Smartphones Can Be A Pain In The Neck By: Rosemarie Daalder, DPT, for All-Care Physical Therapy Center’s Forked River Location Is My Smartphone Causing My Neck Pain? Within the last few years, more and more patients are arriving at All-Care Physical Therapy with complaints of neck pain. These patients with complaints of neck pain are also becoming younger and younger. Typically, neck pain is seen with an increase in age, which makes us wonder: why the change in the age of patients seen with neck pain, and why the increase in the number seen? “Text neck” is the term given to this increase in neck pain complaints seen in patients within the last few years. This is especially seen in the younger populations who have had no prior history of neck pain or injury. So what is ‘text neck’? Text neck is a repetitive strain injury of the neck muscles, the shoulders, and even in the lower back. This is directly related to our posture when we use our smart devices, like cell phones and tablets. On average, a person may spend 2-4 hours a day using their smartphone. The most common symptoms of text neck may be complaints of neck pain and soreness. Some other presentations of text neck may include headaches, upper back pain that ranges from chronic, nagging pain to sharp spasms, shoulder pain and tightness, and even neurological symptoms that can travel down the arm and into the hand. This is called radiculopathy and originates from a cervical nerve being pinched as it tries to exit the cervical spine. All of these possible signs and symptoms of text neck can be addressed by a Physical Therapist.

How does using my cell phone or tablet hurt my neck? Using these smart devices causes us to slouch into a poor posture, which causes extra strain on our bodies. Most of the population (79%) between the ages of 18 and 44 years old have their smartphones on them at all times. Picture yourself looking at your smartphone; maybe it is in your hand right now. Our head drops forward as we look down towards our smartphone. Our shoulders may also lift towards our ears and round forward. This posture changes the natural curve of our cervical spine. We have to admit…some of us can get so involved in our smart devices that time passes right by. An 8-18-year-old on average spends about 7.5 hours every day using some type of smart device. Staying in this position for a prolonged period of time can cause misalignment, muscle strains, and wear and tear of our joints.

Three main things occur as we drop our head down to look at our smartphone. 1. Our neck moves forward 2. Our shoulders round forward and may lift towards our ears 3. Our neck muscles and shoulder muscles then contract Our neck muscles have the job of holding up our head throughout the day against gravity. The head is a lot of weight to hold up all day, on average weighing between 10 and 12 lbs. When we move our head 1 inch forward, this can double the load on our muscles. Your chin resting on your chest when looking at your smartphone creates 60 lbs of force on your neck. You can see an example of the pounds of pressure placed on the neck as we drop our head forward in the provided picture.

The ‘slump position’ that occurs as we relax and browse the newsfeed on our smartphone not only affects our spinal position but can also have an effect on our breathing. The slump position restricts our ability to expand our lungs and take in oxygen. Decreasing our ability to take in enough oxygen forces our heart to pump harder in order to circulate our oxygen-rich blood throughout our body.

How do we fix this?

Some of the effects of text neck or a forward head posture can be addressed with proper exercises and manual techniques that can provide a patient with longlasting relief. At All-Care Physical Therapy, the Physical Therapists have the knowledge and experience to treat neck pain in all age populations. They will develop treatment programs based on that individual patient and help them to return to their previous pain-free lifestyles. To improve our neck pain from using our smartphones, there are a few steps we can take every day in conjunction with Physical Therapy. 1. Straighten up—maintaining an upright posture and proper neck alignment will decrease the load and the amount of work our neck and shoulder muscles have to do in order to keep our head up. Using a mirror can help you see your posture and see how it feels to be in a correct, upright posture. Imagine a vertical line tracing down from your ear to your shoulder. This is proper, upright posture. 2. Look forward when using your smart device. Raise your smartphone or tablet to eye level as the example shows in the picture provided. This will allow you to stay upright and in proper alignment. No more forward head and downward head posture. Same idea when sitting at your computer. Raise the monitor or lower your chair so that the screen is eye level.

Rosemarie graduated from Delaware Valley College in 2007 Cum Laude with a Bachelor’s of Science in Animal Biotechnology and Conservation and a certification in Animal Assisted Activities and Therapy. During her undergraduate certification in Animal Assisted Activities and Therapy, she worked alongside Physical Therapists treating pediatric patients on horseback. This sparked her interest in Physical Therapy, and she therefore changed her career goal from Veterinarian to Physical Therapist. She continued into her physical therapy education at Rutgers-Camden, UMDNJ, and received her Doctorate of Physical Therapy in May 2011. Rosemarie has had clinical experiences in acute, sub-acute, and outpatient settings, as well as a 14-week pediatric affiliation with HollyDell School. Her areas of interest include sports medicine, joint replacement, manual therapy techniques, evidence based practice, and the neurological and orthopedic pediatric population. Rosemarie believes in patient centered practice by incorporating her patient’s personal goals into her treatment plans.

We continually challenge ourselves to be the number one educational resource for our readers. What are your goals for the County Woman this year? First and foremost, my primary goal is to continue the same high standard of magazine content, client service, and sense of community that our readers deserve. Beyond that, we will be undertaking a project this year to survey our readers for their insight and feedback so that we can provide the best readership experience possible. So be sure and keep an eye out for that! Have you learned anything new about yourself since starting this new endeavor? Yes, there are so many discoveries I have made. In particular, I have enjoyed using my creative side more than ever, and have been mastering the art of time management. There are countless things that happen behind the scenes of each issue and to be able to produce and distribute the County Woman on a bi-monthly basis takes an enormous amount of work. What are your goals for the magazine this year? As Production Manager, I have been able to oversee each cycle very closely. These days technology has become a huge part of everyday life. I would like to see the magazine become even more tech savvy. As a millennial, designing an app, or creating the website to become more mobile-friendly would be an ideal incentive to offer our clients. Additionally, I would like to see our social media continue to thrive. Over the past few years, the combination of print and social media is one of the most effective ways to advertise businesses. What do you enjoy most about your job? There are many things that I enjoy about working at the County Woman Publications. From the talented contributing writers, editors, and designers, whom I have learned so much from, to the clients which I have built long-lasting relationships, and lastly, the team of wonderful individuals, whose synergy results in making the publication a success. I would like to thank all of them for their commitment and dedication! If you were not in publishing, what kind of work would you be doing? When I am not in the office, I enjoy spending time with my Chocolate Lab, Bear, and my three cats. I have always had a passion for animals and my love for them continues to grow. What are your goals for the magazine this year? My goal is to remain a quality and leadgenerating tool in our clients’ marketing plan. I would like to reach even further into the communities we serve with helpful information, especially in the areas of health, women, and children. We have exciting growth on the horizon!

