to really discuss with each young woman the issues of pregnancy prevention, STD protection and prevention, available human papilloma virus (HPV) vaccinations, and

an awareness of her body. There may be many other psycho-social issues that surface during this time. Talking about sex in particular may be hard and uncomfortable for a mom or dad to do or for a girl to hear. But nonetheless it is very important. It may be easier to hear it from her doctor. These topics are very important for women in this stage of their lives. I always welcome and encourage women younger than 21 to have a visit with me prior to that age. Ideally, somewhere between 13 and 15 years old is a good starting point. At this visit, a young woman is introduced to the idea and concepts of a gynecologist and the care provided. I keep it upbeat and positive, and base the conversation and details on the level of maturity of each individual. I try not to scare or overwhelm them on their first visit.

Women 30 and above should have HPV testing done with their pap.

I review routine health and screening recommendations. I also take a family cancer history to attempt to identify patients who are at increased risk of breast, ovarian, uterine or colorectal cancers. If patients are identified, I then review available testing for genetically linked cancers of those areas, many of which can be performed in our office. At 35 to 39, I recommend a baseline mammogram. This allows the radiologist a reference to compare future mammograms, which should begin annually at age 40. I also screen women for HIV and vitamin D deficiency. It is recommended for all women ages 13 to 64 to have annual HIV testing, independent of risk factors. This is to identify asymptomatic HIV-positive persons. Annual vitamin D testing is done since low levels have been associated with an increased risk of colon and breast cancers. Testing allows me to identify, educate and potentially prescribe certain supplements. As an aside, the recommended daily requirement for vitamin D is 800 IU. This is available over the counter as vitamin D or D3. I generally recommend the vitamin D3.

For women ages 65 and older (or younger with certain risk factors), a bone or DEXA scan is recommended to diagnose osteopenia and osteoporosis. Early detection leads to early intervention and the hope of avoiding

fractures related to thin bones. As a woman, you’re probably the general or commander-in-chief of your home. There is a tremendous amount of work you do to run a smooth operation. You take care of everyone else first. Realize that you need to take care of you! You need to stay healthy to do all that you do. ACW: What is the most rewarding part about your profession? Dr. Carfagno: The most rewarding part about my role is the delivering of new life into the world. It makes me a part of one of life’s most memorable events.

ACW: What is the most challenging part?

Dr. Carfagno: It can be challenging to balance the demands of taking care of my patients twenty-four hours a day and balancing my own family life. Making time for my wife and children, while being there for my patients can get tricky.

ACW: What sets your practice apart from others like it?

Emotional Investing “Only when you combine sound intellect with emotional discipline do you get rational behavior.” - Warren Buffet We like to think that since the advent of modern portfolio management practices, investing in stocks and bonds has become a cerebral, analytical process with no room for emotion. The truth is that most investors, even institutional investors, are buffeted (no Pun intended) by emotional turbulence from time to time, and that truth is reflected in the volatility of the financial markets. But if a little emotionalism when it comes to investments is unavoidable, too much emotion can be hazardous to your wealth. Here are four symptoms of problem emotions, financial behavior that is inconsistent with sound investment practice. • Fear of loss. Investors are generally motivated by fear or by greed. Behavioral scientists have learned that, for many people, the pain of loss is larger than the sense of satisfaction from a gain of the same size. Similarly, some investors will accept larger risks in order to avoid a loss than they will in seeking a gain. Taken to an extreme, fear of loss leads to investment paralysis. An excessively risk-averse investor may park funds in ultra-safe, low-yielding bank deposits or short-term Treasury securities until a decision is made, accepting long periods of low returns. Or winning investments may be sold off too quickly in an attempt to lock in gains, while losing investments manage to stay in the portfolio indefinitely. • Following the herd. It’s difficult to be a contrarian, to find value that everyone else has overlooked. Many people find it easier to go with the crowd, to own the current hot stock or hot mutual fund. At least that way, if the investment does poorly, one has plenty of fellow sufferers with whom to commiserate. But when “crowd” is defined as one’s family and friends, the crowd’s investment goals may be very different from one’s own. • Hair-trigger reflexes. Markets move on news. In many cases, the first market response is an overreaction, either to the up side or to the down. Sometimes “news” is only new to the general public, and it’s already been reflected in the share price through trading

