Is Your Cycle Normal? Submitted by Salvatore Carfagno, DO, FACOOG

Life is filled with surprises! Your period should not be one of these surprises. Heavy Menstrual Cycles

In this edition, I will be discussing the issue of heavy menstrual cycles. For most women, their menstrual cycle occurs about once every 28 to 30 days. The period may last three to seven days. The flow is described as moderate. Menses start somewhere around eleven to fourteen years old. This monthly gift will continue until fifty-one.

Unscheduled Bleeding

Bleeding outside of your normal cycle day or heavy scheduled cycles are considered abnormal. Unscheduled bleeding is vaginal bleeding at times other than your normal menses. This may be patterned or totally irregular in nature. This can be spotting to period like in flow. If the flow is excessive, it may result in anemia (low hemoglobin).

Heavy Cyclic Bleeding

Heavy cyclic bleeding (plain old heavy periods) is when a woman has regular periods with heavy flow and possibly longer duration with more bleeding days. This may be associated with cramps or pelvic pain. Pain may become severe enough and or bleeding profuse enough to change what you do on a daily basis. To be more specific, pain and/or bleeding can keep a woman from her doing her normal, everyday activities. Women may become fearful to leave their home, as they worry about bleeding through clothing in public. This can force a woman to lose work time, kid time, husband time etc. The cost of sanitary products and replacement clothing may also play a role in how this affects a person.

Evaluating Heavy Periods

In this article, I will be concentrating on heavy periods or menses. The evaluation of this problem starts with a

detailed history. The history can provide direction for selecting diagnostic testing. It can sometimes be difficult for a woman to keep an accurate record of her period. I find it helpful to use a menstrual diary. The patient is given a copy of this and asked to complete and return to the office with it. Initial lab testing includes complete blood count, thyroid hormone and a pregnancy test. An ultrasound of the pelvis will enable a noninvasive way to view the uterus and ovaries. If one is over the age of thirty-five, a tissue biopsy of the endometrium or a D&C may be needed. The pathological report will exclude diseases such as endometrial cancer or hyperplasia. If testing revels a diagnosis of benign heavy menses (hypermenorrhea) then a treatment plan can be made.

Medical and Surgical Treatment Options

Treatment options shall be separated into medical and surgical categories. Both options are designed to control cycle duration and flow. Most commonly, birth control medications are used. Combined oral contraceptives are easy to use and provide cycle control. Other contraceptives that can be used are the vaginal ring, patch or the depo provera shot. The intrauterine device containing a contraceptive medication is a good alternative. It is placed by a doctor in an office setting and provides long-term cycle control. With this alternative, about 30% of users do not get a menstrual cycle. The other 70% usually have a light spotting type of period. The patient has no need to take a pill or place a patch. Surgical options can also be considered. Endometrial ablation is the use of various devices to destroy the endometrium. These include a balloon with circulating hot water, circulating hot water, cryotherapy and electrical current. I use the balloon device or the device employing electrical current as my devices of

choice. Usually these types of procedures are performed in an outpatient surgery setting. The procedure itself lasts only several minutes. Post-op course is usually unremarkable and short. Return to activities is rapid, with very little down time. Post-op pain is usually restricted to menstrual-like cramps. The cramps can be controlled with non-steroidal medications such as Naproxen. The response to treatment is varied and highly individualized. Most patients have no further periods. The amenorrhea rate (no periods) approaches 70%. Of those that have a return of their cycle, it most commonly light in flow and/or short in duration. The benefit of this type of surgical intervention is related to the lack of medication side effects. Birth control medications do have some undesirable side effects. The most notable are decreased libido (sex drive) and weight gain. These are real issues for most women.

Have you been suffering with knee or hip arthritis and been putting it off for months or even years? Many women put off surgery because they just don’t feel they have the time or because they are the caregiver for their children, spouse or their parents. Other times we put off surgery out of fear of surgery itself or the idea that recovery will take months to heal. Today’s technology and surgical techniques have made it possible to recover in weeks rather than months. Maybe it’s time to put yourself first this season and give yourself your life back.

The Basics

Arthritis of the knee and hip joint can prevent you from performing the simplest of activities including tying your shoes, getting in and out of car, climbing stairs, and just simply walking. Arthritis is a progressive condition that overtime causes wear and tear of the cartilage. The treatment for arthritis begins with conservative treatment including exercise, physical therapy, injections, nonsteroidal anti-inflammatories, and weight loss. These options can provide relief and prolong the need for surgery until a later time. The advanced stage of arthritis is when the cartilage is almost completely gone and there is nothing left but bone rubbing against bone causing significant discomfort, instability, and range of motion loss.

The Surgical Approach

Today’s approach to joint replacement surgery has changed dramatically over the years. Gone are the days where you spend days in the hospital and weeks in a rehabilitation center followed by intense outpatient physical therapy. Prior to surgery a CT scan is completed to create a 3D template of your entire leg to build a blueprint of your joint. These images are imported into special software that determines the appropriate size implant, precise cuts to be made, and balancing the joint. A total hip arthroplasty, or replacement consists of removing the diseased (arthritic bones) and replacing it with titanium, ceramic and polyethylene components. A direct anterior approach involves making the incision through the front of the hip. This technique minimizes tissue damage and results in less trauma and pain immediately after surgery. Preserving the muscles, maintains the hip stability resulting in minimal risk of hip dislocation and the need for restrictions after surgery. A total or partial knee replacement or arthroplasty is performed with a small incision on the side of the knee. Sparing the quadriceps muscles and tendons making up the knee joint. Preserving the surrounding tissues allows for early ambulation, a shorter recovery, less blood loss, and less intensive physical therapy following surgery.