What do you find to be the most challenging aspect of your role as Sales Director? Because I travel quite a bit for my work, inclement weather can really put a damper on my disposition and safety. Sometimes, it can also be challenging to educate business owners on the importance of marketing. Spending money to create public awareness can add up. Our goal is to design a marketing plan using the combination of print and online media which best fits the client’s business goals. What is the most rewarding part? I enjoy getting the opportunity to speak with community leaders and hear about their triumphs and issues. Sales in any field can be a very rewarding position when the goal sits ultimately with the success and satisfaction of your client. Our staff takes a vested interest in the outcome for our clients and in the education of our readers. What do you think makes the County Woman so unique among other publications? Our commitment to solid and helpful information creates a connection between our contributors and our readers. We have bridged the gap for many organizations to work together and for community members to learn more about what’s happening in their own backyard.

What do you enjoy most about the Graphic Design Field? To me, graphic design is a way to express yourself and be able to create anything from your imagination. It is a wonderful feeling and highly rewarding for me to be able to create the exact image the client desires to help increase their exposure in the community. What is the most challenging aspect of your position? Working for a publication that covers multiple counties throughout the state and the many businesses far and in between is stimulating and rewarding. With so much going on, we continually try our best to be precise in showcasing exactly what our clients want in each issue. The goal is always to make their pages fresh and exciting so that it grabs our reader’s attention. This is a fast paced position that requires high levels of customer satisfaction, working towards the same goal of making an outstanding magazine with a personal touch. What do you think makes the County Woman so unique among other publications? The magazine remains very special to our readers and writers. ho will have authority to access or manage your digital assets after you die? Until recently, little was certain about how digital assets would be treated after the owner passed away. Oftentimes, no one, not even the owner’s executor, had the authority. Fortunately, lawmakers are finally catching up with the digital age, realizing that executors need access to digital assets in order to finalize the owner’s estate. A handful of states have laws that provide this authority, and many more, including New Jersey, are considering adding it to their statutes.

There are also numerous miscellaneous digital assets that encompass electronic subscriptions, reward programs, blogs, items stored in the cloud, gaming accounts, video sharing accounts like YouTube, movie services like Hulu or Netflix, electronic libraries like Kindle or Barnes and Noble, medical or educational records accessible through portals.

In New Jersey*, an executor still faces a formidable task of determining what accounts the decedent owned and to access those accounts. An executor’s task would be made much less onerous if the decedent left a list of access information and instructions. The executor can then easily access the accounts, address what needs to be done without the added stress of having to determine what accounts existed and figuring out how to gain access.

In today’s highly digital world, it is important that you review how your estate plan handles digital assets, and decide if an update is needed. Appropriate language may need to be included in your estate documents that authorizes your executor to access and deal with the your digital assets. * On July 31, 2017, the Assembly gave final legislative approval to A-3433, known as the “Uniform Fiduciary Access to Digital Assets Act.” The bill passed both houses of the Legislature without opposition and now goes to Gov. Chris Christie for consideration.

Digital assets are not the electronic devices such as computers, smartphones, or iPads or tablets. Rather, digital assets are the information stored on the electronic devices. There are the social media accounts, such as Facebook, Twitter, Instagram, Linked-in, Snapchat, and Shutterfly and other social sites. There are the personal accounts that include a wide range of accounts that include email accounts, financial accounts, auto-pay or paperless billing accounts, personal daily calendar, contact lists, statement information from financial institutions or credit cards, PayPal, Amazon, eBay, Sirius, Spotify, Pandora, and other digital accounts that the owner maintained his or her personal information. There are the digital business accounts that include assets such as digital customer information, databases, trademarks, trade-secrets, websites, domain names,

Lentigo Maligna Melanoma The incidence of melanoma has increased steadily in last decades. The American Cancer Society estimates that 76,380 new melanomas (about 46,870 in men and 29,510 in women) were diagnosed in the United States in 2016. The number of deaths from melanoma is estimated at 10,130 persons (about 6,750 men and 3,380 women). Lentigo maligna is a deadly skin cancer, a subtype of melanoma in situ. It affects women more commonly than men. Lentigo maligna melanoma is one of the subtypes of invasive melanoma that represents 5-15% of cases. Lentigo maligna melanoma most commonly occurs on sun-exposed skin, especially of the face, more so in elderly persons but might affect people of any age. It is characterized by an irregular border, more than one color, and large diameter. Lentigo maligna melanoma is most often found in the head and neck. Sometimes other areas are affected, too. The danger of this type of melanoma is that oftentimes it is mistaken for discoloration called melasma, freckle, age spot, liver spot or sun spot.

Risk factors: • Ultraviolet radiation exposure • Age • Increased number of moles/freckles • Fair skin • Sunburn or sun poisoning • Working outdoor • Genetic: about 10% of melanomas are familial The biopsy, performed by a health care professional (usually dermatologist) to diagnose lentigo maligna, does not influence patient survival or rate of metastasis. Previous concerns that incision into a melanoma promotes its dissemination have been allayed. Lentigo maligna is usually treated with surgery after biopsy has confirmed the diagnosis. Melanoma should be managed by a multidisciplinary team that includes a dermatologist, surgeon, and medical oncologist, as well as other allied health professionals. Since prevention of lentigo maligna is the best option, please do not ignore annual skin checks with your dermatology health care provider.

Dr. Small Redefines What It Means To Be Beautiful A

staff writer at Ocean County Women had a moment to sit down with board certified plastic surgeon, Dr. Stephen Small, to discuss his career and inspirations which led him to establishing his Ocean Plastic Surgery practice in Ocean County, New Jersey. OCW: What led you into the field of plastic surgery?

Dr. Small: When I was 9-years-old, I was involved in a major accident which resulted in second and third degree burns to my right arm, shoulder, chest and face. I am thankful to be alive and because of excellent care I recovered fully. While I still have scars from that injury, I could see my progression with before and after photos of my recovery and was astounded. I thought if I could help just one person, the way that I was helped, that was what I would do with my life. The person who helped me was a plastic surgeon, Dr. Richard Dabb.

OCW: What defines beauty to you ?

Dr. Small: To me, we are all naturally beautiful. The beauty we all possess is unique and can not be defined by an arbitrary look, perception of flawlessness, a measurement, or age. My perspective of beauty and passion as a plastic surgeon has been shaped by my own life experience starting with an injury as a young child. While I have scars that will never go away I look at my appearance not as having an imperfection or flaw but, rather a small aspect that has added to my character to the person I am today. Beauty is enhanced by looking and feeling our best. As your plastic surgeon my goal is to use my expertise and life passion to help you look and feel your best.

OCW: What is your educational background?

