When is the best time to intervene to ensure a healthy pregnancy?

Interventions to improve a pregnancy outcome are most effective when implemented prior to pregnancy. Maternal health and wellness allows for healthy conception. In the weeks prior to a missed menstrual cycle, major organ systems are formed in the fetus. Maternal circulation and hormones support the newly conceived fetus and implantation of placenta. Exposure to toxins and infectious agents will also be exposed to the fetus.

What is the best way for a woman to prepare herself for pregnancy?

We make the following recommendations in accordance with our governing professional bodies (American College of Osteopathic Obstetricians and Gynecologists and the American College of Nurse-Midwives). We assure adequate folic acid intake through a diet rich in folate and multivitamins containing folic acid 400 micrograms. We stress avoiding alcohol and illicit substances before trying to achieve pregnancy as both increase the risk for serious birth defects and developmental disabilities. A woman must quit smoking and avoid secondhand smoke as this too can decrease fertility. Smoking during pregnancy limits a baby’s growth and increases risk for premature birth and placental abnormalities. Be sure and wash all fruits and vegetables as pesticides are used on many farms. We also suggest using natural cleaning agents whenever possible.

Choosing The Right Divorce Lawyer By Marla Marinucci, Esq.

C

hoosing the right divorce attorney can make all the difference in your case, both financially and emotionally. People contemplating divorce often think that they need a “barracuda” lawyer in order to “win.” My first response when somebody makes a statement like this is always to say that there is no winning in divorce—plain and simple. My job as your attorney and advocate is to get you the fairest and best result possible based upon your unique set of circumstances. Divorce, in and of itself, is one of the most traumatic life experiences a person can go through. If you think that hiring the quintessential “barracuda” or Ramboesque attorney will get you anything more than additional emotional trauma, combined with an exorbitantly high legal bill, you have been misguided. In choosing your attorney, a good way to start is to ask friends, family and colleagues for referrals. Most important is that your potential divorce lawyer should specialize in family law, as I do, and not be someone who will take anything that walks in off the street. You wouldn’t want your primary care physician performing your root canals, would you? Same holds true for matters of the law. In addition to specializing in family law, I am very proud of the fact that I am a Fellow of the American Academy of Matrimonial Attorneys (AAML), a distinction held by approximately 75 attorneys throughout this State. Moreover, in Cape May and Atlantic County where I practice, there are only four practicing family law attorneys, myself included, who are Fellows of the AAML. It is an honor to be a member of this group made up of the most skilled and experienced family law attorneys in New Jersey. In addition to being a Fellow of the AAML, I have also been Certified by the Supreme Court of New Jersey as a Matrimonial Law Attorney. Again, another distinction earned by very few attorneys (less than 3%) which demonstrates that an attorney possesses above-average skills and knowledge in their respective field of law. In addition to the designations mentioned above that are specific to the practice of family law, I have always believed in improving myself and my abilities as a lawyer. So much so, that I decided to go back to school last year and get my MBA from Rutgers University and expect to graduate in May of 2020. It has been a whirlwind of an experience to say the least, as I

have learned so much invaluable information and concepts about business, accounting, communicating, dealing with conflicts, etc., all of which has helped me grow both individually and professionally. The skills I have learned have improved my ability to represent clients; have taught me to think differently; and also to evaluate a case from many different perspectives as my pre-MBA days. It is upon this platform of constantly learning and improving oneself that I strongly believe sets me apart from the majority of the family law attorneys out there, who are in large part, complacent, and satisfied doing the same thing over and over again. I have also previously written about my own personal experience marrying a man who had been married once before, and who had a child; getting divorced myself, and remaining very good friends with him, his ex-wife, and everyone in between. Having experienced firsthand everything myself about divorce; being a step-mom; and the various relationship dynamics that result, has also given me the very unique ability to relate to my clients and what they are going through. When you have your first consultation, make sure it is in person and not over the phone because you need to know if you are going to have a good rapport with your potential attorney who is going to be counseling and guiding you through a very difficult time in your life. Also, you should expect to pay a consultation fee for a consultation with a family law attorney. As the saying goes, you get what you pay for. If you are insistent that you will not settle under any circumstances and just want to have your day in court, I may not be the right attorney for you. Most people do not realize that the number of divorce cases in New Jersey that actually go to trial is minuscule. I do not know the exact figures but I can confidently represent that fewer than five percent of divorce cases actually go to trial in New Jersey. Moreover, the trial is what’s called a “bench trial,” meaning that it is decided by one Judge, not a panel of jurors. I am a firm believer that it is always in the best interests of the parties and their children (if there are any) that the case settle out of court. While we have outstanding Family Part Judges in Atlantic and Cape May Counties, where I primarily practice, they will tell you that they are the last person who should be making decisions about your future and much prefer that the parties themselves arrive at an amicable resolution of their matter. Divorce is emotionally traumatizing, no doubt. But trials irreparably destroy lives.

