Skin Cancer: COMMON MISCONCEPTIONS REGULAR SUNSCREEN USE PREVENTS VITAMIN D ABSORPTION

Vitamin D, which is produced in the skin after sun exposure, is known to improve bone health and reduce the risk of certain cancers. Getting an adequate amount of vitamin D is dependent on these three tactics: 1) eating foods that contain vitamin D, 2) taking a vitamin D supplement, and 3) getting a small amount of sun exposure. The fact is that large epidemiologic studies reflecting real life conditions prove clearly that sunscreen use does not cause vitamin D deficiencies. Adequate vitamin D can be obtained safely and cheaply through food and dietary supplements without the risks associated with overexposure to UV radiation. If you are concerned about your vitamin D levels, speak with your doctor. SUNSCREEN IS UNSAFE AND CAUSES SKIN CANCER

Over the years, a few controversies have developed regarding the safety of sunscreen. The safety of sunscreens has been studied in labs and on live subjects by reputable research scientists for many years. The results of these studies provide overwhelming evidence that sunscreens are safe and effective. No current published data has demonstrated adverse health effects on humans from the regular use of sunscreen. IT IS SAFE TO USE TANNING BEDS AS LONG AS YOU USE SUNSCREEN

If so, you’ve fallen victim to one of the most prevalent myths about skin cancer: that tanning before sun exposure prevents skin from burning. This common misconception, along with a host of other myths about skin cancer, could have deadly results: Rates of melanoma—the most dangerous type of skin cancer—are rising, particularly among young women. SKIN CANCER ONLY OCCURS ON SUN-EXPOSED AREAS

Any skin cancer—nonmelanoma skin cancer or melanoma—can arise on any area of the body, regardless of whether or not a sunburn occurred in that location. Skin cancer can develop on any part of your body. That includes places where you’re less likely to notice something new or suspicious—like your genitals, the palms of your hands, the soles of your feet, between your fingers or toes, and underneath your nails. Jeffrey S. Gosin, MD, FACS Dr. Jeffrey Gosin has dedicated his career to the treatment of individuals with vascular and vein diseases. He is Board Certified in both Surgery and Vascular Surgery. He is a graduate of the University of Pennsylvania and Jefferson Medical College. He completed his General Surgery residency at Robert Wood Johnson University Hospital where he was Chief Surgical Resident. After his residency, he remained at Robert Wood Johnson and completed an accredited fellowship in Vascular Surgery, where he received extensive training in traditional open vascular surgery and minimally invasive procedures, performed medical research, and had numerous papers published in scientific journals. Dr. Gosin has a special interest in minimally invasive techniques and new medical technology and constantly strives to provide the most advanced state-of-the-art therapies to his patients. He was the first surgeon in his region to perform a stent graft repair of an abdominal aortic aneurysm, a treatment that is now the standard for most patients. Joanna A., Medical Assistant Joanna has been a medical assistant with Shore Vascular & Vein Center since 2014. She began her career with our group as an extern while she was still in school. After graduation, she was offered a position in our office and has been with us ever since that time. Joanna enjoys working as a medical assistant and helping other people. Being fluent in both English and Spanish, she finds it particularly rewarding to help our Spanish-speaking patients better understand their condition and treatments. When not working, Joanna enjoys spending time with her family and her two dogs. Joanne H., RN, RVT Joanne has been with our group since 1995 and has had a long and illustrious career in healthcare. Joanne began her career as a critical care nurse. Then in 1985, along with Dr. Stephen Gosin, the founder of our original surgical practice, she helped develop the first noninvasive vascular laboratory in the region. She is a true pioneer and expert in vascular ultrasound. Joanne was the director of the Noninvasive Vascular Laboratory at Shore Medical Center from 1985-

2009. During that time, she also performed vascular ultrasound tests in our office. She retired from Shore in 2009, but she has remained active in our office vascular lab since that time. Joanne was instrumental in developing the policies, procedures, and technical qualifications for our office-based noninvasive vascular laboratory to achieve and maintain its accreditation through the Intersocietal Accreditation Commission. Joanne is constantly complimented not only on her skills as a vascular technologist, but also on her bedside manner and compassion with our patients. Arunas S., RVT, RPVI Arunas has been our noninvasive vascular ultrasound technologist since 2002. He earned his Doctor of Medicine degree after graduating from the Medical Faculty of Vilnius University in Lithuania in 1988. Arunas had been practicing medicine for nine years in the field of invasive cardiology before moving with his wife and two children to the United States in 1997. Here he started a new career in noninvasive vascular imaging and technology after graduating from the College of Health Professions at Thomas Jefferson University in 2001. Arunas has also worked at the Hospital of the University of Pennsylvania. He is a highly skilled vascular technologist who maintains the standards required of our lab’s IAC Accreditation. Cheryl G., APN Cheryl is our nurse practitioner. She earned her Master’s Degree from New York University in 1997. She joined Shore Vascular & Vein Center in 2014. Prior to joining our practice she had experience in oncology, infectious disease and in cardiology. Cheryl’s primary focus is working with our aesthetic laser patients. While all of the laser procedures are performed by our physician, Dr. Gosin, Cheryl is very knowledgeable in all aspects of laser treatments and works closely with those patients. She assists patients with their pre- and post-procedural care and helps prepare them for their treatment. In her free time, Cheryl enjoys gardening, volunteer work, reading, and spending time with her family. Lori S., Business Manager (not pictured) Lori is our Business Manager. Dr. Carfagno: It can be challenging to balance the demands of taking care of my patients, twenty-four hours a day and balancing my own family life. Making time for my wife and children, while being there for my patients can get tricky. ACW: What sets your practice apart from others like it?