The reason this is all possible is through a multi-disciplinary approach. From pre-operative planning to discharge, patients are evaluated for medical and social concerns, undergo peri-operative medical screening for patient optimization, educated on expectations, and evaluated prior to leaving the hospital to determine their disposition for discharge.

The Recovery and Beyond

Recovery after joint replacement requires active participation by the patient. Once the surgery is over, rehab begins, but not as rigorous as one may think. Initially after surgery, the first thing you will do is to walk. Yes, you can walk on your new joint merely hours after surgery with no restrictions on weight bearing. You will begin with a walker for stability and safety but can progress to a cane within days. Most people return to the office at two weeks and are walking with just their cane or without any assistive device. Physical therapy begins at the hospital with gentle stretching exercises and simple bending and straightening home exercises. After a total hip replacement, the only recommendation for therapy is walking throughout the day and increasing activity as tolerated. After a knee replacement, the focus is still walking, but includes actively flexing and extending the knee joint. Some patients do not require formal physical therapy due to preserving the tendons and muscles during surgery. Within weeks of joint replacement, patients can return to their previous activity level including gardening, golf, bike riding and dancing. Joint replacement surgery has dramatically improved and now may be the time to for a consultation to determine if joint replacement is right for you.

They just need to be tweaked a bit. I am really looking forward to the holiday season and its clear we could all use a reprieve from the worry and stress of the pandemic. This is a time to exercise our gratitude muscle and take notice of all the ordinary blessings we have in our life. If you are unable to spend time in person with family members this year, be sure and take advantage of technology and set up zoom calls. As we all know, travelling can bring with it risks and the safest thing we can do is stay home. While its tempting to think of all the things we may not be able to do this year, it is more important than ever to try and stay positive. Coolsculpting: Choose Your Provider Carefully!! Coolsculpting has become a popular way to sculpt your body and remove areas of unwanted fat. Because of its popularity, more and more facilities have started to offer treatments. Deciding who does your treatment is an extremely important decision! We often receive phone calls from people wanting to know how much we charge for Coolsculpting. We insist on a free, personalized consultation to give you an honest and reliable answer. Influenza, (also known as the Flu) is a virus that can cause serious illness. Besides a runny nose and cough, symptoms can include high fever, sore throat, body aches, chills, and vomiting. Typically, children younger than 5 years old are at risk of developing flu-like symptoms and can develop very serious health problems. In fact, the CDC recommends an annual flu vaccine for everyone 6 mos. and older. A vaccine can offer your very best defense against the flu and because we are still in a pandemic, this year is more important than ever. Not only does the vaccine reduce the spread of flu to others, it can also reduce flu illnesses, doctor’s visits, missed work, and school days. Flu vaccines are updated each season to protect against the viruses that will be most common. In other words, this season’s vaccine has been updated from last year’s to better match the circulating virus. While flu season is between October and May, it can be contracted anytime throughout the year. Because there are so many different flu viruses, the vaccine is made to protect against three or four viruses that are more likely to make people sick during the season. There are two ways for children to get the vaccine: The Flu shot which is given to children 6 months and older; or the Flu nasal spray which is a spray that goes into the nose. Protection will last about a year so it’s critical to be vaccinated each year. Because babies and young children can become very sick from the virus, it’s important to keep them safe and protected. The flu is sneaky and insidious and can spread like wildfire. Imagine someone with the flu coughs or sneezes near your child. The virus then spreads through the air. Or suppose she touches something (like a toy) that has the virus on it, and then touches her face or eyes. This could lead to infection. Those that have the flu can infect others from 1 day before they get sick, up to 5 to 7 days after. Some serious health problems can be caused by the flu, which include chronic health conditions like heart disease or asthma. In more serious cases, brain problems like encephalopathy, which is a brain disease that can affect the brain’s structure or how the brain works. Pneumonia and sinus problems can also be a factor. If your child does get the flu, offer plenty of liquids. Fever and appetite loss are common with the flu and can lead to dehydration. You will want to encourage your child to rest in bed and slow down. Over the counter medicines to treat symptoms can also help alleviate symptoms but they are to be used at the discretion of your doctor. A cool-mist humidifier is a wonderful investment and can help to relieve coughs and stuffy noses. Honey can help loosen a stubborn cough and can be given to children older than 12 months of age. Usually 1-2 teaspoons will do the trick. While quite rare, some children die from the flu and since 2005 flu-related deaths in children reported to the CDC during regular flu seasons ranged from 37 to 188 deaths. Of course we can’t stress enough the importance of continuing to wash your hands. Since kids often touch their mouths and faces, parents should make sure their kids’ hands are washed with soap and water to remove germs before eating, after using the bathroom, and when they come inside from playing. Also, be sure and keep your kids home from school if they are showing any symptoms.

The 3 C’s: Covid, Custody & Co-Parenting By Marla Marinucci, Esq.

