Social Egg Freezing: Is it Right for Me? By Dr. Allen Morgan Morgan Fertility and Reproductive Medicine Egg freezing allows a woman to take control of her ticking biological clock by banking her eggs for future use. This elective procedure has gained popularity lately due to many large corporations such as Apple and Yahoo now offering egg freezing as an option to their employees who want to focus on career before starting a family.
Adding to the buzz, many celebrities are now openly talking about their choice to freeze their eggs until finding the perfect partner, then undergoing in vitro fertilization (IVF) when the time is right.

• Fran Drescher • Bridget Marquardt • Sofia Vergara • Kim Kardashian • Maria Menounos • Janet Jackson • Jennifer Aniston Beyond the intention of pursuing a career or waiting for the right partner, there are many other factors that motivate a person to elect to freeze their eggs for later use. Age, school, lifestyle concerns, or medical concerns such as family history or a cancer diagnosis may also play a role in the decision to freeze eggs for later fertilization. Whatever the motivation a woman may have to freeze her eggs for later use, there are common questions associated with the process that should be taken into consideration prior to making such a big decision.
What is the optimal age to freeze eggs?
Fertility specialists recommend freezing earlier for better quality oocytes (eggs) as well as embryos. The best time to freeze your eggs is in your 20s and 30s. Certainly over age 42 there is not much benefit to freezing your eggs. Typically, 41 and under is the limit. However, in addition to age, we also take into account hormone levels and antral follicle (resting follicle) count.
Indeed, if we look at IVF data, there is an inflection point in pregnancy rates after the age of 35. Between the ages of 38 to 39, there is an even larger inflection point. So, the best advice a woman’s health professional can give to someone who falls into one of these age groups is to start the process with a specialist as soon as you can. It doesn’t hurt to investigate and there is no obligation to proceed, since this may be an elective procedure.
Will my health insurance cover egg freezing?
Typically, most health insurance companies do not cover elective fertility procedures. However, you should check with your provider directly to confirm your specific coverage details. Often some blood tests, ultrasounds or other pre-retrieval procedures may be covered. If you are about to undergo treatment for cancer, your insurance may offer coverage for fertility preservation. There has been much movement in legislation over the past few years advocating for individuals with medical issues such as cancer.
How do I choose a doctor to discuss my options?
While there are many fertility specialists in our area to choose from, you will want to find a specialist who not only fits your personality style, but who is known as an expert in egg freezing. Also, you will want to decide whether a fertility “clinic” or a solo practitioner better fits your personality style. Generally speaking, smaller practices offer a more intimate, personalized bedside manner, and larger “clinic” style practices provide a bit less personal experience, while both may have high success rates. Last, it is important to select a doctor who utilizes minimal stimulation (fewer medications) while achieving high success in vitro fertilization (IVF) rates, a sure sign of an expert staff and close monitoring of patients.

Millions of seniors rely on Medicare to cover their healthrelated needs. But Medicare isn’t a one-size-fits-all solution. Those who enroll in original Medicare will generally get free coverage under Part A, which pays for hospital visits. You can then opt in to Part B, which covers preventative care and diagnostics. Part D covers prescription drugs. But there is an alternative to Medicare that many seniors choose instead: it’s Part C, otherwise known as Medicare Advantage. Medicare Advantage essentially bundles all your coverage needs into a single plan. You pay a premium but often Advantage plans end up being less expensive than original Medicare, offering at least the same level of coverage, if not more. In fact, many seniors choose Advantage over traditional Medicare because Advantage covers dental, hearing and vision, and changes to Medicare Advantage set to take place in 2019 make it an even more attractive choice. Many Advantage plans also offer coverage overseas. Effective January 2019, Medicare Advantage plans will have the option to cover things like transportation to and from medical appointments and meal delivery to seniors’ homes. Also, plans will be able to offer enrollees coverage for home safety features such as ramps and bathroom/shower support bars. These new benefits lower the chances of injury, so if you’re experiencing mobility issues, it may pay to see how an Advantage plan might help. In addition to these added benefits, soon Medicare Advantage plans will be able to offer home care coverage for seniors who need help with personal
care at home as well as things like cooking, cleaning and errands. This is a big change and one that the home care industry has been advocating for many years. This new benefit supports seniors who wish to age in place comfortably and safely in their own home.
There are plenty of good reasons to sign up for a Medicare Advantage plan for 2019. Starting next year, a new open enrollment period for Advantage will kick off on January 1st and run all the way through March 31st. If you are already enrolled in a Medicare Advantage plan, you’ll have the option at that time to move from one plan to another. Be sure to research different plans to determine which one is best for you or your loved one.

