Shoulder Replacement Options: Total v. Reverse Shoulder Replacement
The total shoulder arthroplasty (TSA) was first performed in the United States in the 1950’s. According to the Agency for Healthcare Research and Quality, about 53,000 people in the U.S. have their shoulder replaced each year. This compares to more than 900,000 Americans a year who have knee and hip replacements. The main goal of shoulder replacement surgery is to decrease pain, while also increasing mobility, strength, and function in order to help the patient return to normal life.
• Total shoulder replacement: TSA is a surgical procedure that replaces the damaged glenohumeral joint with artificial components called prostheses. Typically, the head of the humerus and the glenoid cavity are both replaced. The damaged humeral head is replaced with a metal ball and stem, which is placed into the shaft of the humerus. The glenoid cavity is replaced with a new smooth plastic surface.
Anatomy of the shoulder (glenohumeral joint) The shoulder is made up of three bones: the humerus (upper arm bone), the scapula (shoulder blade), and the clavicle (collar bone). Much like a ball and socket joint, the head of the humerus (the ball) fits into the socket of the scapula, called the glenoid cavity; this makes up the glenohumeral joint. The second portion of the shoulder is called the rotator cuff, and is made up of four muscles (supraspinatus, infraspinatus, teres minor, and subscapularis). These muscles aid in holding the head of the humerus into the glenoid cavity during shoulder elevation, so partial dislocation (subluxation) does not occur.
Who needs a total shoulder replacement? Increased shoulder pain can be debilitating, and can significantly impact one’s quality of life. Patients with progressing shoulder pathology, persistent pain, and loss of function, benefit from a total shoulder replacement. These pathologies include: • advanced osteoarthritis (OA) • rheumatoid arthritis • disease of the rotator cuff • osteonecrosis • fractures of the humeral head Advanced OA is the most common reason to have surgery. Patients frequently hear the term “bone on bone,” which means there has been a significant deterioration of the normal smooth cartilage that lines the joint. Smooth cartilage is important because it allows the ball and socket to glide against one another during movement. Surgical management is considered when conservative treatment, such as rehabilitation, corticosteroid injection, anti-inflammatory medication, and pain medication (analgesics) fail to provide relief.
Is a total shoulder replacement right for you? The decision to have shoulder replacement surgery should be a mutual decision among you, your family, your family physician, and your orthopedic surgeon. As mentioned above, if conservative treatment was performed without relief of symptoms, then shoulder replacement surgery can be a great option to help increase your functional independence and regain your quality of life.

• Reverse shoulder replacement: If you have severe rotator cuff damage or a complete rotator cuff tear, then a reverse total shoulder replacement might be better for you as a total shoulder replacement could still leave you with pain and the inability to lift your arm above shoulder level. In a reverse total shoulder replacement, the socket and metal ball are switched. The metal ball replaces the shoulder socket and a plastic socket is attached where the head of the humerus was. In addition, due to the irreparable damage to the rotator cuff, the deltoid muscle now becomes the primary mover during shoulder elevation.
How physical therapy can help after surgery Although results of a TSA vary from patient to patient depending upon the underlying diagnosis, the results of having TSA are very satisfactory. Physical therapy after a TSA has a high success rate because the procedure involves soft tissue reconstruction of ligaments, muscles, and tendons. Physical therapists are well educated in the precautions and post-surgical care following a TSA in order to increase overall functional activity. The primary goal of shoulder replacement surgery is pain relief, along with the benefit of restoring motion and strength in order to return to sport or other recreational activities. Patients are permitted to use their hand and wrist soon after surgery, and their elbow and shoulder approximately six weeks after surgery for light activity. Unrestricted, active use of the arm may begin as early as eight weeks after surgery. It is important to realize that patients typically will not be pain free until about 6-12 months after surgery. After one year, 95% of TSA patients enjoy pain-free function, which enables them to exercise the shoulder and return to the activities they enjoy.
Kimberly Salvato, DPT Manchester Physical Therapist Kimberly Salvato received her Bachelor’s of Science in Biology and minor in Behavioral Neuroscience at the Richard Stockton College of New Jersey in 2012. Following the completion of her undergraduate degree, Kimberly attended graduate school at the University of the Sciences in Philadelphia and received her Doctor of Physical Therapy in 2015. As a student, Kimberly received extensive training treating patients post-stroke in an acute setting and patient post-surgical in an outpatient orthopedic setting. Kimberly then spent four months on her last affiliation in orthopedics and sports medicine, where she studied under renowned sports physical therapists, athletic trainers, and strength and conditions specialists. Kimberly learned valuable knowledge working with high school and college athletes, predominately with ACL and meniscal injuries. Kimberly believes in using evidenced based practice (EBP) in order to provide high-quality care to her patients. She has even participated in a group research project examining the efficacy of Thera-Band elastic bands versus free weights when performing external rotation of the shoulder, which she hopes will become published. Kimberly’s approach to treatment utilizes various techniques she has learned including: Instrument Assisted Soft Tissue Mobilization (IASTM), Kinesio Taping Method, and manual therapy techniques including Maitland, Mulligan, and McKenzie approaches.

Spring is finally upon us, and after a long, long winter, we couldn’t be happier. On April 6th, our office staff welcomed back the Canada geese couple. As in past years, they immediately started building their nest in the patch of long grass next to the parking lot mailboxes. Each year, we love watching “Mom� patiently sitting on her eggs waiting for the hatching of her goslings. Last year we were fortunate enough to see all six of her goslings on the day they were born! Speaking of “Mom,� on May 14th we celebrate and honor our mothers. In fact, you can read about the history of Mother’s Day on page 57. Then, on June 18th, we celebrate Father’s Day, a day to observe the contribution fathers and father figures make towards their children’s lives. If you are lucky enough to have one or both parents, remember to tell them how special they are to you! On May 29th, we celebrate Memorial Day, a day to honor all of the men and women who have died in service to their country. Many towns will pay tribute to the fallen with parades, decorating with flags, and people wearing a bright red poppy on the left side of their chest. Memorial Day also marks the unofficial beginning of summer. As the weather heats up and all the beach communities come alive with activities, we want to wish you a safe and fun summer. Take advantage of everything the Jersey Shore has to offer, including the many wonderful events you can read about in the Community section of this publication. Don’t Let These Five Wacky Reasons Stop You from Filing a Workers’ Comp Claim! “I’m good, thanks. The comp carrier is taking care of me.” Maybe for the time being, but if you disagree with your doctor’s plan, you’ll probably meet a brick wall. Insurance companies are in business to make money and that means denying things like surgeries, physical therapy and prescription medications in some situations. And no way will they tell you about all the benefits you are entitled to as an injured worker! Can you really trust an insurance company to look out for your best interests? “I have health insurance. I don’t need workers’ comp.” Yikes! Putting your work injury through your private insurance is the wrong move because you are responsible for co-pays and deductibles. Why would you pay for your medical costs when your employer’s workers’ comp carrier pays for everything? “I’m not one of “those people.’” This one really gets under my skin. “Those people” are your friends and family members who need medical treatment and wage replacement because of a random accident. They may never be the same again even after the best medical treatment.
They are not “taking advantage of the system,” but merely exercising their rights. It’s time to change your perspective on this. “My boss is my buddy; I don’t want to get him in trouble.” Really? This has got to be the worst reason of all. Your boss may be great, but if he’s really a friend, he’ll make sure you’re covered by not just telling you about workers’ comp but by encouraging you to file a claim petition. “I could lose my job.” No way. If your company fires you because you filed a comp claim, they’d face serious fines or a major discrimination lawsuit. The irony is that if you don’t file a claim, you’ll have virtually no protection if you are fired while you are recuperating from your injury. And remember…there’s a difference between filing a claim with an insurance company and filing a CLAIM PETITION with the Division of Workers’ Compensation. Elizabeth Jones
A Three-Year Journey with Her Childhood Sweetheart by Her Side Every Step of the Way Written by Claire Brown, BSN, RN, Director of Provider Relations
Elizabeth Jones was in high school when she met her sweetheart, Willie. They dated for a few months, then lost contact with each other for 20 years. Then, sometime later, she was able to reconnect with him on Facebook and they started to renew their relationship. Elizabeth said she was the one who proposed to Willie on the Atlantic City boardwalk. They have been married for seven years now, and been together for even longer. Unfortunately, Elizabeth was to spend a large portion of her life from 2014 to the present day in and out of hospitals, receiving intensive care with Willie by her side every step of the journey. In October 2014, Elizabeth was admitted to a hospital in upstate New York for a routine hernia repair. Although she had previously received a kidney transplant, she was generally a healthy person who lived as normal a life as any Army wife. She had a full-time job and a daughter. Her husband, Willie, an Army serviceman, was stationed in New York at the time. Both Elizabeth and Willie were unaware that this elective surgery would result in a devastating series of medical complications and repeat hospitalizations due to unforeseen events in her recovery. During her postoperative recovery following the hernia repair, Elizabeth’s intestines became severely infected and she sustained a small stroke with recurrent seizures. Due to this, she spent an extended period of time on a ventilator machine. After several weeks, she was able to be liberated from the ventilator and started to make a slow recovery.

