Welcome to the second half of the year, aka SPA+CLINIC’s 25th anniversary season! It’s hard to believe that our humble publication has been around for a quarter of a century, but it’s true! The change this industry has seen over the past 25 years is huge, and with it, this magazine has evolved. We’ll have lots of fun throwback content in our November issue you can look forward to, but for now, Dr Michael Zacharia is taking a look back at his own career and the cosmetic surgery industry over the past two decades on page 68.

An important aspect of the aesthetics industry that pretty much didn’t exist 25 years ago is device technology. Using machines to rejuvenate, tighten, or laser off skin has become the norm and the gold standard for many therapists and doctors thanks to companies like Alma, who are gracing our cover this month featuring Dr Edwina Morgan and her Alma Hybrid laser. Whilst there are many Radio-Frequency Microneedling (RFMN) devices on the aesthetic market, Endymed has a versatile platform to host both RF and RFMN attachments on the one device. Through an in-depth consultation and assessment of my skin (with a concern of some mild acne and acne scarring), my clinician at Melbourne-based Brains Aesthetics entered in the parameters for the Endymed Intensif and set up the device (which was a single-use, non-insulated gold plated tip applicator in sterile packaging) whilst my topical numbing agent was numbing my skin. A single pass over the full face was performed between 0.5mm to 1.5mm depth, which with the numbing and my moderate pain tolerance felt between a 4/10 to a 6/10, with the sensation felt more around bony areas such as the forehead and cheekbones.

Post treatment I did feel quite some heat and noticed slight oedema in the areas treated, in which the Velez Intense Hydration Face Mask was then applied to my skin and greatly reduced these sensations by bringing the surface temperature of the skin down to approximately four degrees. I noticed mild dryness and micro crusting visible on the skin for up to three days post treatment, with a radiant glow to my skin nearly a week later. My acne did decrease, and with subsequent treatments I would expect to see a decrease in scar visibility too. I liked the easy-to-navigate interface yet the flexibility to increase or decrease the intensity of my treatment at the hands of the clinician. Aerolase Erbium and Neo

Having experienced the Aerolase Erbium laser treatment firsthand, I can honestly say it is virtually pain-free, which is remarkable for a CO2 laser. There is, however, a loud rubber band flicking sound that becomes quite therapeutic after the initial shock of the volume of it. Overall I only experienced about four days of downtime, but have to admit I adore the tight feeling of my skin during this period. Soon after, the usual peeling and shedding was intense but it made me extremely happy to see the new fresh baby skin underneath. Now, at week four post-treatment, my skin tone is noticeably brighter, firmer and rejuvenated. My treatment was supported by the Neo Aerolase, a non-ablative partner laser used before my Aerolase Erbium session and again at my four-week follow-up. This combination has proven highly effective, with significant pigment reduction on my forehead. Dr Cosima, who has been overseeing my progress, is extremely pleased with the results. Given the years of sun damage from my youth, she recommends another treatment in a few months to further enhance the outcomes. One big advantage of the Aerolase compared to other lasers is that it’s lightweight and portable so for a physician that has numerous practices in different locations, it’s easily transported. here are certain suburbs in Sydney that are known for their high end Plastic Surgery Clinics and medispas, such as Double Bay, or the CBD. In recent months, a new aesthetic hub has emerged in Western Sydney, with an increasing number of reputable names in the industry setting up shop there. Bella Vista is located 33 km north-west of the Sydney CBD, and gained a stunning new aesthetic clinic last year: La Belle Cosmetics.

“Despite the area’s growing reputation in the medical field, there was a noticeable lack of non-surgical cosmetic clinics. We recognised an opportunity to fill this gap and bring these services closer to the local community, which is rapidly expanding,” says co-owner Matt Dona.

La Belle offers advanced skin and cosmetic treatments using the latest technology to address a range of skin concerns. “What really sets us apart is our commitment to personalised care,” says Matt. “Our highly trained practitioners create tailored treatment plans that focus on each client’s unique goals, ensuring the best possible results. We pride ourselves on providing honest advice and a holistic approach, always putting our clients first to help them feel confident and rejuvenated.

Planning the interior of the clinic was a long process and took almost two years as the team wanted to create a clinic that felt different to others and that exceeded patients’ expectations. To achieve this, they collaborated with Alicia Xiberras Interiors. “Alicia truly understood our vision and brief. She was able to translate our ideas into a design that went beyond anything I had imagined. Her expertise in blending luxury, comfort, and functionality was evident in every aspect of the clinic’s design,” says Matt.

From the elegant hardwood floors to the intricate detailing on the walls and the opulent marble finishes, every element was carefully chosen to create a sophisticated and welcoming atmosphere. The result is a space that not only reflects the brand’s commitment to excellence but also provides a serene and inviting environment for their clients.

When asked about his favourite room in the clinic, Matt has a clear answer: “My favourite room in the clinic is the consultation room. This is where every client’s journey begins and where we create our bespoke treatment plans. The Wellness Event Of The Year

Kris, what have the past 2-3 years been like since you introduced the Spa &Wellness Awards and now, also the Wellness Summit?

The Awards & Summit have taken on a life of their own. To the point where we have gained global recognition, and those who win a Spa & Wellness Award, have doors opening abroad, which is brilliant, and part of the reason we started the awards in the first place, to give great businesses in our region a platform to stand tall on!

The first year was so successful, we decided we needed to add a larger event around the Awards, again, particularly for those in Wellness, there was no such event. So, enter The Wellness Summit.

The feedback and response to both the Awards & Summit has been overwhelmingly positive, which is why we are having a two-day Summit this year and have opened more categories for the awards (especially in longevity & wellness).

How would you describe the wellness industry in Australia?

I’d say it is evolving at a rapid rate. This is driven not just by the pandemic where people had the opportunity to dabble in wellness, but also through losing faith in our health system. People are now investing in their health and longevity.

For someone who has worked in this industry, especially preventative health, for over 30 years, I feel ‘FINALLY!’ people get it! Despite the uncertainty with the economy, spend on wellness is up, especially in the areas of bathing, fitness, recovery/contrast therapy, longevity and anti-ageing.

There is no question that wellness, as a concept, as a lifestyle priority, and consumer value, is a big winner from the pandemic. Especially for Gen Z and Millennials who view their health as a financial priority, and are investing significant portions of their income on it, e.g. by seeking out aesthetic treatments and opting to go to a communal bathhouse as a social gathering over drinking at a bar!

Just look at the number of wellness & longevity centres, bathhouse and medispas opening, or in the pipeline to open. Between Saint Haven and Total Fusion, there’s 15 right there! Certainly what I’m seeing from consumers through Spa & Wellness, is there is a significant increase in people seeking out spa, wellness and result driven therapies. Our audience has grown 300% in 6 months! A true sign of the times.

What can people expect at this year’s Wellness Summit and Awards and who should attend?

This Year’s Summit has a focus on innovation and we have some incredible speakers. We also have some experts in AI and Financial gurus helping businesses weather the ‘cost of living’ storm and still grow a business. There’s also an underlying theme of integration, integrating modern tech with ancient modalities.

We speak to KRIS ABBEY, the organiser behind this year’s Wellness Summit & Awards taking place in Byron Bay on October 20 - 21.

And as always, we have a lot of wonderful immersions people can enjoy so they can experience various wellness treatments and try new, innovative products. What area of the wellness industry are you currently excited about and why?

I’ve always had a huge interest in longevity. Looking and feeling great, epigenetics, biohacking (utilising ancient and modern therapies) and nutrigenomics have been of great interest for decades. I love that these words are no longer obscure and people are understanding that they can really shape the way they age.

I’m also excited by the integration of modern science and technology with ancient treatment modalities. It’s no longer one or the other, it’s taking the best of both! For example, Cryo therapy has been around for centuries, and now with modern science it can be integrated into a facial, or gym session.

