Start 2018 By Taking Care Of Yourself! Out with the old and in with the new, as the saying goes. The year of 2017 will soon be a memory, and 2018 has come. It is now time to look back at this past year and ponder our life’s events. What did we do, didn’t we do? It’s time for a “New Year’s resolution”! This year should be about you and your health. The recommendations and guidelines are clear. All women over the age of 21 should have an annual gynecological exam. This is considered routine, and it is independent of sexual activity. I continue to recommend that women perform monthly self-breast exams (SBE). A woman should start her SBE when mature enough to perform such

a mom or dad to do or for a girl to hear. But nonetheless it is very important. It may be easier to hear it from her doctor. These topics are very important for women in this stage of their lives. I always welcome and encourage women younger than 21 to have a visit with me prior to that age. Ideally, somewhere between 13 and 15 years old is a good starting point. At this visit, a young woman is introduced to the idea and concepts of a gynecologist and the care provided. I keep it upbeat and positive, and base the conversation and details on the level of maturity of each individual. I try not to scare or overwhelm them on their first visit.

Women 30 and above should have HPV testing done with their pap.

Annual gynecological exams should consist of a review of medical and surgical history, medication history, sexual history, menstrual, STD, and urologic history. It would also include the examination of the breasts and abdomen,

I review routine health and screening recommendations. I also take a family cancer history to attempt to identify patients who are at increased risk of breast, ovarian, uterine or colorectal cancers. If patients are identified, I then review available testing for genetically linked cancers of those areas, many of which can be performed in our office. At 35 to 39, I recommend a baseline mammogram. This allows the radiologist a reference to compare future mammograms, which should begin annually at age 40. I also screen women for HIV and vitamin D deficiency. It is recommended for all women ages 13 to 64 to have annual HIV testing, independent of risk factors. This is to identify asymptomatic HIV-positive persons. Annual vitamin D testing is done since low levels have been associated with an increased risk of colon and breast cancers. Testing allows me to identify, educate and potentially prescribe certain supplements. As an aside, the recommended daily requirement for vitamin D is 800 IU. This is available over the counter as vitamin D or D3. I generally recommend the vitamin D3.

For all women ages 21 to 25, testing for gonorrhea and chlamydia are part of the exam. In this age group, education is a key component of the visit. I try

Knowing the feel of the breast tissue will help one distinguish what has been there, what is new, what may be abnormal, etc. Concerns regarding your SBE should be referred to your gynecologist for prompt evaluation.

Varicose Veins: Not Just a Cosmetic Problem

ne of the most common circulatory system problems affecting individuals of all ages is venous insufficiency, also known as “Venous Reflux Disease”. This occurs when one-way valves in the leg veins begin to fail and become leaky. This causes blood to pool in the legs and can lead to the development of large protruding legs veins, known as varicose veins. It can also cause a variety of painful or uncomfortable symptoms in the affected legs. Some of the most common of these symptoms are leg pain, leg fatigue and heaviness, swelling and skin itching. However, some affected individuals do not have any of these risks. The diagnosis of venous reflux disease is made with physical examination and painless Doppler ultrasound testing. Advances in treatment now make relief of the symptoms and appearance of varicose veins possible without the need for hospitalization or painful vein stripping surgery. For some people, wearing special venous support stockings will help control symptoms. Apple Through Pre Trial Intervention (PTI) Redeemed Them

his article serves as a general overview regarding the Pre Trial Intervention (PTI) program. PTI is a supervisory treatment program that was enacted prior to the New Jersey Criminal Code Title 2C being codified. It can be found in NJSA 2C:43-12. The goal is “it is policy of the State of New Jersey that supervisory treatment should ordinarily be limited to persons who have not previously been convicted of any criminal offense under the laws of New Jersey, or under any criminal law of the United States.” The law has gone through many modifications over the years and is constantly changing. When a person is charged with a crime, or in some instances an offense (domestic violence disorderly persons offense), that person may be eligible for supervisory treatment in PTI depending on a panoply of variables. These variables include the PTI Director (court personnel) and their willingness to accept the person into the program, the prosecuting agency and their willingness to accept the person into the program, the charge itself, a victim’s input if applicable, and many other considerations. If a person has received a prior PTI, a conditional discharge or a conditional dismissal, he or she is ineligible for this type of supervisory treatment. NJSA 2C:43-12(g)(1). Ordinarily, when you are accepted into the PTI program, you DO NOT have to plead guilty to the charge(s) subject to certain exceptions found in NJSA 2C:43-12(g)(3). The accused should consult a qualified criminal defense attorney immediately and make a timely application so as not to delay or prejudice their rights. The fee for the application is $75 and almost always no statement that is made to court personnel can be disclosed to the prosecuting agency nor can it be used in a subsequent civil or criminal proceeding. The maximum term limit that a person can be supervised in PTI is thirty-six (36) months. If a person does well, and upon approval of the program director, the prosecuting agency and its consent, and the court’s approval, the term may be shortened or even terminated early. If a person violates the terms and conditions of PTI, dismissal from the program can occur. Again, hire a qualified criminal defense attorney to motion to the court for remaining in the program. If the term is completed successfully, the charge(s) are dismissed. However, the charge and/or arrest will still remain on a person’s record and