Later, he relocated to Atlantic County and married “the girl of his dreams.” He and his wife Dana reside in Linwood and have three wonderful children. His children, ages 17, 15 and 12 are all very active in local sports. When Dr. Carfagno is not in the delivery room, he can be found cheering on his children in their respective sports. Velvet is a luxurious accent that can be added to your home in many different avenues. You could add a green or blue nailhead velvet sofa to your living room that will create a soft contemporary touch with a pop of color. If a sofa isn’t in your future, adding a colorful velvet stool or bench to your family room can also have that same impact to your home. As for my most recent client, we added two velvet armchairs to the ends of their dining room table to create a break in the typical wood chair table set. Blue colors have been sweeping the home magazines for a while now and aren’t going anywhere; in fact, the trend is still going by adding greens and aqua into the mix. I add these trends through wall color, pillows, glass vases, and rugs. While we can never give up on our traditional crystal chandeliers, adding a pop of contemporary lighting like a glass bubble chandelier can make your home young, fun, and unique! Finally I cannot say enough about rattan furniture with cream and lace upholstered detail. Divorces And Snowflakes:

No Two Are Ever The Same By Marla Marinucci, Esq.

W

hen I meet with a potential client for the first time about a divorce, I am often asked the same question after they tell me a little bit about their situation: “What will the judge do?” Understandably, clients are hoping for exact answers but in the bizarro world of family law, where every single case has a different set of facts, there are very few black-and-white answers, and many shades of gray. A seasoned family law attorney will be able to assess your case and render an opinion as to a fair resolution of the matter. However, if you are in search of definitive black-and-white answers to your questions as it relates to divorce and custody, it is unlikely any practitioner will advise with certainty what those answers. This is because every case is different and even the smallest factual nuance can change the outcome of a particular issue The first thing I tell a potential client who asks me that proverbial question is that 99% of divorce cases in New Jersey settle, which means that there are very few divorce trials. After alerting the client that the possibility of their divorce going to trial is less than one percent, I like to share with them an analogy that I coined myself that I find helps them understand just why there are so many shades of gray. That is, that divorces are much like snowflakes in that no two are ever the same. This last thought is so very important for divorcing couples that it bears repeating here:

So when a potential divorce client says something like, “My friend just got divorced and her lawyer got her so much child support…” as graciously as I can, I immediately inform this potential client how every case is very fact-sensitive and how two cases that may appear quite similar can have completely different outcomes. I will then share with this person some words of wisdom that I always share with my clients and which go hand in hand with the snowflake analogy:

Do not listen to your friends’ advice when it comes to your divorce.

While they may have your best interests at heart, with very few exceptions, the “advice” being proffered often leads to unrealistic expectations. For example, if your friend tells you that they got everything in the divorce, or to the contrary, that they got nothing, chances are, that is not the case. Hypothetical: Your friend tells you that their lawyer is the greatest bulldog/barracuda/goes-for-thejugular lawyer of all time because your friend got to keep the house in the divorce. More likely than not, your friend failed to mention that in exchange for the house, they waived their interest in their ex-spouse’s pension, which was actually worth more than the house. In other words, it is a balancing act when it comes to dividing assets and liabilities. New Jersey is an equitable distribution state with an emphasis on the term “equitable.” For the most part, though not necessarily always, this means that marital assets will be divided fifty-fifty. So when your friend brags about keeping the house, keep in mind that the spouse on the other end of that divorce retained an asset, such as a pension, that was worth about the same as the house. While no two divorces are ever the same, there are, of course, some glaring similarities that are present in every case, such as acrimony and hostility. An unfortunate similarity that is often found in cases involving custody disputes is the practice of one or both parents using the children as pawns in the divorce. Quite often it is in the form of either bribing the children with toys or candy in order to win their affection in an attempt to alienate the other parent. My final and most important words of wisdom for purposes of this article are:

Do not use your children as pawns in your divorce.

While I am not a mental health expert, I have seen this time and time again and it only leads to disaster. Your children are undoubtedly your most prized possessions, not pieces in a game of chess.

Yikes, I have diabetes! Now what?

How will diabetes affect my life and my family? What will I have to do? Will I have to take insulin? There are so many questions. Where should I begin? Written By Barbara Greenling, DNP, MSN, APN-C

First things first. Take a deep breath. Your healthcare team is ready for you.

Diabetes mellitus is a chronic disease that affects individuals across the life span. It involves the hormone insulin. Insulin is the key that converts the food you eat to the body’s energy. Diabetes is classified three ways: Type 1, Type 2, and gestational diabetes. In Type 1 diabetes, your body does not make insulin due to beta cell destruction. In Type 2 diabetes, there is a progressive insulin defect. Your pancreas makes insulin, but your body is resistant to it. When your body does not recognize insulin, your blood sugar rises. Gestational diabetes occurs during pregnancy. Elevated blood sugar during pregnancy can harm your unborn child. Symptoms associated with diabetes include excessive thirst, excessive urination, and excessive eating. When your body lacks insulin, as in Type 1 diabetes, or is resistant to insulin, as in Type 2 diabetes, your body sends out signals. Excess thirst, known as polydipsia, is feeling thirsty even after drinking. Excess urination, known as polyuria, is passing more than five liters of urine a day. Excess eating, known as polyphagia, is in response to your cells craving energy. Other symptoms include fatigue, dizziness, blurred vision, and frequent yeast infections. Individuals may also complain of weight loss, nausea, and slow-healing wounds. Insulin is the key that allows food you eat to be converted to fuel needed for bodily functions. As you can see, insulin is vital for many of your body’s functions. How do you know that the diagnosis of diabetes is real? Diabetes is diagnosed by measuring serum blood sugars. Blood sugars are flexible numbers with normal values of 69 to 99 mg/dL. Any elevation in this number is a signal that your body does not have enough insulin or does not utilize the insulin available. When your fasting blood sugar is greater than 126 mg/dL on two separate occasions, a diagnosis of diabetes is made. A 2-hour-after-eating (post-prandial) plasma glucose level greater than 200 mg/dL also confirms the diagnosis of diabetes. Finally, a random plasma glucose greater than or equal to 200 mg/dL also confirms the diagnosis of diabetes. All of these tests are simple blood tests that your healthcare clinician can order. The measurement of average blood glucose is the A1C. It measures the daily average blood sugar two hours after a meal based on a statistical formula. Normal ranges are 4.5 to 5.6%. An A1C greater than 6.5% also confirms diabetes. This measurement will