I mean, seriously, look at what Saint Haven are doing, or Integrative Retreats like RAKxa. At RAKxa you can have aesthetic treatments, cancer treatments or VSEL therapy, while doing an Ayurvedic sleep program (for example).

What are you hoping to achieve with the Wellness Summit and Awards?

On June 13, 2024, Biologique Recherche invited select media and practitioners to an exclusive dinner at The Langham, Gold Coast, to launch the highly anticipated new serum, Progeskin. How adopting a patient-centred approach to communication can help drive your business, according to marketing expert CHLOE MCGRATH

It’s no secret that the most recent changes to how we advertise cosmetic medical treatments under the TGA regulations, specifically those which utilise prescription-only drugs, has led to confusion and a lack of transparency for patients and consumers.

In recent years and months, concerns have been raised about the advertising and marketing practices of the aesthetics industry. Specifically, there has been criticism of the way in which clinics and practitioners promote their services and products, which has led to The Australian Therapeutic Goods Administration (TGA) to respond to these concerns by implementing changes to their advertising regulations, which aim to improve consumer safety.

These recent changes have left some practitioners feeling uncertain about how to communicate with their audiences. However, this new regulatory environment has also presented an opportunity for practitioners to re-evaluate and improve the way they communicate with patients and consumers.

Rather than relying on before-and-after photos, testimonials, and other forms of social proof, practitioners can now focus on educating patients about their concerns rather than the treatment options available to them.

Here are some of our tips on how you can maximise your impact while minimising the risk of non-compliance:

1. Explain to your audience what the changes are and why the noticeable difference in your language, lack of transparent pricing and visual results are not your doing, by choice. We advise not to be political in these types of posts and keep your opinions for or against the changes away from a public forum like this.

2. Shift your focus from selling treatments, to educating patients about the concerns and issues they may be experiencing. It is important for practitioners to help patients to identify their concerns, whether it be wrinkles, volume loss, or other aesthetic issues, so that they can be inspired to take action and book an appointment.

3. Offer complimentary consultations and clearly explain that any holding deposit goes towards treatment if the patient is deemed suitable for treatment.

4. Phone conversations are unlikely to be considered advertising, as per the TGA’s Frequently Asked Questions. This has been one of the most unfortunate misunderstandings of these changes by practitioners. The TGA has said “Solicited advice is less likely to meet the legislative definition of ‘advertise’ than indiscriminately disseminated information.” While it’s not recommended to perform a consultation over the phone, discussing ‘from’ pricing during a

call is not against TGA regulations. This is crucial to ensuring that patients can freely enquire without needing to take time out of their lives to find out how much a treatment will cost them.

5. Your reception team needs to be well-trained and confident in converting enquiries into in-person consultations. This has always been important, but now more than ever.

6. Expand your treatment menu into other areas such as cosmeceutical skin boosters, non-S4 injectables, PDO threads and skin treatments. Allow yourself more treatments to discuss and promote online that will catch the attention of consumers. Once the patient has crossed the threshold of your consultation room, you are free to discuss whatever treatment is best for themincluding Schedule 4 drugs.

7. Harness your experience, your brand ethos and what makes you unique as a medical practitioner. Your mission statement is now more important than ever, if you can’t use testimonials or results to help patients decide that you are the injector for them.

Practitioners can also differentiate themselves by emphasising their unique expertise and experience. Patients are more likely to trust practitioners who are recognised as leaders in their field, and who can provide evidence-based information and guidance. By demonstrating their qualifications and experience, practitioners can build their reputation and position themselves as trusted advisors for their patients.

Ultimately, the changes to the TGA advertising regulations are a step in the right direction towards improving patient safety and promoting a more ethical and transparent approach to marketing and advertising. However, it is important for clinics and practitioners to take an active role in promoting education, transparency, and patient empowerment, and to shift their focus from selling treatments to helping patients identify and address their unique aesthetic concerns. By doing so, they can build trust and confidence among their patients and help to ensure a safe and ethical future for the aesthetics industry.

While the recent changes to the TGA advertising regulations have posed some challenges for the aesthetics industry, they also present an opportunity for practitioners to improve the way they communicate with patients and consumers. By adopting a patient-centred approach, emphasising their expertise and experience, using clear and simple language, and leveraging digital communication channels, practitioners can build trust, inspire confidence, and provide valuable support to their patients. On June 16-18, more than 170 cosmetic nurses attended three days of learning, networking, and upskilling at Juv’ae’s Cosmetic Collective conference. The location was picture perfect Taronga Zoo in Sydney, whose new events centre created the perfect setting for the event. The conference featured a diverse array of sessions designed to enrich and empower attendees across various topics. Highlights included clinical sessions delving into cutting-edge research and practices, insightful workshops on marketing strategies tailored for today’s cosmetic landscape, engaging discussions on harnessing the power of social media, and transformative sessions focused on enhancing leadership skills and fostering meaningful connections with customers and teams.

Besides well respected Aussie experts, the program featured four international KOLs from Germany, Netherlands, Israel and London who shared their techniques and experience in the industry with Juv’ae nurses, who seemed eager to learn from them.

The conference was supported by all major injectable companies as well as CUTERA representing the opportunity for nurses to delve into capital equipment technology. Despite some online chatter and conference discussions suggesting inputting guidelines into ChatGPT or Gemini, I discovered that results were inconsistent. The grey areas where AI platforms speculated or provided inaccurate information couldn’t be relied upon, and the process was too slow. While there are a plethora of services and resources available to assist practitioners, I found that they still fell short in crucial areas like implementation time and service capacity. The question lingered: how could we extend our assistance to a greater number of practitioners?

I realised that what our industry truly needed is a purpose-built AI content-checking platform that could not only identify breaches but also generate compliant content. It needed to seamlessly adapt to upcoming changes, such as the inclusion of non-surgical guidelines, in a user-friendly and fast process. In a case of extreme serendipity, I met my co-founders Xavier and Scott, both experienced in full stack development and deep learning by chance when I sat down next to them at a coworking space and pitched them my idea.

Why Clinical Imaging Is A Non-Negotiable

Still taking your before and afters with a phone? Here’s why you’re non-compliant.

Your results are only as good as your before and after photos show – do we have your attention? You might disagree with this statement, however, from the perspective of your potential clients, and even most of your existing ones, this is true. Before and after images are an integral part of any surgical or non-surgical aesthetic business, not just for your marketing purposes, but also to keep track of your patients’ progress and to safeguard you from possible complaints, or worse, legal cases.

However, if you’re still using a phone or tablet to take your patients’ photos, you’re doing it wrong, and might actually be non-compliant, especially in light of the recent TGA and AHPRA guideline changes. In AHPRA’s ‘Guidelines for advertising a regulated health service’, the following is stated:

Care should be taken when using ‘before and after’ images in advertising a regulated health service as they have the potential to be misleading or deceptive. These images may cause a member of the public to have unreasonable expectations of a successful outcome.

Use of ‘before and after’ images are less likely to be misleading if:

• the images are as similar as possible in content, camera angle, background, framing and exposure

• the posture, clothing and make-up is consistent

• the lighting and contrast is consistent

• there is an explanation if images have been altered in any way

• the referenced treatment or procedure is the only visible change to the person being photographed

The only way to achieve the same camera angle, background, framing, exposure, and lighting, is by having a clinical photography system set up at your medispa or clinic. Using a phone camera automatically distorts the image as phones have several features built-in to ‘optimise’ the photo that cannot be controlled or turned off. The only difference in pre/post photographs should be the results of the treatment the patient has received. In an unregulated patient photography setup, image inconsistencies could include lighting direction, white balance, patient position, backdrop, and more. These inconsistencies hinder the effectiveness of a clinical photograph.

Why is clinical photography important for Merz?

