GENA Systems launches Sky Commander new avionics system designed to solve mission-critical operations problems Salzburg, Austria – GENA Systems announces the new product launch of Sky Commander, the “World’s First Mission-Critical Management SystemTM
(MCMS)”. The new avionics product is designed for the deployment of Flight Director-Pilot-Crew helicopter team workflow. Another exclusive Sky Commander feature is the Consortium Management System (CMS) feature which enables multiple aircraft belonging to different organizations (ex: Red Cross, FEMA, CERT) and participating in a single mission to be managed under one control center for maximum efficiency and responsiveness. FAS signs for GrandNew and AW169 helicopters
AgustaWestland, a Finmeccanica company, is pleased to announce that FAS (Falcon Aviation Services) of Abu Dhabi (United Arab Emirates) has signed a contract for a GrandNew light twin and a preliminary sale contract for two AW169 helicopters. Children’s Medical Center’s new state-of-the-art helicopter at forefront of air medical transport safety DALLAS, TX – Children’s Medical Center (www.childrens.com) has debuted a new state-of-the-art medical helicopter that can fly two patients at a time while setting the highest standard for safety, including two pilots and full night-vision technology. The Sikorsky S-76 C++ is a twin-engine helicopter that operates at higher speeds and performance and provides greater range and smoother rides than smaller medical helicopters. It allows for enhanced safety with two pilots at the controls. The aircraft can fly safely in inclement weather, with certification from its pilots. “Children’s has chosen to be one of the few transport programs and the only neonatal and pediatric program within the United States to fly with two pilots in the helicopter,” said the hospital’s transport services director, Scotti Floyd-
Edgar. “This is one of the greatest safety enhancements a program could bring to their community, the patients and families that we serve and our medical crew.” The dual-pilot system is now used by many medical pilots in Canada and Europe. Safety is paramount to the transport program at Children’s. Since 2003, more than 80 people have died in medical helicopter accidents nationwide, according to National Transportation Safety Board statistics. well,” Floyd-Edgar said. The new helicopter will be staffed with medical personnel specially trained in neonatal and pediatric critical care. It is equipped with pediatric and neonatal equipment. The flight team consists of two pilots, a registered respiratory therapist and a registered nurse, and also can include a paramedic and physician when needed. The helicopter is owned and operated by PHI Air Medical, a wholly owned subsidiary of PHI, Inc. The medical personnel who will fly on board are employed by Children’s Medical Center, while the pilots are provided by PHI. Much of the equipment was donated by members of the community. “As the hospital’s air ambulance provider, we share in the important and unique mission of Children’s Medical Center,” said Paul Julander, director of Traditional Programs with PHI Air Medical. “PHI has always been at the forefront of safety practices and performance standards in the industry, and we look forward to this partnership with Children’s in providing the community with the highest and safest level of care.” The helicopter will transfer patients from other hospitals to Children’s Medical Center, but does not fly to emergency medical scenes. The new helicopter will be easily recognized from the ground by its snazzy purple-and-red graphics and by a large “Children’s Medical Center” logo painted on the bottom.
