Rotator Cuff Injuries Are Not Just For Athletes Shoulder pain is the third most common musculoskeletal complaint, behind neck and back pain, seen in medical practices each year. The cause of shoulder pain can range from uncomplicated sprains to massive rotator cuff tears. During a 6-year span, there were over five million physician visits for shoulder pain attributed to rotator cuff problems. Research has indicated that the incidence of rotator cuff damage increases with age due to degeneration of the tendon. This suggests that as the current population ages, rotator cuff repair will also increase. Although the majority of these conditions are responsive to conservative treatment, some may require surgery. Let’s go over some anatomy: The rotator cuff is a group of muscles and tendons that work together to stabilize the shoulder. The four muscles of the shoulder are the supraspinatus, infraspinatus, teres minor and subscapularis. The supraspinatus muscle is responsible for abduction (moving away from the body), the infraspinatus and teres minor provide external rotation (rotating away from the body), and the subscapularis provides internal rotation (rotating in towards the body). These muscles and tendons connect the humerus (upper arm) with your shoulder blade, or scapula. They also help hold the head of your upper arm bone firmly in your shoulder socket. This combination allows your shoulder to have the greatest range of motion (ROM) in the body.

Rotator Cuff Injuries A rotator cuff injury includes any type of irritation or damage to your rotator cuff muscles or tendons. These injuries can range from impingement syndrome to partialand full-thickness rotator cuff tears. Impingement syndrome is a chronic process that presents itself as shoulder pain. If left untreated, it can progress to permanent changes and eventual tearing of the rotator cuff. External impingement is the most common form of impingement syndrome and is caused by compression of the rotator cuff tendons as they pass through the coracoacromial arch. As this type of compression happens repetitively, it can cause inflammation of the bursa lining the joint and can narrow the space further. Other factors can contribute to the narrowing of this space as well, such as bone spurs and arthritic changes. These changes and the progressive degeneration of the tendons can eventually lead to partial- or full-thickness rotator cuff tears. Common causes and risk factors of rotator cuff injuries may include: • Trauma: Falling on the shoulder or outstretched arm, especially in the elderly, when attempting to break a fall.

• Normal wear and tear: After age 40, normal wear and tear on your rotator cuff can cause breakdown of collagen in the cuff’s tendon and muscles, which makes them more prone to degeneration and injury. You can also develop calcium deposits within the cuff or arthritic bone spurs that can pinch or irritate the rotator cuff. • Lifting heavy objects • Repetitive overhead activities (e.g., throwing a baseball, basketball, freestyle swimming, tennis) • Occupational overuse (painting, carpentry, grocery clerking) • Abnormally shaped acromion, which can make impingement of the rotator cuff tendons more likely. • Poor Posture: Slouching causes your neck and shoulders to take a forward position, which causes the space in which the rotator cuff muscles are located to narrow further and can impinge on the tendons. Signs and symptoms may include: • Pain and tenderness in your shoulder, especially when reaching overhead, reaching behind your back, reaching across your body, lifting/pulling, or sleeping on the affected side. • Shoulder weakness, especially in abduction or flexion. Many people frequently describe having significant difficulties combing their hair, holding a hair dryer, and removing clothing. Immediate onset of weakness, especially associated with sudden trauma, can indicate an acute tear. • Loss of range of motion. • Inclination to keep your shoulder inactive. A visit to your doctor is your next step. They’re likely to ask you several questions: • Where is your pain located? • Does your job or hobby aggravate your shoulder pain? • When did the pain first occur? • How severe is your pain? • What specific movements aggravate or alleviate your pain? • Do you have any weakness or numbness in your arm? In the days before your doctor’s appointment, you can decrease your discomfort by trying some of the following: • Rest your shoulder. Avoid movements that aggravate your shoulder and give you more pain. • Apply cold packs. This can help reduce pain and inflammation. • Take over-the-counter pain medications, with the approval of your doctor. Nonsteroidal anti-inflammatory drugs (NSAIDs), such as aspirin, ibuprofen (Advil, Motrin, others) and naproxen (Aleve), may help reduce pain.

Tests and Diagnosis After the doctor examines you, and if your injury appears to be severe or if they cannot determine the cause of your pain through physical examination, they will most likely order one or more diagnostic tests. These may include: • X-rays • Magnetic resonance imaging (MRI) scan • Ultrasound scan Treatment Options The main goal of any therapeutic intervention for shoulder pain is the return to pain-free function. A number of different factors can help determine the course of action that is taken when trying to reach this goal, such as age, preinjury functional level and general health. • Steroid Injections: Your doctor may use a corticosteroid injection to relieve inflammation and pain. • Physical Therapy: This is the most conservative of all of the options. After modifying your activity and controlling pain, physical therapy can begin. In the first active phase of therapy, gentle ROM exercises are started to prevent adhesions or scar tissue from forming, followed by a strengthening program of the rotator cuff and scapular stabilizer muscles. Various modalities can be used concurrently to aid in decreasing pain and inflammation, such as ultrasound and electric stimulation. • Surgery: If a large tear is present in your rotator cuff, you may need surgery to repair it. The surgery may be performed as an open repair with a 2½- to 4-inch incision or as an arthroscopic repair, which is less invasive. • Arthroplasty: Severe, chronic rotator cuff tears may contribute to severe arthritis. Your doctor may want to perform a total shoulder replacement or arthroplasty.

In my spare time, I enjoy spending time with my family, friends and boyfriend, Greg. Together, we have four energetic children, and by children I mean a 10-year-old Chocolate Lab, Bear and three cats, Bruiser, Bailey and Midnight. In April, my family was blessed with my adorable nephew, Hunter Chapman, who has bought tremendous joy into our lives. I look forward to expanding my career at the County Woman Publications. My history with The County Woman began

to learn all of the facets of this very exciting industry. I’ve always enjoyed meeting with local business owners and medical professionals to learn about what they do. To hear a client tell us they’ve seen exponential growth from the articles we circulate, and a reader thanking us for helpful information they’ve read in our magazine, makes my job incredibly rewarding! I’ve lived in Atlantic County for 25 years. Throughout my time working with The County Woman, I also worked with Jaguar, Land Rover and BMW, while continuing my weekly performances as a pianist/singer in The Society Club at the Borgata for the past eight years. I take the most pride in being a Mom to my son and daughter, two of the greatest gifts in my life.

and am the newest staff member at the County Woman Publications. When I started working for the County Woman Publications, I knew immediately this is where I wanted to have a career. It has allowed me to further my interest in graphic design while being creative in a fast paced business environment. I enjoy designing ads and pages alongside our very talented and experienced graphic designers and learning more about each of the amazing businesses that we feature in these publications. When I am not in the office, I enjoy going exploring with my friends and boyfriend, Mike, either climbing waterfalls or taking long drives to broaden our horizon. It is exciting finding new and different places in and around New Jersey. In addition, I enjoy curling up and watching movies with my two cats, Bella and Little. As a current resident in Atlantic County, I am excited and proud to see all of the businesses grow with each issue throughout the county. Psoriasis Psoriasis is a noncontagious skin condition that produces plaques of thickened, scaling skin. The dry flakes of skin scales result from the excessively rapid proliferation of skin cells. The proliferation of skin cells is triggered by inflammatory chemicals produced by specialized white blood cells called lymphocytes.