In February of 2015, she returned for another surgery at the hospital in New York after it was discovered she had several large intra-abdominal abscesses. A drain was placed in her small intestine and her wound had reopened. Her IV lines had also become infected. This was a long and
arduous time for Elizabeth. She remained hospitalized for several weeks and received a repeat surgery in April 2015. Elizabeth also had many other infections and high fevers throughout her subsequent hospitalizations. Elizabeth was then referred to the Cleveland Clinic. She was instructed to be re-evaluated by the surgeons there. Upon examination and review of her medical records, it was determined that it would be advisable not to receive any further surgical interventions for at least a year. During this time, Willie remained at her side for every step of her journey. Finally, in August 2015, Elizabeth was transferred from the Cleveland Clinic to Acuity Specialty Hospital of New Jersey (ASHNJ), located in Atlantic City, for management of her poor nutritional status and intensive wound care of her abdominal wall opening. Elizabeth had some family support close to Atlantic City and the surgeons at the Cleveland Clinic felt that ASHNJ would be an excellent location for the specialized care she needed while she waited for another abdominal surgical intervention. Once her nutritional status was improved and her abdominal wound was free of infection, it was decided that Elizabeth would be transferred to Thomas Jefferson University Hospital of Philadelphia (TJUH). The surgeons there would evaluate her for further surgery of her abdominal complications. She will have to return to Walter Reed Military Hospital in the spring for a further surgical intervention. Her husband, Willie, has returned to work in the Army Supply Division.

Squamous Cell Carcinoma (SCC) Squamous cell carcinoma (SCC) is an uncontrolled growth of abnormal cells arising in the squamous cells, which compose most of the skin’s upper layers (the epidermis). SCCs often look like scaly red patches, open sores, elevated growths with a central depression, or warts; they may crust or bleed. They can become disfiguring and sometimes deadly if allowed to grow. More than 1 million cases of squamous cell carcinoma are diagnosed each year in the U.S., and (depending on different estimates) as many as 8,800 people die from the disease. Incidence of the disease has increased up to 200 percent in the past three decades in the U.S. SCC is mainly caused by cumulative ultraviolet (UV) exposure over the course of a lifetime; intense exposure in the summer months, as well as the UV produced by tanning beds, all add to the damage that can lead to SCC. Having a fair complexion also puts you at risk of SCC. SCCs may occur on all areas of the body including the mucous membranes and genitals, but are most common in areas frequently exposed to the sun, such as the rim of the ear, lower lip, face, balding scalp, neck, hands, arms and legs. Often the skin in these areas reveals telltale signs of sun damage, including wrinkles, pigment changes, freckles, “age spots,” loss of elasticity, and broken blood vessels. Often SCC is mistaken for rash.
A persistent, scaly red patch with irregular borders that sometimes crusts or bleeds. Squamous cell carcinomas detected at an early stage and removed promptly are almost always curable and cause minimal damage. However, left untreated, they eventually penetrate the underlying tissues and can become disfiguring. A small percentage even metastasize to local lymph nodes, distant tissues, and organs and can become fatal. Therefore, any suspicious growth should be reported to a dermatologist without delay. Anyone who has had precancerous spots called actinic keratosis (AKs) or one squamous cell tumor has an increased chance of developing another, especially in the same skin area or nearby. That is usually because the skin has already suffered irreversible sun damage. Such recurrences typically occur within the first two years after surgery. A squamous cell carcinoma can recur even when it has been carefully removed the first time. That is why you need to be under close lifelong surveillance of your dermatologist.
To avoid or prevent SCC, make these sun safety habits part of your daily health care routine: • • • • • • •

Seek the shade, especially between 10 AM and 4 PM. Do not burn. Avoid tanning and never use UV tanning beds. Cover up with clothing, including a broad-brimmed hat and UV-blocking sunglasses. Wear a hat that covers your ears. Use a broad spectrum (UVA/UVB) sunscreen with an SPF of 30-50 every day. Apply sunscreen to your entire body 30 minutes before going outside. Reapply every two hours or immediately after swimming or excessive sweating. Report any rash that does not go away in a month or two to your dermatologist. Keep newborns out of the sun. Sunscreens should be used on babies over the age of six months. Examine your skin head-to-toe every month. See your dermatologist at least once a year for a routine full skin check exam.

Children with enlarged tonsils can also develop sleep-disordered breathing. Sleep-disordered breathing refers to abnormal breathing or decreased breathing when children are sleeping. Symptoms include snoring, mouth breathing and pauses in breathing. Sleepdisord disordered breathing can mean the child has obstructive sleep apnea. apn Signs of sleep-disordered breathing in children in include hyperactive behavior, sleepiness, inattention and p poor concentration. Children are prone to issues involving the adenoids and tonsils. The adenoids are lymphoid tissue located in the back of the nose. Enlarged adenoids can lead to a variety o of symptoms in children, including mouth breathing, chronic nasal obstruction, chronic runny nose, snoring at night, and trouble breathing at night. Problems with the adenoids can lead to sinus infections and ear infections. The tonsils can lead to infections and sleep apnea. Problems with the adenoids and tonsils are extremely common. In fact, adenotonsillectomy is the second most common outpatient pediatric surgery performed in the United States. Tonsillar issues can affect children of all ages. Tonsillitis is a common problem. The symptoms of tonsillitis include sore throat, red-appearing tonsils, white coating on tonsils, change in voice, uncomfortable swallowing, adenopathy and fever. Children are typically seen by their pediatrician. The pediatrician will m try to differentiate viral infection from bacterial infection. If diagnosed with bacterial tonsillitis, recommended treatment is antibiotics. Children who get recurrent infections may be candidates for tonsillectomy. Some patients develop chronic tonsillitis, which is chronic sore throat and halitosis.

If you suspect your child has problems of the adenoids or tonsils, then you should make an appointment with an otolaryngologist. Commonly known a ENT doctors, otolaryngologists are as sp specialists in the treatment of adenoid and tonsillar problems. At your initial visit, a thorough t history will be obtained. Your child will then be examined. Additional testing may be ordered, such as labs, x-rays or sleep studies. After a thorough consideration of all the information, you and your otolaryngologist will discuss if your child will benefit from adenoidectomy d and/or tonsillectomy. and

Sometimes Good People Find Themselves In The Criminal Justice System-Use An Expungement To Help You

s we stand here in 2017, finances become ever so important in an extremely competitive job market. We are in a technologically advanced society where people can learn about you and your background with the stroke of a computer key. Depending on the agency that is conducting the criminal background check, whether private or public, a wealth of information about you can be yielded. Having a conviction in New Jersey for a crime, offense or otherwise can possibly jeopardize securing a job if you have been involved with the state criminal justice system. Not only can a conviction affect your ability to secure a livelihood, it may need to be disclosed and can affect college applications, graduate school applications, professional licensing requirements, relationships, court proceedings, military entrance, certain jobs requiring a background clearance, housing and insurance. The law regarding expungements is highly technical and therefore you need an experienced criminal attorney to obtain an expungement. An expungement is legally defined as “the extraction and isolation of all records on file within any court, detention or correctional facility, law enforcement or criminal justice agency concerning a person’s detection, apprehension, arrest, detention, trial or disposition of an offense within the criminal justice system.” N.J.S. 2C:52-1a. Expungements apply to juveniles and adults. Our legislature has recognized the role of an expungement. Life experiences and observation suggest that a multitude of reasons exist why a person has landed in the criminal justice system – marital issues, health problems, mental health issues, drug dependency, financial failure, death of loved ones or simply a product of circumstance. Recognizing these unfortunate yet real problems, New Jersey criminal law states in pertinent part that regarding expungements “the primary objective of providing relief to the reformed offender who has led a life of rectitude and disassociated himself with unlawful activity.” N.J.S. 2C:52-32. Not all crimes and offenses are subject to an expungement. For example, motor vehicle offenses for which a person was arrested or convicted are generally not subject to an expungement. See N.J.S. 2C:52-28. A person seeking an expungement application should know that New Jersey requires certain time passage before one is even eligible to apply. In some cases, depending on the facts and circumstances of the individual, it may be a waiting period of ten (10) years, five (5) years, three (3) years, six (6) months or immediately.
Particularly significant are expungements applications that can be filed immediately. The Governor signed a fairly new law to be effective April 18, 2016. See N.J.S. 2C:52-6a et seq. However, many experienced attorneys and persons involved in the state criminal justice system are not aware of it and how it can help their clients. This new law allows for an expungement application to be filed immediately if a person’s charges were dismissed or resulted in an acquittal, with certain exceptions. To illustrate, we recently helped an individual in Essex County. We were successfully able to get his indictable charge downgraded to a simple assault. Thereafter, we got the simple assault charge dismissed. I prepared all of the necessary paperwork and provided it to the judge to transfer over for expungement purposes. There was no waiting period and this process helps clients who are looking for immediate filing and an expeditious expungement of same. The Administrative Office of the Courts (AOC) has provided a criminal directive and legal form to be utilized in these instances. The law on expungements, like many laws, can and often does change from moment to moment. Likewise, the judicial interpretations dictate the roadmap in this technical area. Depending on what you were convicted of, the time frame, what your criminal records are before and after, and a list of other crucial factors determines if you will be afforded an opportunity for a clean slate. When it comes to your life and your livelihood, you need an experienced attorney who will tend to your best interests. Famous novelist Marcel Proust sums it up – “There is no man … however wise, who has not at some period in his youth said things, or lived a life, the memory of which is so unpleasant to him that he would gladly expunge it.”