Clinical photography is important for Merz as we have a very strong focus on the scientific aspects of Medical Aesthetics where high quality photography ensures that patients and their treating healthcare practitioners are able to accurately see results of their treatments. Good clinical photographs are also a really important element of any teaching and conference presentations.

How has the Clinical Imaging system assisted in the training academy?

Merz has installed the Clinical Imaging system at our training academy and all patients that are treated at the academy have before and after photos taken. These are kept as both a record for the treating HCPs if needed but may be used in marketing materials that are developed for demonstrating product results to other HCPs. Photos taken with the Clinical Imaging system are consistently good with photo angles being the same, as well as lighting, at all times.

Why do you think your clients should improve their clinical photography?

Keeping accurate records of patient treatments is important for a number of reasons. Having a pre treatment reference point is important and enables HCPs to demonstrate the changes in the after photographs which at times may be subtle. Some product results may take time to be evident and good clinical photography can assist the HCPs to show results that have been achieved in this extended time period. Current and aspiring KOLs can also benefit significantly by developing a good library of clinical photographs for their presentations.

Woodrow, why is it a bad idea to take before and after images with a phone or tablet?

Practitioners who do this are failing to capture the anatomy accurately and with sufficient detail to have a honest assessment. On top of that, they essentially pressing ‘random’ every time they press the capture button, as automatic settings on those devices cannot be reproduced.

You wouldn’t press random with a laser, don’t do it with a camera.

How have the recent TGA and AHPRA guideline changes made it even more important to have clinical imaging?

All the media attention on cosmetics has highlighted to the public they’re legally entitled to better standards in assessments, record keeping and privacy –standards that have been traditionally lax compared to a focus on marketing and social media. While I think TGA and pending AHPRA reforms were important to do, I don’t agree with some specific choices that will lead to patients being less informed and to see that info from overseas less regulated markets.

How does your software help with skin analysis?

We have two core filters which make use of the billions of colours that exist in raw image data.

These filters isolate certain chromophores in the image to highlight either vascularity in the skin or photo-damage in the epidermis. It really allows patients to understand what is going on with their skin health – and gives clinicians more discussion points during that vital initial consultation.

From the patients’ perspective, why should they also insist on clinical imaging?

We believe it’s something they should demand as a litmus test for standards within a clinic, to see that the specialist treating they are seeing has high quality diagnostic data to educate objectively on what is actually happening with their anatomy.

What kind of aesthetic business would you recommend your photography system to, i.e. even skin clinics that just do facials and maybe superficial lasers etc?

We are in both aesthetic and therapeutic clinics – and the system works equally well for both.

What I feel most proud of is seeing first year injectors have better photos than clinical studies in Ivy league journals. In addition, even established clinics can benefit as the pure time saved by doing photographs efficiently is enormous.

What does a business need to have your setup (in terms of space etc)?

We have installed in all sorts of room dimensions and we confidently recommend a minimum size of 1.2m wide by 2.5m long in a dedicated or shared treatment space.

The key point I wanted to showcase in my presentation is that the Hybrid settings and intensity are adaptable, depending on patient’s lifestyle and allowances for downtime. The article upon which the presentation was based compares a one-off invasive/high setting Hybrid treatment, with two low-moderate setting treatments over a 12 month period. Patient satisfaction, downtime, adverse events etc. It was affirmed that a low-moderate treatment protocol maintained high efficacy of results and increased patient satisfaction, with a lower incidence of post-inflammatory pigmentation. I found this of clinical relevance for physician and patient in the way that superb results can be achieved without prolongation of healing or downtime for the busy patient.

Dr Lain, tell us about your work in Austin, Texas.

I’m the Chief Medical Officer of a group of practices we have with 12 locations in two states. I see medical aesthetic patients every day. I run a lot of clinical trials as well. So I kind of just run around from room to room, seeing everybody who comes in and whether it’s research or clinical, and then I have the administrative aspects of what I do as well.

Do you feel the aesthetic needs of patients have changed in recent years?

Certainly, I think the importance of skincare has seen a resurgence in the last few years, and I think Covid helped with that. As practices realised that skincare was a major revenue source that was really the only one available, it gained much more importance within the practice owners’ eyes. And since then, as you know, companies have really come out with innovative products and innovative delivery systems to enhance the efficacy of the products. I think the importance of skincare is gaining momentum, particularly aligned with procedures.

And that is where SkinCeuticals’ new Cell Cycle Catalyst comes in?

Yes, Cell Cycle Catalyst was developed to be used along procedures, but I would hate to pigeonhole it as only a pre and post procedure product, because I do think that it is very useful to be used as part of an anti-ageing regimen, both because of its gentle, exfoliating properties, which will enhance skin quality, but also its ability to really make the other products within the regimen essentially work better.

What’s the benefit of having several different acids in one product?

The idea here was to use a gentle exfoliating blend that both exfoliates from the very tip top of the skin with the phytic acid, which is such a large molecule that it really just sits on the skin and helps to exfoliate a little bit, all the way down to lactic and glycolic acid, which is the smallest alpha hydroxy acid within the blend. So we’re trying to exfoliate the entire thickness.

We’re trying to do it in a way that is so gentle that it doesn’t cause irritation, and therefore can be used for pretty much every patient. And in addition, we have added the taurine in there, which, if we’re going to ask the cells to accelerate their turnover, to grow faster, we need to give them the energy to do that. And the taurine is the additional ingredient that is novel to any other SkinCeuticals product that delivers that energy to the cells so that we can achieve what we’re trying to with

Can we “overdo” it with accelerating our cell turnover?

I think what people might be referring to is this idea of DNA methylation and telomere shortening, where the idea is, over time and because of photo ageing and just intrinsic ageing, you are going to have changes to your DNA that will at some point, cause the cells to go into senescence and stop replicating. I get that. However, we know that retinoids and exfoliation do have an effect on delaying cellular senescence, and now there are other ingredients that will soon be incorporated that will target cellular senescence as well, so that we are pushing out or delaying the senescent phase of a cell’s life cycle.

Maggie, what was the process of developing Cell Cycle Catalyst like? Any challenges?

I think every single product that we make has the same milestones, but every product has its own challenges. I think of them as like our little babies. For Cell Cycle Catalyst in particular, the challenging part was incorporating the taurine into the formula, because it was new to the group. Initially it went in perfectly. We’re like, ah, this is going to be an easy one, but this is when we saw the crystallisation of the lower temperatures. That was one of the big challenges. And then the second challenge was the scale up. So in other formulations, we have challenges when it comes to the act of stability. We didn’t have the issue with this. So those were, like the two main challenges. I would say it was probably on par. It wasn’t harder or easier than a typical project. It’s probably right where we want it to be but, yeah, we have a whole spectrum of challenges.

How Neogenesis is transforming skins

The talk of the town is stem cell molecules. Stem cell molecules have become increasingly popular as an ingredient in skincare products due to their ability to repair, regenerate and protect skin cells, and to modulate the immune system, as well as, reduce inflammation.

The history of stem cell molecule research dates back to the 1970’s and has since been a field of great interest for scientists. In 1975, Drs Howard Green and James Rheinwald at MIT made an important discovery about human keratinocytes, showing that they could be cultured. This led to further discoveries about tissue regeneration and how stem cells can be used in skin care treatments. By 2008, Dr Greg Maguire had developed the concept of Systems Therapeutics as a new way of thinking about pharmacological products which opened the door for advances in using stem cell released molecules.

The latest science behind healthy skin is a little-known company making a difference to skin health for both women and men. Who is this new company you ask? NeoGenesis is located in the US, UK, Japan and also in Australia. We spoke with Borka Buseska, the contact in Australia for NeoGenesis to find out what all the excitement was about. Borka has an interesting background, giving her some valuable views on the beauty and aesthetics industry. SPA+CLINIC met with Borka at the Alma International Education conference in Spain and as we could tell Borka was very passionate about her involvement with NeoGenesis, we wanted to find out more.