they have to wait six (6) months in order to expunge it. NJSA 2C:52-6(c). Additionally, if a person is rejected from the program, a PTI appeal can be filed arguing that there has been a “patent and gross abuse of discretion” in rejecting the individual. An example would help illustrate how the process works. On January 1, Mr. X gets arrested at a local department store for shoplifting $650 worth of merchandise. He ultimately gets caught on surveillance video and the local police arrest him and charge him with third degree shoplifting, a crime with a maximum punishment up to 3-5 years in state prison with various fines and assessments. He is released on a summons (he has no prior record, lives locally, has no other pending charges and was born in the United States). He hires a qualified criminal defense attorney and makes a timely application for the PTI program. The program director agrees to let him in and the prosecuting agency is at first hesitant due to the department store and its concern of recent spikes in shoplifting, but nonetheless acquiesces. Mr. X comes to court on April 1 on an accusation (waives his right to have this matter presented to a grand jury) and the court admits him into the program for a period of 12 months with conditions, such as making restitution to the department store and keeping away from that department store. Mr. X finishes the program in the requisite time frame and the indictable (felony) charge is dismissed. Mr. X then waits until October 1 (6 months after) and moves to expunge his arrest, with which he is successful, and thus avoids traditional prosecution and a criminal record. As a famous proverb tell us: “Everyone makes mistakes in life, but that doesn’t mean they have to pay for them the rest of their life. Sometimes good people make bad choices. Tired of Sinus Pain & Pressure? How Do I Know If I Am a Candidate for Balloon Sinus Dilation?

Over the past year my whole approach to treating patients with sinusitis has changed. In the past I treated the vast majority of my patients in the operating room, under general anesthesia. This was followed by weeks of recovery with frequent sinus debridements as healing occurred. Now I am treating clients in the office unless their disease dictates otherwise. In-office balloon sinus dilation is changing the way patients with chronic sinus disease are being teated. For decades clients have had to undergo surgery with general anesthesia, extensive postoperative debridement, time missed from work and other activities. Balloon sinus dilation is performed in the office under local anesthesia with patients returning to work in 24-48 hours with little to no need for debridement.

What Are the Risks Associated? Sinus dilation, like any procedure, has risks. However, the complication rate of balloon sinus dilation is less than that of traditional sinus surgery. Your doctor can explain the risks specific to your procedure.

Is It Painful? Local anesthesia is first applied via cotton applicators inserted in the nose. Injections of local anesthetic follow but the majority of patients have no discomfort due to already being “numb.” Post procedure pain medicine is provided but few patients use more than two doses.

What is Balloon Dilation? A small fiber with a balloon attached is threaded into the sinus openings and the balloon is inflated. This widens the sinus and causes permanent bone reformation which allows the sinus to function normally. The bottom line...breathing is much easier!

What Sets Me Apart From Other Family Lawyers By Marla Marinucci, Esq.

O

ver the past few years, I have written a few articles on how to choose the right family law attorney. When I became certified by the Supreme Court of New Jersey as a Matrimonial Law Attorney in 2013, I did an article about the importance of hiring a certified attorney to handle your divorce. Last year, I was accepted as a Fellow of the American Academy of Matrimonial Lawyers (“AAML”), which is considered to be an incredibly high level of achievement for family law practitioners. I again wrote another article telling readers how important it is to have a Fellow of the AAML represent you in your divorce or other family law matter.

These accomplishments are all well and good and definitely things I am extremely proud of; however, in thinking what else I could offer my clients, aside from my legal knowledge and experience, is this: I can relate to what you are going through. Not only have I gone through my own divorce, but my ex-husband, who had previously been married, had a daughter. Thus, I have an ex-husband; my exhusband has an ex-wife; a stepdaughter; and my ex-husband and I have two children together. I can personally and professionally relate to just about everything when it comes to divorce, probably a lot more than most attorneys out there can. I must first acknowledge that my ex-husband’s ex-wife (whom I refer to as “ex-wife #1” as I have assumed the title of “ex-wife #2”) is a pretty amazing woman. My stepdaughter, whom I’ve known since she was 6 years old and who is now engaged to be married, is equally as amazing and I couldn’t be more proud of her. It hasn’t always been rainbows and unicorns because, well, that’s just impossible. But for as long as I can remember, which is more than twenty years, we have all been able to make things work and get along.

For many years during our marriage, my ex-husband and I and our two children would spend Christmas Eve with my stepdaughter and ex-wife #1 at her house, along with my parents, ex-wife #1’s boyfriend’s (who is one of the nicest people on this planet) ex-wife and her husband, and many others. It takes a while to understand all the “ex’s” here but it was always in the best interest of the children that we do things together, such as birthday parties and other special occasions, and most importantly, that we all got along. I would go as far to say that we truly enjoy each other’s company and have shared many laughs over the years. Our children refer to ex-wife #1 as their aunt and they absolutely adore her. She adores them as well. This is how it should be post-divorce. Arguing and resentment take too much energy and its effects are felt by the children. Granted, it is not easy and it does take some time, but it is possible and is always in the best interest of your children for you and your ex to put your differences aside and at least not argue in front of them. Obviously if there is a restraining order in place against one of the parties, then communication is prohibited. But in the majority of split families, communication is permitted and is lacking. It is not possible to coparent your children without communication. After my own divorce, my ex and I and our children took a family vacation together. We wanted to assure the children that we were still a family and always will be. So far I think we have been successful, as ex-wife #1 and my ex-husband were with their daughter. In the end, finances don’t matter. Money comes and goes, but your children don’t. I don’ t expect anyone to rush out and take a family vacation with their ex-spouse and children after their divorce and I realize that my particular case is somewhat atypical. However, when you are searching for a lawyer for your divorce or other family law matter, unlike many talented attorneys out there who practice family law, I possess not only the expertise in this area of law, I have also experienced it personally. Hiring an attorney who has never been a parent or who has never been through a divorce to negotiate a parenting plan on your behalf, and who lacks this personal experience is probably not in your best interests, and especially, not in your children’s best interests.