AUTISM AWARENESS MONTH Dental Tips for Children with Autism Spectrum Disorder By Dr. Elisa Velazquez, Ocean Pediatric Dental Associates Children with autism spectrum disorder (ASD) may have a difficult time with daily oral hygiene skills and visiting a dentist. This is no surprise as dental visits may be challenging for many. The dental setting can be a sensory overload with the sounds, smells, and unfamiliarity of the setting with people with sharp tools. Here are some tips that may help some kids with ASD.
DENTAL HOME CARE Great smiles begin at home. Start brushing your child’s teeth as soon as teeth erupt. Children require routines and it is best to start early. Repeat dental words and action words to teach the child what is expected. This vocabulary is a building block for following the directions necessary to become more independent with home care and visiting the dentist. For example, say, “It is time to brush Johnny’s teeth with the toothpaste to have healthy teeth.” All children consistently do better with a clear set of expectations. Consider the bedtime routine before everyone is exhausted. A half-hour before bed, take a bath, dress for bed, brush teeth, and read a book. Utilize videos on YouTube or create your own video of good brushing. There are many books on oral hygiene that make it fun and educational. Apps for dental care also exist and children who have success with iPads may learn from the apps.
DIET A healthy diet leads to a healthy smile. Individuals with autism may have limited diets and strong likes or dislikes when it comes to food and drink. Do your best to provide a diet that reduces the risk of cavities. Limit sugary drinks, sticky/gummy foods (e.g., raisins, fruit snacks), and acidic foods that can break down the enamel. Napping or going to bed with a bottle or sippy cup containing juice or milk can lead to cavities. Try diluting juice with water, slowly increasing the water over time.
DENTAL VISITS The earlier the better. We encourage AGE 1 be the first visit. If your child is older, schedule as soon as you can. Early exposure and frequency will help your child be acclimated to the dentist. The goal is to plan ahead and establish a step-by-step plan, including reinforcement, to create a positive routine with successful completion. If your child needs small steps to accomplish a dental visit, we encourage them to be part of our DOPAC program. FREE ORIENTATION PROGRAMS (DOPAC PROGRAM) Our pediatric dental offices are child friendly, complete with TVs, toys and games in the waiting room, to help your child feel more at home. To help some children get acclimated to dental care, we offer a FREE MONTHLY PROGRAM for children on the spectrum. Our DOPAC program, Dental Orientation Program for Autistic Children, has been successful for many children. Repeated visits and exposure can be an effective learning program for some children to help them with daily oral hygiene and to acclimate them to an office setting. We also encourage families to read our SOCIAL STORY on our website and watch our YouTube videos with their children to get them familiar with us. This DOPAC program is free but requires registration.

Live Life Dry: Treating Urinary Incontinence It would be difficult to watch an hour of primetime television today without seeing an advertisement for overactive bladder medication or adult diapers. The fact is more than 37 million adults in the United states-one in six-suffers from overactive bladder, urinary frequency, or urgency. Unfortunately, many people dismiss such urinary symptoms to be a part of “getting older” and do not seek medical intervention. The fact is, overactive bladder and urinary retention (the inability to completely empty the bladder) are not a normal part of the aging process, can cause embarrassment, and can diminish the quality of daily living dramatically. Facts Women are twice as likely as men to experience urinary incontinence. This is due to pregnancy, childbirth, menopause and the structure of the female urinary tract. Older women experience urinary incontinence more often than younger women. Bladder control problems may also affect younger people, especially women who have just given birth. In fact, a recent study found 25 percent of women over 18 years old has experienced episodes of leaking urine involuntarily, and one-third of men have experienced loss of bladder control during their adult lives. Only one in eight Americans who have experienced loss of bladder control have been diagnosed. Recent data has shown that women wait and average of 6.5 years from the first time they experience symptoms until they obtain a diagnosis for their bladder control problems. People experiencing bladder control issues often suffer diminished social lives. Even mild symptoms affect social, sexual, interpersonal and professional function. People with bladder control problems often struggle with simple everyday activities, such as working, shopping, or seeing a movie, for fear of embarrassing wetting episodes or not being near a restroom. Also, overactive bladder sufferers are two to three times more likely to regularly experience disturbed sleep, overeating, and poor selfesteem. Diagnosis Bladder control issues can be caused by a multitude of factors. In certain cases, pregnancy and childbirth, obesity, weak pelvic muscles or diabetes can contribute to overactive bladder. Additionally, certain medications or inactivity also can contribute to such issues. In some people, bladder control problems are caused by miscommunication between nerves. Specifically, the brain and sacral nerves, which control the bladder, do not communicate properly, causing the nerves to tell the bladder to release urine at the wrong time. The first step towards treatment is diagnosing the type of incontinence and then pinpointing the underlying cause or causes. We ask our patients to complete a “voiding diary” which provides great insight and aids in diagnosis. The gold standard for diagnosing urinary issues, including incontinence, is called Urodynamic testing or Urodynamic study. This series of tests, all done in one visit, along with the data from your voiding diary will give the physician objective data related to bladder functions. Additionally, physicians often perform a cystoscopy to further assess the urinary tract. A cystoscopy is a test that allows the doctor to look at the inside of the bladder and the urethra using a thin, lighted instrument called a cystoscope. This study helps find the cause of symptoms such as blood in
the urine (hematuria), painful urination (dysuria), urinary incontinence, urinary frequency or hesitancy, an inability to pass urine (retention), or a sudden and overwhelming need to urinate (urgency) and much more. Treatment In some instances, behavior modification may be sufficient to deal with urinary incontinence. You may be asked to limit fluid intake, avoid bladder irritants such as a caffeine and alcohol, and do pelvic floor exercises-“Kegels”. There are also medications commonly used to treat overactive bladder (OAB) and are sometimes prescribed to treat stress urinary incontinence. In certain cases of incontinence involving the miscommunication of nerves to the bladder, a neurostimulator (commonly called a Bladder Pacemaker) may be the best treatment option. Incontinence is not a hopeless problem. There are many treatment options available, however, only proper diagnosis will lead to proper treatment. Although behavior modification and pharmaceutical therapy work for some, they may be ineffective or poorly tolerated by other patients. It is definitely possible to live life dry.

The Dreaded Bowel Preparation For A Colonoscopy… How Bad Can It Actually Be? What if I have questions or I take medications?

