ADULT DAY CARE CENTERS Adult day care is a planned program of activities designed to promote well-being though social and health related services. Adult day care centers operate during daytime hours, Monday through Friday, providing a safe, supportive, cheerful environment. Nutritious meals that accommodate special diets are typically included, along with an afternoon snack. Adult day care centers can be public or private, nonprofit or for-profit. The intent of an adult day center is primarily twofold: • To provide older adults an opportunity to get out of the house and receive both mental and social stimulation • To give caregivers a much-needed break to attend to personal needs, or simply rest and relax Adult day care centers are licensed, monitored group facilities that provide various services including medication management, various types of therapies, and meals.

HEALTH CARE DECISIONS The typical health care patient has changed in recent years. No longer are individuals putting all of their health care decisions into the hands of nurses and doctors. Patients are more informed than ever before and are interested in taking a more active role in their own care. Thanks to the Internet, people are able to access information that, in the past, was not easily available. A few decades ago, medical data may have been elusive and filled with confusing jargon the layperson might not have understood without a crash course in biology. However, today there are different websites that clearly spell out information about certain illnesses and diseases. This means that patients may no longer be walking blindly into appointments with doctors. Facing an illness is never easy, and decision-making abilities can be hampered by emotions and the desire to get well as quickly as possible. Patients who do not have all of the facts may have to rely solely on the expertise of caregivers and physicians when making important health care decisions. By knowing the avenues of information, a patient can find assistance with careful decisionmaking. While learning about an ailment is important, there are some things to consider. • The Internet isn’t foolproof. Many online medical sites are very reliable and offer a wealth of pertinent information. They can be good starting points when seeking out information on a particular condition. It is in your best interest to visit sites that are well-known and monitored by respected medical affiliations. Other sites may be peppered with exaggerated claims or misinformation. Therefore, do not go by online information alone. • Don’t self-diagnose. It can be easy to use the Internet as a means to narrowing down symptoms and to make assumptions about what ailments you may have. Instead of using the Internet to self-

Kinship Care is when grandparents or other close relatives provide full-time child care in their homes for another relative’s child or children. The primary reason for providing this support is because the parents of the children are absent or unable to properly care for them. Kinship Care is the fastest growing type of placement method for children in the country. Families and social service agencies realize that children greatly benefit when families are kept together. Living with a responsible adult relative sends a positive message to children that they still belong and are loved. It also helps them believe it is not their fault their parents are not raising them. As a result, they grow up feeling stable and secure. The need for Kinship Care can come about in a number of ways. •A grandparent observes that grandchildren are always dirty, hungry and maybe even bruised. They may even suspect child abuse. They call Child Protective Services for assistance to ini-

tiate the removal of abused children from the home, or the offer to take care of them in their own homes. •A relative dies, leaving children orphaned. As the closest relative, you consider caring for these children in your home. •A mother or father is contacted by an attorney, courts or social services regarding the incarceration of their child who has children. The grandparents or close relatives do not want the children going into foster care. •Drug and alcohol addiction prevents a relative from responsibility caring for their own child or children, and they abandon them, leaving them with you for weeks. If you are a grandparent 55 or older and you have the primary responsibility for the care of minor children, the Prince George’s County Administration on Aging wants you to know you don’t have to handle this alone. Maryland Access Point (MAP)

MAP is a new initiative that links older persons and adults with disabilities from all income levels to local health and support services. It is the single point-of-entry to access resources for lifelong independence. Whether the question is about understanding Medicare, where to find leisure activities, how to care for a frail older relative or how to locate senior housing, assisted living or a nursing home, MAP is the place to start.

Health Promotion-Living Well

This program offers senior citizens a series of workshops to help them with diagnosed or self-diagnosed chronic conditions while learning skills to manage the symptoms and get the most out of life. Topics covered in the workshops include: medicines, nutrition and physical activity; skills for communicating with your health care team; handling frustration and problem-solving; planning for the future; and relaxation methods.

