NURSING – THE BACKBONE OF HEALTHCARE SYSTEM
W
hat comes to mind when one hears the word nurse is compassion, care, discipline, and knowledge.
For centuries, patients have relied on nurses for their well-being. Medical professionals too depend largely on nurses to tend to the patients once they prescribe the treatment. On nurses lie the onus of monitoring timely medication, food and rest for the patients. This age-old profession has undergone plenty of changes. Nursing - one of the highly specialised professions in the health care sector has been constantly evolving to meet the ever-changing needs of the society. Besides dedication and thorough knowledge of the medical condition, nurses also use their critical thinking and judgement based on their experience and the patient’s medical history, to serve individual patients and help them regain their health. The challenges are aplenty.
Nursing began as part of service to the society in the early 19th century, but professional nursing began with Florence Nightingale who tended to the wounded soldiers in Britain’s Crimean War with 38 other women. She defied the societal norms that did not consider nursing as a respectable profession during that period. They cared for patients of infectious diseases owing to which the morbidity and mortality rate in many cases came down significantly. Nurses transformed hospitals with their compassion, care, skill, knowledge and set a discipline in the hospitals. Today, their services are needed not only for patients in the hospitals but also for private care in homes for the terminally ill or the elderly. Full-time nurses are appointed by those who can afford it. The educational structure of nursing also changed over the years. From nursing schools to colleges, formal education in nursing has undergone many changes to train these health care service providers with changing technologies and advancements in the field. Many countries university degrees in nursing and master’s programmes for specialisation and advanced nursing are imparted. As a career in nursing one can choose hospital-based nursing, community health-based nursing practice or mental health-nursing practice, care of women, care of children, geriatric nursing, advanced nursing, clinical nursing and many more. Geriatric nursing is in huge demand today with the increasing life expectancy and children flying off to far-away shores.
For the profession that was not looked upon as a very lucrative or respectable one, things have changed for good in the past few decades and nursing got its due. Many colleges are working hard to churn out the best of nurses by imparting quality education and training. he question, ‘Why Nursing?’ is often encountered by those who choose Nursing as their major. This is an eternal question and there remains no definite answer. However, Christina Feist-Heilmeier in her book attempts to answer this question by saying, “Every Nurse was drawn to Nursing because of a desire to care, to serve, or to help.”

History in Making The foundation stone of Dr. D. Y. Patil College of Nursing was laid in 2000 and since then there has been no turning back. The college has gradually evolved and grown to become one of the top-ranked institutions for Nursing in the country. Starting from 2014 to 2019, it has completed 59 research publications, 155 dissertations (submitted by Post Graduate students), and 6 PhD theses have been completed and13 are ongoing along with 4 interdisciplinary researches. Prof. (Dr.) Namdeo Jagannath Pawar is the Vice-Chancellor of Dr.D.Y.Patil Vidyapeeth, Pune. Born in 1957, in a village in Sangli district. He obtained his B.Sc. degree securing first class with Distinction (1974) from Shivaji University, Kolhapur. He obtained M.Sc. (Geology) with first class in 1980 from the Pune University and later acquired his Ph.D. degree in Geology, in 1985, from the Pune University. He was recipient of the Young Scientist Award instituted by Indian Science Congress Association Ranchi in 1984 session for his research work. For postdoctoral research, he received the prestigious British Council Fellowship for two consecutive terms in 1985 and 1987 for carrying out research in U.K. Despite hailing from rural based family, Dr Pawar has risen rapidly in the academic circles through sheer perseverance and patience. He has contributed immensely to the fields of teaching and research, especially in terms of his specializations in Hydrogeology and Environmental Geochemistry.

Nursing in India How it all started T

he year was 1854 and a bloody and disorganised international conflict raged on between five nations – United Kingdom, France, Russia, Ottoman Empires, and Sardinia. This four-year clash was dubbed as The Crimean War. The British soldiers were dying, however, faster more painful deaths due to the diseases than from the enemy fire. This desperate situation changed radically after the arrival of a 34-yearold Florence Nightingale with 38 intrepid women accompanying her. She collected data during the war and conclusively proved that the leading cause of death for many of the fatalities was the infections. By changing the sanitation system, the health and hygiene regulations of the wards and care facilities, Nightingale and the 38 nurses brought order to that chaotic hospital and nursed the soldiers. She was hence dubbed ‘The Angel of Crimea’ and the foundational philosopher of modern nursing. In 1857, the Indian Mutiny turned Miss Nightingale’s interests to the health of the Army in India. In 1905,

during the British rule, missionary nurses arrived as members of the Missionary Medical Association. This was the starting point of formalized nursing in India. The increasing need of trained nurses gradually led to the formation of South India Examining Board in 1911, and subsequently North India Examination Board in 1912. Meanwhile, the Mid-India Board of Examination was founded on 1934. After winning the Independence from British Rule, The Indian Nursing Council (INC) Act was passed in 1947 by the Parliament. The expansion of hospital infrastructure and services created a humongous demand for nurses, nursing tutors, health visitors, nurse midwives and nursing administrators.