Erythrodermic psoriasis presents as generalized erythema, pain, and itching. It typically encompasses nearly the entire body surface area. It may be accompanied by fever, chills, hypothermia and dehydration secondary to the large body surface area involvement.

There are different types of psoriasis. Some affect only skin while others affect other vital organs of the body like eyes, oral cavity, nails and joints.

Scalp psoriasis affects approximately 50% of patients. It presents as erythematous raised plaques with silvery white scales on the scalp.

Psoriasis vulgaris is the most common type of psoriasis and involves the scalp, extensor surfaces, genitals, umbilicus, trunk and ears. Plaque psoriasis is characterized by raised, inflamed lesions covered with a silvery white scale. Guttate psoriasis presents as small salmon-pink papules, 1-10 mm in diameter, predominately on the trunk. It frequently appears suddenly, 2-3 weeks after an upper respiratory infection (URI) with group A beta-hemolytic streptococci. This form of psoriasis might cause heart complications if not promptly identified and treated. Pustular psoriasis presents as pustules appearing on the palms and soles or diffusely over the body.

Inverse psoriasis occurs on the flexural surfaces, armpit, groin, under the breast, and in the skin folds. It is characterized by smooth, inflamed lesions without scaling due to the moist nature of the area where this type of psoriasis is located. Oral psoriasis may present with whitish lesions on the oral mucosa. Geographic tongue is considered by many to be an oral form of psoriasis. Eruptive psoriasis involves the upper trunk and upper extremities. Most often, it is seen in younger patients.

Even after plaques have cleared, there may be a longstanding or permanent discoloration. Arthritis, if not controlled, may be mutilating and crippling. It is suggested that psoriatic patients have a higher incidence of cancer, especially lymphoma, but how much of this increased risk can be ascribed to the psoriasis and how much to the medications used for psoriasis is less certain. Nail psoriasis may cause pits on the nails, which often become thickened and yellowish in color. Nails may separate from the nail bed. Psoriatic nails may be indistinguishable from fungal nails so your dermatologist can perform certain tests to determine the diagnosis.

Workers’ Compensation Needs Improvement, Says Washington The Workers’ Compensation system nationwide is exhibiting cracks. Cracks that result in diminished benefits to injured workers, state by state, and Washington is paying attention. Last month I had the honor of presenting the findings of a recent U.S. Department of Labor report to my colleagues at the Ocean County Bar Association. Initially I thought the topic would be rather dry and I’d report on statistics and recommendations. I was incorrect. THIS IS WHAT I FOUND OUT. Social Security: A Woman’s Perspective Regardless of whether you’re a man or a woman, Social Security will not – and was never designed to – provide all of the income you’ll need to live comfortably during retirement. At best, your income from Social Security will supplement your other sources. If you are factoring Social Security into your retirement plan, you should learn all you can about how to enhance your benefits, and how much income you may need from other sources, to be financially comfortable during your retirement years. For women, however, there are some unique factors to consider in the equation. With longer life expectancies than men, women tend to live more years in retirement and have a greater chance of exhausting other sources of income. Because Social Security generally has annual cost-of-living adjustments, you have an inflationprotected benefit for as long as you live. For women, those increases are vital since women generally live longer than men. In addition, Social Security provides dependent benefits to spouses, divorced spouses, elderly widows and widows with young children. While Social Security is neutral with respect to gender (individuals with identical earnings histories are treated with the same in terms of benefits), the following 2015 numbers1 released by the Social Security Administration Office of Research and Statistics highlight how demographic characteristics of women compare with the entire population. • Women reaching age 65 need to prepare for approximately 21.6 more years of living expenses. Men live an average of 19.3 more years. • Women make up 56% of all Social Security beneficiaries age 62 and older and approximately 66% of beneficiaries age 85 and older. • The average annual Social Security income received by women 65 years and older was $12,857 compared to $16,590 for men. • For unmarried women age 65 and older (including widows), Social Security comprised 49% of their total income. In contrast, only 35% of unmarried elderly men’s income and 30% of elderly couples’ income came from Social Security.

Of all elderly unmarried women receiving Social Security benefits, 49% relied on Social Security for 90% or more of their income. • Only 22% of unmarried women aged 65 or older were receiving their own private pensions, compared with 27.7% of unmarried men. • Of the women who were employed fulltime, 56.4% participated in an employersponsored public and private sector plan compared to 53% of men. Ocean Otolaryngology Combats Chronic Sinusitis With New Device Submitted by Dr. Bruce Peters Suffers of chronic sinusitis are turning to Ocean Otolaryngology for new methods for long-term relief. The Ocean County-based practice now offers the Propel® implant, a new medical device that is clinically proven to improve outcomes for sinus surgery. “The Propel® implant is a game changer for our patients who suffer from chronic sinusitis in combination with nasal polyps,” said Dr. Bruce W. Peters, a board-certified otolaryngologist, head and neck surgeon at Ocean Otolaryngology. “The device reduces the regrowth of polyps post-surgery and the need for oral steroids.” Chronic sinusitis affects one in seven adults, according to a 2010 National Health Interview Survey. The condition, which is characterized by chronic congestion, postnasal drainage, pressure, coughing and headaches, is commonly underdiagnosed as allergies. According to the Asthma and Allergy Foundation of America, 49% of sinusitis sufferers have never seen an ear, nose and throat specialist about their condition. Those diagnosed with the condition initially turn to antibiotics, inhaled nasal topical steroids, antihistamines and decongestants. Reports by the Millennium Research Group indicate that 500,000 sufferers who don’t respond to medical treatment undergo endoscopic surgery to remedy their condition. Donna W., a patient at Ocean Otolaryngology, opted for the Propel® implant after suffering from chronic sinusitis. “I had so many sinus infections, and since receiving the implant in July, they’ve cleared up,” said Donna. “I had a very good experience at Ocean Otolaryngology and would encourage anyone considering the Propel® implant to receive it there.”

