Peripheral Neuropathy is a nerve disorder known to affect the lives of millions of people throughout the United States. It is a complication found in several different medical conditions including Diabetes, Peripheral Vascular Disease, Cancer and Chemo-Radiation therapies and Metabolic disorders most often. Overview Of Lateral Ankle Sprains by Alex Sliazis
Do you know someone who has twisted or rolled their ankle? Lateral ankle sprains are amongst the most common lower limb musculoskeletal injuries in today’s athletes and individuals involved in recreational sports and/or leisure physical activities. According to a 2019 consensus statement from the International Ankle Consortium, up to 70% of individuals have been involved in some sort of ankle injury throughout the course of their lives. The following questions may come to mind: What causes this high percentage? Am I at risk? What are my treatment options? Understanding the Anatomy
The ankle is made up of three connecting joints: The talocrural joint (main ankle joint) – consists of three bones: the tibia, fibula, and talus. The distal ends of the tibia and fibula (lower leg) articulate with the talus (in the foot). This “hinge joint” allows for dorsiflexion and plantarflexion movements (upward/downward). The subtalar joint – consists of two bones: talus and calcaneus (heel bone). Primary movements of this “gliding joint” are inversion and eversion (inward/outward turn). The inferior tibiofibular joint – “the ceiling” – consists of the distal end attachments of the tibia and fibula. Ligaments of the lateral ankle (outside aspect): Anterior talofibular ligament (ATFL) – most commonly injured Calcaneofibular ligament (CFL) – usually injured with conjunction of ATFL Posterior talofibular ligament (PTFL) – rarely injured, strongest of lateral ligaments
Common Causes / Risk Factors
The mechanism of injury resulting in a lateral ankle sprain occurs during an unstable moment of ankle plantarflexion with inversion (downward/inward turn). Some examples of activities that may cause this instability: coming down from a jump or descending from a higher surface, stepping on someone’s foot, coming in contact with crack, hole, or uneven surface when walking or running, and wearing improper footwear for activity-specific demands.
Research has found the following risk factors to correlate with lateral ankle sprains: poor conditioning, overconditioning, muscular fatigue, excessive weight and increased BMI levels, non-compliance with stretching and warm-up prior to physical activity.
Who’s at Greater Risk?
Gender is not a strong indicator for higher rates of lateral ankle sprains. However, age and activity levels are more prevalent. Adolescents between ages 10 and 19 years old who are actively involved in organized sports or recreational activities are at a higher risk to sustain such injury.
Signs and Symptoms
• Pain • Bruising/skin discoloration • Swelling of ankle (greatest on lateral aspect) • Difficulty walking • Inability to stand/put pressure on affected ankle • Tenderness with touch • Stiffness or loss of ankle mobility
How Can Physical Therapy Help?
Grades of Lateral Ankle Sprains / Recovery Time*
Grade 1 – mild, painful and damage to a ligament or ligaments without causing instability at the ankle joint. Recovery Time – 1-3 weeks Grade 2 – moderate, painful and damage to ligament causing a partial/incomplete tear and instability to the ankle joint. Recovery Time – 3-6 weeks Grade 3 – severe, painful or nonexistent pain, complete tear of ligament(s) with instability. Recovery Time – 6-12 weeks *Recovery time is variable.
Medical Imaging / Diagnostic Testing
To rule out ligamentous damage or bone injuries, the following tests may be ordered by your physician prior to or after conservative treatment measures: • X-rays: if suspecting a bone fracture • MRI: higher resolution imaging – suspecting bone, ligament, and/or soft tissue damage
Treatment Options
• Immediately following an ankle injury, the first goal needs to be focused towards reducing pain and inflammation. The RICE regimen should be established
within the first 48-72 hours (RICE = Rest, Ice, Compression, and Elevation). Mild grade 1 sprains can heal on their own by using this regimen with inactivity. • Medication – taking over-the-counter pain medication, with your doctor’s approval. Some examples include nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen (Advil, Motrin) and naproxen (Aleve). • If weight-bearing through the involved ankle is too painful, your doctor may give you crutches to allow for a partialweight-bearing status (PWB). • As you progress from PWB to normal walking, you may benefit from wearing a supported ankle brace inside your sneaker. A sample of companies that manufacture braces include AIRCAST, DONJOY, and PROCARE. • Don’t be surprised if you are given a prescription for physical therapy services! Here is a list of the main chronological goals your physical therapist will follow for you to have a successful and healthy recovery: • Reduce pain and inflammation • Regain full ankle range of motion • Restore strength • Facilitate re-education of balance and control • Re-establish speed and agility • Progress to dynamic sport-specific movements • Return to sport and/or recreational hobby
What Happens if I Avoid Treatment?
A previous injury is the greatest risk factor for future injury. Chronic Ankle Instability (CAI) – the occurrence of repetitive episodes of lateral ankle instability resulting in frequent lateral ankle sprains. CAI is common for individuals who do not take the appropriate measures to address their ligament laxity and do not modify their performance.

Alex graduated in 2014 with honors from Lebanon Valley College in Pennsylvania with a Bachelor’s Degree of Science in Health Sciences. He continued his graduate education at LVC achieving his Doctorate of Physical Therapy in 2017. Throughout his graduate studies, Alex completed his first and last clinical affiliations in sub-acute rehabilitation and outpatient physical therapy respectively in his hometown area of Ocean/Monmouth County. In addition, he practiced acute care physical therapy in a hospital’s Intensive Care Unit in the Greater Philadelphia Area. Alex’s passion for outpatient physical therapy services occurred after completing an orthopedic clinical rotation with Clemson Sports Medicine and Rehabilitation in Charleston, South Carolina. During the final three years of his college’s doctoral program, Alex was an active volunteer in LVC’s C.U.R.E. Student Run Free Clinic which provided pro-bono physical therapy services to uninsured individuals of the Lebanon County community. Egg Freezing / Fertility Preservation Is it Right for Me? By Dr. Allen Morgan Morgan Fertility and Reproductive Medicine Egg freezing allows a woman to take control of her ticking biological clock by banking her eggs for future use. This elective procedure has gained popularity lately due to many large corporations such as Apple and Yahoo now offering egg freezing as an option to their employees who want to focus on career before starting a family.
Adding to the buzz, many celebrities are now openly talking about their choice to freeze their eggs until finding the perfect partner, then undergoing in vitro fertilization (IVF) when the time is right.

