Getting To The “Core” Of Low Back Pain By Kathleen Groome, PTA of All-Care Physical Therapy Center, Brick, NJ • 732-451-0010 What is a “core?” Your abdominals, or “core” as some people like to call them, are actually a group of muscles known as the rectus abdominis, external and internal obliques, transverse abdominis, multifidi, diaphragm, and pelvic floor muscles. The easiest way to think of your core is to think of a box: the roof is the diaphragm, the walls are multifidi and transverse abdominis, and the floor is the pelvic floor muscles.
Why are they important? Without having a strong base of support or core, the body must rely more heavily on other muscles due to imbalances. For this reason these muscles are often known as your stabilizers as they allow the rest of your body to move properly. Simple everyday activities such as walking, lifting, bending over to tie your shoes or playing sports may become painful to complete.
I have a six-pack so doesn’t that mean I have a strong core? Not exactly. A six-pack looks aesthetically pleasing; however, unless you train your inner abdominal muscles, it is only a matter of time before you end up with an injury. In fact the six-pack does not involve the entire core; it is mostly comprised of the rectus abdominis. The rectus is unable to support itself without assistance from other muscles located in your abdominal cavity. It can even compress your spine and cause pain if overworked.
I suffer from back pain, can I still work on my core? Yes, you can. If you suffer from back pain, your doctor may recommend you see a physical therapist. Physical therapists are licensed professionals who are medically trained to help patients decrease pain, improve strength, increase mobility, and improve function. A physical therapist can evaluate your symptoms and design a program for you which will allow you to strengthen your core without causing back pain, while preventing injury.
breathing. As with the exercise mentioned above, it is designed to strengthen your inner stabilizers. To start the exercise, you can either lie down or sit upright in a chair with your feet planted firmly on the floor. Place one hand on your abdomen and the other on your chest. Breathe in slowly and deeply through your nose and allow your stomach to rise, then blow the air out of your mouth as if you are blowing out a candle. Do NOT hold your breath. Diaphragmatic breathing is very relaxing, and easy to complete with practice. In conclusion, back pain can result from weakness of the abdominals and back muscles. This weakness causes the back to work harder, which results in back pain. As a result of core strengthening, the back is able to increase its flexibility, which it needs to support the spine.

I want to start a core exercise program, but I don’t know where to start. It is recommended that you consult a professional before beginning any exercise program. If you would like to strengthen your core or inner abdominal stabilizers, let’s first start with the basics by strengthening two important muscles: the transverse abdominis (TA) and diaphragm. Lying down on your back with your knees bent, gently tighten your stomach in towards your belly button. Hold for five seconds without holding your breath. You can repeat this 10 times. This exercise isn’t easy; however, it is extremely important as these muscles are being contracted when we lift or move. You should be able to hold a conversation and contract your abdomen at the same time. Another exercise is called diaphragmatic

She believes in using a variety of techniques which includes manual techniques, joint mobilizations, PNF, muscle energy techniques as well as Regional Interdependence (using movement patterns to identify impairments which may be unrelated to the main complaint, however they contribute to the disability). n a weekly basis, I have a conversation with a family caregiver or someone who is receiving care from a family member or friend. As you would expect, the conversations focus on a legal aspect such as legal documents needed or Medicaid planning. What may come as a surprise is that the conversations often touch upon the impact caring for the individual has upon the caregiver. An estimated 40 million Americans serve as caregivers for their spouse, parent, grandparent, relative or neighbor. Everyday these caregivers provide assistance with many basic needs such as dressing, bathing, cooking, medications or transportation. Often, they do some or all of this while working, parenting, or both. Generally, they are unpaid for their services. Research shows that family members who provide care to individuals with chronic or disabling conditions are at risk to experience their own health issues. Family caregivers are more likely to experience negative health effects like anxiety, depression and chronic disease. It is not uncommon to hear a family caregiver express: “My husband has dementia, but now I'm the one in the hospital!” Older caregivers are not the only ones who put their health and well-being at risk. Many children have assumed the role of caregiver for their parents. These younger family member caregivers, known as the “sandwich generation” because they are responsible for bringing up their own children and for the care of their aging parents, also experience health issues. All caregivers, no matter their age, face an increased risk for depression, chronic illness and a possible decline in quality of life. Yet, despite these risks,
family caregivers of any age are less likely than non-caregivers to practice preventive healthcare and self-care behavior. Caregivers, regardless of age, report problems with sleep deprivation, poor eating habits, failure to exercise, failure to stay in bed when ill and postponement of or failure to make medical appointments for themselves.
While stressful, the job of caregiving does have many rewards. A caregiver is well served if he or she understands the resources that are available to help lessen their stress and enhance the rewards of caring for a sick family member. One such resource is an elder law attorney. An elder law attorney can be instrumental in providing guidance from ensuring that the proper legal documents are in place to giving respite leads and suggestions. Basal Cell Carcinoma Basal Cell Carcinoma, also known as BCC, is a type of skin cancer that begins in the basal cells. Appearance
• Oftentimes no symptoms • Sometimes can be itchy or painful • Non-healing lesion • Redness • Loss of color • Small bump resembling a cyst or a pimple • Swollen blood vessels in the skin • Ulcer • Pearly growth • White waxy lump • Scaly patch • On sun-exposed areas, such as the face and neck

Very common
• More than three million US cases per year • Caused by sun exposure • Diagnosed and treated by a dermatologist (since BCC sometimes resembles noncancerous skin conditions such as psoriasis or eczema) • In certain cases will need a referral to a surgeon
Prevention
• Seeing dermatologist for regular skin checks • Reporting any new mole or growth • Limiting sun exposure • Using sun screen
Treatment
Treatments include prescription creams, radiation or surgery (Mohs surgery, electrosurgery, cryosurgery, wide local excision) to remove the cancer. -Mohs surgery: Surgical removal of skin cancer, one layer at a time, until only healthy tissue is left.
-Electrosurgery: Using electric current to cut tissue. -Cryosurgery: Using liquid nitrogen to freeze superficial BCC. -Wide local excision: Surgical removal of a small area of cancerous or precancerous tissue, along with a margin of normal tissue. BCCs are easily treated in their early stages. The larger the tumor has grown, however, the more extensive the treatment needed. While BCCs seldom spread to vital organs, they can cause major disfigurement and occasionally result in nerve or muscle injury. Certain rare, aggressive forms can be lethal if not treated promptly. When small skin cancers are removed, the scars are usually cosmetically acceptable. If the tumors are very large, a skin graft or flap may be used to repair the wound in order to achieve the best cosmetic result and facilitate healing.

