UNDERSTANDING VEIN DISEASE by Dr. Mark Schwartz MD, FACS, RPVI

Vein disease is a common ailment that affects 30% to 50% of the adult U.S. population. Symptoms may include pain, swelling, superficial blood clots, skin changes and even ulcer formation on the lower extremities. With awareness, however, symptoms can be caught at an early stage and treatments as well as preventative measures can be taken. Typically, women are often more affected than men, but men present with more severe symptoms including skin changes and ulcer formation. A series of one-way valves control the actual flow of the blood in the veins. When valves malfunction and fail to operate properly, disease may develop. In this case, it may allow blood to leak or flow backwards especially in the lower extremities. This backward flow, called venous reflux, is what precipitates symptoms. These symptoms arise from an elevated blood pressure in the veins and may be referred to as venous hypertension. This process leads to an intense inflammatory process both in the vein wall and valves as well as the skin and underlying healthy tissue. Spider veins and varicose veins are two of the more common vein problems in which people are typically more familiar. Spider veins lie just below the skin’s surface and usually appear as red or blue blotches. They are typically not dangerous and are simply cosmetic in nature. Varicose veins also lie just below the

Some surprising health benefits of caffeine

T

hat new coffee bar opened up just around the corner and you have been eager to sample one of their signature blends. But you consistently resist the urge to venture inside. Caffeine is not so healthy for you, right? Caffeine, the most widely consumed stimulant on the planet, has garnered a bad reputation. Some people fear caffeine and it’s potentially jittery and addictive side effects. However, many medical professionals attest that, when consumed in moderation, caffeine actually can have a number of health benefits. Before you skip that morning cup of Joe or choose an herbal blend instead of black tea, consider the following health benefits of caffeine.

Caffeine boosts brain and central nervous system function. Caffeine mimics the shape of another compound in the body called adenosine, which helps the body calm down and become sleepy. Caffeine can fit in adenosine receptors and cause a jolt of energy rather than sleepiness. Harvard researchers have found that blocking adenosine may slow the buildup of a toxic brain plaque that is associated with Alzheimer’s disease. Furthermore, caffeine may help keep dopamine molecules active in the brain and prevent the onset of Parkinson’s disease. Caffeine can help improve mood. The stimulant effect of caffeine may help boost people’s moods, and thus reduce the propensity for suicidal thoughts. In 2013, Harvard’s School of Public Health found that respondents who drank two to three cups of caffeinated coffee a day cut their suicide risk by 45 percent.

Caffeine may lower risk of stroke Studies conducted in both the United States and Sweden found that older women who drink more than a cup of caffeinated coffee each day have between a 20 and 25 percent lower risk of stroke. Similar findings were discovered in older men. Caffeine boosts memory. Studies from Johns Hopkins University showed that a 200mg caffeine pill helped boost memory consolidation. Caffeine offers pain relief. Caffeine is often paired with other pain relievers to bring about faster relief. The Journal of the American Medical Association concluded that when caffeine was combined with other pain relievers, patients required 40 percent less of the other drug to bring the same amount of relief experienced when using just the non-caffeine drug alone. Caffeine on its own can also relieve pain. Consuming caffeine before or after a workout can help reduce muscle soreness. Caffeine may help to prevent cancer. A recent study from Rutgers University pointed out that caffeine prevented skin cancer in hairless mice. Caffeine could open up air passages. People with asthma may find caffeine can improve their breathing. A study published by the U.S. National Library of Medicine determined that caffeine seems to open airways and help asthmatics breathe easier, providing a similar benefit to theophylline, a current asthma medication. Although caffeine can prove beneficial in various ways, individuals should realize that it remains a potent and potentially addictive stimulant. Caffeine also can aggravate anxiety symptoms and interact with certain medications. People concerned about caffeine should discuss their caffeine consumption with their physicians.