No doubt these are unprecedented times. Between the Covid-19 pandemic, the upcoming election, recent crazy weather events (a tornado touched down in Ocean City a few days ago), and a slew of other strange events, it has been difficult to find a silver lining in anything recently. I have always tried to find at least one positive aspect in everything, regardless of how negative a situation is. For example, as family law attorneys, something we have been wanting for years has been the ability to file documents electronically. Other courts and divisions have had electronic filing for many years, but not in the family part. Thanks to the pandemic, we now have electronic filing, and it is awesome. It has made life so much easier, and for the most part has had the effect of expediting things. Additionally, while I was initially not a big fan of having court appearances done via Zoom or other digital platform, I cannot imagine ever going back to having all in-person court hearings and am enjoying not paying $10 to park every time I have court in Atlantic City. I also cannot remember the last time I wore a suit, which has been another huge plus. Granted, in no way do I believe that this pandemic has been a good thing. The point I am trying to make is that out of bad, can come good. Let’s talk about custody and the effects Covid-19 is having on coparenting. Take, for example, the Governor’s order to close all schools starting in mid-March. I was so thrilled to learn of many situations of parents (divorced or just not together anymore) who successfully, and without court intervention, co-parented with each other to modify parenting time in light of the new normal: remote learning. In nearly every case of this kind that I heard about where the parents worked things out themselves, the noncustodial parent’s time increased substantially. No doubt there were plenty of cases that had the opposite result whereby one party used the pandemic as a shield in an attempt to prevent the other party from having any parenting time at all. However, I found that such a tactic was immediately shut down by the Court absent certain unique circumstances that warranted a suspension of parenting time to one party.

There are many cases that, due to distance (i.e., the parents reside in different states far apart), one parent is not able to enjoy much parenting time during the school year because it’s either too expensive (requires plane tickets) or there are not enough school breaks to warrant parenting time beyond Christmas break, spring break, and the summer. Even if an agreement states that one party is entitled to all three-day weekends during the school year, that is hardly enough time for anyone, nor is it fair to the child, because if it involves flying, there will be substantial time spent in the airport on two of the three days, and it’s just plain exhausting. With Covid-19 and the advent of remote learning, now is the time for the non-custodial parent, who rarely gets to see their child/children during the school year due to geography, to enjoy more parenting time. This applies to parents who live near each other as well. From what I have gleaned online, most school districts nationwide are offering a few different options for the 2020-2021 school year, including a hybrid schedule that involves a combination of in-person learning and remote learning, as well as an all-remote-learning option. As of the time of this writing, it appears possible that the Governor in New Jersey could once again order that schools in New Jersey close again out of safety precautions for the staff and the students. Assuming there are no parental fitness issues involved, and the pre-pandemic parenting schedule was determined based on each party’s work schedules at the time, and of course the child actually attending school, if one or both parents are now working from home, AND your child’s school is either one that involves two or three days of remote learning and two or three days of in-person learning, or all-remote learning, now is the time to flex those atrophied co-parenting muscles and take advantage of these unprecedented times. It is the child’s right to spend as much time as possible with both parents. More often than not, one parent spends far too much time and energy limiting the other parent’s time with the child for reasons that have nothing to do with the safety of the child, and for the most part are founded in spitefulness. Remote learning can be done anywhere, near and far. A child who lives with Mom and attends school in New Jersey and whose school has implemented an all-remote learning plan can follow that same plan with Dad, who lives in California, and with whom the child may not spend a lot of time due to distance and school. Temporarily modifying a parenting plan does not require a court order provided both parties agree. This is what co-parenting is all about.

Thoughts On Retirement “Dare to live the life you have dreamed for yourself. Go forward and make your dreams come true.” – Ralph Waldo Emerson In keeping with the theme of this issue of The County Woman, I am going to share some thoughts (not financial) about retirement, because many of you may be thinking of retiring in 2021. One of the recent changes in approaching retirement has been a shift from a complete exit of the labor force to a partial one. Retiring for many may mean the freedom to work only the amount of time they want, and for work to become an optional part of their lives. This may be because transitioning into retirement can be such a difficult process. Restructuring one’s finances from the lifelong accumulation model into a lifetime distribution phase, and the new worries of running out of funds, may contribute to a piecemeal transition. Another part I want concentrate on in this article is that the retiree will have an abundance of unstructured time on his or her hands. Everyone has things to do on their bucket list, but some of those things may be inaccessible due to travel being restricted or other social avenues being shut down due to the pandemic. Newly available and unfamiliar free time at retirement should be viewed as a special gift you have worked so hard to deserve. Many of the great retirement gift ideas are specifically enjoyable time sinks, and may be more appropriate than ever to consider for you, friends and family getting ready to retire. Here are a few ideas: Gift Idea #1: The Joy of Painting – Startup kit The idea of painting without formal training or classes has long been popular. Bob Ross was one of the most successful of the “TV art teachers” with his Joy of Painting. Bob’s gentle voice and approach created a relaxing environment in which to give it a try, giving amateurs the confidence that they might paint something to be proud of. With his step-by-step instructions, even though every participant is following the same directions, they each create something that is uniquely theirs. Best of all, the startup kit has all the needed materials included, so you don’t have to

