Leading Edge Aviation announces expanded turbine training Bend, Oregon — Leading Edge Aviation (LEA), a Bend, Oregon based helicopter flight school, and Central Oregon Community College (COCC) announce the approval of their new Advanced Helicopter Training Course by the Oregon Department of Education and Department of Veteran Affairs This new course expands the existing degree program to include Turbine Transition, Night Vision Goggle (NVG), External Load, and Mountain Flying in our fleet of Bell 206B-III Jet Rangers. Upon completion of their commercial training students will receive 20 hours of instruction in the Jet Ranger with specific training in one of the areas of operation. Students eligible for the Post 9/11 GI Bill will be able to take this new course as part of the degree program, which is fully  funded through the GI Bill. The course

includes Turbine Transition Training, ground instruction in all of the areas and targeted flight training in one of the specialized operations. Depending on the students career goals they can either be trained in flying with NVG’s, External Load or Mountain Flying. “This new course offer students a comprehensive Turbine Transition with a menu based approach to specialized training. This gives our students the most comprehensive turbine training on the market, uniquely positioning them for employment as a commercial pilot.” commented Travis Warthen, Vice President of Leading Edge Aviation. “This makes us one of the few helicopter flight schools in the country able to offer this high level of diverse training to students plus enhances the career path for graduates who are      hired as instructors at LEA.”

Hillsboro Aviation graduate performs highest-altitude long line rescue Hillsboro Aviation— The highest-altitude long line rescue in history was recently performed in Nepal by a cooperative effort between Switzerland’s Air Zermatt and Fishtail Air, a charter helicopter company based in Nepal. Air Zermatt’s Captain Daniel Aufdenblatten, who piloted the helicopter and performed the rescue, is a Hillsboro Aviation graduate.

November 2003. After graduating from the Helicopter Professional Pilot Program, he worked for Hillsboro Aviation as a flight instructor. In 2006, he became a pilot for Air Zermatt. “The training I received at Hillsboro Aviation provided me with a strong foundation that I have used throughout my career and prepared me for operations such as this rescue,” Aufdenblatten said.

Landing in the terrain was not possible so Captain Aufdenblatten, along with Swiss Mountain Guide Richard Lehner, used the long line to lift the stranded climbers. They were evacuated one-by-one to base camp after enduring 36 hours on Mount Annapurna. Aufdenblatten, a Swiss citizen, began his flight training at Hillsboro Aviation in

Prepare for the aftermath of an accident No one plans to have an accident, but it happens all too often. The best time to learn about dealing with the chaos following an accident is not in the midst of the turmoil, stress and shock of the event itself. Whether you are a single aircraft owner or the manager of a large flight department, the time to prepare for disaster is in a relaxed atmosphere when heads are clear.

Before the Accident, Incident, or Occurrence A. Read your policy. Insurance policies are getting more reader friendly. Some are better than others, but all of them will explain issues such as:

• Notification Requirements: Timeliness, specific duties and responsibilities • Liability Coverage: Who is and is not covered, what liability claims are and are not covered, defending law suits, and expenses. • Physical Damage (Hull) Coverage: What is and is not covered, partial loss and total loss, indemnity and betterment, arbitration of disputes, salvage. • Depending on the policy and additional endorsements you have purchased, there may be other benefits and expense provisions in the policy such as: emergency

landing and search & rescue, that you can download, even if you are substitute aircraft, alternate not their customer. These kits include transportation, and personal explanation of terms and forms that expenses. a list of key people and phone numbers you need to contact. Include your aviation insurance broker and insurance company’s claims reporting number in this list. Keep copies in several places, including home and office. Including a copy in your personal planner, aircraft checklist, logbook, and/or flight kit is a good idea. Programming Flight Service (1-800 WX BRIEF) into your cell phone could become handy. Although the phone call will go to the nearest FSS associated with your area code rather than your current location, they have access to everything you need to deal with an emergency. D. Emergency Response Plans. Professional flight departments spend a great deal of time and money building an Emergency Response Program. It may include everything from accident notification to public relations procedures and responsibilities. How hard would it be for the owner of a single aircraft, whether a business or an individual, to create a simple notebook containing some of this valuable information? You may know what to do and who to call, but you may not be in a position to do it yourself. A good aviation insurance specialty broker should be able to give

