Dear readers, I want to take this opportunity to let you know of my intention to exit the role as Editor of this great and prestigious magazine.

I joined the magazine as Editor following the sad loss of the great Larry Browne. It was a great honour to take over the reigns for the last few years.

I had a vision to bring in new features with a diverse group of writers, not only covering some

great technical articles; we got to peek into the lives of our peers, get nutrition and wellness advice. There have been some fantastic business articles written, we've even had the odd book review.

Our arm of the dental profession is at a really great point with a rosey future ahead, in my opinion. We have a great opportunity to provide excellent quality products and properly priced services. I do hope you

all flourish and can ride out the current cost of living crisis the UK is facing. I am convinced we can.

I wish you all well, I am staying in the industry, and I do hope I will get chance to meet up at some of the trade shows in the near future.

He says: “The lecture is intended to provide a hints and tips guide to running a modern business as a whole, not just a dental lab. I will include everything from managing the team to pricing services, working with key clients and even how to part ways with a client should that become necessary. I will take an open and honest approach to how we run a successful lab of 50 people.

There will also be further opportunities to discuss business subjects and seek advice from professionals who can help with everything from health and safety compliance to accounting, HR and more.

There are many reasons why running a business has been difficult in the past few years. For dental laboratories across the UK, the challenges keep on coming and it has become essential to think outside the box in order to overcome the issues and really thrive. This is hard when confined to the lab, so it’s important that dental technicians and lab owners connect with others to find inspiration and learn from their experiences.

Ashley Byrne of Byrnes Dental near Oxford has much experience in running a dental business and he outlines the key challenges most labs are facing right now:

“Running a modern dental lab in these challenging times is really hard work. We have a serious workforce shortage, materials fees are rising, energy costs are painful and we have a rapidly growing demand for services.”

Ashley will be speaking about these challenges and his experience so far in managing them within his own business at the Dental Technology Showcase (DTS) 2023. In a DTS Lecture Theatre session

“Dealing with business growth alongside a dwindling workforce is a big challenge for all of us. We are also faced with rapid digitisation of the industry and growth of the large corporate and DSO market. In the last couple of years, we have seen changes like never before and we need to be agile and dynamic to remain competitive. Demand for the dental technician is at a record high, so whilst we face challenges, we also have access to enormous opportunity if we steer our labs in the right direction.

“The core challenge I aim to help colleagues overcome is motivating and retaining team members. I really hope to help lab owners understand that their most valuable asset is their team – so they are certainly worth the investment. The session will also demonstrate the power of growth through digital processes and technologies. The goal is to work smarter, not harder.”

The DTS Lecture Theatre will feature an array of sessions covering a broad range of topics from fabrication of dentures to ceramics and implant prosthetics. There will also be content focused on the many digital technologies available to dental labs

As an in-person event with more than 1500 visitors anticipated, DTS 2023 will be an excellent platform for networking. Whether you’re looking to connect with friends, find new suppliers or explore advanced training options, this will be an opportunity to engage with the wider profession. It’s also a chance to relax and have some fun with colleagues who understand the challenges you’ve been up against and who may have innovative ideas of their own to share.

Ashley has attended DTS several times over the years as a delegate and speaker. He would encourage colleagues to visit this May to make the most of everything that is on offer: “DTS is the one chance we get as lab owners and dental technicians to catch up, talk to each other, share issues and problems, and hopefully have a beer. It’s full of great lectures, and suppliers sharing knowledge and products. If, as an industry, we are to really shine in these challenging years, we need to get to events like DTS to help each other. We are stronger together when we connect like this. I look forward to seeing everyone at the show, enjoying the lectures, supporting the suppliers who make this event happen and don’t forget, mine’s a pint of a Guinness!”

BUILDING THE ALL-IMPORTANT NEXT GENERATION l

David Smith – IQA, Assessor and Lecturer at Yeovil College – will be speaking at the Dental Technology Showcase (DTS) 2023 to explore the workforce crisis and how we could bring fresh blood into the sector. He says: “I recently visited year 10 students discussing career opportunities. I wasn’t entirely surprised that they hadn’t heard of dental technology, but was concerned that the advisors hadn’t either. This needs to change.

“We started a well-funded Level 3 Apprenticeship at Yeovil College to give people a chance a rewarding and fulfilling career in dental technology. We will begin taking school leavers soon.

PUTTING MENTAL WELLBEING FIRST

lStephen Taylor will be among the many highly-anticipated speakers at this year’s Dental Technology Showcase (DTS).