by those with greater knowledge. The true importance of any news event can only be discerned over the longer-term. Generally, it’s better to watch the market react to news than to be a part of the reaction. Remember that market dips may present the best buying opportunities but they’re also the toughest times, emotionally, for making a commitment to an investment. • Betting only on winners. Some 85% of the new money going into domestic equity mutual funds goes to funds with Morningstar ratings of four or five stars, according to one estimate. This may be one reason that the government requires this disclosure for investment products: Past performance is no guarantee of future results. The disclosure is required because it is true. High returns are usually accompanied by high risks; ultimately, those risks may undermine performance. Abnormal returns, whether they are high or low, tend to return to the average in the long run. Investing on the basis of the very highest recent returns runs a significant risk of getting in at the top of the price cycle, with a strong chance for disappointment. To avoid impulsive decisions that may be tainted with emotion, one needs an investment plan. The best way to moderate the impact of stock and bond volatility in difficult markets is to own some of each. Assets do not move up down in lockstep. When stocks rise, bonds may fall. Or at other times, bonds also may rise when stocks do. The movements of each asset class can be mathematically correlated to the movements of the other classes. Portfolio optimization involves the application of these relationships to the investor’s holdings. Expected returns need to be linked to the investor’s time horizon. Longer time horizons give the investor more time to recover from bad years, more chances to be in the market for good years. We like to say it is time not timing that is most important when developing a portfolio for a client.

We are not called upon to deny the world or escape from it. Instead, we make peace with all that is. Root yourself in faith no matter what conditions you find yourself in at the present moment. It is this great faith that grounds us like an unmovable tree even in the face of the strongest gale. How do we find steadiness in the midst of it all? For Buddhists, the practice of taking refuge is trusting in the three treasures: the Buddha, the Dharma and the Sangha. That unwavering belief in Buddha, his teachings and community, ensures that our practice moves forward in realizing our true nature. We all have the capacity to be awakened beings. This state of being is within us all. Discovering the Buddha within takes determination and a great resolve. When deep faith and unbounded practice are present, we are face to face with our original selves. For many practitioners, the mala beads are a tangible way to engage in an ancient tradition. Picking up the 108 beads allows us to touch an age-old practice. Wearing the mala beads is an effective way of calming the mind and body. Running the hands over each bead in a repetitive way engages the user in a way that soothes. This wordless message is, “Don’t worry about things; worry about the fact that you are worried all the time and address the root of that.” The mala is the teaching itself. The act of a Buddhist monk putting on his/her robe and mala beads is akin to clothing oneself in the truth of the Buddha. Oftentimes practitioners would recite a mantra or chant, “I take refuge in the Buddha, I take refuge in the Dharma, I take refuge in the Sangha.” Namu Amida Butsu is a heartfelt recitation that praises the Buddha’s love and light. Saying Nembutsu sets the minds of those who practice it directly in the presence of the Amida Buddha. It means to think of Buddha. When you are mindful of the Buddha, the Buddha is mindful of you. So it is best not to worry. Buddha will hear you wherever you are, in whatever condition you find yourself. Mala are our delusions. There are six kinds of delusions experienced through the senses. We know them through our eyes, ears, nose, tongue,

mind and body and perceive them in the past, present and future. Sensing them with the two conditions of the heart (pure and impure) adds another dimension as well as with the mind (likes, dislikes and indifference). When multiplied together, we have the 108 mala, which is our source of worry. The Buddhists must care for them as if they were a precious robe or sutra. The string of mala beads contains a guru bead, which is returned to each time you circle around in one direction and then backwards in the other. It is the teacher and the teaching that keep us coming back with every cycle we pray. There is no beginning or end to our delusions as they are inexhaustible. But we vow to vanquish them all. Living in this realm of delusion, we can never arrive in the Buddha’s state of enlightenment. But we still strive to reach it, ending on the threshold of Buddhahood, only to begin again. It is not a fatalistic practice. We receive the teachings of the Buddha with every mala that we say. There is deep wisdom in this simple ritual, as we live in peace in this imperfect world. For every moment of delusion, every act of greed, folly or confusion, there is Amida right beside us, embracing us as we are. Whether we pick up the mala or join our palms together, this is an act of simple faith. All is taken care of and the Buddha embraces us on the spot. Walk like the Buddha, even in the most difficult situation. The Buddha is in you. Have faith in the Buddha within. Take a step for peace and the happiness of all humankind. Take breaths in harmony with your steps and the mind becomes at ease. Each step reinforces your joy and allows for the calm energy of the Buddha to flow through you. Keep that relaxed smile on your lips. Enjoy each step, walking not to arrive but to be present and mindful of everything along the way. There is no strain or struggle. It is without effort and attainment. This is freedom from worry and expectations. Everyone can do it.