you some guidance. E. Don’t forget the Logbooks. A condition of insurance could require your aircraft to meet certain FAA and insurance company certification standards. Why complicate the claims process with incomplete records? F. Safety Equipment. We have first aid kits, fire extinguishers, and other emergency gear in our homes, autos, boats, and RV’s. Whether the FAA requires it or not, why shouldn’t we do the same in our aircraft?

When the Accident Happens If you are involved in an accident, here is a short, common sense checklist you may want to consider: • Arrange First Aid for the Injured • Protect the aircraft or other property from further damage • Notify Authorities and your insurance company • Avoid making statements about the loss (except to government officials) until the insurance company gives permission. • Do not initiate repairs without the insurance company’s authorization Get your broker involved early. They

will help you make the required insurance notification and can be one of the most valuable friends you will have in getting through the process of filing and settling a claim.

Choosing an insurance company because of their financial strength and claims service rather than price could turn out to be the best decision you make. This is where a solid relationship with your broker really pays off. One special note about the flight crew: Past experience has proven that in the aftermath of an accident, even when uninjured and seeming in full control, crew members are in shock. Protect them

and get them medical attention. It is a good idea to keep them away from the press. They are going through a process in which they will analyze what has happened, many times blaming themselves for things that could not have possibly anticipated or prevented. It could take them several hours to days and weeks to sort it all out in their own head. This is no time to be making public statements. In my 35 years as a professional pilot and operations manager, the only people I’ve ever seen that were perfectly calm in the midst of disaster were either well prepared and trained or playing parts in a movie. Whether you are directly involved in an accident or take “the call”, you’ll be glad you thought through the process and wrote it all down.

Is your aircraft producing a profit? The survival of an aircraft operator depends on knowing total costs and revenues necessary to produce a profit. Many operators are not profitable. If an operation is losing money, what must be changed to produce a profit? Financial planning determines the future as an aircraft operator. The entrepreneur that purchases an aircraft and develops his financial plan “on-the-fly” will probably suffer significant consequences. The successful entrepreneur will formulate several financial scenarios for each aircraft using a financial model to produce an after-tax cash flow that can be analyzed using Net Present Value (NPV). This requires an investment of time and effort, but you don’t have to be a rocket scientist. There are many ways to charge for aviation services: $/Statute Mile, $/ Nautical Mile, $/Take Off, $/Landing, $/

Day, $/Elapsed Time, $/Flying Hour, $/ seat, $/Trip, $/excursion, $ for positioning, $/Statute Mile or Nautical Mile for deadhead, $/acre, $/pound, $ for excess ferry distance, $ charge for small patches, $ charge for spot treatment, $/load, plus special charges for fuel surcharge, type terrain, weather, day, night, etc. The type operation determines the conventional method of billing but no matter what billing method is used; revenues must exceed expenses to produce a profit. Since total operating costs are usually expressed as an average $/hour cost, know the conversion factor from your billing method to average $/hour revenue. The fixed costs required to compete in the aircraft industry are significant. Aircraft, insurance, personnel, finance cost, depreciation and support facilities represent a large investment. These entry costs are known but will vary with each

aircraft so it is necessary to financially analyze the fixed costs of each aircraft. The variable operating costs of each aircraft must also be analyzed. Generally, newer equipment has lower maintenance costs than older equipment and this should be recognized in the variable analyses of each aircraft. Combining the fixed and variable costs determines the total hourly operating cost. The biggest factor influencing total hourly operating cost is the annual hourly utilization. Spreading the annual fixed costs over more flight hours reduces the total hourly operating cost. Theoretically, each aircraft has a different total hourly operating cost and therefore a different price the operator must charge in order to produce the same percentage profit per operating hour. Practically speaking, each aircraft model has an hourly charge rate determined by the marketplace.