Dr. Small: I am a Board Certified Plastic Surgeon going into my eleventh year of private practice in Ocean County. My plastic surgery residency was through Ohio University at South Pointe Hospital~ Cleveland Clinic Health System. In Cleveland, I had the opportunity to train with some of the best cosmetic and reconstructive surgeons in the country at the Cleveland Clinic, Metropolitan, Case Western, Lutheran and South Pointe Hospitals. Prior to my plastic surgery residency, I had completed a burn and plastic surgery

Do You Have An Executor? Many years ago, when you and your spouse were younger, when your estate was smaller and your children were still young, someone probably encouraged you to have a will prepared because you were both taking your first extended vacation. You thumbed through the Yellow Pages (remember the Yellow Pages?) and chose a lawyer without finding out whether he or she specialized in the preparation of wills. Sound familiar? During the course of that meeting, the lawyer asked, “Who is going to take care of your estate (your executor) if something happens to you?” Most likely you named each other as executor with a friend or family member as the alternate executor. Now that many years have gone by, your estate is much larger, you have grandchildren, and your alternate executors are long gone, the decision to name your executor is more complicated. Here are some of the duties of an executor: • Collect and safeguard assets • Inventory all assets and obtain valuations • Pay valid debts and collect money owed • Carry out all of the terms of the last will in an unbiased manner • File all final federal and state tax returns • Maintain detailed accounting records If by chance you have those same old wills, it really is time to have new wills prepared, preferably by an elder law attorney. As a matter of fact, your will plan should be reviewed every few years, or when a life-altering experience occurs such as the loss of a beneficiary, a birth, a divorce, or a windfall inheritance, as examples. Before you ask someone or you are asked to serve as executor for a family member or friend, read what follows—

“Why Do You Do That?” Frequently Asked Questions During Mammography For almost 40 years, Toms River X-Ray, CT and MRI Center has been serving the community with diagnostic imaging. That includes our commitment to catch breast cancer early with screening mammograms. Our highly trained, experienced radiologists and compassionate staff as well as the latest technology has been providing this exemplary care. Over the decades we have been asked the same questions by our patients.

Q: Why do you need my old mammograms? A: Every day, we receive new patients into the Toms River X-Ray family.

Q: Why can’t I wear deodorant or powder? A: There are different types of calcifications in the breast. Some are benign and clearly not cancer. Some are

t

Many times these new patients have prior mammograms from another facility. It is extremely important to have the old mammograms for two reasons. First, breast cancer can be very subtle and difficult to identify. This is especially true for patients with dense breasts. The dense breast tissue can obscure the cancer, making it difficult to see. Having prior mammograms, Cyrus Khorrami, MD radiologists are able to see any subtle changes of the breast from year to year. This allows us to detect a much larger number of cancers. Second, some nodules and areas of distortion of the breast look very suspicious. However, if we have prior mammograms, we can determine whether these lesions are new. If a suspicious-appearing area draws a radiologist’s attention and we find it is unchanged in appearance for years, then the chances of being cancer are tiny. This saves a patient the expense of extra testing, the anxiety of thinking she may have cancer, and the discomfort of having a biopsy. Prior mammograms are invaluable to a radiologist when interpreting mammograms. Please make every effort to obtain old mammograms. It helps us all towards our common goal of detecting breast cancer and giving the best possible care to our patients.

In these cases, we need the patient to wash off the deodorant or powder and return to repeat the mammogram. Q: Why do you place stickers around my moles? A: Some people have moles and beauty marks on their breasts. These skin lesions can potentially look like

cancer on mammograms. If these moles are marked for the radiologist, they will not be mistaken for a developing cancer.

Q: Why do you place a BB on my nipples? A: This does not occur in all patients. If nipples are imaged in a perfect profile, then placing a BB is not

necessary. However, some nipples are inverted. Other nipples may be imaged not in a perfect profile due to the patient’s anatomy. In these instances, the nipple can look like a cancer on mammograms. The placed BB helps the radiologist not to confuse an off-center nipple with a developing malignancy.

Q: Why can’t I use creams or moisturizers before an exam? A: For the best examination, it is important to have the mammogram grip the entire breast and apply firm, even pressure. Wearing creams or moisturizers can cause the breast to become slick. The breasts can slide out of position when compression is applied and the breast is not imaged adequately.

How To Change An Irrevocable Trust

Written by Christine L. Matus Esq. When dealing with irrevocable trusts, the ability to effect change can be difficult to understand, presenting more questions than answers. The correct answers often depend on a variety of factors, but a good starting point is state law and the trust document itself. When modification or termination of an irrevocable trust is sought, a possible mechanism is for the trustee or beneficiary to seek a court order. When it comes to interpreting language in an irrevocable trust, appointing a trustee or providing directives to a trustee, a non-judicial settlement agreement may be an alternative to filing a court proceeding. In some states, trustees, heirs, spouses and beneficiaries are among those permitted to enter into a binding non-judicial settlement agreement, so long as the terms do not violate a material purpose of the trust, the terms and conditions could otherwise be properly approved by the court, and any modifications sought are not already provided for in the trust.

Some states also allow trust “decanting” as another means for modifying irrevocable trusts. A trust decanting involves a trustee’s exercise of discretion to distribute trust assets to another trust with dissimilar terms. The ability to decant irrevocable trusts depends on state law and varies from state to state. Even if the trust is irrevocable, it may still be possible to carry out a change in who receives estate assets upon the death of the trust creator through exercising a power of appointment. A power of appointment is

created when one person grants another the authority to dispose of property by designating a recipient of that property. It is not unusual for the creator of a trust to have assigned the surviving spouse or trustee a power of appointment. If this power was granted, and the requested modifications of the trust involve disposing of property or changing a designated beneficiary, exercising a power of appointment could be a means to that end. Some states also allow trust “decanting” as another means for modifying irrevocable trusts. A trust decanting involves a trustee’s exercise of discretion to distribute trust assets to another trust with dissimilar terms. The ability to decant irrevocable trusts depends on state law and varies from state to state. Apple Through Pre Trial Intervention (PTI) Redeemed Them

his article serves as a general overview regarding the Pre Trial Intervention (PTI) program. PTI is a supervisory treatment program that was enacted prior to the New Jersey Criminal Code Title 2C being codified. It can be found in NJSA 2C:43-12. The goal is “it is policy of the State of New Jersey that supervisory treatment should ordinarily be limited to persons who have not previously been convicted of any criminal offense under the laws of New Jersey, or under any criminal law of the United States.” The law has gone through many modifications over the years and is constantly changing. When a person is charged with a crime, or in some instances an offense (domestic violence disorderly persons offense), that person may be eligible for supervisory treatment in PTI depending on a panoply of variables. These variables include the PTI Director (court personnel) and their willingness to accept the person into the program, the prosecuting agency and their willingness to accept the person into the program, the charge itself, a victim’s input if applicable, and many other considerations. If a person has received a prior PTI, a conditional discharge or a conditional dismissal, he or she is ineligible for this type of supervisory treatment. NJSA 2C:43-12(g)(1). Ordinarily, when you are accepted into the PTI program, you DO NOT have to plead guilty to the charge(s) subject to certain exceptions found in NJSA 2C:43-12(g)(3). The accused should consult a qualified criminal defense attorney immediately and make a timely application so as not to delay or prejudice their rights. The fee for the application is $75 and almost always no statement that is made to court personnel can be disclosed to the prosecuting agency nor can it be used in a subsequent civil or criminal proceeding. The maximum term limit that a person can be supervised in PTI is thirty-six (36) months. If a person does well, and upon approval of the program director, the prosecuting agency and its consent, and the court’s approval, the term may be shortened or even terminated early. If a person violates the terms and conditions of PTI, dismissal from the program can occur. Again, hire a qualified criminal defense attorney to motion to the court for remaining in the program. If the term is completed successfully, the charge(s) are