guide your treatment of diabetes. As an individual with diabetes, you can and should expect that an A1C will be obtained every three to four months. The goal for A1C is 6.5% or less. This goal will be determined based on your unique needs and health. The next step in your care will be an individualized treatment plan. Your treatment plan includes multiple steps. These steps include medications, physical activity, sensible eating, and self-monitoring. The key to success in your treatment is diabetes selfmanagement education. With knowledge, you have the power to take control. There are many ways to treat diabetes. Medications, both oral and injectable, are available. Insulin is not a punishment; it is what your body needs. The type of medications depends on your A1C. The American Association of Clinical Endocrinologists (AACE, 2015) has created a clinical practice guideline outlining an algorithm for medication selections. Lifestyle modification is the number one treatment option for prevention and control of diabetes. Based on the value of the A1C, the guidelines provide a clinical medication treatment plan. If your A1C is less than 7.5%, clinical practice guidelines recommend one drug therapy. If A1C is greater than 7.5%, recommendations are for two medications. If A1C is greater than 9.0%, recommendations are for adding insulin. Individuals with diabetes also need to control their cholesterol levels and blood pressure. LDL-C, also known as bad cholesterol, needs to be less 70 mg/dL. Medications to control cholesterol are statins. The goal for blood pressure is systolic pressure, the top number, less than 130 and diastolic, the bottom number, less than 80. Therapeutic lifestyle changes including weight loss, physical activity, and sensible eating remain the hallmarks for treatment of elevated cholesterol, high blood pressure, and elevated blood sugar. Strategies for improving diabetes care are multifaceted. They include optimizing clinician and team behaviors, supporting individual behavior change, and changing the system of care. Diabetes care is patient-centered. A comprehensive diabetes treatment plan includes diabetes self-management education, nutrition, physical activity, smoking cessation, barriers to care, immunization, and management of co-morbidities. Having diabetes is not the problem; having uncontrolled blood sugars is the problem.

Caring for Your Loved One During the Holidays By Maryann Prudhomme, RN Director, Bayada Home Health Care Due to medical advances, people with chronic illnesses are living longer and there are more family caregivers than ever. According to a survey from the National Alliance for Caregiving in collaboration with AARP, more than 43.5 million “unpaid caregivers” in the US take care of a loved one. It can be stressful if you are a caregiver for an aging parent, and during the holiday season in particular, you need to take care of yourself in order to take care of your family. Physical and psychological stressors as an adult child caregiver are created by a number of factors, such as financial strains, limited time alone, and guilt. The added pressures of the holidays, like traveling, preparing meals, buying gifts, and organizing events, can multiply for caregivers and make this time of year something to dread, rather than something to celebrate. This season is for families to gather together and enjoy each other’s company; your responsibilities as a caregiver should not be so overwhelming that they overshadow these festivities. It’s important for families to take time to renew themselves—even when it may seem impossible during the gift wrapping, cooking, and shopping. If during this time it would only add more stress to ask family members to assist you in your caregiving duties, then hiring a home health care company may be ideal. Home health caregivers, such as nurses and home health aides, can give you time to rejuvenate you and your family’s spirits for the holidays and help you care for your loved one as effectively and safely as possible. Asking for help and taking time for yourself is a sign of strength in a caregiver. Happy Fall To All! Fall is my favorite time of the year. Cool temps, holidays, fall foliage. But... Mother Nature’s pesky parasites are still with us. ith every new patient I order a complete laboratory workup including internal medicine, nutritional, and panoramic hormonal laboratory studies. On the second visit, we review the laboratory results and decide which direction to take to reverse their medical woes. There is usually a tremendous amount of information to discuss. What no longer surprises me is the quintessential question nearly every patient asks as he/she leans over my desk and anxiously asks, “What is my cholesterol?” Television advertisements for cholesterol-lowering medication have seduced most to think they will not have a heart attack if they have low cholesterol. Yes, cholesterol is one small part of the puzzle in determining your risk of having coronary artery disease, but there are so many more studies to add to your cholesterol results to determine your chances of medical demise. So I tell my patient that their cholesterol is 200, and their next question is, “Should I take a statin [cholesterol-lowering medication]? My other doctors want me to take a statin.” The simple answer to whether or not you should take a statin is “maybe.” The medically correct question is, “Should I take a statin to decrease the inflammation in my arteries, to decrease the formation of plaque, to decrease the chance that a clot will form, which may break off and cause a cardiac event, to decrease the stickiness of my platelets?” The answer is still maybe. We need many more laboratory results. Of particular importance is a DNA test called SLCO1B1 that tells me if you are in the 60% of patients I will not harm if I prescribe a statin. If you are in the 40% of poor statin transporters, you are more likely to develop myopathy (muscle cell death and consequential muscle pain), elevated liver enzymes, and elevated blood sugar. You can still be placed on a statin, just a very low dose, and Harvard cardiologists suggest perhaps taking the lowest dose of certain statin medications two or three times a week. According to Drs. Bale and Doneen, who wrote Beat the Heart Attack Gene, women should not take Lipitor. Many patients come to me on a statin medication and have very low cholesterol levels. First of all, the regular lipid profile laboratory study only gives you the raw numbers; namely, total cholesterol, LDL (the bad kind of cholesterol), HDL (the good kind of cholesterol), and the cholesterol ratio. You need a different, more sophisticated test for cholesterol that tells you if you have the fat, puffy kinds of LDL particles or HDL particles --- or the small, dense kinds of LDL and HDL particles. The fat, puffy particles are good. The small, dense particles are bad. You need to know that your cholesterol should not get too low because cholesterol is the precursor to the hundreds of hormones that are declining with natural aging. Read Grain Brain