Designer Turns Her Baking Hobby Into a Full-Blown Career AnnMarie Cooney did not grow up dreaming of becoming a baker. Visions of sugar plums and intricately decorated cakes were not dancing through her head as a young girl. By trade, Cooney was a kitchen designer, but retired early to raise her daughters. “With three girls, my job was to raise good women,” Cooney says. It was a special request from her daughter that would eventually turn her into a AnnMarie Cooney highly sought-after cake designer and baker. “It all started because my daughter wanted a cake,” Cooney says with a smile. Cooney’s middle girl, was born with health issues that resulted in special needs. So when she wanted a circus cake for her fifth birthday, Cooney made her the best circus cake around. That was 14 years ago, and her girls quickly caught on to their mom’s special talent. At every birthday, they requested a special cake. “It became, ‘what can we do next’ and ‘how can we top that,’ Cooney says. Then she began making cakes for friends. Cooney’s husband had to build an additional kitchen at her house just to accommodate all of the special cake requests. When Cooney’s youngest daughter entered kindergarten six years ago, Cooney decided she wanted to go back to work. “But, I needed to figure out what I was going to do,” Cooney says. Sadly, Cooney’s grandmother, with whom she was very close, passed away during this time. “I was sitting at home, just very sad, and my cousin called me to meet her at the park with her kids,” Cooney says. However, in the midst of grief, she didn’t feel up to it. “My cousin sad, ‘come on grandma would want us to.’ So I went, Cooney says. On the way to the park, she saw the real estate sign. She checked it out and immediately knew that it would be the perfect location for her bakery, A Sweet Memory Cake Shoppe & Tea Room. “My grandmother left each of her grandchildren a small inheritance and it was just enough for me to open my business,” Cooney explains. Cooney spent about five years at that location before moving to another larger space six months ago just two miles down the road in Waretown. A beautiful picture of her grandmother has watched over her since the day she opened. It’s difficult to simply call Cooney’s creations “cakes.” Some designs are so intricate that they look like art. Cooney and her team created a three-dimensional depiction of Van Gogh’s “Starry Night” that captured the essence of this masterpiece with sweet, swirly curves and lines of vividly hued icing. Then there’s the adorable panda birthday cake that could easily be mistaken for a cuddly toy. The list of unique, edible masterpieces goes on and on, That’s because they average a minimum of 30 specialty cake orders per week, mostly by working with different catering and special event venues in and around Long Beach Island. However, they also have a long list of loyal customers. “Some of our customers started with a gender reveal and now we’re doing their fifth birthday cake,” Cooney says. Customer Kathy Helm of Waretown got her grandson’s birthday cake from Cooney close to five years ago. “It was perfect, I brought a couple pictures of what I wanted and they just did their thing,” Helm says. And she has been coming here ever since. “I’m in sales, so I bring my clients treats from here all the time,” Helm says. She’s also brought her granddaughter to Sweet Memory’s princess tea parties. “Our clients really do become our friends,” adds Cooney. In addition to the special-order cakes, A Sweet Memory, has a selection of seasonal cakes, mini desserts, cookie sandwiches and other baked goods readily available daily. Other creative creations include their cake pies; 521 Route 9 • cakes topped with pie filling and decorated to look exactly like a pie. “Our baking is very seasonal,” Cooney explains.

Winter flavors includes a tasted snowball cupcake, which is a chocolate cake filled with marshmallow and chocolate fudge, frosted with coconut buttercream, rolled in toasted coconut, drizzled with chocolate and topped with marshmallows. Or their gingerbread scone with maple cream. Or the orange, cranberry Christmas scone with orange cream. “My grandmother used to always have her ‘lady friends’, as she called them, over for tea,” Cooney explains, adding that she wanted to honor her grandmother by adding a high tea to her bakery. “We squeezed one table in our previous location and began hosting afternoon teas.” They were all so well-received Cooney has expanded her operation to include a full-time tea room. Decorated with a lovely mix of china and flowers, the tea room serves tea, lunch and snacks from 11 a.m. to 4 p.m. Wednesdays through Sundays. They also offer a high tea, but reservations are required. “Our clients have really created the charm of this place,” says Cooney, who adds almost of her china, platters and place setting have been donated by her customers. High tea ($27.95) is served on tiered platters and begins with an individual pot of tea, chosen from their selection of more than 50 teas. Followed by seasonally inspired salad and homemade soup, four varieties or tea sandwiches, freshly baked scones served with their specialty creams, and completed with a selection of their miniature desserts. “Everything from our meats to spreads, our dressings and chutneys, our soups are all made fresh in-house,” Cooney says. Cooney also places an emphasis on healthy fare. “We source organic whenever possible and offer a large selection or vegetarian friendly dishes,” she says. With the extra space the tea room has added to A Sweet Memory Cake Shoppe and Tea Room, Cooney is able to host special events. “We’ve done cake-decorating classes and other ladies events,” Cooney says. They’ve also hosted princess teas, American Girl teas, unicorn teas and more. They even carry a line of tea geared towards little ones. “We even have a dress up section that ladies and little ladies love,” Cooney says. A little help from above has turned Cooney’s one-time hobby into a business that brings customers joys bite by bite. The business that Cooney started out of her home has grown into a full-time operation with a staff of eight. “We work hard and we work a lot, but we really do love each other,” Cooney says. She says they have become a great team and she couldn’t do all of this without them. Cooney’s brotherin-law, Paul Cooney, recently joined the team as chef and baker. There’s also Sarah Rod, the head decorator, and Cindi Reider, head fondant who works in fine detail. “Sarah and Cindi are so talented, they really are the best around, they’re artists,” Cooney says. Together, what they create can easily be considered art, but at the end of the day-or party or wedding or afternoon tea- and after the last bite, their masterpiece has been turned into a sweet memory.

Unfortunately, caregivers of special needs children and adults know all too well the financial and emotional challenges they face now, and often dread the idea of trying to plan for when they’re no longer around. But taking the time to properly plan ahead for a child’s care in the future will help to alleviate some of the stress being felt currently. First and foremost, visit with an estate planning attorney with extensive experience in the field of special needs planning. These are experts in a complex and dynamic field, in which the rules are frequently changing. For loved ones who may be eligible for governmental assistance programs such as Medicaid or SSI, that eligibility could be lost if the child receives a lump sum from a lawsuit or inheritance (for example, 2018 Medicaid eligibility requires that an applicant own no more than $2,000 in countable assets). If you are planning to leave a relatively large sum of money to a child who may currently be or may eventually be eligible for these types of “means-tested” benefits, it’s critical that the family speak with an attorney about creating a third party Special Needs Trust (SNT) (also referred to as a Supplemental Benefits Trust). These specially designed trusts allow those assets to be made available for supplemental needs or