The Salivary Glands Written by: Dr. Stephen Kupferberg, MD, FACS There are both major and minor salivary glands. The major salivary glands are the parotids, submandibular and sublingual glands. The function of all the glands is to produce saliva to moisten the mouth, aid in digestion, and help protect the teeth from decay. The parotid glands empty into the mouth via a single tube called a duct. It is called Stensen’s duct and it is located in the cheek near the second molar of the upper teeth. The submandibular gland empties under the tongue in the front of the mouth through Wharton’s duct. The sublingual gland has many ducts in the floor of the mouth. There are multiple minor salivary glands located in the lips, cheeks and other areas of the mouth and throat. Several different problems can involve the salivary glands including obstruction, infection, tumors, and other disorders. Obstruction most commonly occurs in the parotid and submandibular glands. It may be due to the formation of stones in the gland or ducts. Strictures or narrowing can also slow salivary flow. When a person eats, the nervous system signals the gland to increase its production of saliva. This may back up and cause the gland to swell. This swelling usually decreases slowly after a meal. The most common infection in children is mumps and affects the parotid glands. This usually occurs in children who have not been immunized. Infections also happen in adults. This often occurs in the elderly who become dehydrated or with severe obstructions. This often necessitates antibiotic therapy. Tumors also occur in the salivary glands. They most commonly present as painless swellings on the side of the face or upper neck. Aggressive tumors can present with involvement of the facial nerve, causing decreased movement in portions of the face. Most tumors
are benign. Pleomorphic adenoma and Warthin’s tumors are the most common benign lesions. Mucoepidermoid carcinoma, adenocarcinoma, and adenoid cystic carcinoma are the most common malignant tumors. Diagnosis is often made by physical examination. A mass can be felt in the cheek or neck. Stones can sometimes be noted by palpation in the mouth. CT scans or MRIs are often necessary to delineate the extent or size of tumors or stones. If a tumor is found, a fine needle aspiration biopsy is often performed to figure out the pathology to guide further therapy. Small stones may be removed by sialoendoscopy. This involves removing the stones through the ducts with a small camera, graspers, baskets, or lasers. Larger stones will necessitate more extensive surgery. Tumors will often necessitate surgical intervention. Tumors involving the parotid gland will often necessitate a superficial or total parotidectomy with facial nerve dissection, as this nerve divides the gland into superficial and deep lobes. Malignant lesions may result in sacrifice of part or all of the facial nerve. Submandibular gland lesions are removed by excising the entire gland. If malignancy is noted, then radiation therapy and possible chemotherapy may be needed.

Preheat the oven to 350 degrees. Lightly grease a 9 inch dish (pie plate) and set aside. Melt the butter and saute the vegetables until they are tender crisp, adding the peas and ham at the end. Let cool slightly. In a small bowl, whisk together the eggs and milk, adding any seasonings that you like. Stir in the vegetables and pasta and then put the whole mixture into the prepared pan. Top with cheese and bake for about 30 minutes or until lightly browned. Serve with a salad and viola! Another great home cooked meal.

Doctors Agree with Dr. Seuss… “Green, Eggs and Ham” are back on the table If you’re like most, a new year brings with it many new resolutions, at the top of most people’s list is losing weight and adopting a healthier lifestyle. For most this means switching out an egg breakfast and replacing it with sugar laden “healthy cereals”, but health nuts and breakfast lovers alike can all rejoice in the latest guidelines announced by the government. Turns out Dr. Seuss was onto something, a little green (vegetables), eggs, and ham can actually be your perfect breakfast. After much debate, the proverbial black sheep of the nutrition world, dietary cholesterol, is finally making its way back to our table again in the form of whole eggs, dairy, lean meats, and some cheeses. The once 300-milligrams per day of cholesterol limit has now been removed, as science has proven that there is very little correlation between the cholesterol we ingest and the cholesterol in our blood.
So What DO you eat in order be fit and healthy? First let’s start with the Green: A great place to begin any healthy lifestyle is with vegetables and fruit, focusing on eating 2-3 cups a day worth of assorted vegetables but can drastically reduce the chances of many diseases. Fruit, although containing some sugar, provides the body with many essential vitamins, minerals, and fiber. Now onto the ham: The next area to focus on when dieting is protein rich foods which provide the body with muscle building (and retaining) amino acids. Some of the best sources for lean protein is fish, poultry, some pork cuts, and lean steak. Fish tops the most nutritionist list because many types contain heart healthy omega 3 fatty acids which have a litany of positive effects on the body. Where the Dr. Seuss diet is missing, add some grains. The final food group to round your diet out with is whole grains. While there is still much debate as to how much carbohydrates the body actually needs, most agree that filling up on fiber-rich whole grains can have a positive impact on health and body composition. Last but not least the highlight of the Dr. Seuss diet is the eggs: You may be wondering when the eggs fit into your diet, alas, with the removal of the daily cholesterol limit, starting your day with several whole eggs is perfectly acceptable and very healthy. A single egg contains around 6 grams of protein, vitamin A, D, E, and K, omega-3 fats, and both antioxidants lutein and zeaxanthin. Now that you don’t have to invest your willpower fighting off the urge of the Dr. Seuss diet, you can focus your willpower on the real diet killer, sugar! What science has found, is that a diet chronically high in sugar, can lead to heart disease and type 2 diabetes. The elimination of sugar from the diet is critical, the guidelines suggest less than 10% of your daily calories should come from sugar. For the average person eating a 2000 calorie a day diet, a single can of soda can push you over that limit. Do these new guidelines surprise you? 15 Law WORKERS’ COMP SCAMS, CONS AND ARTFUL DODGING Unscrupulous employers come up with schemes and scams all the time to limit their financial exposure when a worker is injured on the job. Do you recognize any of these common tricks? THE INDEPENDENT WORKER SCAM: Occurs when companies call employees “independent contractors” to avoid paying taxes, benefits, health care costs, overtime, workers’ comp insurance, etc. These workers lose protections due to this false label. Take-Away: Be wary if your company insists on calling you an independent contractor. You will have to fight for your benefits if hurt on the job. THE CASH PAYMENT CON: Occurs when a company insists on paying workers in cash. The worker is
told this is a great deal because they’ll make more money and pay less in taxes. This gives the company carte blanche to skip out on a workers’ comp claim. And good luck if a worker needs to file for State Temporary Disability or Social Security Disability! No benefits are available to workers who don’t pay into the system. Take-Away: Make sure companies pay you on the books. THE INSURANCE DODGE: Occurs when a company refuses to report an injury to its workers’ compensation carrier, instead advising the injured employee to treat through their private health care. Don’t fall for it! When that occurs, the worker gets saddled with co-pays and deductibles and has little chance of receiving compensation if they suffer a permanent injury. Take-Away: Report claims directly to the workers’ compensation carrier; no need to wait for permission from your employer.

