Peripheral Neuropathy is a nerve disorder known to affect millions of people throughout the U.S. It is a complication found in several different medical conditions including Diabetes, Peripheral Vascular Disease, Post ChemoRadiation therapies and Metabolic disorders being some of the most common. Underlying the above disease processes is the problem of impaired circulation to peripheral blood vessels, particularly the smaller ones (capillaries). Without sufficient blood flow to nerve tissues, oxygen and nutrients cannot get into the tissues and the waste products of metabolism cannot get out. When functioning normally, nerve tissues receive nutrient rich, highly-oxygenated blood carried to them by capillaries, however they are easily clogged by this debilitating process. The peripheral nervous system eventually becomes distressed and damaged due to starvation and toxicity. In advanced cases of diabetic neuropathy, blood glucose can also damage the walls of these tiny blood vessels supplying nerves, especially those in the legs and feet. SYMPTOMS: NUMBNESS, ALTERED SENSATION TINGLING OR BURNING, SHOOTING PAIN, MUSCLE WEAKNESS. LOSS OF BALANCE, COORDINATION, DIFFICULTY WALKING

“We personally invite you to schedule a comprehensive evaluation to determine if you are a MicroVas candidate and to learn more about our One-of-a-Kind Neuropathy Program we are EXCITED to now offer our patients!” -The REHABILITY Team *Most major medical insurances accepted, including Medicare.

MicroVas is a FDA cleared, non-invasive vascular treatment system that delivers electromagnetic energy to targeted areas of the body. After years of research, this technology was originally developed to treat Navy SEAL divers for hypothermia. It has since been engineered as a ground-breaking innovation with the ability to address the root cause of neuropathy, often a lack of nerve fiber circulation in the injured area. MicroVas’ powerful, deeply penetrating waveform causes blood to pump and circulate much more efficiently by significantly raising tissue oxygen levels, resulting in a potent effect on the microcirculation. Studies have demonstrated marked increases in tissue oxygen supply within minutes of initiating treatment. In contrast to other technologies available, this process even results in new capillary formation, laying the groundwork for new tissue growth and repair while accelerating the healing process. MicroVas also accelerates lymphatic drainage as much as ten to thirty-fold, notably reducing swelling and stimulating the body’s immune response. Its therapeutic effects are often dramatic, long-lasting and our unique approach of implementation is designed to help you achieve optimal results for long-term relief. I was suffering with neuropathy for over ten years with no relief from anything else I had tried, including medication. A neighbor recommended a microvas program at Rehability after seeing the great results her husband received having the same treatment there. I was told it could help increase blood circulation in my legs and sure enough, within four weeks I began noticing more feeling in both feet. The therapy team has also helped me with my balance and I’m walking better now than I have in a long time. Thank you to everyone at Rehability for all your help and positivity. – Mary B.

Does Your Financial Plan Work Under the Widest Range of Circumstances? When you are younger, it’s easy to imagine living a long and healthy life. However, as you get older, reality sets in and you begin to realize that an accident or illness can happen at any moment that could drastically change your financial situation. Whether you are a mom, married or single, life insurance is an important part of a financial plan. STAY-AT-HOME MOMS

Debra Serafin, MBA As Director of Financial Educational Services at Innovative Wealth Partners, LLC., my passion for teaching has not stopped upon retiring from public education, but has been transformed into educating clients about financial independence. Open communication, sincerity and the personal touch is our signature approach provided to all our clients.

Losing the parent who stays home with children can devastate a family, emotionally and financially. Many women underestimate how much coverage is necessary to protect their families. As a stay-at-home mom, think about the cost to provide a nanny, a chauffeur, a cook, a housekeeper, a gardener and much more. Salary.com determined that, if paid, a stay-at-home parent would earn $113,000 annually. Without life insurance, would you or your spouse be able to cover that amount until the children no longer need parental oversight? Life insurance can help fill that gap and give your spouse and children some financial stability in your absence. WORKING MOMS Over the past few decades, women’s roles have drastically changed. No longer based primarily at home, women make up 47 percent of the workforce in the United States. (U.S. Census Bureau, Data Ferrett, Current Population Survey, December 2014). As women reach new levels of personal and professional success, it is important to make time to assess your life insurance needs: •

Can your family live in your home without your salary?

Are you adequately insured to replace your income for years to come if you were no longer here?

SINGLE MOMS The emotional attachment between a single parent and child cannot be replaced. However, if mom is no longer there, the child will suffer emotionally and financially. A single mother without life insurance runs the risk of her children’s basic needs not being met if she dies unexpectedly. Life insurance can help provide money to take care of your child’s educational and living essentials many years down the road. SINGLE WOMEN Whether you are single by choice, divorced or widowed, single women are often the heads of their households, providing for every financial need. For single women, the question becomes who will be responsible for debts that are cosigned (car loan, education loan, mortgage, etc.). Aging parents or loved ones on fixed incomes may be responsible and this can be a burden on them. Also, many life insurance policies now come with optional benefits that can provide cash to you should you become chronically ill. This money could be used to pay for a variety of services that you may need. Depending on the life insurance policy chosen, the policy can help accumulate a cash value on a tax-advantaged basis that can be used for lifetime needs, such as funding education or supplementing retirement income. Shannon O’Connor At Innovative Wealth Partners, LLC., we use a team based approach to educate our clients on the strategies to develop a financial plan that works under the widest range of circumstances and prepares for life’s unexpectancies. Knowledge and education are critical to making good financial decisions.

NEXT STEPS Life insurance is an important part of your financial plan. Contact us today to get the conversation started. We’ll be with you every step of the way.

Torticollis: What Is It And How Do You Treat It? Written by Allison Elbeblawy, PTA, Jackson Location What is torticollis?

Treating torticollis with physical therapy

Physical therapy is often prescribed by the doctor and is a successful intervention of the condition. A physical therapist will perform an initial evaluation Torticollis, also known as wry neck, means “twisted neck.” It is most often of the child, which can include measurements and palpation. This muscle is called the sternocleidomastoid or SCM, developed, it is recommended that your child be seen by the physical therapist which is the common abbreviation. The SCM assists in rotation and side or physical therapist assistant as prescribed by the doctor. The recommended stretching exercises performed to treat congenital and acquired muscular torticollis are ineffective and can be dangerous for a child with Klippel-Feil syndrome. It is important to have a thorough examination by your baby’s pediatrician to determine the cause of your child’s symptoms.

What does it look like?

Torticollis is usually spotted in the first couple months of the child’s life. It can be very prominent and can look like your child’s head is tilted to one side and has limited neck movement. Another common sign can be a small bump on the side of the neck. In more severe cases, it can be accompanied by positional plagiocephaly (flat head), which happens when the child consistently sleeps and rests his/her head to one side, flattening that side of the skull. The condition’s severity can also be very mild and may be difficult to detect when looking at your baby. There is no need to worry if you don’t notice because your pediatrician most definitely will. Your pediatrician will provide a physical examination and possibly order X-rays to determine which type of torticollis your child may have. Depending on the type and severity, your doctor may order other diagnostic testing.

FILING FOR DIVORCE: THE BASICS Going through a divorce is a long and often painful journey. Having an experienced attorney to help guide you down the right path is critical. Besides the emotional pain that people often suffer when going through a divorce, there are complex legal steps that must be taken. Having an attorney to represent your interests ensures that you will receive every protection New Jersey state law has to offer to you. THE FIRST STEP in the divorce process is filing a complaint that sets forth the grounds for divorce. The complaint will be served to the other spouse who will file an answer. Your options for filing for divorce are grouped under two major headings: no-fault divorce and fault-based divorce.

To pursue a no-fault divorce, you must cite either irreconcilable differences or separation.

Under irreconcilable differences, you or your spouse must have lived in New Jersey for 12 consecutive months before filing for divorce, experienced irreconcilable differences for at least six months which caused the breakdown of your marriage, and have no reasonable chance for reconciliation. A ground of divorce under separation is allowed if the husband and wife have lived separate and apart in different habitations for a period of at least 18 or more consecutive months and there is no reasonable prospect of reconciliation. Many times it is easier for the party to file under irreconcilable differences as it does not require 18 months of separation.