qualified disability expenses, and yet still allow the beneficiary to retain eligibility for certain governmental benefits. If you will be creating a third party SNT during your lifetime or under your will, it is critical to ensure that beneficiary designations for any qualified plan (401(k), IRA, pension, annuity, insurance policy, etc.) are made payable to the third party SNT and not to the disabled child outright. Failure to do so could result in disastrous consequences such as causing the child to become ineligible for valuable governmental benefit programs. You will also want to speak to your tax advisor regarding any special needs planning you undertake. They will be able to provide necessary advice regarding the tax laws surrounding the gifting or bequeathing of various assets to a loved one. If you haven’t already done so, introduce all of your trusted advisors to one another to ensure that everyone is up to speed with the latest planning taking place. Lastly, if a third party SNT is created, it’s extremely important to consider the use of a professional trustee to manage the trust on behalf of a loved one. While many individuals’ first thought is to name a family member or friend as trustee, there are almost always significant drawbacks to that decision. SNTs are extremely complex, and the laws governing them are frequently changing. Individual family member/friend trustees almost never have the necessary time, experience, expertise, objectivity or energy to devote to the role of trustee, and they may not even be available to serve as trustee because of death or disability. A corporate (professional) trustee is not faced with these issues. In this age of rising health care costs, it becomes even more critical to plan NOW for the care of loved ones with special needs. Assembling a team of professionals ahead of time will ensure peace of mind for the family, as they will know that there is a proper plan in place to care for their loved one when they are no longer able to do so.

Back To School Time Already! Common Concerns For Parents – Acne And Head Lice Self-care guidelines

ACNE

Acne affects 85-100% of people at some point in their lives. While it usually begins at puberty, it can start even younger. Acne most commonly occurs on the face, neck, chest and back, where the most sebaceous glands are located. Mild acne consists of a few papules or pustules and comedones (blackheads). Moderate or severe acne has numerous lesions and may have painful cysts and scarring.

Self-care guidelines

• Cleanse acne-prone areas with gentle soaps or cleansers. • Avoid irritants like alcohol, abrasive scrubs or oils that will clog pores. • Mild acne may respond to weak concentrations of benzoyl peroxide or salicylic acid products. • To prevent scarring, do not squeeze or pick acne lesions.

• Use an over-the-counter medication for head lice as directed (if child is < 2 years old, see your healthcare provider). • Wash bedding and clothing in hot water and dry them on a hot \ cycle >20 minutes. • Seal any potentially non-washable items in plastic bags for 2 weeks. Lice will die in 2 days, nits in 2 weeks. • Vacuum any floors and furniture. • Do not share combs, hats, towels or bedding with a person who has head lice.

Making the Most of Fall Harvest, Compliments of Chef Steph Autumn Vegetable Medley with Rosemary and Nutmeg

Serves 6

9 ounce(s) fennel bulb with stalks 2 cup(s) butternut squash cut into 1/2 cubes 1 1/2 cup(s) parsnip cut into 1 inch thick slices 1 1/2 cup(s) carrot cut into 1 inch thick slices 1 tablespoon(s) olive oil 1 1/2 teaspoon(s) rosemary, fresh chopped 1/2 teaspoon(s) salt 1/4 teaspoon(s) pepper 1 dash(es) nutmeg freshly grated 1/3 cup(s) Parmesan cheese

Preheat oven to 425°. Trim tough outer leaves from fennel. Cut fennel bulb in half lengt wise; discard core. Cut each half into three wedges. Combine fennel, squash, and next 7 ingredients (through nutmeg) in a large shallow roasting pan coated with cooking spray. Bake at 425° for 35 minutes or until vegetables are tender, stirring occasionally. Sprinkle vegetable mixture with cheese, if desired, just before serving.

When would a Revocable Living Trust be appropriate?

In my practice, I utilize Revocable Living Trusts to: 1) Create Privacy: When people would prefer that others not be given the opportunity to see what their Wills say (because Wills are public records, recorded when probated, and Revocable Living Trusts are private family documents and not recorded); and 2) Avoid Ancillary Probate: When clients own real estate in states where probate is difficult, especially Florida (so that we avoid the need to probate the Will in NJ and then again in another state, which is required to transfer real estate only). Because State laws vary widely, it is important to work with a lawyer licensed in New Jersey, and because most lawyers are not familiar with the workings of trusts or tax law, a lawyer who specializes in estate planning.

Turning Point

Summer is moving into Autumn. After the assertive yang of Spring and Summer, Autumn’s approach becomes yin-like or one of acceptance and yielding. At a certain point, we can do nothing but let nature take over. Days leading up to the autumnal equinox will show signs of change. We will experience a slow transformation until we are hanging in the balance. Equal hours of daylight and darkness have spiritual meaning. Look back and be grateful for all the hard work of the months preceding this time and honor how far you have come. What have you harvested? Be willing to accept any mistakes you may have made. As you head into the months of darker days, face your own darkness through self-reflection. Be ready to face any demons or negative energies and emotions that reside in the shadows. In this way we begin to transform ourselves. It is a beautiful opportunity to journey inward. What emerges from the dark and cold? Be present for whatever comes up in the exploration of self. Welcome the ebb-tide of the year, a time to walk the boundary between water and land. The shoreline is changing. Think of twilight, sunset and the waning moon. There is a sense of melancholy as energy reverts inward. We cannot stop the tides from going out or Summer turning to Autumn. So we surrender to love, life, and our destiny. In doing so we are vulnerable. We show the treasure of our true heart and the shape of our tender self as we drop like leaves and flow out like the tide. Autumn is a time to pause from doing, to observe, to let thoughts go deep to the heart of being and the heart of mysteries. The equinox is the bridge between yang and yin, activity and receptivity, sea and shore, high and low tide.

September is the ninth month. Nine is believed to be the number of magic and wisdom. In sea-lore, the ninth wave significance is to go far out beyond the reach of help where exiles and seekers go. It invites you to take a walk in the forest. Or maybe stroll along the shore where tide pools form from ebbing waves. Notice what treasures, precious and exposed, are revealed as the tide goes out. The third wave is for courage, The sixth wave is for perseverance, The ninth wave is for surrender.