The Arthrogram A close friend hurt his shoulder. He had a MRI which showed a little arthritis and tendon inflammation. He had a few weeks of rehab and continued to have shoulder pain. At this point, his orthopedic surgeon ordered a MRI arthrogram. My friend did not know what an arthrogram was and how was it different from his first MRI. Q: What is an arthrogram? How is it performed? A: An arthrogram is the procedure of injecting dye into the joint. After the
dye is injected, images are taken. This is most commonly done by CT or MRI. An arthrogram can be performed on any joint but the most common joints are the shoulders, knees, hips, and wrists. The radiologist cleans the skin using a sterilizing antiseptic and drapes the area with sterile dressings. Using a local anesthetic such as novacaine, the skin and deeper soft tissue is numbed. A very thin needle is inserted into the joint using the x-ray images as a guide. A tiny amount of x-ray dye is injected into the joint to confirm the needle is in the correct area. Then the joint is filled with the arthrogram dye. After the arthrogram dye has been injected, the patient is escorted to either the MRI or the CT scanner (depending on what type of arthrogram they are having).
Q: What is the arthrogram dye? What happens after it is injected? A: If the patient is having an MRI arthrogram, the dye is MRI contrast that
is heavily diluted with sterile saline (mild salt water). If the patient is having a CT arthrogram, the dye is CT contrast that is heavily diluted with sterile saline. Once the dye is injected, the patient has their MRI or CT. Afterwards, the dye in the joint will naturally absorb into the body. Since the dye is mostly salt water, it is safely injected, absorbed, and eventually excreted.
Q: What is the benefit of doing an arthrogram? A: Slightly distending the joint with fluid makes it easier to see inside
the joint. The injected dye helps outline what is inside the joint to help determine whether anything is torn. This is especially helpful to detect labral tears in the shoulders and hips.
Q: Is the procedure painful? A: The joint is made numb with the local anesthetic. Afterwards the joint
can feel sore however this does not last more than 24 hours and is easily treated with Tylenol or Advil.
Q: How should I prepare for an arthrogram? A: No special preparation is usually
required. It is best to wear comfortable clothes without metal (buttons, zippers, hooks, etc.).
The white fluid help outline the structures inside the joint to determine what is intact and what is torn. This patient has a subtle labral tear that can only be seen on this arthrogram.