Children’s has had a dedicated helicopter for more than six years and transfers more than 4,800 acutely ill or injured patients a year to Dallas for medical care. Children’s Medical Center Transport Service is the first pediatric transport team in the nation to receive CAMTS (Commission on Accreditation of Medical Transport Systems) certification in all three modes of medical transportation: fixed-wing, rotor-wing and ground. Children’s also is the first transport program in the nation to have a jet dedicated solely to pediatric transport. The hospital also offers the only Level I trauma pediatric hospital in North Texas. The helicopter will be located on-site and at Transport Services at Love Field Airport at Signature Flight Support. New financing relationship between CHC Helicopter and Mileston Aviation Group CHC Helicopter, the world’s largest helicopter-services company, and Milestone Aviation Group, a leading global helicopter leasing company, have announced the commencement of a significant new financing relationship. The companies have agreed to terms on the sale-leaseback of five aircraft: two Sikorsky S92 and three Eurocopter AS332L2. ntsb reports NTSB Identification: *CEN11CA535* Date: August 01, 2011 Location: Joice, IA Aircraft: Texas Helicopter OH-13H/M74A Injuries: 1 Uninjured. The pilot reported that he was paralleling a highway as he was applying chemical to a cornfield. He gained altitude to clear a homestead and then descended into the field to continue spraying once he cleared the homestead property. He reported that the helicopter impacted an unmarked wire that was strung diagonally between the road he was paralleling and the corner of the homestead property. The helicopter impacted the field and the main rotor blades separated from the airframe, causing the helicopter to cartwheel prior
coming to a stop. The pilot reported that there was no mechanical malfunction or failure of the helicopter prior to the wire strike. NTSB Identification: *ERA11IA436* Date: August 01, 2011 Location: Relience, TN Aircraft: BELL 206 Injuries: 4 Uninjured. A Bell 206 L4 helicopter sustained minor damage from a wire strike during takeoff in Reliance, Tennessee. The pilot, two crewmembers, and one patient were not injured. In a written statement, the pilot stated that the purpose of the flight was to pick up a patient that had been involved in a motor vehicle accident. The landing zone (LZ) was a “large field”
where he performed an approach after performing a “high and low recon” of the area, and receiving a briefing of the obstacles by the “LZ command” that was established by the local fire department. The crew was advised of wires along the road that was north of the field, and large rolls of hay spaced intermittently across the field. He initiated the takeoff, and when the helicopter reached 50 feet and 40 knots, the pilot adjusted the flight controls for a 60-knot climb attitude. About that time, the pilot heard a loud “thud” and saw scuff marks on the windscreen that alerted him to the wire strike. A FAA aviation safety inspector visited the site, and photographed the area. A three-strand group of wires crossed
the west side of the field perpendicular to the takeoff path. When viewed from the point of departure, the wires appeared out of the thick, tall tree lines that bordered both sides of the LZ, and were superimposed against a background of dense woods. There were no additional visual cues (poles, towers, or transformers) available that could be associated with the wires that were struck. during his intended refueling operation. The distraction involved ground personnel entering the helicopter cabin after landing to discuss the next spraying operation with the pilot. After the conversation, the pilot departed without refueling the helicopter. While completing an aerial application, the engine experienced a loss of engine power. The pilot initiated an autorotation and impacted terrain. A postaccident examination of the helicopter showed the tail boom was partially separated and the landing gear skids were bent. The operator reported that approximately 3.5 quarts of fuel were drained from the fuel tanks, and 2 gallons of fuel were considered unusable. NTSB Identification: *WPR11LA373* Date: August 06, 2011 Location: Chino Hills, CA Aircraft: ROTORWAY Exec Injuries: 2 Uninjured. An experimental amateur-built Rotorway 162F helicopter was substantially damaged during a forced landing that followed a complete loss of engine power. The owner was seated in the right seat, and the private pilot was seated in the left seat, which was the normal seat for solo flight in that helicopter. The private pilot was pilot flying (PF). When the helicopter was about 3 miles from CNO, the PF noticed that the engine “sputtered,” and called it to the attention of the owner, who then took control of the helicopter. The engine ceased operating about the same time, and the owner initiated a 180 degree auto-rotation to a vacant field. At touchdown, the helicopter had some forward speed, the landing skids dug into the terrain, and the helicopter pitched nose down. The main rotor struck the ground and also severed the tail boom, and the helicopter rolled onto its left side. According to the owner, the helicopter was built in 2008, and was equipped with a Rotorway 162F 150hp engine. As of its most recent conditional inspection in May 2011, the helicopter had accrued a total time in service of 205 hours. The owner stated that he began the flight with 17 gallons of fuel, that the total operating time for the flight was 57 minutes, and that the engine consumed about 10 gallons per hour.