The 3 C’s: Covid, Custody & Co-Parenting By Marla Marinucci, Esq.

No doubt these are unprecedented times. Between the Covid-19 pandemic, the upcoming election, recent crazy weather events (a tornado touched down in Ocean City a few days ago), and a slew of other strange events, it has been difficult to find a silver lining in anything recently. I have always tried to find at least one positive aspect in everything, regardless of how negative a situation is. For example, as family law attorneys, something we have been wanting for years has been the ability to file documents electronically. Other courts and divisions have had electronic filing for many years, but not in the family part. Thanks to the pandemic, we now have electronic filing, and it is awesome. It has made life so much easier, and for the most part has had the effect of expediting things. Additionally, while I was initially not a big fan of having court appearances done via Zoom or other digital platform, I cannot imagine ever going back to having all in-person court hearings and am enjoying not paying $10 to park every time I have court in Atlantic City. I also cannot remember the last time I wore a suit, which has been another huge plus. Granted, in no way do I believe that this pandemic has been a good thing. The point I am trying to make is that out of bad, can come good. Let’s talk about custody and the effects Covid-19 is having on coparenting. Take, for example, the Governor’s order to close all schools starting in mid-March. I was so thrilled to learn of many situations of parents (divorced or just not together anymore) who successfully, and without court intervention, co-parented with each other to modify parenting time in light of the new normal: remote learning. In nearly every case of this kind that I heard about where the parents worked things out themselves, the noncustodial parent’s time increased substantially. No doubt there were plenty of cases that had the opposite result whereby one party used the pandemic as a shield in an attempt to prevent the other party from having any parenting time at all. However, I found that such a tactic was immediately shut down by the Court absent certain unique circumstances that warranted a suspension of parenting time to one party.

There are many cases that, due to distance (i.e., the parents reside in different states far apart), one parent is not able to enjoy much parenting time during the school year because it’s either too expensive (requires plane tickets) or there are not enough school breaks to warrant parenting time beyond Christmas break, spring break, and the summer. Even if an agreement states that one party is entitled to all three-day weekends during the school year, that is hardly enough time for anyone, nor is it fair to the child, because if it involves flying, there will be substantial time spent in the airport on two of the three days, and it’s just plain exhausting. With Covid-19 and the advent of remote learning, now is the time for the non-custodial parent, who rarely gets to see their child/children during the school year due to geography, to enjoy more parenting time. This applies to parents who live near each other as well. From what I have gleaned online, most school districts nationwide are offering a few different options for the 2020-2021 school year, including a hybrid schedule that involves a combination of in-person learning and remote learning, as well as an all-remote-learning option. As of the time of this writing, it appears possible that the Governor in New Jersey could once again order that schools in New Jersey close again out of safety precautions for the staff and the students. Assuming there are no parental fitness issues involved, and the pre-pandemic parenting schedule was determined based on each party’s work schedules at the time, and of course the child actually attending school, if one or both parents are now working from home, AND your child’s school is either one that involves two or three days of remote learning and two or three days of in-person learning, or all-remote learning, now is the time to flex those atrophied co-parenting muscles and take advantage of these unprecedented times. It is the child’s right to spend as much time as possible with both parents. More often than not, one parent spends far too much time and energy limiting the other parent’s time with the child for reasons that have nothing to do with the safety of the child, and for the most part are founded in spitefulness. Remote learning can be done anywhere, near and far. A child who lives with Mom and attends school in New Jersey and whose school has implemented an all-remote learning plan can follow that same plan with Dad, who lives in California, and with whom the child may not spend a lot of time due to distance and school. Temporarily modifying a parenting plan does not require a court order provided both parties agree. This is what co-parenting is all about.