What you Need To Know About Bio-Identical Hormone Replacement Written by Eric Sauer, R. Ph. NMD. DHPh For over twenty years I’ve educated women and men (as well as physicians) on the importance of balancing hormones. Unlike synthetic hormones, which are manufactured by pharmaceutical companies, Bio-Identical Hormones are special compounds that are only compounded by a compounding pharmacy, and are “yam-based” hormones which can be taken for a lifetime with “NO” side effects. 1. What is Natural Hormone Replacement or Bio-Identical Hormone Replacement? Natural Hormone Replacement Therapy (NHRT) is an attempt to ‘replace’ some of the normal hormones in your body that have declined as a result of peri-menopause or surgical menopause. One of the differences between the natural vs. synthetic hormones is that the natural hormones are “bio-identical,” meaning they are exactly the same as what your body makes. Saliva testing is 98% accurate while blood testing is only 2% accurate. All blood testing measures is the NON ACTIVE FORM, which makes it less accurate to dose the patient correctly. The saliva test is both BIO-ACTIVE and is measured on a cellular level, which clearly makes a significant difference, making it more accurate to treat the patient and dose correctly. Twenty thousand physicians prefer saliva over blood when it comes to measuring hormone levels in the body. 5. Why Do We Need Bio- Identical Hormones? As teenagers for PMS symptoms, since there are over 400 symptoms I won’t name them all. Teenagers with cramps, mood swings, foggy brain, hair loss and irregular periods. I’ve had girls at ages 14 and up taking progesterone lozenges without any side effects. Birth control pills lead to breast cancer in women. Menopause women or those who have had a hysterectomy really need hormones naturally. In this first series of articles addressing “Women in Transition” issues, I wanted to share my own personal inspiring story of transformation. Prior to my marriage: business trips around the world, 12-hour days, designer clothes and social networking were the norm! It was hard work, but I was good at my job in international finance and I felt valued. But after marriage, two children, a seven-year career break and a difficult divorce, I was facing the transition of going back to work that I dubbed the “mum-back.” Here are the five amazing things that happened when I returned:
I decided to help others: During the “mum-back” and “divorce” phases, I realized that there was limited support for women going through difficult transitions. And so the idea for my WHEEL (Women Helping Educate & Elevate Life) group was born. My goal is to help educate, connect and support women in all phases of their lifecycle through a combination of educational seminars and networking events. I realized I deserved to be more than just “Mrs.” or “Mum”: For several years my identity was tied to my husband and children. No one in my inner circle knew anything about me or my past accomplishments. Returning to work made me financially independent, returned my identity and allowed me to be an inspiration to my own children. I no longer talk to them about how to be successful, strong, confident or smart – I just let them observe me and I lead by example! I’m Having Difficulty Swallowing – What Does This Mean and What Can I Do About It? How do I treat my problem swallowing? Written by

W
e take our ability to swallow for granted. It may seem like an easy, natural bodily function, but there are a lot of muscles and tissues coordinating together at just the right moment to move what we eat from our mouth to our stomach. Difficulty swallowing (referred to as “dysphagia”) is the inability to swallow solid food and/or liquid with ease. Some describe this as a sensation of food getting stuck in their throat or chest after swallowing. Others may feel chest pressure, chest tightening, or have choking or coughing after swallowing.
Treatment depends on the underlying cause. Sometimes we can stretch the esophagus if it is narrowed. Medication that reduces acid or that can relax the muscles of the esophagus can also be used. Occasionally, we can inject a muscle relaxant such as Botox into the bottom part of the esophagus. A less common condition, called achalasia, may benefit from surgery. Occasionally, a swallowing specialist can provide exercises to strengthen your swallowing muscles, train you to improve the way you swallow and teach you how to adjust the consistency of what you are eating and drinking so it goes down more easily. All in all, difficulty swallowing is an important symptom to discuss with your gastroenterologist.

What can cause problems swallowing?
Not chewing thoroughly, for example, in someone who might be missing teeth or has trouble using dentures is one cause. A prior stroke can produce weakness in the muscles at the top of the esophagus. Longstanding acid reflux (GERD or “heartburn”) can cause inflammation in the esophagus. A narrowing or scarring of the esophagus after many years of untreated acid reflux can create a roadblock, called a stricture, that will prevent passage of food or drink. Food allergens can also cause inflammation of the esophagus and lead to narrowing. Diseases that affect the muscles and nerves of the esophagus, such as scleroderma and achalasia, respectively, can affect the way the esophagus moves and can disrupt proper emptying of food into the stomach. In some people, the esophagus is overactive and squeezes too hard, which can be painful. Infections of the esophagus can also cause pain and difficulty swallowing. The most worrisome cause of swallowing difficulty is a cancer in the esophagus.

How do I figure out what is causing my swallowing difficulty? The first step in our investigation is usually an upper endoscopy (referred to as an EGD), a low-risk exam under anesthesia that allows us to examine the esophagus and stomach and to take biopsies if necessary. A second test we can do is called a pH test. This is a test you go home with that records acid exposure in the esophagus. A third test, called a barium esophagram, involves drinking a substance that lights up under X-ray. A radiologist takes pictures and videos under X-ray to see how things travel from your mouth to your stomach. Finally, we can perform a test called esophageal manometry. This requires a catheter to be placed into your esophagus for a 30 to 40-minute exam. We will measure the pressures and activity of the esophagus muscles as you swallow small sips of water.