Age, school, lifestyle concerns, or medical concerns such as family history or a cancer diagnosis may also play a role in the decision to freeze eggs for later fertilization. Whatever the motivation a woman may have to freeze her eggs for later use, there are common questions associated with the process that should be taken into consideration prior to making such a big decision.
What is the optimal age to freeze eggs?
Fertility specialists recommend freezing earlier for better quality oocytes (eggs) as well as embryos. Egg freezing is recommended in your 20s and 30s, but it can be done in your 40s. However, in addition to age, we also take into account hormone levels and antral follicle (resting follicle) count.
Indeed, if we look at IVF data, there is an inflection point in pregnancy rates after the age of 35. Between the ages of 38 to 39, there is an even larger inflection point. So, the best advice a woman’s health professional can give to someone who falls into one of these age groups is to start the process with a specialist as soon as you can. It doesn’t hurt to investigate and there is no obligation to proceed, since this may be an elective procedure.
Will my health insurance cover egg freezing?
Typically, most health insurance companies do not cover elective fertility procedures. However, you should check with your provider directly to confirm your specific coverage details. Often some blood tests, ultrasounds or other pre-retrieval procedures may be covered. If you are about to undergo treatment for cancer, your insurance may offer coverage for fertility preservation. There has been much movement in legislation over the past few years advocating for individuals with medical issues such as cancer. The cost is approximately $8500.
How do I choose a doctor to discuss my options?
While there are many fertility specialists in our area to choose from, you will want to find a specialist who not only fits your personality style, but who is known as an expert in egg freezing. Also, you will want to decide whether a fertility “clinic” or a solo practitioner better fits your personality style. Generally speaking, smaller practices offer a more intimate, personalized bedside manner, and larger “clinic” style practices provide a bit less personal experience, while both may have high success rates. Last, it is important to select a doctor who utilizes minimal stimulation (fewer medications) while achieving high success in vitro fertilization (IVF) rates, a sure sign of an expert staff and close monitoring of patients.

About Gastro Esophageal Reflux Disease Written by: Kenneth Glazier, MD Gastroenterologists of Ocean County Most of us at one time or another has suffered from heartburn. We have all seen the ads for the numerous acid suppressing medicines. The question is when to take notice of these symptoms and to seek the advice of a doctor. What is GERD? Gastro Esophageal Reflux Disease (GERD) is a condition related to stomach acid backing up into the esophagus. The esophagus is the food tube that connects the mouth to the stomach. At the bottom of the esophagus is a muscle called the lower esophageal sphincter (LES). The job of the LES is to squeeze the bottom of the esophagus tight to prevent the stomach acid from splashing up and burning the esophagus. It is supposed to stay closed at all times except when we swallow, belch or vomit. The LES can inappropriately relax causing acid to enter the esophagus. When stomach acid enters the esophagus it can damage the lining of the esophagus and cause symptoms such as a burning sensation in the chest called heartburn. Reflux may be so severe that a person may believe they are having a heart attack. Atypical symptoms can also occur like excess belching, hoarseness, sore throat, asthma, or chronic cough.
What is Endoscopy? Upper endoscopy is a painless procedure that is performed in our outpatient endoscopy center. The test may only take five minutes to perform. The test is performed by a gastro-enterologist while you sleep. An anesthesiologist is also involved to provide sedation through an intravenous needle. The doctor will take a pencil thin tube with a light and a camera on the tip and slide it into your mouth and follow the pathway the food takes down the esophagus, stomach and small intestine. These areas can be carefully examined, photographed and biopsied. The risks of complications from the test are very small. There are no alterna-tive tests that can allow the doctor to take important biopsies of the esophagus which are necessary to diagnose Barrett’s esophagus and cancer of the esophagus. As you can see heartburn may not just be a symptom to grin and bear, but may represent a serious health problem. It is therefore very important to talk to your doctor if your symptoms are long standing, require chronic medication therapy or are associated with alarm features.
What can I do to prevent GERD? There are a number of lifestyle and dietary changes that can be tried for most people with GERD. Avoiding foods and beverages that can inappropriately relax the LES is recommended, such as fatty foods, spices, tomato products, coffee, alcohol and sodas. Stopping cigarette smoking and losing weight if overweight can improve symptoms. Behavior modifications such as elevating the head of your bed on blocks or using a specifically designed wedge will help keep the stomach contents and acid in the stomach. Decreasing the size of portions of dinner and eating dinner 2 to 3 hours before bedtime can also avoid night time GERD. What medicines are used to treat GERD? Medicines used to treat GERD include antacids which neutralize acid already produced by the stomach. Other medicines like H2-blockers and proton pump inhibitors suppress the production of acid by the stomach. These medicines are used to prevent symptoms of heartburn and aid in healing of acid induced injury. These medicines are available both over the counter and by prescription. Prescription medications are usually stronger and may give better relief of symptoms. When should I worry about GERD? There are certain alarm symptoms that may indicate a more serious condition than just typical GERD. If someone with heartburn has problems swallowing, weight loss, nausea, anemia, or blood in the stool then they should tell their doctor. Patients who have had heartburn more than five years or have heartburn starting after the age of 45 should also be concerned and notify their doctor. Alarm symptoms may indicate bleeding ulcers, scars in the esophagus called stric-tures or even cancer of the esophagus. If medications are required for more than an occasional use or for prolonged periods of time it is important to discuss with your doctor. Patients with heartburn for a long time dramatically increase their risk for developing Barrett’s esopha-gus and cancer of the esophagus.