Why should customer service within a healthcare organization be any different? For years patients accepted subpar customer service for amazing clinical results, but not anymore. Doctors’ offices, hospitals, and other healthcare facilities are now beginning to realize that they are now not only responsible for providing excellent clinical care, but ensuring patients are having a positive experience as well. A handful of select elite Healthcare Organizations have been rolling out a brand new concept in top level administration. They are now appointing CXO’s, Chief Experience officers, who report straight to the CEO. This position is to oversee overall patient experience. This position is important not just for patients and their families, but for staff as well. It creates a culture and transparency within the building. It creates an environment where nurses know that in addition to providing the best possible nursing care, they are also expected to provide empathy, physical, and emotional support to both patients and families. Visible support from the administration is key to successful experience campaigns. With Social Media influence we see that word of mouth is the best form of marketing, it always has been, so you would think that making sure patients are happy would be a given. Many healthcare organizations are so focused on providing excellence in quality healthcare, that they do not realize that positive health related outcomes may not always translate into an overall positive experience. Patients do not want to be labeled and treated as their diagnosis, they are humans, who have different wants and needs. These patients are most likely going through a very difficult time and need emotional support and something as simple as a smile when they are feeling down will make all the difference. In fact, people are more likely to choose a doctor, hospital, or healthcare organization based on personal experience. Patients want to know that they will be greeted by a friendly staff and that doctors and staff will be available to them when needed. These items may seem small, but they made the patients feel special. They felt that we cared for them as people not as medical charts. Patients knew that Robert, the brother of the CEO, was available by cell phone and he didn’t just pretend to care, he truly did care. This customer service experience became known throughout the facility. Staff knew that Robert would be rounding and if there were issues, he would know. Before you knew it everyone was reporting needs and wants of patients simply because they wanted to beat the big boss to it! It is a beautiful concept that happened naturally and started from the most genuine place… someone caring! Today, all AristaCare buildings are staffed with a Director of Quality Experience who report directly to the administrator. This position created transparency within AristaCare facilities and is constantly helping to improve systems, imperfections, and at the end of the day we are providing better healthcare because of it. Hepatitis C and the Baby Boomer

More than 75% of patients infected with Hepatitis C were born between 1945 and 1965 and are part of the Baby Boomer generation. This has prompted the Centers for Disease Control and Prevention to recommend routine testing for Hepatitis C virus for all Baby Boomers. Hepatitis C is one of the leading causes of cirrhosis, primary liver cancer, and the need for liver transplantation. Unfortunately it often progresses to severe organ damage before any symptoms are present, making it a silent killer. What is Hepatitis C and how do I get it?

Hepatitis C is a viral infection that is transmitted by blood. The virus reproduces in the cells of the liver, leading to chronic liver inflammation. Over the course of decades and without many symptoms, this process leads to a scarred and damaged liver. Risk factors for transmission include drug use,

tattoos, and blood transfusions prior to widespread blood screening practices of the 1990s. Patients with HIV are at an increased risk, as well as those in the healthcare profession, especially after needle sticks from an infected patient. Still others have become infected without an identifiable source.

How do I know if I have the infection?

A simple blood test called a Hepatitis C antibody test can detect if you have been exposed to the virus. If this is positive, then further tests are needed to see if active virus is circulating in your blood. Once your infection is confirmed by blood testing, imaging of the liver with an ultrasound and additional blood testing to further understand the extent of the infection will be required.

How is Hepatitis C treated?

In the past the treatment of Hepatitis C often required up to a year of therapy with a drug called interferon combined with oral medications. Unfortunately this treatment was not very successful and the drugs themselves caused a lot of side effects. Currently there have been major advances in the successful treatment of Hepatitis C. The treatment for some types of Hepatitis C can now be as simple as one pill a day for eight weeks, with a cure rate over 90%. The treatment of Hepatitis C is rapidly evolving, with many new drugs and regimens being developed.

Why a Power of Attorney?

ife is full of surprises. We have learned to expect the unexpected. But just because life is unpredictable does not mean we can’t take steps to prepare for our future and what lies ahead. One important way to prepare for the future and protect ourselves against the unexpected is to have a Power of Attorney in place. A Power of Attorney, or POA, is a document that enables you to appoint someone else to act on your behalf for financial or other legal matters should you become unable to do so. The individual given power is the known as the Agent or Attorney-in-Fact and you are known as the Principal. The POA can be effective immediately or upon mental incapacity. You can limit the authority of your Agent to control some of your legal or financial matters or it can have a broader POA in which your Agent has the authority to control all your legal matters and financial assets. In either case, whether the POA is limited or more broad, creating such a document assigns at least some authority to another individual. Therefore, you need to take care to select a capable and trustworthy individual to serve as your agent.

What are some common bankruptcy myths?

ankruptcy is not something anyone wishes to think about. This is especially so for people who are accustomed to paying their bills regularly and on-time by any means necessary. Unfortunately, this is not always an option in today’s economic climate. As folks figure out ways to survive, many times they think of bankruptcy as an absolute last resort – an end, figuratively and literally – as opposed to the fresh start it is intended to be. This is likely due to a lack of knowledge of bankruptcy laws and only a familiarity with the many myths associated with filing bankruptcy that stop people from being proactive in protecting themselves and moving forward. Let’s start debunking some of those myths in this article:

Myth #1: I will lose my house!

Bankruptcy laws are crafted in order to provide a person every opportunity to try and save their home. In some instances, bankruptcy can also be a tool for stopping a foreclosure and allowing a person to catch up on their mortgage arrears. Strategically, the question becomes more whether a home is worth saving in light of its value in today’s market. It is important to consult with a bankruptcy attorney in order to evaluate these options and protect that which is most important.

Myth #2: I will never get credit again!

Many of my clients are surprised by the amount of credit opportunities they will be inundated with after filing for bankruptcy. This is likely due to the fact that credit card companies know that a person can only file for bankruptcy once every eight years (not that anyone wishes to have to file again), and therefore wish to begin the cycle of using cards again. I always recommend to

clients to start slow by applying for a secured card. Using credit helps to improve your credit; however, it is best to proceed with caution.

Myth #3: I will never be able to purchase a home or vehicle again! This is simply not true. Will it take time to rebuild your credit to the point of acquiring a new mortgage or refinancing? Absolutely. Usually it takes a minimum of two years of rebuilding one’s credit before a mortgage lender will consider you for same. Car lenders are far more lenient, but the important thing to note is that the sooner one files a bankruptcy, the sooner the slate is cleaned. I think it is extremely important for people to move forward with their lives in as expeditious a manner as possible rather than continuing to spin their wheels and getting nowhere.

Myth #4: I will have trouble finding a new apartment!

Although it is true that many apartment buildings and landlords pull credit reports in evaluating potential tenants, this simple caveat also rings true: at the end of the day, cash rules. The money that my clients are able to save in not having to pay minimum fees to credit card companies and the like can be socked away toward putting together a security deposit, first month’s rent, last month’s rent, and maybe something in between. In today’s rental market, landlords are hard pressed to turn away folks who have the ability to put that kind of money down on a new apartment or rental home, especially if they are gainfully employed. I hope this article proved helpful in dispelling some of the myths regularly associated with bankruptcy. Obviously, everybody’s situation is different and fact-sensitive, so it is important that one receives professional advice regarding his or her specific situation. Regardless, the worst thing one can do is to not do anything!

Post Traumatic Stress Disorder:

T

The Mind-Body Stress Disorder of Trauma

he symptoms are traumatic and life-shaking; they are intrusive and frightening. Sleep is elusive because it leads to dreams that are more like nightmares. Flashbacks are irrepressible. A feeling of helplessness, hopelessness, loss of interest, guilt, shame, self-medication and fear of being among people or going places are all part of the myriad of emotional, physical and cognitive symptoms of Post Traumatic Stress Disorder (PTSD). PTSD is pernicious, insidious, and prevalent. Seventy percent (70%) of adults

have experienced some type of traumatic event at least once in their lives; about 224 million people. About 20% of these people will develop some degree of PTSD, which is appreciably 45 million people. One out of nine women will develop PTSD, which means women are twice as likely as men to develop PTSD.