Bad back? Perhaps Pilates can help The difficult journey to the top of any sport will take you into strange and mystical places. I now find myself in Pilates classes, a discipline once only used by the George Balanchine ballet dancers. I take Pilates thanks to the unfortunate combination of being a 68-year old competitive golfer, having a weak core and thus having a very bad back. Up until recently Team Tommy included my orthopedic surgeon, Dr. Peter Langan, my physical therapist Anthony Belo, my golf fitness trainer Kristine Hughes and my swing coach Bob Rittberger. However I kept hearing the words “you must build up your core” and thus eventually, inevitably, inexorably I find myself taking Pilates classes at Healthtrax in Garden City. Pilates was developed in the 1920s by the German trainer Joseph Pilates who moved to New York City’s Upper West Side in 1926. This fitness method, which was then called Contrology, soon became the favorite form of training for all of George Balanchine’s ballet dancers. Pilates is actually a combination of yoga, Zen meditation and fitness training on a mat and by the early 1990s its popularity spread throughout Canada, the United States and the United Kingdom. Today you will find athletes of all kinds using Pilates because it combines strength, balance, flexibility and above all core training. People use it to help prevent back injuries, to improve posture and to flatten their bellies. Ballet dancers used it because it made them long and lean. I use it because I want to keep my competitive golf career going for as long as possible. Kay Trager, one of the Pilates instructors at Healthtrax, told me that Pilates is great for real stretching and real core building and that it’s used by people of all ages. There are over 11 million people who do Pilates regularly and over 12,000 instructors. When I mentioned that I was taking Pilates a friend responded “Oh, now you’re in serious fitness training.” Serious indeed. I recall this past winter visiting Dr. Won Park the South Korean genius who coaches many of the Ladies Professional Golf Association stars and observing

all his athletes in exercise classes each morning. Every class was run by a Pilates instructor. Pilates is not for the faint of heart. When you watch a class it really doesn’t look like much is going on. Lots of stretching and lying on your back with your legs in the air. But try doing that for an hour. I have taken many kinds of fitness classes in an effort to improve my golf game. For years I did hot yoga three times a week. I have been in golf fitness training for a long time and actually I’ve worked out nearly every day since I was in my twenties. But Pilates takes you to a different level. When you are asked to hold those stretches for a full minute two things go through your mind. First you realize this stuff is actually going to do what it claims. But then during the last 30 seconds of these stretches you basically get up close and personal with pain. In fact hot yoga in those 110 degree studios is a walk in the park compared to Pilates. As I chatted with some of the people after class they all said the same thing which is that Pilates builds core strength. Joseph Pilates called the core “the powerhouse” which consisted of the abdomen, the lower and upper back, the buttocks and the inner thighs. I then I realized that the reason the core is often so weak was because it was the area of the body that is never really worked. We all walk and we lift things which give our legs and our arms daily exercise but when do we ever actually use our core. Never. All we do is sit all day long. So the formula becomes simple. If one wants to remain competitive in whatever sport you chose you not only need good lessons, good equipment and fitness training but you will also need some Pilates as well. In my case it may be the only way to permanently get rid of my bad back and at the same time finally get rid of that pot belly that I’ve been carrying around all these years. So thank you Joseph Pilates for developing something that just might save my golf career.