To turn on your supposed heart disease gene requires many maladaptive health behaviors. If your goal is to prevent the chronic degenerative diseases of aging, and you realize that you must participate in your care with the help of your physician, consider getting the following tests: 1) genetic tests such as Apo E, MTHFR, SLCO1B1; 2) inflammation markers such as fibrinogen, hsCRP, Lp-PLA2, MPO; 3) blood sugar tests such as adiponectin, HbA1C, insulin; 4) liver, kidney, muscle tests: creatine kinase, homocysteine, NT-proBNP, vitamin D, CoQ10; 5) hormones: DHEA-S, E2, FSH, LH, progesterone, SHBG, free and total testosterone, TSH, DHT, free T3, reverse T3, cortisol; 6) and, of course, a comprehensive lipid panel; 7) self-care, including measuring your waist/hip ratio, monitoring your blood sugar and your blood pressure, measuring your pounds of fat; 8) taking a judicious supplement program suggested by someone highly qualified in nutritional biochemistry; 9) learning how to eat…and what to eat. Then we put all this information together and create an individualized treatment plan outlining which supplements to take, a personalized exercise program, which medications to prescribe, change, or discontinue, which bioidentical hormones to replace in physiologic doses, and finally stress management. Psychiatry is quite excited that they finally have a quantifiable test for anxiety and depression --- namely measuring inflammatory markers. So number 10 is the most difficult part of your health program --- because constantly speaking darkness, worrying about your very stressful life, and anxiety and depression all cause inflammation, which enhances your chances of having a coronary event and/or some form of dementia. Published medical studies revealing the qualities of healthy centenarians tell us that centenarians are lean, still chop wood, move a lot, are in the upper quartile of most hormonal levels, sleep well, and have a sense of joy --- even in our complex world. So whether or not you shall live to be very old, being healthy is a little more complicated than having low cholesterol.

Atlantic Medical Imaging Offers Low Dose CT Lung Screening

LOW DOSE CT LUNG SCREENING AVAILABLE AT ALL AMI OFFICES Atlantic Medical Imaging (AMI) offers low dose CT lung screenings, a breakthrough new tool in the fight against lung cancer. Today, 85% of patients diagnosed with lung cancer in the United States will die from it within five years. With over 160,000 deaths per year, lung cancer has become the number one cancer killer in America, claiming more lives than breast, colon, and prostate cancer combined. Obsessive-Compulsive Disorder

et’s see…I have to ‘fix’ my closet today. I ‘fixed’ it three times yesterday, but it needs to be ‘fixed’ again. My shoes look disorganized because one of them is out of alignment with the others. I can’t take it when my closet looks this way. My shirts are in order of color, my suits have to be in order according to the season and my socks can only be stacked three pairs high in my drawer. My socks cannot touch my underwear and they can only be stacked two high, and by color. The ties are always the biggest challenge to get right; the colors, sizes, designs never come together the way I like.” “A plane just went over. Every time that happens, I have to knock on wood with my right hand as quickly as possible. I know it sounds crazy, but I’m responsible for the safety of everyone on that plane…crew, passengers, and even the plane itself. If I knock on wood four times as fast as I can after I hear the plane, they will all be safe. If not, there is great risk and I am responsible.” “There are germs everywhere. They are stubborn and hard to get rid of, if ever. I cannot shake hands with anyone. I cannot talk on the same phone someone has been talking on. If someone sneezes in the open air, I just know I will become deathly ill. If I touch anything at all, I must wash my hands for a long time and many times over; there is just no way to escape germs, no matter what I do; but I need to do anything I can to avoid becoming contaminated.” These are only three examples of intrusive, obsessional thoughts with the required compulsive behaviors. An obsession is just that—an intrusive, unsolicited, negative thought. By contrast, a compulsion is a subsequent learned, repetitive behavior that is in some way intended to shut down or satisfy whatever the obsession demands. OCD is ritualistic in effect because the obsessions yield compulsions in an attempt to eliminate the obsessions. People are “trapped by recurring thoughts, images, fears and/or beliefs they cannot control.” OCD is described as “an anxiety disorder in which people have unwanted and repeated thoughts, feelings, images, and sensations (obsessions) and engage in behaviors or mental acts in response to these thoughts or obsessions. “ We can obsess about literally anything. According to Psychology Today, OCD affects about 2.2 million adults in America. It is likely to occur with other disorders such as anxiety, depression and eating disorders. Both men and women are approximately equal in this malady that often appears in early childhood, early adolescence, or early adulthood. OCD is thought to be a combination of genetic and environmental factors. Still other research has found links between untreated streptococcus bacterial infections and the development of OCD. The three forms of OCD are: OCD as a purely genetic disorder meaning it’s really a neurobiological disorder with behavioral manifestation; OCT, described as obsessive disorder Traits that are learned and largely compensatory behaviors; and OCPD described as obsessive compulsive personality disorder “A