PTSD was considered “battle fatigue” or “shell shocked,” particularly for combat veterans. However, anyone at any

age can experience trauma and subsequent PTSD. Sexual and physical assaults, any life-threatening or bodily harmful event, bullying, including cyber bullying, as well as any perceived failure can induce this persistent reaction. When the trauma is severe enough to the psyche, the lack of adequate defense to prevent that extent of damage is now being produced as a “defense deluxe.” This is created to compensate and as an attempt to “protect” from further and future traumas. Hence, “Guard Duty status”—or as our Nobel Laureate, Bob Dylan, mused—“All Along the Watchtower.” This may come in the form of repetitive nightmares, daytime flashbacks of the event(s), increased startle reactions, many physical symptoms from autonomic dysregulation: i.e., “fight/flight” states on low, repetitive simmer, instead of how its designed to be utilized—in acute, brief bursts. There are principally three ways to be traumatized. First, you are the victim of a traumatic event; second, you are the perpetrator of a traumatic event; and third, and most common, you are the witness to someone else’s traumatic event that vicariously traumatizes you—by visual, audio or both sensory incomings.

Those who suffer from PTSD may wonder why they have to relive what’s over, especially because it was terrible enough when it happened in the first place. This act

of reliving the pain over and over is a critical aspect of PTSD. The unconscious mind wants the person to be safe and avoid any subsequent trauma, to that end, it fuels the vigilance by serving up frightening reminders to keep the mind sensitive to cues and providing expensive, false security at best. While there is much to learn about PTSD, there is also a greater understanding about its magnitude of disruption. The fact that PTSD has been moved from the section on anxiety disorders in the DSM-IV to the “trauma and stressor” disorder group in the DSM-V is evidentiary of the significance of this understanding. The autonomic physical symptoms of PTSD, such as increased heart rate, difficulty breathing, any and all GI issues, cold sweats, muscle and head aches, dizziness, vision changes, etc., can readily be coupled with psychiatric sequelae. These include such things as phobias (disproportional fears to anything or anyone, situations, etc.), major depressive disorder, suicide, substance abuse, panic (extreme fear without an identified, overt source), hypervigilance sometimes leading to paranoia of delusional intensity—hence, the “D” or Disorder comes alive and lingers on to torment, with an underlying desire to save.

An example of what this means is that the genetic environment of an individual may play a key role in how one responds to trauma as a cause-and-effect interaction, and not just the trauma by itself that causes an individual to have certain responses. In addition, it has been cited that older studies lack key elements as determinants to PTSD response. These include such factors as age when the trauma was experienced, the extent and the period of time associated with the trauma, and the correlation between the traumas experienced early in one’s life to its effect on adulthood.

TREATMENT AND PREVENTION

There are many studies and findings relative to Post Traumatic Stress Disorder that are well beyond the scope of this article. Scientific insight continues to evolve and change what we know and what we do as a result of these new discoveries and awareness. But what we do know is that the human condition is not without its possibilities and probabilities of exposure to trauma. There is no proven way to prevent its happening, especially for those who have high-risk jobs such as protecting the public, or live in high crime areas, or bear witness to or are in terrible accidents. Fortification against trauma may include such things as stress management and special training for those in high-risk jobs, community safe groups, or spiritual resources. Psychotherapy is “universally recommended” for PTSD. It is recommended to be “preferred over medication” as the first intervention. The American Psychiatric Association (APA), Institute of Medicine (IOM), International Society for Traumatic Stress Studies, among other recognized agencies in the health and medical research communities, are in agreement that specific “trauma-focused psychotherapy” is the best evidenced-based approach to PTSD. The primary reason for choosing pharmacotherapy is the presence of comorbid disorders such as depression, anxiety and psychosis. The most effective medications for PTSD are selective serotonin reuptake inhibitors (SSRIs) such as paroxetine and sertraline. Prazosin, which is used for sleep-related disorders, is the only medication with a grade of “recommended” for treating posttraumatic stress nightmares in the American Academy of Sleep Medicine’s guidelines (Psychiatric Annals, Vol. 46, number 9, September 2016). However, according to some recent studies in Psychiatric Annals, there is evidence that the drug doxazosin has greater advantages than prazosin because it has a longer half-life and only requires daily dosing as opposed to multiple dosing of prazosin. Doxazosin is better absorbed and will not wear off during sleep, leading to nightmares. Because of the advantages of Doxazosin, it may be better tolerated and more efficacious for patients with PTSD.

There are neurobiological reasons and alterations in PTSD as well. It has been demonstrated in earlier studies that certain genes, through “epigenetic mechanisms” (Psychiatric Annals, Vol. 46, number 9, September 2016) associated with the hypothalamic-pituitary-adrenal axis areas of the brain, seem to play some part in regulating the risk for PTSD after exposure to trauma. Newer studies are in place to help determine the differences in PTSD responses caused by the effect of the trauma on the nervous system as compared to how the neurobiology of the

New Breakthrough Test For Alzheimer’s Disease As their parents are getting older, my friends approach me with their concerns. Some of these parents are becoming more forgetful. Some are changing their personalities, becoming angrier, stubborn or confused. My friends ask whether this is a normal part of aging, or if this is dementia. They are terrified that their parents are suffering the beginnings of Alzheimer s disease. Until recently, there have been few tests able to detect Alzheimer’s disease. New cutting edge technology has given us a test that shows incredible promise. Q: What is Alzheimer’s disease? A: Alzheimer’s is a type of dementia that causes dysfunction in thinking, behavior, and memory. It is estimated that 5.4 million American have Alzheimer’s disease. Of that, 5.2 million are age 65 or older. Symptoms usually start mild with slight confusion and difficulty remembering newly learned information. However, symptoms progress over time and can become debilitating, including such symptoms as deepening confusion, personality changes, disorientation and profound memory loss. Q: What causes Alzheimer’s disease? A: We have over 100 billion nerve cells in our brain. These nerve cells connect to each other using nerve fibers and they pass information along. In Alzheimer’s disease, proteins (called beta amyloid) can form and interfere with the way the brain nerve cells function and communicate with one another. The nerve fibers can twist and tangle, further disrupting normal brain function. It is not uncommon for an elderly person without Alzheimer’s to have a few of these beta amyloid proteins and tangles. Those with Alzheimer’s disease have many of these lesions. Q: If I suspect someone I love has Alzheimer’s disease, how do we diagnose it? A: There is no one test that can diagnose Alzheimer’s disease. Usually a physician will conduct a physical exam as well as neurological and mental status testing. Usually an MRI of the brain will be performed to exclude a tumor, bleeding, or stroke as a cause of a patient’s mental decline. There is a new test that is gaining lots of attention. A new radioactive compound called Amyvid (also known as Florbetapir F-18) has been created that binds directly with beta amyloid plaques (the protein that causes Alzheimer’s disease). Using a PET/CT machine, one can detect whether beta amyloid is present in the brain. This has been called “amyloid imaging”.

