University Urology Associates Of New Jersey Parvez Mahmood, MD, FACS Dr. Mahmood has been in practice in Ocean County since 1974. He received his training in New York City at Mount Sinai Hospital Services, New York University Medical Center, Bellevue Hospital Center and Memorial Sloan Kettering Cancer Center. He has active clinical privileges at Community Medical Center in Toms River and Shore Outpatient Surgicenter in Lakewood. He is the former chief of urology at Community Medical Center in Toms River and Kimball Medical Center in Lakewood. Mukaram Gazi, MD, FACS Using the latest advancements in urology, Dr. Mukaram Gazi provides the highest-quality urologic care for his patients. In practice since 2001, this exceptionally skilled, minimally invasive surgeon treats the full range of urinary tract and male reproductive disorders- from no-scalpel vasectomies, erectile dysfunction and urinary incontinence to state-of-the-art robotic surgery for prostate cancer and enlarged prostate (BPH). Alexander Gotesman, MD Dr. Gotesman completed medical school at State University of New York in Stony Brook and completed his urologic residency training program at Mount Sinai Medical Center in New York in 2005. Dr. Gotesman specializes in adult and pediatric ambulatory urology. He has five years of postgraduate clinical research experience and is a member of the Alpha Omega Alpha medical honor society. Dr. Gotesman maintains strong affiliations with local and national medical associations including the American Urological Association and the American Medical Association. He joined UUANJ in 2012.
Michael Nazmy, Jr., MD Dr. Nazmy received his medical degree from UMDNJ/Rutgers Medical School in Newark, New Jersey where he subsequently completed his residency in Urology. Afte residency, he completed a two-year fellowship in Urologic Oncology and Advanced LaparoscopicRobotic Surgery at the prestigious City of Hope National Medical Center in Duarte, California. Dr. Nazmy is Board Certified by the American Board of Urology and sees patients at all three of our locations. Welcome to the Fall Issue Of Monmouth County Woman! It was a summer full of sunny weekends for enjoying the Jersey Shore with family and friends. Now, as this sizzling season draws to a close, we look forward to two special holidays. On September 7th, we celebrate Labor Day to honor the achievements of American workers. Then on September 13th, those of us who are lucky enough to be grandparents celebrate National Grandparents Day, a day to share our love and strength with our children’s children. Although Labor Day signals the “unofficial” end of summer, the “official” first day of fall isn’t until September 23rd. This wonderful season has always been my favorite. I look forward to the lower temperatures and cool, crisp air. The trees will become an orchestra of colors and fall activities will be in full swing along the New Jersey beaches and towns. The September/October issue is an essential resource publication for many different reasons. As with past issues, this one is brimming with useful educational information for you and your families in the areas of medicine, health and wellness, legal issues, fashion and education. With subjects ranging from the benefits of chiropractic and rehabilitation to advice about tutoring, there’s sure to be lots of information here that you’ll find invaluable. On a personal note, I myself have followed the advice of many of the professionals who have written for this publication, and I’ve experienced much success in improving my health. In addition, I have mailed and emailed several articles to my family and friends in Minnesota, Wisconsin, California, Florida, and Illinois. They were also able to use the knowledge in the articles to improve their situations. I know you will experience the same level of success as I have when visiting and working with these professionals. Their knowledge is priceless and wonderful to share! In addition, I have had the privilege of promoting many of the events that help our local non-profits. Attending these events is a win-win situation. You’ll have a great time while helping out a wonderful organization! Please note the recap of events below. If you already have a good consistent strength program, be sure to change it up and challenge yourself differently. It is always important to do this, but especially as you enter into this stage of life. Your body will thank you and reward you by staying much more like the pre-menopausal body you’re used to. There are enough other surprises to experience during menopause, but at least strength training gives you some control over some of them. If you are just beginning a program and especially if you’re post- menopausal and have never lifted a weight in your life, I urge you to get a trainer and learn the proper way. You are much more likely to hurt yourself if you don’t. Additionally a trainer will keep you motivated and inspired. If you have a trainer that is not doing that for you then find a new trainer. If you’ve been overweight even before menopause, then trying to lose weight at this time can be particularly difficult, if not impossible without adding some training into your life. Also, strength training will allow you to keep the weight off much more easily. If you’ve gained weight since menopause then the addition of a strength program will be instrumental in helping you to lose that weight and never find it again. Of course proper diet is essential as well, but even with a great diet, you may still experience the menopausal redistribution if you’re not maintaining your muscle. Good luck to all those women that have entered into this stage. There are some good things about it too, but if you’re trying to keep that girlish figure, now you know the secret!