Affairs Of The Heart By Charles E. Meusburger, MD, FASAM

The heart in Greek Mythology has been proclaimed as the seat of affection, emotion and even of the soul. The Greeks had far too much time on their hands and thought about everything. The heart is also the site anatomically responsible for life itself. It’s power, prowess and regularity allow all of us this precious gift. As an organ, it’s not the most sophisticated. The brain and the kidney are far more complex, subtle and influential; however, the heart rules. You have two kidneys. You can survive, albeit not very well, if you’re brain dead, but not if your pump fails. Cardiovascular disease has been the leading cause of death in the United States and in the world for decades despite raised awareness and better treatments. Its lost this dubious and ominous distinction recently to COVID-19 as the leading cause of death. There are a great many factors and vectors that may converge on this important life-essential little muscle; things ranging from genetics, life style, poverty, exercise, diet, location, other diseases, viruses, substance abuse, toxins, and yes—psychiatric disorders. Some examples of psychiatric disorders that may impact heart health are: depression, PTSD, all the anxiety disorders, loss and grief, substance use disorders, and psychosis. Of course, that list is not all-inclusive, but they are among the more frequent culprits. Depression and anxiety disorders combined, are the two most significant and prevalent mental health disorders causing disabilities worldwide. An estimated nearly 600 million people suffer from these two disorders. And the siege of COVID-19 has definitively increased both of them exponentially. The stress or stressors that are the consequences of depression and anxiety alone

find their way to many physiological sites to wreck havoc, dysfunction, and even death. Stressors are commonly created by a human mind when we face challenges that fan preexisting fears. Defensively and unconsciously, we embellish them and add an extra layer of negative power to the challenges that morphs them into new serious threats until proven otherwise. All of this requires more and more defense against the semi-fabricated and expensive control to defend insidious and hypothetical “what ifs.” Psyche and soma are not separate ‘city states’ under differing governance, but rather two parts intimately and intricately connected to the same whole; each equally and reciprocally affecting the other for benefit or detriment. Stress or stressors eventually, if left unchecked, are clear and obvious threats to our physical homeostasis. Negative psychic forces will generally impact pre-existing physical sites that already have some degree of compromise. Most typically, assorted GI disturbances, exacerbated lower back pain, increased migraine or tension headaches, sleep alterations and yes, cardiac disease; for example: arrhythmias like A-fib, angina,

coronary artery disease, cardiomyopathies, endocarditis, mitral valve prolapse, pericarditis, myocardial infarction (MI-“heart attack”) and over time, CHF (Congestive Heart Failure). Another example of the negative effect of stress or stressors is if a person has a major cardiac intervention, like a cardiac bypass or transplant, and has pre-existing depression, they are more likely to have a 40% decrease in a positive outcome because there is increased morbidity and mortality in such cases. Our hearts serve poetically as the repository of love, affection, compassion and warmth. It is also the physiological engine that electrically pumps our life’s oxygen rich fuel source throughout our bodies’ vast circulatory highway. Clearly, it’s more than essential to protect and preserve the heart’s good health and function. Hence, to that end, a healthy mind will facilitate and support a healthier heart. In order to protect and preserve the integrity of our hearts, eliminate ‘toxicity’ as much as possible, such as toxic people, things and places. Prevention, as in almost all noxious scenarios, is far better and efficient than repair or rescue. Hence, repetition and reinforcement for heart health is always in order. First, EXERCISE, EXERCISE, EXERCISE! Human well-being is in large measure achievable by fulfilling our internal designs, for instance, move the body and stimulate the mind for growth; explore, don’t settle; seek self-modulation and balance; treat any significant psychiatric issue as soon as possible to remission; take nothing for granted, especially your health. We all seek to be “connected.” We are the master species according to some theorists, but our fragility is pervasive and our needs, endless. Be kind to yourself and extend some empathy to others and watch and feel your heart beat well and a little longer and more enriched into that ‘goodnight.’

I know you want that too. The question is, are you committed to it? Every day? According to dictionaries, the word ‘commitment’ is the state or quality of being dedicated to a cause, activity, relationship or anything else. Bottom line, you want to be healthy in 2021? It’s possible – if you make a commitment to it.