Q: Are there any clinic trials for someone to be enrolled? A: : The Centers for Medicare and Medicaid Services (CMS) is currently conducting a huge trial to determine whether “amyloid imaging” using Amyvid can improve clinical Study is positive for Normal Brain detection and patient Alzheimer’s disease outcomes. This trial is called the IDEAS study. Local neurologists and dementia specialists who participate in this monumental program can refer their patients to a facility that can perform this test. Does the test cost anything? A: A patient is examined by a neurologist or dementia specialist who is a part of this study. If there is uncertainty as to whether the mental decline is caused by Alzheimer’s disease or a different type of dementia, the Amyvid test can be performed. Since this test is covered by all Medicare plans, a patient must be a Medicare beneficiary who is 65 years or older. However, the patient is responsible for the co-payment. For those under 65 years old, at this time private insurance does not cover this study.

Fecal Incontinence: Dealing with an Embarrassing Condition What is fecal incontinence? Fecal incontinence, or bowel incontinence, is the inability to control bowel movements, causing the involuntary passage of stool. Symptoms of this often embarrassing disorder range from occasionally leaking small amounts of stool and passing gas, to completely losing control of bowel movements. Classified as a pelvic floor disorder (PFD), fecal incontinence occurs when muscles in the pelvic area cannot support the organs in the pelvis. These organs include the bladder, uterus (women), prostate (men), and rectum. What causes fecal incontinence and PFD? The brain controls the muscles of the pelvic floor through the nerves. Medical conditions or injuries that impact the health of nerves (such as diabetes, Parkinson’s disease, stroke, back surgery, spinal stenosis, or childbirth) can result in weakness or injury of the pelvic floor muscle, resulting in fecal incontinence and PFD. Those with PFD are unable to control the muscles in the pelvic floor to have a bowel movement. People with PFD contract these muscles rather than relax them, and because of this, they either can’t have a bowel movement or they have an incomplete one. PFD also includes urinary incontinence and pelvic organ prolapse. In fact, many people suffering with fecal incontinence also experience urinary incontinence. Urinary incontinence is a loss of bladder control due to bladder muscles that are too weak or too strong. Symptoms range from mild leaking to uncontrollable wetting, and become more common with age. Pelvic organ prolapse is a condition in which the uterus, bladder and bowel may “drop” onto the vagina and cause a bulge through the vaginal canal. Also common is a disorder in which the rectum becomes “stretched out” and slips out of the anus. This is called rectal prolapse. It is estimated that nearly one-third of all U.S. women are affected by one of these types of pelvic floor disorders in their lifetime.

or perform any number of medical tests available, including blood work, X-rays and ultrasounds, to uncover the cause of fecal incontinence. Who is at risk for developing fecal incontinence? Although men and women are at risk of developing fecal incontinence and PFD, pelvic floor disorders occur more frequently in women than men. Nearly one in 10 women older than the age of 40 has fecal incontinence, possibly as a complication of childbirth. Other risk factors include being over the age of 55, overweight, postmenopausal, having a past pelvic surgery, nerve damage or a connective tissue disorder. How is fecal incontinence treated? Fecal incontinence can be treated in a number of non-invasive ways. Treatment usually combines self-care, medicines, physical therapy and home exercise. Treatment depends on the cause of the problem you have and what best fits your lifestyle. Dietary changes often reduce the occurrence of fecal incontinence. As stool consistency is affected by what you eat and drink, your doctor may recommend diet modification and adjusting nutritional supplements taken. If nerve or muscle damage is the cause of the fecal incontinence, the physician may recommend exercises and similar therapies to improve function and strength. These exercises include bowel training and biofeedback. Biofeedback is a technique taught by specially trained physiotherapists that can increase anal muscle strength, allowing patients to learn to relax or contract pelvic floor muscles. Medication sometimes helps patients with fecal incontinence. When appropriate, these may be prescribed by your physician.

How is fecal incontinence diagnosed? Physicians will begin the exam by asking about symptoms, taking a careful history and performing a visual inspection of the anus and perineum. A physical examination will determine the patient’s ability to control the pelvic floor muscles. The physician will check for nerve damage, muscle spasms or muscle weakness. Additionally, the physician may request

If I lost my teeth due to gum disease or decay, does that mean that I can’t have implants? Almost all implants placed in patients who have lost their teeth to periodontal disease or decay have been successful.

If I am only missing one tooth, isn’t it easier to just get a dental bridge? While the dental bridge option may seem like an easier solution to replacing a missing tooth, it usually is not the best option… The primary cause of failure of a bridge is dental caries, followed by endodontic therapy. To make a dental bridge, one or more of the neighboring teeth surrounding the missing tooth site are shaved down with a high-speed drill. This prepares the neighboring teeth for the fitting of a fabricated three-unit bridge that is cemented in place. However, bone loss soon occurs in the area of the missing tooth or teeth, and plaque build-up is common below the margins of the bridge. Since a significant amount of the tooth structure of the neighboring teeth has been removed, these teeth are susceptible to decay and therefore root canal infections. Because the bridge design doesn’t allow you to floss properly between the teeth, the neighboring teeth are also prone to develop periodontal disease. A dental implant can be used to replace just the missing tooth, without requiring any preparation of the neighboring teeth, and can even last longer than a natural tooth. A dental implant is not susceptible to tooth decay or root canal infections. The dental implant option closely mimics the design and function of your natural tooth and is the most secure option with a solid foundation of bone that keeps it stable, withstanding the forces of biting and chewing. An implant feels and functions like a natural tooth and it is proven to outlast all other tooth replacement options.

I am missing a few teeth; why don’t I just get a partial denture made? A partial denture sits over the gums with a framework that hooks onto some of the remaining teeth to provide minor stability. The partial denture lacks a secure connection to the jaw, and without tooth roots to stimulate the bone underneath, the jaw bone in that area will begin to shrink. The changing shape of the jaw bone results in a partial denture that is loose, and that moves and shifts during function. Over time this movement and shifting will usually loosen and weaken the remaining teeth and irritate the surrounding gums, causing pain and discomfort, and sometimes additional tooth loss. A better solution is to replace the missing teeth with dental implants. The dental implants can usually have individual crowns, which allows you the ability to brush and floss. The implants provide stability and stimulate the surrounding bone to prevent significant bone loss. The improved distribution of biting forces prevents unnecessary overloading of the remaining natural teeth, and generally will have a higher long-term success rate when compared to partial denture treatment. The most important feature of this option is that you have a stable treatment that most closely resembles the design of your own natural teeth.

My dentures are hard to wear; will implants help me? Implants can replace removable bridges or dentures, or they can be used to stabilize and secure the denture, making it much more comfortable. There are exciting new techniques that make it possible for a patient to transition from a full denture to a permanent restoration in a short period of time, commonly referred to as “Teeth in a Day” or “All on Four” and “Diem 2.” There is not enough time to fully explain this process, so you will have to look for my future article to learn more.