Assuming you collect the hourly charge rate determined by the marketplace your financial plan should answer two questions. 1) What annual hourly utilization is required to break even on each aircraft? 2) What annual hourly utilization is required to produce the return on investment you desire for each aircraft? After answering these two questions, you have a financial plan showing the annual utilization required to break even and to produce the return on investment you desire at the market hourly rate. Is your aircraft producing a profit? What must you change to be more profitable? Calculating the profitability of the cash flow produced from an aircraft investment is time consuming. ntsb reports NTSB Identification: WPR10FA277 Date: June 02, 2010 Location: Spokane, WA Aircraft: ROBINSON R22 Beta, Injuries: 1 Fatal. On June 2, 2010, about 1630 Pacific daylight time, a Robinson R22 Beta broke apart in flight on approach for landing at Felts Field, Spokane, Washington. The student pilot was killed; the helicopter sustained substantial damage to the airframe and main rotor blades by impact forces. The local instructional flight departed about 1600. Visual meteorological conditions prevailed, and no flight plan had been filed. This was the student pilot’s fourth solo flight. He flew in the traffic pattern with an instructor, and made

a full stop landing on runway 21L. The instructor determined that the student was competent to continue flying solo, and exited the helicopter. The student completed about five more touch-and-go landings. The student pilot reported downwind to the air traffic control tower, and the tower controller stated that he cleared the pilot for landing. He observed the helicopter again on base leg, and it appeared to be at a normal altitude and attitude. He did not observe any smoke or anything falling from the helicopter. He looked away for a few seconds; when he looked back up he did not see it again. Witnesses on the ground observed the helicopter flying the base leg at altitudes

similar to what they had observed other helicopters in the past. Several witnesses reported hearing one or two pops. One witness stated that the rotor sounds went silent, and the engine sputtered prior to hearing the popping sound. Some witnesses saw pieces fly off the helicopter, and then it began to spin counterclockwise. One witness stated that it was V-shaped before it went nose low, and fell straight to the ground. The tail boom, tail rotor, and part of the tail rotor drive shaft separated, and were the first pieces in the debris field. These pieces were several hundred feet from the main wreckage. A 2-foot section of one main rotor blade skin separated, and was about 50 feet from the tail rotor pieces in the direction of

the main wreckage along with nearby tail cone skin pieces. The tail boom had a 45-degree angular dent from top to bottom on its left side that was similar in dimension to a main rotor blade. The main rotor blades rotate counterclockwise. The helicopter came to rest on its right side in the back yard of a residence. Objects within 10 feet of the main wreckage on its east, north, and west quadrants were not damaged. The gascolator contained a blue fluid that smelled like aviation gasoline, and investigators observed no debris or contamination in it. Investigators examined the airframe and engine at a recovery yard. They manually rotated the crankshaft. The