hunt separately for brushes or paints or canvas. The kit is available on Amazon and other retailers, though the bobross.com website was under maintenance at the time of this writing. If the hobby works out, more videos are available on different streaming sites, so more paintings could be done after the startup kit is finished. Gift Idea #2: Gear for outdoor hobbies such as golfing, gardening, or hiking Spending time outdoors and staying active and fit become more important as we age. Some retirees actually find that they become more fit than ever before, because they devote time to hiking, gardening, golfing. Initially during the pandemic, golf courses in NJ were shuttered. However, we have now found that being outdoors has a lower risk of transmission, and the courses have now reopened with additional safety precautions in place. Similarly, many state parks have reopened as well. If you already enjoy these hobbies, the gift may not create a new time sink, but it still encourages the activity. Some don’t realize the impact that quality hiking boots can make in terms of the enjoyability of a hike. Stability and support can be the difference between wanting to do the activity again, or shying away completely. Gift Idea #3: iPad or Kindle, preloaded with games and books Getting an iPad or Kindle and preloading it with books or games in their favorite genre provides a platform for further exploration and creates an additional use for free time. If they don’t enjoy video games, choose digital versions of things they may do already, such as jigsaw or sudoku puzzles. Many jigsaw apps allow you to create your own jigsaw puzzles, so you could load in family photos to surprise them. Let me leave you with these thoughts: “Don’t act your age in retirement. Act

Making the Most of Fall Harvest, Compliments of Chef Steph Autumn Vegetable Medley with Rosemary and Nutmeg

Serves 6

9 ounce(s) fennel bulb with stalks 2 cup(s) butternut squash cut into 1/2 cubes 1 1/2 cup(s) parsnip cut into 1 inch thick slices 1 1/2 cup(s) carrot cut into 1 inch thick slices 1 tablespoon(s) olive oil 1 1/2 teaspoon(s) rosemary, fresh chopped 1/2 teaspoon(s) salt 1/4 teaspoon(s) pepper 1 dash(es) nutmeg freshly grated 1/3 cup(s) Parmesan cheese

Preheat oven to 425°. Trim tough outer leaves from fennel. Cut fennel bulb in half lengt wise; discard core. Cut each half into three wedges. Combine fennel, squash, and next 7 ingredients (through nutmeg) in a large shallow roasting pan coated with cooking spray. Bake at 425° for 35 minutes or until vegetables are tender, stirring occasionally. Sprinkle vegetable mixture with cheese, if desired, just before serving.

Serves 4

1 sheet puff pastry, thawed and rolled out into a 14in square. 4 boneless chicken breasts, cooked 1/3 cup herbed soft cheese 1/4 cup fresh parsley , chopped 1 large egg Optional ingredients-chopped spinach, diced roasted red peppers, additional cheese, etc.

Preheat the oven to 400 degress. Roll out puff pastry, cut into four equal sized pieces. Place 1 1/2 tablesoons of herbed cheese in the center of each piece. Top herbed cheese with a chicken breast and then chopped parsley Next, fold the corners of the pastry over the chicken breast. Place chicken pastry seam side down on a baking sheet covered in parchement. Lightly brush the top with a whisked egg. Bake for 20-25minutes or until crust is golden brown.

Call something an obstacle, it is an obstacle. Call something an opportunity, it is an opportunity. You get into a state of mind where you accept whatever is happening is happening. Even the most awful things happen, when you’re centered, you’ll be okay. If not, the most trivial thing will set you off. It has nothing to do with the experience or the circumstance: it is the attitude that is important.

ways in which we avoid the present moment, in which we avoid practice, and in which we resist what is. It is important to understand that this is a process. We move between aspiration and resistance creating a kind of rhythm. There are the inevitable ups and downs of practice. Don’t abandon the practice. Doubt can be a place to get stuck in negative beliefs or a positive force in practice. Use doubt as an opportunity to experience unfulfilled expectations about practice. Learn to surrender to and reside in the physical experience of what doubt feels like in the body, instead of following the story line of negative thoughts. Gradually we feel a renewed sense of hope and direction. Another major obstacle we encounter in practice is a deep seated desire to feel a particular way, whether calm, clear, spacious or simply free of anxiety. We share the illusion that if we practice long and hard enough, we’ll get what we want. The persistent belief that drives us to do whatever we can to alleviate discomfort is counterproductive. We think that if we just practice harder, we’re sure to feel better. But the key point about spiritual practice is that we don’t have to feel a particular way. Nor do we have to be a particular way. All we can do is experience and work with whatever is arising in our life right now. No matter what is going on or how we feel about it, the essence of practice is to honestly acknowledge whatever is happening in the moment and stay present with our experience of it. In this way we can feel true appreciation for life just as it is. Whatever exists in the world is worth experiencing. We can’t ignore situations in the phenomenal world. Take opportunities to seize it on the spot, right here and now. Free yourself from habitual patterns and view the world with as scared. Fully appreciate this moment, seeing beauty and goodness all around. When open to limitless possibilities, you create a sense of harmony with self and the world around you. “Rare, difficult and wondrous it is to be born into this human life.” Wake up to the joy of being alive each day. Awakening to this rare gift of life, we can accept and endure the wide spectrum of human grief and bliss and become a source of happiness for others.