our doctor has informed you that you need to have a colonoscopy. It may be for a routine screening due to your age or to investigate a problem, but regardless you know the dreaded bowel preparation will be needed. You may have heard horror stories from your friends and family and often it is the thought of the bowel preparation, which stops patients from having this needed test. It is important for you to know that there are many preparation options and adjustments in timing that have made the bowel preparation for today’s colonoscopy much improved over that of the past.
Why do I need a Bowel Preparation? The intent of the bowel preparation for colonoscopy is to remove all debris and stool from the colon so that your physician can have the clearest possible view of the colon. Any stool left in the colon will hinder your doctor’s ability to see small polyps and may lead to the need to repeat the exam.
Will I need to starve before the test?
It is very important that you read all your directions thoroughly. Your doctor and their nurses and office staff are available to answer any questions. Your doctor will instruct you about which medications to take and which to hold. Diabetic medications may be adjusted, but typically blood pressure, heart or seizure medications are taken as usual. Some medications, like blood thinners may be held for up to a week before your procedure and some preps need to be prepared the night before so look at your directions before your scheduled date. Finally, you should realize that although the bowel preparation may not be the easiest part of your exam, it is certainly one of the most important. Your doctor and their staff are there to ensure that the preparation chosen is the right one for you and that you complete it properly, so that the important and needed colonoscopy can be completed as effortlessly as possible.. Starving is not required but you will be limited to fluids only for about 24 hours before your exam. These fluids do have nutrition and taste. You are allowed to drink water, broth, coffee, tea, Gatorade, fruit juices, soda, ice pops and Jell-O. No red fluids should be taken. There should be no pulp or sediment in the liquids. A small amount of milk added to your coffee or tea is allowed, as are sugar or artificial sweeteners.
Will I have to drink a nasty tasting laxative to prepare? There are multiple methods to cleanse the colon and the majority doesn’t have a bad taste. Which preparation is chosen may vary depending on your health and your doctor’s preference. MiraLAX laxative is a colorless powder that can be dissolved into any fluid without leaving any taste or grit. Citrate of Magnesia is a salt solution, which can be diluted with other liquids to make it more palatable. Flavored buffered solutions like Golytely can also be utilized by prescription. Lastly the pill prep, Osmo-prep, can be taken in select individuals without any history of high blood pressure, kidney disease or seizures.
When will I have to drink the laxatives and can I work that day? The timing of your preparation will vary with your scheduled arrival time, your doctor’s preference and your individual health needs. Typically for procedures scheduled before noon you will begin your preparation when you awaken the morning the day before your exam, starting your liquid diet with breakfast and taking the first dose of laxative about 10 am. For those patients with a procedure scheduled later in the day you may have what is called split dose prep. This allows you to have a solid breakfast the day before your exam and start your liquids at lunchtime. You will begin the laxative component of your preparation at five pm the day before the exam and you will awaken early on the day of your procedure to complete the second half of your laxative preparation. With the split dose prep you can easily work the day before your colonoscopy.
When do I stop drinking liquids? Finally, because you will be receiving anesthesia for your examination, your stomach must be empty of all fluid and food for at least four to six hours prior to the exam. Unfortunately, for many people today, it is also where a great deal of stress and uncertainty reside. The word “homeâ€? carries with it many unwritten and personal connotations. It is a word that I never take for granted when meeting with clients in trying to lay out objective options for resolving the financial problems associated with saving or, in some cases, walking away from one’s home. The following are a few of the questions and issues I address with my clients:
When does a “home� become just a “house�? This is not an easily answered question. Some of my clients’ homes have been passed down from generations; others built from scratch. Most are raising children within its walls; others have memories of being raised themselves there. There is value in saving a home that cannot be quantified. However, it is important that these factors are measured in balance with objective, quantifiable facts. Can I afford these payments anymore? Even if I can, does it make sense in light of the home’s value in today’s market? Part of my job is to address these questions directly and provide enough information so that a client can make an informed decision about their family’s future.

I have heard enough horror stories from clients about the rigors and disappointments associated with applying for a loan modification that my best advice is to proceed with caution, especially if a homeowner is presently current with their mortgage payments. Most, if not all, mortgage companies will not consider a homeowner a candidate for a loan modification unless they are at least three months behind in their payments. Therefore, someone who is presently current must make the decision as to whether it makes sense to unilaterally decide to stop making payments. This is treacherous territory, as no mortgage company guarantees approval before you stop making payments. The success stories from a loan modification are few and far between. With no guarantee of approval, one could quickly find themselves behind on their mortgage and at the mercy of the company holding same. Of course,
for people who are already behind, there is no harm in trying; however, it is easy to get caught up in the vicious and never-ending cycle of rejection and/or never getting a response. It is important for people to be proactive, strategically, in not putting all of their eggs into the proverbial loan-mod basket.
*HU IHURY\W[J` ZH]L T` OVTL& Chapter 13 bankruptcy is a means by which a qualified debtor can stop the clock on a pending foreclosure and repay their mortgage arrearages interest-free over a period of, usually, three to five years. There are many factors that go into whether a Chapter 13 reorganization plan is feasible for a person. Income and assets, respectively, play a huge role in whether or not such a plan would be approved by the bankruptcy court. Every case is fact-specific, however, it is important to know that such an option exists. It is imperative that this potential investment into the home be considered along with its present market value. Many times it is at this juncture that folks must consider whether it makes more financial sense to move on from a home through some alternative means, whether by short sale, deed in lieu of foreclosure, or in some cases, a Chapter 7 bankruptcy, which is known as a liquidation. Every case is different, and professional advice on qualifying and making the right decision moving forward is a necessity.

Preimplantation Genetic Testing and In-Vitro Fertilization With the ever-increasing awareness and use of in vitro fertilization (IVF) to achieve pregnancy, women may be surprised to learn that their embryos can be tested to check for problems that may affect their likelihood of a successful pregnancy and the future health of their offspring. Preimplantation genetic testing can be performed on high-quality embryos, and this technology can improve the selection of a genetically normal embryo that in turn has the possibility of reducing future genetic disease.

PGT is a laboratory technique where a few cells of an embryo are removed and tested, providing genetic information that is used to select the best embryo/embryos suitable for transfer. Patients undergo an IVF cycle, and embryos are then tested either for their chromosome number or for a specific genetic mutation.
What tests are done with PGT?
Embryos undergo genetic testing to check for either a specific disease, or are screened to look at the number of chromosomes present. The results tell whether an embryo carries a specific genetic mutation, or if it has a normal number of chromosomes. There are two techniques that correspond to each testing condition. This includes preimplantation genetic diagnosis and preimplantation genetic screening.
What is preimplantation genetic diagnosis (PGD)? For women with hereditary or familial cancers, one way to avoid the risk of potentially transmitting genetic mutations is to perform PGD. PGD involves testing for
specific genetic mutations, such as the BRCA breast cancer gene, cystic fibrosis, and other single gene disorders. PGD can dramatically decrease the chances of disease in patients undergoing IVF.
What is preimplantation genetic screening (PGS)? PGS involves testing an embryo for chromosome abnormalities and chromosome number. This testing becomes more important in older women or in those with advanced ovarian aging following fertility testing. It does not test for specific genetic mutations or diseases. as with PGD. Since many women and men will have eggs and sperm that are not chromosomal normal, use of PGS can help increase the pregnancy success following IVF and embryo transfer.