Harry Levin, Managing & Founding Partner & Colleen Cyphers, Partner

To pursue a fault-based divorce, you must choose the grounds provided under New Jersey state law. These include the following: •

Adultery (this ground for divorce is very fact sensitive)

Abandonment (allowed if the spouse has left you and your home for at least 12 months or more)

Extreme Cruelty (includes any physical or mental cruelty which endangers the safety or health of a spouse or makes it improper or unreasonable to expect the spouse to continue to live with the other spouse)

Addiction (allowed if your spouse has had an addiction to drugs or habitual drunkenness for a period of at least 12 months or longer before filing the complaint)

Institutionalization (allowed if your spouse was institutionalized for a mental illness for a period of 24 or more consecutive months after the start of the marriage and before you file for divorce)

Deviant Sexual Conduct (allowed if the spouse performed a deviant sexual act on the other spouse without consent)

Imprisonment (allowed if the spouse was sentenced to a jail sentence of 18 months or more after the beginning of your marriage)

Once the Complaint and Answer have been completed, you will need to file a Case Information Statement (CIS). The CIS is a form that has a detailed financial analysis of the parties which outlines all the income, expenses, assets and liabilities of the parties.

THE SECOND STEP in the divorce process is to schedule a Case Management Conference (CMC). The purpose of the CMC is to assess the issues of the divorce that will need to be settled. Depending on the county where the Complaint was filed, you and/or your attorney may have to appear. This conference will set timetables as to when discovery must be completed and what experts may be needed. Discovery is the process by which both parties exchange necessary information and, depending on the complexity of the case, may take significant time. THE THIRD STEP in the divorce process is the scheduling and participation of an Early Settlement Panel conference (ESP). The goal of the ESP is to discuss ways to resolve the divorce case. The ESP is comprised of experienced attorneys who make non-binding recommendations as to how to settle your case. These attorneys practice in the county the Complaint was filed and are experienced in family law and with the judge. They provide an unbiased opinion on how to best resolve the matter. If an agreement is not reached at the ESP, economic mediation will occur to try and settle the divorce. This is held by an economic mediator who will try and resolve the financial issues in the case. If this fails to resolve the issues, a trial will be scheduled. A trial is the final step in the divorce process where a Judge will hear testimony from the parties, experts, and witnesses before rendering a final decision. Once the Judge makes the decision, the Trial will be concluded; however, you may be able to appeal the decision to a higher court. This provides a broad overview of the divorce process and what you can expect at the various stages. The attorneys at LevinCyphers have over sixty years of legal experience and are willing to fight to ensure that your legal rights are protected.

Merkel Cell Carcinoma Merkel

cell carcinoma is also called neuroendocrine carcinoma of the skin. It tends to grow fast and to spread quickly to other parts of your body. SYMPTOMS The first sign of Merkel cell carcinoma is a fast-growing, painless red, blue or purple nodule on your skin, most commonly seen on the face, scalp or neck. WHEN TO SEE A HEALTH CARE PROVIDER If you notice a mole, freckle or bump that is changing in size, shape or color, growing rapidly, or bleeding easily after minor trauma, such as washing your skin or shaving, make an appointment with a dermatologist.

RISK FACTORS Factors that may increase risk of Merkel cell carcinoma include: • Excessive exposure to sun or tanning beds. • A weakened immune system including those with HIV infection, those taking drugs that suppress the immune response or those with chronic leukemia. • History of other skin cancers such as basal cell or squamous cell carcinoma. • Increasing age. This cancer is most common in people older than age 50, though it can occur at any age. • Light skin color. Merkel cell carcinoma usually arises in people who have light-colored skin. COMPLICATIONS Even with treatment, Merkel cell carcinoma commonly spreads (metastasizes) beyond the skin. Merkel cell carcinoma tends to travel first to nearby lymph nodes. Later it may spread to your brain, bones, liver or lungs, where it can interfere with the functioning of these organs. Cancer that has metastasized is more difficult to treat and can be fatal.i

Estate Planning Options For Blended Families Written by Christine L. Matus Esq. The dynamics of a blended family, defined as one where at least one spouse has at least one child from a prior marriage or relationship, can complicate financial and estate planning because no off-the-shelf plans apply. It’s important to contact your estate-planning lawyer to ensure complete review of all personal and economic aspects of your family and a resulting plan that works for everyone involved. From designating account beneficiaries to updating wills and trusts, it takes attention to detail to ensure specific wishes are carried out properly. Effective, collaborative planning can address the family’s needs and goals while building trust and helping everyone move forward together. A good place to start is with reviewing and updating beneficiary designations for life-insurance policies and retirement accounts. That’s a simple way to ensure that the proper beneficiaries are noted on all accounts and the proceeds from those accounts end up going to the correct individuals. While any estate planning must be done on a case-by-case basis, the following are options to consider for blended families: RECIPROCAL WILLS Some blended families use reciprocal wills,

where the assets pass outright to the surviving spouse, as their primary estate plan. They do this for the sake of simplicity and to keep their estate planning costs at a minimum. But this type of plan comes with some serious disadvantages. One issue is that it does not provide a great deal of comfort when there are children from a prior marriage, because the first spouse to die has no guarantee that the surviving spouse will provide for his or her children when the surviving spouse ultimately dies. NON-RECIPROCAL WILLS An option that does not require reliance on and trust in the surviving spouse is to have each spouse create a non-reciprocal will, or wills that are not exactly the same and don’t leave the estate outright to the surviving spouse. Under this approach, each spouse

could leave a percentage or dollar amount to the surviving spouse and a percentage or dollar amount to be divided equally between his or her own children, but not the children of the other spouse. It can be difficult to determine at the time the will is made the amount needed to provide for the spouse and an appropriate amount to go to the children, so this type of estate plan likely requires monitoring and updating over time. TESTAMENTARY TRUSTS Creating a testamentary trust that becomes irrevocable upon the death of the first spouse meets the dual goals of providing for the surviving spouse during his or her remaining lifetime and then, upon the death of the surviving spouse, passing the remaining assets to the children of the first spouse to die. Under this approach, a trustee will have to be appointed. Common options include the surviving spouse, a child of the first spouse to die, a third party or a trust company. Appointing the surviving spouse or a child of the deceased spouse has a greater risk of creating family friction, and therefore a third party or a trust company might be more appropriate. Whichever option you choose, any estate plan should remain dynamic and adapt to change when necessary, particularly given the added complexities of a blended family.

Is Your Elderly Parent Fighting You Over Getting Home Care Assistance? Deborah Walsh,

Executive Director, HomeWorks Nothing is harder for a family caregiver than an elderly loved one who refuses needed help. This is a very common and difficult caregiving challenge that families frequently face. Hiring in-home care is oftentimes not a choice that aging parents are agreeable to, despite their desire to remain at home. Mom and Dad often will feel that outside help is a threat to their independence and an invasion of their privacy. Apprehension and distrust of in-home caregivers are also common reasons for refusing home care. Here are some tips on dealing with resistance to help:

peaceful setting. It’s best not to do this at a time of crisis or when you are frustrated and rushed or during the holiday times. 5. Be patient and listen Listen to your loved one’s concerns and objections to the idea of home care. Ask open-ended questions and give time for a response. To build trust, listen with empathy and validate rather than deny their feelings. Conversations may be repetitive and veer off topic. It may take several talks, so be ready! 6. Get the caregiver’s foot in the door An alternative to “caregiver” is to suggest hiring a “home helper,” someone to manage household chores and NOT provide actual hands-on care or “personal assistance.” Often this is seen as less threatening. Emphasize getting help handling the heavy housework and laundry. Suggest help with food shopping and traveling to appointments. Your parents may accept help they would not have otherwise been agreeable to. Your hope is that they will see the value in having assistance by developing a trusting relationship with their caregiver. 7. Call upon a trusted professional Another idea might be to seek the help and advice of a trusted professional who is someone your parents hold in high regard. They might surprise you by their willingness to accept the advice of a longtime family physician, pastor or a family friend in the medical field. 8. Point out the benefits Steer the conversation to the benefits of home care. For example, if a caregiver is grocery shopping and preparing meals, it frees up time for other things. Encourage parents to see how getting assistance with basic tasks can allow them more opportunities to pursue hobbies and personal interests.

1. Observe Take time to visit and watch your parent live in their own home. What are they able to do well, and what are they struggling with? Think about how home care will help make their lives easier. 2. Unity Meet with siblings to determine how other members of the family feel about the situation. When everyone is unified, it shows the parent that everyone has the same concerns, and wants what is best for them. A divided family only causes more problems. 3. Know your options in advance Rather than entering into the conversation blind, do your homework beforehand. Know exactly what services are available in your area and be ready with specific recommendations. 4. Pick the right time and place for the conversation Try to have a relaxed conversation about accepting help before a health crisis, preferably away from your homes. Go out to breakfast or to a park or other For more information, call 732-838-1950 or visit www.UMCHomeworks.org.

Are You Living By What’s True To You

Or By What Your Truth Really Is?