Detours and obstacles are a fact of life. Some arise out of our own psychology and conditioning. Other obstacles are ones that nearly every one faces at one time or another. No matter from where they arise, obstacles are at the heart of practice. But until we see them clearly and how they manifest in our life, it will be difficult for our practice to move forward. Practice begins with an exploration of self. Honestly observing where you are is the first step to leading a life of contentment. How can we move down that spiritual path with wisdom and compassion? It takes courage to become a fearless warrior and face the challenges of the world. See everything as an opportunity to practice and we can handle anything. Not understanding the power of waking sleep is one obstacle to practice. This refers to the state in which we live most of the time, identified with, or lost in our thoughts, our emotion, and our actions. We are addicted to our thoughts and believe that they are the truth. We see through this veil and perceive it as reality. This lack of clarity and presence keeps us from dealing with our emotions. Developing deep insight into the power and magnitude of waking sleep will allow us to recognize the possibilities for inner growth and lead to a more satisfying life. Resistance is a predictable and inevitable way that we move through life. The most basic form of resistance is wanting life to be other than it is. We think and talk about practice rather than actually experiencing our lives. Facing life would mean abandoning our view of how we think the world should be. Putting in the effort takes practice, patience and a willingness to let go. Truthfully speaking, we don’t really want to wake up. But instead we hold on to our beliefs and even our suffering. Afraid of the unknown, we cling to the familiar. Giving into our illusions is easier. Resistance is the ego’s effort to maintain control. Staying Sober During The Holiday Season With the Holiday season quickly approaching, for many of us, our first thoughts are holiday dinners with family and friends: food, drinks, and sharing good times. However, this can also be the time of year that can trigger someone in recovery to relapse. It is easy to get caught up in the mix of all the holiday festivities, and for those in recovery, they may not have the right tools in place. This time can be very trying and detrimental to their sobriety. So, for those in recovery, whether a loved one, or yourself, here are a few things to keep in mind when celebrating the Thanksgiving or Christmas.

First, and foremost, have sober strategies in place If you are in recovery and planning on going to a holiday event or activity that could potentially trigger a relapse, please plan ahead. Possibly go to a Twelve Step meeting before or after the event, and if needed, go to both. It may also be helpful to bring your sponsor or a friend in recovery with you. Remember, an addict alone is in bad company. Have a support system, a friend in recovery that can be “on call” for you to check in with during the event. Finally, always have an exit strategy, which can mean bringing your own vehicle or figuring out the public transportation nearest to you. Whatever you choose, make sure it will enable you to leave if you are feeling tempted or uncomfortable.

Know what your triggers are

It does not matter how long you have been sober. Be aware of people, places, and things. Celebrating the New Year is full of possible triggers. If there is potential for stress or arguments that may threaten your sobriety at a dinner with family or friends, then

difficult decisions may have to be made. You could limit your time to a couple of hours at these events or refrain from attending altogether. Do not be too hard on yourself, or ever feel obligated to attend. You must do what is healthiest for you and your sobriety.

New Traditions

Many people may fall back into old habits when it comes to holiday traditions. One way to avoid this is to create new traditions. Try being of service, step out of your comfort zone into the community, and help those less fortunate. Maybe offer to help serve a meal at a homeless shelter, spend more time with others in recovery and come up with your own holiday traditions. Reach out to a neighbor who may not have family around and share a meal together. The holidays are filled with endless opportunities for spiritual growth by sharing your gratitude and joy with others.

Having loved ones in recovery

If you are lucky enough to have a loved one in recovery, please just be mindful during this time of year. This does not mean you can’t have your holiday dinner or family gatherings and it does not mean that your yearly traditions must stop. All this means is if your loved one chooses to respectfully decline an invite, leave early from a party, or offer a new tradition they would like to start, just be kind and understanding. For some, it is easier to adjust in recovery. Others know that in order to stay sober, they must change a lot of things. Not everyone’s journey in recovery is the same. Just love and support them, maybe even a little extra during the holidays. If you or a loved one is currently struggling with addiction, do not wait. Reach out now!

Thankful, Grateful And Blessed - Success Or Quest? By Gwendolyn Meusburger BSN, MBA JD and Charles E. Muesburger, MD Being thankful, grateful and blessed is a very personal and subjective experience. These states of being are relative to what one feels a strong connection to, and are every bit a part of positivity and light as they are to negativity and dark. The gist of this statement is that it is what makes a person or group of people feel those emotions. What makes one person or group feel delight, glee, happy, thankful, grateful and/ or blessed can be the antithesis of what makes another person or group feel the same exact way; yet they arrive at the same emotional place. What is it that causes different influences to yield the same type of delight even though they may be coming from opposite perspectives and even dark rationales? For example, a person feeding the hungry, taking care of others, smelling beautiful flowers and taking delight in their colors, feeling blessed in their spiritual realms, rescuing animals, teaching others, and assisting those in trouble will feel thankful, grateful and blessed to be able to do what they do. Another person or group may feel delight in taking all of these things away from others because they don’t believe they deserve it; they may believe by destroying certain properties, land and/or people, they are carrying out the “will of their God,” or whoever or whatever they worship or hold in high esteem. The rationale of the people or groups regarding those they wreak havoc upon is that they are evil and should suffer punishment for their beliefs, doctrines, blasphemous behaviors, cultural norms, societal norms, or anything else thought of as unacceptable among such individuals or groups. When such persons carry out heinous acts, they are elated; they feel thankful, grateful and blessed because they have punished those who, in their minds, deserve it. It is just as important to those who carry out such acts to feel positive emotions as it is for those whose acts are the complete opposite and who feel suffering is not desirable or acceptable among humankind. The rationales are different among people and groups who choose either set of actions, among many possible examples, yet they yield the same elating outcome. Why? The answer is the nucleus accumbens. And while the nucleus accumbens is the major purveyor of pleasure and reward, the brain has many pleasure centers. In addition, it is not just the object of pleasure that drives one’s actions, but other areas of the limbic system that drive anger, fear, violence and negative emotions. Such negativity causes the release of biological chemicals fueling one’s state of mind and stimulating the ultimate quest for relief—like a pressure valve releases pent-up energy. The state of the physical being is attacked by the stress response of the body; decreasing the immune system, placing one in a position of fight or flight…or succumb. Even when a person has reason to feel thankful, grateful and blessed, and their immediate world seems to be clicking along well, they can be subject to the shadow of depression. Depression is an unwanted interloper that has a way of sneaking in when least expected. It is a bandit, a thief of one’s well-being that is not necessarily recognized for what it is when it intrudes so stealthily into one’s psyche. When depression is present, one is robbed of their perceived pleasure and/or self-satisfaction, even self-esteem; it is a burglary of the reward