crankshaft rotated freely, and the valves moved approximately the same amount of lift in firing order. The gears in the accessory case turned freely. Investigators obtained thumb compression on all cylinders in firing order except cylinder number two, which was contaminated with soft dirt. Its valves moved freely, and investigators could see dirt blow by the exhaust valve. Investigators manually rotated the magnetos, and both magnetos produced spark at all posts. The carburetor sustained crush damage; the metal floats exhibited hydraulic crush damage. The carburetor filter screen, the main fuel screen, and the fuel screen in the auxiliary tank were clear. The oil filter was clean. Examination of the airframe revealed several fractured components in the flight control system. The fracture surfaces were angular and jagged. NTSB Identification: CEN10FA291 Date: June 02, 2010 Location: Midlothian, TX Aircraft: BELL 222 Injuries: 2 Fatal. On June 2, 2010 at approximately 1400 central daylight time, a Bell 222U, multi-engine helicopter was destroyed after impacting terrain near Midlothian, Texas. The airline transport rated pilot and the mechanic, who were the only occupants, sustained fatal injuries. Visual meteorological conditions (VMC) prevailed at the time of the accident and a company flight plan had been filed for the Title 14 Code of Federal Regulations Part 91 return to service flight following maintenance that had been performed. The helicopter had departed the Grand Prairie Municipal Airport (GPM) Grand Prairie, Texas at approximately 1352, and had planned to return to GPM airport. The helicopter was in cruise flight at approximately 600 feet above ground level (AGL) when several witnesses saw the tail boom and the main rotor system separate from the helicopter. The fuselage was destroyed in an immediate post-impact fire. The main rotor mast separated just below the head. The main rotor system including the hub and blade assembly, pitch change links, and walking beam assembly remained connected to the swash plate assembly and impacted terrain approximately 200 feet northeast of the main wreckage. The

entire tail boom assembly separated just aft of the fuselage and impacted terrain approximately 400 feet northeast of the main wreckage. The wreckage was removed to a secure storage facility for continued investigation. I NTSB Identification: WPR10CA262 Date: May 25, 2010 Location: Kona, HI Aircraft: Robinson Helicopter R22 BETA Injuries: 1 Uninjured. The accident occurred during the student pilot’s second solo flight when he attempted to take off. Both the student pilot and his flight instructor, who witnessed the accident, reported that the student pilot applied too much left antitorque pedal as the collective was raised for the takeoff. The helicopter slid to the left off of the landing pad and then spun to the left. The witness reported that the helicopter spun to the left with its skids on the ground three times, then lifted about two feet into the air and spun left three more times. The student pilot said that he attempted to reduce collective, but the right skid contacted the ground and the helicopter rolled over onto its

right side. The helicopter sustained substantial damage to the cabin and tail. The student pilot said there were no preimpact mechanical malfunctions or failures. NTSB Identification: WPR10WA249 Date: May 20, 2010 Location: Traralgon, Australia Aircraft: BELL 206 Injuries: 1 Fatal. On May 20, 2010, about 0415 Universal Coordinated Time, a Bell 206 L-3 operated by a pilot from New Zealand doing crop spraying operations struck a power line and subsequently collided with terrain near Latrobe Valley Aerodrome, Traralgon, Australia. The meteorological conditions were not reported at the time. The helicopter was destroyed and the pilot, the sole occupant, was killed. The departure and destination airports are unknown. NTSB Identification: WPR10WA245 Date: May 17, 2010 Location: Lucena City, Philippines Aircraft: ROBINSON R44 Injuries: 4 Fatal. On May 17, 2010, about 0715 Universal Time Coordinated (UTC), a Robinson

R-44 II helicopter impacted a home in the Jael Subdivision Brgy Iyam, Lucena City, Philippines. The pilot and his three passengers were killed in the accident, along with one individual on the ground. The helicopter was reported as being totally consumed by the post-crash fire. The helicopter, which had a planned destination of Manila Domestic Airport, departed the oval field at the Lucena City National High School, and crashed about one nautical mile upwind from its point of departure. The meteorological conditions have not yet been reported.