He shares some of the issues he feels dental technicians are facing right now and how he hopes his session in the DTS Lecture Theatre will be of help: “Even without being aware of the problem, a business culture could make people feel uncomfortable in discussing their mental wellbeing. Implant drilling guides The

Implant dentistry is big business these days, both for the dental practises that offer it and the labs that support it. Since I started placing implants 18 years ago, a lot has changed on both the surgical and restorative sides, but especially in the planning elements.

We used to take impressions and ask a lab to make a diagnostic wax-up and then make some kind of stent out of it. If we were careful, the model would be duplicated and then a model would be sectioned through the area of interest.

Now that the price of CBCT scanners has come down and digital dentistry is becoming more popular, many surgeons are asking their labs for drilling guides that are much more complex than the one described above. Why is this?

Well, in the UK at least, litigation against dentists continues to increase.

laboratory, surgeons can know whether they can deliver a certain type of implant restoration based on an accurate CT analysis before they lift the scalpel. Based on this, a treatment plan can then be confirmed or modified so that the patient has a good idea of the complexity and outcome of the case. For technicians, this means they get cases that are less stressful to restore and look great.

Using CT scans to plan implants is not new and there are several packages on the market. One of the newest on the market and now in its second generation version is exoplan from exocad.

With this product, dental surgeons and dental technicians can accurately correlate intra-oral data, either from a physical or digital impression, and combine it with CBCT

data. Once combined, this package allows for easy and accurate planning of implant placements and the subsequent design and production of surgical guides for the accurate placement of those implants. In addition, the plan can be used in combination with one of exocad's programmes CAD to facilitate immediate loading.

Exoplan is a great addition for a laboratory that already uses exocad for design, as many of the tools used are the same. The surgical guide design is amazingly simple and the fabrication is done with a 3D printer, which many labs have already invested in.

In August 2013, the first Military School opened its door to young and determined dental technicians and trainees. Based on the educational concept of Zirkonzahn’s founder, MDT Enrico Steger, this school became a success, earning the 1000th dog tag in February 2023.

“Military School”: for some, the choice of the name will cause confusion. What does dental technology have to do with the military? Enrico Steger, MDT and founder of Zirkonzahn, explains the reasons behind this choice by saying that the strict training of the army contains certain elements that can prepare young people for working life. During training, dental technical contents will replace the military ones, whereas characteristics such as teamwork, simplicity, humility, discipline, loyalty and diligence are emphasised. These are the necessary virtues for the professional life of a dental technician.

At the Military School, participants from all over the world come to the Aurina Valley, in South Tyrol (Italy), to expand their education in a spartan, but at the same time modern and comfortably furnished accommodation. The courses focus on

What is a... CAREERS AMBASSADOR

Hi my name is Andrea and I’m a careers ambassador. What’s involved in being a careers ambassador I hear you ask. Well I’m pleased you asked. Let me explain...

I started out in dental technology around 19 years ago, totally by accident. I had never heard of a dental technician, like many others, I either thought the dentist made most stuff, had some standard size appliances in drawers or quite frankly I just never even thought about it, having never had any dental work done myself other than a couple of fillings.

I have worked in a variety of industries growing up - yes, I know, I don’t look old enough! But it’s true. I started working at the age of 13 with the usual paper rounds, cleaning jobs, then into retail at little local shops and then later in bigger stores such as pets at home. I’ve also had jobs in various factories in various roles and even worked a milk round for a few days – wasn’t for me. My CV is quite long as you can imagine especially as I had so many different roles from the floor up to manager level in so many of those industries. But, it just didn’t do it for me. After I had been made redundant for the 3rd time I decided that rather than going back into the same old jobs I would use it as an opportunity for a change. But a change to what?

I scoured the local papers for something different, but it needed to be full time work with training as I had a house and a young child to look after.

This is when I stumbled upon an advert for a trainee dental technician at a local lab that was within walking distance of my house that I didn’t even

know existed. I went for an interview and look around and I was quite frankly gobsmacked at all the cool stuff they had and what they were making. It was great! They found my interest and enthusiasm quite entertaining I think, and they offered me a job that came with a day release course. Perfect! I have since gone on to develop a reasonable career in dental technology.

So, thinking about the above, how many people out there don’t know we exist? How many school children, school leavers, young adults or older adults don’t even know that doing what we do is an option? How would they know if we don’t tell them?

So, this is what a careers ambassador is essentially, someone who shouts about what we do.