Family Caregiver Program

MFP will help people transition from an institution to community living in an apartment, private home or small group setting. hereever you look, local seniors are finding both independent and communal living arrangements that will help them live active lives. One recurring social adjustment pattern is also a very old one: the multigenerational family household. Overall, extended families have been unusually adept at getting business done, caring for all of their members and finding practical ways to age in place. Case in point: the three-generation

Blue family of Takoma Park. Matriarch Rosalie Blue, 68, has lived in a small, circa-1920s, twolevel bungalow on a shady street since the late 1970s. There she has raised five children. While Blue has used a walker for about seven years, her home remains the essential gathering spot for local family members—the place where two daughters and five grandchildren have meals and spend quality time. “We’re a tight-knit group,” said daughter Diane Brown. “We feel fortunate that we’ve been able to live near each other all these years and can

really give support where it’s needed. Of course, time brings changes, so you have to be prepared.” Recently, Blue has been transitioning from the walker to, occasionally, using a wheelchair, a change that can restrict mobility in a house that is tight on space. “The core challenge to providing

for Mrs. Blue’s mobility needs was redesigning the first floor into a universal design solution,” said Rich Estes, a team leader at Landis Construction, the Washington, D.C.based design build firm that remodeled Blue’s home for wheelchair accessibility. “We had to begin with a 12-by-13-foot kitchen with very few contemporary appliances and two existing bathrooms that were space restricted and quite dated,” he said. “Moreover, we wanted doors and hallways to be compliant with ADA (Americans with Disabilities Act) requirements.” The existing home’s entire first level was configured in a traditional Victorian pattern—with the living room and dining room to the left side of a front-facing foyer and a kitchen, pantry and small bath in


the right rear of the house. Each room was an independent, wall-bound space with narrow doors and small windows, and the house had no air-conditioning and depreciated insulation. More pointedly, there was no means for wheelchair access to a convenient parking place. “What’s always fascinating about space planning,” said Landis Construction principal Chris Landis, “is how dramatically you can improve a working floor plan by adding a fairly small amount of new square footage and rethinking the whole.” By enclosing a seldom-used rear porch

and reconfiguring some floor space, for instance, Estes and his team created a wheelchair-accessible breakfast nook, a mudroom with an ADA-compliant rear door and access to a full bath designed to accommodate Blue’s particular needs. In addition, the newly introduced open plan invites useful interaction between the kitchen and dining room. A food preparation island facilitates cooking and works as a handy serving station for both the custom-designed breakfast

nook and a three-stool dining counter between the kitchen and dining room. “The plan allows the whole family to gather comfortably and for Mom to move about freely,” said Brown. “It’s great for a large group like ours that gets together a lot.” Generously sized, thermally efficient windows add natural light to the home. And, the interior design scheme—which features white and saffron walls, granite surfaces and cherry cabinetry—is warm and uplifting. The two new bathrooms are fully ADA compliant, with all doorways and halls widened for wheelchair access. Finally, a 15-by-20-foot deck was built-out from the rear kitchen door. Here, a pathway through the landscaped backyard leads to an alley for easy access to a waiting car. Best of all, the new kitchen design allows Blue to pursue her love of cooking for her family, confident she has the elbow room to spare. Landis Construction offers workshops on