or satisfaction-setting capacity of the nucleus accumbens until something sets it free. In the presence of depression, the brain cannot produce its “happy hormones” because its biochemistry is different, according to the World Health Organization (WHO). So even when an individual wants to feel good, work, be productive and feel they are a part of life in a positive way, they cannot. The fact is the leading cause of human disability is not physical illness, but depression. Sometimes the ultimate symptom of depression is taking one’s own life out of perceived total despair. WHO International states that 800,000 people worldwide have lost their lives to suicide and that suicide is the second leading cause of death among 17-to-29-year-olds. It is also an alarming fact that according to the DBS Alliance (Depression and Bipolar Support), more than 1.9 million children between the ages of 3 and 17 years old suffer from depression at this time. Over the years the mental health of people in the USA has steadily declined. The statistical data for depression just gets sadder among, but not limited to, age groups, ethnic groups, income groups, gender, blue-collar workers, and high-level professionals. Depression is not just a national problem, it is a global problem, with major depressive disorder topping the list. So…how does one feel thankful, grateful and blessed? They may start with substances and behaviors such as illegal drugs, alcohol, eating disorders, or behaviors representative of acting out to feel a false sense of satisfaction and/or at least an escape from the intruder. These are different types of intruders because they actually seem to stimulate the nucleus accumbens. In the presence of this apparently helpful “Trojan horse,” one feels “happy,” “gone,” no longer under assault for the time, maybe even a bit functional, depending on the substance of choice. And now, with COVID-19 on the prowl like a dark, slithering mist, clouding life, liberty and happiness, it is most difficult to find solace where we would have historically found it. It is hard to hold on to the positive when the world is under siege and families are suffering from job loss, inability to socialize, the huge gigantic “X” factor that represents the unknown for what, when and how we will emerge. Some would say to be thankful, you must be grateful; to be grateful, you must be thankful; and to be blessed, you must be grateful and thankful. However, we could add to that the act of being hopeful, because if we are hopeful, we can find ways to battle the enemy of the mind and body; where goes the mind, so goes the body. So to get the mind in order, it must be relieved of chaos, and relieving chaos may require some help. Help comes in many forms. Things we can do for ourselves in times of peril include taking care of our psyche. Be

involved in something you care about. The absence of social interaction is a negative factor in the fight against depression and sadness. People are social; we are a society of diverse and wonderful other people, animals, flora and many beautiful things. Sometimes we fail to notice what’s close to us because we are consumed by what we cannot see that is affecting us. So be a part of something. Choose your meals wisely. There are many toxins in the things we eat, such as preservatives. They are chemicals that are to supposedly keep things “fresh…” hmmm, really? The chemicals build up in our bodies, causing fatigue and disease, and could, in fact, shorten our life. Exercise to keep the body active and lively by stimulating endorphins that make you feel comfortable and part of the world. Take care of your surrounding environment. Keep it free from clutter and junk; it just adds to chaos and feelings of being trapped. Balance your self-needs with the needs of others. Hydrate yourself to keep the toxins flowing out of your body and reinvigorating your cells in ways that foster youth-retaining balance at the cellular level. Even with all we can do for ourselves, sometimes the mind and body need assistance. Assistance comes in the form of a professional experienced in mental health disorders. First is talk therapy that often begins with cognitive behavioral therapy (CBT), where a person learns to develop coping strategies to manage challenging distortions in their life, in addition to acquiring other skill sets. Dialectical behavioral therapy (DBT) is a form of CBT that focuses on the psychosocial aspects of one’s treatment. Additionally, psychopharmacological treatment may be required to treat the acute phases of mental, behavioral and substance use disorders. The purpose of treatment that includes pharmacological protocols is to assist in diminishing the acutely intruding clinical manifestations of the offending condition/s. However, it is strongly advised not to take medications in the absence of talk therapy. They are complementary to each other. Subsequently, as the clinical manifestations are more controlled, consideration can be given to decreasing and/or altering the treatment regimen that may or may not require taking medications over extended periods of time; the primary purpose of medications is to quiet the intrusive symptoms and enable an individual to describe what they are experiencing in a less encumbered manner. However, several genetic/biological psychiatric disorders often require medications long term to best manage the symptoms. It is individually determined. Other forms of reducing stress and fighting depression include meditation, yoga, and reasonable exercise, hobbies you enjoy, and finding connections to things and/or people that soothe rather than agitate or aggravate; this is tougher to do currently because of COVID and social distancing requirements. For many, socially friendly technology platforms can help as we muddle through this trying time. Free yourself from self-imposed “have tos.” Find what it is that makes you feel thankful, grateful and blessed. After all, in this country we are endowed with three inalienable rights to life, liberty and, oh yes, the pursuit of your deficit in happiness. So let us pursue precisely that—thankfully, gratefully and feeling rather blessed to have these opportunities at our disposal. Don’t forget them—ever!