I know many of you who know me will know that I am generally very quiet and meek. OK, OK, well I used to be! But when it comes to letting people know what we do I am very vocal. I attend various careers fairs for school age children, I have helped NHS careers get their information up to date on their various media. I have been filmed talking about my career journey, have been interviewed for a podcast about it too and its all so we can try and raise awareness of what we do, letting people know the education routes into our industry, letting

When a smile is revitalized using ceramic restorations, aesthetic guidelines are followed and implemented meticulously so that optimal results can be achieved. Smile Design – that's what everybody is talking about when it comes to fixed ceramics. Attention to detail is also an established practice when fabricating restorations. The anatomy of the hard tooth substance is reproduced three-dimensionally during ceramic layering. Dentin and enamel materials form a natural unit. So why should these standards not apply for full dentures too? After all, denture wearers want an aesthetic outcome just like any other patient. These days, younger denture wearers in particular want their dentures to look natural rather than artificial. As a dental technician, if you have the opportunity of completely reworking the red-white aesthetics, then you should take advantage of the opportunity.

INTEGRATED AESTHETICS

After all, the aesthetic guidelines that hold true for all disciplines in dental technology should, of course, also apply for full dentures: the tooth axes, contact surfaces, angle characteristics, cervical dimension, gingival margin and length-width ratio should be designed like natural dentition. The basic requirement in this respect is a denture tooth that meets aesthetic guidelines and provides an anatomical structure similar to that of natural dentition. And that's the challenge: denture teeth generally – for whatever reason – do not meet aesthetic guidelines and have a lifeless and monotonous appearance when it comes to layering. The morphology and texture of real teeth was consistently implemented when creating the mould. The following case report demonstrates the resulting benefits, not just in terms of aesthetics but also for setup.

CASE STUDY:

A 58-year-old female patient presented in the dental practice, as she was dissatisfied with both the function and the aesthetics of her current restoration. When chewing, the partial denture (lower jaw) and the full denture (upper jaw) would slip back and forth and did not provide a secure grip. The denture teeth appeared lifeless and the mucogingival areas were smooth and level with an artificial appearance. Following extractions in the lower jaw, region 31 to 34 did not have any restoration, as the partial denture had not been extended. Only tooth 46 was still present, which the patient insisted she wanted to keep. The molar proved worth retaining and showed no clinical or radiological signs of severe periodontal damage. Once the impact of a partial denture on the only remaining tooth (46) had been explained to the patient, it was decided that the new lower denture should be designed so that in the event of tooth loss, a denture tooth could be added by making minimal changes to the design of the denture. The decision was therefore taken to work around tooth 46 during fabrication of both dentures. The patient wanted a new restoration with a natural appearance that offered her peace of mind with a secure grip when chewing and speaking.

After detailed consultation, she decided to have a new full denture made. For this purpose, the VITAPAN EXCELL denture tooth was selected in the appropriate tooth shape to ensure a natural appearance and setup in accordance with aesthetic guidelines. In the posterior area, VITAPAN LINGOFORM was used. As an initial basis for the work to be completed, anatomical situation impressions were taken with alginate in order to create custom trays in the laboratory. These were used in a second session to take mucodynamic impressions. The first impression was taken using Heavy Body, and the second, fine impression using Light Body A-silicone. Once the master model had been fabricated, this was used as a basis for fabricating bite registrations with wax rims for determining jaw relation. The spatial arrangement of the jaws in relation to one another could be transferred to the articulator using the intraorally encoded registration data. Following analysis of the model, the appropriate wax setup for the patient was started.

INTUITIVE SETUP

Dental technicians have internalized the aesthetic guidelines. In addition to their fundamental knowledge of aesthetics, they spend every day looking at what works aesthetically and what doesn't. A logical consequence is that they find it easier to set up with anterior teeth that comply with these aesthetic rules than, for example, with a set of teeth whose axis and angle characteristics do not harmonize with each other on the dental plate. Even with these kinds of denture teeth, every user will try to find the ideal setup, although they will find it difficult to find the correct tooth position, due to the given fundamental discrepancies. That's why VITAPAN EXCELL was designed with tooth axes, contact surfaces, angle characteristics, cervical dimensions, gingival margins and length-width ratios that consistently meet the generallyapplicable aesthetic guidelines of natural dentition. As a result, setup using this anterior tooth set was intuitive and easy to complete. The wide ridges additionally support optimal shaping of the papilla. For new restorations, it is important to always consider the previous appearance of the patient's dentition. The basis is the anatomy of the oral cavity on the day treatment started, which is where I begin restoration in accordance with the parameters of aesthetics, function and stability. All clinical information must be taken into consideration here to achieve optimal restoration results in the patient's mouth. Due to the position and the mesial displacement of tooth 46, space for the lower anterior setup was restricted and was implemented with overlap in order to facilitate physiological molar contact between the denture teeth. The patient was in agreement, as her natural lower anterior teeth had also overlapped and had never been evened out by orthodontic treatment. In the posterior area, efficient setup with precisely defined centricity, but with the necessary freedom for excentric movement, was ensured by VITAPAN LINGOFORM. The multifunctional occlusal surface design based on the cog wheel principle, is universally suited for all conventional tooth-to-tooth or tooth-totwo-tooth setup concepts.