dismissed. However, the charge and/or arrest will still remain on a person’s record and they have to wait six (6) months in order to expunge it. NJSA 2C:52-6(c). Additionally, if a person is rejected from the program, a PTI appeal can be filed arguing that there has been a “patent and gross abuse of discretion” in rejecting the individual. An example would help illustrate how the process works. On January 1, Mr. X gets arrested at a local department store for shoplifting $650 worth of merchandise. He ultimately gets caught on surveillance video and the local police arrest him and charge him with third degree shoplifting, a crime with a maximum punishment up to 3-5 years in state prison with various fines and assessments. He is released on a summons (he has no prior record, lives locally, has no other pending charges and was born in the United States). He hires a qualified criminal defense attorney and makes a timely application for the PTI program. The program director agrees to let him in and the prosecuting agency is at first hesitant due to the department store and its concern of recent spikes in shoplifting, but nonetheless acquiesces. Mr. X comes to court on April 1 on an accusation (waives his right to have this matter presented to a grand jury) and the court admits him into the program for a period of 12 months with conditions, such as making restitution to the department store and keeping away from that department store. Mr. X finishes the program in the requisite time frame and the indictable (felony) charge is dismissed. Mr. X then waits until October 1 (6 months after) and moves to expunge his arrest, with which he is successful, and thus avoids traditional prosecution and a criminal record. As a famous proverb tell us: “Everyone makes mistakes in life, but that doesn’t mean they have to pay for them the rest of their life. Sometimes good people make bad choices. Dr. Peters OCW: What do you find most satisfying about your profession? Dr. Peters: Being a physician and in particular, an Otolaryngologist, continues to be a very gratifying profession. Helping my patients conquer simple, to sometimes life-threatening situations is truly compelling and satisfying. Beyond anything else, a patient’s heartfelt gratitude strengthens my lifelong commitment to this profession. OCW: What attracted you to the field of Otolaryngology? Dr. Peters: As a third year medical student we had the privilege of starting our clinical rotations. My second rotation was in otolaryngology where I participated in complicated head and neck surgeries. The intricate anatomy of the head and neck was most intriguing to me. I quickly began to develop a passion for this area of sub-specialization and went on to do an otolaryngology residency. OCW: What is the most challenging aspect of your job? Dr. Peters: The most challenging aspect of otolaryngology is without question the difficult airway. There have been many occasions in which lifethreatening emergency measures are necessary in order to reestablish a patient’s upper airway patency. OCW: What are the most common conditions that you treat? Dr. Peters: Our practice treats a broad spectrum of ear, nose, and throat disorders, anywhere from nasal and sinus disorders, chronic ear conditions, and throat tumors, to the dizzy patient. Personally, I have a special interest in skin cancer reconstruction and thyroid surgery.

OCW: How do you stay current with new developments made in your field? Dr. Peters: In order to maintain high quality of care for our patients, I must keep abreast of the latest technologies and techniques in our field. I have found in my personal career the most beneficial means in which to achieve this is by attending the American Academy of Otolaryngology orchestrated meetings and conferences throughout the country. Additionally, I participate regularly in our American Academy of Otholaryngology Head and Neck Surgery’s Home Study Course. OCW: Please share one of your most memorable success stories. Dr. Peters: Although it’s very difficult to pick just one particular case or patient, the situations in which an emergency tracheostomy is necessary in order to save the patient’s life from an airway obstruction is always memorable. Although infrequent, these intense situations are extremely gratifying. Just a few weeks ago, I was involved with an unfortunate 65-year-old woman with metastatic breast cancer to the brain and she developed paralysis of both vocal cords. She was suffering tremendously in the emergency room and required insertion of the tracheostomy tube while awake under local anesthesia. She did amazingly well and couldn’t be more grateful. I wish her all the best.

Dr. Kupferberg OCW: What do you find most satisfying about your profession? Dr. Kupferberg: I love that this specialty allows me to help patients with many different types of diseases involving the ear, nose, and throat. OCW: What attracted you to the field of Otolaryngology? Dr. Kupferberg: I was initially attracted to the field when I was in Medical School. I realized then that it allowed physicians to treat a large variety of diseases

OCW: What is the most challenging aspect of your job?

Dr. Gillespie: After my third year of medical school I realized the two rotations I enjoyed the most were my surgical rotation and pediatric rotation. By being an otolaryngologist, I have the opportunity to take care of both children and adults for a variety of problems that affect the head and neck.

Dr. Kupferberg: There are many challenges that are constantly evolving. We maintain a very high standard of care for all of our patients and that is often made more difficult by the restraints placed upon us by insurance companies and governmental regulations. It is a constant battle to keep up with the continuously growing new regulations placed upon the medical field.

OCW: What is the most challenging aspect of your job? Dr. Gillespie: Dealing with insurance companies and all of the different rules and regulations can be difficult. We spend a lot of time getting medications, radiologic studies, and procedures approved by patients’ insurance companies. The majority of our patients we can help. However, occasionally we will hit a roadblock where an insurance company will not cover something.

OCW: What are the most common conditions that you treat? Dr. Kupferberg: We treat a variety of conditions with both medical and surgical intervention. In children, placement of tubes in the ears to help with ear infections and removal of the tonsils and adenoids are quite common. In adults, treating the nose to improve breathing and the sinuses to improve drainage and lessen infections are also frequent procedures.

OCW: Please share one of your most memorable success stories. Dr. Kupferberg: When a patient comes into the office with a longstanding history of sinus disease and the inability to breathe through their nose, they are often frustrated and unhappy. I recall operating on such a patient who also suffered from chronic sinusitis and nasal polyps. I performed a septoplasty, turbinate reductions, endoscopic sinus surgery. This opened up the breathing passages and allowed his sinuses to drain more efficiently. This resulted in the ability to breathe better and have less sinus infections. The surgery improved his overall quality of life.

OCW: What are the most common conditions that you treat?

Dr. Gillespie: The most common conditions I treat affect both children and adults. For children, I treat lip-tie, tongue-tie, recurrent ear infections, hearing loss, nasal obstruction, tonsillitis, and obstructive sleep apnea. For adults, I treat nasal obstruction, obstructive sleep apnea, chronic sinus problems, chronic tonsillitis, deviated nasal septums, hearing loss and cerumen impactions. I take care of people across all age groups. OCW: Please share one of your most memorable success stories.

OCW: How do you stay current with new developments made in your field?