If so, there is a possibility that you can be suffering from weakening of the pelvic floor muscles because all of the above are common causes of pelvic floor disorder (PFD). A common form of PFD is a rectocele. What is a rectocele? A rectocele is a bulging of the front wall of the rectum into the back of the vagina. A rectocele is caused by the thinning of the rectovaginal septum. The rectovaginal septum is the tissue between the rectum and the vagina. This occurs secondary to weakening of the muscles of the pelvic floor. Other pelvic organs that may bulge into the vagina are the bladder (this is called a cystocele) and the small intestine (called an enterocele). All of these disorders are classified as forms of pelvic floor disorder (PFD). What are the symptoms of a rectocele? A majority of patients who suffer from a rectocele have no noticeable symptoms. When symptoms are present, they are classified as either rectal or vaginal. Women who experience rectal symptoms may be having a difficult time evacuating during bowel movements. Some women actually need to press against the back wall of the vagina or the perineal body (the area between the vagina and the rectum) to have a bowel movement. There are women who even have to manually remove the stool from the rectum. Those who experience vaginal symptoms may have a bulge or a fullness in the vagina, protruding of the vagina, and/or discomfort during sexual intercourse. Many women often have a combination of both rectal and vaginal symptoms. A symptomatic rectocele may lead to excessive straining, an urge or need to have multiple bowel movements, rectal pain, fecal incontinence, and/or stool trapping in the rectocele that will seep out of the anus later on during the day. Significant stool trapping in the rectum leads to an increased urge to defecate, especially when standing. A rectocele can usually be diagnosed by taking a complete history and having a physical examination by your doctor. In many cases further testing will need to be done. There are special x-rays, including defecography, barium enema, and pelvic MRI/defecography, that may be performed to help assess the degree, severity, and size of the rectocele.
Most patients can be treated with medical management. Like other rectal conditions, it is very important to have a good bowel regimen to avoid constipation and straining during bowel movements. By doing this, it will allow for softer stools, which eliminates straining during bowel movements and avoids stool trapping in the rectocele. Another treatment is strengthening the pelvic floor muscles with exercises called Kegels. Biofeedback is a specialized form of pelvic floor training that can be done as an outpatient. Surgical treatment will only be considered after the medical alternatives have not been successful. There are three different surgical approaches used to surgically repair a rectocele: abdominal, rectal, and vaginal. Your surgeon will determine which approach is best for you. Surgery for rectocele is to remove the excess tissue that comprises the rectocele and to strengthen the wall of the vagina and rectum. Colorectal surgeons are trained to diagnose and treat rectoceles and other forms of PFD. Dr. Paonessa is a fellowship-trained colorectal surgeon who has been treating women with rectoceles and PFD for over 13 years. Her approach is conservative management of a woman’s symptoms. Surgical intervention is reserved only for those women whose symptoms cannot be controlled with medical/non-surgical management. Most women exclaim that they cannot believe they waited so long to seek treatment and relief of their symptoms, especially since treatments are quite simple. Women who have received treatment for their symptoms of rectocele/PFD have improved quality of life. The anxiety and social stress and/or isolation associated with the symptoms caused by a rectocele/PFD can be eliminated. Cigarette smoking is injurious to health‌It is written right on the package itself. Then why do 45 million adults continue to smoke? Maybe they don’t know how negatively cigarettes can affect your health. The number of smokers has gone down consistently in the last few decades. In 1965, 42% of the population smoked; it is decreased to 19 percent by 2010. This is due to the increasing awareness about the adverse effects of smoking. Nineteen percent is still a significant part of the population. Tobacco-related illnesses are a major cause of death in adults. • Why do people smoke? Most people start smoking at a young age. 1 in 3 male and 1 out of 5 female high school students used some form of tobacco, based on a survey in 2009. Often, the habit is started due to peer pressure, to impress people who are smokers. The tobacco industry also spends millions of dollars on marketing that attracts youngsters. Once started, smoking is difficult to control. Nicotine is a very addictive substance. It gives a pleasant feeling by working on the brain cells, altering the chemistry, making the person want to smoke more. The habit-forming potential of nicotine is similar to that of drugs that can cause addiction. Smokers develop emotional and physical withdrawal symptoms when attempting to quit. So even when someone wants to quit smoking, the process becomes very difficult. • What are the diseases that are related to smoking? Heart disease, atherosclerosis, lung disease (COPD, Emphysema), vascular disease (blockage of arteries), stroke, cancer‌ the list goes on. Heart disease is the number one cause of death in the Unites States. Smokers are at high risk of getting heart attacks and of sudden death from heart disease. Smoking especially damages the lining of the lungs, resulting in chronic lung diseases such as COPD. COPD is the fourth leading cause of death in the US. There are 12 million people in the US suffering from COPD and 75 percent of those cases are related to smoking. The risk of developing COPD is related to the number of cigarettes smoked and the duration. • What are some of the cancers that are related to smoking? Smoking accounts for 30 percent of cancer related deaths in the U.S., and causes 80 percent of all lung cancers. The risk of developing lung cancer is also proportional to the number of cigarettes smoked in one’s lifetime. Smoking can also cause cancers of the throat, mouth, nasopharynx, larynx (voice box), esophagus and urinary bladder. Cervical cancer and pancreatic cancer are also linked to smoking. • I was already diagnosed with cancer; what is the point of quitting now? This is very common attitude of smokers when they are hit with a diagnosis of cancer. Actually, the treatment of cancer will be more difficult if a person continues to smoke. Side effects such as nausea, mouth sores and loss of appetite are more pronounced in those who choose to smoke than those who decide to quit.

The Left Side of my Abdomen Hurts â&#x20AC;&#x201D; Could it be Diverticulitis? Abdominal pain is one of the most common complaints patients have when they come to my office. There are many causes of pain in the abdomen and pelvis. If the pain is in the lower abdomen, and particularly if the pain is located on the left side, it may be a symptom of diverticulitis. Diverticulitis is the inammation of small out-pouchings of bowel called â&#x20AC;&#x153;diverticula.â&#x20AC;? Although many forms of diverticulitis are resolved with rest, temporary changes in diet, and antibiotics, some extremes cases may require surgery. Early detection and treatment is crucial in fighting this malady. Q: What are diverticula? What causes them? A: First, the â&#x20AC;&#x153;bowelâ&#x20AC;? are the loops of intestine in your abdomen that digest food. All of us have a small bowel and large bowel (also called the colon). â&#x20AC;&#x153;Diverticulaâ&#x20AC;? are small sacs that form off the wall of the bowel. Although they can form anywhere along the intestines, the left colon is the most common location. Nobody knows for sure what causes diverticula to form. One popular theory has to do with pressure from the muscle spasms moving food and stool through the bowels. Food enters the large bowel, where it continues to digest. By the end of the large bowel, the food is now considered stool and needs to be defecated. That is what we call a â&#x20AC;&#x153;bowel movement.â&#x20AC;? The colon has to produce lots of pressure to move heavy/dense stool through its long length. With that buildup in pressure over decades, small bulging pouches can form at weak areas of the colon wall. These small pouches are called diverticula. Once diverticula form, they do not go away. Q: How common is it to have diverticula? A: Having multiple diverticula is called diverticulosis. There are no symptoms of having diverticulosis. This is rare in patients under 30 years old. But it is very common in patients over 60 years old. Diverticulosis is slightly more common in men than women. The stool in your colon moves from right to left. The stool in the left side of your colon is the densest and it is being held until the next bowel movement. This causes increased pressure in the left side of your colon compared to the right. Therefore most diverticula are found in the left side of your colon.
Q: How do you diagnose diverticulitis? A: A physical exam performed by a doctor can diagnose diverticulitis. A CT scan is often performed to see if the diverticulitis is mild and uncomplicated and can be treated with medications alone, or if it requires surgery. Early detection and treatment is crucial.

Q: What is diverticulitis? How is it treated? A: When a diverticula becomes inflamed, it is called diverticulitis. The symptoms include pain, especially in the left lower abdomen, as well as fever, nausea, and change in bowel habits. Mild cases can be treated with antibiotics, a liquid diet for a few days, and Tylenol for pain. Severe cases of diverticulitis can cause the diseased bowel to scar down (called a stricture) and not let any stool pass. In some cases, a hole may form in the bowel (perforation), which can spill stool, acids, and bacteria into your abdominal cavity. In these cases, an abscess may form and requires drainage. In other cases, the diseased bowel can â&#x20AC;&#x153;stickâ&#x20AC;? to the vagina or bladder and form an unnatural connection (called a fistula). In these severe cases, surgery may be required to repair (or remove) the diseased bowel and fix any additional complications.
Q: What are the risk factors to develop diverticulitis? A: Smoking, diets high in animal fat and low in fiber, obesity, lack of exercise, and advanced age are all risk factors. Other risk factors include medications such as steroids, opiate painkillers, and non-opiate painkillers (such as aspirin, ibuprofen, and naproxen).. Q: If I have diverticulosis, can I prevent having diverticulitis? A: To prevent diverticulitis, doctors often recommend maintaining a healthy weight, exercising regularly, and not smoking. High-fiber foods with lots of water can also help decrease the risk of diverticulitis. Q: What about avoiding nuts and seeds? A: In the past, it was thought that foods which contained indigestible particles such as nuts, fruits with very small seeds, and popcorn caused diverticulitis. The thinking was that these tiny nuts and seeds can fall inside a diverticula and cause inflammation. However, a recent large study has shown this was not the case, and patients who ate popcorn and nuts did not increase their risk of developing diverticulitis. That being said, many patients who have had multiple bouts of diverticulitis believe these types of food are triggers.

Her business includes a training school with a Certified Nurse’s Aide program (CNA) and a Certified Home Health Aides program (CHHA), medical equipment with a full line of quality products ranging from lift chairs to beds, and and our exciting new unique boutique which carries mastectomy products, postsurgical/ recovery items, maternity and more. We have certified fitters on staff either by appointment or walk in. She describes herself and the way she conducts her business as very “hands-on” and is lovingly involved in every aspect. Kathleen is that rare individual who truly derives pleasure from helping others. If there is a need, she will find a way to fill it.

Like Kathleen, her various businesses have evolved over the years and each part has played an instrumental part in
helping someone that has been connected to her life. In fact, her home health care profession began while she was caring for two sisters while the family lived in another state. “The family had given me the confidence boost that I needed to pursue my business. They were so happy with everything that I had done with their relatives and felt that I took care of them as if they were my own family and in all honesty, they were.” In the home health care business, rules and regulations are constantly changing. She adds, “Many folks have noticed a huge change with Obama Care and though it does have many benefits, it has created a great deal of change and with that comes different pros and cons.”

One of the priorities in her state approved training school is teaching compassion. In fact, her school has become known for the compassion that is shown through her aides that work with the facility. She adds, “Just by having that ‘Karing’ quality they stand out. I had an administrator personally call me recently to compliment me on it.” Due to the training that her instructors instilled in their students, this gentleman was able to pick out which came from her site. Her instructors are amazing nurses and women. She says, “We share the same beliefs and that’s what makes us work so well together. I am grateful for every single one of them.” The overall objective of the program is to prepare participants to become a CNA or a CNHA. This training will enable them to provide the highest quality of care in a compassionate manner; all while maintaining the client’s dignity and independent. In the end, they will be the ones enhancing the client’s quality of life.”
Her DME Company started when she began receiving calls from her aides about things they needed to keep their clients safe in their homes. She adds, “We wanted to do whatever was necessary to continue to keep our clients at home, happy and safe.” Just some of the products she offers include canes, walkers, wheelchairs as well as items to make everyday tasks easier like pill openers and grab bars. Her journey did not end there. She went a step further and opened up the Mastectomy Boutique. She adds, “We continue to help many women going through a mastectomy. Women need to know that prior to their surgery they are entitled through most insurance companies to receive a compression bra that is equipped for post surgical needs.” They have certified fitters at each of her locations to assist.