A bit about Borka

Borka had a 30-year career in public service and for personal reasons had to retire from that career. She decided to change to something completely different and move into an industry where it is all about making people feel great about themselves.

Borka bought into a franchise – Laser Clinics Australia (LCA) and thoroughly enjoyed her time with LCA, having opened two clinics. In order to help her supply of qualified staff and assist in learning about this new industry, she embarked on opening a Registered Training Organisation (RTO).

The RTO delivered the Beauty Diploma, Advanced Diploma in Laser Hair Removal and the Diploma of Leadership and Management. While doing all this, Borka decided to study and gain an advanced understanding of aesthetics. As a result, Borka now has two University degrees, a Bachelor in Social Science majoring in Applied Economics and an Associated Degree in Health Science and Wellness.

It was during an Aesthetics Practitioner Association Network (APAN) conference in 2022 that Borka met Steve McGee, the CEO of NeoGenesis. They started talking about

Borka became intrigued about the products. Steve McGee was looking to expand in Australia and was looking for someone like Borka.

Given Borka’s background with other products we wanted to know why she decided to go with NeoGenesis.

“It was an easy decision after meeting with Steve. He showed me some before and after images and they were almost too incredible to believe. I told him that, too. Steve took me through the science and published peer-reviewed papers (the gold standard in science in the United States). There was overwhelming evidence of what NeoGenesis was achieving. During the conference, I met an Australian who had been using NeoGenesis Recovery serum, and she showed me her photos (on her phone). The results in such a short time were truly remarkable. I was HOOKED!

The other very important factor to joining NeoGenesis is Steve himself. He genuinely wants to help as many people as he possibly can. He is striving to gift products to 1 million people that are struggling with skin conditions, including people going through chemo or radiation that can’t afford the products. To be able to achieve that, sales in Australia need to increase. I want to be part of an organisation that has a soul like that. It was those two factors that made me decide to join NeoGenesis.”

How is progress going Borka?

“Progress has been very positive, but I would definitely like to see a larger geographic footprint for NeoGenesis. Some states are better than others. I would really like to have clinics in Sydney and Melbourne stock NeoGenesis. I receive a few requests from clients searching the internet asking for locations of clinics. Regional also needs a bit more coverage. I think talking about NeoGenesis and making it a household name will help Steve’s aspirations for the gifting program.”

3 Reasons Why Hyaluronic Acid Is Not Making A Change In Your Clients’ Skin

TANEESA WILLIAMSON explains some common causes for underwhelming results.

Hyaluronic Acid (HA) is often touted as the holy grail of skincare ingredients, renowned for its hydrating properties and ability to give skin a plump, youthful appearance. However, not all clients see the transformative effects they expect. The secret lies in the molecular weight of the HA being used.

1. Penetration Power: The Molecular Weight Matters

The effectiveness of HA largely depends on its molecular weight. High molecular weight HA (HMW-HA) has larger molecules that sit on the skin’s surface, forming a hydrating barrier. This is ideal for immediate surface hydration, making the skin feel softer and smoother. However, it may not penetrate deeply enough to address underlying hydration needs or stimulate collagen production.In contrast, low molecular weight HA (LMW-HA) has smaller molecules that can penetrate deeper into the skin layers. This deeper absorption allows it to hydrate from within, improve skin elasticity, and reduce fine lines more effectively. If your clients are not seeing significant changes, it might be due to using HMWHA which is limited to surface-level hydration.

2. Moisture Retention vs. Lasting Hydration

While HMW-HA excels at drawing moisture to the skin’s surface, it might not provide longlasting hydration. This can be particularly

problematic for clients with dry or aging skin that requires moisture deep within the dermis. HMW-HA can give an initial plumping effect, but it doesn’t always sustain the hydration needed for long-term skin health.

LMW-HA, on the other hand, ensures that the deeper layers of the skin are adequately hydrated, contributing to long-term improvements in skin texture and appearance. By reaching deeper layers, LMW-HA can help maintain skin hydration over time, which is essential for clients looking for more than just a temporary fix.

3. Skin Barrier Function and Irritation

The skin barrier’s health is crucial for overall skin appearance and function. HMW-HA, while excellent for surface hydration, can sometimes disrupt the skin barrier if not formulated correctly, leading to irritation and dryness in some clients. This can counteract its hydrating benefits, leaving the skin in a worse condition than before.

LMW-HA, with its ability to penetrate deeper, tends to be more effective in reinforcing the skin barrier and improving its resilience. This helps in preventing moisture loss and protecting the skin from environmental stressors. For clients with sensitive skin, LMW-HA is often a better choice as it is less likely to cause surface irritation and more likely to provide the desired results without adverse effects.

For clients not experiencing the benefits of HA, the issue might lie in the molecular weight of the HA being used. High molecular weight HA offers surface hydration but may not penetrate deeply enough for lasting change, while low molecular weight HA provides deeper, more enduring hydration and skin improvement.

NADINE DILONG sat down with Clinical Director of iS Clinical, DR CHARLENE DEHAVEN , to learn what makes the brand so successful.

Dr DeHaven, please tell us how you got into working with iS Clinical?

I’m boarded in internal medicine and a lifetime fellow of the American College of Emergency Physicians. I got into anti ageing medicine when it was just sort of coming on, and I started a practice of my own. I had one of the first anti ageing practices in the United States in the late 1990s. There was a group of other people who were very well funded by a wealthy billionaire who wanted to do evidence based treatments in anti ageing medicine. They were very interested in this topic and essentially just gave us carte Blanc. I knew the cardiologist that was associated with them, and, long story short, my practice got absorbed into them.

We all became one group, and that’s where I met Brian Johns, the CEO of Innovative Skincare (iS Clinical), because he did the aesthetics part of that, and I did the internal medicine, the clinical part of that. And so, Brian and I kept in touch, and then he started Innovative Skincare with Alec Call. And they would seek my advice on ingredients and products. And it just grew from there.

I’m so fortunate. As Clinical Director, I used to go around the world and talk to people about science, and and the science behind our products, and we have such quality products. If we were one of those companies that watered ingredients down to make two cents more on this unit, I would have trouble with that, but it’s just not like that here, quality is paramount.

What have some of iS Clinical’s innovations been that have changed the skincare industry?

The first big thing that we started was centered around advances in vitamin C technology. The first articles were published by a large group from Duke back in the 1980s I think. And that was the first time where the medical world really took notice and said, wow, here’s a real study that shows topical antioxidants, vitamin C and then later vitamin E, and they in combination can mitigate free radical damage in the skin. And that was revolutionary, but there were limitations to that technology. The pH had to be 3.5 or less, the concentration had to be 15 to 20% or else it just didn’t get absorbed. And that made formulating difficult, because there are a lot of ingredients you might want to use that work better at a pH of 4. So when we brought

our Advance+ vitamin C to market, that was a big push for a change in the vitamin C technology. It just blew those limitations out of the water, because you didn’t have to have a pH of 3.5 or less.

You could formulate with Advance+ between a pH of 2 and 11. The compound has such versatility. So that product was the first big thing brought to the aesthetics industry, and after that, we made some more advances in vitamin C technology. We brought another derivative. The Advance+ is water soluble. We began to use an ingredient which is lipid soluble. That’s another second generation vitamin C. More recently, we’ve combined vitamin C with a proprietary compound. But what it does is it hugely increases collagen synthesis.

The other thing we did that I really think did a big favour to consumers was when we brought our brightening complex and brightening serum to market, because prior to that time, hydroquinone had been the standard and that’s because it worked, but at a price, and there really was not a botanically based safe brightener that was in existence. Nobody developed one, and that those brightening products were our most difficult products to develop, because the skin wants to pigment. It’s like a defense mechanism, and so we brought forth Brightening Complex and Brightening Serum, which are botanically based, yet effective. There aren’t very many companies who are willing to go to the expense of the ingredients or the formulating science to get a botanically based face brightener.