Focus on Your Heart for a Change The HeartMath Institute in Boulder Creek, California has been conducting scientific research since 1990 on the intelligence of the heart. HeartMath is a form of biofeedback that measures heart rate variability, the natural variation in how our heart beats. This is thought to be an indication of autonomic nervous system balance. The autonomic nervous system is the aspect of our nervous system that helps to regulate arousal (think stress states versus relaxed states). Ninety percent of the communication between the heart and the brain starts in the heart and moves upwards to the brain. It’s the heart that informs the brain how we are feeling emotionally and physically. When we are angry and frustrated, the heart sends those messages to the brain. Brain function is inhibited. We experience foggy thinking and the simplest decisions are difficult to make. No wonder we can’t think clearly under stress! When we are stressed, our heart rhythms are erratic. When we are relaxed, our heart rhythms take on a more regular pattern. Since our heart rhythms vary as our emotions change, we have the power to change our physiology and feel emotionally and physically better by shifting our thoughts. As we focus on our hearts and think of love and appreciation, the brain receives the signal and responds by increasing positive feelings of peace and balance. We feel more emotionally stable and we can think more clearly. If we looked at a graph of our heart rhythms now, they would look smoother and more even. HearthMath researchers talk about a balanced state when the heart, mind and emotions are aligned as coherence. Coherence is different than a meditative state. It is when we are relaxed and at the same time alert and able to carry out our everyday activities. Not only do we feel better when we are in a coherent state, but we also create deeper, more loving connections with ourselves, our families and friends. Think how the world might change if everyone shifted focus from the mind to the heart and approached life and relationships from a place of love, caring, appreciation and compassion. So how can you shift to a harmonious heart rhythm and enter a state of coherence quickly and easily? Practice heart-focused breathing. Follow these simple, yet effective steps provided by the HeartMath Institute: n Eyes can be open or closed. n Bring your attention, your thoughts to your heart center, the area in the center of your chest. n If you like, you can place your hand on your heart to help keep your attention there. n Begin to breathe a little slower and deeper than usual. n Now imagine or pretend that your breath is flowing into and out of your heart area. Take slow, smooth, deep breaths, finding a natural rhythm (If it feels comfortable, inhale to the count of 5 or 6 and exhale to the count of 5 or 6). n Continue breathing in this manner for a few minutes. To increase the benefits of the exercise, add the following steps: n Bring to mind someone or something you appreciate or a person or pet you love. n Imagine a time when you felt a lot of love for them and they for you. n Really feel that love and appreciation in your heart. n Stay with those loving feelings for a few minutes. Do I Really Have To Live With Sciatic Pain? What is sciatica?
Sciatica is low back pain combined with a pain through the buttock and down one leg. The leg pain usually goes past the knee and may go farther to the foot. Weakness in the leg muscles can be a sign of Written Written by by sciatica. They come out of the spinal column low in the back and then go behind the hip joint, down the buttock, and down the back of the leg to the foot. Sciatica is different from other forms of low back pain because while the pain most often begins in the back, it usually travels down one lower extremity. The pain is usually a shooting pain, like electricity. It can also burn like fire or tingle much like the feeling when one’s leg “goes to sleep.”
Irritated Sciatic Nerve
What are the common causes?
Sciatica is caused by irritation of the sciatic nerve. Usually, there is no specific injury that is related to the onset of sciatica. Occasionally, the pain will suddenly begin after lifting something heavy or moving quickly. The following are causes of sciatica: • Herniated discs: this is the most common cause of sciatica. Discs are the cushions between the bones in the back. They act like “shock absorbers” when we move, bend, and lift. They are the size and
shape of checkers. Lumbar Disc There is a tough ring Herniation around the outside and a thick jellylike center inside (called a nucleus pulposus). If the outer edge of the disc ruptures, the center can push through and put pressure on the sciatic nerve, leading to the pain of sciatica (referred to as a herniated nucleus pulposus). • Spinal stenosis, a narrowing of the canal that contains the spinal cord: As we age, the bone can overgrow and put pressure on the sciatic nerve. Many people with spinal stenosis have sciatica on both sides of the back.
Sciatica Home Remedies for Pain Relief
Pain from sciatica often limits one’s activities. Here are some home treatments for sciatica: • Do not bend, lift, or sit in a soft, low chair; the pain will get worse. • Unless someone is allergic or should not take them for other reasons (if someone takes a blood thinner such as Coumadin, for example), over-the-counter pain medicines such as acetaminophen (Tylenol), aspirin (Bufferin or Bayer Aspirin), or ibuprofen (Advil, Motrin IB) will probably help ease the pain. • Try a cold pack to see if it helps the pain. If someone doesn’t have a cold pack, use a large bag of frozen vegetables; it makes a good first aid cold pack. Or have someone massage the sore areas in a triangular pattern with an ice cube. The person should move the ice cube if the skin gets too cold (this may melt several ice cubes). • After the cold massage, try alternating with heat from an electric heating pad to see if it helps the pain. (Do not sleep with a heating pad on the back. It could cause a bad burn.) • If someone doesn’t have an electric heating pad, put a hand towel under hot water, wring it out, and place it on the back. Some physical therapy
experts believe that moist heat penetrates more deeply and gives better relief of pain. (Do not use wet packs with an electric heating pad because electrical shock may result.) • The affected individual may feel better lying on his or her back on a firm surface with a pillow under his or her knees. Another option is lying on one’s side with a pillow between the knees to keep the back straight. Also, one might find that a recliner chair is helpful. • Take it easy, but do not lie in bed for longer than two days because this has been shown to actually worsen the condition. Do activities one is able to tolerate, and do not expect to feel better overnight.