Perhaps the best part about her profession is that it includes many of her own family members. “It’s wonderful to be working with my family but debates do occur because each of us is so passionate about what we do. My family grew with my business as I had started the company while my own children were still in school. It’s nice to see the two of them grow alongside one another. Each of my children play a critical role in my company,” she adds. Hers is not the typical 9-5 job and sometimes the job is never really “done.” The greatest advantage for her is that they all live next door to each other. Their bond is very tight and it enhances her business in immeasurable ways. For Kathy, it all comes down to making a difference in her client’s lives. Contraceptive Use: Which Option is Right for You? With so many safe and effective contraceptive options, each with their own pros and cons, it can be overwhelming to choose the best method. And depending on age, what works for women in their 20s may not be the right fit in their 40s. James P. McDermott, DO, a board certified ob/gyn at Ocean Woman’s Healthcare Group, part of Barnabas Health Medical Group, counsels countless women on various health issues, including contraceptive use.
Q: What are the most popular contraceptive (birth control) options? Dr. McDermott: Fortunately, there are more options than ever before. Birth Control Pills – The most popular option, birth control combination pills contain two hormones: estrogen and progestin. Typically, patients start with a lower dose estrogen pill, which produces lower side effects. Higher dose estrogen pills may cause weight gain, headaches and nausea. If taken as prescribed, birth control pills are 91% effective. But, women need to remember to take the pill at the same time every day.
“I present my patients with an overview of contraceptive options and take many factors into consideration to determine the best method,” says Dr. McDermott. “Age, health, lifestyle, childbearing plans and personal preference are all taken into account. A woman who is over 35 and is a smoker, for example, must avoid an estrogen/progestin birth control pill because it can be dangerous to her health. By learning about the options it becomes more clear what’s best.”
Over the years, the amount of estrogen in birth control pills has decreased significantly. In the 1980s, pills included 50 micrograms of estrogen and today, a low dose pill is just 10 micrograms.
Here, Dr. McDermott provides insight on contraceptives.
Depo-Provera – Taken by injection every three months, Depo-Provera contains progesterone only, and tends to have more side effects than birth control pills. This is not the best option for women who wish to conceive because it takes about 18 months for periods to become normal after stopping the Depo-Provera.
Q: Are contraceptives used only for birth control? Dr. McDermott: While the top reason women use contraceptives is to prevent pregnancy, there are other uses. Contraceptives are also prescribed to treat certain disorders that cause heavy bleeding and menstrual pain, such as fibroids and endometriosis. Many teenage girls who suffer from difficult menstrual cycle symptoms can experience much relief and regularity by going on a low-dose birth control pill. I’ve also seen how it helps them with their self-esteem – they can get along with their lives just like every other teen. A low dose birth control pill can help some women in their 40s have a smoother transition into menopause.
Effectiveness: 91% Birth control pills are a good option for those who want to both prevent pregnancy and improve acne, bad menstrual cramps or other PMS symptoms.
Effectiveness: 94% Vaginal Ring – The ring, which provides a combination of estrogen and progestin, is inserted into the vagina and left in for three weeks. Then it’s taken out for a week, during the monthly period. After the period, a new ring is inserted. This cycle continues. The ring releases hormones that prevent ovaries from releasing eggs. Side effects are the same as the birth control pill. Effectiveness: 91%

Effectiveness: 91% Long-acting reversible contraception (LARC) methods (IUD and birth control implant) Over the long term, long-acting reversible contraception (LARC) methods like the IUD and birth control implant, are highly effective in preventing pregnancy, last for several years, and are easy to use. Intrauterine Device (IUD) – The small, T-shaped plastic device is inserted into and left inside the uterus. The hormonal IUD releases progestin and is approved for use up to 3 to 5 years. The copper IUD, which does not contain hormones, it approved for use for up to 10 years. Benefits of the IUD: once it is in place, women do not have to do anything else to prevent pregnancy; it doesn’t interfere with sex or daily activities. In addition, the IUD can be removed for pregnancy. Side effects: non-hormonal IUD may cause heavier periods and more cramping; but hormonal IUDs can make your period irregular or generally may not get a period at all. Effectiveness: 99.2% Birth Control Implant – This very small flexible rod – about the size of a matchstick - is inserted under the skin of a woman’s upper arm by a healthcare professional. It releases progestin into the body and protects against pregnancy for up to three years. Effectiveness: 99.05% (Effectiveness statistics are according to the Centers for Disease Control and Prevention)

Atlantic City Ballet Ends 34th Season With Swan Lake Of all the great classics, Swan Lake remains the quintessential ballet of all time. It is the story of romance, sorcery and betrayal, and centered on one of life’s most recognizable themes, the struggle between good and evil. Ms. Papa infuses Atlantic City Ballet’s production with a new passion that embodies all the beauty, strength, and grace of classical ballet. The Atlantic City Ballet is bringing this timeless classic to the stage as the grand finale of their 2016/2017 season. Their 34th season will culminate with an end of season fundraiser on June9th at the Golden Nugget in AC. After a short break the ballet will begin its 35th Season by continuing its residency at the Circus Maximus Theater at Caesars Hotel. Their first performance will be Visions on Sunday July 30th. The company will perform every month bringing a new ballet to AC which will include Ballet Goes Broadway, In the Eye of the Storm, Dracula and Nutcracker. There’s a lot in store for the company next season. They’ll be taking their performances on the road with performances at The Strand in Lakewood, NJ and out west with stops in L.A., Arizona and possibly Mexico City, Mexico Founded in 1982, by International Ballerina, Phyllis Papa, the Atlantic City Ballet is comprised of professional dancers from around the world and have earned national acclaim for their technical expertise and exuberant stage presence. Although their home base is in Historic Boardwalk Hall in Atlantic City, the Ballet has performed in theaters all along the east coast. Their repertoire of original works includes such classics as A Midsummer Night’s Dream, Sleeping Beauty and Swan Lake but Ms. Papa is most known for her innovative works such as Dracula, Caught Up In The Swing and her full length Carmen. Since its inception, the Atlantic City Ballet has served as one of the cultural treasures of New Jersey and continues a tradition of dance excellence for future generations of ballet audiences.

Radiation Therapy After Breast Cancer Surgery Dr. Sumy H. Chang Answers Your Questions Following breast cancer surgery, radiation therapy may be recommended as part of the overall treatment. Fellowship trained Sumy H. Chang, MD, FACS, a boardcertified general surgeon dedicated to breast health, says the goal of radiation is to reduce risk of the cancer coming back. A member of Barnabas Health Medical Group, in partnership with Community Medical Center, Dr. Chang provides Ocean County women with access to the latest in prevention, detection and treatment for breast-related issues – including breast cancer. She is also the Director of the High Risk Breast Program at the Jacqueline M. Wilentz Comprehensive Breast Center at Monmouth Medical Center in Long Branch. Here, Dr. Chang provides an overview on breast radiation therapy. Here, Dr. Chang answers some of the most common questions about breast cancer.
Q: When is radiation therapy used for breast cancer?
A: Dr. Chang: Radiation is recommended for a large majority of lumpectomy patients, since there is a higher chance of the cancer returning after a lumpectomy. Usually, it’s early stage and located in one site. In my experience, few women who have undergone a mastectomy will need radiation. In fact, most women choose mastectomy to avoid radiation. This is because radiation can only be administered once to one particular body area in a person’s lifetime. According to breastcancer.org, based on recurrence factors, about 20% to 30% of people are considered at high risk of recurrence after mastectomy. Radiation would be recommended to help reduce this risk by up to 70%. Treatment would be administered to the area where the breast was previously located and in some cases, to the lymph node regions near the site.

breast radiation therapy – the most common type – which starts 4 to 6 weeks after surgery. The other is partial-breast radiation which is typically administered to the immediate surrounding tissue to the surgical site. Partial breast radiation can be administered internally or externally.
Q: What are the side effects of radiation?
A: Dr. Chang: The number one side effect is changes to your skin. After radiation treatment, your skin may itch, burn and peel – similar to sunburn. This uncomfortable feeling may last a few weeks. For most people, the skin will return to its natural state. However, people with darker skin tone may experience lighter or darker skin following radiation treatment.