Living Your Best Life Dr. Nina’s How To Live Your Best Life People come and go in your life, but the right ones will always stay. With time, people can grow apart, perhaps due to geography, other priorities or relationships, physical or mental health conditions, conflict, or learning someone’s true colors. They are part of your story, and as we grow older we learn that our lives have many chapters, complete with new characters and adventures that we enjoy in the moment. Learn to accept this and move on.

You can’t please everyone. People are different, or unique—their lives, faiths, beliefs, values, relationships, thoughts, choices, personalities, emotions, skin color, size, and physical appearance, the list goes on. And when your uniqueness differs from theirs, it bothers them and some choose to criticize or say hurtful things to boost their ego or fill in gaps in their self-esteem. If what you are doing is not harmful, continue doing what makes you happy and what pleases you. After all, as Dr. Seuss states, “Those who matter don’t mind, and those who do mind don’t matter.”

Put aside your pride. It is said that of the deadly

sins—lust, envy, anger, greed, gluttony, sloth—that pride is the deadliest of all, and the root or beginning of all evil. Less dramatically, having a healthy dose of pride is important to healthy self-respect and self-esteem. However, too much can lead to unreasonable behaviors or actions—or the manifestation of the other sins. Often pride is rooted from (unrealistic) fear of embarrassment or shame, and results in the breakdown of relationships or that special bond. When appropriate, don’t be afraid to laugh at yourself, acknowledge you don’t know everything or that you are wrong, and even say you are sorry.

Somebody’s story or feed is just a highlight reel of their best or photoshopped pictures or life event. There’s always going to be something better than what you have. And if that’s what you focus on, it robs you of the ability to enjoy what you have in your life.

Cherish important relationships. To cherish is

to protect and care for someone lovingly or hold something dear—to nurture. And there are many ways to do this: spend time with family and friends; express gratitude towards them; tell them how you feel (e.g., “I love you”) and that they matter (humans have an innate desire to receive and give love); practice forgiveness; perform thoughtful acts for them (even simple ones such as a card, letter, email, or text with good wishes are powerful); and surround yourself with memories such as pictures of your loved one or other mementos.

Don’t let complaining become a habit. It’s

annoying and no one really cares what you’re saying when all you do is complain. Often, complaining is a way of relieving stress, results from frustration and an inability to solve the problem, it’s how you grew up, is part of your family culture/ dynamic, it’s attention-seeking, or is a manifestation of depressive symptoms. And it negatively impacts our mind and body.

Practice empathy. Defined as “the ability to understand and share the feelings of another,” empathy helps build trust while reducing stress and tension. Despite individual differences in empathy between individuals, it can be developed. Make an active effort to listen, connect with someone’s feelings, acknowledge their pain, and show them love.

Lisa Ledden has been a nurse practitioner specializing in dermatology since 1996. Before this, she had a broad background in civilian and military medicine. After graduating from the University of Massachusetts in 1985, she was awarded a military commission and accepted into a competitive internship with the U.S. Air Force. As an officer, she practiced trauma and emergency/critical care nursing and was part of a field medicine deployment team. She was awarded the Air Force Distinguished Service Medal in 1991. After seven years of service, Lisa left the military as a captain to pursue a civilian career. She accepted a position as nurse manager of the Heart Group of Oklahoma (the largest cardiac practice in central Oklahoma) and subsequently managed emergency services at Milford Memorial Hospital in Delaware while pursuing a master’s degree in nursing at Wilmington College in Georgetown, DE. Community Throughout The Covid-19 Pandemic, Bridge Of Books Foundation Continued To Provide Books To Underserved NJ Children But Is Now Asking The Community For Help To Find A “New Home” Throughout the pandemic, From March – October, Bridge of Books provided 29,081 children’s books to those in need. Outside of Bridge of Book’s regular mission, they also delivered 2,500 books to the homes of the elderly and local hospitals.

As with many organizations, the COVID-19 shutdown has had a significant impact on Bridge of Books (BoB). Since 2015, BoB has run their operations out of space donated by Vonage in Holmdel. When companies in NJ went virtual mid-March, Vonage closed their building too, displacing BoB and limiting access to the books that were stored there. With so many people cleaning their houses (because what else do you do during a pandemic?), BoB was receiving daily calls to donate children’s books. When community agencies started to request books for kids in need, BoB immediately pivoted to go “old school” or back to the beginning when they used to collect and distribute books from Founder and Executive Director, Abigail A. Daly’s front porch. As the number of requests increased and as BoB was uncertain how long they would be out of Vonage, they eventually decided to temporarily rent a storage unit in Atlantic Highlands. Since the COVID-19 shutdown in mid-March, BoB has still managed to pull together 43 book donations going to 36 individual providers across 14 separate NJ counties. Total? Just over 29,000 books. All from a front porch and a storage unit and with the dedication, patience and support of a few volunteers and an entire community of book donors and book recipients. While BoB has made it work in order to get books to kids who need them, this a short-term solution because the unit does not provide nearly the amount of space that is needed. Sadly, BoB has recently learned that Vonage will be consolidating their own operations and in doing so will be taking over the space that they had generously been donating to Bridge of Books for the past 5 years which leaves BoB needing to find a new permanent home as soon as possible. Founder and Executive Director of Bridge of Books Foundation, Abigail A. Daly, said, “With the realities of a digital divide among communities across NJ, kids who needed books from us before the pandemic need

them even more now. Alongside the countless NJ organizations working to meet the needs of those most affected by the pandemic, we ARE figuring out how to get books to these kids.” Bottom Line: Bridge of Books needs a new donated home in Monmouth County in order to continue their mission and would appreciate any leads. The space would need to be: • 1,500 - 2,000 square feet • Temperature controlled • Have garage/ loading dock access (2nd floor is OK as long as there is an elevator & garage/loading dock access) • 9-5 access on weekdays plus weekend access.