You know that old-time phrase, “It’s 5 o’clock somewhere!” Well, that is true when you’re looking at the clock and it says 5pm. But it’s not the TRUTH. The Truth would mean it’s 5pm everywhere, it would be universal, and we know that’s not the case. If it’s 5pm in New Jersey, we know it’s 2pm in California, 3pm in Arizona, 4pm in Illinois, 9pm in London, 5am (the following day) in Singapore, and 8am (the following day) in Sydney. All over the world it’s a different time. So 5pm where you are standing is not the Universal Law, it’s not the Truth! However, it certainly is true when you’re looking at the clock and it reads 5pm!

So what’s your Truth? Are you only living by

what is true in the moment or what really is the Truth? Interesting question to ponder, huh!? Do you get up each morning and go to a job you hate because you believe that is what people do to support their families? Have you been overweight most of your life and now just accept it for who you are? Are you stressed going around and around on the treadmill of life just trying to keep up, accepting that this is the way life will always be? Well, I can tell you that there are people who get up every day and love going to their jobs. There are people who struggled with their weight for years, but decided to get it under control and now live a healthy lifestyle. There are people who go at their own pace on

the treadmill of life, not just keeping up, but stopping when they want to explore, slowing down because they can, and speeding up because they want to! My point is, whatever you are telling yourself is true may not necessarily be the Truth. The Truth is who you really are. When you deal with your limiting beliefs head-on, you are able to see the choices you have. A limiting belief is something you accept about life, about yourself, about your world or the people in it that limits you in some way.

We often make a decision about ourselves… I’m not smart enough, I’m not funny enough, I’m not pretty enough, I’m not skinny enough, I’m not fun enough, I don’t have what it takes, etc.…and then we live our lives by those rules, by those limiting beliefs! Who made those rules? Most likely you did. And unfortunately we usually are our own worst critics!

So again I ask, “What’s your Truth?” What

was true yesterday doesn’t have to be your Truth today! You get to decide each and every moment who it is you want to be. That’s the beauty about life: you get a second chance each and every day, minute to minute, to decide who you want to be and what you want to do! So I encourage you to think about your Truth and about the rules you have made up for yourself. And ask yourself, “How well are they serving me?” And if they are holding you back from having a joyous, purposeful life, then stop believing them. Go make up some new rules and create your new story!

I believe in you!

But what if those beliefs weren’t true? What if you haven’t been living the Truth? What if YOU got to decide what was the Truth? What if you were enough? What if you were smart enough? What if you did have a special gift to share with the world? How would your world change? How would your family’s world change? How would society change?

BAHA Implant Offers Better Hearing Submitted by Dr. Stephen Kupferberg Millions of American’s are affected by hearing loss. It affects nearly one in three adults over the age of 65. Hearing loss can lead to social isolation, depression, anxiety and may even be involved with dementia. There are several different types of hearing loss. Conductive hearing loss occurs when sound can not be transmitted from the outside world to the inner ear. The problem may involve the outer or middle ear. Sensorineural hearing loss involves a problem with the inner ear’s hearing organ, the cochlea, or the nervous connection to the brain. Finally, there is mixed hearing loss, which involves some element of both conductive and sensorineural hearing loss. Hearing loss can involve one ear or both. Unilateral hearing loss can lead to the inability to Cochlear BAHA Connect localize sound, head shadow effect, problems in social situations and tinnitus. This type of hearing loss may be System due to chronic outer or middle ear infections, congenital adnormalities, otosclerosis, sudden hearing loss or tumors. If there was serviceable hearing in the worse ear and the ear canal was open then a hearing aid could be of benefit for the patient. If the hearing in the poorer ear was not functional then other alternatives exist. These include contralateral routing of sound (CROS) hearing aids, bone anchored hearing aids (BAHA) or cochlear implants. Unlike traditional hearing aids, BAHA conducts the sound through bones of the skull to the functional inner ear/cochlea on the opposite side. It bypasses the non-functional ear. Sound is delivered to the brain from both sides of the head through the good ear and there is a slight differential in the signals received. This allows the Medtronic Sophono patient to localize where a sound is originating. It does not System make the bad ear become functional. There is no blockage or occlusion of the ear canal, which can occur with a traditional hearing aid.

The BAHA involves a surgical procedure to place a screw into the skull behind the ear of the worse hearing ear. There are two interfaces available to connect the screw to the processor. A direct connection can be made by attaching an abutment to the screw which has its end situated above the skin. The processor snaps directly into the abutment. A second option is a magnetic connection, which is made by attaching a magnet to the screw and covering it with the skin of the scalp. Another magnet is attached to the processor. The magnets are attracted to each other with the tissue of the scalp in-between. Nothing is visible on the skin surface when the processor is not in use. The direct connection gives slightly better transmission of sound. After six to eight weeks, the processor can be utilized to restore hearing. The BAHA offers patient with single sided hearing loss another option to restore their ability to hear sound. It eliminates blockage of the ear canal, which can happen with a traditional hearing aid. It can be hidden under the hair behind the ear. Finally, there are accessories available including a remote control, wireless phone clip, wireless mini microphone, and a wireless TV streamer. All of these can enhance a patient’s success with a BAHA and improve their quality of life.

What is integrative medicine? Integrative medicine is patient-centered health care that looks at all aspects of health and illness. Integrative medicine looks at the whole person, and not just the disease or symptom. It takes into account the person’s lifestyle, from the physical and psychological to the environmental and genetic. It uses all medical systems for healing and treatment, from conventional approaches and mind-body medicine, to old world traditions such as botanicals and acupuncture. It cherry-picks the best of traditional medicine and what is commonly called alternative or complementary medicine. It stresses prevention and less invasive, more natural treatments. Integrative practitioners believe that everyone has an incredible capacity to heal.

What is unique about the Hackensack Meridian Integrative Health and Medicine program? Through our program, we offer a cuttingedge team approach through medical consultations with our nurse practitioner and physician, treatments from an acupuncturist, dietary consults with a nutritionist, and lifestyle improvements with our health coach and health psychologist. Patients who are seen in our center get a comprehensive evaluation of the five pillars of health: resilience, purpose, nutrition, activity and sleep. The team consists of specialists who have exemplary training in integrative and functional medicine, treating the whole person—mind, body and spirit. Patients can self-refer to any of the integrative medicine staff members, meaning if someone wants to see the nutritionist, they do not need to see the integrative physician first. Above all, at Hackensack Meridian Integrative Health & Medicine, each patient partners with an integrative practitioner to create an individualized treatment plan. As a result, our patients become empowered to make healthy lifestyle changes.

Empowering our community to reach the highest level of health and wellness

Hackensack Meridian Integrative Health & Medicine has developed a comprehensive health and wellness program like no other. Unleash Your Smile With You may be somewhat familiar with how the Invisalign® treatment works. Whether you’re considering treatment for yourself or someone else, knowing more about the entire process can help you be more confident in your decision to choose Invisalign and enjoy a better smile every day. A Customized Treatment Plan Is Created For You

Dr. Olcay will take x-rays, pictures and impressions of your teeth, which Invisalign will use to create a digital 3-D image of them. From these images, she will map out a precise treatment plan, including the exact movements of your teeth, and tell you the approximate length of treatment. While every case is unique to each patient, treatment typically takes approximately a year for adults. For teens, the length of treatments is comparable to that of braces.

Receive Your Custom Aligners

Based on your individual treatment plan, a series of custommade, clear aligners is then created specifically for you. These aligners are made of a smooth, comfortable, BPA-free plastic that won’t irritate your cheeks and gums like traditional metal braces often do. Simply wear them throughout the day, and remove them when you eat or to brush and floss your teeth. As you wear each

set of aligners, your teeth gently and gradually begin to shift into place. And because they’re virtually invisible, most people won’t even notice you’re wearing them.

Wear a New Aligner Every Two Weeks

Holmdel Orthodontics 723 North Beers Street Suite 2A Holmdel, NJ 07733

Approximately every two weeks, you will begin wearing a new set of aligners, advancing you to the next stage of your treatment. To monitor your progress, you will also have occasional checkups with your doctor, usually only every six weeks or so. For best results and a timely outcome, you should wear your aligners for 20 to 22 hours per day, throughout your entire treatment. At every stage, you will be able to see how much closer you are to having the smile that reflects the real you.

Your doctor has mentioned that you should have a colonoscopy or endoscopy, but you are concerned about the procedure and anesthesia.

WHAT SHOULD YOU DO?

