4. Mail or Email Mail your completed form or email a scanned copy to tradehelp@leanintree.com.

1. New accounts must provide bank and four trade references, resale tax number, store contact name and store email address. Payment with first order expedites shipment.

2. Terms: 1% 10 days Net 30 Boulder.

3. A return authorization is required for limited exchange of everyday Greeting Cards and Collectors’ Cards.

4. All sales of Assortments, Note Cards, Magnets, Mugs, Note Pads, Bookmarks, Coasters, and Posters are final.