The second biggest side effect of radiation is fatigue, which occurs toward the end of treatment. However, I’ve found that more active women experience less fatigue. Radiation treatment to the breast area usually doesn’t cause nausea or vomiting.
Q: What are the biggest myths about radiation?
A: Dr. Chang: One of the biggest myths is that you will lose your hair. You will not lose the hair on your head; however you may lose any hair that’s on your nipple or lower armpit (near your breast that received radiation). People also think that radiation is very painful. Some patients may feel a slight warming or tingling during treatment, but most patients don’t feel anything.
Q: What is new with breast radiation treatment?
A: At Monmouth Medical Center, we have a special protocol where breast radiation is coordinated with the patient’s breathing so that radiation has less effect on the woman’s organs (lung and heart). It minimizes the radiation scatter effect to the organs. Community Medical Center has a protocol with Georgetown University where they do partial breast radiation for early stage breast cancer as part of a study.

CHRONIC SINUSITIS AND SINUS ALLERGY VICTIMS SUFFER NO MORE!!!!!!!

reetings, readers and friends! I hope New Jersey has not yet presented you with “spring fever.” You know, the coughing, sneezing, congestion, nasal drip, itchy eye, headache, dizzy, miserable feelings of seasonal allergies and chronic sinusitis. It is expected that 2017 will bring on some of the highest mold spore and pollen counts in history and, with that, thousands of individuals suffering with the above symptoms and no new therapies to combat them! At least until now……. Until now, the currently available medications minimize symptoms at best for a small minority of allergy sufferers and often, the side effects of the therapies are equally troublesome if not worse than the symptoms they are intended to resolve. Perhaps the better therapeutic option would be one that addresses the underlying cause(s) of the symptoms rather than the symptoms. Believe it or not, this therapy already exists and has been available for over half a century. This therapy is part of the air we breathe that contains the same irritants causing our allergy symptoms. The therapy is known as OZONE, which is a supercharged molecule of oxygen that is manufactured in nature through electrical storms that also is part of what colors the sky blue and produces the familiar freshness scent of rain. Ozone nourishes and cleanses our body just as it nourishes our plants and cleans our atmosphere. Ozone is used to filter and provide for safe drinking water. It is what protects us from the ultraviolet rays of the sun and, when used medicinally, has the ability to destroy bacteria, viruses, molds, fungus and even cancer cells. You address and treat the underlying problem. More often than not, the cause or etiology of the problem is a fungal infection in those with chronic sinus problems. Mother’s Day
There is historical background on just about everything, including Mother’s Day. The National Women’s History Project has information about the history of Mother’s Day and it is quite interesting. The information provided reflects that Mother’s Day began as early as 1858 when a woman by the name of Ann Jarvis, who was an Appalachian “homemaker,” as the term was referred to at the time, organized what was known as “Mother’s Work Days.” The concept of the work day was to improve sanitation and

prevent death as a result of “disease-bearing insects as well as seepage of polluted water.” Later, in 1872, Julia Ward Howe was known to have established a day for mothers and peace after the Franco-Prussian War. After the death of Ann Jarvis, in an effort to memorialize her mother’s life of activism, Anna Jarvis campaigned for a Mother’s Day resolution from Congress. By May of 1907, the second Sunday was designated as Mother’s Day at a church in West Virginia where Ann Jarvis was known to have taught Sunday school. Also, on the second Sunday in May of 1907, there was a service that was hosted at the Wannamaker Auditorium in Philadelphia, Pennsylvania. It is reported that over 15,000 people showed up for the service. Currently this location is now known as Macy’s; however, the Wannamaker building was known for its prestige and architecture in downtown Philadelphia. By the year 1912, the “custom” of celebrating Mother’s Day on the second Sunday in the month of May had spread across the United States. West Virginia and Pennsylvania had proclaimed Mother’s Day as a national holiday. In 1914 our very own President Woodrow Wilson had signed a Congressional Resolution for Mother’s Day. Keep in mind that it would not be until six years later he would grant women the right to vote. In 1920 Ann Jarvis began to feel as though Mother’s Day was being too commercialized and attempted to have the resolution rescinded. Its purpose then was (and still is today) to celebrate “our mothers” for their strength and commitment to their families and their communities. Although the celebration of mothers around the world is designated on one specific day of the year, I believe it is important to reflect on the efforts made by all mothers on a daily basis. The traditional role of a mother has evolved quite dramatically since the 1800s. Mothers have become more educated with higher-level degrees—they are doctors, lawyers, business professionals; they represent the United States military and serve in combat zones; they run major corporations; they serve as public officials, police officers, firefighters; and the list goes on and on. There are many mothers who work full time and contribute to the household income. Some of these amazing mothers may now even be the head of the financial household.

In addition to out-of-the-home employment, many women essentially work a second shift upon their return to the family home in which they become the “chief operating officer” of the family empire. This position entails the management of everything from transportation for their children to and from after-school activities to dinner and, yes, even laundry. Now, for those dads out there who may feel slighted, they too are an integral part of the family unit. However, it is usually Mom who provides direction and facilitation of the daily schedules. It is also important to recognize those mothers who volunteer at school functions time and time again for all of our children. These mothers appear to effortlessly organize pasta dinners before sporting events, decorate gymnasiums for school dances, bake for school fundraisers and keep working mothers like myself organized. As a society we tend to lose sight of the mothers “who are always there” because they are not looking for anything in return. These are the mothers who are always putting our children before themselves and who make sacrifices to always be there for “the kids.”

Diabetes, Cholesterol, and Blood Pressure—Oh My!

The last couple of decades tell a “good news, bad news” story about diabetes. Diabetes is the disease that makes it tough for your body to control blood sugar. Here’s a snapshot: In the last 20 years, the number of adults with diabetes has more than tripled.1 Nearly one in 10 now has the disease.2 That’s certainly not good news. But there are some bright spots in recent reports. In general, control of blood sugar has improved and rates of serious complications from diabetes have declined a great deal. If you’re at risk for diabetes or already have it, consider these three questions:
1. Are you making good lifestyle choices? Where people haven’t made progress, lifestyle has a lot to do with it. For example, it’s no secret that the obesity epidemic is growing, right along with the diabetes epidemic.1 Think about the lifestyle choices you make. Yes, you’ve heard it all before, but it still holds true: Make healthy food choices—and understand diabetes meal planning. Exercise 30 to 60 minutes most days of the week, move to or stay at a healthy weight, and manage your stress as well as you can.3
2. Do you know your diabetes ABCs? Knowing your numbers is also a big piece of the diabetes story, especially if you are at risk for or have high blood pressure or high cholesterol. High blood pressure is when blood moves through your vessels with too much force. Cholesterol is a fat-like substance that can cause plaque buildup in arteries. Combined, these three give your heart and blood vessels a triple whammy.4 These are the diabetes ABCs: • A is for the A1C test. That’s the test that shows what your blood sugar levels have been the past three months. In most cases, you want to shoot for a number below seven. • B is for blood pressure. A great goal is below 140/80. The top number measures pressure in vessels as your heart beats. The bottom number measures pressure when vessels relax between beats.5 • C is for cholesterol. Ask your doctor or me for the numbers you need.4
3. Are you managing your medicines? If you have type 1 diabetes, you must use insulin. Some people with type 2 diabetes need pills and/or insulin to meet target blood glucose levels.6 Although lifestyle is a first line of defense, cholesterol medications such as statins can help control cholesterol levels.7 As for high blood pressure, there are many classes of drugs to keep this silent killer at bay.5 It may not be easy keeping track of all this, especially if you’re taking multiple medications. That’s what I’m here for. I can explain how each drug works, what side effects to watch for, and how to make sure you’re taking them all properly.

Brenda is a Lead Volunteer with Red Cross Disaster Services who is extremely dedicated to the Red Cross mission and committed to serving her community. Brenda is always at the ready to provide compassionate care to those devastated by disasters and can regularly be found running out in the middle of the night to provide comfort and emergency financial assistance to families who have been displaced by home fires. She also helps the New Jersey Region prepare for largescale disasters by conducting shelter surveys, ensuring space is available should a shelter need to be opened. Brenda also mentors new volunteers, lends a helping hand around the office and supports Red Cross Service to the Armed Forces programs. During her five years with the Red Cross, Brenda has volunteered more than 700 hours and has been on-call and ready to respond to disasters for an additional 1,800 hours!