Perhaps the best part about her profession is that it includes many of her own family members. “It’s wonderful to be working with my family but debates do occur because each of us is so passionate about what we do. My family grew with my business as I had started the company while my own children were still in school. It’s nice to see the two of them grow alongside one another. Each of my children play a critical role in my company,” she adds. Hers is not the typical 9-5 job and sometimes the job is never really “done.” The greatest advantage for her is that they all live next door to each other. Their bond is very tight and it enhances her business in immeasurable ways. For Kathy, it all comes down to making a difference in her client’s lives. Understanding Common Foot Conditions The human foot is a complex structure containing 26 bones, 33 joints and more than 100 tendons, muscles, and ligaments. The components of your feet work together, sharing tremendous pressures of walking. An average day of walking, brings a force equal to several hundred times to bear on the feet. This explains why your feet are more subjected to injury than any other part of your body. FOOT ILLNESS Foot illnesses are among the most common of our health problems, although some can be traced to heredity, many are caused by the cumulative impact of a lifetime of abuse and neglect. Your feet are specialized structures and therefore require specialized care. A podiatrist can make an important contribution to your total health whether it is regular preventive care or surgery to correct a deformity.

ATHLETE’S FOOT

Athlete’s foot is a skin condition usually starting between the toes on the bottom of the feet. It is caused by a fungus that thrives in a warm, dark, and moist environment. Signs of athlete’s foot are dry scaling skin, itching, inflammation, and blisters. You can help prevent infection by washing your feet daily with soap and warm water and drying carefully especially between your toes. Changing shoes and socks regularly will decrease moisture.

BLISTERS

Blisters are caused by skin friction, do not pop them. Apply moleskin or an adhesive bandage over a blister and leave it on until it falls off naturally in the bath or shower. Keep your feet dry and always wears socks as a cushion between your feet and shoes. If your blister breaks on its own, wash the area, apply an antiseptic, and cover with a sterile bandage.

BUNIONS

Bunions are misaligned big toe joints which can become swollen and painful. The deformity causes the first joint of the big toe to slant outward and the big toe to angle towards the second toe. Bunions tend to be heredity based but can also be aggravated by wearing shoes that are too narrow in the forefoot and toe. There are conservative measures that can help to minimize discomfort of bunions such as padding, strapping, and orthotics but frequently surgery is recommended to correct the problem.

CORN AND CALLUSES

Corns and calluses are protective layers of compacted dead skin cells. They are caused by repeated friction and pressure from the skin rubbing against bony areas. Corns ordinarily form on the tops of the toes and calluses usually on the soles of the feet. The friction and pressure can cause pain and may be relieved by moleskin or padding on the affected area. Never cut corns or calluses with any instrument, and never apply home remedies.

FOOT ODOR

Foot odor results from excessive perspiration from the more than 250,000 sweat glands in the foot. Change your shoes daily to let each pair air out, and change your socks twice daily if your feet tend to perspire. Foot powders and antiperspirants can help to lessen the odor.

HAMMERTOES

Hammertoe is a condition in which any of the toes are bent in a claw-like position. It occurs most frequently with the second toe, often when a bunion crowds the second toe forcing the second toe to become deformed. Although this condition usually occurs from a muscle imbalance, it is often aggravated by poorly fitting shoes. Avoid pressure on the toes as much as possible.

on the heel bone, ligaments, and nerves in the area. Stress results from walking or jumping on hard surfaces or from poorly made footwear. Being overweight is also a major contributing factor. Some general health conditions, i.e., arthritis, gout, and circulatory problems can also cause heel pain.

HEEL SPURS

Heel spurs are growths of bone on the bottom of the heel. They can occur without pain, although pain may result when inflammation develops at the point where the spur forms. Both heel pain and heel spurs are often associated with plantar fasciitis, inflammation of the long band of connective tissue running from the heel to the ball of the foot. This may become inflamed with pain especially noticed upon rising from bed in the morning. Treatments may range from exercises, physical therapy, and custommolded foot orthotics to anti-inflammatory medications and cortisone injections.

INGROWN NAIL

Ingrown nails are nails whose corners or sides dig painfully into the skin, often causing an infection. They are frequently caused by improper nail trimming but also from pressure from shoes, injury, fungus infection, heredity, and poor foot structure. Toenails should be trimmed straight across with nail clippers. If painful or infected, your podiatrist may remove the ingrown portion of the nail.

NEUROMAS

Neuromas are an enlarged, benign growth of the nerves. They are most commonly found between the third and fourth toes. They are caused by bones or other tissues rubbing against and irritating the nerves. Abnormal bony structure or pressure from poorly fitting shoes can also contribute to this condition. The most common findings are pain, burning, tingling, or numbness between the toes and the ball of the foot.