As we move into nature’s slumber, begin to feed your heart and soul. Take some time for reflection. Slow down with some breathing techniques or pranayama. Alternate nostril breathing or Nadi Shodhna helps balance the hemispheres of the brain. Bring your dominant hand into Vishnu mudra with the thumb and end fingers up and two middle fingers in the palm of your hand. As you inhale and fill the belly, block off one nostril with the thumb and exhale out the other side. Inhale again and block off the nostril with the end fingers, sending the breath out the opposite side. This is a calming and cooling breath designed to wash away any tension or tightness. Then practice meditation in a quiet space in your home. Sit on a cushion for a few minutes attending to your own breath. Watch and notice anything that may come up without labeling or judging it. Begin to turn inward. Let go of the outside world for just a moment. Maybe some space opens up. Sit with it and be curious. Finally, let your yoga practice be slow and organic. What is your body calling you to do? Yin yoga is helpful during this period of transition. Sustaining simple postures for 3 to 5 minutes allows the body to open up. Yielding and accepting become your watchwords. Allowing things to be just as they are in the “isness” of the moment can prepare you for whatever comes your way. As you deepen your practice, you may notice subtle changes in yourself and the world around you. Being in tune with the natural environment is part of the discovery. As you take this “night sea journey,” you will dive into your soul. It may feel like it has no meaning, shape or direction. This human struggle can offer insight into your deeper being. In your

You have to sit with things and, in due time, let them be revealed for what they are. Let yourself be swallowed by the sea or immersed in the waters of the womb. They both have vast potential for life. This night sea journey takes you back to your primordial self, or original self, your self of possibility. You return to the womb of imagination and are always being born. You may find in that darkness a key to your source, the larger soul that makes you who you are and holds the secrets of your existence. Give yourself over to the transforming natural powers that are mysteriously dark. Allow yourself to slip back into the sea. Watch and wonder but don’t try to make sense of it and interfere with the natural birthing process. Just ask: “Who am I? What is this? What do I want? What do I fear?” Without answering, gain insight into your soul. In the dark moments, you experience the beauty of the soul. Beauty is always present, but we don’t always recognize it. You take in beauty with a sixth sense and deep perception. A dark night of the soul reaches deep through sadness, emptiness and anxiety. That revelation is the beauty of a thing. Just as beauty in nature includes storms, droughts and eruptions, so does a person’s beauty includes emotional storminess, dry periods and explosions. Appreciate the dark elements as well as the light ones. Beauty nurtures the soul by serving spirit. In a dark night, beauty allows you a glimpse of liberation as you transcend pain and suffering. Expose yourself to the beautiful and let it do its work. Just trust it and be open to it. It is everywhere. Discover your own essential beauty despite your imperfections and problems. Acceptance is the beginning of genuine and honest self-love, a requirement for perceiving your beauty. This is the foundation of your well-being. Seeing your own being is the discovering of your soul and giving it the care and attention it needs. This is not a love of self but more a love of the vast and mysterious soul. First see yourself for who you are. Then love your soul. Now you can breathe easily.

A vaccine is a biologic preparation that improves immunity to a particular disease. It typically is made from a weakened or killed form of the microbe. Vaccines got their start in the late 1700’s as a way to combat smallpox epidemics that were commonplace at the time and resulted in high morbidity and mortality. Are Vaccines Safe?

Vaccines are very safe. Before a vaccine is released it must undergo multiple vigorous studies looking at efficacy and safety. Usually this takes many years and involves hundreds of thousands of patients. The most common vaccine side effects are fever, irritability, and redness or swelling at the site of injection, pain and interrupted sleep or appetite. These side effects, if present, usually only last 24-48 hours and can be easily treated with cold packs and oral acetaminophen or ibuprofen. We now live in a world where global travel has become commonplace. Your neighbor may be taking a safari trip to sub-Saharan Africa or your aunt could be spending some time in China. In many cases, diseases, which have been eliminated from the United States, are still endemic in another part of the world. Recently measles was brought back to our area from overseas travel, and the best defense against it is the MMR vaccine for our children and ourselves. A disease like pertussis affects an adult mildly but an infant severely. Hepatitis A doesn’t usually make a child too sick but can put an adult in the hospital. If adults are immunized against pertussis, they are protecting themselves as well as others around them. The same is true when children receive their hepatitis A vaccine.

Growing Bones and Ligaments

The immature bones of children are different from those of adults. The “growth plates” in children’s bones do not finish closing until age 15-17 in boys and 13-15 in girls. When stressed, these plates are more susceptible to injury than the tendons and ligaments that support the joints. Podiatric physicians warn repetitive overuse can cause inflammation of the growth plates. They advise parents to promote diverse physical activities for their children rather than one sport.

Injuries and Treatments

Many children suffer from mild “torsional” imbalances, commonly known as in-toeing and out-toeing. Most children outgrow these imbalances without medical treatment. However, if a child has obvious torsional imbalances, he or she may be more susceptible to injury. If that is the case, keep a close eye out for foot and ankle injuries associated with sports activity. Foot injuries commonly seen in very active children include: Ankle Sprains. In older children, stretched or torn ligaments in the ankle, known as sprains, are more common than fractures. A sprain may cause extensive swelling around the ankle just like a fracture. Immediate treatment is crucial to quick healing. A podiatric physician can provide treatment as well as recommend balancing and strengthening exercises to restore coordination quickly. Fractures. Fractures from overuse in child-athletes are commonly seen in podiatric medical offices. Growth plates are particularly susceptible to injuries, but mid-shaft fractures of the bone also occur. If a fracture is not severe, rest and immobilization may be the best treatment. More complicated injuries may require casting or surgical correction. If swelling and pain persist, see a podiatric physician. Sever’s Disease. An inflammation of a growth plate, Sever’s Disease is often felt as pain behind the heel caused by inflammation of the apophysis, a growth center where a tendon is attached to the bone. Rest, ice, and heel lifts are usually prescribed. Shin splints and stress fractures. Shin splints are microtears or inflammation of the anterior leg muscles, as our Achilles tendon pulls in the posterior region of the leg. Again, rest is most important in healing these injuries. The Hol Truth Holistic Health Coach Jon Levine is on a mission to help his clients claim the health and happiness that is their God-given right. After a job in pharmaceutical sales and seeing his father die from Parkinson’s and brain cancer, it was time for him to create change when it comes to true health. CW: What got you into health coaching?