Your Health Starts Here – Your Commitment: Above all, making a commitment to yourself to be healthy takes just that – commitment!! You can be as educated on weight loss, nutrition, hydration, sleep, stress relief, active lifestyles as you’d like, but unless you make a commitment to being healthy and making good choices, you won’t be able to accomplish your goals and reach your optimal health. It’s a Journey: Your commitment to your health means understanding that it’s a journey that begins with a single step, each day. It means understanding that a lasting commitment is more than a week or a month or any secret, magical number of days. (It’s actually even more than a year. It’s plural. It’s years.) In 2021, each day, commit to take your health to the next level, no matter your age. And avoid quick fixes (that one key object – supplement, shake, formula – that claims it will magically transform your life). A lasting commitment is comprised of small steps leading to milestones that you will overcome and attain healthy results.

Understand: Educating yourself (or in some cases re-educating yourself ) in each area of the 7 Touchstones is important. There are countless wives’ tales and myths in the atmosphere. You’d be surprised how medical science has moved on from yesterday’s advice. By taking the time to properly educate yourself on food nutrition, sleep hygiene, stress management, relaxation tips, hydration, etc., you’ll be much better able to choose efforts (and develop habits) that are right for your health. • Also, be specific. Look again at what the “7 Touchstones of Good Health.” Saying, “I want to be healthy” is vague and difficult to achieve. Or another example is “I want to get quality sleep.” Instead, define precisely what you need for that Sleep Touchstone (quieter environment? new pillow?). Read and educate yourself as you grow in your knowledge about each of the areas. This will help you to make wise decisions. • Write it down and speak it out, regularly. Thinking about or wishing for something is very different than speaking it out and writing it down. When you write it or say it, this helps to bring it to life. It helps to make it real and less likely to be forgotten. • Build trusted support. Invite your trusted tribe to achieve better health goals with you. • Living in the moment. Mindful living is your commitment to the daily practice of being mindful of your decisions, choices and actions – as well as being mindful of your heart meditations. Well-being comes as much from how you approach experiences – how you choose to see and work with (frame) them – as it does from what’s going on around you and what actions you take. Learning how to live healthy in each of the 7 Touchstones of Good Health® can take some time, but it can be a very rewarding experience (and as you grow in your understandings, it gets easier). It starts with your commitment.

American Veterans — Time to Take Action

Nearly one in four Veterans receiving care from VA has diabetes. This is partly attributable to the older average age of Veterans compared to the general US population. The Centers for Disease Control and Prevention estimate that nearly 26 million people, or about 8 percent of the US population, have diabetes. That includes about 11 million persons 65 and older, or about 27 percent of seniors. In the United States, about one-in-four persons with diabetes are not aware that they have the condition. While exact numbers are not available, it is likely that the number is lower for Veterans receiving regular VA primary care. However, many Veterans of all ages are at risk for diabetes because of the high rate of obesity and those who are overweight, estimated at over 70 percent of Veterans receiving VA care.

An individual does not have to achieve drastic weight loss. Losing about five percent of one’s weight will help. In recent studies, a weight loss and exercise program was more effective than medication in both younger and older individuals. The medication was not effective in persons older than 60. The VA’s MOVE! Weight Management Program is available to all Veterans who are overweight or obese and for whom weight management is appropriate. It supports Veterans in developing plans that work for them to lose or maintain weight through balanced diet, physical activity, and behavior change approaches.

For the MOVE! program to be tailored to the individual’s needs, the Veteran can complete a 23 item questionnaire. More information is available at www.move.VA.gov. MOVE! is available in multiple convenient formats including group sessions, telephone-based care, and a new home messaging program called TeleMOVE! which uses the telephone line, provides daily support, and is interfaced with a scale for weekly home weigh-ins. Talk with your Primary Care Team about MOVE! Although the focus is to identify individuals at higher risk for developing diabetes, it is important to remind persons who already have diabetes of the importance of weight loss and physical activity in managing diabetes. Additionally, persons with diabetes or at risk for diabetes should manage other conditions, such as hypertension or high lipid levels, appropriately, and if they have diabetes, they should be screened at regular intervals for early signs of kidney, foot or eye conditions.