WARTS

Warts are caused by a virus, which enters the skin through small cuts or breaks. Children are most susceptible to the wart virus especially between the ages of 5 to 17 years old. Most warts are harmless and benign, even though painful and unsightly.

Do Artificial Sweeteners Help with Weight Loss? Written By Barbara Greenling, DNP, MSN, APN-C

Do you drink diet soda, versus regular, thinking you’re doing your body a favor? Are you under the impression that artificial sweeteners are a healthier alternative, especially for weight loss? You are not alone. Recent statistics show that close to a third of Americans say they use artificial sweeteners daily. Typical examples include aspartame (Equal, NutraSweet), sucralose (Splenda), and stevia (Truvia, Pure Via). They’re all available in packets and are also added to soda, yogurt, and other foods.

But do these sugar substitutes actually help you lose weight? New research suggests they do not. In fact, these zero-calorie additives may have the opposite effect — and possibly even increase the risk of high blood pressure, diabetes, and heart disease. Researchers reviewed 37 studies that included a total of more than 400,000 people. But only seven were “gold standard” randomized controlled trials, which compared people who used artificial sweeteners with a control (placebo) for six to 24 months. Those studies found no clear proof that the fake sugars helped overweight people shed pounds. Perhaps a different approach to dropping the pounds is in order. Instead of focusing on replacing sweetness in your food and drinks, maybe re-training your palette to less – sweet alternatives would be best in the long run. For example, unsweetened iced tea is far more thirst-quenching and refreshing than the sweetened version. You may actually find you prefer it if given the chance! Start to experiment by leaving your comfort zone a bit and trying new ways to limit and enjoy your sweets. This is especially important if you are overweight so as to prevent bigger problems down the line. If, however, you’ve already been diagnosed with type 2 diabetes and you’re overweight, you should get started as soon as possible on a weight loss plan. It

is important to work with a medical professional or registered dietitian to help you figure out a plan that will work for you—a healthy meal plan, physical activity, and realistic goals will help you reach a healthy weight.

There are many advantages to losing weight (and not just diabetes-related ones):

• Boost your energy level • Lower your cholesterol levels (especially important for people with type 2 diabetes) • Protect your heart (also important for people with diabetes, since heart-related complications are very common) • Make it easier to control your blood glucose level As you may already know, losing weight can be a challenge, but don’t let that stop you. Do whatever you need to in order to stay motivated. It is the amount of calories we eat that contributes to weight gain. Make small changes. Learn portion sizes and reduce the amount of snacks in your day to reduce the total amount of calories you consume each day. Find cookbooks with healthier recipes using low-fat options. A Final Note on Weight Loss For some people, losing weight doesn’t help them have better control of their blood glucose levels, and that’s all right. They may need to use medications or insulin to keep their blood glucose level in the normal range, but they should also still eat healthy foods and increase physical activity. Everyone should strive for a healthy weight. Losing weight can help lower your body’s insulin resistance, but if it doesn’t help you achieve better blood glucose control, that doesn’t mean you’ve failed—or that you should give up. Losing weight and then maintaining a healthy weight are healthy choices for life—whether you have type 2 diabetes or not.

There’s a life satisfaction activity that I sometimes ask clients to complete. It goes beyond the general question – and it’s led to many “aha” moments. I’m tweaking it here, for the sake of brevity. So, grab a piece of paper and a pen / pencil.

The Culture of Disrespect

“I

feel alone, ashamed; there is so much turmoil inside me, I feel as if I’m going to explode. How can I get free of this? What will make this stop? I’ve tried everything I know to make this stop, but it just keeps happening over and over. For years I’ve had to deal with this vile conduct and I just can’t take it anymore. I feel hopeless; and I know this isn’t right, but no one can help me. Now I feel as though there is only one thing for me to do to escape this; who will notice or care anyway? I’m stuck in a no win situation. It’s always my fault. I feel so embarrassed and guilty all the time.

“I

like control! So what of it? I’m important and because of that, what I want matters. My desires come first and besides, I’m not hurting anyone ‘cause they want me. I know they can’t show it because of my position, but that’s why I push it a little to get things going. No big deal. It works out pretty good most of the time. And I like it a lot more knowing I’m in control and can determine the flow and outcome I want. I like that I have the control more than the sexual stuff. It’s great to be powerful. For those like me, you know what I’m talking about.” The first speaker is clearly the recipient of unwanted, unsolicited and nonconsensual sexual misconduct. This can have a wide range of factors, i.e. the age of the person when the misconduct began and ended; the frequency and form of sexual misconduct utilized e.g. verbal, inappropriate touch, intercourse in the form of rape, sodomy, etc.; how many people abused the individual during the episode(s); the extent of physical harm or threat thereof; were there others the abused person cares about that were physically harmed or threatened? How much mental and emotional and/or verbal abuse was perpetrated pre, during or past the assault/s in order to indoctrinate and then intimidate? Was the perpetrator/s of the abuse known to and trusted by the abused person; or was it someone (or persons), unknown to them? Were any objects of weapons employed during the abuse (assault)? What were the aftermath, the consequences and dysfunctional pathological states as the result of the abuse (assault)? These are only some of the many variables that come into the understanding of what occurs to a person subject to these horrific events and for how long they suffer them. One of the most astonishing problems is the absolute lack of respect the person tolerates. The term respect is defined as “deference; esteem, reverence, veneration, appreciate.” This is the total antithesis of what the abused person endures from their abuser. For the abuser, it’s what they can get the abused person to do; however they go about it is not relevant. It is the result they control. There are wide spread public displays in many sectors where men proudly display their dominance over women such as in music videos where sexually explicit images sell big time. For those who are easily influenced by such visuals, they can begin to ruminate and fantasize to the extent they believe the idea of sexual dominance is acceptable. Very famous and powerful people do it all the time. So why not act on such beliefs.