FINISHING AND TRY-IN

Since the gradient of the gingival garland is not defined by a stepped transition between the tooth and the cervical area, but the transition is natural, the mucogingival structure could be modeled in a patient-friendly way. The alveolar process, palatal ridges and palatal suture, as well as the incisive papilla, were also modeled in wax in order to simulate the final dentures as accurately as possible during clinical try-in. Following careful analysis, the posterior teeth were adjusted

to a more distal position in order to achieve a more individual and natural appearance. The single tooth (46) was enclosed in a circular fashion by the base to provide the upper setup with greater occlusal stability and to compensate for distal movement of the tooth. The patient responded well phonetically and functionally, and was delighted with the natural appearance of the denture teeth. Thanks to the three-dimensional anatomical structure in multiple layers, and the harmonious shade gradient from the neck to the incisal area, the VITAPAN EXCELL anterior tooth set had an incredibly lifelike appearance. Its natural morphology and nuanced surface texture facilitated a brilliant play of shade and light in the patient's mouth, even during try-in. The distinctive incisal characteristics with a white incisal margin and integrated mamelon structure created an organic shade effect, while the balanced light transmission resulted in natural light dynamics.

COMPLETION AND FINISHING

A silicone key was fabricated from the wax setups; the teeth were removed, cleaned and conditioned in the base and cervical areas for acrylic fabrication. Following repositioning of the denture teeth in the silicone key and prior to pouring, the complete denture base was customized with Castdon polymer, and the vestibular labial shield and palate area individualized using shades from the Denture Art system (both products from Dreve Dentamid, Unna, Germany). Following repositioning of the master model in the key and autopolymerization, both dentures were finished using a rotating instrument. Final polishing was carried out using pumice stone, polishing paste and a polishing wheel. Depending on the age of the patient, the age of the teeth can be controlled by reducing the surface texture to a greater or lesser degree by quickly polishing. In this case, the surface texture of the teeth were consistently avoided by using a handpiece in order to maintain a more youthful appearance for the denture teeth.

HIGHLY AESTHETIC FULL DENTURES

This case shows that by using the right denture teeth in accordance with aesthetic guidelines, and by working with a skilled dental technician, it is possible to achieve full denture restorations that compare favorably with complete ceramic jaw rehabilitations.

The patient was delighted with the highly aesthetic result and the stable seating of her new dentures, and was already more confident immediately after placement. The denture teeth showed a lifelike play of light and gave the patient an age-appropriate appearance again.

OBITUARY: AMEER (ASHTON) SHEERKHAN 18.06.1966 - 10.01.2023

Ashton worked together with many of his work colleagues at Monarch Dental Services Ltd for over 35 years and each and every day, he brought his smiling face to work. Ashton was an outgoing and friendly character and will be remembered for bringing joy and laughter to his work colleagues on a daily basis.

Ashton was a highly skilled Chrome Cobalt

technician who could always be relied upon to construct his work to the highest standard of workmanship. His chromes were like a beautiful work of art.

Later on in Ashton’s career he worked tirelessly with myself, and Dr Peter L’estrange of the Ear Nose and Throat Hospital in London to develop a removable dental appliance for the treatment of slight and moderate sleep apnoea. These appliances brought a dramatic improvement to the health and lifestyle of the patients that wore

them. Many of these appliances are still being worn by the patients who positively benefitted from Ashtons endeavours.

Ashton sadly suffered a debilitating illness for the last 9 months of his life although he continued to work part-time almost up to his death.

All of Ashtons former work colleagues at Monarch Dental Services Ltd would like to express our sincere sympathies to Ashtons family on this sad occasion.
