WELCOME Dear attendees, speakers and sponsors: Welcome to the inaugural Private Exchange FORUM produced by The Institute for HealthCare Consumerism. As the health and benefits industry continues its shift toward a more consumer-centric model, no other emerging area has garnered as much interest and arguably polarized the industry to the extent that private health insurance exchanges have. Over the past few years, we in the industry have heard a variety of opinions on the eventual adoption of private exchanges. We have heard from the naysayers that private exchanges are nothing more than an enrollment platform that shifts more costs to the employee population with little benefit. And we have heard from the soothsayers that proclaim private exchanges will be the future of employee benefits in just a few more enrollment cycles. But what is the most likely path of the private exchange future? This two-day event in Dallas will attempt to unravel the confusing tangle of models, choices and offerings that is the current private exchange market and attempt to construct what the private exchange future will look like for all stakeholders. As evidenced by widespread interest in this inaugural event, there is a real hunger for information right now as each industry stakeholder envisions how their business will change with the arrival of private exchanges. With that in mind, the Private Exchange FORUM agenda has been designed to take attendees on a journey through the process of evaluating and selecting a private exchange model. Whether you’re an employer, broker/advisor or insurer, it is our hope that you will find the education, collaboration and access to solutions that you need to help make the right decisions going forward for your business or clients’ business over the course of these two days. More and more, employers are responding to industry surveys similarly: they see private exchanges as a real option and perhaps the clearest path and means to accomplish their primary goals as it relates to health and benefits: cut health care costs; reduce administrative burden; improve choices provided to employees covered under the company health plan(s); increase access to coverage for those employees not currently under a company plan; and maintain valid options for active and retired employees. Yet, they state that the key questions remain unanswered. PRE-CONFERENCE PRE-CONFERENCE CERTIFICATION COURSE Tuesday, March 31, 2015 8:30 a.m. to 12:30 p.m. Special Certification Program for Brokers on Self-Funding Developed by experts in the market space, this four-hour course instruction will ensure that students understand the key technical components of self-funding and that brokers are better prepared to counsel their clients on the various benefits of self-funding, including eliminating most premium tax, lowering the cost of administration, lowering the cost of claims/administration and lowering the cost of customer service to employees. Additionally, this session has been extended by one hour to now include a primer on captive insurance products for health benefits and their relationship to self-funding, including how private exchanges may be used to manage this offering to employees and dependents. While anyone is welcomed to attend this course, the program is especially suited to brokers who want to gain a better understanding of self-funding options for their clients. CEU credit of four hours is available for this course in all 50 states. GENERAL SESSIONS OPENING GENERAL SESSION Tuesday, March 31, 2015 2:00 to 3:00 p.m. Private Exchanges: What’s Working, What’s Not and What Lies Ahead An expert panel of consultants, advisors and administrators deliberate on the current state of private exchanges and what is to come. With a year or more in the rear view mirror since the new energy behind private exchanges shifted to the servicing of active employees, there is still quite a bit of uncertainty about the future of private exchanges. To a large degree, what lies ahead depends on how exchanges — both private and public — perform over the next few years. On the one hand, several of the largest employers (Sears, Walgreens, Darden and others) have signed up with leading exchange operators. According to Forbes, Accenture, Goldman Sachs, William Blair and other firms predict enrollment on private health insurance exchanges could outpace enrollment on public exchanges by 2018. Other experts are more cautious, saying private exchanges for active employees have yet to prove their bottom-line value to employers. In a PwC and Private Exchange Evaluation Consortium (PEEC) survey, both sides are confirmed. Almost half of employers say they will consider private exchanges for their full-time employees, but less than 25 percent believe they will save money doing so. Come hear this keynote panel discuss what’s working, what’s not, and what lies ahead. Employers see private health insurance exchanges as a real option and as perhaps the clearest path to accomplishing several of their primary goals as it relates to health care and employee benefits — cutting health care costs internally, reducing administrative burden, improving choices provided to employees covered under the company health plan(s), increasing access to coverage for those employees not currently under a company plan and maintaining valid options for active and retired employees. GENERAL SESSIONS CLOSING GENERAL SESSION Tuesday, March 31, 2015 4:30 – 5:45 p.m. What Defined Contribution and Private Exchange Solutions Are Leading Brokers/Consultants Offering Their Clients? A panel of experts from the nation’s leading brokers and consultants divulge their recommendations on private exchanges. Employer-sponsored health insurance is on the cusp of a major evolution. Substantial inflation in health care costs over the past six decades has finally created serious consideration for shifting from a defined benefit approach by employers to a defined contribution model for employees. Enabled by the emergence of private exchanges, employer-funded defined contribution allows for better cost management through dollar limits on future coverage. Employees use that money plus their own contributions to purchase insurance from carriers on the exchange their employer has selected, while enjoying the privileges of greater choice, flexibility and a retail-like shopping experience. Numerous private exchange models can be involved in delivering this defined contribution approach, including: single-carrier exchanges (which are run by insurers and offer only the sponsoring insurer’s plans), multi-carrier exchanges (which are run by third parties and include numerous plan design options from multiple insurers) and exchanges that take advantage of cafeteria plan rules and allow employees to pay their share of the premium on a pre-tax basis. Though there is no single, dominate model yet today, developments in technology and advances in employee choice will certainly speed the shift from the current businessto-employer model to a business-to-consumer approach. This expert panel will share their thoughts on the models, the shift and more. Prior to this, John was senior vice president of consumerism for Cigna. In December 2012, John received the John J. Robbins Senior Memorial HealthCare Consumerism Leadership Award from The Institute of HealthCare Consumerism. John has 27 years of employee benefits industry experience with more than 12 years devoted to consumer-driven health care. In 2000, he was a founder of Consumer Driven Marketing, which served as the mid-market distribution system for Definity Health. Ernie Harris, NPDP Executive VP of Exchange Solutions Maestro Health Ernie Harris is the executive vice president of Exchange Solutions for Maestro Health. Prior to joining Maestro, Ernie served as the general manager for Alegeus Technologies’ exchange business unit. Over the course of his career, Ernie has been at the heart of transformation in several industries. In 1995, while at Baltimore Gas & Electric, he built one of the first turnkey managed desktop solutions responsible for maintaining more than 10,000 nodes. During the early 2000s, Ernie worked with TDAmeritrade to build one of the first global real-time data streaming services capable of distributing trillions of messages to millions of users concurrently. Today, Ernie divides his time between evangelizing the virtues of the online benefits marketplace to insurers, brokers, TPAs, employers and individuals and working with those same constituents to define and deploy an industry leading solution that meets the needs of all in a single platform. __________________________________________________________________

The defined contribution concept remains confusing to many employee benefit professionals. Yet, defined contribution is a very important development in employee benefits. Why? WORKSHOP 102 CONNECT SESSION 102 Private Exchange Trends and Strategies for Retirees Several consultants have established private exchanges aimed mainly at companies ending company-provided retiree benefits, and a recent survey of 595 companies conducted by Towers Watson and the National Business Group on Health found that nearly two-thirds of those employers are likely to eliminate the retiree health plan they offer today soon and steer their preMedicare retiree population to the public exchanges. The shifting to facilitate coverage for retirees through a private exchange is not new. The private exchange strategy originated with Medicare-eligible retirees to offer a wider choice of health plans. Yet it appeals to employers for several additional reasons: to help retirees understand plan choices available; to facilitate enrollment into the plan; to control their future retiree health care costs; to eliminate consulting cost; to gain administrative savings due to outsourcing; and to limit their reported liabilities because of the shift to a defined contribution approach.