The second speaker in this article is depicting some of the mindset, albeit pathological, beliefs and or addictions that can converge on a perpetrator’s mind and assist or even compel them to act on their destructive impulses with impunity and entitlement verging on righteous indignation and denial for the purpose of serving themselves. The rationale utilized by assailants to rationalize their aggression stems from many ill begotten sources. Some expert researchers found that people in power, men in this case, lack the ability to have empathy for others in any fashion, and are unable or perhaps unwilling to correctly interpret other people’s emotional states and perspectives accurately or in real time. These shortfalls of perception, some would label as character deficits, facilitate impulsivity and all types of moral and ethical violations; many of criminal proportions. Power is defined as influence; command; authority; rule; supremacy; influence…” Such is the case of men in powerful positions bolstered by corporate, industry, clergy, the financial dependence of their victims, societal indifference and cronyism, allow them to abuse not just their positions, but victimize and prey on the women and men who they have access to. They subordinate, indoctrinate and eventually, intimidate for the addictive perk of control and absolute power over another in a very invasive, degrading manner; irresistible to many. Arguably, those who come to be in such positions of power do so by being manipulative, unscrupulous, self-serving, aggressive, and devoid of any moral compass and are most likely narcissistically fragile. On the other hand, positions of power are traditionally not for the meek at heart. Good leaders need to be decisive, risk-takers, have the ability to influence and manipulate situations for the good of the business. They must be able to take control of difficult situations and are responsible for the outcomes they achieve, or not. The difference is they do not pathologically abuse these traits to victimize others for their own corrupt gain. The abusive leaders require conquest and adoration to sustain superficiality and false greatness. In short, their deep-rooted insecurities, inadequacies and self-appraised extremisms, require them to seek “filler” for their emptiness at the dismissible, entitled expense of many people—mostly women. They believe they can get away with, or blame these women for their own clandestine disease and debauchery. The centuries old ways of the world have been tainted to toxic levels on many human abuses ranging from slavery and genocide to atrocities in many forms; these include racial, sexual, religious, gender, economic and yes, governmental. Societally, consciously or unconsciously, we have long overlooked these exploitative excesses until there is an outcry of enormous proportion that is repeated and sustained over time. We even reward and applaud many in power who have wealth and authority, but are found to be lacking in basic decency and fortitude. Unfortunately, this implies we have lowered our standards to the point of essentially allowing some powerful people to do many very bad things. The recent flurry of public exposures of the misconduct of men who have positions of power and influence, is just the tip of the iceberg and seriously overdue. The consequences of these “discovered” pathologies remain to be seen; but denial and trivialization of these distressing actions should not be tolerated. These behaviors need to exposed, measured and consistently monitored in order to devise strategies that provide meaningful deterrents to those who perpetuate these crimes and for those being groomed, intentionally or unintentionally, to become thieves of human goodness and inalienable rights coming up next. Prevention is best in conjunction with ongoing vigilance and intolerance. A tipping point for intolerance will require strong conviction and public stalwartness. It needs to be international, politically supported, parentally

Sexually Abuse— What Happens? trained and consistently tracked for transgression and have constantly reinforced improvements. It is very difficult to change quasi-sanctioned human (mostly masculine) behaviors, but not insurmountable. No individual can do this alone, but no individual should capriciously “just let the other guy take care of it.” A concern is the tendency for the public to become weary and complacent, almost unconsciously duplicitous about these painful abuses when they become almost daily reports and allegations. The seriousness and trauma derived from these violations at times become relegated to “the new normal; who is it now and how many; I can’t keep track of all this – it’s everywhere, so what else is happening in the world; etc.” It can go from shock and outrage reactions to even falling into the day’s entertainment and idle party chatter, losing the severity and proactive responses required to correct and eliminate this aggressive predatory behavior now.

The experience and psychiatric consequences of women who are abused sexually and emotionally will be discussed in Part 2.

After which, she received her medical degree from the Philadelphia College of Osteopathic Medicine. Her internship and residency in Family Medicine was completed at the Virtua Health System. M

