In 2016, Evergreen Health experienced incredible growth and change. We completed The Lofts project, providing stable housing for 66 people; hired 89 new staff across all service areas; and moved our medical and mental health practices into our new, state-of-the-art Evergreen Center expansion. But most importantly, we significantly increased and improved the services we provide to people in need. Throughout these changes one thing has remained the same– our values. These are very difficult and emotional times for many of the people we take care of. Evergreen will continue to provide a welcoming, nonjudgmental, inclusive and compassionate environment for people who are underserved by the healthcare system. Together, we will carry on the Evergreen spirit as we work to save more lives and improve the lives of the people we serve. I am inspired every day by the work that we do and the positive impact every inspi single employee has on our community. Evergreen Health* was founded in 1983 to address the HIV and AIDS crisis in Western New York. As breakthroughs in medical therapy were made for HIV and other sexually transmitted infections, the organization broadened its range of life-enhancing services beyond HIV care to

Community Access Services is a not-for-profit organization committed to addressing the epidemic of HIV and other diseases that challenge the health and welfare of urban communities in Buffalo and Erie County by assuring the availability of comprehensive care services including prevention, education and treatment.
The mission of the Pride Center of Western New York is to work with the community to make Western New York a safe, healthy and satisfying place for lesbian, gay, bisexual, transgender and bis queer people to live, work and establish their families.

In 2012, 2 Evergreen restructured to accommodate the ever-broadening programs and services that the agency provided. Chronic Illness Support Living with multiple chronic illnesses can be challenging. Evergreen offers support from Care Coordination to counseling to housing programs to help our clients manage and improve their health.
HIV Care Today, HIV is a manageable chronic illness, often treated with simple daily prescription pill regimens. Evergreen offers comprehensive HIV services: medical care and treatment, pharmacy services, insurance enrollment, individual and group counseling, care coordination and more. Our expert staff helps our clients learn to manage their HIV and the live the life that they want.

HIV Prevention Evergreen is committed to providing education and prevention services to our community. We offer testing and treatment, and are the leading WNY provider for PrEP – a daily pill for HIV prevention.
LGBQ and Transgender Services Evergreen is a safe, affirming place. We understand LGBTQ concerns and provide sensiti sensitive, appropriate care. Additionally, we specialize in transgender services, offering everything from medical care to counseling to educational/social groups. We are dedicated to helping our trans clients feel safe, valued and welcome.
Sexual Health Evergreen’s friendly staff provides comprehensive testing and treatment for gonorrhea, chlamydia and syphilis, as well as chla HIV and Hepatitis C. We offer counseling, an anal health program with on-site anoscopy services, and multiple education and outreach initiatives that provide free condoms throughout the region.
Substance User Health Evergreen provides a safe space for people who use substances to get the services and support they need. We never force anyone to do or be anything; programs are available when our clients are ready. We offer a syringe exchange program, counseling, support groups, medication assisted treatment and a drop-in center for clients to speak with healthcare professionals.
Hepatitis C While Hepatitis C (HCV) can be cured, treatment can be hard to find or stick with. Evergreen offers testing, treatment, pharmacy services, counseling and more. We can help our clients get into and succeed in treatment to cure their HCV.