Many patients have concerns about endoscopic procedures and anesthesia. Patients think they may be awake and be uncomfortable during the exam. Other patients feel they may lose control during the procedure. We have all read things in the news about what may have happened with celebrities such as Michael Jackson or Joan Rivers. You should not delay in having a potentially life-saving medical test such as an endoscopy or colonoscopy due to these concerns. First, and most importantly, the most essential part of having a good experience with anesthesia is having a qualified, trained and diligent anesthesiologist working with your gastroenterologist during the entire procedure. Second, Propofol, which is the most commonly used sedative given for endoscopic procedures, is extremely safe as long as given in the right situation with appropriate supervision.

Patients often ask, “Do I need to be asleep for the procedure?” or “Can I have the procedure without anesthesia?” The reason anesthesia is given is for the comfort of our patients, and also, if patients are comfortable during the procedure, it allows your gastroenterologist to perform a more complete exam, and detect any polyps or other abnormalities easier. Patients in our facility will be closely monitored throughout, with oxygen levels and heart rates being checked constantly. There will be no breathing tubes used for the procedure. Propofol is short acting, and patients wake up feeling refreshed and often ask, “Is the procedure really done?” The most important thing to do to minimize any risks is to listen to your doctor’s suggestions for the prep and procedure day. No solid foods should be eaten on the day of the procedure. Patients can only drink clear liquids up until 4 hours prior to the procedure. This reduces risk of aspiration (food going into the lungs) during the procedure. Continue to take any heart and blood pressure medications (unless specifically directed by your doctor for blood thinners) so that your medical conditions remain stable for the procedure. Make sure to have a driver, as you will not be able to drive after the procedure. Remember, endoscopy and anesthesia are both very safe. Many patients say the anesthesia is often the best part of the exam, and the best sleep they have ever gotten. Make sure to talk to your doctor about if you are due for your colonoscopy. We look forward to helping to keep you healthy.

Is Spine Surgery Right for You? It’s a Matter of Risk vs. Benefit While many people shy away from back surgery, others – like Denise Kordulak – have no choice. The benefits far outweigh the risk. Many people who suffer from debilitating back pain resist surgery – thanks to failed stories from years past. While surgery should be the final option after trying other non-invasive therapies, today’s success rates for spine surgery are significantly different from the past. Take, for example, Denise Kordulak, a 66-year-old retired x-ray technician from Neptune, NJ. Years of pushing, lifting and carrying equipment and positioning patients took a huge toll on her back. Fourteen years ago, she had surgery to relieve her pain and restore the feeling in her feet. While it was somewhat successful, she never returned to normal.

And, when it’s needed – and possible – we use pioneering minimally invasive Atlantic NeuroSurgical Specialists’ Dr. Pinakin Jethwa, a nationally renowned neurosurgeon and spine expert, reviews a patient’s films.

Denise started walking with a hunched back. Last year, the pain became unbearable. Eventually, she lost feeling in her left leg from the knee down, and the pain migrated to her right leg. It was so bad she couldn’t stand for more than a few minutes. Denise contacted Dr. Pinakin R. Jethwa, a board-certified neurosurgeon at Atlantic NeuroSurgical Specialists (ANS).

“When Denise came to me, she was suffering from a complex lumbar spine issue. Compression of the nerves was causing her leg pain. Degeneration of her discs was causing her back pain. And, one of the bones of her spine had slipped out of place, causing her hunched back,” Dr. Jethwa explains. “In an eight-hour surgery, we performed a multi-level spinal decompression and fusion operation.” The parts of the bones and discs that were compressing Denise’s nerves were removed. Screws and rods were placed in the bones to stabilize and fuse the degenerated discs, and the bone that was slipped out of place was realigned into a more normal position.” “Since then, I have been pain free,” Denise says excitedly. “I am a completely new person. I can stand up straight again, and I feel wonderful!” “Denise actually drove herself to the office and, for the first time in years, walked in unassisted,” Dr. Jethwa adds. “She told me she has her life back. In her case – and many others – the benefits far outweighed the risk.” For more information about spinal decompression and fusion, visit us at ansdocs.com or call 732.455.8225.

When It’s Worth the Risk, It’s Worth Integrated Care When the body’s nervous system – the delicate and complex command, control and communication network made up of the brain, spine and neurovascular system – is working properly, it’s taken for granted. It just works the way it’s supposed to. But when it doesn’t, the impact can be dramatic and life-altering. Regardless of the cause – a brain tumor, herniated disc, stroke or any other trauma to the nervous system – having an integrated care team providing medical care is essential. According to Dr. Yaron A. Moshel, codirector of the Brain Tumor Center at Atlantic NeuroSurgical Specialists (ANS), the largest neurosurgical practice in New Jersey, integrated care brings together all of the subspecialties related to one medical specialty for comprehensive care. “For example, at ANS, we have an integrated care system that brings together all brain tumor subspecialties in one place,” he explains. “Our brain tumor center treats only brain tumor patients with all of the experts needed to provide every component of care right here.” ANS’s Brain Tumor Center includes specialists in neurosurgery, otolaryngology, radiation oncology, medical oncology, neurooncology, neuro-radiology, neuro-ophthalmology, neurology, neuropathology, physiatry – as well as consultations in nutrition, alternative medicine and social services. The benefits to this collaborative approach are many – not just for brain tumor patients, but for patients with spine and neurovascular issues as well. Having specialists dedicated to each of these three specialties within neurosurgery – versus generalist neurosurgeons – brings the depth of experience and shared knowledge that enables the most effective multidisciplinary treatment plan to be customized for each patient. ANS’s Spine Center – under the direction of Dr. John J. Knightly – follows the same principle.

other qualified support staff to deliver exceptional care,” Knightly details. “That’s one reason we perform the most minimally invasive spine surgeries in the state of New Jersey.” Another is ANS’s pioneering use of “Big Data,” a powerful tool that helps identify the right treatment for the right patient at the right time. Dr. Knightly serves on the Spine Executive Committee of the American Association of Neurological Surgeons – Congress of Neurological Surgeons, the organization that launched the first national neurosurgical clinical registry. This outcomes database collects and analyzes clinical data that measures the quality and efficacy of common spinal and neurosurgical procedures. Likewise, ANS’s Neurovascular Center comprises a highly trained group of neurosurgeons, radiologists, stroke neurologists, neurointensivists and stroke rehabilitation experts who are immersed in researching new techniques. According to Dr. Ronald P. Benitez, center director and president of ANS, “ANS’s Neurovascular Center is at the forefront of ongoing research, delivering the latest innovative treatments to the most complex neurological disorders, including stroke and aneurysm.” ANS’s success rate for treating strokes is higher than the national average. Regardless of the center, ANS’s team of specialized, board-certified neurosurgeons and professionals are dedicated to delivering exceptional integrated medical and personalized care. ANS is known for its cutting-edge approach to medical therapies – many of which are non-surgical; active participation in life-changing clinical trials; and holistic patient support including medical, emotional, financial, and more.

“The ANS Spine Center brings together board-certified, fellowship-trained neurospine specialists, physical therapists, pain management physicians, chiropractors, and

Q&A with Martin P. Michalewski, M.D., F.A.C.O.G.

What Women Should Know about Female Pelvic Medicine and Reconstructive Surgery Female Pelvic Medicine and Reconstructive Surgery (FPMRS) is an emerging subspecialty of medicine, combining the fields of urology with obstetrics and gynecology. Barnabas Health Medical Group physician Martin P. Michalewski, MD, FACOG, a nationally and internationally recognized expert in minimally invasive laparoscopic Urogynecology and Robotic Surgery, was among the first 500 U.S. physicians to obtain board certification in FPMRS. Here, Dr. Michalewski provides an overview of the developing subspecialty of FPMRS, which helps women restore or maintain a high quality of life.

Q: What is Female Pelvic Medicine and Reconstructive Surgery (FPMRS)? Dr. Michalewski: The field of urogynecology has evolved to be a unique subspecialty focusing exclusively on women's pelvic and abdominal health problems. The American Board of Medical Specialties approved Female Pelvic Medicine and Reconstructive Surgery (FPMRS), also known as urogynecology, as a certified medical subspecialty in 2011, with the first 500 doctors in the United States receiving board certification in 2013.

Q: What types of medical conditions are treated by a physician who is board certified in FPMRS? Dr. Michalewski: Urogynecologists certified in FPMRS focus on benign and malignant pelvic conditions, urinary tract disorders and pelvic floor dysfunction (PFD). This includes urinary or fecal incontinence, urinary urgency, painful bladder syndrome, chronic infections, pelvic organ prolapse, fibroids, endometriosis, adenomyosis, abnormal or heavy bleeding and a number of other female pelvic issues. Women suffering from these conditions may need to avoid social outings, exercise, long car rides or need to wear pads or diapers. Often, there is a significant negative impact on sexual function and/or daily activities, as women must always be worried about access to a bathroom.