At the time of this publication, the nation will have voted for the 46th President of the United States. The year 2020 has challenged Americans in more ways than we could have ever imagined. I have interviewed young and old about their political views. However, one conversation in particular resonated with me and inspired this publication. I was having a conversation with a 12-year-old intelligent young lady who shared that she was not sure who she would vote for if she was eligible to vote and would most likely not exercise her freedom to cast her ballot. I immediately seized the opportunity to educate her about the importance of voting and reaffirmed that her voice matters through her vote.

In celebration of the centennial of the 19th Amendment, I was prepared to revisit publishing a tribute to historical suffragists. However, I came across Nannie Helen Burroughs and was enthralled by her trailblazing and monumental journey as an educator and activist. Nannie Helen Burroughs was born on May 2, 1879 in Orange, Virginia to John and Jennie Burroughs. Nannie’s parents were slaves and her father passed away when Nannie was a young child. Nannie and her mother relocated from Virginia to Washington, DC. Nannie attended schools in Washington, DC and graduated with honors from the M Street High School. Nannie was a “standout” in high school; however, when she applied for a teaching job in the DC area, she was not selected. It has been speculated that Nannie was a victim of discrimination by the elite black community because Nannie had darker skin than other African Americans at the school. Nannie did not perceive this level of discrimination as a setback and instead moved forward with establishing her own school. Nannie petitioned the National Baptist Convention for financial support and also gained support from African American mothers and children. The National Baptist Convention purchased six acres of land in northeast Washington, DC. Although Nannie’s school was gaining some support from the community, Nannie also had her share of setbacks. Nannie was committed to the education of African American women. It was important for Nannie to ensure that women learned to have economic freedom and to effectuate this through education and the right to vote. Nannie Helen Burroughs opened the National Training School for Women and Girls in 1909. The school educated African American women from around the world. Nannie did not receive any funding from white donors. The goal of the school was to develop students to become “the fiber of a sturdy moral, industrious and intellectual woman.” The curriculum was rigorous and focused on academic as well as vocational models. The areas of study included dressmaking, handicrafts, power machine operation, public speaking, music and physical education. Remember this was the early 1900s! Students attended the school from across the globe. Students were also required to be “practicing Christians” in order to attend the school. The course included a religious component and included Sunday school classes. It is important to note that the school did not restrict applicants based upon their race. The vast majority of the student population were from working class families. The school initially started off small with minimal classrooms and with a prominent black history scholar by the name of Dr. Carter G. Woodson as a powerful advocate. Nannie was able to complete the Trades Hall in 1928. The new hall replaced the old building and added more classrooms and administrative offices. In 1964 the school was renamed in Nannie’s honor as the Nannie Helen Burroughs School. The Trades Hall became a private elementary school before closing in 2006. An extraordinary woman who has shown us that through the power of education and advocacy, we can make a difference! As we say goodbye to 2020 and look forward to a fresh new start, I am reminded of a quote from Mehmet Murat Ildan: “In the New Year, never forget to thank your past years because they enabled you to reach today! YOUNG MEN'S & WOMEN’S GROUP

This group is designed to help young people explore and discuss gender specific issues and challenges. Topics include: Toxic Relationships, Body Image, and Perception. WHEN: 2nd and 4th Mondays, 7:00 p.m. – 8:00 p.m.

ART EXPRESSION

This group allows youth to express themselves through different mediums and guided practice. These activities are for any art skill level. Projects include: Drawing, Painting, Collaborative Art, Collages, and Word Expression. WHEN: 1st and 3rd Mondays, 7:00 p.m. – 8:00 p.m.

TEEN RAP

This group provides youth with the opportunity to discuss a variety of different topics that are prevalent in the daily life of youth. Topics include: Peer Pressure, Assertive Communication, Boundaries, and Social Anxiety. WHEN: Tuesdays, 7:00 pm – 8:00 pm

ANGER MANAGEMENT

This is a group to help youth explore anger and its social, emotional and physical effects, as well as ways of successfully communicating and managing it. Topics include: Fight or Flight, Coping Strategies, Triggers, and Forgiveness. WHEN: Thursdays, 7:00 pm – 8:00 pm

TEENS LIVING CLEAN

A support and prevention group for youth interested in drug or alcohol abstinence. Topics include Risks & Protective Factors, Getting Clean, Coping Skills, and Cycle of Addiction. WHEN: Fridays, 7:00 pm – 8:00 pm

FAMILY WORKSHOP

MONTHLY

Once a month sessions: one dedicated to parents/guardians and the other for youth. Topics include: Back-to-School, Suicide Prevention, Bullying, Stress Management/Relaxation & Mindfulness, and Family Engagement with Teenagers.