I was told I don’t have enough bone for dental implants. Can you help me? Yes. Bone loss is not uncommon for people who have lost teeth, or who have suffered from periodontal disease or traumatic injuries. Oral and maxillofacial surgeons are trained and experienced in a multitude of techniques which may be used to reconstruct defects of the jaw bone and ridges in order to place implants. These techniques include bone grafting such as guided bone regeneration, sinus lift grafting, titanium mesh grafting, distraction osteogenesis, and so on. Often the patient will need a sophisticated diagnostic and treatment planning work-up which may require 3D cone beam CT scanning and virtual CAD-CAM technology. Once the foundation of bone is reconstructed, then dental treatment may proceed.

Endometriosis And Infertility

Endometriosis is a condition where tissue normally located in the uterus is abnormally found elsewhere in the abdomen. It affects women primarily from the late teenage years to the time of menopause. Classically, endometriosis is responsible for much of the chronic pelvic pain symptoms in women of reproductive age, and a common cause of infertility. Although the exact cause is unknown, evidence supports a genetic predisposition to developing endometriosis.

What are the symptoms?

Women with endometriosis often experience painful periods and pelvic pain varying from a mild annoyance to severe debilitating pain. It is estimated that endometriosis may be present in up to a third of reproductive age women with pelvic pain, and up to forty percent of women presenting with infertility.

How is endometriosis diagnosed?

A detailed medical history, physical exam, and pelvic ultrasound are common components in the workup of a patient with suspected endometriosis. Medical history may be consistent with painful menstrual periods, including pain with intercourse and pain when having a bowel movement or voiding. Ovarian cysts can be found on ultrasound that may or may not be symptomatic. A definitive diagnosis is based on surgical exploration of the abdomen, with visual evidence of endometriosis.

What causes endometriosis?

Although the exact cause of endometriosis is unknown, this benign disease is thought to arise from a combination of menstrual fluid that backflows from the uterus into the

fallopian tubes and enters the abdomen. Another likely factor is cellular changes that create endometriosis implants in the abdomen, or that an altered immune system response allows these endometriosis cells to successfully survive and grow outside of the uterine cavity.

How does endometriosis affect my chances of fertility?

Women with endometriosis have a strong association with infertility. It is estimated that between 20 and 40% of infertile women have endometriosis, and that the extent of infertility is directly associated with the severity of disease.

What are my treatment options?

Treatment of endometriosis depends on the clinical symptoms, which include pelvic pain and/or infertility. Medical treatment includes the use of anti-inflammatory medications or oral contraceptives in those women with mild symptoms not desiring pregnancy. Medications that decrease the production of estrogen in the body are also being utilized to treat this disease. Although medical therapies address the symptoms, they do not provide a measurable benefit regarding fertility. Therefore, on a case-by-case basis, surgical treatment may be beneficial in restoring ovarian and fallopian tube anatomy, and to remove endometriosis implants. Fertility treatments may often be required to achieve pregnancy. Endometriosis can affect many facets of a woman’s health. It is important that patients be educated about the implications of this condition, and that proper evaluation and care is implemented by their provider. Now the Practice of Yoga Begins…Are You Ready? Written by Laurie Greene, PhD, ERYT, Owner Atha Yoganushasanam (yoga sutras 1.1) “Now the practice of yoga begins”

The first sentence of the bible of classical yoga is this. Atha, which often opens important Sanskrit texts, means “now” and has the connotation of auspicious beginnings…like saying “behold!”: it acts at once as an imperative— “wake up,” we are beginning—and as a “blessing” for those who are ready to begin. It insinuates that we are finally ready to get real, and “to discover the essence of our existence in the core of our own heart and at the center of our being” (Freeman). It signals our ability to be in the present moment (“now!”). Anushasanam refers to a systematic, scientific teaching; a teaching which is only offered to a qualified student—one who is ready to fully commit. Yoga is a very varied practice. There are many different styles, diverse practices, and distinct lineages, but there is also an underlying web of similarity that joins these different approaches. An Indian folktale tells of a man who is digging a well. He begins by digging down five feet and when there is no water, he is discouraged by his hard work, and he climbs out of the hole, moves a few yards away and begins digging again, only to be met by disappointment again and again. This goes on and on, and he never finds water, but labors endlessly. Most traditions of yoga inspire us to engage in deep self-exploration. They ask us to stick with it, work hard, and dig deeper and deeper. What we tend to do in our lives is leave a practice (any practice) just as it becomes challenging. We jump from regimen to regimen, diet to diet, relationship to relationship, career to career. In yoga we are asked to stick to it, so that through the body and breath we can investigate deep and subtle feelings, responses, and reflexes in relation to the conditioned mind. We do this by slowing everything down and observing it closely with fresh eyes. We try to look past the rapid evaluations of our conditioned minds. Atha is therefore about being present, here, in the moment.

The word mudra stems from the Sanskrit root mud, which means “bliss.” This hints at the power of these beautiful gestures to evoke deep feeling in the observer and joy in the practitioner. The word mudra also denotes “seal” in yoga and is employed to explain the process of sealing and strengthening the body’s vital energies. In classical Indian dance, mudras are used to express the vast array of human emotion and experience; to communicate deep feelings and tell intricate stories. In the yoga tradition mudras are used for spiritual concentration, healing, removing obstacles, and other subtle energetic practices. The diverse styles of yoga accomplish this focus and concentration in different ways. Some study texts and philosophy (jnana), some engage in chanting and other forms of devotion to a higher cause (bhakti), some engage in good works (karma), and still others look to the movement of energies in the body to create deep experience (hatha). The body in hatha yoga is seen as an arena for metaphysical experience. It is manipulated through body postures, bandhas (locks), and mudras (body & hand gestures), which enhance the channeling of energy and therefore experience. These practices are ancient and are found in many Eastern artistic traditions. Indian classical theater and dance make use of postures and mudras to feel and express emotion and create transformation in the performers and the audience. Mudras in particular help to focus the gross body energy through delicate and precise movements of the hands. We are familiar with “anjali,” the mudra of prayer, which focuses our bodies on our vital energy and the essential energy of others, but there are many mudras with many intentions.

The Nurse Midwife: Offering a Comprehensive Approach to Care When people hear the term “midwife,” they often envision someone who helps women give birth at home. In reality though, the vast majority – 95 percent – of births attended by midwives occur in the hospital. Plus, nurse midwives do much more than deliver babies: they care for women throughout the lifespan, from a teenager’s first gynecologic exam through pregnancy and menopause. Karen Carola, CNM, director of midwifery at Ocean Gynecological and Obstetrical Associates, part of Barnabas Health Medical Group, has more than 30 years of experience as a certified nurse midwife. Here, she explains the unique benefits of this growing specialty.

Q: What is a midwife? A: A midwife is a trained medical professional who addresses women’s physical, emotional, social and spiritual needs related to childbirth and their reproductive health. A certified nurse midwife (CNM) is educated in both nursing and midwifery, certified by the American Midwifery Certification Board, and

licensed to practice in New Jersey. Most CNMs have bachelor’s and master’s degrees in nursing.