Diabetes and Agent Orange

Diabetes mellitus type 2 is one of the diseases VA presumes is associated with exposure to herbicides, such as Agent Orange. Veterans who were exposed to Agent Orange do not have to prove a connection between their type 2 diabetes mellitus and military service to be eligible for VA benefits. Healthy eating habits and exercise can help prevent this chronic disease. Visit the Agent Orange homepage to learn more about Agent Orange and VA benefits. Heart Disease: It Can Happen at Any Age

Heart disease doesn’t happen just to older adults. It is happening to younger adults more and more often. This is partly because the conditions that lead to heart disease are happening at younger ages. February is Heart Month, the perfect time to learn about your risk for heart disease and the steps you need to take now to help your heart. Heart disease—and the conditions that lead to it—can happen at any age. High rates of obesity and high blood pressure among younger people (ages 35-64) are putting them at risk for heart disease earlier in life. Half of all Americans have at least one of the top three risk factors for heart disease (high blood pressure, high cholesterol, and smoking). You Could Be at Risk Many of the conditions and behaviors that put people at risk for heart disease are appearing at younger ages: • High blood pressure. Millions of Americans of all ages have high blood pressure, including millions of people in their 40s and 50s. About half of people with high blood pressure don’t have it under control. Having uncontrolled high blood pressure is one of the biggest risks for heart disease and other harmful conditions, such as stroke. • High blood cholesterol. High cholesterol can increase the risk for heart disease. Having diabetes and obesity, smoking, eating unhealthy foods, and not getting enough physical activity can all contribute to unhealthy cholesterol levels. • Smoking. More than 37 million U.S. adults are current smokers, and thousands of young people start smoking each day. Smoking damages the blood vessels and can cause heart disease. On average, U.S. adults have hearts that are 7 years than they should be. Watch this video, and figure out your own “heart age” by taking this quiz.

Other conditions and behaviors that affect your risk for heart disease include: • Obesity. Carrying extra weight puts stress on the heart. More than 1 in 3 Americans—and nearly 1 in 6 children ages 2 to 19— has obesity. • Diabetes. Diabetes causes sugar to build up in the blood. This can damage blood vessels and nerves that help control the heart muscle. Nearly 1 in 10 people in the United States has diabetes. • Physical inactivity. Staying physically active helps keep the heart and blood vessels healthy. Only 1 in 5 adults meets the physical activity guidelines of getting 150 minutes a week of moderate-intensity activity. • Unhealthy eating patterns. Most Americans, including children, eat too much sodium (salt), which increases blood pressure. Replacing foods high in sodium with fresh fruits and vegetables can help lower blood pressure. But only 1 in 10 adults is getting enough fruits and vegetables each day. Diet high in trans-fat, saturated fat, and added sugar increases the risk factor for heart disease.