Roasted Butternut Squash and Spinach Soup Submitted by Marcie Seninsky, Diet Coach/Manager at Balanced Life Weight Loss Institute Ingredients - Serves 4
1 ½ Tblsp. balsamic vinegar 1 medium-large butternut squash, halved lengthwise 1 Tblsp. olive oil 2 cloves minced garlic 1 lb. fresh or frozen, defrosted, drained spinach 15.5 oz. can cannellini beans (white kidney), drained 7 cups low sodium chicken or vegetable broth Salt & pepper to taste ¼ tsp. nutmeg ¼ tsp. cayenne pepper ½ tsp. pumpkin pie spice ½ tsp. smoked paprika 4 Tablespoons real bacon bits to garnish
• Pre-heat oven to 425. • Spread balsamic vinegar on a baking sheet and lay squash cut-side down on vinegar. • Roast 20-25 minutes until very soft. • While squash is roasting, put olive oil in a saute pan and saute the garlic till fragrant. • Add the spinach, beans, and 1 1/2 cups of the broth. • Cook it down until there is no liquid and then add salt and pepper to taste. • When squah is cool, remove and dispose the seeds, scoop ot the flesh, and measure 2 1/2 cups into a large stock pot, on medium. • Add the remaining 5 1/2 cups of broth, nutmeg, cayenne pepper, pumpkin pie spice and smoked paprika and bring to a slow boil. • Blend until smooth and return to the stock pot. Add the spinach bean mixture and stir thru. • Spoon into soup bowls and top with one tablespoon of real bacon bits. Enjoy!

As a physical therapist, I am always a strong advocate for the benefit patients can gain from rehabilitation. This is especially true for individuals going through breast surgery and cancer-related treatment. Whether a patient has experienced a double mastectomy, lumpectomy, reconstructive surgery, or a less invasive procedure, physical therapy is essential to a faster and more functional recovery. Patients who have undergone any type of breast surgery may be at risk for a variety of complications, including scar tissue development, limited range of motion and weakness in the upper extremity, postural impairments, lymphedema, and nerve entrapment. Due to the fragile nature of breast tissue and the extensive muscular and neural structure surrounding the breast, patients often experience one if not many of these issues. The great news is that physical therapy can help! Postural impairments are one of the most common problems that patients experience following breast surgery. The nerves and muscles in the breast region can be affected during a surgical procedure, causing weakness, tightness and poor nerve innervation to the muscles that help maintain posture. As a result, patients may experience pain and impaired movement related to scapular winging, frozen shoulder, shoulder impingement, rotator cuff tendonitis, and poor body mechanics. Physical therapy helps to address these issues with postural exercises, extensive stretching techniques, proper undergarment fitting, soft tissue mobilization, and retraining of movement patterns, as well as a home exercise program for long-term relief. Scar tissue development and poor healing of soft tissue structures are also common side effects of breast surgeries. The skin surrounding the breast can become fibrotic or thick and hard causing scar regions to be inflamed and sensitive. Secondary to the trauma to the breast during surgery, abnormal bundling of different types of nerve, lymphatic, and blood vessels can cause a syndrome called cording. This can limit range of motion and make activities of daily living such as wearing undergarments, reaching, getting dressed, bathing and overall self-care difficult and painful. Therapists help to alleviate these symptoms by performing specific types of soft tissue mobilization, stretching, and teaching self-management strategies to patients. Lymphedema, although less common, is another potential problem for patients undergoing breast surgeries and cancer-related treatments. As a result of an insult to the lymphatic system, abnormal swelling can occur in the breast, as well as in the arms and hands which can feel fatigued, heavy and weak. Certified lymphedema specialists use techniques including manual lymph drainage and a series of bandaging which are considered the gold standard of treatment to improve the swelling. While these impairments and limitations that many patients experience following breast surgery can be improved with specialized rehabilitation, the sooner the individual begins a therapy program, the greater improvement he or she will likely achieve. Being aware of these challenges and being motivated to undertake the journey to recovery are key. The patient and her or his therapist can work together to determine the best strategies to improve function and quality of life following breast surgeries and cancerrelated treatment.

f you are thinking about divorce or are concerned that your marriage may be ending there are several steps that you should take before you make that announcement. Once you are divorced you will be required to support yourself with whatever alimony and/ or child support that you may receive. You will be required to obtain your own health insurance and also pay for your own car insurance. If you want to keep the marital home, you may need to refinance it in your own name. The following steps will help you in preparing to file for divorce:
1. Locate all of your financial documents. Figure out what assets you and your spouse own and what debts you and your spouse are responsible for. Locate bank statements, 401K statements, stock account statements, deeds, title to the vehicles, etc. Also obtain copies of all bills, such as the mortgage statement, home equity loan, real estate tax statements, utility statements, automobile payment statement and all credit card statements. Try to determine what your monthly expenses are and figure out a budget for yourself. Copy all of the documents above and put them in a safe place, such as a friend’s home or in a safe deposit box. 2. Locate your tax returns and determine what your spouse’s income is and what your income is. It is best if you have copies of tax returns for the past 5 years and all W-2s. Obtain copies of all of
the above documents and put them in a safe place. 3. Locate information on the health insurance plan of your spouse, home owner’s insurance and life insurance. Find out if you can remain on your spouse’s health insurance if you proceed with a “Divorce From Bed and Board”. Contact a health insurance provider to determine how much it will cost for you to obtain your own health insurance. Determine how much life insurance you and your spouse currently have. Obtain copies of all of the above documents and put them in a safe place. 4. If you have been out of the workforce, take a class or take action to improve your ability to return to your career. If you do not have a career or only have a high school diploma or limited college, look into options at the local community college. You