• Chiropractic treatments or physical therapy is often prescribed for sciatica. • Epidural injections, which are injections of a steroid (commonly known as cortisone or prednisone) into the spine, are sometimes performed for persistent sciatica. • Studies have shown that exercise therapy is helpful to treat back pain that persists for more than a few weeks. Good exercises for sciatica include walking, yoga, Pilates, and other specialized exercise programs.

Healthy Eating Leads To Healthy Smiles! If you want healthy and goodlooking teeth as an adult, it is important to take care of them during childhood. Diet is directly linked to the strength and beauty of teeth- malnutrition is the number one cause for weak teeth and junk food is the biggest culprit when it comes to tooth decay. Vitamins, minerals, and other essential nutrients keep crowns safe from damage and they strengthen them so that your child can enjoy sweets without worry. Naturally, fruits and vegetables are the most important source of nutrients.
Vitamin C is a critical component of dental health. It helps to boost resistance to cavities, and it has the added benefit of strengthening gums. Oranges, kiwis, and strawberries are great sources of Vitamin C and they prevent harmful bacteria from colonizing and contributing to gingivitis. Other powerful ingredients are seeds. Try to include seeds like pumpkin, sunflower and sesame seeds into your kid’s packed lunch- they go perfectly with sandwiches and burgers. Crunchy vegetables like cauliflower, green beans, carrots, and cabbage are not only delicious and readily available in your local grocery store, but they also provide a unique way of helping teeth through the necessity of extended chewing. The chewing process acts as a tooth cleaner, scrubbing the teeth and removing plaque that gathers after meals. Believe it or not, it is also important to encourage your child to have a piece of sugar-free chewing gum daily. Chewing gum for fifteen minutes after meals
is very efficient in polishing the tooth surface and removing food debris from the crevices. It also increases saliva production, which acts as an anti-bacterial agent. All of these foods will benefit your child’s teeth, but it is also important to avoid the problematic foods that can get stuck between teeth and cause cavities. As a parent, it is important to regulate the amount of sweets and sticky candies that your child consumes. Foods like caramel, taffy, hard candy, and soda can cause major destruction in your child’s mouth. If your child cannot resist sticky sweet temptation, it is a good idea to have some sugar-free gum handy to help protect against the harmful effects of these foods. Last but not the least, maintain the habit of seeing a dentist twice a year and brushing at least twice a day. Regular brushing is the most essential part in maintaining healthy teeth both in childhood and adulthood!