4 Ways to Take Control of Your Heart Health

You’re in the driver’s seat when it comes to your heart. Learn how to be heart healthy at any age. Don’t smoke. Smoking is the leading cause of preventable death in the United States. If you don’t smoke, don’t start. If you do smoke, learn how to quit. Manage conditions. Work with your health care team to manage conditions such as high blood pressure and high cholesterol. This includes taking any medicines you have been prescribed. Learn more about preventing and managing high blood pressure and high cholesterol. Make heart-healthy eating changes. Eat food low in trans-fat, saturated fat, added sugar and sodium. Try to fill at least half your plate with vegetables and fruits, and aim for low sodium options. Learn more about how to reduce sodium. Stay active. Get moving for at least 150 minutes per week. You can even break up the 30 minutes into 10-minute blocks. Lisa Ledden has been a nurse practitioner specializing in dermatology since 1996. Before this, she had a broad background in civilian and military medicine. After graduating from the University of Massachusetts in 1985, she was awarded a military commission and accepted into a competitive internship with the U.S. Air Force. As an officer, she practiced trauma and emergency/critical care nursing and was part of a field medicine deployment team. She was awarded the Air Force Distinguished Service Medal in 1991. After seven years of service, Lisa left the military as a captain to pursue a civilian career. She accepted a position as nurse manager of the Heart Group of Oklahoma (the largest cardiac practice in central Oklahoma) and subsequently managed emergency services at Milford Memorial Hospital in Delaware while pursuing a master’s degree in nursing at Wilmington College in Georgetown, DE. At that time, she developed a special interest in dermatology. While continuing to practice in dermatology, she was an adjunct professor in the graduate program at Wilmington College. In 1998, she relocated to Michigan to practice dermatology and expanded her knowledge of cosmetic and surgical dermatology. Nina Braunwald: A Main Artery In The Surgical World Happy New Year! We all have been waiting for this moment with great anticipation. A chance for restored faith, renewed hope and new beginnings. Although many of you may not realize it and may even deny it, COVID-19 has made us quite resilient and resourceful. As we begin the new year and the winter months ahead, it is important to remain motivated Facebook.com. and energized. February is National Heart Month and what better time to honor Dr. Nina Starr Braunwald, the first female to be certified by the American Board of Thoracic Surgery in 1963. Nina Starr was born on March 2, 1928 to Morris and Mary Starr in Brooklyn, New York. Nina attended public schools and had an early interest in working with her hands. Nina had an interest in art and medicine and began her post high school education at the Washington Square College of Arts and Sciences at New York University. Nina’s father was an influence in her life, sparking her interest in the medical field as he was a physician. Nina graduated with a Bachelor of Arts degree in 1949 and immediately transitioned into pursuing a medical degree from New York University’s College of Medicine. In 1952 Nina began an internship and residency at Bellevue Hospital in New York and would continue her work there as the first female surgeon on staff. Nina Starr married Eugene Braunwald, who was also her classmate in college and in medical school. Eugene specialized in the field of cardiology. Nina left her residency and completed a postdoctoral fellowship with a renowned cardiac surgeon from Georgetown University. Nina earned a Master’s degree in Surgery and decided to transfer to Georgetown to complete her residency requirements. During her residency, Nina took a great interest in the field of cardiac surgery. Nina embraced the challenges that came with the training and competition in the field of cardiac surgery. Nina is known to have received training from the chief of the Clinic of Surgery at the National Institutes of Health in Bethesda, Maryland (https://www.ncbi.nlm. nih.gov/pmc/articles/ PMC5408644). In 1968 Nina and Eugene Braunwald relocated to San Diego, California, after Eugene received a job offer. Nina obtained a position as an Associate Professor of Surgery. Nina would later go on to establish the cardiac surgical program at the University of San Diego, California. Dr. Nina Braunwald was not finished adding to her lists of “firsts.” After relocation back to the East Coast, Dr. Nina Braunwald became the first woman to join the surgical faculty at Harvard University in the late 1960s. At the time Dr. Nina was entering into the medical field, let alone as a cardiac surgeon, women were not afforded the same level of professionalism as men. However, Dr. Nina Braunwald kept the focus of her research on the patient, not the politics. Although modest about her career, through extensive research as a cardiac surgeon and with her mentors, Dr. Nina Braunwald developed the “bioprosthesis.” This was a prosthetic valve used to replace the mitral valve in the heart. Dr. Nina Braunwald not only designed the bioprosthesis, she fabricated it too. The first surgery was performed on a 44-year-old female. The significance of the mitral valve is that it regulates the blood flow from your lungs into your left heart ventricle, which is the main pumping chamber of your heart. Dr. Nina Braunwald continued to advance the prosthetic from a cloth-covered mechanical valve known as the “Braunwald-Cutter valve” to the stented aortic homograft, which utilized human tissue to treat chronic thromboembolic disease. Dr. Nina Braunwald was truly a female pioneer in the field of cardiac medicine. She was an incredibly talented cardiac thoracic surgeon who was also a researcher in a field that was dominated by men. Dr. Nina Braunwald had the ability to keep pace with “the boys” and continued to work through all of her pregnancies. It has been reported that she even returned to work just 10 days after delivering her children.

Nina and Eugene Braunwald had three daughters. Nina worked hard to maintain a work/life balance. It is said that she would be a part of the morning routines with her daughters and be home for dinner in the evenings and bedtime. Remember in the 1960s there was no internet or the luxury of working from home, so any additional work that needed to be done after hours, Nina would return to the office after her children were in bed. Nina was known to always put family first. Sadly, Dr. Nina Braunwald departed this life at the young age of 64 on August 5, 1992. Eugene Braunwald established a foundation and scholarships to support women in the field of cardiac surgery in his wife’s beautiful memory. Dr. Nina Braunwald has shown us that women can have an incredibly successful career and a family too! She never gave up and continued to challenge herself in the best interest of others and in medicine. In the pursuit of your new year’s goals and in the best interest of your “heart” (emotionally and physically), I encourage everyone to laugh a little, to dance a lot and don’t forget to breathe!