Dr. Gillespie: Some of my most memorable success stories are when I take care of an entire family’s ENT issues. I have one family that I have placed ear tubes in one child and removed the adenoids and tonsils in another child for sleep apnea. I have also cared for their mother for a thyroid problem. It is very satisfying to see them all back in the office doing well.

Dr. Kupferberg: I read several journals and listen to lectures on Audiodigest that are pertinent to my field. I also attend regional and national conferences to broaden my understanding of different topics in the field.

OCW: How do you stay current with new developments made in your field?

Dr. Gillespie OCW: What do you find most satisfying about your profession? Dr. Gillespie: Having the ability to help people, is the most satisfying part of my job. Every day I see patients with a variety of problems. Through either medical therapy or surgery, I can often make them better. I can help children with

Recipe: Combine sauce ingredients and set aside. Combine soy sauce, oyster sauce, ½ tsp sesame oil, rice wine and sugar in a bowl. Heat remaining sesame oil in a skillet over high heat. Add garlic and cook until golden. Add chicken and mushrooms and stir fry until browned, breaking up chicken while cooking. Add in water chestnuts and soy sauce mixture and heat through. Hemorrhoids: The Bottom Line Did you know we are born with hemorrhoidal tissue, it’s a part of our anatomy? Hemorrhoidal tissue lies within the anal canal and perianal area and consists of blood vessels, connective tissue, and a small amount of muscle. Hemorrhoids are described as varicose veins of the anus and rectum because they are enlarged, bulging blood vessels in the anus and lower rectum. The only time we actually feel hemorrhoids is when they become enlarged or symptomatic. Dr. Paonessa likes to use the analogy: “We all have teeth, but we all don’t have cavities”. Likewise, we all have hemorrhoids, but we all don’t have symptomatic hemorrhoids. There are two types of hemorrhoids, internal and external. Both can have different symptoms and often different treatments. Internal (inside) hemorrhoids form in the anus beneath a lining called mucosa, which is not sensitive to touch, pain, stretch or temperature. As such, internal hemorrhoids are commonly associated with painless bleeding and protrusion during bowel movements. It is only when the hemorrhoid completely prolapses from the opening of the anus and cannot be pushed back inside that it becomes severely painful.

Internal hemorrhoids are classified in four grades: •

Grade One: The hemorrhoid is not protruding from the opening of the anus (no prolapse). • Grade Two: The hemorrhoid protrudes from the anus but it goes back in on its own. • Grade Three:The hemorrhoid protrudes and requires the patient to push it back inside the anus. • Grade Four: The hemorrhoid cannot be placed back into the anus by the patient. This is often very painful. External (outside) hemorrhoids form around the anus and are covered by very sensitive skin. The most common complaints associated with external hemorrhoids are occasional swelling, pressure or discomfort. When a hemorrhoid is symptomatic it becomes a bluish-colored, painful lump outside the anus. If the external hemorrhoid develops a clot (thrombosis) it becomes a hard, painful lump. When the pressure becomes elevated it can cause the thrombosed hemorrhoid to break down the overlying skin and the clot begins to leak out. Even after the hemorrhoid has healed some patients complain about soft tissue outside the anus called an anal skin tag. An anal skin tag is the residual effects from the thrombosed external hemorrhoid. A skin tag may hinder a patient’s ability to clean after a bowel movement. Although there is no treatment for anal skin tags, they can be removed if hygiene becomes an issue for the patient. There are several factors that can cause hemorrhoids although the exact cause is unknown. Upright posture, alone, increases pressure on the rectal veins and can cause them to bulge. Additional factors include aging, chronic constipation or diarrhea, straining during pregnancy, heredity, straining during bowel movements, overuse of laxatives or enemas, and extended periods of time on the toilet (i.e. reading). Here are some symptoms you should be aware of to determine if you may have a hemorrhoid problem. If you are having bleeding during bowel movements, itching in the anal area, pain, sensitive lumps or prolapsing during bowel movements. If you do experience any of the symptoms do not treat with over the counter medication. Call and make an appointment with your doctor. It is very important to understand that NOT all rectal bleeding is caused by hemorrhoids. As such, it is crucial to seek consultation with a specialist such as a colon and rectal surgeon to be sure that the rectal bleeding is not caused by a serious disease such as anal or colorectal cancer. How hemorrhoids are treated depends on how severe the condition is. If the symptoms are mild your doctor may simply have you increase your fiber intake in your Board-certified in general surgery and proctology and a fellow of the American College of Osteopathic Surgeons, Dr. Paonessa specializes in colon and rectal surgery. She has been in practice for 13 years. She completed her colon and rectal surgery fellowship at Lehigh Valley Hospital in Allentown, PA. She is co-editor of the textbook Surgical Treatment of Hemorrhoids, 2nd edition. Dr. Paonessa believes in patient-centered care and preserving each patient’s independence, well-being and dignity. diet (i.e fruits, vegetables, breads and cereals) or by adding an oral fiber supplement. It is recommended that you consume 20-35 grams of fiber per day. It is very important to make sure that you drink 8-10 glasses per day of water. Other beverages that contain caffeine or alcohol will dehydrate you so try to decrease your intake of those beverages. These measures help to create a soft, formed stool thereby eliminating excessive straining during bowel movements. This in turn will decrease the pressure on the hemorrhoids. The use of warm water soaks (sitz baths) will also provide relief. There are several outpatient treatments that may be performed in the office to help treat symptomatic hemorrhoids. These include: rubber band ligation, infrared photocoagulation and sclerotherapy. All of these procedures are painless. Dr. Paonessa’s procedure of choice in the office is sclerotherapy. Sclerotherapy involves an injection of a chemical irritant into the hemorrhoid which causes scarring and shrinkage of the blood vessels present within the hemorrhoidal tissues. This procedure is painless and has few, if any, complications. Sclerotherapy is safe for patients on blood thinners. As with any procedure, you will need to discuss this with your physician. External hemorrhoids may also be treated on an outpatient basis as well. Excison of the hemorrhoidal tissue can be performed. The hemorrhoid will be injected with a numbing agent and excising of the hemorrhoid along with all blood clot (thrombus) is done. In this case the entire external hemorrhoid is removed not lanced, so as to avoid any residual skin tag once healed. This procedure provides instant pain relief for those patients suffering from severe pain. Often hemorrhoids that are not thrombosed can be treated with dietary change and sitz baths. There are other procedures that can be done on an outpatient basis at surgery centers or hospitals. These include: traditional hemorrhoidectomy, Transanal Hemorrhoidal Dearterialization (THD), and stapled hemorrhoidectomy. As with any surgery you will need to have your physician evaluate your condition and see what the best option for you is. With advances in local anesthetic and surgical technique, hemorrhoidectomy is not the painful procedure it had been associated with in the past. Menopause: What to Expect Adam Repole, MD, FACOG, Shares Perspective Night sweats, hot flashes, and skipped periods are all common symptoms leading up to menopause. This time in a woman’s life is filled with fluctuating hormones and a laundry list of what can be uncomfortable symptoms. The good news is that it’s temporary and there are ways to alleviate those symptoms.