I have always been interested in health, whether it was playing sports or figuring out the right nutrition for my body. I sold high blood pressure medication to over 131 doctors in Florida after college, and although it was a nice job financially, it didn’t feel good seeing what I saw. What we have in this country is a disease management model that isn’t working very well. Ironically, my father was diagnosed with Parkinson’s and brain cancer at the same time. It’s very important to me to do something that I am very passionate about and enjoy, and make a difference in people’s lives.

CW: Tell me about your family.

I am a very lucky guy with a beautiful wife and three beautiful kids ages 10, 8 and 5. My wife, Anya, and I have been married for 13 years. We also have a guinea pig named Cherry and a white rabbit named Nibbles.

CW: What do you specialize in?

Nutrition drives a lot of what makes us healthy, but it’s really so much more than that. I believe strongly in looking at the whole person and that’s why my business name is the Hol Truth Health Coach. I deal with mind, body and soul.

CW: What is health to you?

Everyone is a unique individual with different needs and goals. I have to understand my client’s specific needs and address those needs appropriately. Some people need less stress while others need more sleep. Whatever it is, I am the guide on the side to help them reach their health goals.

CW: What is the best approach to health coaching?

You obviously have to have knowledge in a wide range of health topics. Mine being nutrition, but it’s asking high-mileage questions and active listening that comes before everything else. I don’t tell people what to do because that doesn’t work. Instead, I listen to what they are telling me they need.

CW: Tell me about your The Hol Truth Podcast with Jon Levine.

I have several friends who urged me to start a health podcast. I use a local studio called A Shared Universe in Eatontown and interview doctors, health experts and people who have reversed diseases through what I call lifestyle medicine. I carefully screen guests because I will only bring on people I believe in. I love it and it’s a lot of fun. The goal is to educate, empower and entertain.

A good health coach is someone who truly cares and listens to their patients. When a patient feels safe and comfortable, they will open up and the healing process will begin. CW: Describe your coaching style.

Positive, Positive, Positive. Energy is contagious. If I’m in a bad mood, then my client will take on that negative energy. If I’m upbeat and passionate, they will know they are in good hands.

Continuing Innovations In Treatment-Resistant Depression By Charles E. Meusburger, M.D.

Depression and anxiety have been “pathological bookends” ever since man evolved into homosapien stature. They are most commonly experienced simultaneously with one or the other the most predominant. They also tend to exacerbate one

another, especially if the disease state lingers into a chronic condition or repetitive episodes occur. For example, statistically, if you experience one episode of major depression, you are 50% more likely to have a second episode. If you have two discrete episodes of major depressive disorder, you are 75% more likely to have a third; if you have three episodes, then you are likely, by greater than 90%, to have a fourth. Major depressive disorder is a chronic, remitting/relapsing biological disease, and many forms of anxiety (i.e., panic disorder, generalized anxiety disorder and obsessive compulsive disorder) are biological/genetic disorders that commonly accompany the mood disorders. We are very poorly equipped innately to mange these two “bookends.” Therefore, the current standard of care for these disorders is psychopharmacology in combination with varying forms of psychotherapy. The most efficacious medications are those with a well-understood and accessible molecular base in which the medicine is more specific to its target, thereby decreasing the potential for side effects. In more recent years, these “targets” have expanded so as to not just remain focused on the “triad monoamine theory” of serotonin, epinephrine and dopamine. Other neurochemicals have been isolated and proven to play key roles in these diseases and their biological substrates. One of these is glutamate. A previous article discussed the role of ketamine and esketamine in managing treatment-resistant depression (TRD). Simultaneously, there are ongoing glutamate studies that have shown varied results over the years, and recent technology has shed new light on its role.

Recent evidence suggests that the glutamatergic system likely plays a significant role as a “primary mediator of psychiatric pathology.” The action of glutamate is at the synapses of the cells, where there are receptors that bind to it. Postmortem studies in patients who suffered from bipolar depression and major depression show increased levels of glutamate that may have been a key factor in their depression.

Innovations in Depression Treatment

One objective of science in treating depression is to show that decreasing glutamate in patients living with bipolar depression and major depression will be positively efficacious in reducing symptoms of these disorders. Pharmacological results focused on the action of glutamate in treating depression are leading to new innovations in the treatment of depression. Other than ketamine and esketamine, there have been no significant discoveries in treating depression since the 1980s. Fortunately for the many people who suffer from these debilitating disorders, glutamate modulation is showing promise through drugs such as riluzole, memantine and amantadine.

Have You Had Your Annual Mammogram? Breast Cancer Awareness Month

(BCAM), also referred to in America as National Breast Cancer Awareness Month (NBCAM), is an annual international health campaign organized by major breast cancer charities every October to increase awareness of the disease and to raise funds for research into its cause, prevention, diagnosis, treatment and cure. The campaign also offers information and support to those affected by breast cancer. NBCAM was founded in 1985 as a partnership between the American Cancer Society and the pharmaceutical division of Imperial Chemical Industries (now part of AstraZeneca, producer of several anti-breast cancer drugs). The aim of the NBCAM from the start has been to promote mammography as the most effective weapon in the fight against breast cancer. In 1993 Evelyn Lauder, Senior Corporate Vice President of the Estée Lauder Companies founded The Breast Cancer Research Foundation and established the pink ribbon as its symbol, though this was not the first time the ribbon was used to symbolize breast cancer. In the fall of 1991, the Susan G. Komen Foundation had handed out pink ribbons to participants in its New York City race for breast cancer survivors. A variety of events around the world are organized in October, including walks and runs, and the pink illumination of landmark buildings. In the United States, the National Football League promotes breast cancer awareness by incorporating pink on and off the field, and comic strip artists use pink on one day in October. (1)

What are the key statistics about breast cancer?