What is Barrett’s Esophagus? Barrett’s Esophagus is a permanent change in the type of cells lining the esophagus in a reaction to chronic acid injury. This condition is associated with an increased risk for the development of cancer of the esophagus. Esophageal cancer is increasing in incidence more rapidly than any other type of cancer. Barrett’s esophagus is most commonly diagnosed in Caucasian males in their fifties to sixties. Barrett’s Esophagus and cancer of the esophagus are best diagnosed with upper endoscopy.

Think Hard Before Tapping Your 401(k) Balance One of the features that makes 401(k) plans so attractive is that your money is not completely out of reach should an emergency need arise. Most plans allow for loans that are completely tax free if repaid as agreed. (Interest payments will be required, but they will be credited to the account.) In a major emergency, a hardship withdrawal may be permitted, subject to income tax and, usually, a 10% penalty as well. Borrow?
At first glance 401(k) loans may look particularly appealing. After all, you make those payments of principal and interest to yourself. However, if the interest that you pay is less than your borrowed dollars would have earned in the plan, you will slow the growth of your retirement nest egg. Moreover, you pay with after-tax dollars—replacing your original taxdeferred contributions. Loans must be repaid in no more than five years. (Fifteen-year terms are allowed for loans to purchase a home.) If you leave your job before a loan is repaid, you’ll have to pay it off, or the open balance will be considered a premature withdrawal subject to income tax and penalty. Potentially more serious yet, the burden of loan payments may make it impossible to continue your 401(k) contributions.
Withdraw?
It’s not easy to make a hardship withdrawal from your 401(k) account. You must show an “immediate and heavy financial need” for: medical expenses not covered by insurance; the purchase of a principal residence; postsecondary tuition; or to avoid eviction from or foreclosure on a principal residence. Many plans also include funeral and child support expenses. You also must show that you have no other resources reasonably available to meet these costs. This means that you first must fail to qualify for a plan loan. Once you take a hardship withdrawal, you will be barred from contributing to your plan for at least 12 months.
Pay now or pay later
To examine the effect of these options, let us compare the long-term results for Nancy Needful, a hypothetical 35-year-old worker with a $30,000 balance in her 401(k) plan. Nancy contributes $150 monthly to her account. Faced with a sudden emergency need for $10,000, Nancy has three options. Nancy can: A. Take a loan of $10,000 from her plan at an 8% interest rate and cease making contributions until the loan is repaid in five years, making monthly payments of $202.76, and resuming her $150 contribution after five years. B. Make a hardship withdrawal of $12,500 to provide the cash that she needs and cover her income tax and penalty, resuming her participation in the plan after one year. C. Obtain a $10,000 advance on an inheritance, continuing her participation in the plan.

As we see, by taking the advance on her inheritance and continuing plan contributions, earning an 8% return on her investments, Nancy ends up at age 65 with 17.4% more than if she had taken the loan and fully 38.8% more than with the withdrawal. DHPh Genetic testing is a type of medical test that identifies changes in chromosomes, genes, or proteins. The results of a genetic test can confirm or rule out a suspected genetic condition or help determine a person’s chance of developing or passing on a genetic disorder. • DNA testing tells us how the body works. • DNA is the carrier of genetic information in the body. • DNA SNPs (single-nucleotide polymorphisms) are a genetic variance at a single position in the DNA of a person. SNPs can be used to evaluate an individual’s genetic predisposition to developing a disease. • DNA testing tells us what is working and what is compromised.
We can work with our genes to obtain optimal health! • One of the most important pieces of information we can use to improve our health is the SNP. • Most SNPs don’t seem to affect us much but can make a huge difference. • SNPs in the MTHFR gene can create a variety of problems, including obsessiveness to irritability to birth defects to cancer. SNPs can produce these effects but they don’t have to.