Where does topical skincare have its limitations? Can results compare to treatments like lasers?

I think it takes both. A topical is different than a laser. A laser is different than a topical, but the best situation is when you marry them together, because they both tend to do good things and they potentiate each other. You combine the right laser and the right pre- and post treatment, topicals, and you can get a really good result that can keep improving that result, and that’s what you want. You don’t want to have a big, expensive procedure and then start to decline slowly but surely. You want to maintain that and in fact, even get it a little bit better. Many products lack the concentration needed to stimulate stem cells effectively. PRP got a bit of a bad rep for a while, because what’s mostly used out there is cheap tubes, and they don’t have the concentration level for the dose levels to actually regenerate. There’s a certain dose that you must use for the stem cells to be stimulated. So that’s why we use the 10 billion dose, where the generic market out there is really under a billion, if they’re lucky,” she notes. An American Perspective

At this year’s NSS, Nadine Dilong sat down with DR LINA VARGAS from Florida to chat all things patient safety, trends, and challenges.

Tell me a little bit about your clinic, Lumina, in Florida and what treatments you offer.

I’m a vascular surgeon by training, and I used to do the full spectrum of vascular surgery up until 2018 when I gave up arterial treatments. In pretty much the first year of my practice, which opened in 2018, we incorporated aesthetic treatments. So I mainly had patients who I was doing scleral therapy for cosmetic vein treatments, and they were wanting treatment for vascular lesions in other places of the body, under eye veins, the little cherry angiomas, little different vascular lesions here and there. So I found myself go out there and find a really nice platform to be able to do that, while also addressing issues like pigmentation, skin quality, skin texture, scarring. My first device was a Sciton laser, and it’s still one of my favourite devices for that indication.

And then slowly but surely, I started expanding into essentially everything else. My vision for my clinic was to be able to offer my clients, my patients, a comprehensive selection of all the treatments that they may be wanting, not only from the aesthetic standpoint, but wellness and body as well. So essentially, we offer pretty much everything at this point, which is just nice, because when we get a new client, the relationships that we want to build with them are lifelong and we don’t want to just do one thing and then have them have to go somewhere else to get those other treatments they’re after.

Do you feel people are open to comprehensive, holistic treatment plans?

They definitely appreciate it. I spend a lot of time during my initial consultation explaining ageing. Most of my patients are going through ageing, whether early in their 30s or 40s, starting to notice those initial wrinkles or maybe later, where they’re really noticing a lot of changes. And so when we address their specific concern, they’re going to be happy, but then most likely are going to start noticing other things. And so we want to show them what we’re capable of doing, so that they get that optimal result. Most people are actually on board with a comprehensive treatment that doesn’t stop where their specific concern goes.

What are laser regulations like in Florida? How do you ensure patient safety?

In the United States, it differs from state to state. But in general, laser and light devices can only be operated by a physician or a nurse practitioner or a physician assistant. Patient safety starts at the consultation. It’s all about education and about being very real with the patients, explaining the things that can possibly happen, even if they’re very rare, because if you explain that certain things are a part of the process, and not necessarily a side effect or a complication, then when patients are going through them, they perceive it as the treatment working. And this informed consent totally changes the dynamic. If a patient has to reach back out to you after the treatment saying things like ‘I wasn’t expecting this’, you have to calm them down, but if you had talked about that from the very beginning, then they’re not even concerned. They’re not going to call you. They know that it’s part of the process.

Also, just having very clear protocols and communication with everyone internally in my practice, who is performing the procedures that they are

trained and capable of doing. There are procedures that are thankfully very, very safe that essentially anyone can perform. But there are some other, specifically light and laser devices and injectables that require more experience. Safety always has to go first. And I always tell my patients I would much rather not get a result than hurt you or put you in a place where we didn’t want to be. It’s better to go very conservative that very first time, just see how the skin behaves, and then at that point you have a reference. You got the results you wanted. Just stick with those settings, those worked, and they were safe. If you didn’t necessarily get the results you wanted, that’s fine. Now you can maybe adjust a little bit, be a little bit more aggressive, but at the end of the day, it’s all about patient safety and not having complications or side effects as much as we can avoid them.

What would you say is a challenge for aesthetic practitioners at the moment?

I’m an international speaker, and I go to South America quite a bit. And actually there, the main issue is counterfeit devices and treatments. And now in the US, unfortunately it’s things like fake Botox coming from Asian countries, which is really, really scary. Because even though people should not play and not take risks when it comes to medical aesthetic treatments, people do go and find the cheapest thing. A lot of times, when they get these treatments that are not regulated, have not been studied, have not been proven for efficacy or safety, they can actually get hurt. And I thought we were sort of protected in the United States, but unfortunately, we do see that too.

Not long ago a place in Orlando was giving out free ‘Emsculpt’ sessions (when it clearly wasn’t a genuine Emsculpt machine) when they signed up for Semaglutide. Semaglutide is relatively cheap when you compare it with the body sculpting procedure. I mean, that should totally raise all red flags if they’re giving you the expensive treatment for free when you get the injection, the concept itself is interesting.

Unfortunately, people are always going to try to look for the cheaper alternative, and they’re going to find it cheap. I’m not going to lower my prices to compete so doing a lot of education into why this is an investment is probably one of the most important things, which is their health and their wellbeing. And this is not something that you should take risks with.

Have you been to the Non-Surgical Symposium before? What are you excited about?

This is my first time. I couldn’t wait to get here. I absolutely love it. Just being with the rest of the international and the national speakers and connecting with so many people, and learning, and I just love Australia. My only brother actually lives in Sydney, and we love the culture. I’m just very excited about all of the treatments being less and less invasive with less downtime, easier for the patients, easier for the providers, and just the way that we can combine them all to achieve that holistic result and approach. And I kind of like the fact that we’re moving away from fillers and just seeing that trend of more natural results.

From my early inspirations to the current trends shaping the industry, Australia has made transformative strides in the evolution of cosmetic surgery and aesthetic treatments. As a Sydney-based ENT Surgeon with over two decades of experience, I have had the privilege of witnessing this remarkable progress firsthand.

When I began my career, surgical procedures such as breast augmentation and facelifts were among the most sought-after treatments. These procedures were popular in many regions and still are today, particularly North America and Europe, where access to advanced surgical facilities and skilled practitioners were more prevalent. The number of facelifts performed increased approximately 6.6 times from 1990 to 2000* and breast augmentation surgeries have increased by 41% over the past 20 years**. Techniques varied, but common methods included silicone or saline implants for breast augmentation and traditional skin excision methods for facelifts. These surgeries were primarily accessible to those with the means to afford cosmetic enhancements, often leading to significant changes in a patient’s appearance and self-esteem.

A turning point in the 1990s

My training in Los Angeles exposed me to these techniques, highlighting their transformative potential. However, the late 1990s marked a turning point with the rise of non-surgical treatments, such as injectable fillers. This era saw a shift towards minimally invasive procedures that offered effective aesthetic improvements with minimal downtime, a trend that continues to evolve today.

Cosmetic injectables, approved by the Therapeutic Goods Administration in 1996, and dermal fillers provided new options for patients seeking facial rejuvenation without the need for surgery. This shift was significant, with data showing a steady increase in the number of people opting for these treatments, from thousands in the early 2000s to millions annually by the 2020s. Over 6 million surgeries or treatments including cosmetic injectables are carried out annually, a 700% increase in just the last 20 years***.