Dr. Liya Ostrow & Dr. Max Sulla
National Children’s Dental Health Month (NCDHM) is a celebration of awareness of good oral health habits for children. Dentists and other oral health providers across the country take time to focus on preventative oral health care for kids.
Despite being almost 100 percent preventable, tooth decay is the most common chronic disease in children, according to the National Institute of Dental and Craniofacial Research. Cavities plague many youngsters, from babies to those in their late teens. In fact, more than 40 percent of children ages 2 to 11 have had a cavity in their primary teeth, while more than two-thirds of 16- to 19-year-olds have had a cavity in their permanent teeth.
Dr. Liya Ostrow was introduced to Dr. Max Sulla through a colleague while she was in her final year of residency at Lutheran Medical Center in Brooklyn as a Chief Resident. Following the initial interview, Dr. Max Sulla knew that she would complement the Tender Smiles team. Now, after working together for eight years, they complement each other and share many of the same ideals. Dr. Max adds, “We have a shared knowledge of sedation dentistry and our work complements one another well.” Their compassion, knowledge and emphasis on early intervention make them a true “dream team.” In dentistry, there is nothing more important than early intervention. Although they do everything they can to help a child understand the value of their oral health and develop a commitment to healthy mouth habits from an early age, not everything can be prevented. Some children are born with dental problems or with a predisposition to them, and even children who practice good dental habits can develop problems or disease. “In those cases, our goal is to provide the absolute best care for this child with a pleasant, non-frightening experience,” Dr. Ostrow adds.
We recently caught up with each of them to hear first-hand about the way they are making a true difference in the local dentistry field. Dr. Max Sulla graduated NYU College of Dentistry with his D.D.S. with honors and then received his Pediatric Dentistry degree from the University of Medicine and Dentistry of New Jersey. He is the President of the NJ Academy of Pediatric Dentistry and a New Jersey public policy advocate for the American Academy of Pediatric Dentistry. He and his wife, Elana, have three children: Matthew, 16; Jordana, 13; and Viven, 7. In his free time he enjoys traveling, cooking and gardening. CW: Why is it so important for young children to get a head start on their dental health? Dr. Max: Because it encourages a healthier lifestyle overall and prevents decay. CW: When should children begin using toothpaste and how much should we use? Dr. Max: The sooner the better! Starting at birth, clean your child’s gums with a soft infant toothbrush or cloth and water. Parents should use a tiny smear of fluoride toothpaste to brush baby teeth twice daily as soon as they erupt and a soft, age-appropriate sized toothbrush. Once children are three to six years old, then the amount should be increased to a pea-size dollop, and parents should perform or assist your child’s tooth brushing. Remember that young children do not have the ability to brush their teeth effectively. Children should spit out and not swallow excess toothpaste after brushing. CW: How can parents make tooth brushing more fun for toddlers? Dr. Max: Character toothbrushes, lots of praise and reward stickers will definitely help, but most important is doing the same routine daily so they get used to it from an early age. CW: Are you seeing an increase in cavities in young children these days? Dr. Max: Yes, due to a diversified diet frequently filled with junk food and sweetened drinks. CW: What kind of unique health programs do you have for children? Dr. Max: We do school programs given by a registered Dental Hygienist throughout three counties. In addition, this year we began mobile dentistry where team of hygienists and dentists visit local schools to provide comprehensive preventative treatments such as examination, cleaning, x-rays, sealants and fluoride application.
Dr. Liya emigrated from Russia in 1991 at the age of ten. She was a Summa Cum Laude graduate in 2003 from Hunter College in New York. That same year she relocated to Philadelphia to attend the University of Pennsylvania School of Dental Medicine, where she earned her Doctor of Dental Medicine degree, Cum Laude. While at UPSDM she was keenly involved in programs and visits providing underprivileged children and adults with much-needed dental care and education. Advanced studies in her field of Pediatric Dentistry took place at Lutheran Medical Center in Brooklyn, New York, where she served as Chief Resident. Dr. Liya is Board Certified by the American Board of Pediatric Dentistry She lives in Marlboro with her husband, Vlady, who is a pediatric endocrinologist, and their two children, Julian (10 yrs.) and Kayla (7 yrs.), plus an 11-year-old Pekingese named Archie. In addition, Dr. Ostrow just welcomed a beautiful baby boy named Aaron. CW: What is the most rewarding part about your profession? Dr. Liya: I enjoy spending most of my day with children. Being a part of the non-scary world of dentistry and hearing at the end of the appointment, “This was fun.” CW: What is the best part about working with Dr. Max? Dr. Liya: His knowledge, compassion, humor and approach to children are something that I admire greatly. I have learned a great deal from him. CW: What are the best preventative measures parents can take when it comes to dental health for their children? Dr. Liya: Children should see a dentist regularly every six months. This allows dental professionals to constantly educate parents and children on oral hygiene, as well as give nutritional advice. Some examples are to encourage your children to eat regular, nutritious meals and avoid frequent between-meal snacking. Other preventative measures include fluoride and sealants which protect teeth from decay.
By participating in NCDHM, parents and kids learn about good oral health habits, such as how to properly brush and floss. Parents learn how to check their child’s mouth for early signs of gum disease and what proper nutrition is required to minimize periodontal disease. With proper guidance and following some simple tips, parents can help their children retain their beautiful smile for years to come.