What Are The Estate Documents To Update When Going Through A Divorce? By Christine L. Matus, Esq.
The story of a divorcing couple in Arizona demonstrates why you need to update your estate plan at divorce. The couple, who were in their early 40s and had been married for several years, had created a multimillion-dollar business together. When they filed for divorce, the process became bitter and full of disagreements about small issues. They were thinking in the moment about the immediate dollars and cents, but didn’t consider what would happen with assets at death. During divorce, update your will to ensure your assets go where you want them to go. One night the wife had dinner with friends. Sadly, she died in a tragic car accident on her way home. Neither she nor her husband had changed their wills. As a result, everything she had, including the half of the business that belonged to her, went to the husband. Her family was left with nothing. In this case, an interim will could have stated that her share of the marital estate would go to her parents. The lesson: during divorce, update your will to ensure your assets go where you want them to go, such as to your parents or to a child from the current marriage or a previous one. Over time, the mere sight or smell of some foods can make them irresistible! Other types of food leave a sensory memory imprint with the dopamine surge. This explains why many of us tend to be “emotional eaters” and crave “comfort foods.” Another hormone, SEROTONIN, tends to decrease during specific times of the month. Both PMS (pre-menstrual syndrome) and PMDD (pre-menstrual dysphoric disorder) are characteristic of a significant drop of SEROTONIN. Both respond favorably to…CARBOHYDRATES! This clearly magnifies food cravings! Since junk food and processed foods tend to be packed with fat, sugar and salt, they increase the release of DOPAMINE significantly. CW: When should children begin using toothpaste and how much should we use? Dr. Max: The sooner the better! Starting at birth, clean your child’s gums with a soft infant toothbrush or cloth and water. Parents should use a tiny smear of fluoride toothpaste to brush baby teeth twice daily as soon as they erupt and a soft, age-appropriate sized toothbrush. Once children are three to six years old, then the amount should be increased to a pea-size dollop, and parents should perform or assist your child’s tooth brushing. Remember that young children do not have the ability to brush their teeth effectively. Children should spit out and not swallow excess toothpaste after brushing. CW: How can parents make tooth brushing more fun for toddlers? Dr. Max: Character toothbrushes, lots of praise and reward stickers will definitely help, but most important is doing the same routine daily so they get used to it from an early age. CW: Are you seeing an increase in cavities in young children these days? Dr. Max: Yes, due to a diversified diet frequently filled with junk food and sweetened drinks. CW: What kind of unique health programs do you have for children? Dr. Max: We do school programs given by a registered Dental Hygienist throughout three counties. In addition, this year we began mobile dentistry where team of hygienists and dentists visit local schools to provide comprehensive preventative treatments such as examination, cleaning, x-rays, sealants and fluoride application.
Dr. Liya emigrated from Russia in 1991 at the age of ten. She was a Summa Cum Laude graduate in 2003 from Hunter College in New York. That same year she relocated to Philadelphia to attend the University of Pennsylvania School of Dental Medicine, where she earned her Doctor of Dental Medicine degree, Cum Laude. While at UPSDM she was keenly involved in programs and visits providing underprivileged children and adults with much-needed dental care and education. Advanced studies in her field of Pediatric Dentistry took place at Lutheran Medical Center in Brooklyn, New York, where she served as Chief Resident. Dr. Liya is Board Certified by the American Board of Pediatric Dentistry She lives in Marlboro with her husband, Vlady, who is a pediatric endocrinologist, and their two children, Julian (10 yrs.) and Kayla (7 yrs.), plus an 11-year-old Pekingese named Archie. In addition, Dr. Ostrow just welcomed a beautiful baby boy named Aaron. CW: What is the most rewarding part about your profession? Dr. Liya: I enjoy spending most of my day with children. Being a part of the non-scary world of dentistry and hearing at the end of the appointment, “This was fun.” CW: What is the best part about working with Dr. Max? Dr. Liya: His knowledge, compassion, humor and approach to children are something that I admire greatly. I have learned a great deal from him. CW: What are the best preventative measures parents can take when it comes to dental health for their children? Dr. Liya: Children should see a dentist regularly every six months. This allows dental professionals to constantly educate parents and children on oral hygiene, as well as give nutritional advice. Some examples are to encourage your children to eat regular, nutritious meals and avoid frequent between-meal snacking. Other preventative measures include fluoride and sealants which protect teeth from decay.
By participating in NCDHM, parents and kids learn about good oral health habits, such as how to properly brush and floss. Parents learn how to check their child’s mouth for early signs of gum disease and what proper nutrition is required to minimize periodontal disease. With proper guidance and following some simple tips, parents can help their children retain their beautiful smile for years to come.

Young adults are delaying marriage for many reasons, including a challenging job market, falling wages, and growing debt. Others are forsaking it altogether.
Handling personal finances—separately or as a couple As unmarried partners, you need to decide whether to handle your finances separately, together, or in some combination. If you decide to combine finances, you should proceed with caution. You should realize that your relationship lacks many of the legal safeguards of marriage. If the relationship ends, no divorce courts or uniform guidelines exist to separate your shared assets. If you incur joint debt, you are each fully responsible for the entire amount (as is true of a married couple). Also, it’s possible that your partner could drain the funds from a joint account. Have a frank discussion of goals Have a frank discussion of your financial values, priorities, and goals before deciding whether to handle your finances separately or as a couple. Estimate your net worth separately, and also as a couple, if you plan on pooling your financial resources. Decide whether you’ll handle household finances through joint or separate bank accounts. Prepare a budget, identifying which expenses you’ll pay separately and which expenses you’ll share. Weigh the pros and cons before obtaining joint credit cards. Consider protecting yourself with a domestic partner agreement A domestic partner agreement is a written contract between you and your partner that primarily addresses the sharing of income, expenses, and property. It supports your ownership rights and clarifies your intentions for the distribution of your property if you die or your relationship ends. Few, if any, laws specifically govern the rights and responsibilities of unmarried partners regarding the sharing of property and finances. If your relationship ends, there are no uniform guidelines for sorting commingled finances and dividing shared property. By setting clear ground rules up front, a domestic partner agreement can help your relationship run more smoothly and ease the handling of disputes in case of separation or death. Estimating your net worth—separately and together Your net worth is simply your assets minus your liabilities. Put another way, it is all that you own, less all that you owe. A net worth statement measures your financial position. You can use it as the basis for determining how much you’ll each contribute to your common expenses, such as household expenses. With periodic updating, your net worth statement presents a picture of your progress in meeting your long-

term financial goals. It provides information you can use to formulate a sound investment strategy and to plan for your insurance, retirement, and estate needs. You should consider calculating your net worth separately and also jointly if you’re planning to pool financial resources. In calculating your net worth separately, account for only your individual portion of jointly owned assets and shared liabilities. If you prepare a joint net worth statement, put both your names on it and don’t include any separately owned assets.
Developing a budget The thought of developing a budget intimidates many people. Don’t let that happen to you. A mutually agreedupon budget helps to keep finances straight and avoid arguments over money and bills. Begin by agreeing on which expenses you’ll share and which you’ll keep separate. For shared expenses, decide whether you’ll split them equally or in proportion to your income or net worth. Include your share of household expenses as a fixed cost in your budget so you don’t forget to set aside funds to cover it. Credit cards—separate or joint You can open a joint credit card account, or you can add your partner to your existing credit card as an authorized user. • If you apply for a joint credit card, you each fill out a separate credit application and each must pass a separate credit review. If you receive joint cards, you are each fully responsible for all charges on the account—your partner’s as well as your own. You must both faithfully record your charges. An unexpected bill due to a forgotten charge or a lost receipt can derail your budget. If these surprises result in late payments or leave you unable to pay the bill, they can damage your credit rating. Tip: If your relationship ends, request each creditor to close each joint credit card account. Don’t split the cards with your partner, taking some while he or she takes others. You are each still fully responsible for all charges on any joint account. Tip: You can add your partner to your existing credit card as an “authorized user” simply by requesting this of your credit card company. In this case, your credit application is based solely on your own credit rating, and you are solely responsible for all payments on the account. Tip: Even if you establish joint credit card accounts, you should consider the importance of establishing or maintaining a separate credit history.