Freehold Yoga Center The Freehold Yoga Center has been a haven of peace, harmony and spirituality for Yoga students since it’s opening in 1988. Our center is steeped in yogic tradition and encompasses two floors in a historic building in downtown Freehold. Linda Bennett, also known as Yogini, has dedicated her talents and service to the Freehold
Yoga Center for close to 14 years. She completed the 200-hour teacher training in the Sivananda Style with the founders of the FYC program, Omkar, Radha and Devaki. She continues her studies at the Himalayan Institute in Honesdale, Pa., completing a 500-hour certification, as well as others. “Yoga is a practice, a way of joining mind, body and spirit and is much more than physical. I have studied with many different teachers and will continue to do so. We will not be effective teachers if we are not practicing our own yoga. Personal practice is the soil from which we teach.” Her strengths are in the classroom sharing the yoga that has brought healing into her own life. Yogini’s classes are a blend of breath, alignment, and anatomical awareness helping to guide her students to a deeper understanding of themselves. She teaches all levels from beginners to advanced, as well as restorative yoga classes. She has been a facilitator of the Teacher Training program since 2007. Dense Breasts: What Does This Mean? A sad fact is one in eight women will develop breast cancer in their lifetimes. Digital mammogram remains the most effective screening tool we have to detect this disease. Part of the mammogram evaluation by the radiologist is describing the density of a woman’s breast. In Februrary 2014, Governor Chris Christie signed a law requiring radiologists to inform women about their breast density when they have such tests. Many women are finding language in their mammography letters saying they may have dense breast tissue. As a result there has been many questions about what breast density means and how it impacts a woman’s health. Here are some of the more common questions I have been asked about this issue.

What is breast density? Breasts are made up of a mixture of fibrous tissue, glandular tissue, and fatty tissue. The glandular tissue is what produces milk. The fibrous and fatty tissue hold the glandular tissue in place, and they give breasts their shape. Your breasts are considered dense if you have a lot of fibrous or glandular tissue but not much fat. Density tends to decrease with age, especially after menopause.

Are there any other tests I can do?
Dedicated breast ultrasounds in poorly seen or suspicious areas of the mammogram are among the most common and effective secondary tests performed. In my opinion, monthly self breast exams are an excellent way for a woman to take control of her breast health. It is important to become familiar with the way your breasts normally look and feel, and you can notice changes in the look or feel of your breasts. MRI is very sensitive in detecting cancer; however, often it can detect lesions that are not cancer and this can lead to unnecessary biopies. Additionally, not all insurance policies will cover the cost of having a breast MRI. Tomosynthesis (also known as 3-D mammography) is gaining popularity, but it is still uses X-rays like a traditional mammogram and will still have limitations for women with dense breasts.
Dense breasts make it more difficult for doctors to spot cancer on mammograms. The dense breast tissue blocks the x-ray beams and therefore dense tissue appears white on a mammogram. Unfortunately, cancer also appears white on mammograms. Trying to find cancer in very dense breast tissue can be like looking for a snowball in a snowbank. The sensitivity of breast cancer detection in mammograms of women with dense breasts may decrease by 10-20%. Breast cancer develops in the glandular and fibrous parts of the breast. What Is Celiac Disease? How is celiac disease treated?

Who can have celiac disease?

eliac disease is a condition in which the body has an immune reaction to a protein called gluten. Gluten is found in wheat, rye, barley, and many prepared and packaged foods. When someone with celiac disease ingests gluten, the immune response causes inflammation that can damage the lining of the small intestine. Since the small intestine is responsible for absorbing nutrients in our diet, damage to its lining can cause malabsorption.
What are the symptoms of celiac disease?
The symptoms of celiac can be different in each person. In mild disease, one may not have any symptoms at all. The most common digestive symptoms include diarrhea, weight loss, abdominal pain, bloating, and excessive gas. However a small percentage of celiac patients may actually have constipation or even be obese. Since the disease leads to a decreased absorption of nutrients and vitamins, patients could suffer from iron deficiency which can result in anemia (low blood count). Patients can also develop osteopenia and osteoporosis
Anyone can have celiac disease including children and adults. It is estimated that 1 in 141 people in the US have celiac disease and it is most common in Caucasians. It also tends to occur with greater frequency in people who have family members with celiac, those who have type 1 diabetes, autoimmune thyroid disorders, and a skin disease called dermatitis herpetiformis.
How is celiac disease diagnosed?