Q: Why was there a need for the FPMRS subspecialty? How were women with these conditions being helped in the past? Dr. Michalewski: According to data from National Center for Health Statistics, our population is aging and living longer, with the average life expectancy reaching 78.8 as of 2014. Likewise, the number of women experiencing pelvic floor disorders, bladder problems and other similar issues is also growing. One out of every three women over 45, and half of women over 65, suffer from PFDs or their symptoms. Thirteen million U.S. women over age 45 experience stress urinary incontinence and 33 million women have prolapse. There are 3 million women living with fibroids, and, by the age of 60, one out of three will have their uterus removed. These statistics are alarming and speak to the need for treatment. In the past, and still today, there were many myths surrounding gynecological conditions, including the belief that these conditions are “a normal part of aging and women just need to deal with it.” Well, they are not – treatment for these conditions exist. Previously, if a woman had a problem relating to her bladder, pelvic or abdominal area, a gynecologist and an urologist would treat that patient together but if outcomes were not as expected, the patient would bounce back and forth between the two physicians without good resolution. The specialization in FPMRS provides women with a one-stop shop for any issues they may have and one physician responsible for delivering results.

Q: Why are so many women experiencing these issues? Dr. Michalewski: Unfortunately, the risk factors for PFDs have a lot to do with lifestyle and age. Other risk factors include: prior vaginal births; genetic predisposition, hormones and aging; obesity and heavy lifting; and prior hysterectomy.

About Barnabas Health Medical Group Barnabas Health Medical Group is a multi-specialty group practice comprising highly trained and experienced world class primary and specialty care physicians. Barnabas Health Medical Group physicians provide compassionate and innovative cutting edge care for people at every stage of life at easily accessible facilities conveniently located throughout New Jersey. Backed by the vast continuum of resources throughout RWJBarnabas Health, our dedicated physicians and expertly trained supporting staff continuously strive for clinical excellence. For more information, visit www.BarnabasHealthMedicalGroup.org. Previously, it was believed that if a woman was experiencing prolapse, the uterus should be removed. We now know that removing the uterus in these cases only adds to pelvic floor problems that are then more difficult to treat. Also, as mentioned, prior hysterectomy is in itself a risk factor for pelvic organ prolapse and bladder problems. The type of hysterectomy most commonly done today is open abdominal, also known as total abdominal hysterectomy (TAH). After undergoing a TAH, which includes total removal of the uterus and cervix, eight out of 10 women will have problems in the future. Thanks to advancements in technology and medicine, we now know that treating these issues correctly the first time is of the utmost importance. Proper and thorough evaluation, patient education and careful choice of treatment leads to the best outcomes and highest patient satisfaction. Research unequivocally proves that outcomes of high volume surgeons are by far superior, making the choice of physician very important. Patients must do their homework and ask the right questions before deciding on a surgeon.

Q: How are pelvic conditions, urinary tract disorders and other related disorders treated? Dr. Michalewski: Treatment is highly individualized and

includes a stepwise approach, as new options and medications are being approved regularly. In fact, most of the treatments in FPMRS are non-surgical - diet or lifestyle modifications, stress or pain management can often greatly help patients. A host of non-surgical treatments, including acupuncture, neuromodulation, computer-generated pelvic floor muscle therapy, bladder training techniques and Botox® injections, exist that can be completed in the office and have minimal or no side effects. If surgery is required, minimally invasive laparoscopic surgeries, robotic assisted surgery or single incision laparoscopic surgeries (SILS) are the norm – providing the best outcomes, fastest recovery and minimal time in the hospital, allowing patients to go home the very same day. FPMRS is not typically a life-saving area of medicine. However, thanks to advancements in medicine, in nearly all cases, it is life improving for our patients - allowing them to return to living life to the fullest.

There’s Help For Summer Skin Cancer

The good news is that skin cancer is almost 100 percent curable if found early and treated promptly! What is skin cancer?

Cancer is a disease in which cells become abnormal and form more cells in an uncontrolled way. With skin cancer, the cancer begins in cells that make up the skin. The skin is the body’s largest organ. It protects us against heat, light, injury, and infection. It regulates body temperature and stores water, fat, and vitamin D. With skin cancer, new cells form when the skin does not need them, and old cells do not die when they should. These extra cells form a mass of tissue called tumors. Not all tumors are cancer! Tumors that are cancer can invade and damage nearby tissues and organs, and sometimes can spread to other parts of the body. Skin cancers are named for the types of cells where the cancer starts. At the Institute for Advanced Radiation Oncology, we are committed to making your cancer treatment a comfortable and successful experience. We offer individualized High Dose Rate (HDR) Brachytherapy for skin cancer, which minimizes the amount of radiation and side effects on nearby healthy tissues.

What is Skin HDR Brachytherapy?

Skin HDR (high dose rate) Brachytherapy is a radiotherapy technique, which is performed for certain types of basal cell or squamous cell skin cancers. It is a non-invasive curative treatment for skin cancers, which allows direct placement of the radiation over the lesion and provides maximize treatment results with little to no effects on the surrounding areas. It provides an effective alternative treatment with minimal side effects for selected patients and those who cannot be treated surgically.

I nstitute F or A dvanced R adiation O ncology We Specialize In:

• Gamma Knife Radiosurgery • Tomotherapy • Image Guided Radiation Therapy (IGRT) • Stereotactic Body Radiosurgery • Intensity Modulated Radiation

Squamous cancer nose

Three months after HDR radiation

What is the treatment time?

HDR Brachytherpy is typically delivered in 3-5 minutes sessions over the course of six visits over a two week period of time. Skin cancer is effectively eliminated over 90 percent of the time. The treatment is completely painless.

Benefit of HDR Brachytherapy on skin cancer

• Radioactive Seed Implantation (Brachytherapy)

732-923-6890

300 Second Ave., Long Branch, NJ

With this technique, a skin cancer is eliminated without surgery and therefore, without the potential disfiguring effects of removing lesions along the face, ears, and other areas where cosmetic appearance is important to you. The reduction in normal tissue injury means more rapid and complete functional recovery. HDR provides shorter overall treatment times than external therapy beam radiation. Each fraction (session) of treatment can be completed in a matter of minutes.

Anemia:

Ocean Hematology & Oncology Center for Cancer & Blood Disorders

A Symptom, Not A Disease Are you anemic? You don’t have the energy you used to have. You are getting tired easily; many routine day-to-day tasks are getting more difficult. Others are noticing that you look pale; you want to sleep day and night… Chances are, you may be anemic. Anemia is not a disease in itself, rather a symptom of some other underlying ailments. This could be something as simple as a deficiency of iron or other nutritional supplements or less commonly, a major illness such as cancer or bone marrow disorders. What is anemia? There are three types of cells in our blood. Red cells (RBCs), White cells or (WBCs) and Platelets. Red cells contain a red pigment called hemoglobin that carries oxygen to the different parts of the body. White cells help to fight infection. Platelets help to prevent bleeding. Anemia is a condition in which the number of healthy red cells (RBCs) in the blood is low. This results in reduced oxygen supply to different parts of the body. When this is severe, it can cause difficulty in breathing, fatigue, dizziness and lightheadedness. It can diminish the oxygen supply to the heart muscles and can lead to heart attacks. Reduced oxygen to the brain can lead to syncope (fainting). What causes anemia? Anemia can be caused by either a defect of production of red cells from the bone marrow or excessive loss of blood/blood cells. • Iron or B12 deficiency: Blood cells are mainly made inside the bone marrow. (Marrow is the red juicy material inside the bones). Certain nutrients are needed for the production of blood cells; especially RBCs. Iron is a component of the red pigment hemoglobin. In addition, vitamins like vitamin B12 and folic acid are also needed to make blood cells. Deficiency in the diet or poor absorption of iron and B12 can lead to anemia. • Hemolytic anemia: If there is excessive destruction of blood cells a process caused hemolysis, it leads to anemia. This can be hereditary, such as sickle cell anemia, Thalassemia (Meditaranean anemia) or acquired, such as autoimmune hemolytic anemia. In the latter, our body makes an antibody against our own red cells and it destroys the red cells in circulation. • Bone Marrow Diseases: If there are abnormal cells in the bone marrow, due to leukemia or other types of cancers, production of blood cells will be suppressed. Aplastic Anemia is a condition where the production of all blood cells, including red, white and platelets are markedly diminished. • Anemia from blood loss: If you have excessive blood loss (heavy menstrual cycles, bleeding ulcers, polyps or tumors in the stomach and intestines), it can lead to anemia as your bone marrow may not be able to keep up with the extent of blood loss. • Anemia of chronic dis-ease: Certain diseases such as Rheumatoid arthritis, Lupus, cancer and chronic infection can suppress the bone marrow and lead to decreased production of blood cells. Diagnosing Anemia A simple blood test (CBC) can tell you if you are anemic. It measures the hemoglobin and the hematocrit. Normal hemoglobin varies from 12-15 gms in women and 13.5 to 17.5 in men. These normal ranges vary slightly depending on the laboratory. You can also measure the levels of iron, B12 and foliate in the blood. After these preliminary blood tests, you may be referred to a Hematologist (Blood specialist) who can per-form additional tests, including a bone marrow evaluation.