Will you share some costs, paying others individually? Or will you pool income and expenses? There are probably as many arrangements as there are couples. Joint expenses generally include housing-related costs, such as mortgage or rent, utilities, household insurance, property taxes, and food. Personal expenses typically include clothing, car expenses, personal insurance, personal debt, and individual hobbies or interests. You can categorize expenses in whatever way works best for you. You can begin the process by following these steps: • Forecast your annual expenses. Once you’ve identified your joint expenses, forecast them for a year, being sure to include those that come due monthly, quarterly, semiannually, and annually. Divide the yearly total by 12 to get an average monthly cost. • Decide how you’ll split these costs, equally or apportioned in some way. If you decide to apportion your expenses, net worth is a better basis for apportioning them than income. Every month, you each deposit a check from your personal account into the joint account to cover your share. Caution: While joint accounts make sharing expenses easy, there are some big drawbacks to consider. Once you open a joint bank account, you’re each responsible for all checks drawn on that account, plus all bank charges. Record-keeping can become a nightmare, especially if you’re both simultaneously writing checks and withdrawing cash from automatic teller machines. Creditors can seize the funds in a joint bank account to satisfy one partner’s obligation. If your relationship ends, verifying your contribution to the shared expenses may be difficult. One of you can drain the funds in a joint account, while the other may find it difficult to get a restraining order preventing this. Or one of you could simply clean out the account and walk away with the funds. Tip: You can protect yourselves by setting up a joint account that requires both signatures for withdrawals. Also, if you each contribute an average monthly amount, set up an interest-bearing account, since it may be carrying funds from month to month.
Paying expenses from separate bank accounts If you prefer a simple financial arrangement and want to avoid the liability of a joint account, you can keep your finances separate. This is the cleanest approach. One of you can pay the bills and collect from the other. Or you can both split the task, with each of you paying certain bills and squaring accounts monthly. Tip: Always pay your share by check, or get a receipt from your partner. This helps to straighten out any bill-paying errors and also documents your contribution in case of a breakup.
Pooling income and expenses You can also opt to pool your incomes and pay expenses from a joint account. This may work best when you’re in a long-term relationship, when your incomes are comparable, or when you’ve made equivalent tradeoffs, such as one working while the

Before answering this question, you should consider that as an unmarried partner, you’re ineligible to receive spousal benefits from two key sources that many married partners depend on: Social Security and defined benefit pension plans. • Social Security—Social Security does not pay benefits to an unmarried partner. While you may qualify for Social Security benefits based on your own earnings, or that of a deceased former spouse or ex-spouse, you can’t receive benefits based on your unmarried partner’s record. • Defined benefit pension plans—Defined benefit pension plans typically don’t offer benefits to a non-spousal beneficiary. A spouse, in contrast, is legally entitled to receive benefits unless he or she waives that right. A surviving spouse can expect to receive 50 percent of a deceased partner’s pension benefits. If the partner dies before benefits begin, the surviving spouse will receive 50 percent of the accrued benefit.
Consider creating your own safety net separately You should clarify what retirement benefits you can expect to receive from each other. Unless your partner has set up a funded irrevocable trust for you that will withstand a permanent separation, it may be best to plan as if you’re single. Also, you should consider the possibility that your relationship could end before retirement. Will you plan for retirement separately or as a couple? What approaches will you take if you’re ineligible to receive benefits from one another’s retirement plans? Planning for retirement together If you choose to plan for retirement together, here are some approaches you can take: • Use life insurance to fund your partner’s retirement—Life insurance offers a vehicle to provide significant replacement income for your partner. You can cross-own life insurance policies, or you can purchase an individual policy naming your partner as the beneficiary. 1. Cross-owned policies—With cross-owned policies, you each buy a policy on the other’s life. When one of you dies, the surviving partner collects the policy’s death benefit and invests the proceeds to produce the needed income. You may need to demonstrate that you have an insurable interest in each other to cross-own life insurance. Married couples are assumed to have an insurable interest. Unmarried couples who own a house or business together are also considered to have an insurable interest, although only up to the value of their shares of the mortgage or business. You can prove insurable interest by providing evidence of jointly owned assets and, possibly, copies of wills or trust documents. 2. Individual policies—You can also own an individual policy, naming your partner as the beneficiary. • Designate your partner as the beneficiary of your qualified retirement plan—You can name each other as beneficiaries of qualified retirement plans, such as 401(k)s, 403(b)s, and individual retirement accounts (IRAs). Caution: However, upon your death the options available to your partner will generally not be as favorable as the options that would be available to a spouse beneficiary.

These stops in breathing usually last for around ten seconds and are often followed by snorts, gasps, or choking sounds as a person’s body fights to resume breathing again. When you stop breathing during the night, your brain responds by partially waking to send signals to the respiratory system to work harder to get past the obstruction. This repeated stop in breathing is very detrimental to overall health and is linked to a number of medical conditions. These range from depression and sexual dysfunction to increased risk of heart attacks, strokes and premature cardiac death.
Dental Appliances for Sleep Apnea
Before any treatment options can be determined, a sleep study must first be performed to determine the severity of one’s symptoms as it can have a direct influence on the recommended therapy. However, a diagnosis of obstructive sleep apnea (OSA) should be treated and monitored by a trained clinician. There are various types of comfortable dental appliances which can be made by an experienced doctor such as Dr. Ally to treat OSA and improve patients’ health. For moderate to severe sleep apnea patients, most sleep professionals will recommend CPAP therapy as a first-line treatment option. For mild to moderate sleep apnea, a dental device is often the recommended therapy. Dental devices may also be recommended to be worn in conjunction with a CPAP device.