Christine Matus, Founding Attorney Christine Matus was admitted to the Bar of the State of New Jersey and to the U.S. District Court of New Jersey in 1995. She graduated from Douglass College, Rutgers University with a Bachelor of Arts in Economics in 1992. In 1995 Ms. Matus graduated from the Jacob D. Fuchsberg Law Center at Touro College with a degree of Juris Doctor. Kathleen Taranto, Planning Paralegal and Administrative Assistant Kathleen graduated from Ocean County College with an Associate degree in Criminal Justice, followed shortly thereafter with her Associate degree in Business Administration, and later an Associate degree in Paralegal Studies in 1994. She is currently enrolled at Liberty University Online, School of Law, working toward her Bachelor of Science degree in Paralegal Studies with a 4.0 GPA. Kathleen has been a paralegal for many years and has been involved in various areas of the law. Ms. Clark is a strong advocate for the rights of students who require special education programs. The Salivary Glands Written by: Dr. Stephen Kupferberg, MD, FACS There are both major and minor salivary glands. The major salivary glands are the parotids, submandibular and sublingual glands. The function of all the glands is to produce saliva to moisten the mouth, aid in digestion, and help protect the teeth from decay. The parotid glands empty into the mouth via a single tube called a duct. It is called Stensen’s duct and it is located in the cheek near the second molar of the upper teeth. The submandibular gland empties under the tongue in the front of the mouth through Wharton’s duct. The sublingual gland has many ducts in the floor of the mouth. There are multiple minor salivary glands located in the lips, cheeks and other areas of the mouth and throat. Several different problems can involve the salivary glands including obstruction, infection, tumors, and other disorders. Obstruction most commonly occurs in the parotid and submandibular glands. It may be due to the formation of stones in the gland or ducts. Strictures or narrowing can also slow salivary flow. When a person eats, the nervous system signals the gland to increase its production of saliva. This may back up and cause the gland to swell. This swelling usually decreases slowly after a meal. The most common infection in children is mumps and affects the parotid glands. This usually occurs in children who have not been immunized. Infections also happen in adults. This often occurs in the elderly who become dehydrated or with severe obstructions. This often necessitates antibiotic therapy. Tumors also occur in the salivary glands. They most commonly present as painless swellings on the side of the face or upper neck. Aggressive tumors can present with involvement of the facial nerve, causing decreased movement in portions of the face. Most tumors
are benign. Pleomorphic adenoma and Warthin’s tumors are the most common benign lesions. Mucoepidermoid carcinoma, adenocarcinoma, and adenoid cystic carcinoma are the most common malignant tumors. Diagnosis is often made by physical examination. A mass can be felt in the cheek or neck. Stones can sometimes be noted by palpation in the mouth. CT scans or MRIs are often necessary to delineate the extent or size of tumors or stones. If a tumor is found, a fine needle aspiration biopsy is often performed to figure out the pathology to guide further therapy. Small stones may be removed by sialoendoscopy. This involves removing the stones through the ducts with a small camera, graspers, baskets, or lasers. Larger stones will necessitate more extensive surgery. Tumors will often necessitate surgical intervention. Tumors involving the parotid gland will often necessitate a superficial or total parotidectomy with facial nerve dissection, as this nerve divides the gland into superficial and deep lobes. Malignant lesions may result in sacrifice of part or all of the facial nerve. Submandibular gland lesions are removed by excising the entire gland. If malignancy is noted, then radiation therapy and possible chemotherapy may be needed.