The deep plane facelift

While traditional surgeries like rhinoplasty and breast augmentation remained popular, I noticed a growing interest in facial rejuvenation procedures such as brow lifts and advanced facelift techniques. The deep plane facelift, in particular, became a passion of mine, a procedure that not only restores youthfulness but also maintains natural facial expressions and

contours. At my clinic in Sydney, approximately 40% of patients inquire about different types of facelifts, with a significant portion opting for the deep plane method due to its longlasting and natural results.

Over the years, minimally invasive techniques and advanced technologies, such as endoscopic methods, have further refined facelift procedures, leading to the development of the midfacelift and mini facelift. These innovations have reduced recovery times and improved outcomes by targeting specific areas with precision and less invasive methods.

The evolution of facelift techniques has been driven by the desire for more natural results, reduced recovery times, and enhanced patient safety. Early facelifts, which focused mainly on skin tightening, often led to short-lived results and a tell-tale operated look. As patients sought more subtle and lasting rejuvenation, surgeons needed to address the underlying structures of the face rather than just the superficial layers. Advances in medical knowledge, surgical techniques, and technology have enabled this deeper, more anatomical approach. Additionally, the increasing demand for minimally invasive procedures with shorter downtimes has spurred the development of techniques that provide significant results with less disruption to patients’ lives. These advancements have also improved the ability to customise treatments to individual patient needs, further enhancing satisfaction.

The future includes regenerative medicine

A fat transfer, also known as fat grafting, involves removing fat from areas of the body with excess fat, such as the abdomen or thighs, and injecting it into areas needing volume, such as the face. This procedure aims to restore youthful contours, enhance facial features, and improve skin quality through the natural regenerative properties of fat cells. Fat transfer offers a minimally invasive option with longlasting results, using the patient’s tissue to achieve a natural and rejuvenated appearance.

As the field continues to evolve, I am excited to embrace new innovations. With advancements in technology and techniques, the future of cosmetic surgery in Australia holds tremendous promise. If you know me, you know I love W Hotels. I have been lucky to stay at quite a few of them and experience their AWAY Spas, so I was very excited when it was announced that, after Brisbane and Melbourne, it was finally Sydney’s turn to get one. After I enjoy a delicious welcome drink and fill out my consultation form (5 stars for including a question about what size robe you would prefer!), I get a quick tour of the facilities. The waiting area is moody and dark in hues of blue and purple, the post-treatment area includes a variety of drinks and snacks and comfortable pods to relax in, and then there is also a steam room, sauna, and experience showers, which are not for the faint-hearted as the push of a button

releases cold water from the top, akin to an ice bucket challenge (not today for me, thank you).

My beautiful therapist Dorcas guides me to the very generous treatment room with my own bathroom, and explains the Quench treatment to me which I am about to have. A pregnancy-safe (I am 17 weeks at the time of writing this) body scrub, massage, and mini facial using Comfort Zone products. Everything at this spa is lush – the bedding, the robe, the bed itself, it’s all very comfortable and so I enjoy 90 minutes of bliss. Dorcas applies the scrub to my back, arms, legs, and décolletage, and explains that it has soft grains for a manual exfoliation, but also works as a chemical exfoliation, so we leave the product on while Dorcas continues with my facial. After a thorough cleanse, she uses two ice cold globes to give a cryo facial, which stimulates blood circulation and feels very refreshing. This is followed by a deeply nourishing moisturiser, and face and scalp massage.

Dorcas prepares the shower for me with a set of fresh towels and robe, and guides me to the bathroom to shower off the scrub. When I return to the treatment bed, everything’s cleaned as if I had never been there – the spa magic! I lay on my side and receive a back and neck massage using the Comfort Zone Tranquility Oil, a blend containing amaranth oil, grape oil, rose and vanilla oils and vitamin E. I love that its smell is quite subtle and not overpowering. My treatment ends with another quick facial massage.

Dorcas leads me to the relaxation pods where the most thoughtful variety of post-treatment treats awaits me. Non-alcoholic champagne, herbal tea, and vegan chocolates and mint coconut bars made by a Sydney-based baker, Dorcas tells me. They’ve also included a Beysis nail polish for me to take home, which has the exact same shade of mint as the mint coconut bar – not sure if intentional or not, but I love it. I relax and enjoy my treats for a while, before checking out the pool on the 29th floor, which is only accessible to hotel guests and spa guests who have booked a 60-minute or longer treatment. The view over Sydney is stunning. I will be back, no doubt.

The members only wellness club Saint Haven is opening several new locations this year. We speak with Founder TIM GURNER about its

successful concept.

Tim, why do you think Saint Haven has been such a success?

The success of Saint Haven has been truly humbling and is driving us to keep evolving our service, treatment and modality offering as we strive to create a unique experience for our members that isn’t available anywhere else in the country. One of the biggest pieces of feedback we get from our members is that they have never had a place to call their second home where they can escape the craziness of life and surrender with like-minded people.

Our members spend 3-8 hours a day in the club training, eating, working, connecting and relaxing with their shoes off and tracksuit pants on – it truly is their second home. And the attention to detail, the integrated, personalised, data-informed health approach and the ability to spend a whole day within the club to train, work, rest, re-fuel and recover – removes the need for multiple memberships and needless travel time.

What is your background in wellness/wellbeing/longevity?

My wife says that property and design are my passions and wellness is my obsession and it has been since I was about 15 when I started training. My first business was a gym when I was 19 years old, and ever since then I have been lucky enough to work with some incredible health, performance and mindfulness mentors. When I was going through a particularly challenging time in my property career, I engaged performance coach Nam Baldwin and his approach was to train my mind and body as if I was training for an elite sporting event. We introduced breathwork, mindfulness and specific training modalities into my routine alongside science-based modalities like PEMF Mat and Red-Light Therapy.

The creation of Saint Haven was actually a selfish one to create a place I could see friends and like-minded people in a time-efficient way. A place where I

could train, socialise, recover and surrender to life without the outside pressures of work, family and life. I am time poor so wanted to find a way to be able to combine everything in one place.

How did you manage to sell out memberships before opening?

Initially we did one single media interview to introduce the brand and I think people really connected with the story as everyone has similar challenges and there are not many places that help us deal with them.

So we had a huge amount of enquiry from that and then word of mouth was the biggest driver for memberships, and we were blown away to reach over-subscribed prior to opening the doors. We now have over 15,000 people on our waitlist across Australia.

Please explain the social aspect of Saint Haven and why it is so important?

I am a good example. I never get the time I want to be able to connect with friends and family. I am now in the club 3-4 times a week. I train with my longest standing friend on a Wednesday morning who I saw maybe once every three months before, I bump into family and friends in the sauna, or at the bar and we connect in a way I just have never been able to before with life being so busy.

We are also increasingly understanding the link between loneliness, depression and health. Social connection is critical for our health and happiness and is one of the biggest factors in creating the Saint Haven offering. Many of our members have become friends, mentors, even business partners, and we create events and off-site retreats to reinforce this pillar amongst our community.

What are the most popular treatments/experiences at Saint Haven?

Will the new locations be different from the first one?

Toorak is our first Saint Haven Black club, which is the highest level of the brand and offers a more medical and personalised focus. South Yarra will be a signature Saint Haven location and has all the same DNA and ingredients of Collingwood but with a completely different design and feel. They are all in the same family but slightly different all based on their location.

Without a doubt, our signature Fire & Ice breathwork classes, which involves guided breathwork between our sauna and 6-degree mineral ice bath is one of our most popular treatments. Interestingly Sound Healing is also becoming incredibly popular as it offers a very cathartic spiritual experience that people are embracing.

Our full-body NOVOTHOR red light therapy treatment is also always booked out weeks in advance, due to the unbeatable anti-ageing and cellular regeneration properties.

Why did you choose Melbourne as its first location, and where will you open up next and why?