Dr. Christine Ally, originally from Toronto has found herself fulfilling her passion as a dentist on the Jersey Shore which she now calls home. As the Owner of Advanced Sleep and TMJ Center of New Jersey and Sheore Premier Dental Arts she is a highly skilled clinician with extensive experience in the treatment of patients with Obstructive Sleep Apnea using oral appliance therapy. She is a Fellow of the Academy of General Dentistry, Fellow of the International Congress of Oral Implantologists and a Qualified Dentist of the American Academy of Dental Sleep Medicine. Dr. Ally obtained her undergraduate degree from the University of Toronto and her dental surgery degree from The University of Western Ontario. Graduating among the top in her class, she completed internships at New York Presbyterian Hospital and Massachusetts General Hospital followed a residency at Yale. She has spent a significant part of her career offering dental care to medically compromised patients. Dr Ally has had years of advanced training in the treatment of patients with OSA and who are unable to use a CPAP and has helped countless patients overcome this condition with dental appliances. In her spare time, she is an avid photographer. She also treasures her time with her family, traveling and trying new cuisines.
Dana Hurder-Hall is the office manager at Advanced Sleep and TMJ Center of NJ and Shore Premier Dental Arts. Dana was born and raised in Spring Lake, NJ. She attended Manasquan High School where she was able to simultaneously study Dental Assisting with Radiology during her senior year. Dana then went on to earn her Radiology, Certified and Registered Dental Assistant licenses and was employed as an Expanded Functions Chairside Assistant for more than 25 years. After returning to college and earning her Associates Degree in Applied Science, she returned to the dental field as an office manager and Marketing Liaison in both private practice and corporate environments. Dana is also an accomplished Dental Assisting with Radiology Instructor, formerly with Advantage Career Institute, Eatontown, NJ. She currently resides in Howell with her 3 children, Jake, Shannon and Dawson and their yellow lab Camo. She is best recognized for her stellar patient and staff relations and warm, welcoming demeanor. Brigid King is the newest member to join the team at Shore Premier Dental Arts and Advanced Sleep and TMJ as the Practice Administrator. Brigid has many years of experience organizing and managing all aspects of a dental practice as well as her experience working with our Sleep Apnea patients. During her career she has worked with multioffice practices as well as working on the corporate side of dentistry, in New York as well as New Jersey. An examination of the anal canal via an anoscope will also be performed in order to ensure there is not any intra-anal pathology causing the symptoms such as hemorrhoids, warts, a polyp or tumor. Considering the serious diseases that may cause the symptoms of pruritus ani, it is very important not to selfdiagnose, but rather to seek consultation with a physician. In certain situations, a small skin biopsy may need to be performed.
As we enter the New Year, a common ailment affects many individuals. This is the time of year when How is pruritus ani treated? people make New Year’s resolutions The first goal of treatment of pruritus ani is to make a correct diagnosis. That means to eat “healthy” and diet in order to determining if the condition is primary pruritus ani or secondary pruritus ani caused lose weight for spring break or in preparation for by a medical condition/disease. Once this has been accomplished and it is confirmed that the patient has primary pruritus ani, the goal of therapy is to restore clean, dry and the summer. Pruritus ani is a Latin term literally meaning “itchy anus.” It is an uncomfortable sensation around the anus which produces the desire to scratch. Scratching causes irritation of the perianal skin, which in turn causes the desire to scratch more. The skin becomes more irritated and damaged, which causes the desire to scratch even more. This becomes a vicious cycle. Pruritus ani affects 1% to 5% of the population. Although it affects both men and women, it is four times as common in men. This condition is most often encountered in patients in their 40’s to 60’s. Pruritus ani is treated medically. An accurate diagnosis is mandatory for proper care.
What causes pruritus ani?
The symptom of pruritus or itching is common to many anorectal conditions. Disorders such as hemorrhoids, skin tags, fecal soilage and/or incontinence, anal fistulae, anal fissures and anal warts all may be associated with itching. This is termed secondary pruritus ani. However, pruritus ani may be independent of these conditions. This is called primary pruritus ani. The cause of the syndrome in these cases is usually from topical agents applied to the perianal skin and/or the pH of the stool. The pH of the stool is determined by foods which we eat. In addition to inadequate hygiene, overzealous or aggressive hygiene using irritating soaps, fragrances and lotions may cause pruritus ani. Overzealous cleaning and the use of topical steroids can destroy natural skin barriers and cause trauma to the anal skin, making the problem worse. As such, attempting to keep the perianal skin too clean, is harmful and may worsen the problem. The use of baby/toilet wipes is a common culprit for many patients suffering from pruritus. It is very important to note that there are many medical conditions that may cause perianal itching and are treated differently from basic, primary pruritus ani. There are infectious conditions such as bacterial skin infections, fungal infections, parasitic infections (pinworms or scabies) and viral infections (anal warts). Skin conditions such as psoriasis, seborrheic dermatitis, contact dermatitis, and lichen sclerosus may cause secondary perianal itching. Local cancers such as Bowen’s disease or Paget’s disease are also potential causes of pruritus ani. How is pruritus ani diagnosed? In order to accurately diagnose pruritus ani, your physician will obtain a complete history and physical examination. The timing and duration of the symptoms are important factors. Toileting rituals/behaviors, stool frequency and consistency, and hygiene/cleansing routines are noted in order to help make a diagnosis. Travel history, medical history and medications taken are also important. It is crucial to include any supplements taken, topical ointments/creams used and whether or not baby/toilet wipes are being used. A careful diet history is also taken in order to identify any possible contributing agents. Your doctor will perform a physical examination. This will include a careful inspection of the perianal skin and a digital rectal examination. Common findings are irritated, swollen, reddened skin that may have linear ulcerations/cracks. Sometimes
breakdown and irritation. Believe it or not, plain water is the best way to keep the perianal skin clean and healthy. Finally, the last step in treatment is to modify the diet to avoid a change in pH. Foods related to pruritus ani include caffeine, colas, citrus, dairy, tomatoes, nuts and beer. As mentioned earlier, these foods are very common during this time of year while watching football or trying to lose weight.