pervasive pattern of preoccupation with orderliness, perfectionism, and mental and interpersonal control, at the expense of flexibility, openness, and efficiency, beginning by early adulthood and present in a variety of contexts….(DSM 5)” Such traits include workcoholism, inability to throw out objects that are worn out and worthless despite not having any sentimental value. The literature regarding genetics and OCD seems to point to chromosomal aberrations that can occur in a cluster with three other genetic disorders, those being Bipolar Disorder, ADHD and addiction to anything. Theoretically, there are two chromosomes that are next to each other, (we have 23 pairs of chromosomes containing all of our genetic material). Each of the two consecutive chromosomes have a pair of loci, a circumscribed region that contains specific genetic material that may have an alteration in the coding that may manifest certain disorders, like the tetrad of OCD, BD, ADHD and addictions. Some scientists think there is a problem with certain pathways or

neurotransmitters in the brain that create a link between judgment and planning with areas of the brain that filter messages involving body movements (webmd. com/mental-health/obsessive-compulsive disorder). In addition, there is evidence that OCD may be inherited due to a biological vulnerability that enables it to pass from parents to children. Research also indicates that there is a “higher rate of OCD among first-degree relatives of adults with the disorder.” Environmental factors associated with OCD as triggers or exacerbations include situational factors such as physical or sexual abuse, disruptive changes in lifestyle, and relationships. Disorders that occur simultaneously with OCD make it harder to discern as a diagnosis, therefore making it difficult to treat. OCT can be a learned disorder that is compensatory to an array of other disorders likely seen with it. These include eating disorders, substance abuse, personality disorders, attention deficit disorder, anxiety, depression and PTSD; for instance, someone with ADHD may be more prone to procrastination, forgetfulness, poor time management, poor organizational skills and follow through. Such a person may manage these problematic things by putting sticky notes all over as reminders, setting alarms, triple check parking, or whether they turned off the stove, coffeepot, etc. It should be stated that just because an individual exhibits these types of behaviors does not mean they have OCD. Some behaviors such as these are traits a person may develop for good reason. Maybe there was an incident where the stove was left on and caused a fire, or a burglary because the door was not locked causing that person to check the door lock frequently. Hence something can

The Ritualistic Journey of the Mind occur in a person’s life that makes them more cautious than another person may be about the same thing. But overall, since OCD manifests as a neurobiological and emotional disorder, it is considered psychiatric in presentation and treatment. Hoarding is common among people with OCD. It is a very troublesome compulsive behavior that is about collecting things that may have little to no value, but keeping them anyway. Hoarding can be very dangerous, especially if it results in piles and stacks of things that result in an unhealthy environment. In some cases, cleanliness suffers or special concerns may be present because the clutter blocks entrances and/or exists. Hoarding animals or dangerous chemicals can be hazardous to the individual and public. Hoarding can be a symptom of disorders previously mentioned such as depression, posttraumatic stress disorder, and others. They may also present with “skin picking or tic disorders.” The emotional load from OCD can range from annoyance to devastation. The severity of the dysfunction is directly proportional to the emotional pain levels. Frustration, guilt, low self-esteem and worth, embarrassment, depression, even hopeless, helpless states can compound the problem greatly.

TREATMENT

There are two main modalities for treating OCD that have proven to be most effective. These consist of drug therapy and behavior therapy. Treatment has to begin with the appropriate diagnosis in order to achieve the correct plan of care. Screening scales for OCD, i.e. the Yale-Brown OCD scale, can be helpful in corroborating the clinical impressions regarding the presence of OCD.

to attain some level of relief. Behavior therapy consists of a person being incrementally “exposed to the objects or situations that cause them problems.” From there they are requested to delay the associated ritual and over time, with the gradual decrease in ritualistic behavior, may be able to show at least a reduction in the behavior. Since OCD is not subject to having an effective response to traditional treatment, it is important to be patient. Many times augmentation is required, which is the addition of other modalities in order to achieve a combination of therapies that are most beneficial to the patient. There is still much to be learned from ongoing studies relative to OCD. It is a multi-dimensional and a very individualized problem. OCD can be elusive to treatment so it takes time, patience, and perseverance. Only a well trained and experienced mental health professional should diagnose and treat this vexing disease.

Don’t Suffer Alone…

Examples of drugs used in the treatment are selective serotonin reuptake inhibitors (SSRIs), i.e. higher dose Prozac (Fluvoxamine), or Luvox (Fluvoxetine). An older class of drug considered to be effective is the “tricyclic” antidepressant, clomipramine; it is the generic name for Anafranil. This medication, made available in the US in 1990, showed that approximately 60% of patients were helped though it generally takes several weeks to be notably effective. It will not cure the OCD problem, but can assist people to better manage their lives. Anafranil also has several side effects as evidenced by almost all persons who took the medication with seizures being the most serious. These medications, or others are best determined and prescribed according to your physician and are not intended to be prescriptive within this article. Behavior therapy helps individuals to manage the manifestations of the anxiety caused by OCD; specifically in helping to reduce the obsessive, ritualistic behaviors. Interrupting these obsessive-compulsive behaviors can be very unappealing to many who experience them. Sometimes Good People Find Themselves In The Criminal Justice System-Use An Expungement To Help You