Q: Does a woman need both an ob/gyn and a midwife? A: Obstetrician/gynecologists and midwives each perform important functions, and ideally a woman should have both. A CNM must have a physician available, but the doctor need not be present if everything is normal. Our practice has five ob/gyns (physicians) and five midwives, and we all work as a team. Patients can see a midwife, a physician, or a mixture – whatever they prefer. The physicians’ primary focus is on the physical wellbeing of mother and baby. They are always available to monitor high-risk pregnancies, handle any complication or medical emergency that may arise, or perform a cesarean-section if that becomes necessary. The midwives take a holistic approach, addressing a wide range of patient concerns – from fears about giving birth to guidance in breastfeeding. We help empower women and support them to have the

Q: Can women still receive an epidural (pain medication during childbirth) with a midwife? A: Yes. We discuss all the options for pain relief, so each woman can make the decisions that are right for her as an individual.

Q: What else should women know about midwifery? A: Again, I’d like to emphasize that we’re here for women at every stage of life – not just when they’re having babies. Q: Are midwives becoming more popular? If so, why? A: Yes, the percentage of midwifeattended births has risen nearly every year since 1989. I attribute a lot of it to Facebook. Younger women talk to their peers and post their experiences on social media, so word is getting out more and more about midwives.

Q: Are women who use midwives less likely to have a C-section? A: Yes. In one study, for instance, the C-section rate for a midwifeattended birth was 17 percent, versus 32 percent for a physicianattended birth at the same hospital. There are several reasons. First, we encourage women to keep moving during labor, which helps the baby rotate and descend. We also allow labor to progress by just waiting and not trying to rush things along. In addition, we try to uncover any issues that might be slowing things down, and address them. For instance, a woman’s fears can actually keep labor from progressing.

Make Breast Health Part of Your New Year’s Resolution New Year. New Habits. New You. While dropping weight, quitting smoking and getting organized are popular New Year’s resolutions, add this one to your list: breast health. Fellowship trained Sumy H. Chang, MD, FACS, a board-certified breast cancer surgeon, reminds women to make breast health awareness a year-round commitment. A member of Barnabas Health Medical Group, in partnership with Community Medical Center, Dr. Chang provides Ocean County women with access to the latest in prevention, detection and treatment for breast-related issues – including breast cancer. She is also the Director of the High Risk Breast Program at the Jacqueline M. Q: When should mammograms

start and why?

Dr. Chang: I recommend that women at “average” risk - those who have no family history of breast cancer - begin regular mammogram screenings at age 40. Mammograms are still the best screening tools available to detect breast cancer. And detecting breast cancer at an early stage may spare more women from undergoing chemotherapy. We still diagnose a fair amount of breast cancer in the 40-45 year old age group. In this age range, the breast cancer is usually a more aggressive form. While there’s no way to predict who will get breast cancer, detecting it early will lead to less toxic treatment.

Q: Is a self-breast exam worth the effort? Dr. Chang: Many women are confused about this topic. And it’s no wonder as there is varying information. The bottom line is women should conduct regular self-exams. Women know their bodies better than anyone and what’s “normal” and what’s not normal for them. My advice is to persist if you know something is wrong and be your own biggest advocate. Most of the time, the patient’s history and physical exam make the diagnosis for the doctor. In breast cancer patients, it can alert the doctors to perform additional tests and lead to a diagnosis of cancer. Many women get a breast exam at their annual gynecological exam. But, during that one-year span a lot can happen. So, this year, make a self-breast exam a new “habit.”

Q: What’s new with aesthetics? Dr. Chang: If you are diagnosed with breast cancer, you should understand that treatment has evolved significantly over the years. The first thing on most patients’ minds is survival, and often the last thing is aesthetics. However, as a breast cancer surgeon, I know patients will care about the aesthetics of their breasts once their treatment is done. So, I provide a long-term perspective as well and make sure they will be happy in the future with their body image. I’m proud to say that most of my patients are very happy with their aesthetic

outcome. Based on the type of their breast cancer surgery, women have reconstruction options as well as clothing options, such as customized mastectomy bras and camisoles, and breast forms.

Q: How would you describe your role with patients? Dr. Chang: Many times, I’m the first person to inform a woman that she has breast cancer. I guide them through the next 6 months – 1 year. I understand that going through this journey can be very traumatic and I gently ease my patients into the transition of treatment. Most of my patients become very close to me and my staff; we become their family. As a surgeon, I feel it is a privilege that the patients place their trust in us to operate on them and I see myself as their advocate in the operating room while they are sedated. I serve in the same role during breast cancer treatment, which involves a multidisciplinary approach with radiologists, pathologists, plastic surgeons, radiation oncologists, medical oncologists and genetic counselors. I also help to identify my patients’ psychological, social and economic needs – whether they have family, health insurance or financial concerns that may interrupt or delay their treatment. One great resource that I rely on is the Cancer Support Community – a free program that provides programs and services, which help people affected by cancer transition back to everyday life. My goal is to minimize the psychological and physical impact of breast cancer treatment on my patients in the long term.

When Does Forgetting Become An Issue? Is it memory loss or am I just being forgetful? Did I just lose my keys or will I never remember where I put them? Is it OK that I can’t remember how to do basic chores because I’m “getting older”? The first answer brings good news because you are asking these important questions, not someone on your behalf, and, even more important:

When your memory has significantly declined, is poor compared to performance typical for your age but DOES NOT interfere with independent, day-to-day functions, you can be described as having Mild Cognitive Impairment or MCI. Dementia is an advanced deterioration of MCI and means that you no longer are able to independently perform day-today functions. Alzheimer’s Disease or AD is the most common form of dementia, and we know that a proteinbased plaque exists in the brains of these patients at abnormally high levels. We also know that this plaque begins to accumulate and clump together within the brain as early as a decade or more before you may experience any memory loss, impairment or dementia. On a positive note, we are living longer and could prepare to enjoy this new stage of living proactively and productively. Exercise, proper diet and remaining busy,

active and engaged are proven factors that minimize developing symptoms of dementia. But the research, proper diagnosis and treatment options leave much to be desired and are far from where we need these areas developed at this era of science, technology and awareness.

Cardiac Breakthrough Slipping Away At The Jersey Shore… The Jersey Shore has suffered some great losses in the recent past due to Super Storm Sandy and the Nor’easter…and now, another great loss seems inevitable. Cough And Cold Season Is Here - Are You Ready?

Believe it or not, cough and cold season is upon us. Our nights are becoming very cold and children have been back in school for about four months now. This is the season everyone dreads. It seems we see people with the sniffles everywhere— at work, at the mall and even at home. Now is the time to take some steps that will reduce your risk of catching a cold and stock up on things that will help with the symptoms when the inevitable happens.

YOU CAN REDUCE YOUR RISK OF CATCHING A COLD OR FLU. HERE ARE SOME HELPFUL HINTS: WASH YOUR HANDS FREQUENTLY-This will help stop the spread of germs. Everything you touch potentially has cold germs on it—door knobs, handrails, pens, pin pads, etc. Also, keep hand sanitizer easily accessible—keep it in your car and your desk at work. REDUCE STRESS-Whenever a big day is coming up, or a deadline you have been rushing to meet, you are more likely to become sick. Relax, your health is more important. Also, try to get at least eight hours of sleep per night. EAT WELL-Certain foods combat colds. Try to include as much of the following as possible in your diet: broccoli, cauliflower, garlic, onions, citrus fruits, and whole grains. Some evidence also exists supporting using vitamin C and Echinacea to boost your immune system. DRINK PLENTY OF WATER-Drinking plenty of water helps flush germs out of your body. Water also helps keep your mucous membranes moist, allowing them to trap the virus and dispose of it.