The treatment for celiac disease consists of a complete gluten free diet. In addition to foods, gluten can be found in packaged goods, medications, supplements, and even certain make up products. Some common foods which needed to be avoided include bread, pasta, pastries, cereal, beer, and many sauces and condiments. Foods which don’t include gluten are rice, corn, potatoes, soybeans, fruits and vegetables, gluten free breads and pasta, as well as wine and distilled alcoholic drinks. Many grocery stores in the area have sections devoted to gluten free products. In fact, many vacation spots and cruises will offer gluten free menus. It is best to consult with a dietician that specializes in understanding a gluten free diet and lifestyle. Your doctor can monitor the response to treatment by following antibody levels. These will usually normalize within 6 months to a year. You may also be prescribed vitamins to help treat any nutritional deficiencies. About 10 percent of people may have symptoms despite following the diet carefully. While other conditions may contribute to ongoing symptoms, the most common culprit is inadvertent ingestion of gluten. A small percentage of people can have refractory celiac disease,
If you feel you may have celiac disease, it is important to see a doctor before eliminating gluten from the diet. Initial testing for celiac includes a blood test which looks for high antibody levels to gluten. These include tissue transglutaminase antibody and endomysial antibody. Over 90 percent of people with untreated celiac disease have increased levels in the blood. Before having these tests, you should continue to eat a normal diet that includes gluten. If these antibodies are elevated, you will need an upper endoscopy. During this procedure, the gastroenterologist will examine the small bowel lining with a flexible instrument that has a camera on the end. A tissue sample, or biopsy, will be taken to assess for damage under a microscope. If the diagnosis of celiac is confirmed, you will be tested for many nutritional deficiencies including iron, folic acid, vitamin B12, Services: and vitamin D. What are complications of untreated or undiagnosed celiac disease?
In addition to having ongoing gastrointestinal symptoms, patients with untreated celiac disease can have malnutrition. Deficiencies of important minerals and nutrients can lead to changes in hair, skin, or nails, ongoing diarrhea, and even changes in taste or smell. Malabsorption of calcium and vitamin D can lead to issues with fertility and miscarriage.
Some people also experience lactose intolerance. Another dreaded but rare complication of untreated celiac is small intestinal cancer (lymphoma). In children, undiagnosed celiac disease can lead to diarrhea, short stature, delayed puberty, and certain neurological conditions. It is essential to meet with your doctor regularly and undergo monitoring of antibody levels and any nutritional deficiencies. While embarking on a gluten free diet can seem like a daunting task, there are many resources available to patients to make the journey easier and manageable.

So I Have DVT? (Deep Venous Thrombosis) Part II Girija Surya, M.D.

This is part 2 on DVT, where different interventional treatments are discussed. These are important in preventing POST THROMBOTIC SYNDROMEPTS, a condition with chronic swelling of leg affected by unresolved DVT, with skin changes and ulceration in the leg. In the first part of this article we discussed acute DVT (deep venous thrombosis) and the guidelines that physicians use in prescribing the various methods of prevention. There are different medical treatment options with intravenously administered heparin or subcutaneously injected Lovenox (low molecular weight herapin). There are also other, newer oral medications now used instead of Coumadin with results that are just as effective and without the patient requiring repeated blood tests. Today, we will be looking at interventional treatment of acute and chronic DVT.

What is acute DVT?
When a patient is diagnosed with DVT, they are treated with intravenous heparin as a continuous drip as a blood thinner, followed by an oral blood thinner agent such as Coumadin. Sometimes the patient is treated with a subcutaneous injection of heparin followed by oral blood thinners. The leg or arm where the DVT was diagnosed is followed by serial ultrasounds to make sure the blood clot blocking the vein clears in response to the treatment.

a method that breaks up the clot in DVT, is now used and the patient with DVT is placed on IV heparin and brought to the cath lab. While there, a catheter is placed into the vein with the thrombus (clot) under ultrasound, and intravenous tissue plasminogen activator (TPA) is delivered into the clot for 24 hours. During this time the patient is monitored in the intensive care unit. After that time, the patient is returned to the cath lab for a repeat venogram, where a small amount of dye is injected to visualize the vein and evaluate the reopening of the occluded vein.

When the clot is in the iliac vein, only 15-30% of veins recanalize with standard anticoagulation alone, as opposed to the popliteal vein, where recanalization takes place in up to 90% of cases with standard treatment. In the iliac vein, the clot remaining in the vein transforms into fibrous tissue between three to six months, and up to 1-year collaterals develops. This fibrous tissue into the vein blocks venous outflow and is thought to be the major cause of PTS. So the aim of thrombolyticvtherapy is to resolve the thrombus (clot) in the acute place.

There are new modalities that combine the mechanical element with the chemical method (anticoagulation). The mechanical element is supposed to speed up the process of thrombolysis by disrupting the thrombus. There are 3 types of catheters available: 1. The EKOS EndoWave system where the catheter uses ultrasound waves to break up the clot.
between 2 balloons, and after mechanical and chemical thrombolysis using a sinusoidal wire that rotates and injecting blood thinners, the solution of the broken-up clot and the blood thinner is suctioned. Since the blood thinner is isolated in the segment that is closed on both ends, systemic spread of blood thinners is minimal, reducing the risk of bleeding. 3. The AngioJet power pulse system uses a catheter inserted into the thrombus, and using a complex mixture of rapid fluid streaming and hydrodynamic forces, the thrombus is broken up, and through the catheter tip is suctioned out. The evidence of these devices’ effectiveness is largely derived from retrospective studies. Purely mechanical devices, which remove the clot and do not use tPA or urokinase blood thinners, are also available. These are known as the Lazarus device and AngioVac. There are some drawbacks with these devices and details should be discussed with your physician to understand them better.