Breast cancer is the most common cancer among American women, except for skin cancers. About 1 in 8 (12%) women in the US will develop invasive breast cancer during their lifetime. The American Cancer Society’s estimates for breast cancer in the United States for 2014 are: • About 232,670 new cases of invasive breast cancer will be diagnosed in women. • About 62,570 new cases of carcinoma in situ (CIS) will be diagnosed (CIS is non-invasive and is the earliest form of breast cancer). • About 40,000 women will die from breast cancer After increasing for more than 2 decades, female breast cancer incidence rates began decreasing in 2000, then dropped by about 7% from 2002 to 2003. This large decrease was thought to be due to the decline in use of hormone therapy after menopause that occurred after the results of the Women’s Health Initiative were published in 2002. This study linked the use of hormone therapy to an increased risk of breast cancer and heart diseases. Incidence rates have been stable in recent years. Breast cancer is the second leading cause of cancer death in women, exceeded only by lung cancer. The chance that breast cancer will be responsible for a woman’s death is about 1 in 36 (about 3%). Death rates from breast cancer have been declining since about 1989, with larger decreases in women younger than 50. These decreases are believed to be the result of earlier detection through screening and increased awareness, as well as improved treatment. At this time there are more than 2.8 million breast cancer survivors in the United States. (This includes women still being treated and those who have completed treatment.) (3)

How can National Breast Cancer Awareness Month make a difference? We can use this opportunity to spread the word about steps women can take to detect breast cancer early. Take action to increase awareness about breast cancer and breast cancer screenings. • Ask doctors and nurses to speak to women about the importance of getting screened for breast cancer. • Encourage women ages 40 to 49 to talk with their doctors about when to start getting mammograms. • Organize an event to talk with women ages 50 to 74 in your community about getting mammograms every 2 years. • Display posters about breast cancer screening in your organization’s break room. • Ask health professionals from your local hospital or clinic to share information about mammograms and early detection with your employees or members. • Distribute materials about breast cancer screening at a local health fair. • Partner with local women’s organizations, community groups, and senior centers to reach women ages 40 and older with important information on breast cancer screening. • Host a breast cancer walk with your organization’s members. Nurse Practitioner, Certified Dermatology Lisa Ledden has been a nurse practitioner specializing in dermatology since 1996. Before this, she had a broad background in civilian and military medicine. After graduating from the University of Massachusetts in 1985, she was awarded a military commission and accepted into a competitive internship with the U.S. Air Force. As an officer, she practiced trauma and emergency/critical care nursing and was part of a field medicine deployment team. She was awarded the Air Force Distinguished Service Medal in 1991. After seven years of service, Lisa left the military as a captain to pursue a civilian career. She accepted a position as nurse manager of the Heart Group of Oklahoma (the largest cardiac practice in central Oklahoma) and subsequently managed emergency services at Milford Memorial Hospital in Delaware while pursuing a master’s degree in nursing at Wilmington College in Georgetown, DE. At that time, she developed a special interest in dermatology. While continuing to practice in dermatology, she was an adjunct professor in the graduate program at Wilmington College. In 1998, she relocated to Michigan to practice dermatology and expanded her knowledge of cosmetic and surgical dermatology. Karen grew up in the Philadelphia suburbs and moved to South Jersey when she married her husband John, of 30 years! Dr. Peggy Avagliano Atlantic Medical Imaging Dr. Peggy Avagliano came to Atlantic Medical Imaging (AMI) after completing her fellowship training in Women’s Imaging at the University of California San Francisco. She has been the Head of Women’s Imaging since joining AMI in 2000. Dr. Peggy is President of the AMI Foundation, a charitable organization that is dedicated to enhancing the level of healthcare for community residents by collaborating with local community groups to support programs that address critical health needs. She also serves on the Board of Gilda’s Club South Jersey. Dr. Peggy has been married 30 years to her high school sweetheart, Peter, and has raised four children: Christina, 27; Katie, 25; Michael, 24; and Amanda, 22. Barbara Greenling, DNP, Reliance Medical Group Barbara was born in Trenton, eventually settling in Atlantic County in 2005 while working on her medical degrees. Many years of diverse experience working in the medical field as she continued her education finally culminated in a Doctor of Nursing Practice degree in 2013. She joined Reliance Medical Group in 2014 and now has her own practice in Egg Harbor Township, an accomplishment of which she takes immense pride. Crossroad for Angels: Helping Survivors Overcome Their Past Eve Cortes, founder of Crossroad for Angels, strives to educate, help and inform survivors of sexual abuse and incest. Her Story When I walked into my new apartment for the first time and saw the surreal view from the window, it took my breath away. I walked out to the deck and looked out towards the ocean and watched the waves flow back and forth. I was drawn to the ocean, the sound of the waves and the seagulls as they glided by. It brought me back. Back to a place I had spent years trying to forget. As a child I had always lived by the ocean; the sounds and smells of the shore always brought me back to the abuse. I knew my brother loved me, but was this love? It started off with small things like an innocent game of playing house. I was the mom and he was the dad. “This is what mommies and daddies do,” he’d say, demonstrating with my Barbies how they should kiss and touch. Eventually the toys went away and our game of house turned into him on top of me, touching me in the living room, not uttering a word. The silence was deafening. I knew this was wrong, but he was loving me. This was love. Someone loved me. One morning I went downstairs and saw my brother in the living room, where that awful green velvet couch

was, that couch. I told him I wanted a new teddy bear. He told me that he probably couldn’t get me one on that particular day. That’s when I made my threat. If he didn’t get me the bear, I would reveal our secret to the world. He promised me he’d try. The next night, after I had fallen asleep, I woke up to screams. It was my mother crying and screaming for her son. The neighbor was over and had seen me come down the steps. She took me to her place and sat me down to tell me that my brother had killed himself. For years I blamed myself, but I have finally come to terms with the fact that this was all out of my control. One day I was watching my daughter, who, at the time, was four years old, and it hit me. I was her age when it started, and seeing how innocent and vulnerable she was helped me realize I was not at fault. I couldn’t have been, because I was once her. A million mistakes later and years of therapy have helped me heal. I still have eons of healing to do, but I am a great work in progress. I love my new apartment now. I embrace the past and have learned I cannot change it. Whenever I look out into the ocean and can see where the sky and water meet, I know there is something greater out there guiding me. What I experienced with my brother was not love, it was abuse. I know that now and the ocean no longer reminds me of the pain. It brings me hope. I have finally realized that the only way to forget was to remember...