BREATHE FRESH AIR-Try to breathe as much fresh air as possible. Recycled, contaminated air can make you sick! Get outside as much as possible. Cold air will not give you a cold! For the most part, everyone catches some degree of a cold at least once a year. Once you feel the symptoms coming on, then what? For starters continue the above tips. Also, cover your mouth and nose when you sneeze or cough (no one else wants your germs). And most importantly-CONTINUE TO WASH YOUR HANDS FREQUENTLY. There are many cough and cold medications on the market that can help with your cold or cough symptoms. Decongestants in a pill form or a nasal spray can help with a stuffy nose. Antihistamines can help with itchy, watery eyes. Cough suppressants help control a nagging cough. Cough drops or throat lozenges help with a sore throat. Tylenol or Motrin can help any body aches or fever. Before taking any medications, please consult with your pharmacist or doctor to be sure the medication you are buying doesn’t aggravate any existing medical conditions or interact with any current medication you are taking. THIS IS VERY IMPORTANT. Many over the counter cough and cold medications can cause adverse effects. Don’t be afraid to ask us, that is what we are here for. Once you have determined which medications are safe, keep some at home so that you are prepared when that cold comes along. January is National Blood Donor Month January is National Blood Donor Month and one of the most difficult times of the year to maintain a sufficient blood supply. Severe winter weather and seasonal illnesses often contribute to a seasonal decline in blood and platelet donations.

“Since her diagnosis, Chloe has required hospitalization, surgery, chemotherapy, and many, many blood transfusions,” said Camille Dewing, a family friend. “Chloe and her family have not had an easy journey, but blood donations given by generous donors have helped to save her life and encouraged her to keep fighting.” Transfusions of red cells and platelets can help save a patient’s life, or dramatically improve their quality of life from day to day. Leukemia and other cancers can cause anemia and internal bleeding, chemo and radiation can lower blood counts, and cancer-related surgeries can result in blood loss. “Patients like Chloe don’t get a holiday from needing blood, and the need for donors is constant,” said Dewing. “Chances are there’s a child or a family waiting for the gift you can give.”

Acupuncture

Understanding The Value Of Acupuncture What is acupuncture? Acupuncture is a treatment involving the insertion of fine needles into the skin to relieve symptoms (such as pain), strengthen the body’s immune system and balance the body to facilitate healthy body function. How does it work? Acupuncture works by stimulating the body’s qi (pronounced chee). Qi is the body’s life energy. It flows throughout the body in a series of channels. Each channel is connected with a specific organ and has specific physiological functions, as well as emotional and spiritual components. When the qi flow is disrupted, the body can experience dysfunction, ill health and/or pain. When the qi is flowing freely through the channels, all body functions, such as Sleep, Digestion, Energy, Immunity, Concentration, and Mobility, are enhanced. Does it hurt? The insertion of the needle is generally painless. Many patients do not even realize when the needle has been inserted. This is because acupuncture needles are solid and hair-fine, unlike a medical hypodermic needle, which is thick and hollow. Once all of the needles are in place, the patient usually becomes very relaxed. The lights are dimmed and gentle music is played. Many patients fall asleep during the treatment. How many treatments does it take to feel relief? Some patients feel some relief after the first treatment. Typically, patients feel some consistent relief after three to five treatments. It is usually recommended to receive two to three treatments for two to three weeks, followed by an evaluation to determine the need and/or frequency of further treatment. Complete relief varies from case to case. In general, acute symptoms resolve quicker than chronic or complex conditions. How long is a treatment? During a treatment needles are retained for twenty to forty minutes, depending on the patient and the condition being addressed. Is it safe? Yes. All needles are pre-sterilized and packaged for single use. During treatment, the acupuncturist opens the needle packages and inserts the needles, using proper clean needle technique. When the treatment is over, the needles are removed and placed in a biohazard container, just like at a hospital or doctor’s office. Does insurance cover acupuncture? Most insurance companies have plans that cover acupuncture, though coverage varies from plan to plan. The best way to find out if you have coverage is to come in for a consultation, and let the acupuncturist verify your benefits with your insurance company.

Who can be treated with acupuncture? Everyone from infants to seniors can be treated with acupuncture. Treatments vary accordingly. Infants and toddlers may be treated with acupressure techniques or acupuncture with no needle retention. Children tend to respond very quickly to very little stimulus. With adults and seniors, sensitivity and response to treatment varies, therefore different treatment techniques can be used to achieve the best results for each individual.

CASA Celebrates A Decade Of Providing Child Advocacy For Children In The Foster Care System Written By: Vicki Weiss, Executive Director What is CASA of Ocean County? CASA of Ocean County is a non-profit organization that exists to recruit, train and supervise every day-concerned citizen to serve as a child’s voice in court. A CASA Volunteer is a compassionate caring adult that remains in the child’s life until the child has a safe and permanent home. 10 Year Anniversary for CASA of Ocean County Help CASA celebrate its vision to provide a CASA Volunteer to every child in foster care that needs one by volunteering as a Court Appointed Special Advocate. There are over 750 victims of abuse and neglect living in out of home placement in Ocean County. By no fault of their own, innocent children from our community have experienced physical/ and or sexual abuse, neglect, witnessed domestic violence, live in extreme poverty and have parents that are struggling with addiction or mental health issues. Children, from ages ranging from birth to 21, are appointed to the CASA office so we may provide them with a CASA Volunteer of their own. The CASA Volunteer ensures the child’s emotional and physical needs are being met while they live in foster care. What do you need to do to become a CASA Volunteer? Besides patience and a giving heart for children, the CASA Volunteer needs time to donate to be

the one person that the children can count and rely on. CASA provides a 30-hour training class for the volunteers to teach them how to become child’s rights advocates in a very complicated court system. A CASA will undergo a background check, visit the child once a month, prepare court reports and attend hearings, and participate in on going training activities. As a Judicial Volunteers they assist the court by visiting foster homes, day care providers, schools, medical providers, and will inform the Judge in a court report. The volunteer will make recommendations to the court so the Children in Court Judge can make the best decisions for the best interest of the children. What does advocacy include? Every child’s situation is different but here are examples of how a CASA can change a life of a child: • Serve as a voice in the courtroom to keep children from being moved from foster home to foster home. • Request the child’s right to have parent, sibling, and family visitations. • Visit schools and ensure the foster child is receiving a quality education. • Assist with clothing and basic need issues. • Recommend annual health visits and therapy. • Monitor a parent’s court order for rehabilitation and help the child from lingering in foster care.

Advocate for the adoption, reunification and Kinship Guardianship so a child will have the safe and forever home that they deserve. How can you receive more information? The CASA office hosts monthly Informational Sessions and in person interviews to provide volunteers with answers to all their questions.