On top of that, the conference, like most AOWIE events, was open to the public. Keynoting the conference was Carla Hall from the ABC hit television show, “The Chew” and Dolores Catania, star of the Bravo TV show, “The Real Housewives of New Jersey.” Both ladies did a phenomenal job sharing their own personal entrepreneurial journey. Their words were priceless and received an overwhelming positive reaction from the sold-out crowd of innovators! Specifically, Carla proved to the audience that when you have passion and focus, all things are possible. On the other hand, Dolores placed emphasis on the importance of perseverance and independence. It is clear she is a business savvy woman and a rockstar mom who motivates her children to do their best each day. AOWIE was blessed to have mounds of other talented, iconic speakers grace the stage including Tracey Washington Bagley, Executive Producer of the ABC television show, “Here and Now” hosted by Sandra Bookman. Another special guest included, Delores Spruell Jackson, Producer of the ABC television show, “Live with Kelly.” Other notable speakers in attendance included Singer/Songwriter, Alyson Williams, Ralph McDaniels, Founder of Video Music Box, Lora Gaston from Gaston Business Accelerator, Shontavia Johnson, renowned Patent Attorney, Tara Ackaway, Social Media Strategist, Public Relations Expert and Founder of Social Wise Communications, Michelle Irene, Inventor and Author, Sonia Alleyne, Marketing Expert and Inventor, Maggie Linton from Sirius XM Radio, channel 126 and last but certainly not least, Cindy Russo, On-Air Host of QVC Television. Due to the power music holds, music has always been a focus for AOWIE. One of the organization’s taglines is, “What’s Your Song?” All AOWIE speakers and performers walk onto stage to a song that inspires and empowers them to be the best version of themselves as part of their introduction to the audience. Audience members love to be involved by proudly wearing their “What’s Your Song?” t-shirts and sharing their favorite songs with one another. At this past conference, the audience had the honor of listening to the headliner band, CSN Songs perform. The band received several standing ovations and smiles galore. CSN Songs is a local tribute band that performs some of Crosby, Stills, Nash and Young’s biggest hits. The AOWIE culture coupled with great music sure does know how to bring great people together. NATIONAL CANCER SURVIVORS’ DAY is on June 4, 2017

Celebrating Life… While Living with Cancer Submitted by Dr. Sarah Easaw (Dr. Sarah Easaw is a board-certified Hematology and Oncology specialist, practicing in Ocean County, NJ)
ust before I turned 30, I was diagnosed with stage III breast cancer. My daughter was six months old at the time. Doctors said I needed surgery, chemotherapy and radiation. I was devastated… I didn’t know what the future would hold for me and our family…” Tina,* now 36, was explaining her story at the Cancer Survivors’ Day celebrations conducted by our practice, Ocean Hematology & Oncology. She came to the party with her two beautiful children… two-year-old Mark* and Elena,* who will soon be seven. Later Tina was dancing to the country music with the two of them. Several people joined – many of them have survived cancer; some are still dealing with it. But this is a day they want to forget about their struggles, not thinking about the months of chemotherapy, radiation and surgery that most of them had to go through. Not worrying about the uncertainty of the future. They are not concerned about the disability that their disease and the treatment had left them with. This is a day they want to celebrate! They are celebrating life. Statistics show that there are 15 million cancer survivors living in the United States. By 2026 it is estimated that the number will reach 20 million. Cancer treatment has improved dramatically in the last two decades. More and more people are living with cancer, as early detection and treatment have improved. Two out of three patients diagnosed with cancer are expected to live for five years or more, according to the CDC. Living with cancer is stressful. But life after cancer has its own challenges. The National Cancer Survivors’ Day (NCSD) Foundation was started about three decades ago to recognize cancer survivors and their families and to address the problems they face. Every year they organize events nationwide to honor cancer survivors and their caregivers. National Cancer Survivors’ Day is an annual “Celebration of Life” that is held in hundreds of communities nationwide, and around the world, on the first Sunday in June. NCSD states: “It is a CELEBRATION for those who have survived, an INSPIRATION for those recently diagnosed, a gathering of SUPPORT for families, and an OUTREACH to the community.” On this day, thousands gather across the world to honor cancer survivors and to show the world that life after a cancer diagnosis can be fruitful, rewarding and even inspiring. According to the definition given by NCSD, anyone who is diagnosed with cancer is a survivor. Challenges start the day you are told about the diagnosis. They do not end when treatment ends. In fact, for many, the anxiety level increases after the treatment is over. During active treatment, patients are seen by their health care professionals very frequently. When the treatment is done and they are labeled “cancer-free,” a whole new set of problems awaits them. This could be the beginning of anxiety and depression in many, as the security of being with their health care team is no longer there. Many feel like they are set free into a new world, and they have to start a new life, as if they have to learn to walk again.

Even with the new advances in cancer treatment, there are side effects that could linger on for months or years. This would make going back to their previous lifestyle difficult. Although most survivors are able to return to work, they may not be able to perform at their fullest. This, along with the time lost from work during the treatment, can cause significant financial strain. A cancer diagnosis can tear families apart. Although many families get together to support, not everyone is able to cope when a loved one is diagnosed with cancer. Being a caregiver for a cancer patient is not easy. This has led to many separations and divorces, not necessarily from lack of love and affection, but due to the inability to withstand the stress. It is not rare that the survivors feel physical and emotional loneliness at the end of their tiresome journey. The fear of “what if the cancer comes back?” is real for most survivors. Any new symptom, even trivial, can raise tremendous anxiety. This can lead to many doctors’ appointments and unnecessary diagnostic tests. The physical, psychological, financial and emotional strain that survivors and their families undergo has to be dealt with seriously. We have been hosting an annual Cancer Survivors’ Day celebration for the past eight years. It started out as a small gathering in our waiting room. This event has been so wellreceived by our patients and their families; each year the number of attendees has increased. Now we are holding the event in an outside venue. The tremendous support from wellwishers and the dedication of my staff make this event very colorful and successful every year. But it is the survivors and families who are the stars of the day. To see 55-year-old Carolyn,* battling throat cancer; 60-year-old Mary,* a two-time breast cancer survivor, widow of Robert,* who succumbed to blood cancer five years ago; and the children of Tina,* three and seven years old… all come to one stage to dance, laugh, sing and share their stories. To see the joy and happiness on their faces… that is what survivors’ celebrations are all about. Mrs. Francis,* mother of 23-year-old David,* who was diagnosed with CML last month, put it very nicely as she was leaving the party. She lost her older son nine years ago. She said, “Today was my older son’s death anniversary. Usually I sit at home and cry every year, this day. Today, I would have done the same, more so since Daniel was diagnosed with cancer too. But then I thought of the invitation to this event. I told Daniel, we are going to celebrate life today. I went to the cemetery in the morning, prayed for my sons and came for this party. This has been the most wonderful thing that happened to us in a while. I have never enjoyed this much in a long time! Thank you for giving me an opportunity to celebrate life…!” Yes, celebrating life, one moment at a time, is what Cancer Survivors’ Day is all about! *names changed for patient privacy

Ready to Stand Up for a Child Who Needs You? Abused and neglected children need more than the system can provide: • • • •
A six-year-old girl rescued from an abusive father only to be bounced around to eight foster homes and six different schools over the course of two years. Four children separated from one another and placed in different locations after their mother was jailed. A young boy prescribed 12 different psychotropic medicines by healthcare professionals who have no access to his medical records or ability to follow-up on his care. A teenage girl kicked out of her house, and forced to sleep on a cot in the hallway of a group home until she turned 18—when all her possessions were handed to her in a garbage bag as she was officially “aged out” of the system.

These are not isolated stories. They are everyday realities of children in the foster care and child welfare system. A system full of devoted and deeply caring people that is simply too overburdened and under resourced to protect the rights and needs of vulnerable children. The solution is a CASA volunteer—a specially trained advocate, appointed by a judge, who fights for the rights of children during the most vulnerable time of their young lives. A trusted adult empowered to ensure that each child is well cared for—able to find a safe, loving, permanent home. Volunteer Your Time to Change a Child’s Life! Nobody longs for a safe and loving family more than a child in foster care. As a CASA volunteer, you are empowered by the courts to help make this dream a reality. You will be the one consistent adult in these children’s lives, vigilantly fighting for and protecting their fundamental right to be treated with the dignity and respect every child deserves. You will not only bring positive change to the lives of these vulnerable children, but also their children and generations to come. And in doing so, you will enrich your life as well.
What Do CASA Volunteers Do? CASA volunteers listen first. Then they act. Volunteers get to know the child by talking with everyone in that child’s life; parents and relatives, foster parents, teachers, medical professionals, attorneys, social workers and others. They use the information they gather to inform judges and others of what the child needs and what will be the best permanent home for them. Who Can Be a Volunteer? You do not have to be a lawyer or social worker to be a volunteer. We welcome people from all walks of life. We are simply looking for people who care about children and have common sense. As a volunteer, you will be thoroughly trained and well supported by professional staff to help you through each case. You must pass a background check, participate in a 30-hour pre-service training course and agree to stay with a case until it is closed (a year and a half on average). Interested in helping children but not ready to commit to becoming an advocate? Then consider sponsoring a child and providing them with a CASA of their own.