So be sure and keep an eye out for that! Have you learned anything new about yourself since starting this new endeavor? Yes, there are so many discoveries I have made. There are countless things that happen behind the scenes of each issue and to be able to produce and distribute The County Woman on a bi-monthly basis takes an enormous amount of work. It would not be possible without team members performing to their full potential.

I love designing the layout for each issue and developing the different themes. Another part I enjoy is creating beautiful ads so that the client’s vision comes to life. What is Sleep Apnea?
Many people who snore may not simply be snoring but could be suffering from a more serious health condition called Obstructive Sleep Apnea (OSA). Only about 10% of patients with the condition have been diagnosed. It is where a person stops breathing many times throughout the night and is caused by a physical blockage or collapse of the upper airway while asleep resulting in a chronic lack of oxygen to the brain and body. Over time it takes a toll on your body and causes or contributes to other significant and medical problems.
How does it affect our health?
Sleep apnea is called the silent killer and is a very serious condition which should not be taken lightly. If left untreated it damages your cardiovascular system and can cause high blood pressure, heart disease including arrhythmias, rapid heart rate, heart attacks and strokes. Other conditions related to sleep apnea include depression, mood disorders, memory loss, low sex drive, restless leg syndrome and acid reflux or GERD. Studies now show that in some cases a person can decrease their lifespan by as much as a decade if they have untreated sleep apnea.
What symptoms do people with sleep apnea have?
Symptoms depend on the severity of the condition and some patients may show none if their condition is mild, none the less it needs to be treated. The most frequent sign is loud snoring and sometimes gasping for air while sleeping. How much Chocolate is sold on Valentine’s Day?
Valentine’s Day is a serious chocolate holiday. Let’s face it; the chocolate heart box has become a symbol of the day. Worldwide nearly 58 million pounds of chocolate are sold during the week leading up to Valentine’s Day. At our store we sell over 100 pounds of chocolate covered strawberries between the day before and Valentines Day.
What does tempering mean and what is the best way to temper chocolate at home?
Tempering chocolate is a process of heating and cooling chocolate to a precise temperature in order to keep the fat solids in the cocoa from separating out. Tempering machines are the best way to temper chocolate. However, for tempering chocolate at home without a tempering machine can be done. The best way is to heat 2/3 of the chocolate over a double boiler. Remove the chocolate from the heat and add the remaining 1/3 chocolate pieces stirring frequently until all the chocolate is melted and smooth. Tempered chocolate will appear glossy and have a nice snap when bitten.

RESOLUTION REBOOT: A New Year, A New You!

very year it seems we resolve to eat better, exercise more, lose ten pounds, stop stressing, and start meditating. But while our intentions are good, our follow-through is often lacking. Research has found that after just six months, more than 50 percent of us have given up the goals we set in January. Not this time! The Oprah Magazine featured a series of rules on how to make resolutions so you can set yourself up for success- all year long. We share four of them with you here!