ost of us know someone who returned from the grip of a cancer diagnosis. But no one has ever known someone with profound dementia or Alzheimer’s disease (AD) who was cured and returned to his or her previous state of good health with a pristine memory. Previous medical training taught us that there was no way to accurately diagnose AD without doing an autopsy of the deceased brain. That is no longer true. Many of my patients are caring for aging parents with AD. Too many of my patients have told me that their mother is sitting in a wheelchair holding a baby doll, and not knowing who they are. One patient was visiting both parents with AD at a nursing home and his doctor asked him, “How does it feel to be looking at your future?” Oh my. After hearing this statement of emotional medical malpractice, I made it a priority to study and hopefully learn how to prevent AD. In a recent stroke of luck, I met an amazing scientist who is doing research injecting nanobots into brains to scavenge bacteria that precipitate the onset of AD. I told him that the mission statement of my medical practice is to prevent heart disease, strokes, dementia, and AD. He told me to read an amazing book that was just published called The End of Alzheimer’s – The First Program to Prevent and Reverse Cognitive Decline by Dale E. Bredesen, MD. Dr. Bredesen is internationally recognized as an expert in the mechanisms of neurodegenerative diseases such as AD. After more than 30 years of research and publication of over 200 scientific medical articles, he describes the 36 major causes of AD. Dr. Bredesen developed the biological rationale for ReCODE (reversal of cognitive decline). Yes, you are reading correctly—a program to prevent and reverse dementia. There is a genetic test called ApoE that partially determines your risk of developing AD. ApoE comes in three varieties—ApoE2, ApoE3, and ApoE4. You get one gene from each parent. ApoE2 portends the lowest risk for developing AD, ApoE3 puts you at a higher risk, and ApoE4 puts you at the highest risk.

Just because you may have inherited ApoE4—this is powerful information to do everything on the ReCODE protocol to prevent your genes from expressing themselves. There are three types of AD. Type 1 is inflammatory (hot) and there are laboratory tests to measure the amount of inflammation, such as increased hsCRP (highly sensitized C-reactive protein), decreased albumin/globulin ratio, increased interleukin-6, increased tumor necrosis factor, deficiencies of nutrients, and deficiencies of at least eight hormones. There is also a Type 1 and 2 (1.5), which is glycotoxic (sweet). Take your blood sugar! Type 2 is atrophic (cold) and the overall support for brain synapses has dried up. Laboratory tests for levels of hormones—including thyroid (not just TSH levels), cortisol, DHEA-S, estrogen, progesterone, testosterone, and pregnenolone— are usually suboptimal. Vitamin D is often profoundly low. Just FYI, Vitamin D turns on over 900 genes, suppresses tumor formation, and is essential for creating and maintaining brain synapses. Other tests for diagnosing Type 2 AD is insulin resistance, or insulin is too low, and homocysteine may be high. Your homocysteine should not be higher than 7.5, regardless of what the lab calls a normal range. Type 3 is toxic (vile). Characteristic biomarkers are as follows: strikingly low zinc levels, triglyceride levels that are disproportionately low compared to cholesterol levels, MRI brain studies demonstrating many brain areas have atrophied (shrunk), MRI findings called FLAIR (fluid–attenuated inversion recovery), in which there are multiple abnormal small white spots on the MRI, a high copper/zinc ratio, abnormal PET scans, abnormal spinal fluid, hormonal abnormalities such as LOW cortisol (most think they have high cortisol leading to weight gain, not always true), low DHEA, high reverse T3 (rarely tested), low free T3 (rarely tested), low pregnenolone, low estradiol, low testosterone, and other hormonal abnormalities. Type 3 AD patients frequently have high blood levels of toxic chemicals such as mercury or mycotoxins (produced by mold). Mercury is found in tissues such as the brain and bone, so blood tests aren’t an accurate indicator of its presence. Patients often ask me how long I’m going to take bioidentical hormone replacement therapy. Since the top item on my bucket list is to see a patient the day I die at 112, I say until the end. I have amazing patients who work with me. They study, ask lots of questions, learn to eat clean, replace many hormones and nutrients, monitor their blood sugar and blood pressure, floss and use a Sonicare toothbrush twice a day (the silent bacteria in your mouth go to your brain), find exercise they love, get adequate sleep, become sexually active again, practice periodic fasting (I don’t eat from 7 pm to 7 am…and I don’t eat three hours prior to bed), and become lean and muscular again. My program to die old feeling as young as possible takes 20 minutes a day. So yes, this is a commitment. But so is getting your hair done, getting manicures/waxed/injected, shopping for the perfect outfit, and applying cosmetics. Your life, your choice.

Believe it or not, cough and cold season is upon us. Our nights are becoming very cold and children have been back in school for about six months now. This is the season everyone dreads. It seems we see people with the sniffles everywhere—at work, at the mall and even at home. Now is the time to take some steps that will reduce your risk of catching a cold and stock up on things that will help with the symptoms when the inevitable happens.

Bob adds, “We continue to grow as a result of our loyal customers who refer our company to others. Our customers and our employees are the most important people in our business. We are dependent on them.” CW: Tell us about the main services you provide. Bob: We have a walk-in retail business for copying, blueprint copying, fax and e-mail service along with document scanning. We also offer commercial printing for letterheads, business cards, forms, invitations and graphic design. Finally, we offer large format color printing for banners, posters, indoor and outdoor signs, vehicle graphics and vinyl wraps. CW: What is the single most important aspect of your business that sets it apart from others like it? Bob: We provide very fast turnaround for most projects by our courteous and knowledgeable staff. We take a “can do” attitude