A

s we stand here in 2017, finances become ever so important in an extremely competitive job market. We are in a technologically advanced society where people can learn about you and your background with the stroke of a computer key. Depending on the agency that is conducting the criminal background check, whether private or public, a wealth of information about you can be yielded. Having a conviction in New Jersey for a crime, offense or otherwise can possibly jeopardize securing a job if you have been involved with the state criminal justice system. Not only can a conviction affect your ability to secure a livelihood, it may need to be disclosed and can affect college applications, graduate school applications, professional licensing requirements, relationships, court proceedings, military entrance, certain jobs requiring a background clearance, housing and insurance. The law regarding expungements is highly technical and therefore you need an experienced criminal attorney to obtain an expungement. An expungement is legally defined as “the extraction and isolation of all records on file within any court, detention or correctional facility, law enforcement or criminal justice agency concerning a person’s detection, apprehension, arrest, detention, trial or disposition of an offense within the criminal justice system.” N.J.S. 2C:52-1a. Expungements apply to juveniles and adults. Our legislature has recognized the role of an expungement. Life experiences and observation suggest that a multitude of reasons exist why a person has landed in the criminal justice system – marital issues, health problems, mental health issues, drug dependency, financial failure, death of loved ones or simply a product of circumstance. Recognizing these unfortunate yet real problems, New Jersey criminal law states in pertinent part that regarding expungements “the primary objective of providing relief to the reformed offender who has led a life of rectitude and disassociated himself with unlawful activity.” N.J.S. 2C:52-32. Not all crimes and offenses are subject to an expungement. For example, motor vehicle offenses for which a person was arrested or convicted are generally not subject to an expungement. See N.J.S. 2C:52-28. A person seeking an expungement application should know that New Jersey requires certain time passage before one is even eligible to apply. In some cases, depending on the facts and circumstances of the individual, it may be a waiting period of ten (10) years, five (5) years, three (3) years, six (6) months or immediately.

Particularly significant are expungements applications that can be filed immediately. The Governor signed a fairly new law to be effective April 18, 2016. See N.J.S. 2C:52-6a et seq. However, many experienced attorneys and persons involved in the state criminal justice system are not aware of it and how it can help their clients. This new law allows for an expungement application to be filed immediately if a person’s charges were dismissed or resulted in an acquittal, with certain exceptions. To illustrate, we recently helped an individual in Essex County. We were successfully able to get his indictable charge downgraded to a simple assault. Thereafter, we got the simple assault charge dismissed. I prepared all of the necessary paperwork and provided it to the judge to transfer over for expungement purposes. There was no waiting period and this process helps clients who are looking for immediate filing and an expeditious expungement of same. The Administrative Office of the Courts (AOC) has provided a criminal directive and legal form to be utilized in these instances. The law on expungements, like many laws, can and often does change from moment to moment. Likewise, the judicial interpretations dictate the roadmap in this technical area. Depending on what you were convicted of, the time frame, what your criminal records are before and after, and a list of other crucial factors determines if you will be afforded an opportunity for a clean slate. When it comes to your life and your livelihood, you need an experienced attorney who will tend to your best interests. Famous novelist Marcel Proust sums it up – “There is no man … however wise, who has not at some period in his youth said things, or lived a life, the memory of which is so unpleasant to him that he would gladly expunge it.”

A Passion for the Law: Meet Attorney Ehsan F. Pearce Trial Advocacy competition and given the Excellence in Litigation Award from the Center for Forensic Economic Studies. I attained Dean’s List and was the President of the Minority Law Students Association. ACW: What attracted you to the field of criminal law? Ehsan: While in law school I took many courses related to criminal law. Some required by the institution, and others were elective classes. I took Criminal Law, Criminal Procedure, Advanced Criminal Procedure, Juvenile Law, Pennsylvania Criminal Law Practice, Advanced Trial Methods, Federal Rules of Evidence, Constitutional Law, and some introduction to White Collar Crime. I excelled in these classes and found them to be both interesting and challenging, as issues of criminal law concern life-altering events. Criminal law is a topic that everyone can relate to as we are constantly

seeing it on the news and social media from various trials, arrests, and day-to-day crimes being committed. ACW: What is the most rewarding aspect of your profession? Ehsan: When a client seeks the advice of an attorney, it is usually for a problem. However, when in need of a criminal defense attorney it is more serious because a person’s life and liberty are in jeopardy. My role is to provide sound advice and the best possible outcome based on the current law and facts of their case. While this is challenging, it is also highly rewarding. When clients come to us, they could be facing imprisonment, serious fines, loss of a driver’s license or potential future job, separation from family, and a multitude of other hardships. To be able to help ease their pain and obtain a favorable result is very gratifying knowing that my set of skills and experience brought forth peace and alleviated their fears. ACW: What are the most common issues you see in your profession? Ehsan: It really varies, though rarely have I seen pure criminality. People land themselves in the state criminal justice system for a myriad of reasons resulting from drug and alcohol issues, mental health issues, marital problems, financial troubles, and most often lapses in judgment. Fortunately, in New Jersey there are options we can help them with. Drug offenders can partake in drug court rehabilitation if eligible. The one time mistake making individual may be afforded a diversion program of Pre Trial Intervention (PTI) and we are seeing a rise in an effort to assist individuals with mental health issues. For lower level offenses, jail time can potentially be removed in lieu of probation with strict conditions such as anger management, community service, and or a host of other things deemed necessary by the court. Additionally, if a person has already been convicted and has led a law-abiding life, we can provide expungement of a criminal record services if eligible. Reducing Winter Wandering Six out of 10 people who have Alzheimer’s disease or a related dementia will wander. However, during the winter season when temperatures dip, the days become shorter and homes are filled with more friends and family than usual celebrating the holidays, wandering episodes increase. “All those different people in the home of someone who has dementia can be disorienting and frightening for them,” explained Mary Beth Lewis, programs and services coordinator at the Alzheimer’s Association Delaware Valley Chapter Atlantic County Branch Office. “Even if every visitor to the house is a member of the family, they might be a stranger to the person with Alzheimer’s, who won’t recognize them and might even think they are in someone else’s house.” This can cause someone who is scared and confused to want to “go home” during a big family gathering like Thanksgiving and slip out of the house without anyone noticing. “There are a few signs that caregivers can look for ahead of time that could indicate whether their loved ones with dementia are at risk for wandering,” said Lewis. Caregivers should watch to see if they: • Return from a regular walk or drive later than usual. • Try to fulfill former obligations that are no longer a part of daily life, such as going to work. • Try to “go home” even when at home. • Are restless, pace or make repetitive movements. • Have difficulty locating familiar places like the bathroom, bedroom or dining room. • Check the whereabouts of familiar people. • Act as if they’re doing a hobby or chore – going through the motions, but nothing is actually getting done. • Feel lost in a new or changed environment.