What is the treatment of Anemia? Treatment of anemia depends on the cause. Taking iron pills or B12 can treat iron and B12 deficiency. In cases of unexplained iron deficiency, you have to look for occult causes of bleeding, especially from the stomach and intestines. An endoscopy and colonoscopy becomes essential in such cases, especially, if you are 50 or older. Anemia from other causes can be improved by treating the underlying illness. Role of ESAs (erythrocyte stimulating agents): A hormone called erythropoietin made by the kidneys stimulates the production of red cells from the bone marrow. This is now commercially available as Procrit/ Aranesp injections and is used to treat certain types of anemia. These drugs should only be used under the supervision of a Hematologist or other physicians familiar with their use.

Got An Itch That Can’t Be Scratched? Dr. Paonessa helps to make sense of your symptoms Do you suffer from aggravating symptoms such as itching, burning and discomfort as well as a “wet” or moist feeling in the perinal area? You may be suffering Pruritus Ani. Pruritus ani is a Latin term literally meaning “itchy anus”. It is an uncomfortable sensation around the anus which produces the desire to scratch. Scratching causes irritation of the perianal skin which in turn causes the desire to scratch more. The skin becomes more irritated and damaged which causes the desire to scratch even more. This becomes a vicious cycle. Pruritus ani affects 1% to 5% of the population. Although it affects both men and women, it is four times as common in men. This condition is most often encountered in patients in their 40’s to 60’s. Pruritus ani is treated medically. An accurate diagnosis is mandatory for proper care. What causes Pruritus Ani? The symptom of pruritus or itching is common to many anorectal conditions. Disorders such as hemorrhoids, skin tags, fecal soilage and/or incontinence, anal fistulae, anal fissures and anal warts all may be associated with itching. This is termed secondary pruritus ani. However, pruritus ani may be independent of these conditions. This is called primary pruritus ani. The cause of the syndrome in these cases is usually from topical agents applied to the perianal skin and/or the pH of the stool. The pH of the stool is determined by foods which we eat. In addition to inadequate hygiene, overzealous or aggressive hygiene using irritating soaps, fragrances and lotions may cause pruritus ani. Overzealous cleaning and the use of topical steroids can destroy natural skin barriers and cause trauma to the anal skin, making the problem worse. As such, attempting to keep the perianal skin too clean, is harmful and may worsen the problem. The use of baby/toilet wipes is a common culprit for many patients suffering from pruritus. It is very important to note that there are many medical conditions that may cause perianal itching and are treated differently from basic, primary pruritus ani. There are infectious conditions such as bacterial skin infections, fungal infections, parasitic infections (pinworms or scabies) and viral infections (anal warts). Skin conditions such as psoriasis, seborrheic dermatitis, contact dermatitis, and lichen sclerosus may cause secondary perianal itching. Local cancers such as Bowen’s disease or Paget’s disease are also potential causes of pruritus ani. How is Pruritus Ani diagnosed? In order to accurately diagnose pruritus ani, your physician will obtain a complete history and physical examination. The timing and duration of the symptoms are important factors. Toileting rituals/behaviors, stool frequency and consistency and hygiene/cleansing routines are noted in order to help make a diagnosis. Travel history, medical history and medications taken are also important. It is crucial to include any supplements taken, topical ointments/creams used and whether or not baby/toilet wipes are being used. A careful diet history is also taken in order to identify any possible contributing agents. Your doctor will perform a physical examination. This will include a careful inspection of the perianal skin and a digital rectal examination. Common findings are irritated, swollen, reddened skin that may have linear ulcerations/cracks. Sometimes the perianal skin may be thickened. An examination of the anal canal via an anoscope will also be Board-certified in general surgery and proctology and a fellow of the American College of Osteopathic Surgeons, Dr. Paonessa specializes in colon and rectal surgery. She has been in practice for 13 years. She completed her colon and rectal surgery fellowship at Lehigh Valley Hospital in Allentown, PA. She is co-editor of the textbook Surgical Treatment of Hemorrhoids, 2nd edition. Dr. Paonessa believes in patient-centered care and preserving each patient’s independence, well-being and dignity. performed in order to ensure there is not any intra anal pathology causing the symptoms such as hemorrhoids, warts, a polyp or tumor. Considering the serious diseases that may cause the symptoms of pruritus ani, it is very important not to self-diagnose, but rather to seek consultation with a physician. In certain situations, a small skin biopsy may need to be performed. How is Pruritus Ani treated? The first goal of treatment of pruritus ani is to make a correct diagnosis. That means determining if the condition is primary pruritus ani or secondary pruritus ani caused by a medical condition/disease. Once this has been accomplished and it is confirmed that the patient has primary pruritus ani, the goal of therapy is to restore clean, dry and intact perianal skin. This will help to alleviate the symptom of itching. The first step is to alter the consistency of the stool so as to avoid seepage of stool from the anus. This may be accomplished with diet modification and adding a fiber supplement. The second step in treatment is to discontinue any offending agents such as cortisone ointments, hemorrhoid creams, lotions, perfumes, soaps and baby/toilet wipes. Topical agents containing alcohol are very irritating to the perianal skin. Likewise, any topical agent that causes the perianal skin to remain moist should be avoided. This includes Vaseline, witch hazel, A & D ointment, antibacterial ointments, etc. Additionally, it is important to remember not to over clean the perianal area. This causes further skin break down and irritation. Believe it or not, plain water is the best way to keep the perianal skin clean and healthy. Finally, the last step in treatment is to modify the diet to avoid a change in pH. Foods related to pruritus ani include: caffeine, colas, citrus, dairy, tomatoes, nuts and beer.

Where can you find help?

Dr. Paonessa is very familiar with diagnosing and treating pruritus ani as well as all the other diseases/conditions that may be mistaken for pruritus ani. Since there are many serious diseases that may be mistaken for primary pruritus ani, it is very important to see a physician trained in treating this condition. It is incredible how a fairly simple ailment such as pruritus ani can alter a patient’s daily life and affect his/her quality of life. Don’t hesitate to pick up the phone and call for a consultation. Stop suffering and begin to live life again.

Less Painful Treatment Of Varicose Veins Is Possible!

ClariVein®- Safe, FDA Approved

v

ClariVein® patients experience up to 74% less pain than other minimally invasive peripheral vascular treatments. For this reason, Dr. Girija Surya and team recommended the ClariVein® treatment when it fits in with the patient’s course of treatment. The procedure’s advanced technology allows us to eliminate the problem vein without causing you pain, scarring or down time. Unlike older varicose vein procedures, it is a form of endovenous therapy termed MOCA- for mechanic- chemical ablation. This procedure uses a combination of mechanical “irritation” and sclerosing solution to seal the problem vein off permanently. What sets ClariVein® apart from other venous procedures, like Endovenous RF or Endovenous Laser, is that it does not utilize heat to treat the vein. The removal of heat from the procedure is a huge benefit, as there is no longer a need for tumescent anesthesia and the risk associated with heat therapies is no longer an issue. This shortens the time the procedure takes, reduces pain and also eliminates bruising. Benefits of ClariVein® • Reduced Pain • Reduced Recovery Time • Short Treatment Time • No Anesthesia Required

Joining the program is Gail Maheshwari Cristelli

Yoga University

Tuesday, August 1, 2017 • 6-8 PM • $25 As you explore the chakra system, you reach a greater understanding about yourself and your daily practice. The chakras create a system of energy that keeps our body, mind, and spirit healthy. It is important to practice self-care to promote balance within this system. You are expending and absorbing energy every time you practice. Many things have an effect on the system: emotional upheaval, illness, neglect of the system, and programming. Harmonizing the chakras will help you balance the flow of energy in your system and will make the overall experience of your practice more enjoyable. In this workshop you will learn how to address imbalances and realign the chakra system through creative integration of asana, pranayama, meditation and mantra. Prepare to leave class with a personal “toolkit” of ideas and inspirations that can easily be incorporated into your life both on and off the mat. Please bring a journal with you.