Well, in a way, selfishly so Aimee and I could enjoy it first! But of course Melbourne made sense for our initial club location as our head office is in Melbourne and our design team and I were meticulously involved in every single detail of the club’s creation. We now have three sites either open or under construction in Melbourne and will turn our attention to Sydney and Queensland next, before looking further abroad to LA and other global outposts. Ideally, we’d look to have 10 – 15 clubs globally by 2030.

How often do Saint Haven members usually come in?

It’s really interesting as most are in 3-7 days per week because they may train 2-3 days per week, recover 2-3 days, and use the co-work and restaurant for entertaining. It is not uncommon to see someone there at 6am and they will still be there for a meeting at 4pm. Our café has two bookable, soundproof phone booths which can be used for private phone calls and zoom meetings so our members really can be here all day for work, leisure, nourishment, training and recovery.

Please tell me about your collaboration with James Vivian.

James Vivian is one of Australia’s most in-demand facialists and we were very keen to collaborate with him and offer our members a hand-picked menu of his signature facials to give our members even more efficiency of time. We worked with James to create a series of exclusive treatments that can be enhanced with some of the other offerings at Saint Haven like Cryotherapy.

Why is a signature scent important and what was the process of creating one like?

A lot of our members talk about turning the corner outside and getting the hint of our scent and they are already transformed to a new person. The entry experience for our members was an important part of the brief when creating Saint Haven.

Walking through the glass doors and passing through the threshold before opening the second set of doors signifies physically leaving your day behind, and entering a sacred space which is serene, calm and memorable. Kate notes that thoughtfully considering how, and in which succession, spaces will be experienced has been pivotal: “Establishing thresholds and transitional zones is critical to creating areas of decompression.”

At Drift Residences, this is exemplified by a sensory, arched entry passage residents transition through unveiling the Wellbeing Centre – an airy expanse suffused with ethereal natural light. A distinct shift in acoustics further denotes the transition, with the soothing murmurs of a cascading waterfall enveloping those who pause beneath its flow. The zones within the wellness precinct are carefully separated, ensuring each area is entered with purposeful intention. The secluded spa relaxation lounge, offering both private and communal treatment rooms, remains cloistered from the yoga deck, replete with sauna and steam rooms, which are in turn distanced from the pool facilities encompassing mineral pools, a cold plunge, and more.

“Throughout the design, we have curated an oasis of planting that thrives in tropical climates,” says Kate. Eschewing the manicured look, lush planting is seamlessly interspersed amongst the sunken lounges and daybeds, extending onto the terraces to forge an inviting, calming sanctuary.

With natural air-purifying qualities, the planting, which includes cascading vines, serves to soften the architecture’s lines, while vibrant floral accents and variegated foliage introduce visual dynamism.

With affluent buyers demanding far more than just luxury homes, a higher calibre of living experience is expected. Skilfully blending premium materials, thoughtful spatial planning, and the integration of nature can realise a deeply sensory and restorative environment. With rising demand for amenities that support remediation therapies, including salt rooms, and even snow rooms, for those seeking the pinnacle of beachside living, design studios like Plus Architecture are setting a new benchmark for what’s achievable.

Launching Your Dream Wellness Business

TOvercoming common hurdles as an entrepreneur according to SAMANTHA DUNN.

hinking about starting your own spa or wellness business? You’re not alone. Wellness is booming, and opportunities continue to blossom as the industry grows and diversifies. But let’s face it – taking the leap can be daunting. We recently surveyed over 70 aspiring entrepreneurs to discover what’s holding them back. Here’s a look at some of the common challenges you might face and how to tackle them with confidence.

‘Can I afford it?’

First off, the biggie: money. The number one question we get asked as consultants is, “how much will it cost?”. Sadly, there is no easy answer. You first have to design your business model and then cost it. A better way may be to set a budget and then value engineer the business to work within that. While it’s tempting to start with all the bells and whistles, it’s often wise to start small, be smart with your spending, and invest in upgrades as your business gains traction. Also, stretch your budget by looking into creative funding options like crowdfunding or pre-sales.

‘Where do I start?’

So, you’ve got this amazing idea, but now what? Almost half of survey respondents felt stumped as to how to get started – not knowing what to do, or how to do it. The temptation is to start with visible elements such as logos or interior design, however the secret is to start with strategy. You need a solid plan. Sketch out your vision, set clear goals, and define your unique selling points. Next, identify your target customers and the menu of services needed to satisfy them. From here, it is easier to create a roadmap to safely navigate the startup jungle.

‘How can I stand out and attract my customers?’

This is a major concern for over one third of respondents, and with good reason. With ‘wellness’ popping up on every street corner, standing out can be tough. Finding your niche is key. What makes your business unique? Once again, the answer is grounded in strategy. Differentiating your business starts with market research, from which you craft your unique selling points (USPs). By consistently applying your USPs through every aspect of your business, you can create a strong, unique brand that will attract your tribe.

Other Challenges

Other fears and concerns included:

• Lack of experience – either in Wellness or in business ownership. That’s a natural fear, but remember; everyone starts somewhere. The Wellness industry is welcoming, so dive in and soak up knowledge. Mentorships, groups, associations, conferences and online courses can be your stepping stones to success.

• Lack of clarity – surprisingly 10% of respondents knew they wanted to start a wellness business, but weren’t clear on what they want to do, why they want to do it, or what ‘success’ would look like. The danger with this lack of clarity is that a lot of time and money is wasted in finding your path, resulting in many businesses failing to launch altogether.

• Fear of failure – starting a business is risky, and fear of failure can paralyse the best of us, but it’s part of the entrepreneurial game. The key is to ‘know thyself’. Are you a risk taker or do you need more safety? If the latter, mitigate risk by collaborating with consultants or established brands to leverage their knowledge and experience.

• Fear of the unknown – almost one third of survey respondents worried about the risk inherent in “I don’t know what I don’t know”. Building a trusted team is crucial, including legal and financial advisors, and specialists in key areas such as branding and digital marketing. Success is a team sport, so don’t feel that you have to go it alone. Starting a wellness business can and should be exciting. Embrace the challenges, leverage the resources at your disposal, and seek help when needed. With passion, planning, persistence and just a bit of courage, you can turn your wellness dream into a flourishing reality.

New Injectable Findings

Could climate affect anti-wrinkle dosage requirements?

Recent research highlighted in Plastic and Reconstructive Surgery (July 2024) underscores how climate influences the dosage requirements for anti-wrinkle injections. Traditionally used for cosmetic purposes, these injections, specifically Botulinum toxin A to the glabella, have shown varying dosage needs based on geographical sun exposure levels. The study, involving 523 women aged 35-60 who underwent treatments between 2012 and 2019, was conducted at two centres – one in the United Kingdom and another in Malta.

In this comparative cohort study, researchers categorised the UK centre as having low sun exposure (winter months) and the Malta centre as having high sun exposure (summer months). Among the participants, 292 were from high-sun areas and 231 from low-sun areas. Over the course of the study, patients attended follow-up visits every three weeks, with doses adjusted until achieving full clinical paralysis.

The findings indicated a notable disparity in dosage requirements between the groups. Specifically, those in high-sun areas required significantly higher average doses compared to their counterparts in low-sun areas (29.2 units vs. 27.3 units; P = 0.0031). Even after adjusting for age in multivariable analysis, the difference remained statistically significant (P = 0.00574). Researchers emphasised that this dosage discrepancy persisted despite controlling for potential confounding factors.

“While the observed difference in dosage may not have substantial health economic implications in terms of vial usage, it underscores the importance of personalised treatment protocols,” remarked researchers in their publication. They highlighted that rigid treatment guidelines might lead to undertreatment in regions with higher sun exposure if not adjusted accordingly.