Leah Schwartz, Owner/Instructor of Breathe Studio, Barre and Pilates Leah’s journey started with her love for barre. She connected with the fluidity of the movements and how her body felt. Pilates was a natural progression. The more the she practiced, the more she desired to perfect her skills. In 2013 Leah began her training and certifications in Barre, and in 2016 she completed her reformer certification from Balance Body. Through out her education she has collaborated with mentors of varying practices that have diversified her technique and knowledge, and molded her abilities as an instructor. As any instructor will tell you, it takes a few tries before you find where you belong. After working for multiple gyms and studios, gaining as much experience and knowledge as she could, Leah began working at Breathe Studio for Marlena Baylinson. Little did she know when she accepted this once a week class, Breathe would become her forever home. In March of 2016, Marlena gave Leah an unimaginable offer, the chance to continue and grow the program she had started. With great gratitude, Leah accepted and became the owner of Breathe. Rebecca Kolas is both a Certified Lice Technician and Owner of Lice Lifters of Ocean County in Toms River, which opened in August 2016 and serves Ocean, Monmouth and Atlantic Counties as the premier lice removal company. The 28th Historic Dennisville Christmas House Tour is our premier fund raising event. On the 3rd Saturday of December a minimum of 8 beautifully decorated 18th and 19th century historic homes, as well as several public buildings and 2 churches, are open to the public. Please think about starting your holiday season with a festive glimpse of history in Dennisville at Christmas time!

Tickets are $15.00 and will be sold at the Dennisville Post Office, 24 Hall Ave. in Dennisville the day of the tour beginning at 3:30 pm. Advance ticket sales will be available online at www.dhhoa.org beginning October 1, 2017. Dennis Township Middle School Student Government students will serve as greeters to distribute programs and get tour goers started on their way. Houses open at 4:00 pm. The Post Office will be open for stamp sales as well. Soup and sandwich supper at the South Dennis Trinity United Methodist Church beginning at 3:00 pm until food runs out ($8.00 adults, $3.50 children under 12). Refreshments and comfort stations at the Dennisville United Methodist Church Social Hall, the Dennis Volunteer Fire Company and the Dennis Township Old School House Museum. The Middletones Carolers (Middle Township High School a cappella vocal quartet) will perform through the village in period costume. Alice Belanger McGuigan’s illustrated “Historic Dennisville: A Walking Tour” will be available for sale at the ticket center. Dennis Volunteer Fire Company will be open with new and antique equipment on display. Light refreshments will also be available there. The Dennis Township Old School House Museum will be open to visitors. You might want to take a quick look tonight and make arrangements to return for a more in depth visit at a later date.

The front enclosed porch shelters a brick well. Each and every room is lavishly decorated with mid century reminders of Christmases past: blow mold figures, painted and glass C9’s and C7’s, plastic table ornaments and festive cardboard decorations. Having never been on the Dennisville House Tour, the house had its inaugural opening during the 2016 tour and was very well received. Keep your coat on as there is no heat in the house!

It’s Pumpkin Time!!

A

s we say au revoir to summer, let’s say bonjour and bienvenue to autumn…and pumpkins. Our love for all that’s pumpkin has grown by leaps and bounds—in fact, at one time they were shunned. During the early days of settlers to the United States, pumpkins were “considered food of desperate times.” And those who ate them were mocked and called a “pumpkin eater” or “pumpkin roller.” But somewhere along the way, this radiantly orange fruit made a comeback and is associated with the fall and Thanksgiving celebrations. And today, pumpkin-flavored items are all the craze with sales reaching $414 million from the end of July 2016 to the end of July 2017!! So what is it about one of autumn’s favorite foods that makes us go bananas “pumpkin”? It may be because not only are they highly delicious, but at the same time they are highly nutritious. Considered a very low-calorie fruit, it is void of fat and cholesterol, but chock-filled with fiber, vitamins, minerals, and antioxidants. Let’s take a closer look at all that’s pumpkin.

Dr. Nina’s What You Need to Know About Pumpkins Health Benefits • Eyesight This plump fruit is one of the richest sources of beta-carotene

around. Beta-carotene is a powerful antioxidant that protects cells from damage and can be converted to vitamin A, an essential vitamin that aids in keeping our vision “20/20.” • Fills us up Foods rich in fiber make us feel full and help curb total calorie intake. And pumpkins and their seeds are a good source. Additionally, fiber in our diets aids with regular bowel movements. • Heart health Pumpkin seeds contain compounds called phytosterols which are associated with reducing “bad cholesterol”—named because it contributes to fatty plaque buildup inside our heart’s arteries that blocks blood flow. Pumpkins can help keep the the flow of blood, flowing. • Blood pressure reduction Potassium works to decrease blood pressure by balancing out the negative effects of sodium. And the good news, pumpkins are a good source of potassium and contain only trace amounts of sodium. • Cancer fighter. Beta-carotene is a powerful antioxidant that is believed to play a role in cancer prevention. And, too, pumpkin seeds contain plant sterols which are linked to fighting off some cancers. • Glowing skin Compounds found in pumpkins are believed to decrease wrinkles, increase collagen production (responsible for smooth, youthfullooking skin), and boost the skin’s renewal process. • Sleep tight The seeds contain tons of an amino acid called tryptophan that is involved in sleep/sleepiness. • Blood sugar control A recent study showed that rats treated with pumpkin extract had lower blood glucose levels, increased insulin secretion,