PACE Program

Beacon Of Life: A Program Of All-Inclusive Care For The Elderly Imagine a place where your mom or dad’s entire health care needs comes from ONE location. A place where doctors, nurses, therapists and other health care professionals work together and partner with families and care givers to deliver exceptional care to the elderly population who, without the Program of All-Inclusive Care for the elderly (PACE), might need nursing home care. Beacon of LIFE provides an alternative to nursing home placement. This option can dramatically improve quality of life while allowing elderly individuals to stay living safely and independently in their homes with PACE services. The Beacon of LIFE program is open to seniors age 55 and older who live in the Monmouth County PACE service area, are certified by the state to need nursing home-level care and able to live safely at home with the support of PACE services. Susan Skola, PACE provides a team of health care professionals, who are Executive Director experienced in caring for the elderly. At Beacon of LIFE we are committed to helping our participants and their families make health care decisions that are meaningful for them. Beacon of LIFE offers Medicare Part D prescription drug coverage, if eligible. In addition, Beacon of LIFE will support family members and other caregivers with caregiving training, support groups and respite care to help families keep their loved ones in the community. Care and services are provided at the PACE center, home or an alternate setting based on the needs of the individual. All PACE centers meet state and federal safety requirements and include adult day programs, medical clinics, personal care, activities, meals, and occupational and physical therapy facilities. Most importantly, the PACE program becomes a second family to many seniors with limited support and resources. An interdisciplinary team (IDT) provides highly coordinated care for each PACE participant. This program is available at no cost to eligible participants that have Medicare and / or Medicaid. Private pay is an option for eligible participants that do not have Medicaid. Very similar to nursing homes, PACE tends to attract participants who are older and have very high care needs. Most PACE participants live alone in the community (only seven percent live in nursing homes). A participant in PACE can stay in the program through the end of their life unless they choose to dis-enroll. The care of the interdisciplinary team follows the participants through all care settings, including hospital, rehabilitation, and nursing home stays. Beacon of LIFE PACE reduces hospital and nursing home use for two reasons: PACE emphasis on preventative care and regular monitoring by an interdisciplinary care team (IDT). PACE’s capitated financing creates incentives to reduce the total cost of care. However, the main focus of the PACE program is to help seniors live safely in the community for as long as possible.

PACE STATS: • The first PACE site opened in 1971 in San Francisco • 122 sponsoring organizations operate with 233 PACE centers across the country in 32 states. • PACE services over 40,000 Seniors Nationwide. • The average age of a participant is 80-years-old

As she grew older, she developed a passion in the creative aspect of baking and cake decorating, which led her to the Connecticut Culinary Institute. She graduated with a degree in baking and pastry arts and began working in the culinary world where she gained a great deal of experience perfecting her craft. Prior to joining the team at Chocolate Carousel, she worked in the industry creating high-end custom cake designs and has appeared on Food Network cake challenges. Recently, Liz joined Chocolate Carousel as the Executive Assistant/Custom Cake Designer. She enjoys working with a company that challenges her and expects her vey best. Chocolate Carousel’s commitment to excellent quality and customer service attracted her to their organization. She loves working with the other team members and having the ability to use her creativity everyday.

Jennifer Cardella Jennifer Cardella started as an intern at Chocolate Carousel in 2009. She graduated from Southern New Hampshire University in 2010 with an associate’s degree in Baking and Pastry. She was honored to receive the “Baker of the Year” award at her graduation. Shortly after graduating she joined the team at Chocolate Carousel. She loves putting her twist on recipes, making them her own. Over the years she worked hard to grow within the company to her current role as lead Pastry Chef. Equal to her love of baking, Jen enjoys decorating and is an accomplished wedding cake designer. Having started her career at a young age, Jen is looking forward to a bright future as a pastry chef.

Nicole Iuzzolino Born and raised in Central NJ, Nicole Iuzzolino’s interest in the culinary field started back when she was in middle school and discovered the Food Network. She was inspired to take a cake-decorating course in high school and quickly realized her passion for pastry. Nicole graduated from Johnson & Wales, Providence, RI campus in 2008 completing the Baking and Pastry Arts Program with an Associate in Science. Upon graduation, she was offered a cake decorator position at a bakery in her hometown. Nicole is thrilled to be part of the design team at Chocolate Carousel. She has had the opportunity to expand the scope of her work, use her creativity in decorating a variety of cakes, and work with brides to help them create their dream wedding cakes.

Lauren White At a very early age, Lauren discovered her love and talent for baking, and the kitchen quickly became a place of comfort. As the middle child between her two sisters, Lauren’s baking quickly became one of her many endearing qualities to her family, who have always been her most eager taste-testers. When it was time for her to pursue college and start a career path, she had no hesitation in chasing her dream of turning her baking hobby into a rewarding career she would love. Lauren studied Baking and Pastry at Johnson & Wales University and received her degree in 2015. Upon graduation Lauren joined the team at Chocolate Carousel as a cake decorator and chocolatier. Lauren pursued her dream and couldn’t be happier having the opportunity to use her talents and creativity everyday.

Ami Gravina Ami Gravina went to the University of Cardiff, Wales and obtained a Bachelors degree in Accounting and Management. Her love of baking and cake decoration stemmed from baking for family and her children’s birthday cakes. The elaborate and delicious cakes Ami would make for the holidays and parties always created quite a stir. She was soon designing cakes for friends, family and neighbors for all occasions. After training in pastry and cake design and many years of experience she joined the Chocolate Carousel team in 2016 where she specializes in carved and fondant cakes, figures and gum paste flowers. She loves to try new mediums to create beautiful cakes, but loves the look of a classically elegant cake. With an eye for aesthetics and attention to detail, Ami uses her skills to carefully create handcrafted cakes with amazing creativity.

Fun Time At The Shore – It’s Summer Time – Not Slammer Time!

A

Criminal Charges – How We Can Help You!

s summer is underway, thousands upon thousands flock to the Jersey Shore in Ocean, Monmouth and Atlantic Counties. While we all endeavor to have fun, a peaceful time, certain things can lead to involvement with the criminal justice system. Although sunbathing and cookouts are prevalent on the beaches, use and misuse of alcohol and other substances can lead to criminal charges. This article serves to illustrate in a very basic fashion many common charges that people unfortunately encounter during the summer months at the shore and how EFC LAW PRACTICE can help you regarding these types of legal matters. One must first understand how the state criminal justice system works in its simplest forms. The criminal code known as Title 2C provides a framework for criminal charges that are brought on by state, county or municipal law enforcement. Charges are classified as indictable crimes (which the lay person knows as a felony) and petty disorderly persons / disorderly persons offenses (which the lay person knows as a misdemeanor). There are also municipal ordinance violations which fall into neither category and, of course, motor vehicle violations such as speeding, driving while intoxicated and careless driving. All of these categories have statutory penalties that may include imprisonment, serious fines, loss of driving privileges, probation and subsequent consequences such as immigration issues, loss of security clearance, voting ability, jury service and a multitude of other things. But there is help – utilize our services by calling for a free consultation to see what defenses, both legal and factual – can assist you. Indictable crimes are those where a punishment in excess of six months is authorized. They are categorized as first, second, third or fourth degree crimes. Generally speaking, first degree carries a term of imprisonment from 10-20 years, second degree is 5-10 years, third degree is 3-5 years and fourth degree is up to 1.5 years. A petty disorderly persons offense carries a maximum punishment of 30 days in jail a disorderly persons offense carries a maximum punishment of no more than 180 days in jail. Municipal ordinances carry a maximum punish of up to 90 days in jail, up to 90 days of community service, up to a $2,000 fine or a combination of all three. The most common charges seen at the shore include the following: disorderly conduct, domestic violence, criminal mischief, public urination, simple assault, open container of alcohol, shoplifting, defiant trespass, possession of controlled dangerous substances, possession of marijuana under 50 grams, possession of drug paraphernalia, noise violations, possession of fireworks and