By definition, menopause occurs when a woman does not have a menstrual cycle for 12 months, marking the official end of her reproductive period. In North America, the average age for menopause is 51, however, Dr. Repole sees women younger and older as female family history is a factor.

The length of perimenopause – the time period before actual menopause when a woman experiences symptoms – varies from a few months to a few years. Other symptoms may include: decreased libido, vaginal dryness, changes in menstrual flow, breast tenderness and mood swings.

“My role as a gynecologist is to reassure my patients that perimenopause is a normal transition to menopause – and there’s help for them. During this stage, they may experience a unique set of symptoms – like diminished libido and pain or dryness with intercourse – but they don’t need to suffer in silence,” says Dr. Repole. Dr. Repole notes that various forms of low-dose hormone replacement therapy or low-dose contraceptives help alleviate symptoms of perimenopause. The American Congress of Obstetricians and Gynecologists (ACOG) advocates regular breast self-exams and screening mammograms. During well visits, other tests, like bone density, are performed too.

Diagnosed With Breast Cancer:

What’s Next?

October is Breast Cancer Awareness Month

The biopsy shows that you have breast cancer. In addition to the shock and emotional turmoil, a thousand questions go through your mind. “The surgeon said that he took everything out, so why do I need chemotherapy?” “What are my chances?” “Am I going to die from cancer?” “My friend only needed to take a pill, why am I not getting that?” Not everyone diagnosed with breast cancer is treated the same because no two tumors are alike. Even after the lump in the breast is removed surgically, there is a risk of cancer growing back in the same breast or spreading to other organs, a process called metastasis. The risk of metastasis is based on a number of factors. Age, menopause, the stage of the cancer and characteristics of the tumor cells (grade, hormone receptors called ER and PR, and Her-2) all determine the risk of relapse or metastasis. Age: In general, young women tend to have more aggressive tumors and are more often treated with chemotherapy than older women. Stage: Breast cancer is staged from Stage 0 to Stage IV; Stage 0 is pre-cancerous and is also known as carcinoma in situ (DCIS or LCIS). Stage 0 cancer does not metastasize and does not require chemotherapy. Stage I is a small tumor (<2cm) confined to the breast; when the tumor has spread to the lymph nodes or if the tumor is >2cm it is Stage II. Stage III includes tumors that have spread to lymph nodes in the neck, have affected more than 4 lymph nodes, or have spread to the skin and chest wall. When the cancer has metastasized to other organs (commonly to the bones, lungs or liver), it is Stage IV. The higher the stage, the higher your risk is of relapsing; chemotherapy, hormonal therapy or both are recommended to reduce that risk. Tumor Characteristics: Certain features on the cancer cells can predict the aggressiveness of the cancer. Cancer cells are graded from I to III, grade I being the least aggressive and grade III being the most. The tumor cells are also tested for hormone receptors on the surface of cancer cells, called ER and PR (estrogen and progesterone). Tumor cells are also checked for a protein called Her-2. About 25-30% of all breast cancers are Her-2 positive. Her-2 positive patients are treated with a drug called Herceptin. Who Needs Chemotherapy? Even if the visible tumor is removed surgically, you may have cancer cells floating around in your system that can deposit in different parts of the body. This can happen a few months to several years later, producing metastasis. Research has shown that this risk can be reduced by giving chemotherapy for a few months. In addition, hormonal therapy and Herceptin are used in appropriate patients. After surgery, your surgeon will refer you to a Medical Oncologist who will then determine the kind of treatment you may need. Women who have very early stage breast cancer may not need any treatment and should be observed periodically. Most women with ER and PR positive tumors are treated with Tamoxifen (before menopause), or Arimidex (Anastrazole) or Femara (Letrozole) (after menopause). In addition, your medical oncologist may advise you to get chemotherapy based on the other factors mentioned above. The most commonly used chemotherapy drugs are Adriamycin, Cytoxan, Paclitaxel (Taxol), Docetaxel (Taxotere), and 5 Flurouracil (5 FU). The type of chemotherapy and duration used are also based on the risk of metastasis. Chemotherapy is given for approximately 4 to 6 months, starting about 3 to 4 weeks after surgery. Chemotherapy is often administered as an intravenous infusion. To minimize damage to the veins and to ensure proper administration, a device called a port-a-cath can be inserted below the collarbone. This is temporary and can be removed after treatment is completed. Side Effects Chemotherapy can cause nausea, fatigue, vomiting, hair loss, mouth sores, low blood counts and numbness in the extremities. These are all reversible and improve once treatment is discontinued. With the use of newer medications, nausea and vomiting can be prevented considerably. Breast Cancer Survivorship: Sumy H. Chang, MD, FACS, Sheds Light on What Happens After Treatment While breast cancer is the most frequently diagnosed cancer in women, the good news is that survivorship is on the rise. According to Sumy H. Chang, MD, FACS, a fellowship-trained, board-certified breast surgical oncologist, “Following breast cancer treatment, many women face challenges transitioning back to their lives. They want to know what to expect and what steps to take on the road to recovery.” Dr. Chang is a provider with Barnabas Health Medical Group (BHMG), and is affiliated with Community and Monmouth Medical Centers, RWJBarnabas Health (RWJBH) facilities. She notes that during the post-surgery period, there is risk of breast cancer recurrence, depending on the patient’s initial tumor. “Monitoring your health and reporting any changes to your doctor is critical for survivorship,” she says. Keep these points in mind posttreatment: Hormone blocking treatment: After surgery, most women take hormonal therapy medicine to reduce the risk of recurrence. Tamoxifen, the most popular drug taken for five years after breast cancer surgery, is used to treat premenopausal women. According to a recent study, women with hormone-

receptor-positive breast cancer who took hormonal therapy after surgery had a lower risk of breast cancer in the opposite breast. Aromatase inhibitors are taken by post-menopausal women for five to ten years after their breast cancer treatment. Surveillance: Women should report any changes in breast health to a physician. A breast self-exam (BSE) forces women to pay attention to how their breasts typically look, so they’ll be more aware of any irregularities. Many patients experience discomfort at the surgical site or the entire breast. Common changes following treatment include new lumps or bumps, aches and pains, and numbness or tingling. Although some of these changes may be side effects of surgery, chemotherapy and/or radiation, they should still be reported to a doctor. Follow up mammograms: Stick to a routine of annual mammograms along with two to four breast exams a year by a provider, such as a medical or radiation oncologist. Skin changes: Some women may experience long-term changes from radiation, including skin color or tone, and thickness. Pores may be a little more obvious, too.