Spinal Cord Stimulators May Get You Back on Your Feet Again
D
r. Mariam Ghobriel, Chair of the Division of Pain Management at CentraState Medical Center uses Spinal Cord Stimulators for her patients suffering from spinal stenosis. However, there are other candidates for this treatment and in many cases it can be very effective for the treatment of the following: • Those suffering from back and leg pain. • Patients with degenerative disc disease who are not good surgical candidates, or who have failed surgical correction. • Diabetic neuropathy or neuropathy caused by chemotherapy .
Ghobriel adds, “This procedure is effective for patients with the above diagnoses. They often report at least a 50% reduction of the pain immediately after the procedure and up to 80% in a lot of cases. Patients are able to manipulate the device to where they cover different parts of their bodies at different times. This gives them control over flare ups, fluctuations and changes in the intensity or distribution of painful areas.” Dr. Ghobriel will typically have the patient undergo a trial phase, what she likes to call a “test drive.” She adds, “The patient receives temporary leads through a needle puncture under X-ray guidance. It feels just like having an epidural injection, but instead of injecting medications, we pass a lead around the spine. This is connected to an external device that the patient wears for a few days.” Patients leave her office with surgical tape on their back and an external controller that they are trained to use. It doesn’t require much more skill than basic reading and the ability to follow simple instructions. Dr. Ghobriel compares it to operating a cell phone. Once you’re home that day you will have a chance over the next
3-5 days to try it out and see if it can adequately manage your pain. After that time, a follow up visit to her office is required where the lead will be removed. This procedure is similar to having an IV catheter removed. She will then read the device and look at your usage. Based on whether you liked it or not and if it helped your pain, the decision would be made to proceed to the permanent implant which is also a simple same day procedure. It takes just under two hours to complete the whole process in both steps for most patients. In fact, there are very few restrictions and zero recovery time for the procedure itself. “The recovery time is only for the sedation to wear off which is the standard 24 hours in most people,” she adds.

Of course, in some cases the procedure may not be effective and Dr. Ghobriel is quick to mention that it will not work for everyone. That is why the trial is so critical. It’s not for everyone but for those who are candidates, it can be the difference between being able to get out of bed in the morning or not being able to care for yourself and having to depend on others.”

MCW: Tell us about your educational background.
Dr. Morgan: After completing my bachelor’s degree at Columbia University, I attended medical school at Far Eastern University in Manila while a Captain in the U.S. Army Reserves. I then did my residency in Obstetrics and Gynecology, both at Long Island Jewish Medical Center, New York, and Jersey City Medical Center. Finally, I completed my Fellowship in Reproductive Endocrinology and Infertility at The University of Kentucky.
MCW: What is the best part about working in this specialty? What do you find the most rewarding?
Dr. Morgan: Working with my team of nurses, embryologist, and technical staff to help my patients carry a pregnancy they thought would never happen is amazing. We all work together with one goal in mind; that couples’ future baby. The joy and happiness they receive is the best part. I love what I do from start to finish. Of course, it is the final product that is the most awesome part for me, especially when they come visit with their bundle of joy.
MCW: What is the most challenging part?
Dr. Morgan: The most difficult part of being a fertility specialist is to witness the frustration some couples experience. It seems unfair for these young healthy couples to have to go through something that is supposed to be natural. The truth is, science is not perfect and we can’t help everyone, but we’re getting better all the time.
MCW: Why do you think fertility testing is more common these days?
Dr. Morgan: Fertility testing is more common today for a number of reasons. First, the percentage of couples experiencing infertility has increased substantially in recent years. Today, 1 in 8 couples report requiring assistance to start a family. Of those couples, 90 percent report they wish they had started the fertility process sooner. Moreover, many experts believe that number is actually higher since many couples don’t report such issues due to the stigma surrounding infertility. Also, many young women are now choosing to freeze their eggs while they are young and healthy in order to pursue careers and become pregnant at a later time. So, there is a buzz out there about that.
MCW: How has your profession changed over the past 20 years?

Dr. Morgan: Pregnancy rates with assistance have improved dramatically. Many more can achieve their dream today than ever before. In terms of technology, we now have a safer and more effective egg retrieval process, a new and better embryo culture and freezing technique, and a process called ICSI, which revolutionized treatment for male factor infertility. This allows fertilization to take place even in cases of severe male factor infertility. The greatest improvement Dr. Morgan has seen in this field over the last 20 years is that of access to treatment. He adds, “When I started, there was no insurance coverage for infertility treatment. Then, several states recognized infertility as a disease and a disability, creating laws requiring insurance coverage for fertility treatment. One of those states is New Jersey. Increased insurance coverage equals increased access to care. That is why we accept ALL insurance whereas some other facilities do not.”
MCW: Who is a good candidate for fertility testing and why?
Dr. Morgan: A fertility specialist should test any woman who is under 30 and has been trying for one year or more right away. The longer a person waits, the more difficult treatment might become. If a woman is close to, or over 30 years old and has been trying to become pregnant for more than six months, she should make an appointment immediately. The health of a woman’s eggs deteriorates over time. Additionally, any couple with a background or family history of reproductive system issues should be tested as soon as they decide to have children. Many issues can be greatly minimized the earlier the problem is identified.
MCW: What are some suggestions for couples that are trying to conceive?