In an interview with Medscape Medical News, Dr Pooja Sodha, Director of the Center for Laser and Cosmetic Dermatology at George Washington University, Washington, DC, noted the study’s implications. “The findings emphasise the necessity of tailoring neuromodulator treatments not only based on gender but also considering lifestyle and climatic factors,” she stated. Dr Sodha also pointed out other critical factors such as injection technique,

evaluator blinding, prior treatments, and differences in botulinum toxin products that could influence treatment outcomes.

This research not only sheds light on the nuanced application of anti-wrinkle injections but also underscores the importance of adapting clinical practices to local environmental conditions. By recognising these variations, healthcare providers can optimise treatment outcomes for their patients effectively.

The LaseMD U LTRA is an easy laser to learn to use. It’s user friendly, customisable but not complicated. It’s a good first resurfacing laser to add to the tool box, versus a fractional ablative CO2 laser. All laser practitioners in Queensland need to be licenced and therefore have adequate training.

How do your patients feel about LaseMD treatments, i.e. what is their feedback in terms of comfort, downtime, and results?

Our patients have regular LaseMD ULTRA treatments, once or twice a year, which is a great sign that they are liking the treatment and the results! Most don’t find it too uncomfortable, and we certainly manage any pain well. Recovery is around 5-7 days, sometimes after 3 days the patients don’t look too bad.

It’s the results that the LaseMD ULTRA delivers that keeps our patients coming back for more.

How does the LaseMD ULTRA stand out compared to other fractionated devices on the market?

I have worked with other fractional resurfacing lasers, non-ablative and ablative, and the LaseMD Ultra is a pleasure to use.

Please explain the comb tip and what kind of scalp treatments you perform.

The “comb tip” on the LaseMD Ultra enables us to treat the scalp by parting the hair to apply laser energy directly to the scalp hair follicles and thereby stimulating hair regrowth. We can use the laser for treatment of any type or cause of alopecia (hair loss).

Do you use KeraFactor products –how do they work?

Yes, we use KeraFactor treatment serum during a LaseMD Ultra treatment as an adjunct for hair growth. The laser creates channels in the scalp skin to enable penetration of the KeraFactor serum which contains a high concentration of growth factors and peptides to stimulate new and thicker hair growth.

Hormonal changes affect more than your waistline – they can also have a detrimental impact on your teeth and gums. “A drop in oestrogen can lead to less saliva production, which allows bacteria to thrive. What You Need To Know About Off-Label Prescribing

DDR GIULIA D’ANNA explains the correct and safe way to prescribe and perform off-label treatments.

ermal fillers, and in particular botulinum toxin, are some of the most common pharmaceuticals prescribed for offlabel use. Yet many practitioners remain in the dark about recommended guidelines for use and the inner workings of off-label prescriptions. Though botulinum toxin is frequently used to treat the masseter and temporalis muscle, its use is considered ‘off-label’ as it is not officially registered for this purpose with the Therapeutic Goods Administration (TGA).

Clinicians who use Dysport to treat horizontal forehead lines may be surprised to learn its application for this purpose is also considered to be off-label. With so many go-to pharmaceuticals being used outside of their TGA-defined purpose, it pays to know what off-label prescribing actually entails. Crucially, practitioners must familiarise themselves with the clinical requirements needed to keep patients safe and informed of best practice.

What is off-label prescribing?

’Off-label’ prescriptions refer to medicine that is used in ways other than what is specified by the TGA in its product information document on the Australian Register of Therapeutic Goods (ARTG). When a drug is prescribed for another indication, at a different dose, via an alternate route of administration or for a patient of an age or gender outside the registered use, it is considered to be off-label.

Since the Therapeutic Goods Act technically does not regulate clinicians, how a device or medication is to be used in an “off- label” way is a decision that every practitioner must make. Despite this, it is common

for practitioners to run into trouble due to their lack of clarity on how to properly administer off-label medicine. The off-label use of hyaluronidase to dissolve fillers, for example, has been the subject of an AHPRA investigation as the regulator seeks to tighten oversight of off-label usage.

Why pharmaceutical companies don’t obtain more listings

The high rate of off-label use of pharmaceuticals amongst practitioners can partially be attributed to the lack of incentives for pharmaceutical companies to seek approval for additional indications, especially if there is unlikely to be any additional revenue for the company.

Due to the significant time and cost of undertaking additional research and obtaining regulatory approvals to have each new use listed on the ARTG register, companies are reluctant to sink millions of dollars to expand the listings for a single drug. Once a pharmaceutical is available on the TGA register, companies prefer to rely on practitioners to make sensible use of the drugs, and undertake the appropriate procedures to acquire patient consent.

Off-label use guidelines

Patient consent must always be established every time a drug is used in an off-label application. Every time a clinician decides to use a drug or device for off-label treatment, they must ensure they have informed consent from the patient.

This should only ever be done after extensively weighing up the patient’s risk-benefit profile which takes into account the risks and

benefits to the patient, and the evidence supporting the safety and efficacy of the proposed treatment. In determining the appropriateness of using a drug off-label, there should be sufficient evidence to support its efficacious and safe use, and an overall favourable harm:benefit ratio for the intended clinical use and population. Practitioners should proceed with treatments when they have decided that they are strictly beneficial to the patient and there is a good body of evidence for the intended off-label use.

Several guides published by the Council of Australian Therapeutic Advisory Groups state that in the event of harm to the patient, “if the off-label use of the medicine in a particular situation is accepted by the practitioner’s peers as constituting competent professional practice, and the patient has given informed consent for its use, then prescribing off-label should not imply negligence.” This implies that there must be a significant body of evidence demonstrating the use of a drug in an off-label way has been tested over a period of time, and there is general consensus among the practitioner’s peers that such use is considered routine.

Off-label use is very common with botulinum toxin drugs, and ample studies of the use of botulinum toxin for the treatment of masseter hypertrophy and bruxism support this. However the evidence of scientific studies do not preclude the practitioner from their responsibility to inform the patient of the off-label use of this drug.

Some practitioners may not be aware that the use of a cannula with dermal filler is also considered off-label, as the delivery method for the drug is different to the ARTG listing. While this is considered the safest delivery method, and is the medical standard in cosmetic injecting, it is again the practitioner’s responsibility to gain consent from their patient for the drug itself as well as the administration of an off-label delivery method.

Is off-label use legal?

In short, the answer is ‘yes”, with the caveat that there must be a large body of evidence that states that the benefit outweighs the risk to the patient, and such treatment is generally accepted amongst your peers.

The onus is on the practitioner to consider whether it is appropriate to use the pharmaceutical in an off-label way.

Using a needle with dermal filler, for example, is associated with a risk of around 1:6,410 per vascular incident according to global experts in cosmetic injecting. If clinicians deliver dermal filler via a cannula, the risk becomes 1:40,187, which in light of a number of high-quality and patient-based studies, translates to a safety profile that is very beneficial for the patient.

To help practitioners with their off-label decision making, a number of professional organisations including the Council of Australian Therapeutic Advisory group have issued guidelines in relation to offlabel medicine use. It is accepted practice that practitioners inform patients and document consent when prescribing off-label. This includes an open discussion about known and unknown benefits and risks. It is crucial that the prescriber documents the reason for off-label use in the patient’s record and ensures that patients are aware of the treatment procedure, along with any reviews that may be required.

Off-label best practice

Every practitioner needs to ensure that patients understand that they are using pharmaceuticals outside of its TGA-sanctioned purpose. Should an adverse event occur following off-label use, clinicians will be relying on informed patient consent they obtained before the procedure. Without it, any potential legal issues would be difficult to defend.

Practitioners who routinely engage in the off-label use of pharmaceuticals are encouraged to examine their consenting process, as well as consent forms, to ensure they are fit for purpose. Even for common drugs like botulinum toxin and dermal fillers, there needs to be clear procedures in place to protect patients and practitioners in the warranted and ethical administration of off-label drugs. If in doubt, it’s recommended that clinicians always err on the side of caution to save themselves from potential minefields further down the line.