Dr. Paonessa is very familiar with diagnosing and treating pruritus ani as well as all the other diseases/conditions that may be mistaken for pruritus ani. Since there are many serious diseases that may be mistaken for primary pruritus ani, it is very important to see a physician trained in treating this condition. It is incredible how a fairly simple ailment such as pruritus ani can alter a patient’s daily life and affect his/her quality of life. Don’t hesitate to pick up the phone and call for a consultation. Committing to ‘Memory’ – Designer Turns Her Baking Hobby Into a Full-Blown Career
AnnMarie Cooney AnnMarie Cooney did not grow up dreaming of becoming a baker. Visions of sugar plums and intricately decorated cakes were not dancing through her head as a young girl. By trade, Cooney was a kitchen designer, but retired early to raise her daughters. “With three girls, my job was to raise good women,” Cooney says. It was a special request from her daughter that would eventually turn her into a highly sought-after cake designer and baker. “It all started because my daughter wanted a cake,” Cooney says with a smile. Cooney’s middle girl, was born with health issues that resulted in special needs. So when she wanted a circus cake for her fifth birthday, Cooney made her the best circus cake around. That was 14 years ago, and her girls quickly caught on to their mom’s special talent. At every birthday, they requested a special cake. “It became, ‘what can we do next’ and ‘how can we top that,’ Cooney says. Then she began making cakes for friends. Cooney’s husband had to build an additional kitchen at her house just to accommodate all of the special cake requests. When Cooney’s youngest daughter entered kindergarten six years ago, Cooney decided she wanted to go back to work. “But, I needed to figure out what I was going to do,” Cooney says. Sadly, Cooney’s grandmother, with whom she was very close, passed away during this time. “I was sitting at home, just very sad, and my cousin called me to meet her at the park with her kids,” Cooney says. However, in the midst of grief, she didn’t feel up to it. “My cousin sad, ‘come on grandma would want us to.’ So I went, Cooney says. On the way to the park, she saw the real estate sign. She checked it out and immediately knew that it would be the perfect location for her bakery, A Sweet Memory Cake Shoppe & Tea Room. “My grandmother left each of her grandchildren a small inheritance and it was just enough for me to open my business,” Cooney explains. Cooney spent about five years at that location before moving to another larger space six months ago just two miles down the road in Waretown. A beautiful picture of her grandmother has watched over her since the day she opened. It’s difficult to simply call Cooney’s creations “cakes.” Some designs are so intricate that they look like art. Cooney and her team created a three-dimensional depiction of Van Gogh’s “Starry Night” that captured the essence of this masterpiece with sweet, swirly curves and lines of vividly hued icing. Then there’s the adorable panda birthday cake that could easily be mistaken for a cuddly toy. The list of unique, edible masterpieces goes on and on, That’s because they average a minimum of 30 specialty cake orders per week, mostly by working with different catering and special event venues in and around Long Beach Island. However, they also have a long list of loyal customers. “Some of our customers started with a gender reveal and now we’re doing their fifth birthday cake,” Cooney says. Customer Kathy Helm of Waretown got her grandson’s birthday cake from Cooney close to five years ago. “It was perfect, I brought a couple pictures of what I wanted and they just did their thing,” Helm says. And she has been coming here ever since. “I’m in sales, so I bring my clients treats from here all the time,” Helm says. She’s also brought her granddaughter to Sweet Memory’s princess tea parties. “Our clients really do become our friends,” adds Cooney. In addition to the special-order cakes, A Sweet Memory, has a selection
of seasonal cakes, mini desserts, cookie sandwiches and other baked goods readily available daily. Other creative creations include their cake pies; cakes topped with pie filling and decorated to look exactly like a pie. “Our baking is very seasonal,” Cooney explains. Winter flavors includes a tasted snowball cupcake, which is a chocolate cake filled with marshmallow and chocolate fudge, frosted with coconut buttercream, rolled in toasted coconut, drizzled with chocolate and topped with marshmallows. Or their gingerbread scone with maple cream. Or the orange, cranberry Christmas scone with orange cream. “My grandmother used to always have her ‘lady friends’, as she called them, over for tea,” Cooney explains, adding that she wanted to honor her grandmother by adding a high tea to her bakery. “We squeezed one table in our previous location and began hosting afternoon teas.” They were all so well-received Cooney has expanded her operation to include a full-time tea room. Decorated with a lovely mix of china and flowers, the tea room serves tea, lunch and snacks from 11 a.m. to 4 p.m. Wednesdays through Sundays. They also offer a high tea, but reservations are required. “Our clients have really created the charm of this place,” says Cooney, who adds almost of her china, platters and place setting have been donated by her customers. High tea ($27.95) is served on tiered platters and begins with an individual pot of tea, chosen from their selection of more than 50 teas. Followed by seasonally inspired salad and homemade soup, four varieties or tea sandwiches, freshly baked scones served with their specialty creams, and completed with a selection of their miniature desserts. “Everything from our meats to spreads, our dressings and chutneys, our soups are all made fresh in-house,” Cooney says. Cooney also places an emphasis on healthy fare. “We source organic whenever possible and offer a large selection or vegetarian friendly dishes,” she says. With the extra space the tea room has added to A Sweet Memory Cake Shoppe and Tea Room, Cooney is able to host special events. “We’ve done cake-decorating classes and other ladies events,” Cooney says. They’ve also hosted princess teas, American Girl teas, unicorn teas and more. They even carry a line of tea geared towards little ones. “We even have a dress up section that ladies and little ladies love,” Cooney says. A little help from above has turned Cooney’s one-time hobby into a business that brings customers joys bite by bite. The business that Cooney started out of her home has grown into a full-time operation with a staff of eight. “We work hard and we work a lot, but we really do love each other,” Cooney says. She says they have become a great team and she couldn’t do all of this without them. Cooney’s brother-in-law, Paul Cooney, recently joined the team as chef and baker. There’s also Sarah Rod, the head decorator, and Cindi Reider, head fondant who works in fine detail. “Sarah and
Cindi are so talented, they really are the best around, they’re artists,” Cooney says. Together, what they create can easily be considered art, but at the end of the day-or party or wedding or afternoon tea- and after the last bite, their masterpiece has been turned into a sweet memory.