HEALTH AND CARE ISSUES By the Prince George’s County Area Agency on Aging

B

eing a senior or living with a disability is not what it used to be and these unique populations are demanding change to keep up with the times. Here at Prince George’s County Area Agency on Aging (AAA) we strive to meet the challenges of every demand head on through our many programs and services to ensure the highest quality of living for seniors, the disabled and their families. The AAA likes to say “Assistance starts at the Maryland Access Point (MAP) Unit.” This group grew out of a new initiative of the Federal Administration on Aging to further develop Aging and Disability Resource Centers (ADRC) around the country. In Maryland the ADRCs are now called Maryland Access Points. Each serves as a single point of entry to meet the needs of older adults, their families and those with disabilities. Prince George’s County’s MAP office was one of the first six sites in the state. The MAP unit consists of the Senior Information and Assistance Program, the Senior Health Insurance Program (SHIP), the Senior Medicare Patrol Program (SMP) and the Family Caregivers Support Program. In addition, to facilitate seamless access to services, staff from the local Health Department, Department of Social Services, Mental Health and Disability Administration, and the local Center for Independent Living (Independence Now, Inc.) is co-located within the MAP office.

The Senior Information and Assistance Program (I&A) serves as the primary entry point to those who call or visit the Area Agency on Aging’s MAP office seeking up-to-date information on services and resources available for seniors and the disabled. Information and assistance is provided about in-home care services, affordable housing, real estate taxes credits, transportation, senior legal and financial assistance, long-term care options, Medicare savings programs like QMB and SPDAP, and other resources.

Home visits are done as needed to assist those who are unable to manage various business transactions on their own. The Senior Health Insurance and Assistance Program (SHIP) provides personalized counseling and assistance to Medicare beneficiaries and their caregivers negotiating the complexities of Medicare and other insurance programs. On a daily basis, SHIP counselors provide Medicare beneficiaries and business partners accurate, understandable and objective information, counseling, training and assistance with a wide range of health insurance issues involving Medicare, Medicaid and long-term care insurance. HIPs (as they are commonly referred to) also provide referrals for the uninsured and under- insured. The Prince George’s County SHIP has also implemented a volunteer program. Dedicated SHIP volunteers provide the one-on-one, objective counseling, often about low-income savings programs like extra help with prescription drugs and the Qualified Medicare Beneficiary (QMB) program for co-payment and/or premium relief.

The Senior Medicare Patrol (SMP) was created to help prevent health care fraud and abuse and to save millions of dollars for tax payers annually. In Prince George’s County, this program is made up of volunteers who educate Medicare beneficiaries on the nefarious effects of fraud and to teach them how to detect and prevent it whenever possible. SMP works in tandem with SHIP. Seniors and the disabled are given examples of fraud tactics and how to protect themselves from becoming victims. They are also shown how to detect Medicare billing errors for services they have not received. The Family Caregivers Program provides vital information and assistance to family members and other caregivers who look after the needs of older individuals, as well as grandparents or older relatives caring for children. While care giving is often rewarding, it can also be financially, physically and emotionally stressful. In some cases, people caring for an older relative or a friend do not always recognize

they need assistance or time to care for themselves. Services provided by the Family Caregiver Program enable the caregiver to reduce out-of-pocket spending, and to provide a safe and healthy environment, enhancing the quality of life for the care services recipient. In Prince George’s County, Caregiver specialists provide caregivers with current information on opportunities and services available in the community and direct them to the appropriate program. In addition, specialists assist caregivers with the coordination of services and care by assessing situations to identify problems, screening for eligibility for programs and determining other services to help the caregiver and receiver. Caregiving Guide/Kinship Care Guide – The Family Caregiver Program has developed, published and printed the Caregiving Guide and the Kinship Care Guide that contain helpful information to support traditional caregivers as well as kinship caregivers. Over 20,000 guides have been distributed to Prince George’s County residents and their families throughout the United States since 2002. The Caregiving Guide is also available on the Prince George’s County website. Qualified Medicare Beneficiary (QMB) / Specified Low-income Medicare Beneficiary (SLMB) Programs Enrollment – The Administration on Aging Senior Health Insurance Program (SHIP) has provided assistance to more than 1,000 Prince George’s County low income residents who were dropped from the QMB/SLMB Medicaid programs in the last several years. The SHIP program staff and volunteers helped residents to reapply for the benefits. The programs pay for Medicare insurance premiums and/or co-pays and deductibles for qualifying individuals.