Survivorship on the Rise The American Cancer Society reports that breast cancer death rates declined by 36 percent from 1989 to 2012 due to improvements in early detection and treatment, avoiding approximately 249,000 breast cancer deaths. Reconstruction: Depending on when cancer treatment is completed and the type of reconstruction the patient chooses, it can take a woman a full year to complete her reconstruction. Healthy lifestyle: It is important to stay active, but some women may need to come to terms with their “new normal” based on the type of cancer treatment they had. “The best thing is to learn to adjust,” says Dr. Chang. Back To School Time Already? Health Problems That Spread At School Kids should learn to share, right? But, gee whiz…. When it comes to bugs and other contagious health problems in school, does your kid have to get everything that’s passed around? Whether kids are packed into a classroom or messing around on the playground, they do tend to share lots of health problems in school. What can you do about it? And when should you be concerned? Here are a few things to think about. Does head lice top your list of concerns? Although these creepy insects might disgust you, it may help to know that they don’t cause diseases or other health problems – other than maybe a red, rashlike reaction. Of course, that doesn’t mean you want to ignore them since they spread really easily. Follow up with the doctor if your child complains of an itchy scalp or you catch sight of tiny white eggs firmly attached at hair roots. These are often confused with dandruff. Your child’s doctor may prescribe a treatment and other over-the-counter (OTC) shampoos or rinses. Follow directions closely and be sure to ask me if you have any questions.1 Viral Infections And Other Diseases Viral infections such as chicken pox are common, too. Many are contagious before skin lesions appear. Be sure to keep your child home until the sixth day after the rash appears unless all lesions are dry and crusted over. To prevent this infection, have your child vaccinated.1 Fifth disease is another viral disease. It causes a lacy rash on arms and redness on cheeks that looks like the result of a well-placed slap. Unless your child feels too sick, there’s no need to stay home. That’s because the disease only spreads before symptoms appear. However, tell the school so female employees of childbearing age can be notified. Fifth disease can severely hurt a developing fetus.1 Although there are many kinds of hepatitis, hepatitis A is the most

common type in children. This virus is in blood and bowel movements, so hand washing is really important to prevent its spread. A child with hepatitis A should stay home until a week after the onset of illness and until any jaundice (yellowed skin) disappears. Another disease spread through bodily fluids is HIV/AIDS. Although it can cause anxiety among parents, remember that casual physical contact – such as hugging, holding hands, or sharing a glass – does not transfer this virus.1 Colds And Flus Then, there’s the run-of-the mill colds and flu. Deciding whether or not to send your child to school can be a challenge. General rule of thumb? If there’s a fever, keep ‘em home – until the fever’s been gone for at least 24 hours.2 If there’s no fever, more than likely it’s a cold and it’s okay to go to school. When in doubt, check with your child’s doctor. And, don’t forget the flu vaccine, which is recommended for everyone 6 months and older.3

Assisted Living 56 Questions To Consider When Moving A Parent Into Assisted Living Part 2 Deciding to move a parent into assisted living is a big decision – and one that is best done by making numerous small decisions. In the July/August 2017 issue we focused on four aspects in order to make your parent’s move to a new living arrangement as smooth as possible. To begin, making a list of questions is the most important. Checking out local and national referrals along with public services through your state’s Department of Human Resources can be helpful in finding a good place for them to live. To understand how to pay for it, you will need a clear understanding of your parent’s financial resources. To continue, we will focus on three more aspects in order to make your parent’s move to a new living arrangement quite simple. What medical, emotional, financial and family resources will my parent and I need? Be proactive! Dealing with medical paperwork, coordinating schedules, pulling together resources in ways that will be a stretch because you have never faced this challenge before is rough enough. You will also be dealing with the emotions that inevitably arise when you have to deal with what is likely the final chapter of a loved one’s life. It is easy to get a little overwhelmed, and that is okay, but you will be more effective in accomplishing what you need to get done if you try to stay on top of things. Get a flexible file, or use whatever file system works for you, and collect all medical records and all drug information (name of drug, purpose, dosage). Keep these updated and take them with you when your parent is examined, evaluated, when a new doctor is seen, and when new drugs are prescribed. If your parent has someone else take them for doctor visits, be sure they have a copy of this information as well so it travels with your parent. Join a caregiver group. To find one, call your local senior center, church, or local assisted living communities. The aging of the baby boomer generation will provide you with plenty of chances to find good company on this new journey in your life. Caregiver groups can be an invaluable place to get advice and emotional support. Help your parent stay engaged with family and peers, even if it’s a challenge. Have family take turns calling and visiting. Try to coordinate visits and calls with friends and neighbors. Be sure that wills, living wills and power of attorney are in order. Get as much information as you can about bank accounts, insurance policies, last wishes, names of

doctors, lawyers and ministers. This is often a difficult subject for parents and children; in our family we approached it by having the adult children discuss and take care of these tasks at the same time as our elderly parents. Engage as many family members as you can to work through this process. Too often things get dumped on a dutiful daughter or son while the rest of the family looks on. Even if everyone doesn’t agree on what is done, everyone should be heard and involved. If there is extreme disagreement in the family, you may want to have an elder care attorney or minister mediate. WHAT CAN THEY TAKE WITH THEM? Spring Oak Assisted Living allows you to personalize the apartment you choose. We have no restrictions on wall hangings or mounted flat-screen TVs. We also allow pets. This all depends on where they’re going and how much living space they will have. Take favorite pictures and other small treasured things that may be decorating their walls; familiar things can make them feel more at home more quickly, and it gives them things to talk about with their new neighbors. Make a floor plan of their new residence on paper or on the computer and do a scaled layout of their furniture. Then decide what they will take and what to do with the excess so it’s not in your basement or an expensive storage facility for years to come. Some communities allow very small dogs and cats; most do not. Ask the community for a list of do’s and dont’s. HOW MUCH MIGHT THEIR LIFE CHANGE? Will You Speak Up For A Child In Need? Abused and neglected children need you to be their voice There are children living in our community that have no one to speak up for them. They are the over 700 children residing in Ocean County’s foster care system. When parents are unable, unwilling or unfit to care for a child, the child is removed from the home. The time they spend in foster care is often confusing, scary and, unfortunately, leaves them feeling isolated and alone.

No child should ever feel that they are alone and without a voice.

This is where a CASA volunteer comes in. A CASA volunteer is a specially trained advocate, appointed by a judge, who fights for the rights of children during the most vulnerable time in their young lives. They are a trusted adult empowered to ensure each child is well cared for and to help ensure the child finds a safe, loving, permanent home.

But nearly 90% percent of the children in our community who need a CASA volunteer do not have one. Over 700 children in Ocean County don’t have the hope a CASA volunteer can offer. Every foster child deserves a CASA volunteer. Stand up for these children. Stay connected as we fight to ensure every child has a safe, permanent, loving home.

Furthering her studies, she has completed the 500-hour level of training at the Himalayan Institute in Honesdale, PA. Her teaching style is mindful with emphasis on alignment and breath. She has been teaching at the Freehold Yoga Studio Center since 1994, which is a very special place for her, and also teaches at Wall Township municipal building and Central State Wellness Center.