a host of many others. Usually, if the charge is an indictable crime, it will be handled by the Attorney General or County Prosecutor’s office of the municipality where the incident occurred. If it is a petty disorderly persons, disorderly persons, ordinance violation or motor vehicle summons, the municipality where the incident occurred and their municipal court will handle it. If it is a combination of both types, the Superior Court handles the indictable crime and takes hold of the offenses as well to help ensure uniformity. Tragically, a combination of large crowds, the summer heat, abuse of alcohol or other substances and familial issues can spell a recipe for disaster that can lead a law abiding citizen into the throes of our state criminal justice system. For younger adults and teens in school, this experience may also have ramifications within their own school regarding administrative issues. When the person is charged, they are supposed to get served with a criminal complaint that usually contains an S (Summons – released on their own recognizance) or a W (Warrant – generally taken to jail and now subject to the Criminal Justice Bail Reform effective January 1, 2017). It is imperative that either way you protect your rights by contacting us immediately. The benefit of sooner rather than later is that the incident is fresh in your memory, we can, if appropriate, obtain a defense investigator to gather statements from witnesses or alleged victims before the prosecuting agency can speak with them and we can notify law enforcement and the prosecuting agency that we represent you and you wish to assert your 5TH Amendment right to remain silent – thus preventing the former from contacting you anymore. The above applies to all adults age 18 and over and likewise to in state residents, tourists visiting from other countries and out of state residents. Additionally, if the above type matters are committed by a juvenile (under age 18), the New Jersey Superior Court, Chancery Division, Family Part will hear these matters. The difference being that the juvenile court considers rehabilitation first and punishment second and the adult court considers punishment first. Use our experience of almost 12 years of exclusive criminal law practice, of which 10 years as a former assistant prosecutor and now serving as defense counsel to assist you in this troubling time. We handle all aspects of state criminal law, starting with arrest to plea, trial, sentencing, appeal and every other type of matter in between. As Britain’s Prime Minister William E Gladstone stated, “justice delayed is justice denied.” Do not sleep on your rights, especially when it includes loss of liberty!

Red Cross: Stay Safe In and Around the Water This Summer Whether you’re heading out of town or enjoying vacations at the Jersey Shore this summer, many families will be taking to the water. Every day, about ten people die from unintentional drowning, according to the Centers for Disease Control and Prevention. It is the leading cause of injury deaths for 1-4 year-old children. The American Red Cross is stressing the importance of constant, active adult supervision and ensuring that everyone learns to swim as critical layers of protection to help prevent drowning.

WATER COMPETENCY According to a Red Cross survey, more than half of all Americans (54 percent) either can’t swim or don’t have all of the basic swimming skills. The Red Cross and its Scientific Advisory Council recommend that everyone learn to perform critical water safety skills, referred to as ‘water competency.’ The skills, performed sequentially, include being able to: 1. Step or jump into the water over your head; 2. Return to the surface and float or tread water for one minute; 3. Rotate in a full circle and find an exit; 4. Swim 25 yards to the exit; and 5. Exit from the water. If in a pool, be able to exit without using the ladder.

Red Cross swim lessons help people develop skills and water safety behaviors that help them be more comfortable and safe when they are in, on and around the water. People can find age-appropriate water orientation and Learn-to-Swim programs for themselves and their family members by contacting their local aquatic facility and asking for American Red Cross swimming and water safety programs or by visiting redcross.org/takeaclass.

Mother Theresa On May 13, 2017, my family celebrated my grandmother’s 100th birthday. The celebration was attended by family, friends and community members. There were two Proclamations presented to her from both the Township of Mantua and Gloucester County. It was truly an amazing experience. I never dreamed that I would have had a grandparent live to be 100 years old. I recall as a child thinking that my grandparents were “so” old and yet they were only in their 50s and 60s at the time. This feature is a historical snapshot of the life lived by an amazing woman who continues to touch the lives of many in her own special way even at the age of 100. Theresa DiBernardi was born in Philadelphia, Pennsylvania on May 13, 1917. Theresa was one of five children. Theresa’s parents (my great-grandparents) arrived to the United States from Italy and landed in South Philadelphia. Theresa’s father died from TB in his 40s and Theresa’s mother died from breast cancer when Theresa was only 16 years old. After Theresa’s mother became ill, Theresa went to work in an attempt to support her siblings. Theresa went to work at a “tailor” shop called Goodi Mate, where she met the love of her life, Gene Concordia. Gene (otherwise known as my grandfather) was a true old-fashioned tailor. He went to design school and had the ability to hand-make suits, pants, and anything you wanted. Theresa and Gene married in 1935. After their marriage, Theresa and Gene took in Theresa’s siblings and they all lived together until one by one all of the siblings got married and moved out of the family home. Theresa had one sister who was only 10 years old when their mother had passed away. Theresa raised her siblings; however, “Mary” remained with Theresa until after she was married and ended up purchasing her first home a few houses away. Theresa and Gene had five children of their own: Vera, Louis, Ronald, Gene and Johnny. Theresa and Gene raised their children in Mantua, New Jersey, where they lived together for 49 years in a three-bedroom, 1½-bath home. Theresa had remained in that same house until recently, when her age left her unable to manage her split-level home independently. Theresa also did not drive until much later in life, and she was driving well into her late 80s back and forth to the local market. Theresa and Gene were active members of the local Catholic Church, where she attended weekly Mass services with the entire family. I recall as a young child attending services with my father’s entire family on Saturday afternoons or Sunday mornings and then getting together for family dinners after church. Sunday dinners at my grandmother Theresa’s house were always an event, the grown-ups in the dining room and the children in the kitchen. I could not wait to graduate to the dining room! Although I must say I preferred the kitchen after I was privileged enough to sit at the dining room table. Everyone contributed to some aspect of the family meal. If my grandmother Theresa was making homemade raviolis, everyone was at the table prepping the table with flour, stuffing ricotta cheese and cutting the edges of those delicious cheese squares! My grandmother was the matriarch of the family unit. As mentioned above, Theresa and Mary had a very close relationship. Although they were only six years apart, Theresa was a mother to Mary. They did everything together and even

worked at the same candy factory until Theresa had her youngest son. Mary would eventually relocate to California to be close to her children and Theresa would visit with her. Being born in 1917 afforded Theresa many life experiences. Theresa was only three years old when the 19th Amendment was passed which paved the way for her to vote when she became of age. Imagine living in a world where you were told what to wear and not afforded the freedoms that we have today. Theresa has also lived through the Great Depression and World War II, through which she has seen family and friends recruited for active duty. One of Theresa’s sons also served in the military. Theresa managed a household in a time where food supplies were limited. Again, she was a young woman with stressors that most adults face today. In the late 1980s and early 1990s, when the economy was a bit more stable, Theresa and Gene would often take trips to Atlantic City. They enjoyed gambling and visiting their son who worked for the casino industry at the time. Theresa was introduced to many celebrities through the years that included Johnny Mathis, Tony Bennett and yes, even Donald Trump! Theresa had celebrated many good times with family and had also endured many unfortunate events throughout her life. She has not only had to say goodbye to her parents at a young age, my grandfather passed away in his early 70s; her son, who was in the prime of his life, passed away in his 40s; and last year her son-in-law, who proclaimed that she would outlive everyone in the family, passed away. Theresa has displayed amazing resilience in the face of darkness. Theresa’s remaining children continue to care for her and manage her older adult responsibilities. Theresa was a woman who managed a household through war, financial crisis, grief and at times loneliness and yet always had a cup of coffee ready for visitors. Theresa was an amazingly self-sufficient woman who was incredibly independent. Theresa currently resides in a nursing care facility. The current family totals include 16 grandchildren, 27 great-grandchildren and 9 great-great-grandchildren. Amazingly, she knows each and every name. Monmouth Film Festival Returns This Summer August 10-13, 2017 Prepare for another great season of top-quality, independent cinema in Monmouth County!

Business & Finance

Last December, Monmouth Film Festival (MFF) made its mark during its inaugural event at the Two River Theater in Red Bank – attracting audiences from across the tristate area; receiving rave reviews and top ratings. The festival and its programming team search endlessly to find the hidden gems amongst the film industry. As the President of the company states, “There are no favorites, no special treatments and no “good old boys club.” Every film gets the same fair opportunity to be chosen.” Film critic Joan Ellis of the Two River River Times wrote, “The production quality, scripting and acting of all the films was extremely high and the subject matter was refreshingly original.”

The festival is currently accepting submissions for its 2017 season, which will take place August 10 through August 13, 2017 at Two River Theater. Submission categories include: Feature, Short and Student films (all genres), Screenplays, Trailers, TV Pilots & Web Series. For more submission information, visit www. MonmouthFilmFestival .org/submissions. The festival will feature special events in August including the Industry Networking Event and Industry Film Panel- two MFF exclusive events aimed to promote, connect and educate current and aspiring artists. Last Year’s panel included esteemed guests from all corners of the industry including Producers, Distributors, Filmmakers/ Directors and more! For young and aspiring filmmakers, be sure to stay upto-date with the company for educational opportunities, including internships and workshops available in 2017.