Dr. Morgan: Understand the female cycle and know when it’s best to conceive. There are lots of of smartphone APPS for that now. Men and women should stop smoking cigarettes, which is linked to low sperm counts and sluggish motility as well as double miscarriages rates, and it is toxic to eggs. Decrease your drinking, as alcohol can reduce the production of normally formed sperm needed for a successful pregnancy. Watch your weight. Both overweight and underweight men and women can have fertility problems. In women, it can cause ovulation problems. Exercise at least 3- 5 hours a week. Most fertility problems are easy to fix and do not require expensive treatments.

Diagnosed With Breast Cancer: What’s Next?
T
he biopsy shows that you have breast cancer. In addition to the shock and emotional turmoil, a thousand questions go through your mind.
determine the kind of treatment you may need. Women who have very early stage breast cancer may not need any treatment and should be observed periodically. Most women with ER and PR positive tumors are treated with Tamoxifen (before menopause), or Arimidex (Anastrazole) or Femara (Letrozole) (after menopause). In addition, your medical oncologist may advise you to get chemotherapy based on the other factors mentioned above. The most commonly used chemotherapy drugs are Adriamycin, Cytoxan, Paclitaxel (Taxol), Docetaxel (Taxotere), and 5 Flurouracil (5 FU). The type of chemotherapy and duration used are also based on the risk of metastasis. Chemotherapy is given for approximately 4 to 6 months, starting about 3 to 4 weeks after surgery. Chemotherapy is often administered as an intravenous infusion. To minimize damage to the veins and to ensure proper administration, a device called a port-a-cath can be inserted below the collarbone. This is temporary and can be removed after treatment is completed.

Not everyone diagnosed with breast cancer is treated the same because no two tumors are alike. Even after the lump in the breast is removed surgically, there is a risk of cancer growing back in the same breast or spreading to other organs, a process called metastasis. The risk of metastasis is based on a number of factors. Chemotherapy can cause nausea, fatigue, vomiting, hair loss, mouth sores, low blood counts and numbness in the extremities. These are all reversible and improve once treatment is discontinued. With the use of newer medications, nausea and vomiting can be prevented considerably.
In general, young women tend to have more aggressive tumors and are more often treated with chemotherapy than older women.
Stage
Breast cancer is staged from Stage 0 to Stage IV; Stage 0 is pre-cancerous and is also known as carcinoma in situ (DCIS or LCIS). Stage 0 cancer does not metastasize and does not require chemotherapy. Stage I is a small tumor (<2cm) confined to the breast; when the tumor has spread to the lymph nodes or if the tumor is >2cm it is Stage II. Stage III includes tumors that have spread to lymph nodes in the neck, have affected more than 4 lymph nodes, or have spread to the skin and chest wall. When the cancer has metastasized to other organs (commonly to the bones, lungs or liver), it is Stage IV. The higher the stage, the higher your risk is of relapsing; chemotherapy, hormonal therapy or both are recommended to reduce that risk.

Certain features on the cancer cells can predict the aggressiveness of the cancer. Cancer cells are graded from I to III, grade I being the least aggressive and grade III being the most. The tumor cells are also tested for hormone receptors on the surface of cancer cells, called ER and PR (estrogen and progesterone). Tumor cells are also checked for a protein called Her-2. About 25-30% of all breast cancers are Her-2 positive. Her-2 positive patients are treated with a drug called Herceptin.

~ Interview With Dr. Nina Paonessa ~ An Expert in Offering Concentrated Expertise in All Areas of Colon and Rectal Surgery As a board certified general surgeon and proctologist, Dr. Nina Paonessa believes in patientcentered care and preserving each patient’s independence, well-being and dignity. Specializing in colon and rectal surgery, Dr. Paonessa has been in practice for over 10 years. She completed her undergraduate education at Siena College in Loudonville, NY with a Bachelor of Science in Biology, Magna Cum Laude, and as a member of the honor society, Alpha Kappa Alpha. After completing her undergraduate studies, Dr. Paonessa completed medical school at Lake Erie College of Osteopathic Medicine in Erie, PA and completed her colon and rectal surgery fellowship at Lehigh Valley Hospital in Allentown, PA.
CW: When did you first decide to become a surgeon?

