The Medi Awards is a program sponsored by United Charitable Programs (UCP) a 501(c)(3), 509(a)(1) charity. Our mission is to recognize individuals and organizations that have made significant contributions to advance healthcare quality and access as we forge collaborations within the Patient-Centered Medical Neighborhood. Engaging local communities will strengthen Medical Neighborhoods and help deliver on the Triple Aim of the “Patient-Centered Medical Home” (PCMH) - Improve Health Outcomes, Enhance Patient Experience and Reduce Costs.