Meditation Guidelines 1. Choose a quiet, comfortable spot where you will not be disturbed by other people or by the phone. This extends to pets as well. Choose a time when you will be free from essential duties. 2. Ensure physical and mental confort. Wear loose, comfortable clothes and empty your bladder and bowel. 3. Assume a poised position.It is essential to have a straight back without rigidity, a comfortable body, and stillness. The ideal meditation posture is one in which you can sit for the duration of the meditation without having to move the body unnecessarily. 4. Close your eyes. This makes it easier to concentrate. 5. Relax your muscles – starting with your scalp down to the tips of your toes. Become aware of each part of your body in succession, letting go as much as you can each time you exhale. Starting with your forehead, become aware of tension to some degree as you breathe in and then let go of any obvious tension as you breathe out. This need only take a minute or two. 6. Become aware of your breathing. Notice how the breath goes in and out. Listen to your breathing – it should be regular, slow, and rhythmical.
7. Choose a word, a sound or object to focus on. The meditation object can be physical like a fruit, a flower vase, a candle, or a mandala, or a word or phrase repeated mentally or aloud, or a body rhythm like breathing. Count your breath from 1 to 10 on exhalation and start again. Try several methods until you find one that is right for you. 8. Don’t think about the “right” way to meditate. Don’t think about the “right” way to meditate. If you are worrying about whether you are doing it right, then you are not meditating. But don’t be critical of yourself – just shift your attention back to the object of your meditation. 9. Passive awareness. This is very important. You need to develop a passive and relaxed attitude toward distraction. Each time you become conscious that your mind has wandered (and this will happen many times), just bring it back easily and effortlessly to the object of your meditation. As you become more experienced, distracting thoughts and images will lessen. Accept that they are inevitable and maintain an attitude of indifference to them. 10. Regular practice. Practice at least once a day for 10 to 20 minutes. With regular practice it becomes easier to still the mind and meditation becomes deeper and more peaceful.

The Top 5 Issues in Divorce and Special Needs Children needs children, it is imperative to accurately assess the true needs of the child.
I recently helped a divorce attorney prepare for a case in which there is a special needs child. Divorce is not easy and is especially complex when a special needs child is involved. When the parties have a special needs child, it is critical to make sure the parents carefully consider the present and future needs of the child.
3. Child Support. In New Jersey, there are special considerations allowed to enter when calculating child support, including the disability of the child. Be sure to have an attorney who is able to gauge the needs of the child beyond the basic calculations and formulas that are used for child support.
IMPORTANT ISSUES TO FOCUS ON: 1. Governmental Benefits. If the child is receiving governmental assistance, it is important to involve a lawyer familiar with special needs trusts. The parent and lawyer should anticipate, to the extent possible, the current and short- to mid-term custodial and financial needs of the child so that an appropriate parenting schedule and amount of support can be set in order to avoid going to the Court, which may be costly, to make modifications or to obtain an increase in support.
4. Life insurance. Typically, in a divorce, the payer of child support is required to obtain life insurance to guarantee child support in case the payer dies. When there is a child with special needs, it’s important to take precautions that any life insurance proceeds should not go to the child directly but instead to the special needs trust.
Frequently, children with disabilities are recipients of government benefits such as Medicaid and SSI. These benefits are means tested. In other words, in order to be eligible, there’s only a certain amount of assets that your child can have in his or her name in order to qualify for these benefits. To ensure protection of your child’s benefits, it’s important that the divorce attorney include language for the creation of a special needs trust. Our office frequently helps family lawyers provide the proper language and craft the special needs trust so that the child support is paid directly to the trust instead of to the child, thereby reducing the fear that benefits would be eliminated.
5. Education. Oftentimes, the child may receive special education services. If so, the custody order must clearly outline each parent’s role in the educational decision-making process. Unless the custody agreement grants one parent sole legal custody or revokes a parent’s educational decision-making rights, it is assumed that both parents will have the right to attend IEP meetings, the right to consent to services in educational placement, and the right to request mediation and due process hearings.
2. The Daytime Needs of the Child. Because of the type of condition of a special needs child, does a parent need to be available during the day to take care of the child? On the other hand, there have been cases where a parent has essentially tried to “exploit” a child’s otherwise minimal disability in order to convince a Court that employment outside of the home was not feasible. In cases involving special

Serves 4
1 sheet puff pastry, thawed and rolled out into a 14in square. 4 boneless chicken breasts, cooked 1/3 cup herbed soft cheese 1/4 cup fresh parsley , chopped 1 large egg Optional ingredients-chopped spinach, diced roasted red peppers, additional cheese, etc.
Preheat the oven to 400 degress. Roll out puff pastry, cut into four equal sized pieces. Place 1 1/2 tablesoons of herbed cheese in the center of each piece. Top herbed cheese with a chicken breast and then chopped parsley Next, fold the corners of the pastry over the chicken breast. Place chicken pastry seam side down on a baking sheet covered in parchement. Lightly brush the top with a whisked egg. Bake for 20-25minutes or until crust is golden brown.

WHAT IS A UROLOGIST? A urologist is a physician who has been trained to treat disorders of the male and female urinary tract (bladder, ureter, kidney, and urethra) and the male reproductive system. Think about what happens when you continue to resort to your usual pattern. You’ve heard the old saying (excuse the poor grammar), “If you always do what you always did, you’ll always get what you’ve always got.” When we take the same action over and over again in response to a stimulus (i.e., an emotion), neural pathways are formed in the brain. Through reinforcement and repetition, a habit is created. So, when you feel in need of comfort, you default to your usual habit – maybe junk food eating, or smoking, or drinking, etc. Maybe you recognize the absurdity of your comfort-seeking solutions, but feel unable to change things up. You are capable of changing, though; your mind is able to embed newer and better patterns. It takes a bit of effort, and

the use of the conscious and subconscious mind.
• Consider, first, if something in your life needs changing. Admit it – and work on it. Get some help, if
necessary, to make those changes. Talk to friends, a therapist, a coach, etc. • Think about positive comforting strategies. They’re different for everyone, depending on personality and interests. Some examples might be . . .
o Going out for a walk. o Taking a hot bath or shower. o Journaling your thoughts. o Having a soothing cup of tea. o Listening to music you enjoy. o Doing some stretches or yoga poses. o Knitting, painting, playing a musical instrument. • Make it easy to take those actions. For example,
keep a journal around, keep your workout clothes / sneakers in plain sight, have a variety of herbal teas available.
• Close your eyes and mentally rehearse using your improved comfort strategies. Notice yourself
taking the positive actions you plan to take. Mental rehearsal is a powerful strategy for making change. Notice the details. • Take steps to lower your overall stress level so that you’re better able to remain on an even keel and can handle life’s bumps; for example:
o Learn to do centering breaths (a.k.a., deep belly breaths or diaphragmatic breaths) o Pay attention to your negative self-talk and challenge it. o Get daily exercise – at least 30 minutes a day. o Practice mindfulness / meditation techniques.

Jill Collier, MD
Omar Tamimi, MD
Doctor Omar Tamimi joined the practice in 1992. He is Board Certified in Gastroenterology. He is a graduate of Rutgers University and University of St. George’s Medical School. His medical training was completed at Seton Hall University, St. Joseph’s hospital, Patterson, NJ. Since completing his medical training he has performed over 25,000 endoscopic procedures and has vast experience in all aspects of the practice of Gastroenterology. Doctor Tamimi had been a member of the executive committee at Kimball Medical Center since 2003 and served as the Chief of the Medical Staff from 2007-2009. Dr Tamimi is currently Medical Director of the Endoscopy Center of Ocean County. Doctor Tamimi and his wife Patty have three children with whom they are very involved. Doctor Tamimi enjoys outdoor sports, vacationing and spending time with his family and is a big fan of Rutgers sports teams and Bruce Springsteen.
Edgar Bigornia, MD
Doctor Edgar Bigornia is Board Certified in Gastroenterology. He is a graduate of Brandeis University and attended Medical School at SUNY Health Science Center at Brooklyn (Downstate) in New York. His Internal Medicine training was completed at Albert Einstein College of Medicine and he served as a Chief Medical Resident at Booth Memorial Hospital. His Gastroenterology fellowship was at New York Medical College. He is the recipient of the Lawlor Award for the best scientific paper submitted by a fellow at the American College of Gastroenterology meeting. He previously practiced in Poughkeepsie, NY. He enjoys outdoor sports with his family. He listens to all types of music and relaxes by playing the piano and guitar.

Doctor Jai Mirchandani is Board Certified in Gastroenterology. He graduated from Johns Hopkins University and the University of Medicine and Dentistry-New Jersey Medical School. He completed his medical residency at the University of Illinois at Chicago, and his Gastroenterology training at New York Medical College in Valhalla, NY. He served as an attending physician and instructor at Maimonides Medical Center for three years, where he was the Director of Capsule Endoscopy, Director of the Colon Cancer Screening program, and the Assistant Program Director for the Gastroenterology Fellowship program. Dr. Mirchandani has presented half a dozen abstracts at the national gastroenterology meetings, and his work has won two awards at the American College of Gastroenterology. He is the author of a chapter published in “Curbside Consultation of the Pancreas.” Dr. Mirchandani and his wife, neurologist Lakshmi Nadiminti, D.O., are the parents of two young children. Dr. Mirchandani enjoys spending time with his family, listening to music and is an avid sports fan.

Doctor Collier is currently Medical Director of the Endoscopy Center of Toms River. She was chairperson of the Division of Gastroenterology at Community Medical Center from 20102104. Doctor Collier and her husband Douglas have two children. She enjoys skiing, reading, traveling and being involved with her children’s activities.
Kenneth Glazier, MD
Doctor Kenneth Glazier is Board Certified in Gastroenterology. He graduated from the University of Michigan and Northwestern University Medical School. He completed his medical residency at Boston City Hospital and his training at Columbia University College of Physicians and Surgeons in Manhattan then Robert Wood Johnson Medical School in New Jersey. He has extensive experience in the treatment of Inflammatory Bowel Disease at the Crohn’s and Colitis Center of New Jersey and published a manuscript on the use of 6MP in treating Crohn’s disease. He is presently a Clinical Assistant Professor of Medicine at Rutgers-Robert Wood Johnson Medical School and joined the group after five years in private practice in the Marlboro-Freehold area. Dr. Glazier is currently Chairperson of the Subsection of Gastroenterology at Community Medical Center. Doctor Glazier and his wife, pediatrician Kim Glazier, MD, are the parents of two children. Dr. Glazier enjoys sports including golf and softball.

In our rush to experience the world, whether it is through educational, work, or travel opportunities, many women are choosing to delay childbearing until later in their lives. When the time does arrive and they are considering getting pregnant, women may be surprised to learn that their age directly impacts the likelihood to conceive on their own.
When is it time to think about having children?
Planning for a pregnancy should not be limited to the attempted time to conceive. Rather it should include proper pre-pregnancy nutrition and vitamin intake, recommended daily physical activity, and an overall healthy lifestyle. In addition, an understanding of a woman’s “fertility clock” should be an integral complement to this preconception care model.
How common is infertility and when does my fertility start to decrease? Infertility affects 1 in 8 couples in the United States. Many causes of infertility exist, stemming from both the female and male perspective. A “fertile couple” has approximately a 20% chance of achieving pregnancy in a given month, with 85% achieving pregnancy over the

scary, yet common question many people are forced to ask themselves on a regular basis. With unemployment steadily high and the number of under-employed folks even higher, the nature of today’s economy has left the majority of people grappling with difficult financial questions such as the one posed at the beginning of this paragraph. This is especially in New Jersey where the cost of living is amongst the highest in the country. When is it time to consider bankruptcy as an option? The following are some signals to look out for:

Credit cards are often a necessary evil. In a perfect world, we would all use our credit cards sparingly and with discretion, allowing ourselves the opportunity to pay for a family vacation or a special gift over time. Colorectal Cancer: Are You Aware? What’s The Big Hype? Nina Paonessa, DO, FACOS, an established colorectal surgeon practicing in Ocean and Monmouth Counties, answers questions about colorectal cancer, one of the leading cancers in both men and women.
sedation. Based upon the findings on colonoscopy, your colorectal surgeon or other specialist may recommend other testing such as blood work, a CT scan of the abdomen and pelvis, pelvic MRI and/or ultrasound.
How common is colorectal cancer and who is at risk? Colorectal cancer is the second most common cancer in the United States among both men and women. It affects 140,000 people annually and causes 50,000 deaths. The average person has a one in 20 chance of developing colorectal cancer in his/her lifetime. This risk is increased if there is a family history of colorectal polyps or cancer, and is even higher still in those patients with a history of breast, uterine or ovarian cancer, and patients with a history of extensive inflammatory bowel disease such as ulcerative colitis or Crohn’s colitis. Additionally, a person’s risk for developing colorectal cancer increases significantly with age.
How is colorectal cancer treated? Nearly all cases of colorectal cancer require surgery. Radiation and chemotherapy are sometimes required in addition to surgery. If the cancer is diagnosed and treated in the earliest stages, approximately 80-90% of patients are restored to their normal health. The cure rate decreases to 50% or less when the cancer is diagnosed in the later/advanced stages. Less than 5% of all colorectal cancer patients require a colostomy or “bag,” thanks to modern technology. Dr. Paonessa prides herself in being able to maintain this standard of care in her own practice. Not only is she able to minimize the number of colostomies/”bags” created for her patients, but she performs a majority of her colorectal cancer surgeries using laparoscopic surgery. Laparoscopic colorectal surgery is a minimally invasive surgery in which small incisions are made to perform major surgery. This allows for shorter hospital stays, decreased incisional pain and earlier return to work and other activities.
What causes colorectal cancer? It is generally agreed that nearly all colon and rectal cancer begins in benign polyps, called adenomas. These pre-malignant polyps, or growths, occur in the wall of the colon and/or rectum and may eventually increase in size and become cancer, or medically speaking, carcinoma. This is known as the “adenomacarcinoma sequence.” Several factors have been mentioned in the literature that may increase a person’s risk for developing colorectal cancer and include: age, a diet high in fat and cholesterol, an inactive/sedentary lifestyle, obesity, smoking and environmental exposures. What are the symptoms of colorectal cancer? Unfortunately, polyps and early cancers do not produce any physical signs or symptoms. Therefore, symptoms usually indicate late or advanced disease. The most common symptoms of colorectal cancer are rectal bleeding and changes in bowel habits such as constipation or diarrhea. These symptoms are common in many other disorders of the gastrointestinal tract so it is very important to have a thorough examination should you experience them. NOT ALL RECTAL BLEEDING IS DUE TO HEMORRHOIDS. It is very important for patients to remember this and seek evaluation if they are experiencing rectal bleeding. Other symptoms such as abdominal pain and weight loss are usually late symptoms and quite possibly indicate advanced/ extensive disease. How is colorectal cancer diagnosed? Evaluation of colorectal cancer is first begun with a complete history and physical exam, including a digital rectal exam and possibly testing the stool for blood. A colonoscopy is the most definitive test used to diagnose colorectal cancer. A colonoscopy is performed by a colorectal surgeon or other specialist in order to get a direct visualization of the inside of the colon and rectum. A colonoscopy is a safe, painless, outpatient procedure performed under a light Board-certified in general surgery and proctology and a fellow of the American College of Osteopathic Surgeons, Dr. Paonessa specializes in colon and rectal surgery. She believes in patient-centered care and preserving each patient’s independence, well being and dignity.
Can colorectal cancer be prevented? Colorectal cancer is one of the few cancers that is preventable. Obtaining a screening colonoscopy at regular intervals is the single best way to prevent colorectal cancer. Your colorectal surgeon or specialist is able to review the guidelines for screening colonoscopy based upon your age, medical history and family history. Although not definitively proven, studies published in the literature mention additional factors that may lower your risk of developing colorectal cancer and include: eating a high-fiber, low-fat diet, maintaining a healthy weight, and engaging in regular, vigorous exercise.

“Why Did I Get Cancer, Doc?”

This is a question I hear at least once a day. Whether it is Mark, who was diagnosed with leukemia at age 45, or Marge, who was diagnosed with colon cancer at age 80, or Mary, who was diagnosed with breast cancer in her thirties, they all have the same question, ultimately: “Why me? Why did I get cancer? How long did I have it? What could I have done to prevent it?” The answer is not simple. Scientists have been researching for decades to find the answer to this question. In fact, there is no one simple explanation to why one person developed cancer, while another person in the same environment and genetic background may not develop it in his or her lifetime. But scientists have found links between certain things and the development of cancer. It is known that cancer is caused by changes in one’s DNA. DNA is a component of the nuclei in the cells. When the DNA is damaged somehow, it makes the cells multiply in abundant numbers. Usually, abnormal cells that are made in the body are destroyed by the body’s own defense mechanism, a process called ‘apoptosis’ or programmed cell death. When there is a defect in apoptosis, cells accumulate and become tumors. The damage to the DNA can be caused by several factors. There are some changes or mutations that are inherited and are present from birth, which can later make a person susceptible for cancer. Only a small percentage of cancers develop this way. In the majority of cases, certain environmental factors play a major role. Some factors that play a role in the development of cancer are: • Lifestyle factors (nutrition, tobacco use, physical activity, etc.) • Naturally occurring exposures (ultraviolet light, radon gas, infectious agents, etc.) • Medical treatments (chemotherapy, radiation, immune system-suppressing drugs, etc.) • Workplace exposures • Household exposures • Pollution Substances and exposures that can lead to cancer are called carcinogens. Some carcinogens do not affect DNA directly, but lead to cancer in other
ways. For example, they may cause cells to divide at a faster than normal rate, which could increase the chances that DNA changes will occur. Carcinogens do not cause cancer in every case, all the time. Substances labeled as carcinogens may have different levels of cancer-causing potential. Some may cause cancer only after prolonged, high levels of exposure. And for any particular person, the risk of developing cancer depends on many factors, including how they are exposed to a carcinogen, the length and intensity of the exposure, and the person’s genetic makeup. There are several substances that are proven to be carcinogens. Still others are felt to be involved with a high risk for cancer, but their role is not substantially proven. The International Agency for Research on Cancer (IARC) is a part of the World Health Organization (WHO) that works on identifying causes of cancer. IARC classifies potential carcinogens into five different groups, depending on the level of evidence that links each to the cancer. Some of the known Group 1 carcinogens are: alcohol, afflatoxins, benzene, coal tar, hepatitis C virus, radioactive materials, tobacco smoking, etc. A detailed list of the carcinogens is available on the American Cancer Society’s website, cancer.org. Even though exposure to carcinogens increases the chance of developing cancer, a variety of other factors also play a role. While most of it is beyond our control, there are some things which a person can do to reduce their chance of getting cancer. 1. Practice a healthy lifestyle and good dietary habits. Eat more fruits and vegetables and lower the amount of fatty foods. 2. Exercise daily. 3. Avoid obesity. 4. Quit smoking. 5. Avoid excessive alcohol use. 6. Avoid prolonged exposure to sunlight. Use protective sunscreen. 7. Get regular medical check-ups for screening. This includes screening mammogram, PAP smear, colonoscopy, chest CT in heavy smokers, and dermatology evaluation for screening against skin cancer.

Q: Will I need chemotherapy?
A: Some patients will absolutely need chemotherapy, also known as chemo. However, for the majority of our patients, whose cancer was detected on mammograms, chemo is usually not necessary. We determine if chemo is needed after surgery when we can see the size of the tumor and whether lymph nodes are affected. In some cases, we perform additional testing on the surgically removed tumor to determine whether chemotherapy will help the patient. Q: Will I lose my hair with chemo and radiation?
A: It depends on the chemo regimen
used. Some chemo drugs do not cause hair loss, but the majority of breast cancer patients lose their hair temporarily. In extremely rare cases, patients may experience permanent hair loss, although I haven’t seen this in my patients. Radiation treatment is only on

A: That depends on a number of factors. One factor is tumor size in relation to breast size. If there’s a little lemon-size tumor in a B-cup breast, we may opt for a mastectomy. A lumpectomy in this scenario may result in a distorted look and significant asymmetry for the patient to feel comfortable with. I know patients will care about aesthetics once their treatment is done and as a breast cancer surgeon, my goal is to remove the cancer while maintaining the best aesthetic outcome for the patient. Another point is that following a lumpectomy, radiation treatment is recommended for most patients. However, some patients may not want radiation, or they’ve already had radiation to the breast and will need a mastectomy. Some patients may need mastectomy if they have multiple cancers in one breast.
Q: Will my health insurance
cover breast reconstruction?
A: Yes. Reconstruction is covered not only for the side of the breast with cancer but the other side to achieve symmetry. I work closely with my patients so that patients are referred to see a plastic surgeon prior to surgery so we can start the reconstruction process immediately if the patient chooses to have it.

A friend of my parents’, Linda, came to me with a problem. Lately, she has been dizzy with partial loss of vision in her right eye. When her doctor examined her, he heard an abnormal sound in her neck (called a “bruit”). His concern was she had carotid artery stenosis and was at risk to have a stroke. Her doctor recommended a carotid ultrasound. Upon hearing this, Linda was very worried and wanted to know more.
Q: What is the carotid artery? What is stenosis? A: We have two carotid arteries located on each side of our neck. It is the pulse we feel when we lightly press on the side of our neck. The carotid arteries carry blood from the heart to the brain. Over time, a waxy substance called plaque can build up in the carotid arteries. Plaque is made up of fat, cholesterol, and blood cells. This plaque can harden and start to calcify. Plaque building up can narrow the inside of the vessel decreasing the amount of blood going to the brain. This is called stenosis and can lead to a stroke. Another fear is that a piece of plaque will become loose and travel with the blood to the brain. This can also lead to a stroke. Q: What causes this plaque build up? A: The mechanism of plaque build up is complex. However factors that accelerate plaque build up and stenosis include high blood pressure, high cholesterol and fat in the blood, smoking, and diabetes Q: What are the symptoms of carotid stenosis? A: Sometimes there are no symptoms and the condition is instead detected during a physical exam. Sometimes carotid stenosis can cause a person to have dizziness or vertigo. Sudden partial loss of vision can be from plaque dislodging from the carotid artery wall and traveling to occlude the blood supply of the eye. In severe cases, a patient can present with small, temporary strokes called transient ischemic attacks (TIA). The danger of TIAs is that they often lead to much larger, potentially debilitating strokes. Q: What is a carotid ultrasound? A: An ultrasound is a machine that uses sound waves to see inside the body. A probe is placed on the body.
Sound waves are transmitted and received by the probe and a computer creates pictures of the body. This works much like sonar on a submarine. Ultrasound is extremely safe, and uses no radiation. The test is inexpensive and it can detect blood flow in real time. The ultrasound can detect plaque and the degree of narrowing inside the blood vessel. It can also detect direction and speed of the blood flow. Treatment can be implemented to decrease the amount of plaque and restore blood flow. Q: How is this test performed? A: All jewelry around the neck is removed. It is recommended for our patients to wear open neck shirts. The patient lies on a table, facing upwards. The ultrasound technologist places the probe on both carotid arteries and takes pictures. A radiologist interprets these images. Q: Are there any other tests that can detect stenosis? A: Sometimes the blood vessels are very tortuous which limits detailed evaluation. Also, the ultrasound cannot see the blood vessels when it enters the skull on its way to the brain. In such cases, a CT scan or MRI can be performed to image the carotid arteries. Q: If I have stenosis, what can I do? A: If the amount of plaque is mild and not causing significant stenosis, medications can be used to reduce the amount of cholesterol and fat in the blood as well as to control blood sugar. Exercise is highly encouraged. Smoking cessation is a must! If the stenosis is critical, a vascular surgeon maybe needed to clear the blocked arteries.

onna Kolomeyevskaya, MD, is a fellowship-trained gynecologic oncologist specializing in surgical and medical therapy of women with gynecologic cancers, as well as precancerous disease of the female genital tract and complex benign conditions. HPV and Cancer: What you need to know HPV (human papillomavirus) is the most common sexually transmitted infection, affecting most sexually active people at some point in their life. The infection is asymptomatic, and one can get infected and re-infected many times throughout one’s life. There are many strains of the HPV virus and it is the “high-risk” forms of HPV that can cause cancers of the cervix, vagina, vulva, oropharynx, anus, penis, and scrotum. Whereas “low-risk” strains of HPV cause genital warts. “We’re not used to thinking of cancer as a sexually transmitted disease,” says Dr. Kolomeyevskaya. “But in the case of cancers caused by HPV, that’s what it is.” The good news is we are better at screening patients: women get co-tested for HPV at the time of their Pap smear. Then, any abnormalities are closely followed and treated, if necessary. Getting vaccinated against HPV before the onset of sexual activity reduces the likelihood of contracting it or developing high-grade precancerous changes or cancer. Dr. Kolomeyevskaya emphasizes safe-sex practices to lessen the chances of transmission of other sexually transmitted diseases (STDs) that also potentiate HPV infection. The body can clear the infection on its own – especially in those who are young and healthy – but people can easily become re-infected, and partners can pass it back and forth.
Early Diagnosis, Prevention The most important step women can take to protect themselves against HPV-related cancers is to have regular, routine visits with their gynecologist. Routine examinations and Pap smears conducted by gynecologists with HPV co-testing have resulted in a dramatic decrease in the incidence of cervical cancers in the United States. Women should be aware of the “contributing factors” that raise their risk of developing HPVrelated cancers. These include smoking, infection with chlamydia or genital herpes, multiple partners (or partners who have had multiple partners, or partners with cervical dysplasia), early onset of sexual activity, etc. People with a suppressed immune system, such as those with

HIV or organ transplant patients, are at greater risk of HPV infections. HPV vaccines were developed to prevent chronic HPV infection. One of the HPV vaccines approved by the Food and Drug Administration and commonly administered now is called Gardasil 9. The vaccine is recommended for boys and girls at age 11 or 12 – before they become sexually active. Young people can receive the vaccine up to age 26, although it’s less effective once a person has been exposed to HPV. “Parents should talk to their child’s pediatrician about getting the HPV vaccine early – well before the onset of sexual activity,” says Dr. Kolomeyevskaya. “Through early prevention, the younger generation has the opportunity to avoid the types of cancers that my colleagues and I treat every day.” It is important to understand that condoms do not fully protect against STDs, including HPV, and adolescents should also be aware that transmission is also possible during oral sex.
Treatment There is no treatment that eradicates HPV, and the highest regression rates are seen in teenagers and young adults with competent immune system. Patients can be treated for HPV- induced cancers and pre-cancerous conditions of the genital tract and oropharynx, as well as genital warts. In the rapidly advancing field of medicine, it is important for patients to receive state-of-the-art care. “Do not hesitate to ask questions, and discuss with your physician current standards of care and guidelines,” Dr. Kolomeyevskaya advises.

Chelation Intravenous (IV) Chelation Therapy Hardening of the arteries is a growing threat to every American. We are currently facing a very real epidemic of this illness—with symptoms ranging from poor circulation in the extremities to kidney disorders and heart disease. It is estimated that 67.7 percent of all the disease-precipitated deaths in this country (heart attack—54.7 percent and stroke–13 percent) are the result of one form or another of hardening of the arteries. A person diagnosed as having atherosclerosis is usually offered very few choices in dealing with the illness. If the blocked arteries are large enough to be operated upon, the victim takes his chances with bypass or “cleanout” surgery. However, if the stricken blood vessels are too small for surgical techniques, then the patient must become resigned to living with this seemingly incurable and progressive disease. Unbeknownst to most people, however, there is a unique medical process that can clean out the arteries and renew blood flow throughout the entire body—without surgery! This exciting process is intravenous chelation therapy. Certain nutrient, or chemical, agents used for the chelation process include vitamin C, lactated Ringer’s solution (which resembles the blood serum in its salt constituents), EDTA (ethylene diamine tetraacetic acid), various weak organic acids, plus other agents such as PTC or Plaquex. By far, the two most frequently used agents in chelation therapy are EDTA and PTC. Scientists have demonstrated that when the EDTA solution is injected into the bloodstream, just one of the beneficial things it does is trigger the breakdown of components of plaque and lock onto, or chelate, heavy metals. Plaque is the internal “corrosion” that commonly clogs the arteries of middle-aged and elderly (and some young) people. EDTA stimulates the processes by which the cellular components of the plaque, such as heavy metals, mucopolysaccharides, calcium, cholesterol, collagen, fibrin, foreign proteins and other debris, release their hold on the internal wall of the arteries. These particles then become a part of the EDTA molecular solution and leave the bloodstream through the kidneys to be finally eliminated in the urine. As this occurs, the hardened arteries become more flexible. The narrowing of their central channel (the lumen) is halted, and the blood flow is unclogged. Moreover, tests reveal another benefit from chelation therapy. EDTA IV injections can help pull calcium from other parts of the body in which it is abnormally deposited, such as in traumatized tendons, inflamed bursae, kidney stones, arthritic joints and strained ligaments. But IV EDTA does not remove calcium from the bones and teeth, where the mineral serves a very useful purpose. Rather, EDTA chelation therapy tends to strengthen the bone by reactivating dormant but healthy bone-forming cells. For this reason, IV EDTA and its associated nutritional oral chelating agents have been useful in treating osteoporosis.
Diagnostic tests taken before and after chelation treatment reveals that impaired circulation is often restored to normal by this medical process. After the clogging material is removed from the human arterial wall, a smooth, clean, and nearly unblocked surface is thought to be left behind. Thanks to chelation and PTC therapy, people with hardening of the arteries and/or free radical pathology can hope to reverse symptoms. But what about the millions of patients suffering with various forms of degenerative disease who don’t know about chelation/PTC therapy who fail even to get the oral chelating advantages of vitamins, minerals, enzymes, amino acids, antioxidants, and other supplemental nutrients? Many sufferers are also denied the option of surgery, for a variety of reasons: lack of funds, no health insurance, just plain ignorance about health matters, an unhealthy lifestyle, a disinterested family doctor or misinformation about the treatment from competing physicians. An estimated 60 percent of all coronary surgeries including bypass, stenting and angioplasty are done unnecessarily as reported by Harvard’s Dr. Grayboys. That is, those having had surgery (bypass, angioplasty or stenting) DON’T LIVE A SINGLE DAY LONGER, THEY HAVE THE SAME QUALITY OF LIFE-ACTIVITY LEVELS ARE UNCHANGED, AND THEY ARE ON THE SAME MEDICATIONS—OR MORE THAN THEY WERE PRIOR TO THE SURGERY! In addition, the anatomical lesions that are addressed with these procedures are not where the pathology lies, which is in the walls of the blood vessels (endothelial layer) not the lumen. Far too often, those who are the sickest and in most need of and could benefit most from these invasive surgeries are denied them in fear of altering statistical results. In other words, if they were to die or have complications from surgery, it would critically alter the results obtained and deter patients from opting for surgery as a treatment for their coronary artery disease. The TACT (Trial to Access Chelation Therapy,) a nationwide medical study, has been completed on the effects of chelation on those with established CAD (coronary artery disease) and proved beneficial. It is unlikely however, because of the monetary business practices of medicine, that chelation therapy will become mainstream in the near future. Leave Your Job, Leave Your Retirement Plan? When you leave a job – whether due to layoff or for any reason – you are often faced with a decision about your 401(k) or other retirement plans. Over time, you may be in the same position as many other employees who have accumulated substantial balances in these employer plans that are designed for tax-advantaged retirement savings. If so, you may find yourself having to make a decision about whether to keep your 401(k) funds with your former employer, roll them over to an IRA, or pay the taxes and cash out. For this reason, it is important that you understand the implications of each option. Some employers offer the opportunity to maintain your retirement account at your former company. On the other hand, by rolling over your funds to an IRA, you can maintain control, manage the funds any way you want, and remain subject to IRA rules alone rather than the limited investment selection – and perhaps restrictive distribution policies – of your former employer. Note, however, that while you are permitted to take loans from your 401(k) plan, this is not possible in an IRA. And depending on the investments used to fund the IRA, charges and expenses could be higher or lower than those you would incur inside your 401(k) plan. At your direction, your employer can transfer your distribution directly to another qualified plan or to a rollover IRA. A rollover occurs when you withdraw cash or other assets from one eligible retirement plan and contribute all or part of it within 60 days to another eligible retirement plan. Under this option, you would direct your plan administrator to make a direct and taxfree transfer of funds from your former employer’s plan to a rollover IRA at a financial institution of your choice. That way, you maintain the tax-deferred status of your retirement account, consolidate all retirement accounts for easier management, and benefit from increased investment flexibility. When you roll over a retirement plan distribution, you generally don’t have to pay tax on it until later when you take cash withdrawals. By rolling over, your funds have the potential to continue to grow tax-deferred. Job changers should resist the temptation to spend down their retirement
savings, whether they are moving to a new job or plan a hiatus from work. When you fail to roll over, you not only pay tax on the amount you receive, but you may, if you are under age 59½, be subject to an additional 10% penalty on the early distribution you receive. Sound financial planning dictates that you would draw on other funds first, leaving retirement plan spending as a last resort. If you are simply moving your IRA from one institution to another and you do not plan to use the funds, you should consider making an IRA transfer and not a rollover. This is a simple direct transfer from IRA to IRA between financial institutions to better manage and grow your retirement assets. When considering rolling over assets from an employer plan to an IRA, factors that should be considered and compared between the employer plan and the IRA include fees and expenses, services offered, investment options, when penalty-free withdrawals are available, treatment of employer stock, when required minimum distribution begins, and protection of assets from creditors & bankruptcy. Investing and maintaining assets in an IRA will generally involve higher costs than those associated with employer-sponsored retirement plans. You should consult with the plan administrator and a professional tax advisor before making any decisions regarding your retirement assets. Healthy Bones For A Healthy Spring CALCIUM…
Do not forget about me! Taking calcium—either through diet or supplements—is a little like making deposits into a bone bank account. If you keep a healthy balance of this mineral, your body is able to build and maintain bone, which is a living, ever-changing tissue. Not only does calcium keep bones and teeth Jay Greco, RPh healthy, it also helps with clotting blood, sending signals between nerves, and regulating heart rhythms.1,2 But if you fail to make calcium “deposits,” your body starts to pull calcium out of your bones and “overdraws your account.” Worse than a painful overdraft fee, however, this calcium imbalance can cause porous, weakened bones. This condition is called osteoporosis. In the U.S., it affects 8 million women and 2 million men, resulting in 1.5 million fractures each year.1 How much calcium do you need? It varies, depending on your age. Your preteen or teen needs more. Banking calcium at this stage is really like making an early investment in “retirement.” The National Academy of Sciences recommends: • 1,300 mg/day for children ages 9–18 • 1,000 mg/day for men and women ages 19–50 • 1,200 mg/day for men and women ages 50 or older1,3 Some guidelines suggest 1,500 mg for postmenopausal women who don’t take estrogen.2 That’s because—due to a loss of estrogen at menopause— women start to have bone loss earlier than men.1 If you take calcium supplements, it’s best to divide the dose. Take half in the morning and half at night.2 Let me know if you have questions about different types of calcium. I can also discuss any medications that might
interfere with calcium absorption. And I can give you tips on how to handle any calcium side effects, such as constipation or indigestion. To limit side effects, be sure to take no more than 2,000 mg daily.2 Foods rich in calcium include dairy products, dark leafy greens, dried beans, nuts, and calciumfortified foods, which may include orange juice, cereal, and bread.1,3 Some recent studies suggest that food sources of calcium may be better than supplements. Last year, a study reported on in the American Journal of Clinical Nutrition showed that participants who got most of their calcium from diet had better bone density than those who got most it from supplements.4 It’s too soon to know why, but it might have to do with better absorption or other food compounds that enhance bone health. What else can you do to prevent bone loss as you age? Try weight-bearing exercise, such as walking or running, and get enough vitamin D.3 This vitamin helps your body absorb calcium. To get enough of it, you can spend at least 15 minutes in the sun several times a week.5 You can also take supplements or get food sources of vitamin D—in salmon, tuna, or fortified milk, margarine, or cereal.6 Try to get 800 International Units (IU) daily, but no more than 2,000 IU, which can be toxic.2 Any brand or our store brand is available at JERSEY SHORE PHARMACY

When Does Forgetting Become An Issue? Is it memory loss or am I just being forgetful? Did I just lose my keys or will I never remember where I put them? Is it OK that I can’t remember how to do basic chores because I’m “getting older”? The first answer brings good news because you are asking these important questions, not someone on your behalf, and, even more important:

When your memory has significantly declined, is poor compared to performance typical for your age but DOES NOT interfere with independent, day-to-day functions, you can be described as having Mild Cognitive Impairment or MCI. Dementia is an advanced deterioration of MCI and means that you no longer are able to independently perform day-today functions. Alzheimer’s Disease or AD is the most common form of dementia, and we know that a proteinbased plaque exists in the brains of these patients at abnormally high levels. We also know that this plaque begins to accumulate and clump together within the brain as early as a decade or more before you may experience any memory loss, impairment or dementia. On a positive note, we are living longer and could prepare to enjoy this new stage of living proactively and productively. Exercise, proper diet and remaining busy,

100 Women Campaign Sets Bars At Ocean County College In 2015, women from across Ocean County came together to help launch the Ocean County College Foundation’s 100 Women Campaign! Nearly two years later, this exciting campaign has grown to include more than just 100 women, but has helped more than 50 young women blossom in both their academics and personal lives. According to Heather Barberi, Executive Director of the Ocean County College Foundation, the mission of the “100 Women Campaign” is to bring together dynamic, innovative, and accomplished women who will support each other’s life and career goals, share their insights and experiences with students, and encourage education through Ocean County College. “In 2015, it took less than six months for the Campaign to bring together over 100 incredible women in the community who are passionate about education and giving back,” Barberi said. “The Ocean County College Foundation is thrilled that the 100 Women Campaign had so much success since its launch,” she explained. “Over one hundred incredible community leaders were recruited, nearly $15,000 in direct scholarship support was provided to OCC students, and over 50 students participated in the mentoring program. We are ecstatic with the results, and can’t wait to build on the program as we move forward.” In the coming years, the goal of the “100 Women Campaign” will be to raise its first $50,000 in order to retain and reward talented students at Ocean County College through scholarship support. Additionally, these students will be mentored throughout their tenure at Ocean County College. “This type of support is known to have a tremendous impact on the lives of students, creating a positive, crucial difference in ensuring their future success,” Barberi continued. The “100 Women Campaign” also seeks to provide an avenue for connections to be made among all the members, and networking events will take place throughout the year. The group recently volunteered at the local Salvation Army’s Holiday Toy Shop, and there are four upcoming activities planned for early 2017. Barberi explained, “The passion and excitement surrounding this campaign from the start has been amazing and we are looking forward to great things in the future!”

March Is Red Cross Month The American Red Cross sets aside the month of March as Red Cross Month, a time to celebrate the every day heroes who help their neighbors when they need it most. These heroes are the Red Cross volunteers, blood donors and financial contributors who make the work of the Red Cross possible. On Saturday, March 18, the Red Cross will recognize the incredible impact three local honorees have made in the community during its annual Red, White and Bling gala. Honorees include the Bonfiglio family of Tommy’s Tavern and Tap and Allyson Gilbert, both of whom are recipients of the Clara Barton Community Humanitarian Award, and HealthSouth Rehabilitation Hospitals of Tinton Falls and Toms River, recipient of the Clara Barton Corporate Humanitarian Award.

CASA Celebrates A Decade Of Providing Child Advocacy For Children In The Foster Care System Written By: Vicki Weiss, Executive Director What is CASA of Ocean County? CASA of Ocean County is a non-profit organization that exists to recruit, train and supervise every day-concerned citizen to serve as a child’s voice in court. A CASA Volunteer is a compassionate caring adult that remains in the child’s life until the child has a safe and permanent home. 10 Year Anniversary for CASA of Ocean County Help CASA celebrate its vision to provide a CASA Volunteer to every child in foster care that needs one by volunteering as a Court Appointed Special Advocate. There are over 750 victims of abuse and neglect living in out of home placement in Ocean County. By no fault of their own, innocent children from our community have experienced physical/ and or sexual abuse, neglect, witnessed domestic violence, live in extreme poverty and have parents that are struggling with addiction or mental health issues. Children, from ages ranging from birth to 21, are appointed to the CASA office so we may provide them with a CASA Volunteer of their own. The CASA Volunteer ensures the child’s emotional and physical needs are being met while they live in foster care. What do you need to do to become a CASA Volunteer? Besides patience and a giving heart for children,
the CASA Volunteer needs time to donate to be the one person that the children can count and rely on. CASA provides a 30-hour training class for the volunteers to teach them how to become child’s rights advocates in a very complicated court system. A CASA will undergo a background check, visit the child once a month, prepare court reports and attend hearings, and participate in on going training activities. As a Judicial Volunteers they assist the court by visiting foster homes, day care providers, schools, medical providers, and will inform the Judge in a court report. The volunteer will make recommendations to the court so the Children in Court Judge can make the best decisions for the best interest of the children. What does advocacy include? Every child’s situation is different but here are examples of how a CASA can change a life of a child: • Serve as a voice in the courtroom to keep children from being moved from foster home to foster home. • Request the child’s right to have parent, sibling, and family visitations. • Visit schools and ensure the foster child is receiving a quality education. • Assist with clothing and basic need issues. • Recommend annual health visits and therapy. • Monitor a parent’s court order for rehabilitation and help the child from lingering in foster care.

American Historical Theatre actress Kim Hanley will portray Annie Oakley, an exhibition shooter who competed in a sport dominated by men. This program is funded by the Horizons Speakers Bureau of the New Jersey Council for the Humanities, a state partner of the National Endowment for the Humanities.

The Jackson Branch has a packed schedule of DIY “Make It!” activities for kids, teens and adults, such as Cheerio Bird Feeder crafts, an Aromatherapy heating pads activity, Salty Sand Art, Kid Community Coloring, “Let’s Build the Largest,” LED Fun with DIY squishy circuits, mini light sabers, and LED throwies.