Dr. Paonessa: I discovered my love for the specialty during my second year of general surgery residency. I began to notice how patient outcomes varied based upon the training/skill of the surgeon. Colorectal surgery is a field in which specialized training truly impacts patient outcome. For example, in certain cases, the difference between patients having a permanent colostomy versus bowel continuity after colorectal resection is dependent upon the skill/training of the surgeon. Additionally, I had the privilege of doing a one-month rotation on the colorectal surgery service of Dr. Douglas Wong at Memorial Sloan Kettering Cancer Center in New York City. It was here that I truly fell in love the specialty of colorectal surgery.
CW: What do you like the most about being a colorectal surgeon?
Dr. Paonessa: For me, the most rewarding part is when I can make a positive impact on a patient’s quality of life and well-being, whether it be curing a patient of colon or rectal cancer and restoring bowel continuity, to ridding a patient of their hemorrhoid symptoms, to freeing a patient from the social isolation caused by their symptoms of fecal incontinence. A fringe benefit of my specialty is that I have the privilege of meeting so many wonderful and unique individuals. It is truly rewarding working with each of my patients.
CW: What is the most difficult part of being a colorectal surgeon?
Dr. Paonessa: Explaining to a patient about their diagnosis of cancer is the most difficult part of being a colorectal surgeon. Even in many cases in which the cancer can be cured, dealing with a diagnosis of cancer is stressful and at times, life-altering. I truly feel for the patients and try to make the difficult road ahead of them a bit easier.
CW: How would you describe your practice’s philosophy?
Dr. Paonessa: In my practice, my staff and I share the same philosophy of “the patient comes first.” No matter if it is benign or malignant disease for which a patient is coming to the office, we understand that it is a stressful and oftentimes an embarrassing situation for them. In my office, each patient’s privacy and dignity is preserved and respected to the utmost degree. We realize that just because we take care of colorectal problems every day, does not mean that patients are used to dealing with these issues.
CW: What are the most common reasons why a patient would schedule an appointment at your practice?
Dr. Paonessa: Men and women of all ages schedule appointments at my office for many reasons. These include hemorrhoids, fissures, colorectal cancer screenings via colonoscopy, treatment of colorectal cancers including laparoscopic surgery, squamous cell carcinoma of the anus, fecal incontinence and pelvic floor dysfunction.
CW: Tell us a success story that you have experienced over the years.
Dr. Paonessa: We have had many success stories over the past 10 years with patients. One of my favorite success stories is that of a woman who was hospitalized for perforated sigmoid diverticulitis with a large intra-abdominal abscess. I was requested to see her as a second opinion as the first surgeon proposed immediate surgery consisting of sigmoid colon resection and end colostomy. After careful history and physical exam and review of her CT scans, I proposed a plan of bowel rest, IV antibiotics and percutaneous drainage of the abscess. This allowed for the abscess to reduce in size to merely a small cavity. The patient was eventually taken to surgery, during which she had a sigmoid colon resection and colorectal anastomosis, without even a temporary stoma (bag). She recovered uneventfully and was able to return to work.

CW: Describe some of your favorite leisure activities.
Dr. Paonessa: Outside of being a surgeon, I enjoy spending my free time with my family. I also enjoy cooking and trying new recipes, reading, dancing and spending time at the beach.

The short answer… it depends. Because of the magnitude of ingredients that can go into a compounded prescription it is impossible to say until we submit a claim to the insurance company. However, most insurance companies will cover compounded medications. Once we have a prescription, we can submit a claim and then let the patient know what the out of pocket expense would be.

When a child hates the taste of their medicine, how can a compounding pharmacy help?
We can compound oral medications into pleasantly flavored suspensions, solutions, concentrates, freezer pops, ‘gummy bears’ or lozenges, in colors that entice the child to take the medication. A palatable formulation is more likely to improve compliance and minimize spillage or waste during administration. Lollipops are an ideal alternative to “swish and swallow” medications that need to be retained in the mouth for a prolonged period of time. Most drugs can be compounded into transdermal gels that can easily be applied to an appropriate site, such as the child’s wrist, for absorption through the skin.

The Pies Have It

W
hen it comes to fall desserts, the pie certainly takes center stage. Whether it be apple, cherry, pecan, or pumpkin, fall is pie season. Pies come in many varieties, shapes and sizes. They can have one crust or two, be topped with crumb or lattice dough, and filled with a variety of different fillings. Pie came to America with the first English settlers and quickly became one of the most traditional desserts in America. American Pie council has found that apple pie is the most popular flavor in America, followed by pumpkin, chocolate, lemon meringue and cherry.
Pies can be made either “onecrust,” where the filling is placed in pastry dough and baked without a top crust or “two-crust,” with the filling completely enclosed on top and bottom within the pastry shell. Blind baking is used to partially cook a pies crust before the filling is added. This helps keep the crust from becoming soggy. If the crust of the pie requires much more cooking than the filling, it may also be blind-baked before the filling is added and then cooked for a shorter period of time. Most importantly, always use the freshest local produce you can find to make your pies taste fresh and delicious. Here are some tips to help make your pie making a little easier and your final product look and taste terrific. • Always preheat your oven and bake the crust alone for about 10 minutes before filling. • When blind-baking a pie crust, always prick the pastry thoroughly with a fork before baking. The steam created in baking can escape through the holes so the crust won’t puff up. • Prepared pie fillings will bake for a shorter period of time than fresh fruit fillings. For fresh fruit allow 35-45 minutes for baking. • If the crust is browning too much and the