airworthiness directives AD#: 2010-12-51 This Emergency Airworthiness Directive (EAD) is prompted by a report of a missing control-rod bushing (bushing) from a 90-degree tail rotor gearbox (TGB) installed on a Model AW119 MKII helicopter. The Agusta Model 119 helicopters also have the affected TGB installed; therefore, they are also included in the applicability of this EAD. This condition, if not detected and corrected, could result in abnormal vibration and damage to the tail rotor system, loss of the yaw control function, and subsequent loss of control of the helicopter. We have reviewed Agusta Alert Bollettino Tecnico No. 119-38, dated March 25, 2010 (ABT), which specifies inspecting the TGB, part number (P/N) 109-0440-06-103, to verify the presence of the bushing. If the bushing is not installed, the ABT specifies replacing the TGB and associated parts with a “new” TGB assembly, P/N 1090440-06-105. Also, the ABT specifies if the bushing is installed, reidentifying the TGB “by installing an additional nameplate” with P/N 109-0440-06-105. European Aviation Safety Agency (EASA), the airworthiness authority for Italy, notified the FAA that an unsafe condition may exist on these helicopter models. EASA advises of a missing bushing in the TGB of a Model AW119 MKII helicopter. EASA also advises that “this condition, if not detected and corrected, could cause abnormal vibration of the tail rotor controls possibly

leading to their damage and consequent loss of the yaw control function.” EASA classified the ABT as mandatory and issued EAD No. 2010-0059-E, dated March 26, 2010, to ensure the continued airworthiness of these helicopters. This EAD differs from the EASA EAD in that we refer to flight hours as hours time-in-service (TIS). We also do not refer to a compliance date of June 30, 2010. We added the requirement of the thickness gauge being no wider than 10 mm. We added the determinate that if the depth between the tail rotor control rod (rod) and the hub-locking nut (nut) is between 4 mm and 6 mm, the bushing is installed. We do not require an additional nameplate but require reidentifying the TGB P/N with an etch pen by changing the last three digits of the P/N from -103 to -105. These helicopter models are manufactured in Italy and are type certificated for operation in the United States under the provisions of 14 CFR 21.29 and the applicable bilateral agreement. Pursuant to the applicable bilateral agreement, EASA has kept the FAA informed of the situation described above. The FAA has examined the findings of EASA, reviewed all available information, and determined that AD action is necessary for products of these type designs that are certificated for operation in the United States. This unsafe condition is likely to exist or develop on other helicopters of these same type designs. Therefore, this EAD requires, before further flight, removing the forward boot from the nut and inserting a 0.3 millimeter (mm) thickness gauge,

If the depth measurement is greater than 6 mm, this EAD requires, before further flight, replacing the TGB and the associated parts with airworthy parts. The actions must be accomplished by following specified portions of the ABT described previously. This rule is issued under 49 U.S.C. Section 44701 pursuant to the authority delegated to me by the Administrator, and is effective immediately upon receipt of this emergency AD. 2010-12-51 AGUSTA S.p.A.: Directorate Identifier 2010-SW-045-AD. To prevent abnormal vibration and damage to the tail rotor system, loss of the yaw control function, and subsequent loss of control of the helicopter, do the following: (a) Before further flight, remove the forward boot, P/N 109-0135-10, from the hub-locking nut (nut), P/N 109-013512, as shown in Figure 1 of Agusta Alert Bollettino Tecnico No. 119-38, dated March 25, 2010 (ABT). (1) Insert a 0.3 millimeter (mm) thickness gauge, not exceeding 10 mm in width, between the tail rotor control rod (rod) and the nut as shown in Figure 2 of the ABT until the gauge stops. (2) From the face of the nut, measure the depth the gauge is inserted between the rod and the nut before it stops: (i) If the depth measurement is between 4 mm and 6 mm, the bushing, P/N 1090135-14-101, is installed. Within 5 hours time-in service, reidentify the TGB, P/N 109-0440-06-103, by using an etch pen

Note 1: Installing a new nameplate by following the Compliance Instructions, Part II, of the ABT satisfies the reidentification requirements of the TGB P/N in paragraph (a)(2)(i) of this AD. (ii) If the depth measurement is greater than 6 mm, before further flight, replace the TGB, P/N 109-0440-06103, with TGB, P/N 109-0440-06-105, and replace the associated parts listed in the Accomplishment Instructions, Part I, paragraph 4, of the ABT with the associated parts listed in the Accomplishment Instructions, Part I, paragraph 5, of the ABT.