to every single request. It really comes down to good old fashion customer service. CW: How did you become involved in printing services? Bob: About 16 years ago we relocated into a larger facility on Fire Rd. in Egg Harbor Township. Being as we were located on a major highway and surrounded by commercial and retail businesses, we decided to create a new division focused on retail and commercial printing. Already in the digital copier equipment business, it seemed like a natural diversification. I felt that we would be able to leverage our expertise in the equipment business to be successful in the commercial printing business. CW: How does it feel to have your children follow in your footsteps? Was it a natural progression for them? Bob: It’s extremely rewarding. I think many parents would love to have their children follow in their footsteps especially if they own a business. It presents excellent opportunities and challenges for everyone involved. I feel very fortunate to have both of our sons working in the business. CW: Tell us about the vehicle wrapping services you offer. Bob: We have provided vehicle wrapping and lettering for over four years with over 100 projects completed. They are mostly advertising graphics along with partial wraps and a few full color changes. This is an extremely labor intensive and technical procedure. We provide a complete solution from concept and design, through production and installation. Ethel Harrison, LPN, BAYADA Home Health Care Ethel has been with BAYADA Home Health Care for 22 years— first as a BAYADA Home Health Aide, and now as a BAYADA Nurse. Previously a casino worker, she was drawn into health care by an experience in the hospital. “My father became very ill with pneumonia, and was in the ICU,” Ethel recalls. “As I watched his nurse, I thought, ‘I can do this!’” Harrison took a certification course at Shore Memorial Hospital and started working as a BAYADA Home Health Aide to see if the field was right for her. She soon discovered her work to be very rewarding, and went on to earn her licensed practical nurse degree and begin her career in home nursing. “I truly love helping people,” Harrison tells us. “And I like working in the home care setting where I can work one-on-one with clients. Myths, Darn Myths, and Mixed Messages

“T

he great enemy of the truth is very often not the lie, deliberate, contrived and dishonest, but the myth, persistent, persuasive and unrealistic.” Truth be known, when it comes to our health, it is important to study the facts to understand how behaviors or change of behaviors may benefit us and those we love. The healthcare industry is constantly making new breakthroughs and reporting important facts on studies. However, the Internet, testimonials by friends and neighbors, and marketing gimmicks have put out a ton of misinformation about exercise. And it may be difficult to discern. We all want to be in our best possible health and shape, and when we read something that “makes sense” or seems possible, we are likely to believe it even if there is no science to back up the claims. Let’s take a few moments to debunk some commonly held exercise myths.

Dr. Nina’s What You Need to Know: To Debunk Exercise Myths Myth: I can exercise my way out of a bad diet

From a practical perspective, exercise, while critical to a healthy lifestyle, cannot neutralize or combat a bad diet. In order to burn 500 calories, a 160-pound adult would need to walk at 3.5 miles/hour for 80 minutes; jog at 6.5 miles/hour for 40 minutes; cycle in spin class 39 minutes; mow the lawn 60 minutes; swim laps 53 minutes; or Zumba 50 minutes. It is an awful lot more than most people realize! And brings some credence to my saying that a moment on your lips can mean an eternity on the treadmill or bike.

Myth: You sweat toxins out of your body

Our body has effective, built-in toxin elimination mechanisms via our liver, kidneys, and gut—not by sweating. The primary function of perspiration is to regulate body temperature. When our body is hot, we sweat and the moisture makes our skin feel cooler; when it evaporates into the environment, it takes with it the heat. Sweat is mostly comprised of water (99%) and a small amount of salt, proteins, carbohydrates, and urea. “Trace,” or minuscule, amounts of toxins may be detected in sweat, but they get rid of less than 1% of the body’s total toxin content. It should be noted that heavy sweating can cause you to become dehydrated and, as a result, force your kidneys to conserve water and hang onto toxins. So the next time you know you will be breaking out a sweat, take a bottle of water with you.

Myth and Truth: You must stretch before you exercise

There is no scientific evidence that static stretching—defined as stretching while you stay in place such as bending over to touch your toes or pulling your ankles towards your hips—before a workout can prevent injury or decrease delayed-onset muscle soreness. In fact, some studies have shown that static stretches lasting longer than one minute can strain your muscles and slow you down. On the other hand, dynamic stretching, such as swinging your arms or walking lunges, increase your range of movement and blood and oxygen flow to your muscles prior to exertion. This may help not only improve performance, but decrease your risk for injury. And the truth be told, it can be challenging to navigate through all the information, tidbits, and advice that is thrown our way. Separating fact from fiction is key to finding the best workout regimen for our lifestyle and body type.

Myth: Spot training will give me that six-pack

Doing a thousand crunches a day is likely to burn calories and make your abdominal muscles sore and toned, but it does not necessarily translate to seeing those “rock-hard abs” we so desire. Although it seems logical, spot training, or exercising one part of our body, with the hopes it will look amazing may leave us disappointed. Unless the fat that lies above those muscles disappears, we will not be able to see those toned muscles. In order to get that