and more insulin-producing cells compared to those not given the extract. In fact, pumpkin is used as a holistic treatment for diabetics in Asia. • And, too, research shows that pumpkin and pumpkin seed consumption is linked to improved prostate health and immune system function; increased testosterone levels; enhanced mood; and anti-inflammatory benefits. In order to achieve these health benefits, we need to utilize the actual pumpkin, not the artificial flavorings. Make sure to choose pumpkins that are intended for cooking (smaller, with thinner skin), as opposed to decoration (typically large but are watery, stringy, and not as tasty). Whenever possible opt for whole pumpkin fruit that is completely developed—when you tap on it, you hear a fine woody note, it is heavy in your hand, and has a stout stem. And avoid those with bruises, cuts, or wrinkled surfaces. Also, a ripe pumpkin can be stored for several weeks at room temperature and up to three months in a refrigerator or other cool environment. Some of us enjoy the convenience of canned pumpkin; it obviates the need to cut, carve, saw, or cook. But make sure to choose pure canned pumpkin— without sugar or syrup.

Some ideas from experts to incorporate pumpkin into our meals include:

• Pumpkin chunks can be added to a roasted vegetable medley or practically any other dish, or as a side (the way that we prepare potatoes). • Pumpkin puree can be stirred into soup, stew, or chili; to flavor smoothies; add a fun autumn twist to pancakes; included in homemade pasta sauce; or as a spread or butter substitute. You can also utilize it as an oil substitute for baking recipes (e.g., if it calls for 1 cup of vegetable oil, use ½ cup pumpkin puree and ½ cup vegetable oil). • Pumpkin seeds can be enjoyed on their own or as a salad or meal garnish. One easy method to prepare seeds is to wipe them off with a paper towel after removing them from the pumpkin and then placing them as a single layer on a cookie sheet, and roasting them at 160 degrees for 15-20 minutes (this preserves their healthy oils). Pumpkin seeds are also sold, ready to go.

Covert Operations: Women in the Military In October of this year, I was a participant in a training alongside law enforcement. There was a mix of police officers, corrections officers and Port Authority officers, all from various departments in New Jersey. Hidden amongst the group were two veterans who served active duty in the military and then entered in the law enforcement sector upon their discharge. One of the veterans was a female who modestly identified herself as being a veteran. Fascinated by this officer’s modesty, I began contemplating how many other women have dutifully served our country and yet have remained “under the radar” without recognition for their commitment for their service.

the line of duty. By World War II, the number of females who enlisted increased dramatically to 350,000. The Japanese also captured 67 women who became POWs for three years. By 1942, American women were stationed in England, France, Australia, New Guinea and the Philippines. The military branches had created reserves for females. Specifically, the Navy created Women Accepted for Volunteers Emergency Service. The Marines created Semper Paratus and the Marine Corps Women’s Reserve. The Air Force created Women Air Force Service Pilots. In 1991, 40,000 women had served in the Gulf War. The Gulf War was the first time that men and women were deployed together to engage in active warfare. Although women were permitted to serve in the war, they were limited to certain positions, and “by assignment” they were kept from combat zones. It would not be until 2013 that females would be approved to serve in combat zones. There are two females who received the Silver Star Award. The first was Leigh Ann Hester for her “exceptional valor” in close-quarter combat as a result of her leadership in the Iraq War. The second was Monica Lin Brown for her heroic effort to protect wounded soldiers in her platoon by risking her own life and running into the line of fire to save their lives. It is reported that she placed her body over her fellow soldiers to protect them from further harm. As noted above, women were not permitted to serve in combat zones. It would not be until 2015 that an order would be signed permitting women to serve. As part of the training, women were required to complete Army Ranger School. In 2015, Captain Kristen Griest and First Lieutenant Shaye Haver were the first females out of 96 students to graduate from Army Ranger School, which was also the first year the school accepted females. Currently there are only two women who have held the rank of general. The first was General Ann E. Dunwoody, who retired from active duty in 2012. The second is General Janet C. Wolfenberger (USAF). There are many amazing females who have served our country and have remained “under the radar.” These women appear modest and yet have had a significant impact on our military units. Through their perseverance they have leveled the battlefields to stand side by side with their male counterparts. The motto of the military has evolved to include women to “be all they can be.”

As I began researching for this feature, histiry has reflected that women in the military traditionally were assigned to “auxiliary” roles. However, women served in the military dating back to the Revolutionary War, the Civil War and the Mexican Wars. It was reported that women would disguise themselves as men and enlist into all branches of the military. In 1782 Deborah Sanson Gannett from Plymouth, Massachusetts, enlisted in the Army under the name of her deceased brother. She was injured in the line of duty two times and was so concerned her identity would be revealed, she even tended to her own surgical needs. She, alongside of her male counterparts, fought together in the Revolutionary War for 17 months. Deborah was compensated 22 years after her service and was granted a pension from the United States Army. Following in the footsteps of Deborah, Margaret Corbin was known to have entered the battlegrounds with her husband, albeit under cover as a man! Margaret became famous when her husband was killed and she continued to load and fire the cannon against the British. Elizabeth Newcume boldly joined the military in 1847. Elizabeth spent 10 months on active duty disguised as a man. After it was discovered that she was female, she was immediately discharged.