What Your Doctor Wants You to Know: Eat Your Fruits & Vegetables, Every Day

T

he sunny days are shortening, signaling that fall has fallen upon us. And as we say adieu to yummy summer fruits and veggies—blueberries, cherries, watermelon—we welcome a great variety of tasty fall fruits and vegetables. There is compelling evidence that a diet rich in fruits and vegetables can lower the risk of heart disease, stroke, diabetes, macular degeneration, obesity, high blood pressure and some cancers. However, research reports that most American adults and children are not even close to eating enough in their daily diets. Fruits and vegetables—both fresh and frozen—are powerhouses when it comes to essential vitamins, minerals, and nutrients, as well as fiber and antioxidants. While I have underscored this importance before—it bears repeating! And too, we are entering another spectacular season of pumpkins, pears, and Brussels sprouts. It is recommended that adults consume 1.5-2 cups of fruit and 3 cups of veggies…every day. And did you know that within the array of varieties, colors, textures, and tastes—the vast majority of fruits and veggies do not contain fat or cholesterol. As a result, they are a healthy alternative to salty, fatty, sugary, and processed foods and can help maintain or reach a healthy weight. Let’s take a look at how we can achieve these goals and take a step towards better health.

Dr. Nina’s What You Need to Know: Your Fruits & Veggies This Autumn, Every Day!!

1. Mushrooms. While they grow year-round, most wild, edible mushrooms only appear in the autumn. For being fungus, they have innumerable health benefits: high in protein, particularly for a veggie; low carbohydrate content; and rich in fiber, iron, calcium, and other compounds that are believed to fend off cancer. Research has shown that diets high in mushrooms may inhibit estrogen production. And estrogen is a major factor in the development of breast cancer.

2. Pears. Pears are one of the highest fiber fruits and contain more nutrients

4. Sweet potatoes. I suspect that the saying “the best of

both worlds” was invented with sweet potatoes in mind. They are fun like potatoes, sweet in their own right, while being filled with nutrients. Sweet potatoes are rich in beta carotene; vitamin B6, which helps lower our risk for heart attacks; vitamin C, which can help our immune system fight off icky germs (especially during this time of the year); and iron, which helps with red blood cell production.

5. Citrus. This diverse group ranges from oranges to tangerines to lemons and limes to grapefruit and more. While they are bestknown for being a good source of vitamin C, citrus also contains antioxidants from the flavonoid group. In a study published by the American Journal of Clinical Nutrition, a diet rich in flavonoids decreased the chances of dying from heart disease by 40 percent. While it is not entirely clear as to the reason, it is believed that flavonoids improve blood flow through coronary arteries and decrease blood clot formation and “bad” cholesterol levels. 6. Kale. Over the last few years, this ugly duckling has made a comeback as a

beautiful swan. Some have even called it a superfood because it contains such high amounts of vitamins A, C, and K as well as calcium. Compared to spinach, kale has more vitamin C and its iron is more absorbable by our digestive systems.

7. Cabbage. Like the other members of the cruciferous veggie family (kale, broccoli), cabbage contains plenty of nutrition. Research shows that it can aid with weight loss, promote heart, brain, and skin health, and even help to fend off cancer. In particular, it contains sulforaphane, a compound that bestows the bitter taste characteristic of cruciferous veggies, and, too, blocks an enzyme that plays a role in the growth of cancer cells. Edamame, grapes, green beans, celery, cauliflower, broccoli, and carrots are also noteworthy, well-known fall and winter veggies. Seasonal fruits and vegetables help us embrace the great flavors and nutrients. The bounty of autumn packs a lot of spectacular punch for our taste (and all our senses). As fall cools, we can heat up our meals with these flavorful, fun, and healthy fruits and veggies. Enjoy a trip to the pumpkin patch, an apple farm, explore new recipes, and incorporate these tasty and fun fruits and veggies. And whether fresh or frozen—today and every day - I hope you will eat more fruits and vegetables. You will be glad you did!!

3. Pumpkin. A fall and winter favorite, its radiant orange color results from

the high amounts of beta-carotene, an essential nutrient that helps maintain eye and skin health as well as neurological function. Additionally, it contains plenty of fiber, vitamin K, and even some protein while being low in calories and scarce in fat. And let’s not forget and throw away the seeds. Studies have shown that these yummy seeds may help decrease prostate gland enlargement as well as the risk for kidney stones and depression. One easy-to-implement method to prepare seeds is to wipe them off with a paper towel after removing them, and place them (single

Women’s History Toni Morrison: Literary Legend

It’s not what you say but how you say it. A writer has the ability to captivate minds, young and old, and allows us to escape into a literary world of our own, even if for a brief period of time. It is easy to take for granted the amount of time, energy and emotional exhaustion that goes into writing a book. We order on Pbs.com. Amazon with the click of a button and some may even venture into a bookstore. It is not until we are immersed into the storyline and end up emotionally exhausted from a book’s ending that we can even attempt to fully understand our writer. This publication is dedicated to Toni Morrison, who was a literary legend, editor and professor with a twist of trailblazing depicting the lives of African Americans. Born with the name Chloe Anthony Wofford on February 18, 1931 in Lorain, Ohio to George and Ramah Wofford, Chloe was the second child of four in a family who came from victims of slavery. Chloe would later change her name to Toni as a pen name to identify herself in her writings. Toni was encouraged to read as a young child, and it has been reported that when she entered into an integrated school, she was one of the few NYTIMES.com who had the ability to read, which was not typical for an African American child in the 1930s and 1940s. Toni was privileged to graduate from high school and attend Howard University. After Toni completed her undergraduate degree, she continued on to obtain a Master’s degree from Cornell University. Toni continued her passion for the literary arts through her teaching, first at Texas Southern University, and then she returned to her alma mater to teach English at Howard University. Toni married Harold Morrison in 1958. Together they had two children. It would be only a few short years before Toni would enter into the struggles of single motherhood. Toni’s husband, Harold, was originally from Jamaica and he returned to his home in 1963, leaving Toni with her firstborn and another on the way. Toni, alone with a toddler and pregnant, decided to return to her home in Ohio to live with her family. It would not be long before Toni was back on her feet. After a year living at home, Toni relocated to Syracuse, New York Cnn.com to work for a textbook publisher as a senior editor before moving on to work for Random House. Toni did not only work full time as a mother of two children, she worked full time writing novels and essays, and published her first novel in 1970, entitled The Bluest Eye. By 1977 Toni had published another book, entitled Song of Solomon, which put her among the elite to have received the National Book Critics Circle Award. Toni did not stop there. In 1988 she won the Pulitzer Prize and the American Book Award for her writing in Beloved. Beloved not only was recognized as a best seller, it would later become a movie with the amazing Oprah Winfrey in the cast.