NTSB Identification: *CEN11LA553* Date: August 06, 2011 Location: Metone, IN Aircraft: Hyde William R Helicycle Injuries: 1 Uninjured. An experimental amateur-built Hyde Helicycle helicopter sustained substantial damage when it landed hard during an autorotation after a loss of engine power. The private pilot was not injured. The pilot reported that he was about 100 yards from the landing zone at 100 feet above ground level (agl) when the engine “flamed out.” He attempted to execute an autorotation, but the helicopter hit hard. The landing skid broke and the main rotor, tail rotor, and tail boom sustained substantial damage. NTSB Identification: *WPR11LA377* Date: August 08, 2011 Location: Deer Park, WA Aircraft: Robinson Helicopter R22 BETA Injuries: 2 Uninjured. A Robinson Helicopter R-22 Beta sustained substantial damage following a loss of in-flight control and a subsequent hard landing. The flight instructor and his student were not injured. The flight instructor said that this was the second training flight for the student. The student was practicing hovering over the landing pad when the aircraft began to drift. The flight instructor announced that he was taking the flight controls, but the student hesitated in letting go. Control of the helicopter was momentarily lost and the aircraft made a hard landing. Subsequently the skids were spread, the firewall and fuselage were wrinkled, and the fuel tank’s mounting straps was compromised. NTSB Identification: *WPR11LA379* Date: August 14, 2011 Location: Los Banos, CA Aircraft: BELL 47G-3B-2 Injuries: 1 Uninjured. A Bell helicopter 47G-3B-2 Soloy impacted trees following a loss of engine. The helicopter sustained substantial damage to the tailboom and to the main rotor blades. The agricultural flight had just departed from the tender truck about 1245. The pilot reported that just after takeoff he observed the rotor rpm decaying and attempted to regain the rpm by lowering the collective. The rpm did not recover, and, due to the low altitude operation, the pilot was forced to land into the almond grove. The helicopter was recovered from the accident site for further examination.
NTSB Identification: *ERA11WA472* Date: August 19, 2011 Location: Brazil Aircraft: AGUSTA AW139 Injuries: 4 Fatal. An Agusta, AW139, helicopter impacted into the Atlantic Ocean, near an oil rig, the helicopter sank in waters with a recorded floor depth of 100 meters (328 feet). The 2 person flight crew and 2 passengers have not been located and are presumed killed. The flight had just departed P-65 and was en route to Macae Municipality, Rio de Janerio, Brazil, at the time of the accident.

fell during their third pass. He reported that the wing walker was to wait for a cue from the helicopter pilot when both aircraft were in position for the transfer. He stated that the wing walker jumped to reach the skid on the helicopter before it was in position, letting go of the handle on the airplane. The wing walker was unable to regain a hold of the handle on the airplane and fell. The airplane pilot noted that the wing walker did not normally let go of the handle on the airplane until his arm was wrapped around the skid on the helicopter. The helicopter pilot reported that the aerial transfer was planned to occur on the third pass. The initial passes went according to their plan. However, on the third pass, the wing walker attempted to grab the helicopter skid prior to the briefed transfer point. The helicopter pilot stated that the wing walker released his hold on the airplane handle, and lunged with both hands for the helicopter skid before the aircraft were in position. He reported that the wing walker attempted to go back to the airplane, but was unable to grab on to anything. The wing walker subsequently fell approximately 150 feet to the ground. The wing walker was transported from the scene in critical condition, and subsequently died at a local hospital. NTSB Identification: *CEN11FA599* Date: August 26, 2011 Location: Mosby, MO Aircraft: EUROCOPTER AS-350-B2 Injuries: 4 Fatal. A Eurocopter AS-350-B2 helicopter, sustained substantial damage when it impacted terrain during an autorotation following a loss of power. The pilot, flight nurse, flight paramedic, and patient received fatal injuries. The purpose of the air medical inter-facility transport flight was to transport a patient from the Harrison County Community Hospital to Liberty Hospital. The request was received by the company’s communication center at 1719 and the pilot was notified at 1720. He reported that he lifted off with two hours of fuel and 3 persons onboard and was en route to Bethany, Missouri. Approximately 28 minutes later, at 1758, the helicopter landed at the Harrison County Community Hospital helipad to pick up the patient. While the helicopter was shut down on the helipad, the pilot contacted the company’s communication center by