Sheri Glick has 25 years of experience in the health and healing field. She has always been curious about the body/ mind connection and ways to restore, nourish and heal from the inside out. She is a 500-E-RYT with Yoga Alliance and has taken thousands of hours of training throughout the country in yoga, therapy and hands on healing. She is a level 4 Reiki Master and has certifications in Neuromuscular Therapy and Cranial-Sacral Therapy. She is a licensed Massage Therapist, providing her clients with hands on treatments and one-on-one yoga therapy. She received her 200-hour training with the Freehold Yoga Studio and was so inspired, she continued on her yoga journey with a 300-hour training at the Phoenix Rising Yoga Therapy School and an additional 500-hour of training in integrated yoga

therapy through the Urban Zen Foundation in New York City. Her most recent training was 300-hours with Rodney Yee and Colleen Sandman Yee in Sag Harbor, New York. She is also certified to teach mediation through Integral Meditation. Sheri uses aromatherapy with most of her private sessions, as she views essential oils as “magic in a bottle.”

Upon finishing her 200 hour Yoga Teacher training from Freehold Yoga Center in 2011, she put her skills to use by helping students with health issues and physical limitations. Training as a Reiki Master and other healing modalities has brought more to her teaching practice. Presently, she teaches at Chelsea Assisted Living, SCAN at Monmouth Mall, and Freehold Yoga Center, in addition to private and group sessions from her home. She furthered her training in Children’s, Restorative, Chair and Yin Yoga as well as Yoga for Everyone.

Jennifer Scamorza Jennifer Scamorza is the director of Yoga University, a program designed to bring yoga and mindfulness practices to schools, businesses, and events. Jennifer is passionate about creating a positive and productive learning environment for students and professionals. She has fifteen years of experience in the NJ public school system including classroom instruction, coaching, curriculum development, and team leadership. Marianne has been with the practice from its inception in 1997. Twenty years ago, Marianne joined our then new pediatric practice and has been influential in the evolution of Ocean Pediatric Dental Associates into the largest pediatric group in Ocean County. She is our office manager and is affectionately referred to as our Mama Mare. Marianne is part of the heartbeat of our practice. Virginia A. Mensah M.D., FACOG, Reproductive Science Center of New Jersey Dr. Virginia Mensah is a specialist in Reproductive Endocrinology and Infertility. Born in Indiana but raised in New Jersey, she always felt a strong connection to the Garden State. She proudly completed her bachelor’s degree in Genetics and Microbiology at Rutgers University. She then received her medical degree from the University of Illinois at Chicago and went on to complete her Obstetrics and Gynecology residency at the prestigious Johns Hopkins Hospital. She completed her fellowship training at the Women and Infants Hospital of Brown University, with top specialists providing infertility care. Rebecca Kolas, Certified Lice Technician and Owner, Lice Lifters of Ocean County

Rebecca Kolas is both a Certified Lice Technician and Owner of Lice Lifters of Ocean County in Toms River, which opened in August 2016 and serves Ocean, Monmouth and Atlantic Counties as the premier lice removal company. Since that time she has opened a second location, Lice Lifters Central Jersey in Hamilton which serves Mercer, Middlesex, and Burlington Counties. Rebecca was introduced to the Lice Lifters franchise when her two youngest children contracted lice. Lourdes Laraya-Cuasay, MD, FCCP, FAAP, Barnabas Health Medical Group

Fellowship-trained pediatric pulmonologist Lourdes Laraya-Cuasay, MD, is a board-certified pediatrician who has been providing compassionate, comprehensive care to children with pulmonary disorders for more than four decades. Dr. Laraya-Cuasay chose to specialize in pediatric pulmonology to help children and infants struggling with breathing problems, such as asthma or inherited lung disease. Pediatric pulmonology, a new field at the time, gave her an opportunity to become a pioneer in this subspecialty. Dr. Laraya-Cuasay completed a fellowship in child growth and development at Children’s Hospital of Philadelphia, and a second fellowship in pediatric pulmonology and cystic fibrosis at St. Christopher’s Hospital for Children in Philadelphia. She served as director of the Pediatric Pulmonary Division and Cystic Fibrosis Center at the University of Medicine and Dentistry of New Jersey-Robert Wood Johnson Medical School and Robert Wood Johnson University Hospital. Dr. Laraya-Cuasay is a fellow of the American Academy of Pediatrics and the American College of Chest Physicians, and has written several articles on pediatric pulmonary subjects. Pain Free After 10 Years Out-patient procedure provides relief where other treatments have failed.

Florence Fialkowski, a grandparent from Toms River, has always enjoyed an independent, active lifestyle. Even after retiring from her job as a secretary at Fairleigh Dickinson University in 1986, Florence has always maintained her busy, on-the-go routine. Incredibly, Florence has also suffered for nearly 10 years with chronic back pain. “I never had any accident or injury to my back, it just slowly started to hurt more and more every day. The pain eventually grew so strong that I realized that I needed help. So I went to my primary care physician who sent me for an MRI.” The MRI of Florence’s spine showed severe spinal stenosis. Her doctor explained that it is a narrowing of the spinal canal—the area the spinal cord and nerves travel through. When this area becomes narrower, the spinal cord and nerves can get pinched and squeezed causing pain. Florence’s physician put her on a regimen of Tylenol and physical therapy. After a few weeks and no noticeable relief in symptoms, her physician suggested she consult with Dr. Dharam Mann at Garden State Medical Center. Dr. Mann specializes in interventional pain medicine, a field where physicians are specialty trained to treat patients with acute and chronic pain. The leading cause of spinal stenosis is wear and tear on the spine due to aging,” says Dr. Mann. “In fact, the most common direct cause of spinal stenosis is osteoarthritis, where the cartilage that cushions joints starts to degenerate due to age.” This meant that Florence’s condition was irreversible, and treatment would be geared towards treating the symptoms of her condition, to help manage her pain. Florence and Dr. Mann discussed several different treatment options and Dr. Mann began treating Florence’s pain with epidural steroid injections. These types of injections are commonly used for her condition because the steroids are able to reduce spinal cord inflammation which relieves the pressure on the nerves, alleviating the pain. However, the amount and length of relief varies among patients and after a few rounds of treatment which provided only a few weeks of relief, Dr. Mann suggested a minimally-invasive treatment option called a spinal cord stimulator (SCS). “A spinal cord stimulator is an implantable system that delivers electrical impulses to nerves in the spinal cord,” says Dr. Mann. “These impulses are able to mask pain signals before they reach the brain. The device is proven to provide significant long term pain relief and reduce or eliminate the need for pain medications. As an intervention for chronic back and/or leg pain, spinal cord stimulation can be an effective alternative or adjunct treatment to other therapies that have failed to manage pain on their own.” Florence had her spinal cord stimulator implanted in April of 2014. The procedure was performed in Dr. Mann’s out-patient surgery center, and Florence was discharged within an hour. Despite minor soreness near the implant site, Florence reported 100% back pain relief since having her procedure.