In 1993 Toni received the Nobel Prize for Literature, and in 1996 she was awarded the National Endowment for the Humanities, along with another honorable recognition that included the National Book Foundation’s Medal of Distinguished Contribution to American Letters. In 2012 Toni received the Presidential Medal of Freedom from President Barack Obama, which is the nation’s highest civilian honor. President Obama described Toni Morrison as “one of our nation’s most distinguished storytellers.” Toni’s writings depicted struggles of African American females and mothers along with the trials and tribulations of African American children and, most notably, slavery. Toni wrote fiction and non-fiction along with writing librettos (opera). Toni’s amazing ability to make her characters come alive in her writings allows readers to relate to a world that one can only imagine, and the books’ characters have made lasting impressions on the many who have been faithful to Morrison’s work. Toni Morrison passed away on August 5, 2019 at the young age of 88. Her legacy will continue to live on through her literary works that will continue to provoke readers to think outside the box. Although society is littered with technology, there is nothing better than being able to physically turn the pages of a captivating novel as we anxiously anticipate what will happen next. Through her literary talent, Toni Morrison has given us hope in the face of adversity, has displayed amazing resilience and strength in the face of darkness, and has gifted society with characters that we want bring home to dinner or offer a hug through her ability to make her readers all feel part of the story. In this role, Rita currently supervises a Re-Entry program for incarcerated individuals as well as a community based diversion program for consumers experiencing mental illness and involvement with the criminal justice system. New Research Shows Healthy Lifestyle May Offset Genetic Risk of Alzheimer’s Disease If you have a brain, you are at risk of developing Alzheimer’s—a degenerative brain disease for which there is no cure or prevention. Alzheimer’s disease is the sixth-leading cause of death in the United States. Someone develops the disease every 65 seconds and one in three seniors dies with Alzheimer’s or another dementia. It kills more people than prostate cancer and breast cancer combined. Today an estimated 5.8 million Americans live with the disease. In South Jersey, as many as 85,000 residents are living with Alzheimer’s or other dementia. Barring the development of medical breakthroughs, by 2050 the number of Americans age 65 and older with Alzheimer’s or other forms of dementia could nearly triple. There is, however, hopeful news: Research presented at the recently-held Alzheimer’s Association International Conference (AAIC) 2019 suggests adopting multiple healthy lifestyle habits provide high levels of benefit for brain health and may even offset genetic risks of Alzheimer’s disease and other dementias. Healthy choices for maximum brain health include eating a healthy diet, exercising regularly, not smoking, drinking alcohol just moderately and doing activities that stimulate the brain. Keeping blood pressure within the prescribed ranges has proven beneficial to brain health, too. Notably new research findings suggest combining multiple healthy lifestyle choices may counteract genetic risk of dementia. The Alzheimer’s Association’s U.S. POINTER Study found those study participants who followed four or five healthy lifestyle habits had a 60 percent lower risk of Alzheimer’s disease. Those who combined two or three healthy choices had 39 percent lower risk. While there is no cure for the disease yet, these are all proactive measures everyone can adopt to promote brain health. Start now. It’s never too late or too early to incorporate healthy habits. • Hit the books. Formal education will help reduce the risk of cognitive decline. • Butt out. Quitting smoking can reduce risk of cognitive decline to levels comparable to non-smokers. • Follow your heart. Obesity, high blood pressure and diabetes negatively impact cognitive health. • Heads up! Wear a seat belt and use a helmet when playing contact sports or riding a bike. • Fuel up right. Eat a balanced diet higher in vegetables and fruit. • Catch some ZZZs. Not enough sleep may result in problems with memory and thinking. • Take care of your mental health. Seek treatment for depression, anxiety or stress. • Buddy up. Staying socially engaged may support brain health. • Stump yourself. Challenge your mind. Build a piece of furniture. Study a language. Play strategy games. • Break a sweat. Engage in regular cardiovascular exercise that elevates heart rate and increases blood flow.

The Milton & Betty Katz JCC and the Greater Atlantic City Chamber announce the return of the Active Adult & Senior Expo on Thursday, September 26, from 10am to 2pm at the Katz Jewish Community Center, 501 N. Jerome Avenue, Margate. The Milton & Betty Katz JCC and the Greater Atlantic City Chamber Active Adult & Senior Expo, in partnership with AtlantiCare, has become the largest active adult health and wellness exhibit in southern New Jersey. Its focus on lifestyle and active aging for baby boomers and seniors has reached more than 3,000 adults, seniors and caregivers each year in its eleven-year history. Thousands of area residents will attend this lifestyle, health and active aging expo for baby boomers and seniors for free health screenings by AtlantiCare. The event includes access to information on products and services from vendors, a panel of speakers on various topics pertaining to active adults in our community, and interactive demonstrations and classes offered by the fitness staff at the JCC. The morning speaker panel provides guests an opportunity to ask the panel of professionals questions on various health and wellness topics being covered. Free shuttles will be available from the Boscov’s parking lot in Egg Harbor Township, as well as 55 and older communities throughout Longport, Margate, Ventnor and Atlantic City, beginning at 10am and operating every 30 minutes. Caregivers and younger adults are encouraged to attend to gather valuable information and gain a better understanding of the valuable services offered in our community.