Mumps is a viral infection that frequently causes hearing loss by inflicting damage to the cochlea (inner-ear organ of hearing). “Really loud restaurants drive me ballistic,” Lowe told the Pittsburgh Post-Gazette. “I live in a mono world. I wish I could [hear in] stereo.’’ Some hearing aid styles offer directional sound for 360° hearing, feedback cancellation, and enhanced noise reduction for noisy or windy environments to help you enjoy your restaurant experience. Stephen Colbert Though there’s nothing funny about hearing loss, Colbert is able to accept and even make light of his impairment. The Emmy-winning talk show host was in elementary school when doctors found a tumor in his right ear, which required surgery. During the procedure they also had to remove Colbert’s eardrum, leaving him deaf in that ear. Catching childhood hearing loss early is essential to ensuring academic, social, and professional success. For instance, a recent study from the University of Southampton found that teenagers who had been identified as deaf by the time they were 9 months old had much better language skills than children who had not been screened. These findings illustrate the longer-term benefits of early hearing loss detection. Jane Lynch Even though Lynch works in an environment where hearing is essential, the Glee star is deaf in her right ear. Her impairment was most likely the result of a high fever in infancy, but the loss was not discovered until she was 7 years old. “My brother was switching his transistor radio from one ear to the other,” she explained in her 2011 memoir, Happy Accidents. “I said, ‘You can’t do that. You can only hear out of one ear.’ He said, ‘No, I can hear out of both!’” Earbuds allow you to listen to music anywhere, anytime, and for long periods. That’s awesome, but it’s not so great for your hearing. The decibel level (the sound pressure) and the length of listening time affect how much damage is done. Sound becomes damaging at 85 decibels (the sound level of a bulldozer idling). Try the 60/60 rule: Listen to your device at 60% volume for 60 minutes at a time. Jodie Foster Though she keeps quiet about her personal life, Foster has spoken about her hearing loss, telling a Chicago Tribune reporter that she’s not very good about taking care of her own health needs, especially “this hearing-loss thing” and her vertigo attacks. She has been photographed wearing a hearing aid. Dizziness can be a symptom of many different diseases and disorders, but repeated dizzy spells are commonly associated with vertigo, which often results from a small mineral fragment detaching itself inside the cochlea and moving freely throughout the fluids of the inner ear. Three secrets to making morning workouts a reality R

outine exercise is one of the keys to a healthy life. When paired with a healthy diet, regular exercise can promote longterm health and reduce a person’s risk for a host of conditions and diseases, including diabetes, heart disease and stroke. While many adults are aware of the importance of routine exercise, finding time to break a sweat can be difficult. In fact, a 2013 study from researchers at the Centers for Disease Control and Prevention estimated that nearly 80 percent of adults in the United States do not get the recommended amounts of exercise each week. (Note: The World Health Organization recommends that adults between the ages of 18 and 64 get at least 150 minutes of moderate-intensity aerobic activity each week, or at least 75 minutes of vigorous-intensity aerobic activity each week.) Hectic schedules dominated by obligations to work and family can make it difficult to find time to exercise. But working out in the early morning, before the kids wake up and coworkers’ emails begin flooding your inbox, can help men and women meet the WHO’s recommended exercise guidelines and might even provide a noticeable energy boost throughout the rest of the day. Men and women can expect to run into some obstacles when first establishing a morning workout routine, but a few simple strategies can help make that transition go more smoothly.

1. Prepare the night before. Waking up with a to-do list in the morning takes away from the time you have to exercise. Rather than fumbling for your gym clothes and sneakers in the darkness of your pre-dawn bedroom, lay out your workout gear before going to bed each night. Lay out your work clothes as well so you spend more time exercising in the morning and less time choosing an outfit.

2. Set your alarm earlier than you intend to wake up. Few people wake up the moment their alarm clocks first go off. Snooze buttons may help you ease into your wakeup routine, but those post-snooze button minutes are minutes you could be spending getting your recommended daily exercise. If you’re a snooze button devotee and you want to be up by 6 a.m., then set your alarm for 5:50 a.m.

3. Get a good night’s sleep. In 2015, the National Sleep Foundation convened experts from sleep, anatomy and physiology, as well as pediatrics, neurology, gerontology and gynecology to reach a consensus on sleep guidelines for people of all ages. The panel recommends adults between the ages of 26 and 64 get between seven and nine hours of sleep per night, noting that anything less than six hours or more than 10 hours is not recommended. A good night’s sleep will help you feel more energetic when that alarm goes off, even if it is going off before the sun rises. Exercise and sleep go hand in hand, as studies have shown that routine exercise actually improves quality of sleep because exercise is a beneficial stressor to the body, which responds to the stress of exercise by increasing the amount of time the body spends in deep sleep. Many adults struggle to find time for daily exercise, but early morning might be the best time for busy men and women to break a sweat.