Frida Kahlo In the fall my daughter had been talking to me about a book she was reading for her Spanish class. Every day we talked about the story. My daughter encouraged me to “look up” this woman she was reading about as I would really love her. As I was preparing for this feature of the new year, I was not sure the timing would be such that I could write about the infamous female that my daughter had been reading about and who had appreciated her story. I acquiesced to her request and as I began my research, I was truly amazed by such an extraordinary individual. Born with the name of Magdalena Carmen Frieda Kahlo y Calderon (a/k/a Frida Kahlo) on July 6, 1907 to a German father and a Mexican Indian mother in Mexico City, Frida was one of seven daughters. At the tender age of six years old, she contracted polio, which left her with a deformed leg. After recovering from her illness, at the age of 14 Frida enrolled in a school in Mexico for medicine. Frida had aspirations of becoming a physician as a young child. Her education was short-lived after Frida was impaled by a metal rod during a bus accident. Frida suffered injuries to her spine, pelvis, collarbone and ribs. After the accident, Frida would spend two months in the hospital in a full body cast recovering from her injuries and then another month at home. Frida was known to have endured intense pain. As a means of coping with her pain, Frida’s father encouraged her to paint. Frida began painting self-portraits and had said, “I paint myself because I am often alone and I am the subject I know best.” Frida was known to be an extremely prolific painter and painted the true reality of her pain and her life. Frida was known to embrace her Mexican culture and painted very colorful pictures of herself and also included symbols of her heritage. In a few of her paintings she would incorporate a monkey into the painting, which for Frida was a symbol of protection. Frida’s artwork was discovered by a famous Mexican painter by the name of Diego Rivera. Diego Rivera admired Frida’s artwork and encouraged to her to continue to pursue a career in art. Frida and Diego began a love affair that would result in a marriage that was not supported by Frida’s parents due to the significant age difference between them. The marriage blossomed into an extremely tumultuous relationship. Both Frida and Diego engaged in relationships outside of the marriage and they even divorced once and then reunited. However, it is reported that they both reportedly deeply cared for one another. Frida

was known to have had a temperament similar to a man back in the 1900s. She was “loose” with her words, had no problem using profanity and also was known to smoke and drink alcohol, which was not considered very “lady-like” for this historical time period. Throughout Frida’s life she continued to struggle with her health and had to undergo a serious of operations as a result of her injuries. She endured amputations to her toes and leg. Frida lived in constant pain both emotionally and physically. This was a time where there were no medications to ease the pain. However, Frida continued to paint pictures that had meaning in her life of significant time periods. Frida’s artwork was discovered by Andre Benton. He had an interest in hosting an art exhibit to feature Frida’s artwork in France. Frida’s health was deteriorating and she was hospitalized; however, she would not allow this to deter her ability to attend her debut of her exhibit. Frida had her hospital bed transported to the gallery and set up so she could attend her exhibition! It is reported that the Louvre had purchased one of Frida’s paintings. This was the first piece of artwork purchased from a 20th-century Mexican artist, let alone a female. Frida Kahlo passed away on July 13, 1954 at home. Frida’s cause of death is unknown as there are reports that reflect her passing was as a result of depression and other reports reflect it was as a result of medical complications. As the feature comes to a close, I encourage everyone to reflect on Frida’s life and her ability to overcome many traumatic and difficult moments in her life. “I never painted dreams. I painted my own reality” was another quote by Frida. It is obvious that Frida’s artwork was a true expression of her reality and how she was able to process her life, albeit good or bad. As we begin the new year filled with new beginnings, I encourage everyone to remember Frida and allow her history to motivate you to keep moving forward in 2018! During her undergraduate studies in psychology, Rita developed a strong passion for women’s history after completing classes in political science. As an activist, Rita has raised awareness of the importance of women’s history through lectures, history lessons in elementary and middle schools and creating women’s history displays at local libraries. Rita also utilizes her knowledge of women’s history to empower women and young girls in her current role as a mental health therapist.

Ruth Newman Shapiro (RNS) Cancer & Heart Fund recognized by the Association of Fundraising Professionals (AFP) NJ Southern Chapter November 16 – National Philanthropy Day Ruth Newman Shapiro Cancer & Heart Fund members (not in order) Terri Marakos, Rose Marie Maglicco, Patricia McLaughlin, Joanne Kenny and Audrey Fischer, president. RNS was nominated by the AtlantiCare Foundation for its longstanding partnership with AtlantiCare Regional Medical Center (ARMC). The partnership began in 1968 with a generous gift for the establishment of a cobalt therapy unit of the then Atlantic City Medical Center. It was the first of its kind in the area offering revolutionary advancement in radiotherapy for the treatment of cancer in the post-World War II era. Throughout the years as cancer treatment technology advanced, the RNS Cancer & Heart Fund remained diligent in its mission to assist ARMC in being able to offer cuttingedge treatment and continued to provide funding to help change treatment options in our area. In 1993, with a Founder’s Gift from RNS, the RNS Regional Cancer Center at AtlantiCare Regional Medical Center was established. At the center, a team of specialists work with the patients from the diagnosis stage through treatment choices. The center has one of the most advanced radiation treatment pieces of equipment in the region, being one of the only facilities to provide AcQsim, the most sophisticated tumor localization technology available. Additional support came in the form of the purchase of two Mobil Mammography Vans and the establishment of The Rose Newman Live at Home Hospice program at ARMC. According to Samantha Kiley, Executive Director, AtlantiCare Foundation, their support – in excess of 5.5 million dollars over the years - has helped AtlantiCare provide the finest facilities and equipment in our area, specifically for the diagnosis and treatment of cancer and heart disease. “Most recently, RNS has committed a lead gift of $100,000 toward the AtlantiCare Foundation’s 40th Anniversary Campaign in support of the purchase of a new linear accelerator for the AtlantiCare Cancer Care Institute,” Kiley said. “It is because of these reasons and more, that we were excited to nominate the Ruth Newman Shapiro (RNS) Cancer & Heart Fund and celebrate them today – National Philanthropy Day.”