telephone and notified them that about half way through the flight from KSTJ, he realized that he did not have as much fuel onboard as he originally thought. After a discussion about possible fueling and re-routing options, the pilot elected to stop en route at KGPH for fuel, and then proceed to the Liberty Hospital helipad to drop off the patient. The person, who was providing flight following to N352LN at the company’s communication center, informed the pilot that Liberty Hospital was 62 nm, and that KGPH was 58 nm distant, respectively. About 1811, the flight departed from the Harrison County Community Hospital helipad. About a minute later, the pilot contacted the company’s communication center and reported that he had 45 minutes of fuel and 4 persons onboard and was en route to KGPH. He asked the flight follower at the company’s communication center to contact the fixed base operator at KGPH to let them know that the helicopter was inbound for fuel. At 1841, the helicopter impacted a farm field about 1.7 nm miles northnortheast of KGPH. There was no postimpact fire. NTSB Identification: *CEN11WA617* Date: August 27, 2011 Location: St. Ferdinand, Canada Aircraft: ROBINSON R44 Injuries: 4 Fatal. On August 27, 2011, at 2115 eastern daylight time, a Robinson R44 helicopter, sustained substantial damage when it impacted terrain shortly after takeoff near St. Ferdinand, Quebec, Canada. The pilot and three passengers sustained fatal injuries. NTSB Identification: *WPR11WA421* Date: August 28, 2011 Location: South Malekula, Vanuatu Aircraft: HUGHES 369D Injuries: 1 Fatal,2 Minor. On August 28, 2011, at 1500 universal coordinated time, a Hughes 369D, Vanuatu registration YJ-HEL, collided with the ground after the attached long-line became entangled in the rotor system near Malekula, Vanuatu. The helicopter was operated by Vanuatu Helicopters under the pertinent civil regulations of Vanuatu. The pilot was fatally injured, and the two passengers received minor injuries. The helicopter was substantially damaged.
NTSB Identification: *CEN11CA638* Date: August 29, 2011 Location: Menominee, MI Aircraft: ENSTROM F-28F Injuries: 1 Uninjured. According to the pilot, the purpose of the company test flight was to measure collective stick forces with the collective bias disabled. The pilot, who was the helicopter’s only occupant, lifted the helicopter into a hover and then hovered clear of any obstacles. He reported the collective controls were heavy as expected, but the helicopter was very controllable. During his attempt to position a force gauge between his hand and the collective grip, the pilot lost control of the helicopter and it landed hard onto the ramp. The helicopter’s tail rotor blades impacted the ramp and the helicopter came to rest upright. A postaccident examination of the helicopter showed the tail rotor blades were bent, and the tail rotor drive shaft was sheared. The pilot reported he should have taken an observer or other personnel on the flight to assist while he concentrated on flying the helicopter.
NTSB Identification: *ANC11LA099* Date: August 30, 2011 Location: Unalaska, AK Aircraft: Robinson Helicopter R22 BETA Injuries: 1 Uninjured. A Robinson R22 Beta II helicopter sustained substantial damage following a loss of engine power and subsequent emergency landing approximately 60 miles southwest of Unalaska, Alaska. The pilot reported that approximately 9 miles from his destination, the engine ran rough for about three engine revolutions before losing total power. He entered an emergency autorotation, and landed the helicopter in a shallow bay, approximately 150 feet from the shoreline. After contacting the water, the helicopter rolled to the right, and sank in shallow water. The pilot reported substantial damage to the tail rotor drive system.
Q & A with Question: A friend of mine was recently “ramp checked” by an FAA inspector. Do you have any guidance as far as what to expect during a ramp check, and a list of items that I may be asked to produce?
Answer: A ramp check doesn’t have to be a scary prospect. FAA inspectors conduct routine surveillance of normal airport activities--checking that both the pilot and the aircraft are legal and safe. Some of the routine items that the inspector may request include a pilot certificate, medical certificate, photo ID (per FAR 61 .3(a)(2)), and the aircraft registration and airworthiness certificates. One common question that arises in ramp check situations is whether pilot logbooks need to be carried in the airplane, and therefore if they need to be produced for review by the inspector. The answer to this
question varies, depending on the certification level of the pilot or flight instructor, and the type of flight. For instance, student pilots and flight instructors with sport pilot ratings may need to possess their logbooks depending on the circumstances.