Anchor Monuments donated teddy bears to The Children’s Art Therapy “Buddy” Program at Riverview Medical Center in Red Bank, NJ in April 2019. Anna Farro, owner of Anchor Monuments, realized the importance of the program when her daughter attended during the loss of her grandparents. It benefited her family through the grieving process and helped her daughter with providing the support she needed to overcome the loss. This program provides the support and tools needed in helping children deal with grief and long-term illnesses of family members and loved ones. The “Buddies” are a part of the program that provide a feeling of comfort and safety for children going through a tough time in their lives. Each child attending the art therapy program will name their bear and it is theirs to keep.

SCAN programs sustain independence, health and happiness among people fifty years of age and older in Monmouth and Ocean Counties by using education and technology to create an environment where the aging population can continue to grow intellectually as well as physically and emotionally, thereby allowing older adults to thrive – not just survive. SCAN is a place where Active Adults meet to be Informed, Empowered and Inspired. Throughout the year, we will continue to introduce new programs and services that complement our mission.
SCAN Partnered With Monmouth University For A Unique Service Learning Experience During The Fall Semester Our SCAN members had an exceptional opportunity to extend their learning experience during the Fall Semester by attending a class at Monmouth University. Specialist Professor Sue Polito, a Geriatric Nurse Practitioner and a faculty member in the School of Nursing and Health Studies, taught the class called Human Aging. 15 SCAN members attended the class along with the MU students. One goal of the Human Aging intergenerational class was to help younger folks develop empathy for aging adults when they were exposed to the real-life situations of the SCAN members. The younger students had the opportunity to meet and interact with their SCAN classmates who are well advanced in the aging process. The more senior class participants had the chance to share the realities of aging with their younger counterparts. MU students also joined their older counterparts at classes at SCAN throughout the semester, choosing subjects such as Art, Bone Up Against Osteoporosis, Dance for Fitness, Spanish & Italian Language classes, How to Use Technology, Learn to Play Mahjong and more. As the students gained a better understanding of the realities of the aging process, they gained valuable insight on how to improve their own health as they continue to age. It was a wonderful experience for both SCAN members and the Monmouth University students that benefited everyone who attended.

Industry experts from Sunrise Senior Living will provide an informative and visual presentation on everything you always wanted to know regarding assisted living and other senior living options. Attend this workshop and leave with expert knowledge on how to determine the best approach to senior care. This workshop will walk you through the 5 Wishes and the conversations you should be having with family, friends and healthcare providers about your wishes in advance of a serious illness. The 5 Wishes are (1) the person I want to make care decisions for me when I can’t; (2) the kind of medical treatment I want or don’t want; (3) how comfortable I want to be; (4) how I want people to treat me; and (5) what I want my loved ones to know. Discussion points will also include POLST and some of the decisions at end of life such as DNR/DNH/DNI, feeding tubes/IV, hospice, and endof-life doula. Instructor: Jennifer Dral

Axelrod Pac Announces 2020 Musical Season
Enjoy a taste of Broadway at the shore!

The Axelrod Performing Arts Center is producing three professional hit musicals for their 2020 season: MATILDA, GREASE, and 42ND STREET. Subscripting pricing is available for multiple shows and makes a great holiday gift for theater lovers. MATILDA will debut on March 6 and run through March 20. Based on
the fan-favorite children’s book by Roald Dahl, this adult take on childhood is a perfectly captivating show. Directed and choreographed by Broadway’s own Ricky Hinds, this award-winning musical captures the pitfalls of childhood, the lucid imagination that steals your attention, and what it means to yearn for something greater.
GREASE, based on the unforgettable movie musical, will take the stage May
29-June 14. Join greaser Danny Zucko and the rest of his pack, as well as Sandy Dumbrowski and the rest of the Pink Ladies, as they journey through high school, love, friendship, and rivalry in the 1950s. Packed with energetic songs paired with memorable dances, such as “Greased Lightnin’” and “You’re the One that I Want,” GREASE will electrify the Axelrod.
42ND STREET, centered around aspiring chorus girl Peggy Sawyer of
Allentown, PA, comes to the Axelrod November 6-22. Inspired by Busby Berkeley’s esteemed show-business movie musical, 42ND STREET captures the
journey of Peggy, who finally gets her big break on Broadway when Dorothy Brock breaks her ankle, and director Julian Marsh asks her to fill the vacancy. Don’t miss out on this tap-dancing extravaganza! “Our 2020 season is filled with excitement,” says Axelrod Artistic Director Andrew Deprisco. “Each show features some of Broadway’s leading talents along with the energy and passion that is unique to live, professional theater.” The Axelrod’s 2020 season can be purchased via a subscription program that is perfect for lovers of musical theatre and makes a wonderful holiday gift. The subscription, which is currently on sale, guarantees the best seats at every show at the best price. Premium season subscribers pay $171 for 3 shows, saving $30 or $120 for two shows, saving $14.