pie is not completely cooked, cover the top of the pie loosely with aluminum foil to shield the crust from the heat. • Always allow your pie to sit and cool for at least 15 minutes before serving. Animal Photographer Kim Levin Displays Her Captivating Photography In Two New Books If you are among the pet obsessed, then Kim Levin’s two new books titled, Pack Mentality and Cat-panions must find a way to your bookshelf. These hardcover gift books explain the unique ways of friendship from a funny, 4-legged point of view. Monmouth County’s own Kim Levin was responsible for the gorgeous photography throughout the books and writer Megan Colleen McGlynn wrote the humorous content. How did you and your writing partner come to select the images for this book?
The process for Pack Mentality was relatively easy because I had a lot of images with more than one dog. Once we had the group of images we wanted to feature then Megan wrote copy to match. In some cases, she had a great line that we knew we wanted include in the book so I tried to find the right image to fit. Cat-panions was a bit more challenging because we had the content, but not as many cat images depicting friendship. I had to set up a range of photo shoots in homes with more than one cat. This way, I had the opportunity to photograph several cats at the same time. Several people in the Monmouth County area participated, including the Monmouth County SPCA, where I photographed many of the cats in their care.
Some of Pack Mentality’s lines include, “You’re the one I want to go off-leash with.” and “You are the grass beneath my paws.”
How long did the process take?
We submitted the original book concepts last September and final books were due to the publisher in January of 2015. The books came together within six months.

Are most of the animals featured in the books “local” pets?
Yes, a majority of the animals in both books are local. Some of the dogs in Pack Mentality came from my local photography shoots. Anyone who has a dog or cat in the books is excited to see their animal featured.
Have any of the cats from the SPCA found homes as a result of your book?
When I photograph at the MCSPCA I provide the shelter use of all the images for their facebook page and website to help assist in getting the animals adopted. The good news is several of the cats photographed and featured in the book have new, loving homes. I am hoping the books can help to get more animals adopted and spotlight rescue animals.

Both Megan and Kim are animal advocates and their goal is to donate a selection of books to animal charities of their choice. Kim adds, “Each month we will select a shelter that will receive donated books and a Molly&Fig ™ gift basket for their shelter or fundraising event.” Additionally, the Monmouth County SPCA will be carrying the books in their Freehold Adoption Center and Eatontown shelter as a way to raise money for the shelter. Kim lives in Little Silver with her husband John O’Neill, a local musician and music teacher. They have two children and are the proud parents of a rescue terrier/ hound mix. Antoinette Gingerelli, from Howell, NJ, is a rising junior at Rutgers University majoring in Political Science, Women’s and Gender Studies, and Middle Eastern Studies with a minor in International and Global Studies. She is a proud member of Douglass Residential College, a program at Rutgers dedicated to empowering women and developing leadership skills. At Douglass, Antoinette serves as President of the Douglass Friends of the United Nations Population Fund (UNFPA) where she has found a passion for global health. She also serves as President of the Rutgers Women’s Political Caucus encouraging women to run for public office and become actors in the political sphere. Antoinette is a Harvard Public Policy and Leadership Fellow and Rutgers Lloyd C. Gardner Fellow. This past year EduCare Foundation Inc. selected Antoinette as Ms. EduCare 2015 to promote the important role education plays for young people, especially young women. Additionally, Antoinette serves as a County Committeewoman in her hometown of Howell. She also acts as the American Association of University Women (AAUW) Youth Representative to the United Nations, advocating for the rights of women and girls. As a Hurricane Sandy survivor, Zucconi is constantly giving back to the community, donating her time to various charities and causes. Beyond The Laces:
The True Gift Of A Professional Athlete We are pleased to announce the release of Bob Salomon’s new children’s book titled, Beyond The Laces.
B
ob co-created and promoted an award-winning book called A Glove Of Their Own. Through this book and with the support and endorsements of professional athletes from all sports, Bob succeeded in teaching children the importance of “paying it forward” and giving back to those who have less and have gone without. Bob’s team used this book as a fundraising vehicle to deliver new and used sporting equipment to children in need.
This time, Beyond The Laces uses football as the theme. Bob and his team are inspiring even more children through kindness by harnessing the true gift of the professional athlete, which is the positive influence they have on them. This book will unite ALL sports, not just football, to help children in need and to never give up. The team’s mission is to inspire through kindness. Our goal is to get this book in as many hands as possible. With this inspirational message and the help of the our professional athlete network, we are reaching children and Herman Edwards families by organizing events at hospitals, schools, non-profits and charities, and Doug Glanville, youth sports groups and anywhere there is a child or family in need. Through ESPN Commentators these events, our book sales, corporate sponsorships and private donations, we are reaching people who share the same vision.