Soon after, 1960 saw the first masters, course – a twoyear postgraduate program in the College of Nursing, Delhi. The introduction of Multipurpose Health Worker’s Scheme following Kartar Singh’s Committee allowed changing most of the categories under unipurpose programmes can choose to get reassigned to multipurpose work. Thus, auxiliary nurse midwife (ANM) started replacing ASHAs, trained Dais and served at primary health centres and subcentres. In 1987, the Government of India appointed a High Power Committee on nurses and the nursing profession focusing on the need of teaching proper nursing education.
The future of healthcare in India lies in mainstreaming the health agenda. Directing the healthcare practices with the adequate use of technology in a sustainably developed manner. As per NHP’s 2017 recommendation, more new medical colleges, nursing institutions, and ensuring quality and updated education, are all ways to strengthen the sector. Studies into the profession have clearly shown that we achieve better patient outcomes when hospitals are staffed by more nurses who have completed their baccalaureate degree. With a good nursing workforce, the country can finally release the stress caused to the healthcare system by the Covid-19 pandemic.
The standard of education and code of ethics are regulated by INC and SNC. The colleges and universities do play an important role in maintaining quality and standard of education. Being a job-oriented profession, nursing is quite a rewarding profession too. The wide gap with respect to healthcare industry in India can be filled by employing and training of 2.4 million additional nurses to meet the growing demand.
The current healthcare scenario in the country is dramatically different from the past and it is the health system that shapes the educational pathways. e are all familiar with the story of Leo Tolstoy “How much land does a man need?” A young man takes up the challenge of running all day to cover as much land he can before sunset so that he can be owner of all the land as the king of land has decreed. In his greed for more, he keeps on running too far and too late to return. The major tragedy is that he dies in his attempt by over exhaustion. The final words of the king are cryptic, “All that a man needs is six feet of land, but alas this man never could understand it!” The story is an apt example of what exactly success is. Earl Nightingale defined success as, “Success is progressive realization of worthy ideals.” Indeed, success cannot be generalized, it is relative. For someone it might be living a decent life with all his needs met for other it may be possessing million dollars in her account for someone like Dr. Baba Amte it may be serving the tribal people with medical aid. The crux of what Earl Nightingale says above is found in Bhagvad Geeta when Lord Krishna admonishes Prince Arjun to do his duties as a warrior and fight and not worry about the fruit. Exactly this is what happens even today, people think that the 'fruit' is what defines success. Lord Krishna is telling us not the fruit but efforts in the right direction are what define whether one is successful or not. Once you know that you are doing what your are called for and meant for, you don't have to worry about the fruit or success, you have to keep on doing what you are best at. This is where contentment comes into picture. Benjamin Franklin once said, “Contentment makes a poor man, rich and discontentment makes a rich man poor.” Cambridge Dictionary defines Contentment as, “the feeling of being happy and satisfied.” Most of us are not happy in life because we are not content. Our minds usually deceive us in thinking that happiness is outside there. The 21st
Century world is fraught with technological advancement; we have everything at our disposal at a click of a button. But our thirst for more never seems to be quenched. The commercials thrive on our desire for more, they will tweak with the objects, refine and make it better and leave us wanting for more. The major culprit here is our fickle mind and our inability to keep it focussed on the present. When our minds are beset by the regrets of the past and the worries of the future, we fail to be content and happy. In that vein one can say contentment or real success is being able to focus on ‘what is’ and accepting it. Of course this does not mean that one should not dream big dreams or one should not aspire to be more. We should be able to balance both while being content with what is, we should have worthy ideals and goals to strive for. But we should be able to distinguish in the fact that it is not the goal but the striving that is what is success and excellence. Abraham Maslow, the American psychologist in his famous ‘Pyramid of needs’ actually was pointing out the very thing we are discussing it. There are two facets to his theory; one, success is relative for someone it may be ‘food, clothing, shelter, for other it could be relations, for someone else it could be being popular. The second aspect to his theory is found in the apex of the pyramid, which he termed as ‘Self Actualization’. Self Actualization means becoming everything that we are meant to become. Note here Maslow is pointing out all the below rungs of the pyramid represent ‘deficit needs’. All the below rungs talk about possessing something to make us happy but at the apex, ‘self actualization’ means striving for becoming all that one is meant to become, all that is his or her Karma. That is what the Bhagavad Geeta talks about we saw, striving and not the fruit is success! It is beautifully depicted in Robert Browning’s Poem, ‘The Grammarian’s Funeral’:
